Introduction

Welcome to Community Fitness Center, we are happy to have you as part of our fitness community! This Membership Handbook is provided as an informational reference regarding amenities, hours of operations, memberships, policies, and procedures. Please let Fitness Center staff know if you have any additional questions regarding our policies and procedures.

Hours of Operation

Monday—Friday: 6:00am—9:00pm Saturday: 7:00am—5:00pm (Labor Day to Memorial Day) Saturday: 7:00am—3:00pm (Memorial Day to Labor Day) Sunday: 7:00am—3:00pm

Closures • Closed: New Year’s Day, Easter, Memorial Day, Fourth of July, Labor Day, Thanksgiving, Christmas Day • Closes at Noon: Christmas Eve, New Year’s Eve • Hometown Fest (typically held around July 4th): Based on the schedule of Hometown Fest, hours of operation may be adjusted. Members will be given notice at least one month in advance. Community Fitness Center may modify its hours or close the facility due to hazardous weather conditions or necessary maintenance issues. Community Fitness Center staff will inform members as early as possible through postings and email blasts. To join our Fitness email list, visit palatineparks.org and select “Join Our Email List”. Memberships will not be prorated, refunded, or extended due to facility closures.

Open Gym

Open Gym Basketball at Birchwood Recreation Center, Community Center, and Falcon Park Recreation Center are included in memberships at Community Fitness Center. Hours vary weekly and can be viewed at palatineparks.org.

Track

Track usage at Community Center and Falcon Recreation Center are included in memberships at Community Fitness Center. Both tracks are open according to facility hours unless otherwise posted.

Facility Entrance

All members must enter Community Center through the front entrance on Wood Street.

Member Engagement

It is the goal of Community Fitness Center staff to ensure members and guests are feel they are a part of the fitness community. We provide the following member engagement opportunities listed below:

Quarterly Newsletter The Community Fitness Center newsletter is published on a quarterly basis, in March, June, September, and December. The newsletter is emailed to all members and is also available for community members in the Fitness Center and lobbies of Community Center, Birchwood Recreation Center and Falcon Park. The newsletter contains staff and member highlights, Fitness Center news and updates, recipes and more!

Open House An Open House is held annually at Community Fitness Center for both current and prospective members. During this event, the center is open to the public.

Member Appreciation Days/Raffles Throughout the year, several Member Appreciation Days and Raffles are held.

Promotions Throughout the year, several promotions are offered on both memberships and personal training. Promotions will not be extended past their intended date.

Referral Program

Active Community Fitness Center members can receive a free month when they refer a friend to sign up for a membership. Both the current and new member will receive a free month.

Guidelines

The following Guidelines apply: • Only new members who have not been a member in the past three years will qualify for this offer. • Referrals can only be made by current members. • Referral card must be submitted at the time the new member enrolls.

Community Fitness Center Memberships

Community Fitness Center offers several payment options for community members, ages 14 and over:

Annual Membership Members who purchase an annual membership pay in full for a 12-month membership. At the end of their 12-month term, they will need to re-enroll to maintain membership.

Month-to-Month Membership Members who purchase a monthly membership pay their membership dues on the 1st of every month (1st Month Fee is paid at the time of registration). Monthly memberships are ongoing and must run for a minimum of three months. As these memberships are ongoing, members must submit a membership cancellation request form to cancel their membership. More information on the Cancellation Policy can be found on page 9. Monthly members also have the option to put their membership on hold by submitting a membership hold form. More information on the Hold Policy can be found on page 9.

Initiation Fee All new and inactive members are required to pay a $25 Initiation Fee.

Discount for Annual and Monthly Memberships • Youth: Student ages 14-18 will receive a 10% discount on their membership. • Senior: Individuals 62 years of age or older will receive a 10% discount on their membership. • Additional Member: ◊ Additional household members will receive a 15% discount on their membership. ◊ Household members must reside in the same household, at the same address.

Non-Resident Pricing Non-Residents of Palatine Park District pay an additional 25% for Fitness Center memberships.

Daily: Residents and Non-Residents can purchase a $7 daily fee for one visit. Discounts do not apply towards the initation fee.

Punch Pass: Residents and Non-Residents can purchase a 10-visit pass. Passes are non-refundable and non-transferrable. Discounts do not apply. Seasonal Student Passes: Seasonal student passes will be promoted throughout the year. These passes are non-refundable and non-transferrable. Students ages 18-24 must present a valid student ID to register.

Electronic Fund Transfer (EFT)

Month to month memberships are automatically charged to the members credit card on the first business day of each month. Members pay the first month at the time of enrollment. The first month total will be prorated dependent on the enrollment date. It is the responsibility of the member to provide the Palatine Park District with updated credit card information. If the payment does not go through on the provided credit card, the member will receive an email showing the balance on their account. In addition to the email, staff will call to inform the member of the balance on their account. If the balance is not paid within 30 days, a $25 late fee will be added to the account and the account membership will be suspended.

Suspended Account

In the case of a 30 day past due balance, the account will be suspended. A suspended account means the household will be unable to use their membership(s) or register for park district programs or activities.

Cancellation Policy Community Fitness Center month-to-month members may cancel their membership after three paid months. Only members with a month-to-month membership can cancel their membership. Members must submit a Membership Cancellation Request Form (pg.16) at the Community Center or Birchwood Recreation Center with attention to Alec Owens (Assistant Facilities Manager) or emailed to aowens@palatineparks.org. Cancellation’s must be received by the 20th of the month in order to cancel for the upcoming monthly bill. If received after the 20th of the month, members will be responsible for the next month’s bill. Forms will only be accepted if entirely completed and signed. Upon cancellation, the member will receive a confirmation email from staff. No refunds will be issued. The three-month paid requirement to cancel a membership can be waived with a physician’s note for a medical exemption. Annual members are allowed to cancel their membership with a note from a physician, excusing them from exercise

Hold Policy To accommodate members who may need a seasonal/temporary membership, Community Fitness Center month-to-month members can put their membership on hold for a minimum of one month, and a maximum of six months. Membership must be active for at least three paid months before a hold can be processed. Members must submit a Membership Hold Request Form (pg. 17) at the Community Center or Birchwood Recreation Center with attention to Alec Owens (Assistant Facilities Manager) or emailed to aowens@palatineparks.org. Hold request’s must be submitted at least seven days prior to the membership hold date. Upon submission, a one time $5 maintenance fee will be charged at the time of the membership is placed on hold. Forms will only be accepted if entirely completed, signed and the maintenance fee is paid. Members who put their membership on hold will not have a re-enrollment fee and will not have any rate increases that may occur while they are away. Memberships and payments will be deactivated on the start date listed on the form. During this time, the member will be unable to use their membership and their monthly payments will not be charged. Memberships and payments will automatically reactivate on the end date listed on the form. The three-month paid requirement to put a membership on hold can be waived with a physician’s note for a medical exemption. Annual members who wish to put their membership on hold due to medical reasons, must provide a note from a physician that includes the time frame they are away.

Permanent Locker Rental Policy

• Locker rentals are available for Community Fitness Center members at the rate of $5 per month. Locker rentals must be the same duration of the membership and the fee must be paid in full. • If a member signs up for a locker after the start of their membership, the rate will be prorated. • Fitness Desk Attendants can sell the locker pass, but the locks are distributed by the Assistant Facilities Manager. • Locks and combinations will be supplied by the Palatine Park District and must be returned at the end of the rental term, unless the term has been renewed. • Lockers are made available for members use to store personal items necessary for use at the fitness center (i.e., body wash, hair products, change of clothes). • Lockers are not to be used to store items which violate Park District policy including; weapons, drugs, cigarettes, alcohol, flammable substances or other contraband. • The Park District assumes no responsibility for loss or damage of any item in a locker, locked or unlocked. • Member is not to leave any food or beverages in the locker overnight. • Park District staff has the right to inspect any locker not abiding by the guidelines set forth in this policy.

Personal Training

Community Fitness Center has several personal trainers on staff. Trainers are certified to work with different age groups and abilities. Personal Training can be purchased at an additional rate.

No outside personal trainers can train members without written permission from Palatine Park District per the Palatine Park District ordinance listed below: Section 2.08: Commercial Sale, Rental, Exhibition, or Distribution of Goods or Services: No peddler, vendor or any other person involved in an endeavor for profit shall engage in the commercial sale, rental, exhibition, or distribution of goods or services, including without limitation the giving of instruction or lessons for a fee, upon District Property unless he has received a permit, license or contract therefore from the District.

Code of Conduct

In part the District’s mission is to provide high quality, safe and wholesome programs, parks, facilities, and related services for all residents and guests. The District has developed, implemented, and enforced a code of conduct for all residents and guests participating in recreational opportunities on District property. This code of conduct is the minimum expected behavior of every participant to ensure the safety and positive experience of all involved. The District uses a three-pillar positive behavior program as the platform to define the expected behavior of all participants. This platform is aligned and consistent with the program used within the local elementary schools to create consistency among children and parents whether at school or play at the park district. At Palatine Park District we abide and model the positive behavior system with regard to how we operate the District and interact with our participants. These expectations and guidelines are the common thread and endorsed culture of how we conduct business and expect of all participants, staff, volunteers and general public as a means to provide a healthy and safe environment to enjoy all the opportunities offered by the District. The following section illustrates the core elements and expectations of the Positive Behavior Program and the general guidelines of the District’s code of conduct policy.

Fitness Center Rules • Members must check in at the Fitness Desk before working out. • Non-members must pay the daily fee before working out. • All participants must be 14 years of age or older to enter and utilize the Fitness Center. • Appropriate attire must always be worn. ◊ Shirts must cover the torso from mid-stomach to above the chest, not containing offensive wording or graphics. ◊ Shorts providing coverage from the waist to thigh. ◊ Close toe, athletic shoes be worn. ◊ With the exception for water and sports drinks, food and beverages are not permitted in the Fitness Center, unless provided by Fitness Center Staff. •

Please step out of the center to take a phone call.

• Capturing images of another member without their permission is prohibited. • Abuse of equipment, other patrons and staff will not be tolerated; offenders may be asked to leave and will not be issued a refund. • Only Palatine Park District staff can conduct personal training. • Palatine Park District is not responsible for lost or stolen items. • When listening to an audio device, patrons must use headphones. • There is a 30-minute limit on cardio equipment when all machines are being used.

Storage

Cubbyholes are available in the Fitness Center for members to store their personal items while they workout.Palatine Park District is not responsible for lost or stolen items. Members may also utilize the locker rooms located on the first floor of Community Center. Lockers are for day use only, free of charge. Lockers can be rented on a permanent basis at $5 per month. For more information see the Permanent Locker Policy (pg.18)

Equipment Use

To ensure all participants have access to cardio equipment, users must limit their time to 30 minutes. A waitlist for cardio equipment is available at the Fitness Desk during peak times. Out of courtesy for others all members are expected to re-rack weights after use. Free weights should not be dropped. In addition, all mats, accessories and stretching equipment must be put away after use. In an effort to keep our Fitness Center clean, and prevent the spread of germs, members are encouraged to wipe down equipment after each use, with the provided sanitizing wipes.
