The original Panciera Memorial Home was founded in Rhode Island by Mark’s grandfather in 1932 and then migrated to Hollywood in 1962 where Mark worked with his father before expanding the business with the Boyds. Don and Kathy Boyd began their business in the late ‘50’s in Hollywood as well and then grew it with their sons Larry and Patrick in Pembroke Pines. We are so proud of that tradition and now we have the fabulous fortune to usher in the 4th generation of caregivers with Mark Anthony. Local Woman Showers Love on the Homeless

T

here are angels among us. It may not seem like it. But there are. People with such a strong sense of empathy that they change their lives in service of the less fortunate. Jeanne Albaugh is one of these people. In 2016 Jean and a small group of friends founded Showering Love. Showering Love is a 501c3 Non-profit that provides mobile showers, new undergarments, socks, hygiene kits, and other

supportive services for the homeless. The organization provides these services by using a novel idea: a bus named “Grace.” “Grace” is a 44 foot former Miami City Bus that has been converted into a self-contained mobile shower platform equipped with 2 handicap-accessible private showers and restroom facilities. Jeanne knows a thing or two about being homeless. She spent ten years on the streets of Broward County without a home. After some unfortunate events in her life, Jeanne went from living on Park Avenue to a park bench. “I scurried around from place to place and slept wherever I could,” said Jeanne, “I begged for money, holding a sign under a bridge. People graciously gave me food and money, but, without a place to shower, my hygiene and personal appearance deteriorated.” Jeanne took advantage of numerous programs to help the homeless addressed her issues and became self-sufficient. She, however, will never forget those dark desolate nights and decided to dedicate herself to

helping homeless people regain their dignity. In addition to personal hygiene, Showering Love offers haircuts, backpacks, wellness checks, hot meals, and non-perishable food. They also connect homeless people with health care programs. Most importantly, Grace provides opportunities for guests and volunteers to break down barriers, and form relationships within the community.

I

n 1993 The Solar Car Challenge was established to help motivate students in science and engineering and to increase their awareness about alternative energy awareness. High schools across the nation and around the world are all encouraged to participate by building roadworthy solar cars. Of course, Broward County Public Schools (BCPS) very own Western High School always loves a challenge. The school’s Solar Cats made their mark a

few years ago when they won the “Solar Challenge Classic Division” in 2018. The eager students learned how to plan, design, engineer, build, race, and evaluate solar cars while demonstrating how green technology creates a better world. Most recently, the champions returned to the track for the 2021 Solar Car Challenge in Fort Worth, Texas to compete in the new Electric-Solar Powered Division which seeks to make solar car racing a reality by incorporating solar panels and a two-passenger vehicle capable of urban driving. The Solar Education Program provides a safe environment for teams to display their solar cars. Qualified teams had the opportunity to drive their solar cars during a closed-track race at the Texas Motor Speedway. High school teams from 38 states, as well as Canada, Mexico, Costa Rica, Puerto Rico, and the Bahamas, traveled to The Lone Star State to feature their solar car projects. Over 200 solar car projects were featured at the anticipated event. The ambitious Solar Cats debuted their new electric car, the “E-Cat”,

which they constructed from the ground up using batteries powered by the sun. “The challenge gives students an opportunity to learn skills they can carry forward in their academic and professional careers, such as mechanical, electrical and software engineering, as well as problem-solving, team building, and project management,” said Western High Assistant Principal Christine Graf. “The students have spent months working on their car. This is an opportunity to demonstrate ingenuity and celebrate their efforts.” Congratulations to Western High School Solar Cats leaders and team members: Rohan Jaison, co-captain and telemetry director, Justin Fox, co-captain and system architecture, Fernando De Achurra, mechanical design director, Brian Abin, Terry Russel, Nadia Auguste, and Ethan Kaplan.

outh Florida’s premier dance school is ready to get back to business as usual. For nearly thirty years, Dance theatre has been an industry leader in children’s dance. However, when Covid-19 arrived, health and safety concerns prevented all students from participating in their regular activities, including dance students. True to form, Dance Theatre adapted to the “new normal” in spectacular fashion, and kept its students dancing throughout the pandemic.

According to Dance Theatre’s owner Michelle Gerlick, the past eighteen months have given rise to the worst, but also the best of times for her dancers. “When Covid19 required us to temporarily discontinue all studio classes, my teachers and I had to learn how to conduct classes online. It was no simple task. We had hundreds of classes, involving thousands of dancers, and we had to ensure that everyone could access the proper “meetings”. When it was finally

deemed safe to return to the studios, we knew we had to do everything possible to protect the health of our dancers. First, we installed Reme Halo air cleansing systems in all HVAC units at every studio. They kill over 99% of all coronavirus particles, including Covid-19. Next, we divided our dance floors into six foot boxes to provide adequate social distancing. Finally, we established guidelines concerning masks, temperature stations, and sanitization.” Wondering whether all of these efforts worked? They did! Not only did Dance Theatre emerge from the depths of the Covid-19 pandemic without an outbreak, but there wasn’t a single case of transmission resulting from its’ class activities. Not a single one. “I’ll be honest.” Michelle continued “There were many days where I broke down and cried. I was worried about my students, their families, my family and friends, my business. But an amazing thing happened along the way. I realized just how special the bonds we’ve created at Dance Theatre really are. I witnessed how much we mean to our families, and in turn, how much they mean to us. When we asked them to Zoom, they did. When we were forced to film recital in small masked groups, they showed up and dazzled. When we invited them back into the studios, they ran in smiling, thrilled to be together. We’ve all lost so much through this ordeal, but we’ve gained something too. We’ve gained a greater appreciation for one another, for the precious gift of life, and for the privilege of seeing and instructing our amazing dancers. I’ve never been more inspired to continue doing what I love, and make Dance Theatre the greatest school on Earth.”

For those of you unfamiliar with Dance Theatre, it offers a truly comprehensive program where dancers ages 3 through adult, beginner through advanced, can take classes in every possible discipline. They include the basics, like ballet, tap, jazz, lyrical, hip hop, acrobatics, musical theater and pointe, as well as specialty classes like strength and flexibility, leaps and turns, etc. In addition, dancers who excel and aspire to become professionals also have options. This year they will perform the halftime show at the Dolphins vs. Jets game, Miami Heat basketball games, and travel to New York City to open the 95th Macy’s Thanksgiving Day Parade. In addition, the best dancers may be invited to join Dance Theatre’s elite competitive team, DTX, where they can travel to local, regional and national conventions and hone their skills against the best from other schools. DTX just returned from the Inspire National Championship in Nashville, Tennessee where the team brought home several top awards in virtually every category, with seven of their pieces chosen for the Inspire No Limits Challenge. Stroke Myths By Dr. Shaye Moskowitz

M

yth 1: There was a warning sign, I was having a stroke.

A stroke is the sudden loss of blood supply to a part of the brain. Typically, a clot blocks flow and the brain down stream of that vessel stops working. What we see clinically is what the brain is not able to do. Since this is sudden, they rarely are warning signs to the stroke itself. There are circumstances where mini strokes (TIAs) happen and warn that there may be a bigger future stroke that could happen, but those are exceptions. The best approach is to know what the signs of a stroke are so when it happens, it quickly recognized.

Myth 2: I cannot have atrial fibrillation because I don’t feel anything so why do I need a blood thinner?

Atrial fibrillation, particularly the kind that comes and goes, increases the risk of having a stroke. In fact, the overall risk is estimated at 5% per year and approximately 5 times the rate of the average population.

Blood thinners are important to reduce that risk. Unfortunately, many people have fibrillation without any sensation that there is a problem, and many people feel their heart racing even when they are not in atrial fibrillation. How someone feels is a poor predictor of whether they have atrial fibrillation and whether they need a blood thinner to reduce their stroke risk.

Myth 3: There’s nothing to do about a stroke.

Historically, there were few treatment options for stroke and hopeful recovery, rehabilitation and prevention were all the health care field could offer. This could not be further from the truth. Treatment options include medications to dissolve the clot, procedures to physically remove larger blockage or surgeries to address pressure from a brain bleed, and are options for select patients. Of course, recovery, rehabilitation and prevention are still important!

Myth 4: An aneurysm is an instant death sentence.

An aneurysm is a weak spot on the wall a blood vessel that is feeding the brain.

While the rupture of that aneurysm overall carries high mortality, it is an infrequent event and the risk only accumulates over years. In fact, aneurysms are fairly common and are seen in approximately 5% of the population. The vast majority, however, do nothing. Since some do unfortunately, we have to make decisions individually considering not just the aneurysm but the other risk factors someone might have to justify a repair. This certification signifies that the hospitals’ stroke program meets rigorous standards for providing care to all stroke patients including endovascular embolization and surgical clipping of brain aneurysms, tPA administration and mechanical endovascular thrombectomy (EVT), a minimally invasive procedure used to remove a blood clot from the brain during an ischemic stroke.

It’s Time to Review Your Estate Plan!

hings change. Despite all the time and thought that went into creating your will or trust, life happens. Often these changes affect your estate planning. If you don’t regularly review and adjust your plan it may not achieve your desired results when you need it most.

How often should I review my documents? We recommend reviewing your revocable living trust or will, along with your advance legal (health) directives, at least every three years. This review is especially important if you experienced any major life changes since your documents were executed, including:

New family members, or a loss of a loved one Child that has turned 18 or 21 A recently married or divorced beneficiary Beneficiary concerns, such as financial issues, substance abuse problems or disabilities Creation, sale, or closing of a business Purchase of a new or second home, or acquiring an investment property Changes to your beneficiary or successor trustee designations Changes in your relationships with your beneficiaries and/or successor trustees

Can a change in the law affect my documents? Yes, very definitely. Let’s look at a common estate planning document, a durable power of attorney. Several years ago financial institutions lobbied the Florida legislature to change the applicable law to protect banks and brokerage firms from liability if a durable power of attorney is misused or forged. The new law suggests certain “magic” language which, if included in the durable power of attorney, provides this protection, which makes financial institutions much more likely to honor the document. If your document was drafted before 2011, it won’t contain this magic language. Another change to the law which took effect on October 1, 2015, affects your health care power of attorney. The Florida legislature has revised the law to clarify confusion surrounding when a health care power of attorney becomes effective. Your health care power of attorney can be effective immediately, but only if you so designate. If not, your treating physician must first determine that you are unable to manage your own care before they will take instruction from your designated agent. The new law also offers new choices in terms of designating a health care surrogate for a minor child. Because of this change in the law, there is new language that should be part of your health care power of attorney. If your health care power of attorney was executed before October of 2015, it should be updated.

How do I know if laws have changed that may affect my estate planning documents? The best way to is to contact your estate planning attorney for a periodic review of your plan. Many attorneys will also affirmatively contact their clients if relevant laws change. Life can change in an instant. What are you waiting for? THE REALITY OF EQUAL TIME SHARING

s the law stands today, equal time sharing is not presumed to be in the best interests of the children in all situations. I will however state that it is becoming the norm, rather than the exception. It is believed that frequent contact with both parents will result in a better adjusted child. Of course, every case is different and as such, it is difficult to have a set rule for all cases. Parent’s work schedules, travel schedules and locations should be considered. A parent who must leave for work prior to a minor child having to wake up for school could present a problem. A parent who does not have a set schedule such as pilots or firefighters also require some planning. Parents who choose to live a distance apart can also complicate the situation. Of course, the other factor which is always considered is child support and how equal time sharing affects the monthly amount to be paid. Child Support in Florida is based upon several factors, one of which is the number of overnights a minor child spends with each parent. An equal time-sharing schedule will result in a reduced amount of support, since it is presumed that each parent will have significant “out of pocket” expenses if each has an equal amount of time with the child. This concept of equal time sharing has existed for several years now. I have recently met with prospective clients who seem to share the same issue. A parent has equal time sharing pursuant to their Parenting Plan, but in reality, the parent is not actually exercising his/her 50%. The result is that the parent who is not following the schedule is receiving a financial benefit by a reduced child support payment, but is not sharing in the responsibility of caring for the child. Is there a way to correct this situation, even though the parties had previously reached an Agreement? The answer is yes and a correction to this situation can be accomplished by a Petition for Modification. The moving party (the parent who is in fact receiving less support than he/she should) must allege that since the parties entered into their Parenting Plan there has been a substantial change of circumstance, that was not anticipated at the time the Parenting Plan was agreed to. To be successful it is essential that a calendar be kept, listing the actual overnights being exercised by each parent. Just alleging that mom or dad doesn’t have the children 50% of the time, without some additional proof, will probably not be a strong or successful argument if it were to go before a judge. If the Petition is successful, a new time-sharing schedule would be determined and child support would be recalculated based upon the actual number of overnights each party is exercising.

A

nkle arthritis involves the loss of cartilage which decreases joint function and ultimately results in pain, stiffness, swelling, and warmth. As the cartilage wears the bones in the ankle joint grind on each other. There are several types of arthritis in the ankle. The most common types are osteoarthritis, a condition which is often a result of previous trauma to the ankle or leg and rheumatoid arthritis. Rheumatoid arthritis is an immune disease resulting in inflammation of the joint lining and in advanced stages will lead to cartilage, bone, ligament, tendon, and muscle damage. Conservative treat-

ment for these conditions may involve medication, injections, activity modification, physical therapy, and bracing. When conservative treatment fails surgery may be considered. This involves fusing (locking the ankle permanently) or replacing it with an artificial joint as are done in the knee or hip joints. The procedure is referred to as total ankle arthroplasty or total ankle replacement. There are several ankle replacement systems available to the surgeon. Some total ankle replacement systems require more bone removal in order to obtain the desired result, others are referred to as resurfacing systems which remove less bone and essentially replace the surfaces of the joint. Certain ankle replacement systems are used for revision

surgery. Today’s total ankle replacements are essentially anatomic and provide the patient with the ability to reproduce the natural movement of the ankle joint. One of the most advanced systems utilizes the Prophecy technique developed by Wright Medical. The Prophecy technique allows precise implantation of the device by using specialized CT

scan imaging before surgery. Bioengineers will subsequently map out a customized plan for the individual patient which enhances the success of the operation. The alignment of the ankle is crucial as a neutral (straight) ankle will perform the best in the long term, otherwise, ancillary procedures are required to straighten the ankle and foot.

The specific ankle replacement will depend on the patient’s age, level of disease, and level of activity. The success of an ankle replacement is influenced by the condition and quality of the bone, the type and severity of arthritis, the condition of the muscles and tendons around the ankle, ankle alignment the weight, age, overall health of the patient, and finally the patient’s commitment to physical therapy and rehabilitation. The success rate is very good with modern ankle replacements oftentimes lasting from 12-18 years. Studies show that patients preferred the outcome of ankle replacement over the fusion which bonds the joint in one position to eliminate all movement. However, like all surgery, there are potential complications. These may include infection, scarring, nerve damage, and loosening or wearing of the implant.

Postoperative recovery generally requires four to six weeks in a short leg cast or fracture boot with physical therapy starting thereafter. The postoperative period is an added advantage over fusion which in contrast requires several months of immobilization in a cast.
