What are bio-markers?

What is a palatal crib?

A “bio-marker” is a molecule found in body fluids, blood, or tissues. It is a sign of a normal or abnormal internal process, condition, or disease. Bio-markers can also be used to determine how well a person responds to treatment. Cancer bio-markers can be called “tumor markers” and encompass a wide variety of molecules. We usually obtain them from blood and use them to help indicate whether the cancer has progressed or recurred. They are unfortunately not 100% specific and can be falsely positive so we do not use them for screening, but we do use them in some cancers at diagnosis, during treatment to assess response to therapy or after treatment to monitor for recurrence. Newer bio-markers are being developed to assist in early detection.

Raynauds syndrome or Raynauds phenomenon, as it is more commonly referred to, is a typical set of symptoms that occur in the fingers and toes of some people when they are exposed to cold temperatures or stressful situations. The digits will turn either white or blue which is related to the blood vessels constricting. This can be painful or cause numbness. It usually resolves on its own, and this is characterized by the digits turning red as the blood vessels open up and blood flows back through the dilated channels. This is often a benign and self-limited problem, but can be associated with more serious diseases such as lupus or scleroderma. You should discuss your symptoms with your health care provider.

It is attached to the upper teeth and its wire framework is closely adapted to roof of the mouth, just behind the upper incisors. The idea is to decrease the pleasure associated with the sucking habit by preventing the thumb or finger from touching the roof of the mouth and, thereby, decreasing the “suction” that is achieved. However, it is important to remember that there are many psychosocial factors which can be associated with a sucking habit and these must be considered before a palatal crib is recommended. It is important for the patient to understand that the palatal crib is an aid to help them stop their habit and not a punishment. What is the difference between a pandemic and an epidemic?

What is the most common vision problem in adolescent children?

These are easily confused words that scientists use to describe the spread of infections. Many infections happen by unfortunate chance, through contact with our unclean world, like with skin infections. Other infections occur from our contact with people, like the flu. When an infection occurs more often than expected by random chance and the disease is spread between people, scientists call this a human epidemic.

Common vision problems in adolescent children include: farsightedness, nearsightedness, astigmatism, focusing disorders, eye turns or muscle imbalances and amblyopia (commonly known as lazy eye). However, the most common vision problem in school age children is myopia or nearsightedness. With this condition the child can see objects close up but has a hard time seeing objects at a distance. The most common symptoms of nearsightedness are children complaining they cannot see or are having difficulty seeing the board, squinting and headaches.

Epidemics cover areas as small as a classroom or office, and as large as a continent. When an infection spreads to many continents, the epidemic is called a pandemic. Once an epidemic is found, we try to limit contact with infected people. The best way to prevent most epidemics in humans, however, is to vaccinate against the disease before it becomes an epidemic. Even though myopia can occur at any age, it is prevalent in school age children between the third and eighth grade. The main developmental factors for nearsightedness are genetics and visual stress. If a parent or grandparent is nearsighted, the child has a chance of inheriting this same condition. Visual stress can be caused by excessive reading, computer work and video games. Nearsightedness can be easily diagnosed during a yearly eye health exam. Treatment includes prescribing eyeglasses and often contact lenses for children involved in sports. Dr. Ashley C. Why do spicy foods, more so than bland foods, tend to cause heartburn? Spicy foods can make your heartburn worse for a number of reasons. They can directly irritate your esophagus and make it more sensitive to the acid reflux that causes heartburn. This however may be the least likely reason. Some foods can directly lower the resting pressure of your lower esophageal sphincter. This is the smooth muscle “valve” at the bottom of the esophagus where it connects to the stomach. With this “leaky valve” as a result of the lower pressure it is much easier for acidic gastric contents to reflux into the esophagus and cause heartburn. Spicy food that is high in fat can also delay or slow emptying of food from the stomach. When the stomach stays full it makes it easier for acidic stomach content to “spill” over and back into the esophagus. Also spicy food eaters may be overweight and obesity is a major factor in causing acid reflux. Everyone responds to food differently and results of dietary change will vary from person to person. THIS WINTER, IGNORE THE URGE TO HIBERNATE words | RICH ELLIS


There are so many reasons to stay active and physically fit during the winter months, and so many reasons not to. Unfortunately, the reasons “not to” too often win out, and understandably so. It’s cold outside, and dark, both of which make maintaining an outdoor fitness regime all but impossible if you’re trying to get in a few miles, a few holes or a few sets before or after work. We’re also busier than usual this time of year with holiday preparation, travel, parties and, of course, shopping. It all adds up to the death knell for maintaining any consistent exercise, fitness or activity routine from about November through January. Oh sure, the gym gets crazy-crowded come January 1 when all those wellintentioned-but-too-often-unrealistic resolutions kick in, but those crowds and their intentions don’t last. Besides, waiting until the calendar hits a new year to resume any type of physical activity is too late. By then the damage has probably already been done. Here’s one reason why.

Remember those holiday parties mentioned earlier? Yeah, there were a lot of them. October 31 ring any bells? Whether you were “checking” the kids’ candy after they went to bed or grabbing “just one more” miniature pig in a blanket (can anyone resist those?) at the Johnson’s annual masquerade party, this is where it all begins. Then we roll right into Thanksgiving, the office holiday party, the kids’ school party, the neighborhood holiday progressive dinner, Christmas dinner—on two consecutive days so both sides of the family are covered, New Year’s and, if we really are intent on riding the party train into late winter, let’s throw in Valentine’s Day for good measure. Starting to see a pattern emerge? We’re eating, drinking and being merry all winter, which isn’t a problem in and of itself, but quickly becomes one if we’re not counteracting these festivities with any type of regular exercise or activity. Preventing weight gain is one of numerous benefits that local health and wellness experts have identified as reasons to maintain a regular fitness program over the holidays and throughout the winter. Sean Archulet is the fitness coordinator at the YMCA’s Downtown location in Lynchburg, which along with the Jamerson Family YMCA and the Y Express in Lynchburg constitute the YMCA of Central Virginia’s locations. “This is the time we see people really get into ruts,” Archulet explains. “A lot of excuses come up; they’ll say, ‘Well, I’ll wait for New Year’s and get into my New Year’s resolutions,’ but then they’ve fallen so far behind that now they have unachievable goals for their New Year’s resolutions.” Archulet says that consistency in exercising during the winter months is key, particularly because the types of foods people tend to eat over the holidays cause them to put on even more weight, and come the beginning of the year when they weigh in, the result is discouraging. That discouragement, combined with the time change, less daylight and cold temperatures makes it even more difficult for people to get motivated and stay motivated about exercise, Archulet says. “If they can stay consistent during these months, they’re normally great to go by the first of the year and then when it gets warmer outside, they’ve actually accomplished more than they planned on,” he says. Maintaining a level of fitness over the winter also helps with injury prevention when spring arrives and people resume a more active lifestyle outdoors. In addition to consistency, Archulet stresses the importance of having achievable goals when it comes to exercise. For example, instead of setting a goal of losing 20 pounds over the holidays, he recommends a more realistic and achievable goal of not gaining any weight. Another goal, he says, could be to exercise just a couple more days per month. Achievable goals help people see and measure their progress more readily. Winter also is the time when illness levels tend to spike as people are affected with colds or the flu. This is another area in which exercise has positive benefits because it helps boost the immune system. www.ourhealthvirginia.com | 21

While improving physical well-being is an obvious benefit that’s tied directly to staying fit and active throughout the winter, it isn’t the only benefit. Whether it’s due to anxiety caused by family tensions and the increased amount of time spent with family, or financial pressures, or simply a jam-packed, end-of-year schedule, the winter holidays can be stressful and take their toll on mental well-being. Exercise is a proven, natural stress reducer, which is something many people could use more of this time of year. Exercise has also been proven to help prevent or lessen the effects of seasonal affective disorder or SAD. SAD may be tied to diminished levels of sunshine and causes some people to experience depression and feelings of lethargy during the winter months. Since weather and lack of daylight are often the primary reasons people cite for their activity levels dropping off during the winter, it’s important to remember that fitness centers—or gyms or sports facilities or whatever terminology is being used to describe them—can be an easy and fun outlet for maintaining activity levels. Modern fitness centers are far removed from the outdated image some people have of a gym: a lot of free weights and many very muscular people standing around lifting them. Instead, today’s fitness centers offer a wide variety of family fun and fitness for all ages, from children to senior citizens. At the Y, members will find something for everyone, Archulet says. Thirty different aerobics classes, racquetball, basketball, yoga, water fitness and a seemingly endless array of exercise and strengthening equipment are just a few of the features there that can help encourage people to stay active at a time when it’s the most difficult to, but also the most important. Archulet says that software and apps, such as ActivTrax, are also proving increasingly popular at the Y for both trainers and members as a way to track progress toward goals on a more personal level. The Y’s new CrossFit affiliation is an added benefit that Archulet says has seen a positive reaction as more than 60 members have signed up already for the fitness program. And in December, the Y’s new aerobics studio will open, allowing for larger class sizes in a room dedicated for that purpose. But perhaps the Y’s and other fitness and health and wellness centers’ most important offering is the accountability that they encourage in members to maintain their fitness program over the winter, by having a dedicated place to exercise, clearly defined goals to move toward and friends to exercise with. After all, if you’re going to partake of all the fun that the holidays offer, and chances are you will, wouldn’t it be more enjoyable to experience those festivities guilt-free and feel great throughout the winter months? This year, you can, simply by making a commitment to staying active during that time of year when it’s the most difficult to be active.

With all the rhetoric about the nation’s new healthcare law, it seems that it’s getting tougher and tougher to find accurate, reliable information on what the law includes, what it does not include and how to sign up for health insurance. The problems become especially challenging because a great deal of the information out there is wrong, intentionally or otherwise. Most Americans have only very vague information on what the law does and does not do. That information gap, combined with the extreme political volatility wrapped around the healthcare law, has created plenty of opportunities for those on all sides of the debate to define the new law in ways that serve their own political purposes. And the flawed launch of the government’s healthcare site—an especially catastrophic rollout in states like Virginia that have opted to be included in the federal Medicaid expansion rather than craft their own— makes it even more difficult and frustrating to get clear, accurate information. Even the highly respected Consumer Reports took to task a well-known conservative television personality who misconstrued the law in an effort to discredit it. But our bodies are not members of any political party. And there is arguably nothing more important than access to healthcare. That’s why OurHealth took a look at many of the vehicles that purportedly bear information on the new law, to try to find the sources that include reliable, accurate facts, stated in a clear and understandable way. www.ourhealthvirginia.com | 25

First, the basics: The Patient Protection and Affordable Care Act (ACA), otherwise known as “Obamacare,” became the law in March 2010. In June 2012, the U.S. Supreme Court upheld the constitutionality of the law, and the law was a major issue in the 2012 election that returned President Barack Obama to the White House. The ACA represents the most significant regulatory overhaul of the country’s healthcare system since the passage of Medicare and Medicaid in 1965. Its purpose is to make health insurance available and affordable to the estimated 14 million Americans who do not have it. Among the major features of the new law: • Insurers will not be allowed to turn people down or charge higher premiums for preexisting medical conditions. • Almost all legal residents of the U.S. will be required to have health insurance of some kind by a March 2014 deadline. Most people who already have health insurance can keep the coverage they have if they are satisfied with it. • People who remain uninsured may be subject to a fine at income tax time. • To make health insurance more affordable, low-income and moderate-income households will receive financial assistance to assist with insurance premiums, and some households will qualify for reduced out-of-pocket costs.

The signup period for health insurance began October 1, over the Internet, via tollfree telephone and through a network of independent insurance agents (who are paid by commission from the insurance companies—not by you, the buyer). Coverage begins as soon as January 1, 2014, if you have signed up by this December 15, and most people must have coverage by the end of March 2014.

So those are the basics. Now, where to go for details? A cautionary note: please rely only on sites not linked to a political party. Sites linked to any political party are controlled and staffed by individuals who are committed mainly to the success or failure of the healthcare program, and information on those sites is inescapably biased. That said, there are a number of good places—especially on the Internet—to go for help. Chief among them would be the Health Reform section of the Henry J. Kaiser Family Foundation website (http://kff.org/health-reform/fact-sheet/summaryof-new-health-reform-law/) and its outstanding downloadable Summary of the Affordable Care Act. The Kaiser Foundation is a universally respected, independent organization that has become a trusted source of information in a healthcare world dominated by vested interests. The downloadable Summary of the ACA, although a bit lengthy at almost 13 densely packed pages, is very clearly written, organized in easily digestible sections and is eminently trustworthy. The ACA is a federal law, and federal laws are long. The law itself numbers some 900 pages (not 33,000 as some have claimed). So to boil all that language down into 13 understandable pages is something of a feat. Want to read the law for yourself? Go to the website for the Federal Register (https:// www.federalregister.gov) and do a search for “Affordable Care Act.” You’ll see 897 items on the law and associated documents. (Unfortunately for people who do not have access to the Internet, the most upto-date information is available online. There is some good printed information available—look for information produced by the U.S. Department of Health and Human Services—but printed information cannot be updated as quickly as online information.) The venerated consumer information organization Consumer Reports is also a major leader in making information available on the ACA. Its masterful work on the ACA is available for everyone (http://www.consumerreports.org/cro/health/insurance/ health-care-countdown/index.htm) and is a constant work in progress, including what will eventually be a list of more than 100 articles covering every aspect of the new healthcare law.

As we approach the deadlines for participation in the Affordable Care Act, we at OurHealth think we can best serve our readers by preparing frequent and useful articles on what the act entails and how it may affect you. Consumer Reports (www.consumerreports.com) provides not only information on the law, but also a nifty guide to registering for health insurance on www.healthcare. gov, the balky Health and Human Services website that is the focus of so much attention while bugs are being worked out. And, especially if it is repaired by the time you read this, the “official” Obamacare website, along with the Virginia companion site (www.enroll-virginia.com), is the absolute best way to register for health insurance on the Internet. Via phone, reach the registration site at 800.318.2596. The federal government site (www.healthcare.gov) is the front door to enroll in healthcare coverage, and it is also the best way to avoid scam artists, who are becoming more active as the deadline draws near. Lynchburg Dental Center (LDC) has been serving the needs of patients in the greater Lynchburg area since 1974. As technology has improved and evolved, so have the services offered to patients. Hearing loss affects about one in ten Americans. We are the area’s only hearing aid center with medical doctors and audiologists. We are also trained in both the medical and surgical treatment of hearing, ear infections, balance disorders, ringing or ear noise (tinnitus) and other related conditions. Otolaryngologists also manage congenital (birth) disorders of the outer and inner ear. About 35 million people develop chronic sinusitis each year, making it one of the most

common health complaints in America. Care of the nose and sinuses is one of the primary skills of otolaryngologists. Management of the nose includes allergies and sense of smell. Communicating (speech and singing) and eating a meal all involve the mouth and throat. Ingredients: 1 cup golden raisins 1/2 cup sun dried cranberries 1/2 cup sun dried cherries

1 teaspoon baking soda 1 teaspoon baking powder 3 Omega-3 eggs 1/2 cup toasted pecans, broken

1/2 cup dried apricots, chopped

Zest of one lemon, chopped coarsely

Zest of one orange, chopped coarsely

1/4 cup candied ginger, chopped 1 cup rum 1 cup Stevia 5 ounces unsalted butter (1 1/4 sticks) 1 cup no sugar added apple juice

Directions:

1. Combine dried fruits, candied ginger and both zests. Add rum and microwave for five minutes to re-hydrate fruit. 2. Place fruit and liquid in a pot with the Stevia, butter, apple juice and spices and heat mixture to a boil stirring often. Then reduce the heat and simmer for five to ten minutes.

3. Remove from heat and cool for at least fifteen minutes. (Bring batter to room temperature before proceeding.) 4. Heat oven to 325 degrees. 5. Combine dry ingredients and sift into fruit mixture. 6. Quickly bring batter together with a large wooden spoon, then stir in eggs one at a time until completely mixed, and then stir in the nuts. 7. Pour batter into a 10-inch non-stick loaf pan and bake for an hour. 8. Check cake by inserting toothpick into the middle of the to see if it is finished. If it comes out clean, it’s done. If not, allow to bake an additional10 minutes. 9. When cake is completely cooled, seal in a tight sealing, food safe container. The longer you allow the cake to sit over the next week or two, the richer the flavor will become.

Healthy Sweet Potato Pie: Makes 8 servings

Directions:

1. Boil sweet potatoes whole in skin for 50 to 55 minutes, or until soft. 2. Run cold water over the sweet potato, and remove the skin. 3. Blend potatoes in a blender and pulse for about one minute.

Ingredients: 2 large sweet potatoes 2 tablespoons organic butter, softened 1/2 cup local honey 2 tablespoons molasses 1/2 cup 2% organic milk 2 large omega 3 eggs 1/2 teaspoon ground pumpkin pie spice 1/2 teaspoon ground cinnamon

4. Place sweet potatoes in a bowl, add butter, and mix well. 5. Using an electric mixer, mix in honey, molasses, milk, eggs, cinnamon and vanilla. 6. Beat on medium speed until smooth. 7. Pour contents into an unbaked pie crust. Bake at 350 degrees for one hour, or until knife comes out clean from the center of the pie.

Advancements in medical technology have revolutionized how patients and their families approach healthcare. Technology has empowered people to research, ask more in-depth questions and seek aggressive treatments. Proactive, preventative screenings and tests allow many life-threatening diseases—such as cancer—to be detected early, significantly increasing survivor rates.

The early detection of lung cancer and smoking cessation significantly increases the survival rates and provides the best chance of survival when accompanied by routine screenings.

The invention of the x-ray in 1895 transformed medical diagnosis and treatment.2 This continually improving tool allows medical professionals to see concerns below the skin’s surface without ever making an incision. However, limitations occur because there is no depth to the images and make it difficult for physicians to see concerns from different angles. This limited visibility hinders the diagnostic power of a simple x-ray. Therefore, inventors and engineers, connected to the medical community, continued to research new ways to improve diagnostic equipment. Subsequently, the evolution of computers led to the invention of a computed tomography (CT) scan in the early 1970’s. Continual improvements in this technology now allow this scan to provide three-dimensional, detailed images of the internal workings of our bodies with much greater clarity than traditional x-rays. The depth and resolution (clarity) have greatly improved in the last three decades, which has led to the increased use of CT scans in medical facilities around the world. CT scans, sometimes referred to as “CAT scans” allow physicians to pinpoint precise locations of blood clots, infections, tumors, or other potentially fatal threats to improve early diagnosis and treatment. These scans also allow surgeons to perform a biopsy with much greater accuracy. The Federal Food and Drug Administration (FDA) reports that CT scans also allow medical professionals to monitor the effectiveness of therapy, such as cancer treatments.3 A study performed from 2002-2010 by the National Cancer’s Institutes National Lung Screening Trail (NLST) concluded that 20% fewer lung cancer deaths occurred with patients receiving a low dose spiral computed tomography scan compared to a traditional chest x-ray.4 However, CT scans are not without risks. A recent article reposted by USA Today reports that CT scans deliver 70 times more radiation than traditional x-rays.5 “Most scientists and regulatory agencies agree that even small doses of ionizing radiation increase cancer risks, although by a very small amount,” reports the American Cancer Society. Additionally, CT scans can produce false positives, leading to possible unnecessary biopsies and undue stress and cost to patients and their families. Therefore, some patients are opting to completely avoid the potentially life-saving screening. Although low, most providers agree the value of the information gained by CT scans far outweighs the risks from cumulative radiation exposure and the risks from no screening at all.

Lung Cancer—The Silent Killer Heavy smokers and former smokers have a 1 in 7 lifetime risk of developing lung cancer. Lung cancer causes almost 160,000 deaths in the US every year—more than breast, prostate and colorectal combined. Approximately 90% of all lung cancers are caused by smoking. 85% of all lung cancer patients die because the disease is usually detected late. Annual screening of smokers—leading to early detection—will save a significant number of lives.

“He’s tired, so tired,” says Laura. “Since I can remember, everyday for him has been an 18-hour day on the farm. He’s goes to the doctor for a check-up about once a year, but doesn’t really take the doctor’s advice. He loves my mom’s cooking too much and he won’t put down those cigarettes he smokes. I know it has to be catching up with him.” Laura’s concern for her father’s years of unhealthy choices is not unwarranted. A poor diet that has contributed to him being overweight and years of smoking have him at risk for several conditions, including lung cancer. “I worry about lung cancer all the time,” Laura continues. “My grandfather and my uncle – Dad’s father and brother – died from it. Granddaddy went to the hospital and was diagnosed with lung cancer the same day he died. My uncle found out he had lung cancer after he had a heart attack. He lived three months. Dad gets so winded doing his work. I know he’s getting older and slowing down, but he’s not that old.”

PAULA’s Test (Protein Assay Using Lung cancer Analytes) is a blood test that identifies asymptomatic patients who should be considered for follow up. It measures a panel of protein biomarkers associated with lung cancer. An elevated score indicates the possibility of early stage lung cancer and should be investigated further.

Not wanting to see her father continue the cycle, Laura began researching to seek the latest technological enhancements available on her father’s behalf. Options range from traditional x-rays, to more detailed computed tomography (CT) scans. Her options now also include the recently released PAULA’s test (Protein Assay Using Lung cancer Analytes). Laura’s father is not showing significant enough symptoms for his primary care physician to recommend a CT scan of his lungs, but a heavy smoking history and being over the age of 50 makes him a prime candidate for the new blood test. According to the American Cancer Society, lung cancer causes more deaths every year than breast, prostate and colorectal cancer combined.1 This startling statistic has many family members, like Laura, asking new questions of their doctors. The early detection of lung cancer and smoking cessation significantly increases the survival rates and provides the best chance of survival when accompanied by routine screenings.6 Sadly, once symptoms appear, there is an 85 percent death rate from the disease.7 Therefore, researches have been working diligently to develop new methods for the early detection of lung cancer. conducting research and recently released a new, pre-screening blood test created to detect tumor proteins in at-risk lung cancer candidates. Genesys Biolabs consists of a team of PhD and MD level scientists and a management group with extensive experience in the diagnostics industry. Peter Tunon, Vice President of Personalized Medicine

The test was developed to help identify which patients who should be more thoroughly examined without threatening radiation use. “Although the test is not a conclusive test,” explains Peter Tunon, Vice President of Personalized Medicine, “It will give patients and physicians a much more clear indicator if a CT scan or other diagnostic procedure should be requested.”

A definitive lung cancer diagnosis can only be made through a tissue biopsy. The PAULA’s test is named in memory of Paula Shore, who lost her life to cancer in 2005. The blood test measures a panel of protein biomarkers that are commonly associated with lung cancer; three tumor proteins and one additional analyte (a substance that is subject of a chemical analysis). The biomarkers in the panel are then used in combination to generate a composite score. An elevated score indicates that the patient may have early stages of lung cancer according to information supplied by Genesys Biolabs. Those who may benefit from the PAULA’s test are high-risk (defined as those with a smoking history of 20 or more packs per year), asymptomatic smokers and former smokers over the age of 50 who are not receiving annual CT scans. The test can be

requested and completed in a physician’s office through a simple blood draw. Tests are then sent to Genesys’s federal CLIA (Clinical Laboratory Improvement Amendment) certified laboratory for analysis and results are returned in a physician’s report within a few days. Currently, lung cancer is often detected in the late stages after patients have developed a chronic cough or other severe symptoms. The PAULA’s test is being found to improve the detection of early-stages of lung cancer without the repeated exposure to cumulative radiation and the risks of false positives.

“Physicians know that patients have an 80% five-year survival rate if lung cancer is caught early (stage 1A). Studies have shown that people who receive a clinician’s recommendation for cancer screening are more likely to be screened than those who do not receive a recommendation.8 Therefore, it’s critical for healthcare professionals to be aware of life-extending options such as the PAULA’s test. CLEARED FOR TAKE Bill Glahn, age 79, shares his story of how his restored vision has given him back his independence— and his wings.

Glahn, of Lynchburg, is one of more than 2 million Americans suffering from macular degeneration, a chronic, primarily genetic eye disease causing damage to the macula, resulting in central vision loss.1 Damage to the macula affects the sharp central vision needed for important activities like driving and reading. The most common form of macular degeneration develops as a person ages. This is known as age-related macular degeneration (AMD). According to the Eye Diseases Prevalence Research Group, approximately 7 million Americans have a high risk of developing AMD, with close to 3 million projected to suffer from the disease by 2020.

There are two types of AMD: dry macular degeneration and wet macular degeneration. According to Golnaz Javey, MD, of Piedmont Eye Center in Lynchburg, approximately 10 percent of patients suffering from AMD have wet macular degeneration. “This form is progressive in nature and patients tend to lose vision very quickly if not treated. This is what Mr. Glahn suffers from,” she explains. The vision loss in wet AMD is caused by abnormally leaky blood vessels that leak blood or fluid into the macula, causing damage to this portion of the retina. AMD often goes undetected until it reaches more advanced stages, since noticeable symptoms, including vision loss, don’t occur in the intermediate stages. Symptoms in the more advanced stages of the disease include blurred vision, vision waviness and loss of brightness or color in sight. Dr. Javey also explains there is no cure for macular degeneration, only treatment to slow down vision loss. “It is very important for people who have a family history of macular degeneration to have routine annual screenings.” Early detection and treatment of wet macular degeneration may help reduce long-term vision loss and greatly improve immediate vision for many.

Glahn has always had poor vision and relied on corrective lenses. However, he was fortunate is some ways. Because of his eyesight, he routinely scheduled annual appointments with his eye doctor. As a result of his proactive eye care screenings, Glahn’s doctor was able to closely monitor his eye health and several years ago had recommended Glahn undergo cataract surgery to improve his vision. Once that surgery was complete, his vision was much better—for a little while. A few years after his surgery, he started noticing some alarming symptoms. “It was like somebody turned off the lights,” he shares. “I would walk into a room, in the middle of the day, and have to turn on all the lights.” His vision was so dim he had to stop driving at night and could no longer ride his motorcycle. Glahn, a certified pilot, also knew he would fail his medical exam allowing him to renew his pilot’s license without some type of treatment for his deteriorating vision. This loss of independence was frustrating for Glahn and prompted him to once again call on the specialists at Piedmont Eye Center for consultation. “I always had high regard for Piedmont Eye Center. When I needed a doctor for my uncle about 15 years ago, that’s where I took him. They were friendly and efficient,” says Glahn. “It only made sense to make my appointment with them.” After a thorough evaluation almost two years ago, Dr. Javey, a board-certified retinal specialist, diagnosed Glahn with wet macular degeneration. After reviewing several treatment options, Dr. Javey recommended a nonsurgical treatment. The new drug called EYLEA® was approved by the U.S. Food and Drug Administration (FDA) in November 2011. EYLEA® (aflibercept) is administered by an ophthalmologist through injections in the eye every eight weeks (compared to every four weeks with previous treatments). “This drug cuts the number of needed treatments in half,” explains Dr. Javey. 76 | www.ourhealthvirginia.com

The injections work by blocking the effects of a substance (vascular endothelial growth factor) that promotes the growth of new abnormal blood vessels in the retina. Upon Glahn’s arrival at Piedmont Eye Center he answered questions and completed preliminary paperwork and then sat through a routine exam. “He would be one of our first patients to receive this treatment,” says Dr. Javey. The nurse prepared him for the injections by placing numbing drops in each eye. “The needle is only in your eye for a second and I had no pain at all,” says Glahn. Each visit lasts about an hour and a half total. The actual procedure takes only about 10 minutes. Patients are monitored closely after the treatment and once it is confirmed their intraocular pressure is normal, they are free to leave. “It [the treatment] really works,” exclaims Glahn, who saw dramatic results in just two or three days. “It was like somebody turned on the lights!” Glahn, almost two years after beginning treatments, can now see 20/30 with corrective lenses and has many opportunities afforded back to him, including daily tasks that many take for granted. “I can drive at night with great confidence! I can ride my motorcycle again, read novels and complete crossword puzzles,” he shares. “Dr. Javey also helped me keep my pilot’s license; since I started treatments, I am able to maintain my thirdclass medical certificate. A MINOR INCONVENIENCE, OR SOMETHING FAR MORE SERIOUS?


Heartburn again.

No problem. Just reach for the Tums or Mylanta and in a little while everything will be all right. … Maybe. … Maybe not. Heartburn, or gastroesophageal reflux disease (GERD), is a common complaint among a large swath of the population. But what many people don’t realize is that in some cases, left untreated, heartburn can cause life-threatening side effects. Bikram S. Bal, MD, who is board-certified in internal medicine and board-eligible in gastroenterology, recently joined Centra Southside Community Hospital and is opening Centra’s new Southside Center for Gastrointestinal & Liver Diseases office, located on the hospital campus. He says that heartburn can mean different things to individual patients.

“There are a variety of symptoms that a lot of patients would describe as heartburn and I think the spectrum goes from people describing a sour taste in their mouth, nausea, all the way to what you or I would actually think of heartburn, as burning in your chest,” Dr. Bal says.

What it is, and what causes it “Heartburn is really a symptom that comes up when people are having acid reflux,” Dr. Bal explains. “The acid in the stomach is refluxing back into their esophagus or sometimes into their mouth and they are experiencing the burning sensation and describing it as heartburn. It’s important to remember that not every patient experiences heartburn’s symptoms in the same way.”

Acid refluxing into the esophagus can often be traced to one of two things— increased production of stomach acid or improper functioning of the body’s normal mechanisms that are supposed to prevent the acid from rising up from the stomach. “If you are having food that is harder for your body to digest, which is food that is high in fat content or very high in carbohydrates, your body is going to make more acid and some of that acid is coming back up into the esophagus.” Dr. Bal says heartburn is on the rise nationwide, and that a lot of that increase has to do with these changing diet patterns. A second reason for acid refluxing back into the esophagus is the malfunctioning body mechanism that Dr. Bal referred to previously. “At the end of the esophagus, where the esophagus meets the stomach, there is something called a lower esophageal sphincter that normally stays closed and only opens when food is going down,” he explains. “In some cases where there is a hiatal hernia, your lower esophageal sphincter is herniating into the chest, and it is incompetent and allowing the acid to reflux back.” Obesity is a frequent contributor to the lower esophageal sphincter’s not working properly, and given the rise in obesity rates, it’s another reason why heartburn is on the rise. “Obesity is a big-time contributor,” Dr. Bal explains. “When you have a lot of fat sitting around your midsection, when you lie down or sit back, that fat will press on the lower esophageal sphincter and then it will sort of splint it open and when it splints open, that’s when you start having the reflux.”

When to seek medical help While many people suffer from heartburn occasionally, not everyone needs to seek medical treatment to alleviate their symptoms, and Dr. Bal says patients should consider several factors when deciding whether they need assistance. The first is to what degree the heartburn is affecting quality of life. If heartburn is being experienced only a couple of times a month, then it’s probably something that can be handled with an over-the-counter heartburn medication on an as-needed basis and medical treatment isn’t necessary.

If, however, the heartburn occurs regularly and causes the patient to avoid eating the foods they enjoy, Dr. Bal views that situation as affecting quality of life to an unacceptable degree and recommends a visit to a primary care physician or gastroenterologist. Similarly, if a patient has to take an over-thecounter medication to alleviate heartburn symptoms four or more times a week, Dr. Bal recommends they seek help. A third situation in which medical help is warranted for heartburn is when patients experience complications. “You can have complications in terms of developing ulcers in your esophagus, which makes swallowing very painful. Or you can develop scarring in your esophagus, which actually causes food to start getting hung up in your esophagus, which is called dysphagia,” Dr. Bal explains. “Those are the real red-flag signs. If you are having pain in your chest when you swallow, or your food is getting hung up when you swallow, you should seek immediate medical attention.” When Bernice Spears, 72, a Charlotte Courthouse resident, experienced chronic coughing that prevented her from sleeping through the night, she knew she needed help and turned to her primary care physician, who in turn referred her to Dr. Bal. She had been experiencing heartburn for six or so years, she recalls, and was already on a prescription medication to help treat it, but the chronic coughing being caused by acid reflux showed that additional measures needed to be taken. “Dr. Bal performed an endoscopy on me and saw that I had acid reflux and Barrett’s esophagus,” Spears explains. Barrett’s esophagus is a condition in which the tissue lining the esophagus begins to change as a complication of GERD and the chronic exposure to acid. The condition is more prevalent in white males and its presence increases the risk of developing cancer of the esophagus. “He put me on medication that I’ll have to take the rest of my life but it got it under control. I don’t wake up coughing and I can eat whatever I want.” Spears’s case is unique in that not every patient with heartburn symptoms needs an endoscopy.

Diagnosis and treatment “The diagnosis is symptom-based,” Dr. Bal explains, “such as if you come into my www.ourhealthvirginia.com | 81

office and say ‘I’m having burning in my chest’ or ‘I’m getting this sour taste in my mouth.’ One of the things that people often don’t realize is that they are waking up in the night, coughing.” That’s a symptom too, because when you’re lying down, gravity doesn’t help the acid stay down in the stomach and it’s actually coming up into the chest. Those symptoms, particularly if they occur after a heavy meal, are classic signs of heartburn. Based on a patient’s symptoms and their age, there are several treatment options. The first, Dr. Bal says, is to simply treat the patient with the appropriate medication and not perform any type of diagnostic tests, such as an endoscopy. A second option is to perform an upper endoscopy to see if damage has been done to the esophagus and, based on the findings, prescribe the appropriate medication. At the end of the day, however, the easiest first course of action for many patients to try in combating their heartburn is lifestyle modifications. Dr. Bal said those should include: • Don’t overeat • Avoid heavy, fatty, fried foods • Avoid eating three hours before bed • Stop smoking • Lose weight Close attention to heartburn’s symptoms, its impact on quality of life and any “red flag” warning signs that may be present can help patients decide whether their heartburn is a minor inconvenience a couple of times a month, or a more serious condition that warrants expert medical care.
