What is the NSF/ANSI 456 Vaccine Storage Standard?

Why is the NSF/ANSI 456 Vaccine Storage Standard being developed? Will all vaccine refrigerators and freezers need to meet the standard? Is it mandatory or voluntary? Do my current refrigerators and freezers meet the new standard?

The NSF/ANSI 456 vaccine storage standard seeks to define the construction and performance characteristics of refrigerators and freezers to further enhance vaccine cold storage. The standard was developed to normalize performance expectations and to allow the CDC a reference standard for user inquiries.

The NSF/ANSI 456 vaccine storage standard is specifically a voluntary standard, there is no timeline for adoption or governance of compliance. It will not be required by any governing body that vaccines be stored in NSF/ANSI 456 Certified units. The CDC Toolkit remains the gold standard for vaccine storage governance. Current purpose-built systems may meet the requirements. A central goal of the standard is to delineate acceptable performance from performance typical in “dorm” style and home use refrigerators and freezers.

What are the product features and performance requirements included in the new standard?

The new standard covers construction details, controller requirements and temperature variation. During testing, temperature is monitored using vaccine simulator ballast devices and must maintain specific temperature ranges at steady state and through short door openings.

Will American BioTech Supply products meet the standard?

Yes, American BioTech Supply has designed new compliant products and is in the process of testing specific cold storage units with an independent third-party agency.

What is American BioTech Supply doing to prepare for the new standard?

American BioTech Supply is a member of the NSF/ANSI 456 committee designing the new standard. Our input into the committee will help the new standard be adopted for use by the CDC and vaccine manufacturers and fill industry gaps in performance.

What began seven years ago as a grassroots effort with one technician working in partnership with the PharmD has grown to include three full-time technicians and three full-time pharmacists. Still, the specialty pharmacy’s mission has remained the same — to enhance care and improve the overall health and wellbeing of their patients.

What is the NSF/ANSI 456 Vaccine Storage Standard?

Why is the NSF/ANSI 456 Vaccine Storage Standard being developed? Will all vaccine refrigerators and freezers need to meet the standard? Is it mandatory or voluntary? Do my current refrigerators and freezers meet the new standard?

The NSF/ANSI 456 vaccine storage standard seeks to define the construction and performance characteristics of refrigerators and freezers to further enhance vaccine cold storage. The standard was developed to normalize performance expectations and to allow the CDC a reference standard for user inquiries.

The NSF/ANSI 456 vaccine storage standard is specifically a voluntary standard, there is no timeline for adoption or governance of compliance. It will not be required by any governing body that vaccines be stored in NSF/ANSI 456 Certified units. The CDC Toolkit remains the gold standard for vaccine storage governance. Current purpose-built systems may meet the requirements. A central goal of the standard is to delineate acceptable performance from performance typical in “dorm” style and home use refrigerators and freezers.

What are the product features and performance requirements included in the new standard?

The new standard covers construction details, controller requirements and temperature variation. During testing, temperature is monitored using vaccine simulator ballast devices and must maintain specific temperature ranges at steady state and through short door openings.

Will American BioTech Supply products meet the standard?

It is important to note that this new NSF/ANSI 456 vaccine storage standard does not necessarily meet specific “vaccine manufacturers” storage specifications. INTRODUCTION Headquartered in Kinston, North Carolina, Neil Medical Group is a trusted pharmacy services partner for long-term/post-acute care and assisted living providers across the southeast. Neil Medical, founded in 1984, distributes medical equipment and supplies, drugs and surgical products, as well as providing pharmacy, therapy, and consulting services. As a long-term care pharmacy, Neil Medical functions as a distribution warehouse, while also focusing on successful patient outcomes. In order to develop a strong customer base, Neil Medical continuously strives to improve accuracy, efficiency, and productivity, while also focusing on cost containment. “As a high-volume pharmacy, we needed to evolve and move away from the outdated punch card system we had been using for more than 30 years, not an easy change,” explains Ross Brickley, vice president of pharmacy services at Neil Medical. “Our goal was to improve pharmacy workflow, increase productivity, and provide exceptional service to our customers; but we needed a partner.” Ultimately, Neil Medical selected Noritsu as their partner. “Noritsu’s automated pharmaceutical packaging technology not only allowed us to efficiently automate drug packaging, but provided us with the ability to ensure accuracy. CAS E S TU DY INTEGRATING AUTOMATION FROM SCRIPT TO PATIENT ADMINISTRATION Because pharmacy workflow impacts every element of a pharmacy, Noritsu began by conducting a detailed workflow analysis. They examined how pharmacists and staff spend their time, how quickly facilities get access to their medications, how much space and staff are required to accomplish each task, the complexity of patient needs, and the accuracy of medication dispensing and administration. To maximize Neil Medical’s pharmacy workflow, the Noritsu team suggested site visits and phone calls with existing Noritsu customers to review essential technology and implementation options. The first recommendation was a Xana 4001 strip pouch packager, which would provide improvements in speed, accuracy, and safety while reducing errors in pharmacy production. Brickley confirmed his team’s experiences, “strip pouch packaging has ensured that patients have the correct dosage, making the administration of meds much easier for staff, and improving medication compliance.” The second recommendation was an MDM-3 medication detection machine. Verifying medications can be very tedious, but it’s essential. Medication detection machines save valuable time by helping to identify pill pouches that must receive extra scrutiny ultimately ensuring confidence and accuracy in every dispense. By automating the medication detection into a scan plus review with verification while automatically adding the cutting and sorting into one system, Neil Medical was able to improve accuracy at a cost savings versus 100% manual review by PharmD’s and extra pharm tech labor. In the end, Neil Medical has implemented the Noritsu solution across four pharmacies to date, purchasing five Xana 4001 pouch packagers and four MDM-3 medication detection systems.

POSITIVE OUTCOMES The results have been quite impressive for Neil Medical. “We are now at an 80% conversion rate,” explains Brickley. “With four operating pharmacies, this has been a remarkably fast conversion, especially considering the two-year process took place during a global pandemic.” In addition, Neil Medical has already seen a notable 20 to 25% improvement in medication dispensing accuracy. “The strip pouch packaging system has not only improved our pharmacy efficiency and safety, but the ability to tie into our eMar [electronic medication administration record] system creates less steps in the administration process,” explains Brickley. Brickley indicated that he worked closely with Noritsu to ensure eMar integration for his facilities. “eMar integration was a key advantage of Noritsu’s NexusRx software,

Performing the Test

The Pyrosate® Kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.

Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Compliance packaging may seem like a relatively new concept due to recent attention, but this form of packaging has been used for decades. From strip, to blister or “bingo” card packaging, to the more basic type seen in home daily planners patients purchase on their own, the concept is the same; to breakdown complex daily pill burden regimens into separate neat and convenient dosing “buckets” for each time of the day. This in-turn helps patients understand when to take specific medications, and makes it easier to carry needed doses for a specific period of time such as on vacation. Providers, nursing homes, and long-term care centers also favor this type of packaging due to its simplicity and safety. When considering automated solutions or options for your pharmacy and patients, there are a few items you may want to understand.

Q. What are some considerations for compliance packaging you should think about when promoting this business to patients/providers and clients? Irrespective of the automation solution, having solid key performance indicators (KPIs) are critical in showing a clinical benefit and outcome to patients leveraging this service intervention option. Using a recognized clinical standard quality metric of compliance such as Proportion of Days Covered (PDC) is of value in general. In fact, PDC is of exceptional value for patients with chronic conditions such as diabetes and hypertension. Tracking the PDC in the instance where patients have specific chronic conditions, along with an intentional continuous effort by your pharmacy team to move patients into and remain within the top-tier percentage, will resonate well with the interested audience for clinical effectiveness. This strategy helps meet Centers for Medicare and Medicaid Services (CMS) rating standards and alleviates performance adjustment fees.

Q. Can the vendor assist with an ROI analysis

that assesses the upfront capital investment of an automated device/solution and the resulting organic growth of patients, in comparison to your current specific manual process steps?

The vendor should be able to assist with a labor analysis of when your capital investment will “break-even” after moving away from manual labor (a pharmacist and/or technician-driven process) to automating certain filling steps such as pharmacist check, using the acquired device. The vendor may also compare all dispensing forms and the relative costs associated with each one, as well as provide data that is suggestive of what this new service will attract in new patients leading to organic growth and revenue. It’s advisable to conduct adequate research on your own before engaging a vendor and don’t forget to include the cost of supplies and packaging. 48

Q. Considering only a finite number of drugs can be stocked within a device with a seemingly infinite number of combination dosing regimens, how does the automated system’s process or device manage drugs that are not stocked within the device, yet still need to be accurately incorporated within a finished compliance package dosing regimen packet for a patient? There should be a “semi” automated process where drugs that are not stocked within the device can be accurately incorporated into the final dosing regimen packet. Solutions such as barcoding of the drug into a barcoded tray or similar compatible process to ensure that drug being added semi-manually is accurately dispensed into the packet/blister card at the correct dosing interval according to the provider’s order. This process should be auditable and have a sound pedigree of the drug’s origin and its final dispensing, should the need arise for detailed tracking.

Q. What pharmacy management systems do the automated device and accompanying device’s software program interface with? How integrated is the device and software with my pharmacy system? Ideally, you want to keep the patient’s profile, receipt of e-prescriptions, prescription transcription, processing, adjudication, error resolution, A/R, etc. housed within your existing pharmacy management system with non-adherence-packaging patients for ease of patient management and data reporting. Once orders are ready for filling, completed medication adherence packaging prescriptions are then either held in a specific compliance packaging queue (for batching or future processing if you wish, based on your desired workflow) or sent to the device’s software program for immediate packaging. The level of single or bi-directional interface may vary, with the latter being the “gold standard” for posting back


Q. In addition to the capital cost for the automated device, what other peripherals are recommended from the manufacturer based on your desired workflow to increase accuracy and efficiency, and what are their costs? The largest cost for a compliance packaging solution will certainly be associated with the base automated device unit. The cost of these items adds up, so be cognizant of what is a need versus a nice to have.

Ask whether there are additional costs involved should a downtime event happen after weekday hours or during the weekend. A less than twenty-four on-site response time is ideal when the need arises. Software updates should be standard with no additional costs for basic updates, while upgrades or enhancements may come at a cost. So, gain a good understanding what these costs are upfront. Finally, knowing how much time will need to be dedicated by a member of your team to perform periodic maintenance-specific tasks must be taken into account. You may discover that these tasks may use up as much time in labor hours as a manual process.

ltru Specialty Pharmacy is a part of Altru Health System, which serves people in the city of Grand Forks as well as much of North Dakota and extending into Minnesota. What began seven years ago as a grassroots effort with one technician working in partnership with the PharmD has grown to include three full-time technicians and three full-time pharmacists. Still, the specialty pharmacy’s mission has remained the same — to enhance care and improve the overall health and wellbeing of their patients.

Package Performance Qualification The specialty pharmacy did its first round of testing in 2019. “We started with our refrigerated packouts, which need to be kept between 2ºC and 8ºC. We tested in cold weather to make sure packages were staying in range for the amount of time we anticipated shipments to take,” says Rosemary. In Phase 1, Altru included two different wireless sensors. An M-300 measured temperatures inside the medication packaging at regular intervals and an S-400 monitored the external ambient temperature in the outer box. Test packages were sent to Temptime, who analyzed the data captured. “We learned a lot in that initial evaluation of our packaging,” Rosemary says. Temptime gave Altru guidance on coolers and other packing materials best for cold chain shipments to help improve their packout design. “We made a few tweaks with the intent of giving patients even more confidence in the medications we deliver — and Temptime was a great help through the entire process.” Altru then moved on to Phase 2 for further testing of the passing Phase 1 package design. In Phase 2, test packages were prepared in the same way, but this time shipped to various geographies using different modes of transportation such as courier and FedEx. Again, Temptime collected the time-temperature data, using the information to prepare a PPQ Report that validates the qualification of the packouts.

Aside from ensuring packages performed the way they needed them to, Rosemary says one of the most significant benefits has been instituting best practices throughout the shipping processes. “Now every shipment of refrigerated medication is packed the same way,” she says. “In the past — there were no specific guidelines. So one person might include one icepack in a shipment where another might put in three, which could cause medication to freeze.”

Altru is currently finishing its second round of PPQ testing. “We’re in the process of testing controlled room temperature (CRT) and refrigerated shipments during the summer to ensure packouts are keeping medication from getting too hot,” says Rosemary. “With all we learned in our previous testing, the process this time around was much smoother and we successfully passed Phase 1 on the first try.”

In the end, she says, PPQ testing just delivers peace of mind – for the pharmacy as well as for patients.

Results demonstrate that the Altru Retail Pharmacy shipping boxes are capable of providing the desired temperature environment for shipment of temperature sensitive products, maintaining temperature within 2°C to 8°C for a minimum of 33-hours. All recorded temperatures inside the shipping containers were within the lower limit of 2°C and an upper limit of 8°C for up to 33-hours.

The specialty pharmacy was also pleasantly surprised to find that the PPQ testing would help them with their URAC accreditation, which they began working toward in October of 2019. “A lot of URAC’s requirements are centered on providing documented proof that medication temperatures are monitored and maintained, and everything we’re already doing will apply.”

Next Steps Altru’s next step will be to test CRT packages during the colder seasons and retest refrigerated packouts. “Regular, seasonal PPQ Testing is just so important,” Rosemary says. “Our patients trust us — and we need to continually earn that trust with every single shipment that goes out.” More than that, she adds, most of the drugs they deliver — such as cancer medications — are critical to patients. “They have to arrive on time and in good condition, it’s imperative.”

INTRODUCTION In February 1987, Michael Weinstein and several other activists founded the AIDS Hospice Committee in Los Angeles. Their goal: to provide medical and palliative care to individuals living through the final stages of AIDS.

As the management of HIV and AIDS improved, the Committee shifted its focus to helping individuals live with the disease — and became the AIDS Healthcare Foundation (AHF) in 1990. About two decades later, AHF acquired the MOMS Pharmacy chain and rebranded the chain as AHF Pharmacy. Today, AHF is the largest global AIDS organization, providing cutting-edge medicine and advocacy to more than 1.6 million people in 45 countries. The organization’s mission is to rid the world of AIDS via a network of thrift stores, strategic partnerships, and pharmacies. STREAMLINING DELIVERY Promoting business growth is the main function of Epicor’s Eagle N Series. “The management solution works with an organization’s pharmacy management system (PMS) to provide point of sale, inventory, purchasing, accounting, and marketing capabilities that independent pharmacies need in order to maximize business performance,” explains Zachary Pfeil, industry marketing principal at Epicor. “The system optimizes pharmacy practices and is extremely easy to learn and navigate.” One of the most critical pieces of AHF’s Pharmacy operation is prescription delivery. “When we first began implementing our new delivery system, we quickly realized that using laptops was not a good solution for us because the VPN connection process was too challenging and time consuming for drivers,” says Zellers. To solve the problem, Epicor proposed the Dispatch and Delivery Management System (DDMS). “Originally established for flower and lumber delivery — and designed to eliminate all manual processes — this technology was exactly what we needed,” explains Zellers. “Epicor helped us adapt DDMS to the pharmacy world. The solution not only provides convenience and accuracy, but it greatly enhances customer satisfaction. In the new retail world we live in, customers expect more, so easy delivery, tracking, and accounting/billing are essential to creating a successful patient experience.” DDMS displays real-time order status and enables drivers to use smart phones to complete tasks, such as sending out emails when medications are out for delivery, taking photos of the delivery site, and capturing signatures. The system even reminds customers to refrigerate their medications after delivery. These new capabilities create enhanced accountability and trackability in their delivery process, says Zellers, which is particularly critical for insurance purposes due to the often high cost of these medications. The barcode scanning technology automates the entire process and moves the order status through the dispatch and delivery process. Additionally, Epicor’s GPS tracker is an add-on feature that provides a map of the service area, which ensures that drivers are taking the fastest route possible. “During the height of the COVID pandemic, many of our facilities shut down. As a result, the number of our deliveries — compared to in-person pick-ups — increased drastically,” says Zellers. “What would have been a chaotic situation was made manageable with DDMS. Because the system is so easy to use, we were able to pivot and focus our efforts on delivery. In fact, many staff members became drivers with the help of the DDMS system.” In addition to an improved delivery system, AHF was looking to enhance its pharmacy workflow. Epicor suggested INET, an online portal for customers to manage their accounts via online payment systems. “Before INET, we generated a large number of invoices and mailed them out to customers. Now this process is completely automated, saving us a lot of time and money on labor and postage costs,” says Zellers. “While we were sending out thousands of monthly statements, we are now down to only about 300 invoices each month. That’s a huge improvement.”
