hen Preveon Specialty opened its doors, their primary patient-base was in the immediate area— most less than a two-hour drive away. They decided that instead of shipping via courier, they would have their own drivers hand-deliver specialty medications to patients in close proximity. For those patients who lived outside the region and required medications to be shipped, Preveon knew they wanted to include a temperature indicator to ensure that refrigerated medications stayed within a safe temperature range during transit and alert patients if their medication was exposed to temperatures that could negatively affect its effectiveness. Case Study: Preveon — Delivering Peace of Mind With Every Specialty Medication From Temptime

Business Challenge In April of 2017, Preveon Health, a care management company based in California, launched Preveon Specialty— their specialty pharmacy division that dispenses medications to treat a range of inflammatory conditions. While they are licensed in 23 states, most of their nearly 2,000 patients live close by in the Inland Empire region of Southern California, including Riverside and San Bernardino counties as well as parts of Los Angeles. This, says Jefmar Dickey, Business Manager for the specialty pharmacy, is one of the things that sets them apart. “The majority of our medications require refrigeration and need to stay within a certain temperature range,” he explains. “The fact that we can have our own drivers hand-deliver medications right to a patient’s door is something a lot of large specialty pharmacies just can’t offer.” Not only does this mean they can deliver medications quickly, decreasing the risk of temperature excursions, it also adds a much appreciated personal touch. “Rheumatoid arthritis, ulcerative colitis, multiple sclerosis and many other inflammatory diseases are chronic,” says Jefmar. “So

It is actively converting away from the cycle-fill model because it is more efficient for them to fill on-demand using their integrated DOSIS automation. They have already eliminated prepacks from their workflow “to reduce inventory and extra touches.” Using DOSIS Systems to automate filling cards and printing time-pass labels, an 18,000card pharmacy doubled in size without adding any staff on the floor. At another pharmacy doing 180,000 cards per month with 75% filled by DOSIS, the return-to-stock feature allowed them to efficiently track and re-use cards filled for scripts that are canceled before shipping. The Xana strip packager then utilizes active cassette and tray management to better dispense medications in an uninterrupted workflow. In advance of a batch run, the number of NDC’s allocated as cassetteloaded drugs can be placed (expandable up to 1,000 unique NDCs), thereby minimizing the labor previously required to be delivered manually via tray.

and at the same time manage labor costs and reduce operational hours. To accomplish these goals, high-volume pharmacies are challenged to find the right equipment that can automate drug packaging efficiently and reliably while ensuring accuracy.

THE DECISION Automated pharmaceutical packaging technology is a growing industry, aiming to address the needs of an increasingly large population of complex patients and the pharmacies that serve them. Numerous manufacturers are vying for prized pharmacy space. “I was in the market for new automation,” said Mick Blazier, Pharmacist-in-Charge at EverSpring Pharmacy in St. Louis. “There are a number of big players in the field, of course. Another pharmacy that I had a relationship with used Noritsu, so I also included them in my search.” EverSpring exclusively targets long-term care, including assisted-living facilities and developmentally disabled group homes, so Blazier’s interest in improving the pharmacy’s adherence packaging was strategic. During the course of his research, he met with a handful of pharmacy automation market leaders, including Noritsu. “I was most impressed with what Noritsu offered, both in terms of their machinery and their customer support.” EverSpring ultimately made the decision to purchase two Noritsu Xana 4001 strip packagers as well as their MDM-3 medication detection machine.

THE RESULTS The addition of Noritsu’s technology resulted in substantial improvements in EverSpring’s production time, workflow, and overhead costs. “We were running 16-hour days on average; sometimes we were so behind, we had to run 20 or 24 hours a day with the older machines,” said Blazier. “The new machines decreased our run time to an average of 10 hours a day and increased our output by 25%, while decreasing our payroll hours by about 50%.”

CAS E S TU DY EverSpring’s new packagers combine lower labor costs and improved features. Custom-labeled medication pouches provide important information in clear, crisp detail, making it easier for patients and caregivers to read. The Noritsu equipment also allows EverSpring’s staff members to make more efficient use of their work time. When the outdated machines filled pouches, they still had to be hand-checked by a pharmacist for safety. With the addition of medication and pouch detection through the MDM workstation, a digital image and associated data are archived for every pouch ever produced. “A pharmacist sees all of [the images] on screen and can visually inspect a blown-up image of each pouch if necessary,” Blazier said. The MDM workstation has reduced staffing needs by two FTE pharmacists and also allows seamless connectivity between EverSpring’s existing pharmacy management system and eMar administration. EverSpring’s accuracy has also benefited. “The combination of Noritsu equipment we selected has tremendously cut down on medication errors leaving the pharmacy,” Blazier said.

THE CONCLUSION Ultimately, customer service was the main driver in Blazier’s choice of Noritsu Pharmacy Automation, and he has not been disappointed. “They had an onsite presence here for two weeks after the installation, and we are always communicating with Noritsu about ways to make the system even better,” said Blazier. “If we ever have a problem, they either log in remotely to take a look immediately or send one of their dedicated field techs here within a couple of hours.” As pharmacy growth continues, Blazier acknowledged that EverSpring would turn to Noritsu again for additional packaging machines. “They are so efficient, and the support is that good.”

Performing the Test

Q. As both president of the Alliance for Pharmacy Compounding as well as owner of a compounding pharmacy, what are you seeing lately in the world of pharmacy compounding? It is really an interesting time in the world of pharmacy compounding. The demand and need for pharmacy compounding continues to grow, and compounding pharmacies are doing a great job to fulfill the needs of our providers and patients. The biggest challenge our profession is facing is keeping up with the constantly evolving standards, rules, and other regulations. While compounding pharmacists and their teams have done a great job in rebuilding their pharmacy labs to keep up with these standards, we are now seeing that these standards will constantly change. This constant change can be a challenge for a pharmacy to keep their pharmacy labs compliant and in turn, continue to compound preparations in a timely manner and at an affordable price to the patient. Like every other profession, COVID-19 has impacted compounding pharmacies as well as vendors and wholesalers that supply to the pharmacies. However, the pandemic served as an excellent marker for how a 503A compounding pharmacy can offer aide to the healthcare space. Specifically, we worked with Congress, FDA, State Departments of Health, and Boards of Pharmacy to compound and deliver some of the drug shortage medications for our health systems. This was an amazing moment in that both the public and private entities quickly and efficiently worked together to provide medications necessary to treat COVID-19 that were on the FDA shortage list until the pharmaceutical manufacturers were able to ramp up their production. It was great to see 503A pharmacies, the state regulators, DEA, FDA, and Congress work together to help provide these products when patients needed them the most.

Q. What examples are you seeing that perhaps interrupt patient access to compounds? One example is that the FDA is proposing a Memorandum of Understanding (MOU) with the boards of pharmacy and or state legislatures to report compounding pharmacy activities on behalf of the FDA. What compounders are concerned about is that this is an unfunded mandate and the states will more than likely opt out — restricting pharmacies to ship only less than 5% of their products across state lines. Obviously, this is a huge concern for 46

many pharmacies that are licensed in multiple states, and especially for those pharmacies that are located at a border with an adjacent state, and have a good number of patients in both states. Another example is more rural states which may not compound certain drugs or have the equipment required that pharmacies in other states specialize in. The patients in that rural state are succumbed to only what their state pharmacies have to offer.

Q. What can compounding pharmacists do to help ensure that patient access isn’t interrupted? There are three major actions compounding pharmacists and their pharmacy team can take to ensure their patients maintain access to compounded medications. First, make sure that compounding pharmacies strive for best practices, with the goal of formulating preparations to the highest standards of quality and safety. We’re constantly reminded of what transpired with NECC over eight years ago, and everyone is responsible to ensure this never happens again. The second is to join a pharmacy organization. Being a member of organizations like Alliance for Pharmacy Compounding provides pharmacy professionals the latest in regulations and rules, but is also a sounding board and advocate voice to provide feedback on the consequences of current and proposed rules. The third action that a pharmacist and their team can take is to be engaged with their state congressman and local government officials. Inviting a congressman to their pharmacy for a tour and explanation of their operations gives the congressman first-hand experience on what role pharmacy compounding plays for the community. It allows them to see inside a pharmacy compounder’s world and how they are serving patients in that congressman’s valued area. It is best to have compounding pharmacists dictate the future of their profession rather than somebody else dictate it for them, and to have the support of your congressman and local officials is crucial in ensuring the profession maintains strength and support.

Q. Looking forward, how do you see compounding pharmacies combat COVID-19? The pandemic is a perfect example of how compounding pharmacists can take on a greater role for their profession and as the “drug experts.” Specifically, compounding pharmacists and the profession of pharmacy as a whole, have access to drug information


Looking forward, there are opportunities for pharmacists to administer vaccines, for prevention of COVID-19 and other viruses. There will undoubtedly be additional testing available and pharmacists are more than capable of administering those tests to patients in their communities. Now is the time for pharmacists to arm themselves with those skill sets and be prepared to take on these patient needs and demands.

Q. What other opportunities are available for compounding? Although it’s challenging to look forward as we’re navigating through our current pandemic, we think there are a plethora of opportunities for compounding pharmacists. As mentioned before, we talked a little bit about point-of-care opportunities with respect to COVID-19. Also in this new environment, telepharmacy will play a greater role to continue patient access to compounds. In addition, there is an opportunity to partner with pharmaceutical companies who are looking to find a pharmacy to compound medications in clinical trials. It’s a model that a few pharmacies used in the past, but now we’re starting to see more of a need for pharmacy-pharma partnerships to supply products for drug studies.

patients’ needs. Many compounding pharmacies are providing products today that weren’t necessarily at the forefront of our minds a few years ago. On the flip side, some of the products that we provided years ago are no longer available today. I think that’s what’s so interesting and fascinating about compounding is that we have the ability to adjust to what our patients, providers, and healthcare community needs. Along those same lines, we’re also seeing pharmacies get more specialized. Some pharmacies are adjusting their practice models to focus on compounding a smaller portfolio of compounds as opposed to being the jack-of-all-trades. For example, certain compounding pharmacies may provide only animal health products, eye care products, or sterile injectables. I believe if pharmacies and pharmacy owners adopt the principle that, “the only constant in life is change itself ”, they are in a much better position for a successful future. And that future is bright!

Q. What would you say to perhaps a reader that is a pharmacy student? Would you recommend that they consider a career in pharmacy compounding? I think every pharmacy student should look at pharmacy compounding as a career option. Pharmacy compounding really encapsulates all that pharmacy has to offer and so many opportunities to use their pharmaceutical sciences background. Compounders support prescribers in a clinical setting by advising on unique strengths and dosage forms. We work under a collaborative practice arrangement to modify therapy, tailored to each patient’s unique needs. Compounders deep dive into pharmaceutical science by understanding the kinetics and dynamics of drugs, the chemical structures and properties of the medications, and math and calculations equations that support the compounded preparation. All these factors are used to formulate the best pharmaceutical compound which is then used for a patient’s specific condition. It’s a constant challenge, but we love it!

resources, education, and drug knowledge. Pharmacists can give patients an unbiased view of which supplements or medications can support healthy immune systems and aid in the fight against coronaviruses and other infectious diseases. And although the data isn’t perfect, it is adequate for a pharmacist to offer recommendations based upon their professional judgment. There is quite a bit of inaccurate intel floating around the internet and social media platforms. Unfortunately, patients act upon that inaccurate information, resulting in unsuccessful treatment plans or even possible harm to themselves. As pharmacists, we’re accustomed to being worthy stewards of drug information, and it’s our job to advise the healthcare community about the appropriate therapies available to help them manage patients.

Q. Do you think the future is bright for pharmacy compounding? Absolutely! I think one of the most intriguing things about our profession is that we can constantly reinvent ourselves to fulfill our

Dr. Shawn Hodges graduated with a Doctorate in Pharmacy from Texas Tech University, School of Pharmacy, in 2001. Upon graduation, he was awarded a one-year fellowship program with Eli Lilly, focusing on the drug approval process. Upon completion of the fellowship, Shawn practiced pharmacy in a community setting and worked as a regional director for the nation’s largest hospice pharmacy provider. Currently, he is a pharmacist and owner of Innovation Compounding, a compounding-only pharmacy located just outside of Atlanta that employs 39 pharmacy team members. Shawn also consults both

practitioners and their patients on men’s health, women’s health, and pain management. Also, Shawn serves as the president for the Alliance for Pharmacy Compounding’s (APC; formerly IACP). On January 1, 2020, Shawn assumed the presidency position. APC is a non-profit, advocacy/regulatory heavy organization that represents thousands of pharmacy compounders. On a personal note, Shawn, his wife, and daughter reside in Marietta, GA. They enjoy rafting, running, riding their horse Pockets, and spending time on the trails of Kennesaw Mountain.

hen Preveon Specialty opened its doors, their primary patient-base was in the immediate area— most less than a two-hour drive away. They decided that instead of shipping via courier, they would have their own drivers hand-deliver specialty medications to patients in close proximity. For those patients who lived outside the region and required medications to be shipped, Preveon knew they wanted to include a temperature indicator to ensure that refrigerated medications stayed within a safe temperature range during transit and alert patients if their medication was exposed to temperatures that could negatively affect its effectiveness. That’s when they started researching and found Temptime’s LIMITmarker® visual temperature indicator. They quickly learned that all their patients wanted the same assurances and peace of mind the LIMITmarker provides— and today include the temperature indicator with all of their heat-sensitive medications, regardless of how it’s delivered.

Business Challenge In April of 2017, Preveon Health, a care management company based in California, launched Preveon Specialty— their specialty pharmacy division that dispenses medications to treat a range of inflammatory conditions. While they are licensed in 23 states, most of their nearly 2,000 patients live close by in the Inland Empire region of Southern California, including Riverside and San Bernardino counties as well as parts of Los Angeles. This, says Jefmar Dickey, Business Manager for the specialty pharmacy, is one of the things that sets them apart. “The majority of our medications require refrigeration and need to stay within a certain temperature range,” he explains. “The fact that we can have our own drivers hand-deliver medications right to a patient’s door is something a lot of large specialty pharmacies just can’t offer.” Not only does this mean they can deliver medications quickly, decreasing the risk of temperature excursions, it also adds a much appreciated personal touch. “Rheumatoid arthritis, ulcerative colitis, multiple sclerosis and many other inflammatory diseases are chronic,” says Jefmar. “So

patients receive regular deliveries, usually monthly, and get to know the drivers personally over time.” Preveon did have a number of patients in other states and areas of California, however, so some medications would be sent using a package delivery service like FedEx or UPS. As a result, Jefmar began to analyze and document their entire cold chain process—both to get an understanding of how to most efficiently ship medications out of the local vicinity as well as for URAC and ACHC accreditation.

Solution Based on industry best practices, Jefmar knew they would need some sort of temperature indicator for the medications they were shipping out. He had seen a few providers at Asembia’s annual conference the year before, so he started doing some research. “When I reached out to Temptime, they answered me right away,” he says. “They really broke the process down for me—even steering me in the direction of a more cost-effective, industry-standard solution over the one I’d originally thought we needed.” Preveon started including Temptime’s LIMITmarker heat indicators with their FedEx shipments. These single-use indicators are applied directly to the pharmacy’s heatsensitive medications and give patients an easy-to-read irreversible visual cue that lets them know if temperatures have gone above a certain threshold for an extended period of time, which may have caused damage to the medication. “With LIMITmarker, it’s easy for patients to confirm that their medications have stayed in the right temperature range,” explains Jefmar. “And even if they aren’t sure, we can tell them right away just by having them answer a couple of simple questions about the color of the indicator.” It wasn’t long before Preveon realized that they should be including a LIMITmarker on all temperature-sensitive medications—even those delivered by their drivers. “When we started hand delivering our specialty medications, we found patients were unfamiliar with the process,” says Jefmar. “They Zebra Technologies | Temptime

were used to getting medications shipped to them via FedEx or UPS and expected some sort of indicator in the package.” So even though medications came directly from the pharmacy and included icepacks, patients were still concerned about temperature exposure. “We didn’t have any way to validate the efficacy of the medications for them,” he says. This led to an increase in patient calls, expensive medication replacements and, in some cases, even insurance claims that typically required extra paperwork from the pharmacy. As a result, Preveon found themselves discarding and replacing medications they couldn’t confirm to be viable even though they might have been. That’s why within three months of launching the specialty pharmacy division, they began including a LIMITmarker with all their heat-sensitive medications, even if transit time was as little as 30 minutes. They also trained all their drivers to explain the temperature indicators to patients, even though they are extremely simple to interpret.

The Result First and foremost, Jefmar says, including the LIMITmarkers in all their deliveries gave patients peace of mind. “Now patients can easily see that a medication has stayed in range,” he says. “It’s also helped our pharmacists confirm whether or not medications need to be replaced—saving us money on unnecessary replacements in the long term.” An unexpected advantage was that the decision to put LIMITmarkers on all their temperature-sensitive medications actually made workflows easier. “If the medication needs to be refrigerated, it gets a LIMITmarker applied to it as soon as it comes in,” Jefmar explains. “We no longer have to separate medications that are being shipped out from those scheduled for hand delivery.” Aside from efficiency and cost savings, having the temperature indicators in place is also an important part of the accreditation process. “Both URAC and ACHC want to see how we track medications and what our quality assurance processes are,” he says.

CASE STUDY Taking Temperature Monitoring One Step Further Preveon turned to Temptime to help with package performance qualification (PPQ) testing with their packaging as well. “For URAC in particular, we need to be able to validate that our packouts maintain the stability of medications in extreme conditions—both when delivered by our drivers or shipped via delivery service,” says Jefmar. Locally they can see temperatures as high as 110ºF. However, their packouts also need to perform in the colder climates of other states they ship to. For their PPQ testing, the pharmacy chose Temptime’s M-300, a Bluetooth®-enabled mobile sensor and datalogger. Using the mobile app, Preveon was able to monitor temperature readings and geolocation data via smartphone and tablet. In addition to viewing information on the app, data is also stored in the cloud, which was used to generate reports for accreditation compliance. During their first round of PPQ testing, the pharmacy found that some of their medications were getting too cold. “The results helped us adjust our packouts for more accuracy, and also gain more clarity on whether we could send certain medications via two-day delivery or overnight,” Jefmar says. Preveon successfully achieved both their URAC and ACHC accreditations.

Account Management: Our business model is to evaluate a pharmacy’s

route and STAT work, and provide dedicated drivers, management, and dispatch staff to create a ground-up solution for our partner pharmacy. We operate with this model because we understand that every pharmacy is different, and requires detailed attention for our services to be truly effective.

Driver Requirements: In addition to the interview process, all drivers

must complete a drug test and a background check. These are updated every six months. All drivers have files that are continuously monitored to ensure all documentation is in compliance. All drivers will be required to be in uniform. The uniform consists of a plain navy polo shirt, an ID badge with the driver’s name and picture for identification purposes, khakis or clean professional workpants, and close-toed shoes. If a pharmacy has any additional preferences, we are open to making alterations to better suit your needs.

Electronic Signature Capture: Given our focus of servicing long-term

care pharmacies, we are acutely aware of the importance of signature capture, and the effect that it has on both pharmacy operations and the audit process. We currently utilize Delivery Track with most of our client base and are open to learn and use any scanning process preferred by our pharmacy partners.

Technology: In addition to staying up to date on the latest pharmacy

scanning technology, we have GPS tracking of drivers, as well as an online portal for order entry and order updates.

Reporting: It is common practice for us to provide daily Delivery Track

reports detailing the scanning percentages from the previous night’s delivery. Additionally, we provide our weekly invoice that outlines each STAT delivery from the invoicing period, which can allow the pharmacy to see what facilities may be ordering an exorbitant number of STATs. Reports can be customized in any way to better serve the pharmacy.

Accessorial Charges: Due to our pharmacy-first mentality, we do not charge any accessorial charges such as waiting time, holiday charges, or after-hours fees. Fuel surcharge is only charged if fuel exceeds $4.00/ gallon. We believe in creating partnerships with pharmacies, and do not think the practice of adding extra charges is in line with that philosophy. GPO Affiliations: Mideast Delivery Solutions has partnered with

MHA and Net-Rx as a delivery provider. Member pharmacies receive discounted rates, as well as a rebate on their delivery spend.
