n Thursday, March 11, President Joe Biden signed into law “The American Rescue Plan Act.” After signing the legislation, he addressed the nation, highlighting that COVID-19 vaccines will soon be available to all adults. As a community news publication, we steer clear of politics, and we will do so regarding the legislation and the President’s policies. We see that day as a watershed moment; it was the beginning of the pandemic’s end. The COVID pandemic has affected us all in different ways. Many of us lost loved ones. Our families lived isolated from each other for over a year. Kids stayed home from school for up to three semesters. Small businesses, especially ones where people gather, have suffered. As our collective immunity becomes broader, it will be more critical than ever to support our local communities. The character of where we live is more threatened now than ever before. National brands and Internet retailers are efficient and predictable but make communities indistinguishable from one another. Let’s make a point to support locally-owned restaurants and venues. Go to a sporting event at a local high school or go for a walk in one of our fabulous parks. As we come out of our COVID hibernation, take stock of all the great things living in South Florida has to offer. It is a new beginning. Sincerely,

ea Level Rise (SLR) is a serious issue in Southeast Florida and will have a significant impact on homes, communities, and businesses if nothing is done sooner than later. Numerous local governments and universities are putting forth efforts to prepare a plan for the impact of the rising seas. Additionally, non-profit organizations and individuals are also addressing the problem including teenager Dylan Bober of Hollywood. As part of the Sea Level Rise Solutions Project, the ambitious Cooper City High School sophomore took Sea Level Rise to a new level. Due to the pandemic, the 5th Annual King Tide Flooding Event was hosted virtually and explored ways to communicate climate risks. The student-moderated webinar encouraged the
90 participants to take live polls to gauge climate change awareness and impact. They also had the opportunity to have an interactive discussion with an esteemed panel of scientists and community leaders. “The threat of climate change is one of the most pressing issues we face globally as well as locally in the coastal community of Hollywood. We need to make our cities more resilient and raise awareness about the problem of sea-level rise. The first step is being able to talk effectively about climate change. With our theme, ‘What Climate Change Means to Me’, our goal was to bring home the impact of sea-level rise on South Florida and discuss ways to get our community to care and act,” explained Dylan Bober. “I am encouraged that many of the attendees were high school students,

bout a year ago, Cesilia Riquelme was diagnosed with brain cancer. This horrific form of cancer interferes with the brain’s functions such as muscle control, memory, and other normal body functions. The sweet sixteen-year-old teenage girl and her family were devastated when they received such news. Not only was this disease difficult for her, but her parents hated seeing how it made their typically high-spirited daughter sad and depressed. Cesilia always wanted a puppy and she desired one even more since she was now fighting for her life. They wanted to give their only child everything they could during such a trying time, but it was difficult. However, they received the amazing news that Make-A-Wish ® Southern Florida wanted to grant her life-changing wish to have a puppy. “My daughter always wanted a little puppy. This has been an incredibly difficult year and this makes her feel better emotionally and

ours, mine, and ours. Sadly, in America the average marriage lasts only seven years, with one out of two marriages ending in divorce. As over seventy-five percent of divorced individuals eventually choose to remarry, blended families now outnumber traditional nuclear families. And while this family dynamic has become the new normal, it’s a far cry from what was portrayed on the classic television series, “The Brady Bunch.” If you are part of a blended family, you face some unique estate planning challenges. These include providing for your current spouse if he or she survives you, and making sure that when your spouse dies your children will be taken care of. Obviously, after the first spouse passes, the family dynamic can change dramatically; what the surviving spouse ends up doing with his or her assets can often be unpredictable. Let’s explore just a few possible estate planning options. Your spouse. A simple, albeit not wise, approach would be to leave everything to your spouse. As your spouse adores your children, you have no doubt that he/she will take care of your children after you are gone, right? Keep in mind that an unintended consequence of this approach might be totally disinheriting your own children! Consider this: After you are gone if your spouse becomes incapacitated it is very likely that your spouse’s children will take over the financial affairs of the family. You don’t need a vivid imagination to see how that might turn into a disaster. What happens when your spouse dies? Or remarries? Without thoughtful planning, your assets may ultimately be inherited by your stepchildren, or even by the person who marries your spouse! Proper legal planning can avoid this potential pitfall. Your children. Another solution is to leave assets to your spouse and your children. While this guarantees that your children will not be disinherited, this path may lead to unintended consequences. hese days many individuals going through a divorce try to handle the matter without the assistance of an attorney. They feel that it is better to try and settle the matter without spending large sums of money on legal representation. In all honesty, simple divorces can easily be handled by the parties. Simple divorces are cases in which there are no minor children and no real assets or liabilities to divide. There is a great deal of assistance through the courthouse or the internet to “walk you through” the process. HOWEVER, sometimes people can be as they say “penny-wise and pound foolish”. In an effort to save money on attorneys’ fees they may wind up giving up too much or perhaps not receiving his/her share. An individual who represents him/herself is held to the same standards as a licensed attorney. A judge will not give an individual the benefit of the doubt, just because an attorney is not sitting by his/her side in court. This can often present a problem in a more complicated case. I have often reviewed Marital Settlement Agreements which were entered into without the assistance of counsel. Often these individuals come to see me after the fact, to request assistance in attempting to “get them out” of the Agreement. Unfortunately, it is extremely difficult to undo what has already been done. A Marital Settlement Agreement or a Mediation Agreement has the same validity as any binding contract. In addition, as I previously stated, individuals are expected to know the law and it is not an excuse to later say “I wasn’t represented by an attorney”. I have often been told by individuals, who had initially attempted to represent themselves, that judges don’t seem to listen to them in court. Now, this could simply be a matter of perception or a true statement. It may also be because non-lawyers are not familiar with the rules of evidence and as such, certain testimony may not be permissible. A court proceeding is controlled by rules of evidence and rules of procedure, which are in many ways illogical and foreign to the average individual. Many attorneys offer free initial consultations and those that don’t usually charge a nominal fee for the initial meeting. If you have any doubts about the law as it pertains to your situation, it would be in your best interests to consult with an attorney. However, an attorney cannot ethically represent both parties in a divorce and therefore can only advise one of the parties. Therefore. initial consultations should take place without your spouse. Lastly, if you have considerable assets such as real estate or a business; or you anticipate a time-sharing issue, you should definitely consult with an attorney to discuss these matters. A little bit of experience can go a long way and ultimately save you money.

When Should I Worry? By Dr Michael M Cohen DPM FACFAS
A
flat foot or fallen arch is referred to medically as pes planus. Foot and ankle specialists understand that the flatfoot is a frequently encountered problem in children. In this condition the foot collapses or loses the gentle curving arch on the inner side of the sole. A pediatric flatfoot is generally divided into 2 sub-groups. The first, a flexible flatfoot is characterized by a normal arch during non-weight bearing and a flattened arch while standing. The flexible flatfoot may be painful (symptomatic) or not painful (asymptomatic). The second of the subgroup is referred to as a rigid flatfoot which is characterized by a stiff flattened arch regardless of whether the child is weight bearing or not. Such deformities include a congenital vertical talus, a tarsal coalition (a condition where the bones in the foot did not separate properly by childbirth), peroneal spastic flatfoot, or traumatic causes. A flatfoot may exist as an isolated problem, or as part of a larger clinical entity. This may include genetically lax ligaments, neurologic or muscular problems, genetic syndromes, and other collagen vascular disorders such as rheumatoid arthritis. Flexible flat feet are generally considered normal in young children as babies lack a normal arch. The arch subsequently forms fully between the ages of 7 and 10. A study published in a reputable medical journal evaluated 835 children, approximately half of which were boys and girls equally. Using a laser surface scanner, the children’s feet were measured carefully. The results indicated that the prevalence of flexible flatfoot in the group of 3-6-year old children was 44% but the prevalence of symptomatic (painful or pathological) flatfoot was less than 1%. Additionally, 13% of the children were overweight or obese. During adulthood, 15-25% of people may have flexible flatfeet. Most of these adults never develop symptoms. Adults with flexible flat feet likely inherited their condition due to the laxity of their ligaments.
Pediatricians and parents are often the first to recognize foot deformities in infants and children, but too often the problem may go unrecognized for a long period of time. When a flatfoot is being evaluated, a foot and ankle specialist would include a thorough family history which involves a review of the medical conditions including a history of trauma or problems during childbirth. Investigating the progressive flatfoot in a child’s family often gives a physician an idea how the condition will evolve. After this a comprehensive physical exam should be conducted. The foot and ankle specialist will want to evaluate the foot for areas of tenderness and observe the child during gait (walking). There is a prominent bulge in the arch, a heel that leans to the outside resulting in toes which are easily visualized from behind and referred to as the “too many toes sign”. Besides evaluating the foot, the child with a flat foot must be examined for possible contributing factors occurring above the level of the foot and ankle which would cause the foot to compensate by becoming flat. This would include the hip and knee, and lower leg. At times a leg length discrepancy may result in a flatfoot condition as well. The flatfoot is often associated with a number of subjective symptoms which would include pain in the foot leg and knee as well as other postural complaints. Interestingly a flatfoot has been shown to have a direct relationship with medial compartment knee arthritis and bunion deformities later in life. Additionally, a study published in a pediatric journal indicated that a relationship between adolescent bunions and flat feet was 8 to 24 times greater than expected. The flatfoot can result in decreased endurance and voluntary withdrawal from physical activities. At times a parent may relate that the child is clumsy and prone to falling frequently. Children may find it difficult to climb or run and prefer not to participate in sports. Imaging studies may include standing x-rays, CT scans, or magnetic resonance imaging. This will help make a determination as to what the cause of the flatfoot deformity is, particularly if it is rigid. Additionally, a blood test can be ordered to rule out certain arthritic or inflammatory conditions if a larger clinical picture is suspected. The nonpainful or asymptomatic flexible flatfoot may be physiologic or non-physiologic. Most flexible flat feet are physiologic, asymptomatic, and require no treatment. Physiologic flat feet follow a natural history of improvement over time. Periodic observation may be indicated to monitor for signs of progression. In these patient’s treatment is generally not
indicated and patients often do very well. The non-physiologic flatfoot is characterized by progression over time. The degree of deformity is much more severe in non-physiologic than in physiologic flexible flatfoot and is visibly much more evident causing the patient to compensate in several ways. Pain and symptoms resulting from flexible flat feet include discomfort on the inside of the foot pain, on the outside of the ankle, leg, and knee. Initial treatment for the flatfoot deformity includes stretching exercises, orthotics, at times immobilization if the child is limping, physical therapy to strengthen muscles and tendons which hold up the arch, and stretching exercises for the heel cord which is often very tight. Nonsteroidal anti-inflammatories may also be used, but not advised on a chronic basis. If there is a positive clinical response and symptoms improve the patient will just be monitored. Lastly, when all non-surgical treatment options have been exhausted surgical intervention can be considered. This may be provided in a variety of ways depending on the diagnosis and degree of deformity. Surgical treatment may consist of tightening of the ligaments and lengthening of tendons, a technique referred to as arthroereisis which involves the insertion of a small metallic implant through a small incision between the joints of the foot to restore the arch, to more aggressive traditional treatments such as cutting and realigning bones, known as osteotomies, or fusing the bones in the hindfoot together to stabilize the very deformed unstable flatfoot. In conditions such as a tarsal coalition, the area involved in the coalition is cut away so the bones can move more freely on each other.
