Polk’s Drugs transitions to next generation of pharmacy management with the help of QS/1® Polk’s Drugs is a small chain of pharmacies based in Mississippi. The Ocean Springs store, which opened about a year ago, is unique in that it also includes a gas station. identical functionality but is physically adapted for 3"x 9" cards commonly used for narcotics or to fit in med carts configured for strip-packaging. Each DOSIS C60 card holds up to 16 doses.

n DOSIS U60: Based on the L60 platform, DOSIS U60 adds the

functionality of full unit-dose labeling within the robot. Each blister of each card can be printed with your requirements of NDC bar code, drug name and strength, lot number, expiration date, etc.

n DOSIS mPAQ Printing: There are two primary aspects to DOSIS mPAQ Printing: directional (or work zone) printing and med-pass printing. Directional printing can route labels throughout the pharmacy based on prescription data, medication location, etc. Med-pass (or time-pass) printing automatically highlights color-coded labels to match med-pass data and times for patients and their facilities.

Polk’s Drugs transitions to next generation of pharmacy management with the help of QS/1® Polk’s Drugs is a small chain of pharmacies based in Mississippi. The Ocean Springs store, which opened about a year ago, is unique in that it also includes a gas station. For manager Jesse Murphy, PharmD, choosing the right technology was crucial to ensuring his pharmacy not only opened with a strong start, but continued to grow and operate as efficiently as possible. “Our greatest concerns as we look toward the future are looking for ways to improve our profitability, maintain patient safety and implement technology,” Murphy said. QS/1 has helped Murphy address many of those concerns. That’s not something that we can say with other systems.”


C A S E S T U DY The fact that the system is so easy to use has made it easy to train others as well, Murphy said. “What may have taken weeks or months to get accustomed to with previous software, now often only takes a couple of days.” What’s more, a lot of SharpRx’s features are exactly where you would expect them to be, he related. “They make sense from a workflow standpoint or from an organization standpoint,” he said. “One of the best things about it is that it keeps the important tools front and center. They’re very easy to find, they’re in logical places, and it’s very easy for you to navigate to the different elements. You’re not overwhelmed with too much information or too many functions. But if you need additional functions, they’re easily accessed.” That’s something that other platforms he has used haven’t always done a good job of handling, he said. “They either overwhelm you with too much or give you too little control.” SharpRx makes using multiple screens easy as well – something Murphy and his staff really like since they may have many screens open at one time. “We really like the multi-screen features. It allows us to view multiple patients, multiple prescriptions and multiple drug or prescriber entries without having to leave what we’re doing.” “Phone calls are no longer a problem because it’s easy to switch to a new patient and access what they are looking for, fill prescriptions, add refills to our queue and then immediately go back to what we were doing. It’s very easy to leave what we’re doing,

Make Changes to Labels and Forms Easily “Another benefit is we can update our prescription forms, and make them clearer and easier to read,” he said. “And, we can do it within minutes ourselves.” Similarly, SharpRx has helped the pharmacy overcome one of its biggest challenges – label creation. “One of the biggest issues we had with past systems is when you wanted to make a change to the label.” Rather than requiring requests for changes from a third party, “SharpRx has a simple drag-and-drop format to adjust forms, so you can make a change immediately,” he said. “It takes a fraction of a minute to make and save, and you can see the results right away. That saves a lot of time.”

C A S E S T U DY Simplify Adjudication and Inventory Murphy said SharpRx has done a lot to simplify some of the more difficult aspects of processing prescriptions. “We no longer struggle with adjudication and inventory,” he explained. “It speeds up the processing of prescriptions so our customers are in and out very quickly. It has a very good system for tracking prescriptions so that we can pinpoint when and where a prescription was filled and picked up and when that patient would be due for the next fill. It has done a really good job of helping us maintain our inventory so that medications are not out of stock as frequently.” “Adjudication is something that is always a struggle with any pharmacy system,” he said. “With SharpRx, most prescriptions go through without any intervention on my part. We no longer have to continually work to get prescriptions to adjudicate perfectly. The software handles most of that.” Better Patient Record Management Managing patient records and linking family members to the same account has also been a snap with SharpRx, Murphy said. “When someone picks up a prescription, the system will notify you if you have

prescriptions for other family members. Also, it will notify you if it is in a separate bag or in the fridge, and that’s been a real handy system.” “As part of our transition, we also switched signature management from paper to electronic. That has always been a difficult thing to do in prior systems. With SharpRx, it’s easy to access and retrieve those signatures,” Murphy said. What’s Most Important in a Pharmacy Management System? When choosing a pharmacy management system, Murphy said the system must be powerful enough to handle the challenges a busy and ever-growing pharmacy faces. Yet, it must be intuitive enough so that staff can begin processing prescriptions quickly. “When I think about what I want my pharmacy software to do, it really needs to do the grunt work,” he said. “It needs to do the paperwork. It needs to do the things that don’t require a pharmacist or a technician for intervention. I want software that is not going to work against me, but is going to work for me. These are all things that SharpRx has excelled at,” he said.

PRIMARY INTENDED OUTCOME(s) An aging population has increased the demand for long-term care (LTC) in our nation’s healthcare delivery system. The typical LTC patient is living with more than one chronic condition and relies on as many as 10 different prescription drugs. Nursing facilities often contract with specialized LTC pharmacies to provide support and continuous medication management for complex health concerns. Because senior patients’ needs happen around the clock, and very often on a STAT-needs basis, the Centers for Medicare and Medicaid Services (CMS) requires LTC pharmacies to have an emergency backup system in place. When these requirements are handled internally, in-house staff must put in extra hours and shoulder backup duties after they’ve left work. This approach introduces the risk of employee burnout as well as the potential inability to service customers when they need you most. But there is a better way. THE CHALLENGE Managing a LTC pharmacy means balancing equally the requirement to maintain a dependable backup system, the wide variety of customer needs, and the important goal of sustaining and boosting employee morale. Tim Rowland, former COO for Strategic Health Partners/Senior Care Pharmacy, recently managed a group of seven pharmacies. “In two LTC pharmacies that we established, we initially handled the backup, including STAT orders, needs ourselves,” he said. “As we grew, however, the number of these orders became a bit overwhelming.”

Ultimately receiving as many as 30 after-hours or STAT requests per day meant that staff pharmacists were being repeatedly interrupted during their normal duty or awakened in the night and potentially having to come into work the next morning exhausted. Jason Sutton is the director of LTC pharmacy operations at Medicine Chest Pharmacies. His challenges involve getting smaller pharmacies off the ground quickly and efficiently. “Independent pharmacies can find it hard to meet all of their customers varied needs,” he said. For example, when you are rolling out a stand-alone pharmacy, you may not have sufficient resources to quickly locate an unusual drug and manage STAT delivery. “In many cases, that kind of turnaround would not be able to happen for us without help.” STAT orders are particularly disruptive and negatively impact efficiency, MedCall addresses this directly.

THE DECISION For Rowland, cost played a large role in his decision to seek out an external backup pharmacy service. MedCall provided reliable and comprehensive backup coverage for approximately $5,000 per month, delivering a substantial cost reduction of more than 55%. “We were evaluating 24 hours, 7 days a week staffing at our Strategic Health Partners Pharmacy in Utah,” Rowland said. “We needed to cover eight hours a day of non-staffed time. It would have cost $13,000 a month to hire a pharmacist for 56 hours a week.” Staff satisfaction was also a concern. “It’s not easy to find great team members. THE RESULTS Rowland’s LTC pharmacy serviced facilities that were spread across a large geographic area, which meant delivery services were a key part of the business. “At times, because of weather or some other process related problems, a delivery could not get on a flight — meaning it would not have been possible to get the drug where it needed to be until the next day.” When it comes to STAT IV medications and first-dose needs, timely delivery can mean life or death. CASE STUDY The TCGRx Beacon Pharmacy Inventory Management System Helping pharmacies do more business with less inventory.

PRIMARY INTENDED OUTCOME Inventory is typically a retail pharmacy’s largest financial asset. One key decision for every pharmacy manager is how much inventory to keep on hand. Having too much inventory can mean less available cash; too little inventory, however, creates an inability to fill prescriptions in a timely manner and a loss of patients. Success relies on the pharmacy manager’s ability to maintain accurate and current records of drugs on the shelves, as well as prescriptions filled. Effectively, inventory management accuracy is critical to ensuring retail pharmacy profitability. One company, TCGRx, is leading the way in effortless perpetual inventory control. THE CHALLENGE One of today’s greatest challenges in pharmacy is the continued and growing high cost of drugs and an unprecedented boom in treatment innovations. In 2015, total drug spending in the United States reached $364 billion, with $98 billion attributed to specialty drugs. By 2020, expenditures are expected to reach $483 billion overall and $212 billion for specialty meds. For pharmacy owners, this environment equates to a huge amount of beneficial, valuable inventory to manage — and protect.

CASE STUDY BEYOND INVENTORY CONTROL Part of Beacon’s design could be thought of as ‘intentional disorder.’ With Beacon, pharmacists and techs may not know where a specific drug is shelved, but the computer does. This approach provides invaluable protection against medication filling errors. As the Hurricane team found out, randomized shelving also offers the ability to confound potential burglars and thwart the epidemic of pharmacy robberies that happen every day in the United States. Hurricane’s Beacon system was completely installed over one weekend during the pharmacy’s normal closed hours. “We scanned in $800,000 worth of inventory,” recalled Holt. The system went live on Monday; on Thursday night, Hurricane Family Pharmacy was robbed. Cameras caught the burglar standing in the pharmacy, clearly confused as to what drugs to steal and their locations. “Of course he was looking for controlled substances,” said Holt. In a panic, the robber grabbed five bottles and fled. What he wound up with was chewable amoxicillin tablets, Poly-Vi-Flor infant vitamins, and blood pressure medication – none of it worth anything on the street, and a relatively minor financial loss to the pharmacy. Holt later learned the burglar had staked out Hurricane Family Pharmacy in the week leading up to Beacon’s install in order to map out the high-valued drug locations. Without Beacon, Holt knows his losses would have been far more significant.

THE DECISION The first Hurricane Family Pharmacy location opened in 2009 in Hurricane, Utah. Pharmacist and company president, Cliff Holt, recalled those early days, when “you would just pray for [a customer] to walk in the doors.” Those worries didn’t last long, as the business quickly grew. Today, Hurricane cares for more than 14,000 patients in their community. The pharmacy’s growth led to some new needs in terms of drug packaging and inventory management. “About a year and a half ago, we outgrew the packager we had been using,” said Holt. They needed something new. He'd heard about TCGRx and decided to visit their home office in Powers Lake, Wisconsin, where he was also introduced to the Beacon Inventory Management System. Holt was impressed by the model pharmacy TCGRx houses on-site and, in particular, the compact layout maximizing its footprint. “I couldn't quit thinking about that. I was out of space — we were running out of room in the bays, and I had two pharmacists working at one counter, along with one filling technician — there wasn't an inch to spare.”

THE RESULTS Hurricane Family Pharmacy purchased TCGRx’s Beacon Inventory Management System in June 2016. The decision to invest in Beacon meant articulating and creating a customized design to meet Hurricane’s specific needs. TCGRx’s mechanical engineer worked with Holt to structure and install Beacon’s modular, mobile components in an optimal configuration for the space. Hurricane's drug storage footprint was reduced by more than 50%, allowing more space for filling of prescriptions. Workflow increased, thanks to three high-density filling stations located right where the technicians stand, and instead of one filling technician, Holt now has three. “The drugs are literally in reach now,” said Holt, “not steps away, but actually within arm’s reach.” At the same time that revenues were growing, Hurricane’s inventory was reduced by $200,000. This is possible because Beacon’s perpetual inventory system knows exactly what drugs are on the shelves, in what amounts, and accurate expiration dates. “Beacon knows the dollar value of our inventory down to the penny,” Holt said. That knowledge alone is an outstanding ROI.

CASE STUDY In addition, Beacon prevents unnecessary and costly waste. For Hurricane, what used to be one or two bins of expired drugs each month has been reduced to less than half of one bin. “That kind of waste really bothered me,” Holt said. Now when an order is made, Beacon pulls medications in order of return-to-stock drugs first, partial bottles next, and then shortest expiration time. Losses due to expiration dates are simply no longer an issue. Using Beacon makes sense from a sustainability standpoint, as well. “Every other piece of technology we’ve had, we have outgrown and had to scrap,” said Holt. “[With Beacon] I can simply keep adding on and incrementally increase the system as we need it.”

THE CONCLUSION Beacon offers pharmacy managers the opportunity to reduce and control their inventory, streamline and increase workflow efficiency, and boost patient care and consultation time. All of this is possible without remodeling the pharmacy or imposing a huge learning curve on pharmacy staff, and with no lapse in patient safety. Hurricane's business grew from 400 filled prescriptions a day before Beacon, to an average of more than 600 today — with the same number of staff members and more than a 50% reduction in the drug storage footprint. And they haven’t hit capacity yet. “We could handle another 30% or 40% increase in growth right now, the way things are laid out with Beacon,” Holt said.

Pyrosate has been developed as an easy-to-use Limulus Amebocyte Lysate (LAL) gel-clot test that enables rapid testing and does not require special laboratory supplies. The easy-tofollow, step-by-step illustrated instructions allow the user to set up assays within minutes. In addition to FDA licensure, the Pyrosate® Kit is now available in sensitivities of 0.03 EU/mL, 0.125 EU/mL, and 0.25 EU/ mL, in both 10-pack and 30-pack formats. With the introduction of this licensed version, in addition to research and water testing, Pyrosate is ideal for dialysis centers, compounding pharmacies, and pharmaceutical and medical device companies.

Performing the Test

The Pyrosate® kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.

Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage. About SMP Pharmacy Solutions South Miami Pharmacy (SMP) was founded in 2003 by Dr. Armando Bardisa with a goal to improve the quality of life for their customers and patients through personalized service. In order to provide customized service for their patients, SMP offers hard to find medications, compounding services, one-on-one fertility specialists and medication therapy management for specialty pharmacy patients.

Considering that they ship approximately 10,000 prescriptions each month from their two Miami facilities, many to South America and Caribbean locations, safe shipment of temperature-sensitive products is particularly critical to their focus on patient care and quality.


Needs Drive Indicator Evaluation “Particularly during the summer months, we would receive calls from our patients stating that the ice packs were melted and they were not sure if the medications were still effective and safe to use,” said Brian Brito, Vice President, whose pharmacy expertise and commitment to quality pharmacy services have helped drive SMP’s success. “Plus, we also needed to implement this cold chain procedure for our specialty pharmacy accreditation and state requirements.” “Temptime had been trying to get us to use their heat indicators for quite some time,” he added. “Quite honestly, we were hesitant when thinking that it might cause more calls and customer confusion. Additional considerations were cost, implementation of a new system to the workflow, and extra time added to the packaging process.”

Shipping Studies Prove Value

Making The Choice SMP evaluated several temperature indicators but landed with LIMITmarker, noting that they knew it would be easy for their customers to read, and reliable at a reasonable price per unit. Brian noted that none of the other options provided the flexibility and technical support they were seeking. They also liked that LIMITmarker is easily customizable, and have incorporated it into their own indicator card, with their logo, instructions and contact information—further demonstrating their commitment to strong bonds with their customers.

Despite those concerns, SMP is completely dedicated to patient safety and satisfaction, so they agreed to work together with Katie Kraverath from Temptime on developing and executing shipping studies. Convinced that gathering solid data on exactly what was going on with their shipments would reveal the answer, SMP worked with Katie and Temptime to set up the temperature-monitored test package shipments to various locations, along with FedEx, who handles SMP’s specialty pharmacy shipping.

“We were surprised to find out how hot the packages got—even with double packing and ice packs,” said Brian. “It opened our eyes to the fact that sometimes the package sits in a truck at 110ºF heat, or misses the flight, and sits even longer. Using an indicator shows our patients that we are committed to them.”

Return On Investment

Despite their initial concerns, SMP encountered no real significant challenges. They incorporated the new temperature indicator SOP into their workfl ow at both shipping facilities, and trained staff to respond to patient questions about the indicators. “Role playing was particularly helpful,” explained Brian.

“The most important return is that the temperature indicator helps us give both our patients and staff peace of mind,” said Brian. “Again, these tools like LIMITmarker support our position on patient safety and commitment to superior quality care.”

“With every shipment we send a patient letter that explains how the temperature indicator is used, read, and when to call us if the indicator shows the temperature limit was exceeded,” said Brian. SMP is now including a customized LIMITmarker card and patient instructions with all refrigerated shipments that are sent out via FedEx, and also with their couriers for in-person deliveries.

The direct ROI for SMP has been savings of thousands of dollars on what would have been spoiled medications and reshipment fees. Also, the increased volume of physician referrals has not been quantified, but SMP has confirmed a positive impact on their business.
