What is Mohs Micrographic Surgery? Frederick Mohs, MD was the early pioneer who developed this unique approach to skin cancer removal. It is a very precise technique, which involves removal of the skin cancer immediately followed by microscopic examination of the tissue. If the margins of the tissue removed are clear at that point, then the procedure is complete. If there are any positive margins identified by the Mohs’ surgeon, then that area is mapped and further removal is carried out, but only in the area known to have remaining skin cancer. Therefore, preservation of tissue is accomplished while not compromising the goal of complete cure. The procedure is repeated until the area is cancer free. With so many Americans reaching an age at which skin cancer development is becoming a reality, there is an increased need for identification and removal of carcinomas of the skin. The cure rate for the most common skin cancers (basal cell and squamous cell carcinomas) is 99% when the Mohs’ surgery technique is employed. This high cure rate, coupled with the lowered risk of facial disfigurement, makes this a very useful approach to skin cancer removal. Susan Dorsey, MD

What is presbyopia? Presbyopia or “arms-not-longenough-itis” is an age related decrease in the ability of the eye to autofocus, especially for reading. As we age, the lens inside the eye loses its flexibility and can’t change shape as easily to focus up close. This causes symptoms near or just after the age of 40. At that time, prescription glasses or contact lenses can be of help. We recommend all those over the age of 40, who are having any issues seeing up close, either with or without correction, have a comprehensive eye examination to be evaluated for presbyopia. What is a somatic dysfunction? Common lingo in osteopathic medicine, but technical jargon to a non-clinician, the term “somatic dysfunction” refers to a problem in the body’s structure causing a subsequent problem with its function. One of the tenets of osteopathy states that structure and function are related. Take the case of a patient who just raked leaves for two hours, then develops back pain. He presents to his doctor with a noticeable, abnormal rotation in his spine. This rotation impairs his ability to move correctly and causes pain. As a means of simplicity and conformity, we name the disorder based on how the patient presents. If, for example, thoracic segments six to nine are stuck side bending to the left and rotating to the right, that’s what we call it. That’s his somatic dysfunction. My 8-year old seems to have difficulty hearing certain things during conversations –could he have experienced a sudden hearing loss? A permanent sudden hearing loss is rare but possible. There are several other causes of missing things in conversation. Ear infections that cause middle ear fluid can affect hearing. Auditory processing deficits can also cause difficulty. These deficits occur when a child’s brain and nervous system do not respond well in difficult listening environments. A third possibility, sensorineural hearing loss, typically affects some pitches more than others. This can lead to a loss of crucial speech sounds during conversation. The best step is to have an audiologist evaluate your child’s hearing to determine what is causing the deficit and to what degree. Richard Harrell, PhD

Do whitening toothpastes really work? Yes and no. Whitening toothpastes can remove surface stains. They contain abrasive particles that polish off stains such as those from coffee and smoking. They cannot change the natural color of your teeth or rid stubborn deep staining. These toothpastes are also very useful after professionally whitening your teeth to maintain your new bright smile and keep the stains away! A few whitening toothpastes contain Blue Covarine that binds to the teeth, and creates an optical illusion that makes yellow teeth appear whiter. To lighten the color of your teeth, professional whitening with carbamide or hydrogen peroxide is needed. Yellow and brown teeth whiten the best. Gray teeth tend be a little more stubborn. Professional whitening can be done in the dental office in about an hour with a product called Zoom. Results from 5-14 shades lighter can be seen after one visit. How can spinal bracing help in the treatment of juvenile scoliosis? Having scoliosis can compromise normal musculoskeletal development in young patients and can adversely affect normal organ function. Scoliosis can occur at any age, but the most common treatable type is adolescent idiopathic scoliosis. This affects more female children than male, and typically occurs at or just before age ten. The general idea is to diagnose the scoliosis early in the child’s development and brace the curves if they are progressing rapidly or are greater than an acceptable degree. Treating a scoliosis curve with a spinal brace greatly decreases the child’s chances of having pain or deformity later in life. Bracing is possible with flexible curves by creating forces generated on the asymmetries of the torso; and bracing allows the child to participate in the treatment of their scoliosis. When the child is shown the X-ray or observes physically how the spine is being corrected, their compliance and outcomes typically improve. Hank Hinshaw, CPO

donation of building to CHIP of Roanoke Valley Carilion has announced the donation of the building they had previously leased to Child Health Investment Partnership (CHIP) for the last 12 years. CHIP’s non-profit headquarters, located at 1201 Third Street in Roanoke is now owned by CHIP, and will provide stability to an organization that provides medically-focused home visiting services to over 1000 low-income children annually in the Greater Roanoke Valley.

Kissito PACE… a year of success keeping the elderly in the community
Kissito PACE held it’s Year Anniversary and invitation-only “homecoming” event for participants and family members, Thursday, Nov. 20 in Roanoke, VA. The participants shared stories of how Kissito PACE has enriched their lives. The Kissito PACE program in Roanoke provides all-inclusive care for the elderly and is part of the PACE program of the Commonwealth of Virginia. It is a home- and community-based alternative to a nursing facility. Participants get to remain in their own homes while commuting two to three times during the week to the 10,000-square-foot Kissito PACE center for socialization, exercise, dining, and medical care. Participants in Roanoke agreed to allow select members of the press to attend their exclusive celebration because they wanted their stories to be told in upcoming news features.

Painful. Debilitating. And one of the most common reasons
patients visit a podiatrist.
Custom-made foot orthotics help most find relief and prevent its reoccurrence. Kim Wood was just 39 years old, yet she felt like a 90-year-old woman. The excruciating pain was at its worst in the morning, when she got out of bed and every stride felt as though she were stepping on a nail. In a futile attempt to avoid the pain, she tried changing the way she walked. This only made the pain spread to her ankle and the side of her foot, causing her to make even more pronounced changes to her gait, spreading the pain to her hips and back. “My whole body became so stiff, and as a result, it made me feel as though I had aged tremendously,” Wood explains. “When the pain’s really bad, it makes you feel so old. Even just sitting for an hour car ride would result in me becoming stiff.” The pain wasn’t always there. Wood was once an active runner. She loved the sport, having discovered at age 38 the health benefits that came with it. She’d run three miles at least four times a week and was in great shape. “Before I began running, I had already lost 15 pounds so I felt like I was in a good place to start exercising,” Wood says. “And after I started running, I noticed I could still enjoy eating all the foods I love and maintain my weight.” Her new-found hobby and healthier lifestyle wouldn’t last long, however, because plantar fasciitis was lurking in the background.
What is plantar fasciitis? “Painful inflammation of the plantar fascia—the ligament that connects the heel to the arch—is frequently characterized by significant heel pain,” explains Charles M. Zelen, DPM, with Foot and Ankle Associates of Southwest Virginia. “It usually occurs where the ligament attaches to the heel bone, and results in severe pain that can be very disabling, preventing exercise or even daily activities.” Charles M. This very prevalent condition is one of the most common reasons that patients seek the advice of a podiatrist, Dr. Zelen says. It’s also known as “heel-spur syndrome” because as the ligament pulls on the heel bone, it causes a spur to form. This spur is actually a piece of bone being pulled off the heel, and Wood was experiencing it in both feet. “Over-activity and weight gain are two of the most frequent causes of plantar fasciitis,” Dr. Zelen says. “We diagnose it with a combination of a clinical examination and X-ray evaluation, with the most important feature being the presence of pain or discomfort on the heel bottom.”

Treating plantar fasciitis Ben Sigmon, a certified prosthetist orthotist with Virginia Prosthetics & Orthotics, Inc., evaluates Kim’s foot for pressure points—indicated by callouses—and any swelling that the orthotics will help correct.
Pain wasn’t the only health problem Wood experienced with plantar fasciitis. Because she could barely even walk without significant pain, let alone run or participate in other forms of exercise, both her weight and blood pressure rose. She needed to find a solution, and fast. “I tried everything,” Wood recalls, “over-the-counter foot orthotics, different shoes, cortisone injections, a foot stretching device – all with little or no success. Looking back, I wish I had educated myself more about the condition.” While each patient is unique, Dr. Zelen said most patients with plantar fasciitis will see the condition improve through the use of several available treatment options, including physical therapy and stretching exercises, cortisone injections, and orthotics. If those options fail, then surgery and several newer treatments are available. As mentioned earlier, Wood tried orthotics to alleviate her foot pain, but they didn’t solve the problem, mainly because they were off-the-shelf orthotics and not customdesigned for her unique foot and specific condition.

After examining her, Dr. Zelen recommended custom-made foot orthotics. Inserted into the shoe, orthotics provide the cushion and support the foot needs to prevent injury and enable healing from injuries that have already occurred. “Using custom foot orthotics as a functional therapy is probably the number one most important treatment option,” Dr. Zelen says. “They provide physical support to the foot, take the pressure off the ligament on the bottom of the foot, and prevent the ligament from being stretched. Orthotics also help control pronation – when the foot hits the ground, the arch stretches and flattens, allowing the foot to turn inward abnormally.” Even people without plantar fasciitis can benefit from wearing foot orthotics because they help prevent abnormalities and injuries from occurring when walking or running, he adds. Wood took her prescription to Virginia Prosthetics & Orthotics, Inc. in Roanoke where Ben Sigmon, CPO, evaluated her. Sigmon is a Certified Prosthetist Orthotist, educated and trained specifically to evaluate patients and then design, make and fit patients’ prostheses – artificial limbs – or orthoses, which cover a wide variety of bracing devices for various parts of the body, including the feet.
The foot’s impression is captured in foam so that an exact replica can be made in plaster. The orthotics are then handcrafted from the plaster mold, ensuring a comfortable fit and correcting her condition.

After checking Wood’s alignment by watching her walk down the hallway and examining her bare feet, Sigmon said he noticed slight swelling in the foot and tenderness to the touch – both attributable to the plantar fasciitis. “She has a moderately aggressive arch, and as a result, the heel and ball of her foot are bearing most of the pressure as she walks. This can worsen the pain associated with plantar fasciitis,” Sigmon explained. “What I want to do is use the custom inserts to cushion the feet and provide better support for her arch.” Sigmon also noted that Wood’s arch collapses slightly when she walks and an off-theshelf orthotic would not be aggressive enough to provide the necessary results. “Everyone’s feet are so different and even the tiniest change can yield big results,” he explains. “Custom-made orthotics are functional and designed specifically to correct

Emerging treatments include: • shock wave therapy • platelet injection • injection of amniotic membrane filled with growth factors. Dr. Zelen recently authored a study that was published in Foot & Ankle International – the journal for the American Orthopedic Foot and Ankle Society. The study looked at using dehydrated human amnion/ chorion membrane (dHACM) injection to treat the inflammation associated with Plantar Fasciitis. the individual patient’s problems. No off-the-shelf or mail-order orthotic can match or provide the level of fit and correction as compared to one that is custom measured, fabricated, fit and modified specifically to match a patient’s unique foot and the condition they are experiencing and trying to correct.” Sigmon began the design process for Wood’s foot orthotics by placing her bare foot into a small rectangular box filled with a solid, impressionable foam and rolling her foot gently from outside to inside, as she would when walking or running. The foam provides a very detailed picture of her foot which is then used to make a plaster cast that’s an exact replica of the foot. The orthotics are then handcrafted from the plaster mold, ensuring a comfortable fit and helping correct the condition. “Plantar fasciitis is very common in runners as a result of stress and overuse,” Sigmon explains. “One of my goals in fitting the orthotics is to help the patient avoid surgery and achieve success through the use of orthotics, which is a much less invasive alternative.” In addition to the custom-made orthotics, Sigmon also treated Wood with a dorsal night splint. This orthotic device is worn only when she sleeps and keeps her foot at a 90-degree angle, perpendicular to her leg, allowing the Achilles tendon and calf muscles to stretch throughout the night. “Most people sleep with their toes pointed down, allowing the muscles and tendons to contract and tighten throughout the night,” Sigmon says. “Then when they take that first step in the morning, everything is forced to suddenly stretch, causing pain and possible injury. Wearing the splint at night prevents this from occurring and promotes healing.”
The prognosis for a full recovery. According to Dr. Zelen, Wood should make a full recovery. “The majority of patients – better than 90 percent – don’t have a recurrence of plantar fasciitis, if they continue to do their stretching exercises and wear good shoes with their orthotic inserts. “ Because the injury is so debilitating, it’s uncommon for a patient not to seek treatment for their plantar fasciitis, and Dr. Zelen cautions patients about ignoring their condition and trying to live with the pain. Left untreated, prospects for a full recovery aren’t as positive because permanent ligament and nerve damage may occur, leaving patients with chronic pain. And even with foot orthotics, recovery isn’t overnight, Sigmon cautions. “It’s not an immediate cure, but in a couple of weeks she should see an improvement,” he says. “Patients have to transition to the orthotics by wearing them for an hour the first day, then two hours the second day, and 32

so on, allowing their body to gradually adapt to the changes that the orthotics are causing.” Wood said it took her only about two or three days to get used to having the orthotics in her shoes. “I noticed right away that my stride is different,” Wood explains. “I felt like before I was putting more pressure on my heels. Now with the orthotics, I have a more balanced stride.” Even after returning to a full exercise schedule, which Sigmon recommends only after she’s worn the orthotics for a month, Wood’s orthotics will last anywhere from a year and a half to more than three years. “They are made from plastazote foam, cork, and other materials that help them retain their shape and resist compaction,” Sigmon explains. “Over time, they will begin to wear because of a number of factors, including the user’s weight and activity level, and need to be replaced. Our feet also change periodically, which is another reason to have the orthotics evaluated by an orthotist on a regular basis.” As for predicting who might be at greater risk for contracting plantar fasciitis, Sigmon said that’s difficult to do just by examining someone’s foot since the patients he sees with the condition are split almost evenly between having flat feet (pes planus) and having a normal or high arch (pes cavus.) He does feel that shoe selection can help prevent the condition and recommends people choose an athletic shoe that offers more support. One method he employs in evaluating a shoe is to grasp it at the heel and toe and see how easily it twists from side to side. The easier it twists, the less support it provides to the feet. Wood hasn’t put away her running shoes just yet. She wears them daily, along with her foot orthotics, looking forward to the day when she can start running again, get back in great shape, and maybe even eat a few of those foods that she loves.

Rather than a ‘resolution’, why not make a ‘commitment’ to be fit as you ring in the new year? Here are some great tips to get started from the legs up! Did you know you burn the most calories when you exercise your legs? That’s because they’re home to your body’s biggest muscle groups, so it takes more energy to move them. For maximum results in getting the legs you want, the secret lies in workouts that use slow movements that purposefully fatigue the muscles. The first phase of an exercise is done to a slow count of 10 seconds. When you reach the most difficult part of the exercise – where gravity and the nature of the

1. Place feet a little wider than shoulder-width apart, hips stacked over the knees, and knees over ankles.
1. Step forward with right leg and keep left leg back. Step should be comfortable and allow you to keep balance. You should be able to feel the stretch in your gluteus and hamstring muscles.
3. Extend the arms out straight so they are parallel with the ground, palms facing down 4. Initiate movement by inhaling into the belly, and unlocking the hips, slightly bringing them back. Keep sending hips backward as the knees begin to bend. 5. While the butt starts to stick out, make sure the chest and shoulders stay upright, and the back stays straight. Keep the head facing forward with eyes straight ahead for a neutral spine. 6. Engage the core, and exhale while driving through the heels to return to standing. With bodyweight in the heels, explode back up to a standing position, driving through the heels.
2. Lower body by bending legs. Right leg should create a 90-degree angle at the knee. Left knee should dip down toward the floor until it almost touches. Remember to stretch only as far as you can without being uncomfortable and feeling unsteady. 3. Maintain posture by keeping back straight. Right knee should be directly above foot without moving too far forward. Keep feet completely on the ground, relying on both feet for balance. 4. Raise body back up without standing up completely. 5. Repeat these steps with left leg forward.
#3. Side Leg Lift on Exercise Ball 1. Kneel on a yoga mat with exercise ball on one side. 2. Rest one side of body against exercise ball and push up, so bottom leg is underneath you on the mat. 3. Extend the leg on the opposite side of the exercise ball straight out to the side. Rest your body against the side of the exercise ball, and support with one hand. 4. Opposite hand is on the floor. Back should be straight and shoulders square.

Ingredients: 3 eggs ¼ C local honey 2/3 cup unsweetened pumpkin puree 1 teaspoon lemon juice 1/3 cup coconut flour 1 teaspoon baking powder 1 tbsp cinnamon 1 teaspoon ground ginger 2 teaspoon ground nutmeg 1/2 teaspoon salt *add water as needed
Filling Ingredients: 2 tbsp honey 8 ounces reduced fat cream cheese, softened 4 tablespoons butter
6. Bake in a preheated 375° oven for 15 minutes, or until cake bounces back when touched lightly with finger. 7. Turn cake out onto a clean dish towel. Starting at narrow end, roll towel and cake up together; cool. Unroll.
3/4 teaspoon vanilla
8. Combine the honey, cream cheese, butter, and vanilla; beat until smooth.

9. Spread over the cooled cake and roll up. Chill.

The line between healthy and unhealthy gets fuzzier every day with blogs, articles, tweets, pins, and broadcasts continually reporting that everything, from food to diet and exercise, is slowly killing us. With the line so blurry between seemingly healthy lifestyle choices and the reports of things that are quietly leading to human demise, how does one push forward and make good choices? To begin with, do a little research on health topics to try to get an idea of what the media has sensationalized and what may actually be factual, and build your healthy lifestyle around it. Diet and nutrition seem to be the top choice for exaggerated fact and information, such as perpetuating reports that all sugars are deadly, meat and dairy are assassins, and carbohydrates slowly dig your grave. But what about the misleading “healthy” substitutes, such as “light” salad dressings filled with sugar and sodium or diet meals packed with preservatives and high levels of the chemical diacetyl?
In the end, there’s no one-size-fits-all recipe for a perfect diet. Nutritionists and dieticians are still dusting off the color-coded food pyramid and advising consumers to plan meals and diet based on Body Mass Index (BMI), health and weight goals, and FDA-suggested daily intake levels. Tricia Foley, RD, MS, OurHealth’s resident registered dietician, says moderation and balance are important when considering nutrition. “According to the current dietary guidelines, it is important to have variety in our diet, however consuming processed and or refined foods on a regular basis could have negative implications on our health.” That’s why the federal government has developed standards and guidelines for acceptable ranges for macronutrients (carbohydrates, protein, and fat) and dietary references and upper limits for micronutrients (vitamins and minerals). You can log onto the website myplate.gov to find individualized information on how many calories per day you should consume and what percentage of your diet should come from each macronutrient. “Eating a variety of nutrients in the right balance is required for a healthy body” says Tricia. “Eliminating or eating certain nutrients in excess could lead to vitamin or mineral imbalances and/or chronic disease.” While it isn’t a perfect science, with moderation and a customized diet, nutrition is easily sustainable day-to-day. What about diet and nutrition’s partner in crime—exercise? The mantra used to be that all exercise is good and healthy, helping build muscle and eliminate fat, essentially adding years to a person’s life. But now, conflicting reports claim that certain exercises are detrimental to the human body or even that exercise itself contributes to injury and long-term health complications. Here again, as with many things in life, there is fine print associated with each study, as well as exercising in general. It can be summed up like this: all exercise has risk associated with it when it’s pushed to extreme levels. When choosing a fitness regimen, it’s always best to alternate strength training (weights, machines) with cardio (running, cycling). This is an approach used by fitness professionals because 96

it gives the body time to heal, keeps exercise fresh and nonroutine, and allows muscles and ligaments to move differently each time. Trainers and physical therapists also note that people with heart disease and other potentially harmful conditions should consult with a doctor and fitness professional before creating a workout regimen. Much like nutrition, each body needs a custom plan to ensure physical success and the results you want. While nutrition and exercise are outward-facing issues to tackle, mental health and stress also affect us and our day-to-day lives. With so many stress studies on the market leading to therapy and medication, what can truly be trusted as sound advice to combat the issue proactively, and are the media and research studies correct in labeling all stress as harmful and negative? When it comes to stress, not all of it is considered harmful to our health. There are varying types of stress, including eustress, distress, and neustress. For example, eustress is positive for the most part. Some may think of marriage or the arrival of a newborn baby as stressful, but it’s not necessarily a stressful situation that is negative. Most individuals, though, normally think of distress as negative. The human organism needs a degree of stress to make us better, more productive, and more successful at our endeavors. An important factor when dealing with stress is something most humans take for granted and do not factor into a healthy living plan—sleep. Stress levels affect nutrition and a human’s inclination to exercise or be active, and stress levels go up considerably with inconsistent sleep patterns. Erratic sleep also makes us feel hungrier, which in turn leads to weight gain. So at the end of the day, the old adage “everything in moderation” rings surprisingly true in the face of studies and reports claiming that everything in life is deadly. Moderating the intake of high-calorie or sodium rich food can improve overall nutrition. Incorporating a purposeful, active exercise regimen can improve cardiovascular health while assisting in weight-loss, and maintaining a consistent sleep schedule and a low level of harmful stress improve day-to-day interactions and health. Don’t let the supposed studies and media worry you into an early grave. Moderation and educated lifestyle choices are the keys to a healthy life.

Living with HIV

In each edition of Our Health, we bring these medical issues into the light—and dedicate these stories to the courageous patients and families living with them and the healthcare providers and researchers who commit their lives to treating and seeking cures for these enigmatic conditions.
In the spring of 1988, Frank House was sick. He had a litany of symptoms— fever, nausea, swollen glands, rash, dizziness, chills—and none of it was going away. He saw several physicians and, after having some tests, was told his symptoms pointed at cancer or maybe severe allergies. It wasn’t either. Frank wasn’t surprised he had contracted HIV, the human immunodeficiency virus that causes acquired immunodeficiency syndrome or AIDS, because at the time he was living a double life. “Imagine having a high-paying job, being married with several small children, and unfortunately also leading a double life,” he says. “That was me for a number of years in the 1980s. I was closeted, gay and undeniably attracted to men. The day I found out I was [HIV] positive was about as traumatic as one can imagine, especially considering my profoundly dishonest and deceitful circumstances.”

What’s the difference between being HIV positive and having AIDS?
“It was only because of one doctor who suggested I have an HIV test that it was diagnosed,” Frank, now 61 and living in Roanoke, says. “I had to repeatedly call the doctor’s office for the test results, as none of the nurses wished to convey the bad news, that I was HIV positive, to me.”
“We measure the amount of HIV in the blood and that’s called the viral load. The main thing that HIV attacks in the body is one of our immune cells called the CD4 cells. When our CD4 cells get below a level of 200, that’s when we get all the opportunistic infections. When you start getting opportunistic infections, that’s called AIDS. It’s AIDS when your CD4 count is 200 or below.” (Thomas Kerkering, MD, veteran HIV/AIDS physician, Carilion Clinic Infectious Disease)
“I was quite aware beforehand that I was probably infected, from the very onset of the symptoms. I was much more concerned about how I could possibly face my wife and my family [about] being gay than I was with facing the prospect of having AIDS. The inner anguish, guilt and suicidal thoughts were overwhelming. I didn’t care if the virus killed me, for I felt I deserved that fate.” Frank told his wife and fortunately, after being tested and retested, she was found to be HIV negative. He wasn’t, however, ready to come clean on everything. “I told my family right after receiving the diagnosis,” Frank says. “I lied to them. I told them I thought I might have received the virus from a female prostitute, versus random male sexual partners. Regardless of that attempt to deflect the gay inference, it did not go well. “Deceit and dishonesty were deceit and dishonesty. There was no sympathy from anyone, no consoling from family and friends. There was disgust toward me and [a] sense that even they thought I was fully deserving of my illness and terrible fate: to die a horrible death.”

Frank is not alone in his struggles. In September, the Virginia Department of Health reported there are 2,232 individuals in Southwest Virginia living with HIV or AIDS. For its purposes, the region includes not only Roanoke and what one would usually think of as Southwest Virginia, but also Lynchburg, Danville, Martinsville and the counties surrounding each. In the city of Roanoke alone, 518 people are living with HIV or AIDS.

Nationwide, AIDS.gov reports that “more than 1.1 million people ... are living with HIV infection.” The demographic most seriously affected is described as “gay, bisexual, and other men who have sex with men.” African Americans are said to “face the most severe burden of HIV,” particularly young, black men. Thomas Kerkering, MD, an HIV/AIDS specialist and chief of Carilion Clinic Infectious Disease in Roanoke, has been treating HIV/AIDS patients since 1981, the very beginning of the epidemic. His clinic currently follows about 750 HIV/AIDS patients living throughout Southwest Virginia. Dr. Kerkering says most of the men he treats are those who have sex with men and most of the women are heterosexual, although they might have been intravenous drug users. He was quick to point, however, that HIV/AIDS “could be in all the demographics” and gives as an example married, bisexual men from higherincome groups or those “who are shooting up on the side” and “their wives have no idea that they were doing either of these things.” After attempting suicide and spending several months in a psychiatric ward, Frank eventually admitted he was gay. Over the years, he’s also had to come to terms with the emotional trauma of being HIV positive and, a few years after his initial diagnosis, being told he has AIDS. “I believe any chronic illness has a way of affecting the individual emotionally,” he says. “It can lead to deep discouragement and depression, a feeling of hopelessness and isolation. HIV/AIDS has perhaps a unique level
of stigma associated with it that affects a person’s self-esteem, his or her ambition and their outlook on life. A perception of being unworthy and of having deserved the illness.”
“When I came and saw AIDS patients, it was just strange. They were treated worse than animals. Left alone. People were scared. They were afraid it was airborne and the government wasn’t telling them. That’s how I got interested in those patients, because I really felt bad for them.”
“Socially, I had difficulty in wanting to meet and possibly date others, for fear they would reject me once I told them I was HIV positive. For a number of years, I would only pursue friendships and beyond with others who were also HIV positive.” Muddasar Chaudry, MD, an infectious disease physician with LewisGale Physicians in Salem, was shocked at how HIV/AIDS patients were being treated in the U.S. when he immigrated to this country from his native Pakistan in the late-1980s. “I did my training in Newark, N.J.,” Dr. Chaudry says, adding that there was no AIDS epidemic in Pakistan. “That was in the late 80s, early 90s, and that was the time when the HIV epidemic was at its peak. Newark and New York City and the west coast were the epicenters.” Before he was diagnosed, Frank says he didn’t know much about HIV or AIDS, but he did have some ideas about the disease. “I had the preconceived notion that there was a direct inference by the general public that those who got HIV/AIDS were mostly gay or [intravenous] drug users,” he says. “I also had the notion that there were no medications available to treat the virus, and that life spans were short.” “I knew from the media that the illness caused horrible suffering and death. I also felt there was outright discrimination toward people who had HIV/AIDS, including from doctors, employers, family and friends. All of these ‘notions’ were found to be correct.” Frank eventually found emotional help through HIV/AIDS support groups in Las Vegas and Phoenix, places he lived prior to moving to Roanoke eight years ago. Once in Roanoke, he discovered the Drop-In Center, a program of the Council of Community Services in Roanoke, which provides free HIV testing and support. “The local Roanoke Drop-In Center has and continues to provide needed support and activities for myself and many others dealing with HIV/AIDS,” he says. “Apart from what this valuable local organization does—testing, support, prevention, etc.—they have been a catalyst for me to share my story with many others in groups, at schools, and through the media.” “It is through the telling of my story that I seek to encourage everyone to live their lives authentically, honestly and without shame. I believe strongly that the ‘closets’ and leading double lives are profound risk factors for developing significant mental and physical health problems, including HIV/AIDS, not to mention to ultimate turmoil and devastation it brings to the surrounding family members and friends.”

How is HIV
NOT SPREAD?
• HIV does not survive long outside the human body (such as on surfaces), and it cannot reproduce.
It is not spread by: • Air or water. • Insects, including mosquitoes or ticks. • Saliva, tears, or sweat. There is no documented case of HIV being transmitted by spitting. • Casual contact like shaking hands or sharing dishes. • Closed mouth or “social” kissing. • Toilet seats. (CDC.gov)
been re-wetted while cleaning up a crime scene or a bloody article of clothing, for example. “Drying conditions generally kill the virus,” Dr. Kerkering says. “Once it’s dead, it’s dead.” Treatment options for those with HIV/AIDS have vastly improved since the 1980s. When Dr. Kerkering first started seeing HIV/AIDS patients, he says he attended about a halfdozen funerals each month. There were no medications available until the early 1990s, he says, and then there were only one or two. Muddasar Chaudry, MD, an infectious disease physician with LewisGale Physicians in Salem
“It wasn’t until 1996 that we had a number of what we call different antiretroviral medications,” he says. “Even with those, sometimes it would be up to 42 pills a day.” As a long-time survivor of HIV/AIDS, Frank has dealt with many challenges relating to
medication. Early on, because of the lack of approved medications in the U.S., he says he “went underground to obtain drugs from overseas which were not FDA approved.” Since drugs have been available domestically, Frank says he’s had to “keep switching to the newest drugs available to even stay marginally ahead of the virus. Thus, my virus is highly resistant but is controlled because of sensitive drug resistance testing and a multitude of new drugs and combinations available. The newest antiviral pills are actually combinations of two or three antiviral drugs for ease of compliance, etc.” Dr. Kerkering says most of his patients take just one pill a day and that HIV isn’t considered the death sentence it used to be. In fact, he says, he and other physicians “have to make sure we’re treating everything else— heart disease, diabetes, etc.—because that’s what’s going to do them in before HIV does.” While treatments may be effective, they’re not cheap. National Public Radio reported in January 2012 that HIV treatment costs an estimated to $2,000 to $5,000 monthly, in excess of half a million dollars over a lifetime. Frank estimates the “street cost” of his medications at $5,000 per month, but says that with the exception of when he had to get medications from overseas, he’s always been able to get them at low cost. Frank says he initially got medication through private insurance, and later through Social Security Disability and Medicare supplemental insurance, with assistance from the Virginia AIDS Drug Assistance Program. Virginia ADAP is primarily funded through the Ryan White CARE Act, a federal program named for an Indiana teenager diagnosed with AIDS in the mid-1980s.
What is antiretroviral therapy? Standard antiretroviral therapy (ART) consists of the combination of at least three antiretroviral (ARV) drugs to maximally suppress the HIV virus and stop the progression of HIV disease. Huge reductions have been seen in rates of death and suffering when use is made of a potent ARV regimen, particularly in the early stages of the disease. He adds, “A lot has changed in these 26 years since I was infected. There is a much better awareness of how the virus is transmitted and how that can be prevented, along with many more effective drugs to combat the virus, and less discrimination and stigma against people living with HIV/AIDS.” Still, despite three decades of HIV and AIDS being part of the vernacular, misconceptions persist, particularly about how the virus is transmitted. The Centers for Disease Control and Prevention reports that in the U.S. “HIV is spread mainly by having sex or sharing injection drug equipment such as needles with someone who has HIV. Only certain fluids—blood, semen, pre-seminal fluid, rectal fluids, vaginal fluids, and breast milk—from an HIVinfected person can transmit HIV.” “These fluids must come in contact with a mucous membrane or damaged tissue or be directly injected into the blood stream (from a needle or syringe) for transmission to possibly occur. Mucous membranes can be found inside the rectum, the vagina, the opening of the penis, and the mouth.” When asked how easy or difficult it is to contract HIV, Dr. Kerkering used what the World Health Organization and others call the “Rule of Threes.” “Let’s just say, of the three blood-borne pathogens—Hepatitis B, Hepatitis C and HIV—if you get exposed to Hepatitis B, and you haven’t gotten your vaccines or anything, you have a 30-percent probability of coming down with Hepatitis B. With Hepatitis C, three percent, [and] HIV 0.3 percent,” he says. Dr. Kerkering emphasizes that infected blood or bodily fluids need to “actually get into your system. It could be a teensy amount of blood, but it still needs to be liquid and needs to get into not-intact skin. Getting it on your skin isn’t going to hurt you. [It has to be] a cut or some other excoriation. Just by getting droplets of blood on intact skin, that’s not enough.” There also is little to no risk associated with dried blood, even blood that has www.OurHealthVirginia.com

“Everyone should be tested, unless they’ve never had any sexual activity,” Dr. Kerkering says. “The reason is that it’s so easy to treat now and you don’t have to die from it.” With all of the treatments and financial assistance available to patients, however, the website AIDS.gov reports that one in six HIV-infected people, almost 16 percent, are “unaware of their infection.” Both Dr. Chaudry and Dr. Kerkering say more people need to be tested for HIV. That HIV/AIDS is now a treatable condition with good outcomes, however, has created a problem: some people aren’t taking the virus seriously. “We had that initial group of gay men or men who had sex with men — the Rock Hudson era — who got totally afraid of it and had all their friends die from AIDS,” Dr. Kerkering says, adding that today’s young adults “never had that experience, and since it’s just one pill, once a day, they don’t worry about it anymore and are practicing reckless sex. We have a lot of 18-, 19- and 20-year-olds coming in with HIV.” Dr. Chaudry says the gay community did a good job at increasing awareness about HIV/AIDS in the early years of the epidemic. “They stirred up the politicians [and] got money to support the education programs,” he says. “That brought in a lot of attention from the media and government, because they were organized and educated. If it had started out in the heterosexual community I’m not sure we could have been as pumped to jumping on and working on it. The response probably would have been somewhat blunted.” Educational efforts should be increased, Dr. Chaudry says, starting in schools, and even with door-to-door campaigns. And not only in the lower-income neighborhoods, where posters and other HIV/AIDS awareness materials are already commonplace. People in higher-income brackets, “might not like the idea that you have put up signs in their neighborhoods,” he says, “even though it’s for their own good, even though their kids might be getting help for future generations.” Dr. Kerkering agrees. “We should have [posters] everywhere,” he says. “They should be in every blue blood country club.” www.OurHealthVirginia.com

yourself from becoming infected,” he says. “Although HIV/AIDS is now considered a chronic, manageable disease with long life expectancies and not the death sentence it once was, it is still serious and can profoundly change your life. “Know that many people, not just gays, get HIV/AIDS. The virus can infect anyone, given the opportunity. Know the facts, get tested and be safe.”

Drop-In Center Makes Getting Tested Quick, Easy When it comes to getting people tested for HIV and Hepatitis C, the people at the Drop-In will do almost anything. “The test that we use was developed to be a field test,” Pam Meador, the Drop-In Center’s director, says. “We’ll pitch a tent at a festival. We’ll go to a community center, a club, just anywhere where we can have some kind of confidential space and a flat service.” “We do that so we can reach the folks who would not normally come through our front door. We want to make sure everyone has an opportunity to get an HIV or Hep C test if they’ve ever been at risk for either one. Before the Drop-In Center was formed, we did testing out of cars, [in] people’s homes, literally any place that we could do a test safely.” Pam Meador, Director of the Drop-In Center in Roanoke, VA
The Drop-In Center, located at 356 Campbell Avenue SW in Roanoke, has been offering free HIV testing since 2004, and started offering free Hepatitis C testing in 2013. It also partners with area health departments, corrections facilities, drug treatment centers, churches and other organizations to provide testing, education and other services. “We do education at a lot of places, group-level interventions,” Meador says. “We do those at schools, at drug treatment centers, at shelters — just anywhere [where] someone may call us and say, ‘I’ve got a group and they need the basics about HIV, sexually transmitted diseases and Hepatitis.’ “We provide individual-level interventions [and] prevention counseling. It could be that we’re going to focus on life goals [or] that we’re going to work with them to get to their next medical appointment. Everything is centered about prevention and linkage to care.” As its name indicates, those seeking testing can literally drop in anytime the office is open: from 9 a.m. to 1 p.m. and 2 p.m. to 4:30 p.m., Monday, Tuesday, Thursday and Friday. The DropIn Center also has satellite offices at Virginia Tech and Radford University. Testing is free and involves a quick finger prick, and results are back in 20 minutes.
