What is mild cognitive impairment?

Mild cognitive impairment is a disorder in which the individual experiences cognitive deficits of a more serious nature as opposed to those of the normal aging process. For many, it is normal to experience some forgetfulness as aging occurs. However, individuals with mild cognitive impairment may experience increased memory loss, the inability to recall important events, loss of train of thought in conversation, and poor judgment. There may be feelings of anxiety, depression, or even indifference. Friends and family members may initially perceive the deficits. A visit to the primary care provider should be the first step in diagnosis. Advanced age, diabetes, hypertension, elevated cholesterol, and depression are thought to be a few of the risk factors for development of mild cognitive impairment. Further assessment by the medical provider will assist with diagnosis. Some individuals with mild cognitive impairment may advance to a diagnosis of dementia, even Alzheimer’s disease, though not all individuals with cognitive impairment will develop dementia.

Regional Anesthesia is basically a way of providing pain relief without necessarily needing to be asleep. It can be used for "awake"surgeries or to limit or even completely block the most intense pain during the recovery period. Some examples of regional anesthesia include spinals, epidurals, and peripheral nerve blocks. Peripheral nerve blocks are frequently used to provide post operative pain control for patients undergoing the more painful orthopedic surgeries, such as an ACL repair for the knee or a rotator cuff repair for the shoulder. These blocks are performed by injecting a small amount of local anesthetic (numbing solution) around the specific nerve that sends the pain signals from the operative area to the brain. These nerve blocks can last anywhere from 12 hours to over 24 hours depending on the solution used and the patient's particular anatomy.

Stress Urinary Incontinence, or SUI, is urinary leakage associated with any increased abdominal pressure such as coughing, sneezing or laughing. It is most common in women who have had multiple childbirths. Other risk factors include prior pelvic surgery such as hysterectomy, menopause, advanced age, excessive weight and smoking. All of these conditions can lead to weak pelvic floor muscles which don’t properly support the urethra when a woman strains, allowing urine to leak from the bladder. There are no medications currently used to treat SUI. Initial (and preventative) measures include kegel exercises to strengthen the pelvic floor muscles. Should these exercises fail, additional treatments are available from urologists or gynecologists. While dozens of procedures have been developed to treat SUI, most surgeons agree that sling procedures to support the urethra are the most successful. It is important to know that the FDA controversy surrounding mesh does not apply to procedures used to treat SUI. To find out which treatment is right for you, begin by talking to your physician about your symptoms and, if necessary, a urologist or gynecologist can tailor the therapy to address your specific condition

Is sunscreen needed in the winter, or does it only need to be worn during the hot summer months? Excellent question! There is definitely a common misunderstanding that sunscreen is only important during the summer or on “sunny” days. This is simply not true. Most of the sun’s damaging rays reach the earth all year round. Many of these rays are not blocked by the ozone layer, clouds, or ordinary window glass, and many do not produce significant heat or visible light. This makes it difficult to recognize our true exposure to the sun, and we all receive far more exposure than we may believe. The UV radiation in these rays is very damaging to our skin, and exposure over time results in pre-cancerous and cancerous skin growths. UV damage is also the major cause of skin aging (wrinkled, discolored skin). For these reasons, it is very important to protect exposed skin, such as the face and neck, with broad-spectrum sunscreen of at least SPF 30 every day and all year long.. Jason D. What is the difference between an endemic, and epidemic and a pandemic?

How can spinal bracing help in the treatment of juvenile scoliosis?

Endemic means a specific disease transmission is common in a certain geographical location. For example, malaria is common in warmer tropical regions of the world. These higher temperatures allow mosquitoes that transmit malaria to flourish. When a person is bitten by the malaria infected mosquito, transmission may occur. The World Health Organization (WHO) and The Centers for Disease Control and Prevention (CDC) track disease transmissions throughout the world and can identify diseases that are common or endemic to certain regions.

Having scoliosis can compromise normal musculoskeletal development in young patients and can adversely affect normal organ function. Scoliosis can occur at any age, but the most common treatable type is adolescent idiopathic scoliosis. This affects more female children than male, and typically occurs at or just before age ten. The general idea is to diagnose the scoliosis early in the child's development and brace the curves if they are progressing rapidly or are greater than an acceptable degree. Treating a scoliosis curve with a spinal brace greatly decreases the child's chances of having pain or deformity later in life. Bracing is possible with flexible curves by creating forces generated on the asymmetries of the torso; and bracing allows the child to participate in the treatment of their scoliosis. When the child is shown the X-ray or observes physically how the spine is being corrected, their compliance and outcomes typically improve.”

An epidemic, also known as an outbreak, occurs when more cases of a disease are transmitted above what is expected. Depending on the disease, an epidemic could impact a few people or great numbers of people within a region. A pandemic occurs when there is widespread disease transmission over multiple countries. Pandemics usually involve novel, or new, viruses that spread rapidly from human to human. The WHO declared the H1N1 Influenza outbreak of 2009 as a pandemic. Alan Pearson donates $200,000 for Nursing Scholarships Alan B. Pearson, a local philanthropist and major donor to Centra recently contributed $200,000 to the Centra Foundation towards nursing scholarships for students at Centra’s College of Nursing. With this gift, his combined donations make it one of the largest personal gifts ever made to the Centra Foundation, now exceeding over $4 million.

Pearson has a long history of supporting Centra’s College of Nursing. In 2013, he contributed $100,000 to the College, again, earmarked for nursing scholarships. “I am delighted to donate this money that is strictly for scholarships,” said Alan B. Pearson. “This is money to get an education started or money for those already in school to keep them moving forward and money for current nurses to continue their education,” says Pearson.

Best Bedside Manner Awards Medical expertise and compassionate care should always go hand-in-hand when it comes to treating patients. The Voting Process In May 2014, a direct mail campaign announcing voting for the 2014 Best Bedside Manner Awards was sent throughout the Lynchburg and Southside communities. Lynchburg Dental Center Adds New Doctor Lynchburg Dental Center (LDC) is celebrating their 40th anniversary. The practice has been serving the needs of patients in the greater Lynchburg area since 1974 and as technology has improved and evolved, so have the services offered to patients. Their growing team offers comprehensive dental care in a comfortable, technologically advanced environment. Dr. Brad Lentz has recently joined the Lynchburg Dental Center team to assist with the growing patient base. He attended the University of Pittsburgh to obtain both a Bachelor’s of Science degree in Chemistry and his Doctorate of Dental Medicine. He completed a General Practice Residency at the Veteran Affairs Medical Center in Pittsburgh, Pennsylvania. 1. Step forward with right leg and keep left leg back. Step should be comfortable and allow you to keep balance. You should be able to feel the stretch in your gluteus and hamstring muscles.

3. Extend the arms out straight so they are parallel with the ground, palms facing down 4. Initiate movement by inhaling into the belly, and unlocking the hips, slightly bringing them back. The line between healthy and unhealthy gets fuzzier every day with blogs, articles, tweets, pins, and broadcasts continually reporting that everything, from food to diet and exercise, is slowly killing us. With the line so blurry between seemingly healthy lifestyle choices and the reports of things that are quietly leading to human demise, how does one push forward and make good choices? To begin with, do a little research on health topics to try to get an idea of what the media has sensationalized and what may actually be factual, and build your healthy lifestyle around it. Diet and nutrition seem to be the top choice for exaggerated fact and information, such as perpetuating reports that all sugars are deadly, meat and dairy are assassins, and carbohydrates slowly dig your grave. But what about the misleading “healthy” substitutes, such as “light” salad dressings filled with sugar and sodium or diet meals packed with preservatives and high levels of the chemical diacetyl?


In the end, there’s no one-size-fits-all recipe for a perfect diet. Nutritionists and dietitians are still dusting off the color-coded food pyramid and advising consumers to plan meals and diet based on Body Mass Index (BMI), health and weight goals, and FDA-suggested daily intake levels. Tricia Foley, RD, MS, OurHealth’s resident registered dietitian, says moderation and balance are important when considering nutrition. “According to the current dietary guidelines, it is important to have variety in our diet, however consuming processed and or refined foods on a regular basis could have negative implications on our health.” That’s why the federal government has developed standards and guidelines for acceptable ranges for macronutrients (carbohydrates, protein, and fat) and dietary references and upper limits for micronutrients (vitamins and minerals). You can log onto the website myplate.gov to find individualized information on how many calories per day you should consume and what percentage of your diet should come from each macronutrient. “Eating a variety of nutrients in the right balance is required for a healthy body” says Tricia. “Eliminating or eating certain nutrients in excess could lead to vitamin or mineral imbalances and/or chronic disease.” While it isn’t a perfect science, with moderation and a customized diet, nutrition is easily sustainable day-to-day. What about diet and nutrition’s partner in crime— exercise? The mantra used to be that all exercise is good and healthy, helping build muscle and eliminate fat, essentially adding years to a person’s life. But now, conflicting reports claim that certain exercises are detrimental to the human body or even that exercise itself contributes to injury and long-term health complications.

Here again, as with many things in life, there is fine print associated with each study, as well as exercising in general. It can be summed up like this: all exercise has risk associated with it when it’s pushed to extreme levels. When choosing a fitness regimen, it’s always best to alternate strength training (weights, machines) with cardio (running, cycling). This is an approach used by fitness professionals because it gives the body time to heal, keeps exercise fresh and non-routine, and allows muscles and ligaments to move differently each time.


Trainers and physical therapists also note that people with heart disease and other potentially harmful conditions should consult with a doctor and fitness professional before creating a workout regimen. Much like nutrition, each body needs a custom plan to ensure physical success and the results you want. While nutrition and exercise are outward-facing issues to tackle, mental health and stress also affect us and our day-to-day lives. With so many stress studies on the market leading to therapy and medication, what can truly be trusted as sound advice to combat the issue proactively, and are the media and research studies correct in labeling all stress as harmful and negative? When it comes to stress, not all of it is considered harmful to our health. There are varying types of stress, including eustress, distress, and neustress. For example, eustress is positive for the most part. Some may think of marriage or the arrival of a newborn baby as stressful, but it’s not necessarily a stressful situation that is negative. Most individuals, though, normally think of distress as negative. The human organism needs a degree of stress to make us better, more productive, and more successful at our endeavors. An important factor when dealing with stress is something most humans take for granted and do not factor into a healthy living plan—sleep. Stress levels affect nutrition and a human’s inclination to exercise or be active, and stress levels go up considerably with inconsistent sleep patterns. Erratic sleep also makes us feel hungrier, which in turn leads to weight gain. So at the end of the day, the old adage “everything in moderation” rings surprisingly true in the face of studies and reports claiming that everything in life is deadly. Moderating the intake of high-calorie or sodium rich food can improve overall nutrition. Incorporating a purposeful, active exercise regimen can improve cardiovascular health while assisting in weight-loss, and maintaining a consistent sleep schedule and a low level of harmful stress improve day-to-day interactions and health. Don’t let the supposed studies and media worry you into an early grave. Moderation and educated lifestyle choices are the keys to a healthy life.

Living with HIV

This series explores diseases that can be devastating to the individuals and families they affect—yet no one is talking about them. That is, until now. In each edition of Our Health, we bring these medical issues into the light—and dedicate these stories to the courageous patients and families living with them and the healthcare providers and researchers who commit their lives to treating and seeking cures for these enigmatic conditions. ✦✦✦ In the late 1980s, William’s life could best be described as out of control. In his 20s and living in Pittsburgh, William (his name has been changed for privacy) was married to one woman and living with another. He also was addicted to crack cocaine. To fund his drug habit, William says he began “prostituting” himself, having sexual relations with a gay male friend. Little did he know, things were about to get worse.

“He was already infected, but he was told at the time not to tell anyone, not even his partner,” William says, talking about his male friend and the human immunodeficiency virus, also known as HIV. “He never told me and we had unprotected sex, and I became [HIV] positive.” At the time, William says he didn’t know anything about HIV, the virus that causes acquired immunodeficiency syndrome, or AIDS. He didn’t have any symptoms, he hadn’t been sick, and he had no idea anything was wrong when he went in for a series of routine health screenings.


“I had just gotten out of [drug] treatment and ... it was suggested I go to my doctor and get tested for everything,” William, now in his 50s, says. “They did all the testing and everything else was fine. At first, they told me I had AIDS. They made a mistake about my [CD4] count. After that, they sent me to a hospital in Pittsburgh, Presbyterian Hospital, and that’s where they found out my real count and I’m only HIV positive.” One of the first people William told about his diagnosis was his boss at the fast food restaurant where he worked. After trying to call in sick and being told, “The only way you’re going to call off is if you’re dying,” William told his boss that he was in fact dying. “He was really shocked but he was very concerned,” William says, adding that he and his boss “drove around and we talked and stuff and he was very understanding.” There were two other people William had to inform: his girlfriend and his wife. Both were at risk of being HIV positive after having unprotected sex with him. He says his wife was “very understanding.” His girlfriend was not. “She worked at a hospital but she’s very germaphobic,” William says, adding that when she found out she could be HIV positive “she was attempting to jump off of a bridge. Her sisters got there in time to stop her.” Fortunately, neither woman tested positive for HIV. When asked how easy or difficult it is to contract HIV, Johanna Brown, MD, an infectious disease physician at Medical Associates of Central Virginia, used what the World Health Organization and others call the “Rule of Threes.” She compared the chances that someone who has an occupational exposure to infected blood — while working in a hospital, for example — might get Hepatitis B, Hepatitis C or HIV. “We know from the data ... that you have about a 30-percent chance of transmission for Hepatitis B, a three-percent [chance of] transmission of Hepatitis C and a 0.3 percent [chance] of transmission of HIV,” she says, adding that it’s unknown how many sexual exposures it takes to become HIV positive. “Not 100 percent of people that come in contact with infected blood will become infected with HIV.”

What is a CD4 count? CD4 cells are a type of white blood cell that fights infection. Another name for them is T-helper cells. CD4 cells are made in the spleen, lymph nodes, and thymus gland, which are part of the lymph or infection-fighting system. CD4 cells move throughout your body, helping to identify and destroy germs such as bacteria and viruses. The CD4 count measures the number of CD4 cells in a sample of your blood drawn by a needle from a vein in your arm. Along with other tests, the CD4 count helps tell how strong your immune system is, indicates the stage of your HIV disease, guides treatment, and predicts how your disease may progress. What’s the difference between HIV positive and having AIDS? “We measure the amount of HIV in the blood and that’s called the viral load, and then the main thing that HIV attacks in the body is one of our immune cells called the CD4 cells. When our CD4 cells get below a level of 200 that’s when we get all the opportunistic infections. When you start getting opportunistic infections, that’s called AIDS. It’s AIDS when your CD4 count is 200 or below.” (Thomas Kerkering, MD, veteran HIV/AIDS physician, Carilion Clinic Infectious Disease)

William, who now lives in the Lynchburg area and works at a local restaurant, had a tough time early on, dealing with the misconceptions and shame surrounding HIV. Oftentimes, people said and did things that upset him.

“I was going to pick up my son and he said, ‘I can’t hug you.’ I said, ‘Why?’ and he said, “Because you’re sick.’ I went through a lot of hurtful things,” William says. “I would be pointed out as the person with AIDS. ... People quit being my friend, stopped being friends with me. I would go to someone’s house and eat and they’d throw away the spoon. People didn’t know.” Even today, after 30 years of HIV/AIDS being part of the vernacular, the stigma persists. Dr. Brown says confidentiality is a great concern for her patients, particularly in a town the size of Lynchburg, “where neighbors know your business.” Efforts are made at the clinic to help patients safely and confidently


disclose their HIV status to partners, she says, and the fact that the clinic treats other types of infectious diseases helps, too.

“There are 400 people walking near you that have this disease and they all do everything in their power to keep their disease as confidential as possible because the stigma is still so high,” Dr. Brown says, referring to the number of HIV/AIDS patients treated by Medical Associates, the only clinic of its kind in Central Virginia. There are still many misconceptions about the disease, particularly about how it’s transmitted. According to the Centers for Disease Control and Prevention, “HIV is spread mainly by having sex or sharing injection drug equipment such as needles with someone who has HIV. Only certain fluids — blood, semen, pre-seminal fluid, rectal fluids, vaginal fluids, and breast milk — from an HIVinfected person can transmit HIV. “These fluids must come in contact with a mucous membrane or damaged tissue or be directly injected into the blood stream (from a needle or syringe) for transmission to possibly occur. Mucous membranes can be found inside the rectum, the vagina, the opening of the penis, and the mouth.” Dr. Brown adds that HIV can’t be transmitted by “saliva, urine or stool, as long as they’re not contaminated with blood” and it’s “not airborne.” She says a common misconception that the virus might spread by being kissed or spit upon could have been perpetuated by the use of an oral test that’s used to screen for HIV. “We have a rapid test of oral fluid,” she says. “We don’t like to call it saliva. It’s a little pad that you apply to the gum and rub it on the gum. It’s a very good way to do rapid testing for HIV. There’s no contact with blood at all, but somehow maybe some people have gotten confused [that] if you can do www.OurHealthVirginia.com

rapid testing in the mouth you are somehow secreting virus in the saliva, but that’s not the case.” On that line, getting tested is easy and Dr. Brown believes everyone should be tested as part of “routine care.” “If only one could be tested at least once in a lifetime, that would be wonderful,” she says, adding that one reason the epidemic is still prevalent is many people don’t know they are HIV-positive. Nationwide, AIDS.gov reports that “more than 1.1 million people ... are living with HIV” and that about one in six, almost 16 percent, don’t know it yet. The most seriously affected are described as “gay, bisexual, and other men who have sex with men.” AfricanAmericans are said to “face the most severe burden of HIV,” particularly young, black men. In September, the Virginia Department of Health reported there are 2,232 individuals living with HIV or AIDS in Southwest Virginia. For its purposes, the region includes not only Roanoke and what one would usually think of as Southwest Virginia, but also Lynchburg, Danville and Martinsville and the counties surrounding each. In the city of Lynchburg alone, there are 203 people living with HIV or AIDS. Dr. Brown, who has practiced in Central Virginia for eight years, describes her clinic as “rural” and says that traditionally patients were split about 50/50, men/women, and most were heterosexual. Lately, however, she’s seen a change. It’s “almost like going back to the early beginnings” of the epidemic, she says, when the disease was called “gayrelated immunodeficient syndrome ... a horrific term for this.” While “men who have sex with men” has been the commonality in urban areas — New York, Baltimore and San Francisco, for example — she says the Southeastern epidemic has always looked a little different. Dr. Brown says public health workers aren’t yet sure what to make of this. 76

How is HIV not spread? HIV does not survive long outside the human body (such as on surfaces), and it cannot reproduce. It is not spread by: • Air or water • Insects, including mosquitoes or ticks • Saliva, tears, or sweat. There’s no good explanation as to why this is happening.” Dr. Brown says “better prevention methods” are needed to address the spread of HIV. Over the past 30 years, she says, “we’ve seen this phenomenon called condom fatigue. There’s only so many ways you can preach safe sex and condom use. Many people get sick of hearing it over and over.” Another reason the message might not be getting through today is that HIV/AIDS doesn’t look like it did in the 1980s and 90s, when images portrayed in the media were shocking.

“I had to make a choice: take this medication and live 10 years or not take it and live a normal life of five years,” he says. “The side effects were terrible. ... I didn’t stay on that medicine for too long. Now, I take three pills a day and it’s great. During that time, I missed a lot of doses because I didn’t want to take it. A big part of it was I didn’t want to take it every day. It was a constant reminder that I was HIV positive. I didn’t want to take the medication. I wanted to forget.”

But now, what happens is you don’t see anybody die like that of AIDS, unless they choose not to take the therapies.” With improvements in antiretroviral therapies, many people with HIV take just one pill per day, rather than what Dr. Brown describes as “buckets” early on. Side effects also have diminished. “”When I first found out [I was HIV positive], I was taking about six or seven pills a day, but the medication at the time was very toxic,” William says. “You pretty much knew who was on what medication. You’d have a big stomach and no butt. It would bloat [you].” At one point, William says, he took 12 medications, twice a day. The side effects were so bad — nausea, hallucinations and insomnia, to name a few — he considered quitting altogether. (continued on page 81)

What does ‘viral load’ mean? When healthcare providers discuss your “viral load,” they are talking about the level of HIV in your blood. Knowing your viral load helps your provider to monitor your HIV disease, decide when to start treatment, and determine whether or not your HIV medications are working once you begin taking them. There is usually a relationship between the viral load and the number of CD4 cells you have. Typically, if your viral load is high, your CD4 count will be low — making you more vulnerable to opportunistic infections. the antiretroviral medications you take for your HIV disease work to keep HIV from reproducing in your body. It lowers your viral load and protects your immune system. (Aids.gov)

Today, there are even ways to help HIV-affected couples get pregnant and deliver HIVfree babies.

“Nowadays, if you have discordant couples, one HIV-negative, one HIVpositive, there is a safer way to get pregnant,” Dr. Brown says. “There are methods to wash out the sperm. Sperm carries HIV. If the female partner is negative and the male is positive, they wash out the sperm and [use] the ‘turkey baster’ method. It has to be done at a university [medical] center.” If the woman is HIV positive and the man negative, Dr. Brown says, the woman needs to protect her partner at the time of conception and decrease the risk of HIV transmission to her unborn child by having “no detectable virus in your blood.” This is done through antiretroviral therapy. Without therapy, Dr. Brown says, there’s a 25-percent chance of transmitting HIV to the baby. With treatment, it’s about five percent. “There are ways that discordant couples can safely have a baby,” she says, adding that you should “never say 100 percent in medicine. That would be a lie.”

financial help available through a federal grant named for Ryan White, an Indiana teenager diagnosed with AIDS in the mid-1980s. “This varies state to state and some have waiting lists due to poor funding,” she says. “With this grant, we also try to help with co-pays for our insured patients and do everything we can with patient assistance programs, etc., to make sure cost does not get in the way of patients affording and taking their medication.” Treatments costs have never been an issue for William, who says he gets most of his medications through the local health department or Medical Associates. “There’s a lot of help out there for HIV-positive people,” he says. Today, area physicians say HIV/AIDS is no longer a death sentence and people with the virus can live as long as those without it. “If you take your medications the way you’re supposed to and treat your body well, [and] you remain in care, you can safely say you will live as long as anyone [else],” Dr. Brown says. “Now, an HIV diagnosis may not translate into a sooner death diagnosis. Certain diseases are more common in HIV patients, but the overall encouraging message to bring is ... you can live as long as anyone without it.” William says the physicians at Medical Associates have even suggested he get a primary care physician to deal with his other medical concerns. “I feel good,” he says. “I’m probably healthier than most people. My CD4 count is 900-something, which is good. My viral load is undetectable. I just got put on a medication for high cholesterol. I’m not going to die of AIDS or HIV. I’ll probably have a heart attack or something. I take my medication every day. I work a part-time job. I live pretty much a normal life now.”
