MAYOR’S FOREWORD Dear Partners, I would like to take this opportunity to recognise and thank you for the important work you do in caring for the needy. The South West Cares Referral Guide was introduced in 2013 to streamline referral processes between help agencies and community organisations. It also provides ready information for social and community workers to assist the vulnerable. We have updated and improved this referral guide with more information. It has been refreshed and made userÂ-friendly. I hope you will find it useful in your course of work as this aims to also give you an easier and clearer understanding of the referral process. A GOOD RESOURCE FOR GRASSROOTS VOLUNTEERS Our grassroots volunteers come from all walks of life. They are eager to lend a helping hand to the needy residents but may not know enough sometimes. Our residents too, they do not know where and who to look out for assistance when required. The South West Cares Referral Guide is helpful as our grassroots volunteers can easily sieve out relevant resources for residents whenever required.

COMPREHENSIVE AND HANDY REFERENCE FOR SOCIAL WORKERS Our social workers can follow the flowcharts and sieve through the assistance that can be provided for the needy families. South West Cares Referral Guide helps our social workers connect to the necessary community resources available in the district on the go. Support Services for Women AWARE provides support for women by providing a helpline, counselling services, a legal clinic and a befrienders service assisting women in dealing with hospitals, police and the courts. Pioneer Generation SERVICES The Pioneer Generation Office was set up to conduct personalised, face-to-face outreach to the Pioneers and their families on the Pioneer Generation Package in order to: • Thank them for their contributions to the Nation; and • Assure them that the benefits will make healthcare costs more affordable. The PGO was set up in late 2014 to gather and train community volunteers to reach out to Pioneers. Senior Activity Centres, SENIOR ACTIVITY CENTRES (CLUSTER SUPPORT) and SENIOR group homes Senior Activity Centres are drop-in centres for seniors living in rental blocks. They provide information and referral, and organise social recreational activities for the seniors with the aim to promote bonding and interactions with others.
Senior Activity Centre (Cluster Support) provides social support to seniors who have little or no family support. To enable the seniors to age-in-place within the community for as long as possible, the SAC(CS) provides these services to help the seniors. Senior Group Homes bring vulnerable seniors with some physical impairments together in cluster of rental units.
