New BASILICA technique created by the NIH, for coronary artery occlusion in some TAVR heart procedures, recently performed by interventional cardiologists here in South Florida

iguel Diaz, MD, FACC, FSCAI, an experienced and expert clinical and interventional cardiologist and Marquand Patton Jr., DO, FACC, FSCAI, FSVM, FASE, FASNC, RPVI, a highlyskilled and expert interventional cardiologist specializing in structural, coronary and peripheral interventions, both with Tenet Florida Physician Services (TFPS) and on-staff at Palmetto General Hospital in Hialeah, have recently completed one of the first BASILICA procedures ever in the State of Florida. The BASILICA procedure is performed

Marquand Patton Jr DO interventional cardiologist when there is high-risk for coronary artery occlusion during transcatheter aortic valve replacement (TAVR). While a BASILICA procedure is rarely needed, it can be a life-saving measure in rare cases when the implanted TAVR leaflet occludes the coronary artery. The BASILICA technique involves using a catheter to direct an electrified guidewire through the base of the left coronary cusp leaflet into a snare in the left ventricular outflow tract (LVOT).

Then, according to the National Institutes of Health, National Heart, Lung and Blood Institute (NIH), the

guidewire is used to split the leaflet so that it cannot block the coronary artery once it has been pushed aside by the transcatheter heart valve.

The NIH-developed BASILICA technique prevents obstruction in heart valve replacement cases.

Furthermore, according to the NIH, the BASILICA procedure has shown successful results for certain high-risk patients. The NIH adds that this novel technique has proven successful in preventing coronary artery obstruction during transcatheter aortic valve replacement (TAVR), a rare but often fatal complication. Called the Bioprosthetic Aortic Scallop Intentional Laceration to prevent Iatrogenic Coronary Artery obstruction (BASILICA), the technique will increase treatment options for highrisk patients who need heart valve procedures and might not otherwise be treated. The findings by researchers at the National Institutes of Health have been published in the Journal of the American College of Cardiology: Cardiovascular Interventions (June 2019).

Dr. Diaz and Dr. Patton Jr. have recently begun performing a high-volume of MitraClip, and TAVR procedures. Together, with Dr. Hans Rutzen Lopez, EP, they have also begun implanting the WATCHMAN Device.

According to Boston Scientific, the makers of the WATCHMAN Device, the WATHCMAN is a permanent implant designed to close the left atrial appendage in the heart in an effort to reduce the risk of stroke.

According to Abbott, makers of the MitraClip, the MitraClip is a less-invasive mitral regurgitation treatment option for certain people with mitral regurgitation who are not suitable for surgery.

In 2017, Dr. Diaz and Dr. Brijeshwar Maini, an interventional cardiologist and Regional Medical Director of Transcatheter Therapies for Tenet Health, performed the first MitraClip NT case in

1 cucharada de pimentón dulce 2 cebollas blancas grandes 2 Pimentón Rojo 1 hoja de laurel 6 dientes de ajo 3 tomates maduros 1 taza de guisantes (petit pois) 1 cucharada de cúrcuma (tumeric) 1 cucharada de azafran 1 ramita de perejil 700 gramos arroz redondo para paella 6 cucharadas de aceite de oliva 300 gramos de camarones pequeños 300 gramos de escalopes 300 gramos de calamares 300 gramos de pulpo 1/2 docena de muelas de cangrejo moro 300 gramos de almejas 300 gramos de ostras 300 gramos de mejillones (mussels) 1/2 docena de langostinos 1 cola de langosta 1 cabeza de cherna (pescado fresco) sal y pimienta al gusto

Preparación base del caldo

1. Poner en una olla a fuego medio dos cucharadas de aceite de oliva y las cabezas de los camarones, revolver por 1 minuto.

2. Anadir litro y medio de agua agregándole la cabeza de pescado, el perejil, la hoja de laurel, 3 dientes de ajo, 1 cebolla cortada en cuartos, pimienta y sal al gusto dejándolo hervir por 40 minutos colándolo a un pyrex.

Preparación en la Paellera

3. A fuego vivo situar la paellera con 4 cucharadas de aceite de oliva, añadiéndole 1 cebolla en cuadritos, los 3 ajos en cuadritos, el pimiento en cuadritos y los 3 tomates rallados, la sal sofriéndolo todo por cuatro minutos.

4. Agregue los escalopes, el pulpo y los calamares con el polvo de pimentón removiéndolos por 2 minutos y añadiendo el arroz, los guisantes, el azafrán removiéndolo por otros dos minutos.

5. Añádale ahora el caldo que preparo anteriormente moviéndolo un minuto y espere a que hierva.

6. Decore la paella con los camarones, los langostinos, las muelas de cangrejos, la langosta, los mejillones, las ostras y las almejas como en la foto mostrada arriba.

7. El toque final es un movimiento leve hacia delante y hacia atrás de la paellera para asentar sus sabores e ingredientes sobre el fuego muy bajo para que no se queme por 15 minutos.

8. Apague el fuego, tape la paellera con su tapa o papel de aluminio por 10 minutos y sírvala caliente. Bon Apetite y Disfrute con Carmen-Zabor, Inc.,

or 40 years, Elsa worked for the United States Equal Employment Opportunity Commission (EEOC), an agency charged with the responsibility of enforcing laws which prohibit employment discrimination on the basis of race, color, sex, religion, national origin, age, disability and sexual orientation. She began her career as an Investigator and worked her way up to Enforcement Manager of the Miami District Office, and Director of the San Juan, Puerto Rico office. During her tenure, she was responsible for the management and direction of daily enforcement activities over a sizeable workforce.

She was also designated as Outreach Coordinator.

As such, she conducted training for employees, employers, organizations and the legal community throughout the State of Florida and the island of Puerto Rico.

During her tenure she received countless awards and commendations, including EEOC’s Award of Excellence, Civil Rights Pioneer Recognition, Outreach Program Award, as well as the Federal Employee of the Year Nomination.

After Elsa retired from the EEOC, she began her community activism.

She has served on multiple community boards and City of Miami Beach Boards, such as; Ayuda,

Aumentan los Beneficios de Seguro Social en el Año 2020

ada año, anunciamos el ajuste anual por costo de vida (COLA, por sus siglas en inglés). Por ley, los beneficios federales aumentan cuando aumenta el costo de vida, según lo medido por el Índice de Precios al Consumidor para Trabajadores Asalariados Urbanos y Trabajadores Administrativos del Departamento de Trabajo (CPI-W). Por lo general, hay un aumento en la cantidad de beneficios que las personas recibirán cada mes, a partir del siguiente enero.

Cerca de 69 millones de personas que viven en los EE. UU. verán un aumento del 1.6 por ciento en sus beneficios de Seguro Social y pagos de SSI en 2020.

Otros cambios que ocurrirán en enero de 2020 reflejan el aumento en el índice salarial promedio nacional. Por ejemplo, la cantidad máxima de ganancias sujetas al impuesto sobre la nómina del Seguro Social aumentará a $137,700 de $132,900. ach year, we announce the annual cost-ofliving adjustment (COLA). By law, federal benefits increase when the cost of living rises, as measured by the Department of Labor’s Consumer Price Index for Urban Wage Earners and Clerical Workers (CPI-W). Usually, there is an increase in the benefit amount people will receive each month, starting the following January.

Nearly 69 million Americans will see a 1.6 percent increase in their Social Security benefits and SSI payments in 2020.

Other changes that will happen in January 2020 reflect the increase in the national average wage index. For example, the maximum amount of earnings subject to Social Security payroll tax will increase to $137,700 from $132,900. The earnings limit for workers who are younger than “full” retirement age (age 66 for people born in 1943 through 1954) will increase to $18,240.
