By Angela Caswell-Monack, DO, Board Certified OB/GYN I was inspired to write about the IUD (intrauterine device) after a visit with a new patient recently. She was referred to me for surgical removal of an IUD that was placed in 1971. It made me think reverently about how far healthcare for women has come. When I was a young woman, considering what career I might want, I decided to go to medical school specifically to become an OBGYN. I was inspired by feminists and the civil rights movement, and I wanted to be a part of the progress that women have made and continue to make. When I was a medical student, I was told by an ancient and wise doctor that the first IUD was a pebble that was placed in the uterus of a camel by nomads in the Middle East many, many years ago. Of course, I believed it since it made sense that a foreign body such as a pebble could cause enough irritation within the uterus to prevent camel sperm from getting through. However, after being a practicing physician for several years, I now think that the pebble story might have been a myth. However, as a story goes, it proves the point that the first methods of preventing pregnancy helped build the world of today’s modern IUD. An IUD or intrauterine device, is a device that is placed within a woman’s uterus to prevent pregnancy. The first known use of the IUD was in 1909 when Dr. Richard Richter of Poland claimed success in preventing pregnancy using an intrauterine ring. Over the decades, there were many iterations of the IUD. There were silver versions and glass versions and 14k gold versions. In the past, these methods were generally reserved for only the wealthiest women.
In the early 1970s, the Boston Women’s Health Collective was starting to publish “Our Bodies Ourselves” and radical changes were happening for women in the U.S. In 1971, the IUD known as the Dalkon shield (a beetle shaped IUD with a braided string) was produced and soon it was implicated in several deaths and resulted in more than 200,000 lawsuits. The issues with the Dalkon shield seriously hurt the reputation of IUDs as women questioned their safety. We still discuss the Dalkon shield with patients on a regular basis. Globally, IUDs continued to be popular and make big strides. But in the US, the birth control pill took over the contraceptive world, and cervical caps, diaphragms and sponges were manufactured….all less effective options. Around the same time, a new type of IUD was created, taking on the modern T-shaped design we still see today. Today, more and more women are interested in the IUD because it provides an effective, safe form of birth control. The IUD is now the most common form of contraceptive in the world! There are two main types of IUDs—progestin IUDs and copper IUDs. The progestin IUDs (such as Mirena, Liletta, Skyla, Kyleena) contain a small amount of progesterone-type hormone that keeps the lining inside of the uterus thin and gives lighter or nonexistent periods. In addition to contraception, it can also be a treatment for heavy periods. The copper IUD (Paragard) is completely non-hormonal and works as an excellent contraception for women who cannot have hormonal contraception for medical reasons or for those who choose to avoid hormones. The copper IUD can also be inserted as emergency contraception for up to 5 days after unprotected intercourse. An IUD is a long acting reversible contraceptive. So, you can “set it and forget it.” IUDs are approved

to be used for three to 12 years (depending on which IUD is chosen) and women of all ages are candidates, including women who have never been pregnant. They are very effective (better than 99% effective) for birth control and have many advantages. An IUD is long acting, is rapidly reversible, does not require regular compliance, has few side effects, is private and discrete, does not interfere with spontaneity of sex, is cost effective, and allows a women to be in control of fertility.

Delaware Eye Optical With September around the corner and kids preparing to return to school buying clothes, backpacks, paper and pens, parents should consider vision as part of this preparation. Eye exams are routine for many children whose visual problems were caught early and corrected with eyeglasses or contact lenses depending on their age. Early in children’s eyecare frames are designed with safety, comfort and durability in mind. Preschool, kindergarten and grade school ages often find frames that are colorful, flexible and in shapes that fit the typically narrow pupillary distances as well having pads for the bridge of noses that are less developed. Today’s children are more aware of style and brands they are exposed to at an early age such as Fisher Price, Adidas, LINDBERG Kids, Solo Bambini, Rec Specs, and dilli dalli to name a few. Even though parents are responsible for paying the bill and making sure their children receive proper medical care, in this case eyecare, it is very important for the child to be part of the frame selection process. This will go a long way toward the child wearing and taking care of their new eyewear
The prescription is always the main consideration when choosing a child’s eyewear. When selecting frames the optician should be able to read the doctor’s prescription and have suggestions on frame styles that best fit the child but also lenses that provide safe, clear vision. Lenses, depending on the prescription, can be thicker either in the center (plus lenses) or the outer edge (minus lenses). Frame selection can cosmetically help children feel more comfortable in these cases. Frames with wider temples can hide some of the edge thickness on a minus prescription. Those with the plus lenses can benefit from having a frame where the pupil fits as close to the geometric center of the frame as possible. The lens with less decentration (moving optical center) creates less thickness and better peripheral vision. There are many lens materials in the market place and each have different aspects from safety, to cosmetic, weight, clarity or specific applications that can be added. For children polycarbonate has been the standard for many years due to its characteristics of light weight, absorbing UV, thinness and primarily safety. It is a very durable impact resistant material long prescribed for children which had its origin in industrial eyewear. Trivex or Phoenix (Hoya’s brand) are more recent materials that qualifies for children

18 and under, we feel is better than polycarbonate. The two materials have similar characteristics but Trivex or Phoenix is more scratch resistant and has a higher abbe value so vision is slightly better. Obviously safety is primary for children’s eyewear along with the prescription and fashion. Eyewear today can help them through the early years when development is so critical. Sports eyewear is critical for children involved in almost any sport. Polycarbonate and Trivex can provide impact resistance from balls or flying elbows on a basketball court for example. Liberty Optical provides an array of sport frames cushioned at the bridge and temples with wide sweeping lenses for great peripheral vision. This area of eyecare has
grown and offers much more variety than in past years. Team colors, different shape designs and various temple concepts allow wearers to customize the sports eyewear depending on what they enjoy from biking, baseball, basketball, motorcycling, shooting, to fishing and many others with eyewear that have both safety and performance in mind. We begin fitting children with contact lenses around the age of 12 or 13 depending on the child’s personal habits and maturity. Parents are usually involved in the decision process for obvious reasons. Contact lenses require more discipline and responsibility on the child’s part due to the health implications if directions are not followed. For those who are able to wear them successfully
they offer an alternative and can help those whose prescriptions are burdensome due to the lens power. We offer many different alternatives from pre-school age all the way through college age for children looking for fashion or just function. Over the years we have observed an improvement through technology both in lenses and frames providing children vision on a par with adults and their needs Obviously there are many areas within children’s eyecare I haven’t touched on. BREAST CANCER: Prevention Saves Lives By: Lisa Schirtzinger, Cancer Screening Nurse Navigator, Nanticoke Health Services Breast cancer is something commonly associated with pink and with October. But, breast cancer is serious business. The good news is, early detection saves lives. When detected early, breast cancer has over a 95 percent survivability rate. But, you must detect it early and that’s why it is important to be screened regularly when you are over 40 years of age. Breast cancer starts when cells in the breast begin to grow out of control. These cells usually form what is called a tumor that can often be seen on an x-ray or felt as a lump. The tumor is malignant (cancer) if the cells invade surrounding tissues or spread (metastasize) to distant areas of the body. Breast cancer occurs mostly in women. However male breast cancer is not uncommon. Breast cancers can start in different parts of the breast; however, most begin in the ducts that carry milk to the nipple (ductal cancers) and some start in the glands which produce breast milk (lobular cancers). Breast cancer spreads when cancer cells enter the blood or lymph system and are carried to other parts of the body. If cancer cells have spread to the lymph nodes, there is a higher chance the cells could have traveled through the lymph system and metastasized (spread) to other parts of the body. The more lymph nodes with breast cancer cells, the more likely it is the cancer may be found in other organs. DIAGNOSIS The first sign of breast cancer often is a breast lump or an abnormal mammogram. Breast cancer stages range from early, curable breast cancer, to metastatic breast cancer, with a variety of breast cancer treatments.

SYMPTOMS Mammograms can detect breast cancer early, possibly before it has spread. No two people are exactly alike, so no two breast cancers have exactly the same symptoms. Breast cancer symptoms vary widely — from lumps to swelling to skin changes. Many breast cancers have no obvious symptoms at all. A lump may be too small to feel or cause unusual changes you can notice on your own. Often, an abnormal area turns up on a screening mammogram, which leads to further testing. According to the American Cancer Society, any of the following unusual changes in the breast could be a symptom of breast cancer: • swelling of all or part of the breast • skin irritation or dimpling • breast pain • nipple pain or the nipple turning inward • redness, scaliness, or thickening of nipple or breast skin • a nipple discharge other than breast milk • a lump in the underarm area These changes can also be signs of less serious conditions that are not cancerous, such as an infection or a cyst. But, it is important to have any breast changes checked out promptly by a doctor.
The Importance of Cancer Screenings in Delaware: From 2011-2015, there were 3,994 new cases of breast cancer (or 134 cases for every 100,000 women). Over those same years, 185 women died of breast cancer (22 for every 100,000 women in Sussex County). Screening for Life (SFL) is a program provided by the Delaware Health and Social Service Department to encourage residents to schedule cancer screenings such as mammograms. If cancer is detected, SFL can also assist in enrolling you in a program that covers costs of your cancer treatment. Nanticoke Health Services Care Coordinators and the Cancer Screening Nurse Navigator provide education and can assist individuals in obtaining a Screening for Life application. Nanticoke is also proud to be a recipient of Susan G. Komen® as well the Fund for Women grants to provide qualified women in Sussex County financial assistance to obtain breast cancer screenings and treatment. CERVICAL CANCER By: Lisa Schirtzinger, Cancer Screening Nurse Navigator, Nanticoke Health Services The American Cancer Society estimates 12,000 women are diagnosed with cervical cancer each year and 4,000 die of the disease. It is the third most common diagnosed gynecologic cancers among women 20 to 39 years old. However cervical cancer is preventable with screenings and vaccinations. The majority of cervical cancers are caused by the human papillomavirus (HPV) infection. HPVs are a group of more than 200 related viruses. The virus can be spread through sexual activity including oral sex. HPV causes cancer by infecting the epithelial cells which cover the inside and outside surfaces of the body. A healthy immune system can recognize and eliminate these infected cells. However when they are not destroyed a persistent infection results, causing infected cells to continue to grow. Prevention and Cervical Screening Studies have shown that HPV vaccines prevent HPV infection. The Centers for Disease Control and Prevention recommendations for
vaccination are as follows: • Routine vaccination for males and females 11-12 years of age with 2 doses of either HPV2 or HPV4. • Catch-up vaccines for males through age 21 and for females through age 26, if they were not vaccinated when they were younger. • For any man who has sex with another man through age 26. • For men and women with compromised immune systems including those living with HIV/ AIDS through age 26 if not fully vaccinated when they were younger. The vaccinations are not effective at treating established HPV infections or disease caused by HPV. Correct and consistent condom use is also recommended to prevent HPV infection. Screening for Life, a program provided by the Delaware Health and Social Services Department, encourages Delawareans to schedule cancer screenings. “Broken Heart Syndrome” Is a “broken heart” a true medical condition or is it just a figure of speech to describe someone’s emotional state after some great personal loss or stressful situation? It may come as a surprise to many people but the answer is both. This condition is also known as “stress-induced cardiomyopathy”, a rapid and severe heart muscle weakness, typically as a consequence of sudden emotional or physical stress: unexpected illness, injury or loss of a loved one. It can also be from a severe medical condition, surgery, an accident, intense fear, domestic violence, severe pain, financial loss, even public speaking or a surprise party. The exact cause is unknown, but it is clear that most of the affected individuals are postmenopausal women between the ages of 58 and 75 years, in about 90 percent of cases. The symptoms are very similar to those of a heart attack, with chest pain or pressure and shortness of breath being the most frequent ones. It is not completely understood why this condition affects mainly women, but most investigators believe that after menopause the decline in estrogen levels renders patients more susceptible.
It was first observed in Japanese women in the early 1990’s and the name of takotsubo cardiomyopathy was used originally. This name describes the characteristic appearance of the left ventricle (heart’s main pumping chamber) as seen in cardiac studies that resembles the traditional Japanese octopus trap. There is ample evidence to suggest that the massive release of certain hormones in the bloodstream, such as adrenaline and noradrenaline, as a natural response to stress, under certain conditions causes the heart muscle cells to stop working properly, especially in the lower part and the “tip” of the left ventricle. Stress-induced cardiomyopathy can be serious medical condition because it involves the sudden onset of congestive heart failure due to the heart muscle weakness, low blood pressure and potential shock or life threatening heart rhythm problems; it is, however, rarely a fatal illness. Furthermore, it is almost always reversible with most patients recovering completely within 2 to 4 weeks. The main differences between a heart attack and “broken heart syndrome” is

that heart attacks occur as a consequence of blockages or blood clots in the coronary vessels (heart vessels that supply blood flow and oxygen/ nutrients) to the heart muscle cells that cause those cells to die or to suffer permanent damage. In the stress cardiomyopathy patients, the coronary vessels are open and the blood flow is usually normal. It is the “stunning” of the heart muscle cells by adrenaline and other hormones that creates the muscle weakness and this process is, for the most part reversible. Another interesting finding in this broken heart syndrome
is that it does not tend to recur in the same patient. In other words, once somebody experiences this syndrome, even if other stressful events come along in their lives, the same phenomenon doesn’t tend to happen again. Learning to cope better with life stresses can prevent it in the first place. The management of this medical condition is similar to the treatment of congestive heart failure with medications such as diuretics, betablockers and ace-inhibitors as well as aspirin and in some cases blood thinners on a temporary basis. So yes, it is quite possible
to have a “broken heart” literally, as a true medical illness that is brought about by a heightened physiological response to stress, especially in women. The good news is that in almost every case it is a temporary condition and full recovery is the expected outcome. There is no way to differentiate this situation from a heart attack, before hand. Therefore, it is very important to seek immediate medical attention for symptoms of sudden chest pain or pressure and shortness of breath. It might be the case that your heart doctor could tell you, in the end: “Just a broken heart, already on the mend”.

With time, age and gravity, our facial structures tend to lose some of the youthful volume it once had. The cherubic apples in our cheeks fall, leaving us with a drooping jaw line that becomes jowls, and deep lines form around the nose and mouth areas, known as the nasolabial folds. Fortunately there is a solution for these pesky issues of aging. Physicians can now treat their patients with an in-office procedure known as dermal filler injections. What are Dermal Fillers? Dermal fillers are medical products that are injected by a trained physician to fill in facial areas where volume has been lost, so that it is possible to lift those cheeks back up and fill in the deep lines around the nose or nasolabial folds, and the depressions around the lower mouth and chin areas known as the marionette lines. Treatments with dermal fillers are a wonderful way to restore a more youthful appearance. However, what has not been so wonderful is how long they lasted, which has been temporary, on average from 6 to 18 months, as the products are slowly reabsorbed and eliminated by the body. While caring for a loved one in the home, it is important to take care of oneself to avoid becoming burned out. It is important to notice the signs and symptoms of being overwhelmed. We understand that caring for a spouse, partner, friend, or family member can sometimes become difficult and overwhelming. Caring for someone who is chronically ill, a person with memory impairment, or a disabled person can potentially come at the expense of the caregiver’s quality of life. In addition to maintaining a healthy, active lifestyle outside of caregiving responsibilities, it is important for those caring for a loved one to learn ways to avoid health hazards and stay well-informed of any changes in their loved one’s condition. If you’re caring for an older adult, Always Best Care recommends these tips to help manage stress before caregiving leads to burnout. Being aware of some of the warning signs can help caregivers properly manage stress and protect themselves. ery few people are aware of how much they use their teeth on a daily basis. It is not until an individual begins to lose teeth and their oral health and quality of life declines that they begin to understand the impact of impaired oral function on their daily life. Patients with missing and/ or failing teeth suffer from malnutrition, reduced self esteem, and self confidence. They possess a high risk for developing oral conditions such as boney atrophy. Patients also suffer from periodontal disease, cavities, root canal infections, and serious bite problems called “bite collapse.” In addition to serious dental conditions, patients with severely failing dentitions can also be more at risk for serious medical conditions such as: cardiovascular problems, stroke and diabetes, complications if pregnant, with a high risk of preterm and low birth weight babies. There are several solutions for individuals with failing dentition. The typical sequence of therapy to address these complex dental conditions could require at least three surgeries. First, the teeth are extracted and the bone is allowed to heal. Patients will temporarily function in a traditional upper or lower denture, which at times impacts the patient’s diet choices. Next, the implants are placed and require a healing time of three to four months. A third surgery is done to expose the implants. The start to finish time is six to eight months. During this time, patients use removable dentures to function.

It is important to find a health care practitioner who is knowledgeable in treating UI. A thorough examination from your physician should be performed to rule out any medical conditions that may be causing your incontinence. Kegel exercises are designed to strengthen the pubococcygeus muscles located around the bladder opening. You can identify these muscles by attempting to stop and start your urine stream. Stopping your urine stream is only a way to identify the muscles used for Kegel exercises or to see if you are improving. Once you have learned how to contract the pelvic floor muscles, try doing Kegel exercises frequently throughout the day such as while stopped at a traffic light, or when talking on the phone. Hold the muscles contracted for about three seconds, 12 to 15 repetitions, resting for a few seconds between each exercise set. Do these at least three to six times a day. If you are having difficulties identifying these muscles, there are health care professionals that can assist you. Physical therapists who specialize in gynecological physical therapy can help people strengthen their pelvic floor muscles to help improve bladder control. If muscles are very weak, your physical therapist may use: EMG/biofeedback: The therapist places a sensor over the pelvic floor muscle that “reads” the activity in the muscle. A wire connects the sensor to a computer screen and the client learns how to perform Kegel exercises

All You Need to Know About Medical Marijuana … But Were Afraid to Ask! By Mark Lally, CEO, First State Compassion Center The subject of medical marijuana can stir up strong emotions among doctors, scientists, parents, policy makers and the public. I hope this feature will answer some of your questions and abolish many of the myths that create barriers to pain relief and improved quality of life. As CEO of Delaware’s first medical marijuana distribution center, I’ve been humbled to witness firsthand the incredible relief from suffering patients have experienced using medical marijuana, also known as medical cannabis, after receiving their doctor’s certification for its use. My background is in law enforcement. I have a very disciplined approach to any kind of substance use. Working with medical marijuana patients has taught me and our entire First State Compassion Center (FSCC) team the importance of compassion and taking a sound therapeutic approach to alleviating suffering.
7. Are there ways I can take medical marijuana without smoking it? Yes. FSCC has various ingestion options available for our patients including capsules, concentrates and vape alternatives. 8. Can medical marijuana cure my disease? Medical marijuana generally relieves symptoms of disease such as pain relief and loss of appetite, but it is not a cure. 9. How do I know that the product is safe? Has medical marijuana undergone the same standard, rigorous pharmaceutical testing as other drugs? All our medicines are meticulously tested for contaminants at FSCC prior to being evaluated in the State’s testing lab. 10. I have never used marijuana before. How will it make me feel? Will my health insurance cover medical marijuana? Most health insurance companies do not cover medical marijuana yet, but FSCC does have programs to assist patients who cannot afford their medicine.

CoolSculpting Non-Surgical Fat Removal non-surgical treatment for removal of fat from localized pockets of excess fat. This non-invasive procedure represents an alternative to surgical liposuction by using a novel cooling technology that permanently removes fat cells from treated areas.

How does CoolSculpting Work? The technology behind CoolSculpting was developed by Harvard scientists who noted that fat was destroyed by cold under certain conditions. CoolSculpting is a result of their efforts to harness and refine this application of cold to selectively remove unwanted fat deposits in a safe manner. During a treatment, your unwanted fatty area will be pressed and chilled to a temperature 12 degrees above freezing. At this temperature the fat within fat cells crystallizes. When the fat within the cells crystallizes, it damages and kills the fat cell. Skin and other tissue is not damaged as the water in other cells is not crystallized, only fat. The body will then eliminate the killed fat cells over time. The best news is that we do not make new fat cells. Fat cells merely get bigger or smaller as they store more fat or lose fat. So in essence the fat removal from CoolSculpting is permanent. And we have seen instances of patients who if they gain weight, the weight will not return to the treated areas. What are the most common areas to be treated? Many people on a weight loss program continue to have stubborn hard to lose areas, no matter how much dieting, sit ups or weight they lose. Such stubborn areas are typically around the abdomen, the love handles and inner thighs. Once you are over forty, losing fat in these areas is extremely difficult. These are
the areas that CoolSculpt can address. Basically if there is an area you can pinch several inches, that area can be treated and eliminated by CoolSculpt. Most of our patients are interested in losing fatty areas in the upper and lower abdomen, the muffin top, waist, brat fat, back fat, love handles, and “mommy pooches.” We can also treat arms, thighs and saddle bags.

Are there any side effects from CoolSculpting? The side effects from a CoolSculpting treatment are minimal. It is possible to have bruising after the treatment, which resolves in a week. It is common to have numbness or decreased sensitivity in the treated area lasting 1 to 2 weeks. In some instances patients will develop a burning tingling sensation 5 days after the treatment which can be controlled with a Lidoderm patch. There are virtually no long term negative side effects.

Respite Care Provides Much-Needed Short-Term Breaks
D
eciding to move a loved one into a skilled nursing or assisted living community is a big decision and often a hard one to make. Frequently, a caregiver will feel like their loved one doesn’t require quite enough care to warrant placing them in a community. And while keeping a loved one at home can be wonderful and rewarding, it can also be physically exhausting and emotionally draining. Most caregivers find the amount of time they spend providing care to be greatly more than they originally anticipated, which can lead to stress, depression, and even a weakened immune system. The receiver of the care can sometimes feel the burden and become ridden with guilt. When this is the case, respite care can offer all parties a much needed short-term break. Harrison House of Georgetown offers respite care in a calm and relaxing setting. Loved ones are cared for by skilled nurses and physicians, 24 hours a day. Maria Purnell chose Harrison House for her father when her mother needed a break. She stated, “Having two parents at home, both in their 90s, made it really hard for Mom to take care of Daddy, especially with his early dementia. It has been a big load off of Mom’s shoulders, not having to do the bathing, dressing, and making meals. I would recommend respite care to anyone in need of a break.” Respite care stays typically range from five to 14 days, but they can really be however long a caregiver needs to recoup and feel reenergized to provide adequate care. While in the care of Harrison House, a respite care resident receives all of the benefits provided to long-term residents. As soon as they are admitted, a team of highly skilled physicians and nurses develop a personal care plan based solely on their needs, which can include medication management and/or dementia care. They are free to socialize and interact with all other residents and participate in the daily activities, which can range from happy hours to concerts to field trips. Respite care residents are free to take advantage of the full-service

All the staff is exceptional.” Audrey Conner, another respite resident mirrored this sentiment. She stated, “The staff is absolutely great. This is my fifth time being at Harrison House and it has been a great experience all five times.” Respite care can also be used as a trial to see how a loved one will adapt to living in a skilled nursing community. Nancy Burgess stated, “I have had a very hard time with the thought of placing my mother in a nursing facility. I am so appreciative of the team of professionals at Harrison House who know exactly how to give professional care to the residents and help the families through the acclimation process of leaving their loved one […]. You allow family members to cease from being caregivers and go back to becoming daughter, son, aunt, uncle, niece, cousin or whomever we need to be to just love on them.”

Dr. Hudson is a ’95 Cape Henlopen High School graduate and continued her educational career at the University of North Carolina at Wilmington where she completed her Bachelor’s degree in Physical Education. While attending UNCW, Dr. Hudson suffered an injury playing softball and she sought treatment with a local chiropractor. After a few adjustments, Dr. Hudson was back on the field. Now being a true believer, she decided to further her education in the natural healing art. She attended Life University in Atlanta, GA for 3 years, and then transferred to Texas Chiropractic College in Houston. To clear the fog, Dr. Jeff Sklar, Medical Director of Chiropractor Services at a nationally renowned cancer treatment center, responded to some of the most common myths surrounding chiropractic care. Myth: Cancer patients should avoid chiropractors because they can break bones and spread the cancer. The research does not support such conclusions. In fact, a 2005 study, published in the journal, Seminars in Oncology Nursing, encouraged the use of chiropractic services for cancer patients, concluding that a review of written resources suggested it appeared to have benefits for relieving their pain and suffering. Your chiropractor will want to be kept up to date on your evolving condition, including the type and location of the cancer, current blood work, most recent imaging and the treatment regimen, in the interest of the delivery of care. We also know that this therapy may not be for every cancer patient. For example, chiropractic services may not be recommended when a bone metastasis is found or the patient’s platelet counts are low. Recommendations for chiropractic care should be made in close consultation with the patient’s oncologist. Myth: Chiropractic adjustments hurt.
Although practitioners vary in their degree of skill and methods of treatment, we offer a gentle, non-invasive style of adjusting with the goal to be pain-free. This approach may lead to patients experiencing quick and lasting relief from a number of musculoskeletal complaints common in cancer patients, including back and muscular pain and neuropathy. Myth: Once you go to a chiropractor, you must keep going for the rest of your life. Continuing chiropractic visits is not mandatory, and some patients stop seeing chiropractors after their pain or other side effects have subsided. Those who continue to receive regular chiropractic care often choose to keep up the treatments because they believe the adjustments contribute to an overall sense of health and well-being. Many patients have reported sleeping better, having fewer to no headaches and experiencing fewer sinus issues, fewer colds and a sense of improved energy and vitality. Because cancer and its treatment may cause a host of side effects and other complications, Dr. Sklar recommends that cancer patients interested in chiropractic care look for a chiropractor not only trained and experienced in working with cancer patients, but one who works closely with a team of other cancer experts all focused on that patient’s care plan. Sinus disease, including recurrent infection requiring antibiotic along with chronic congestion, pressure and post nasal drip year round, is the most common health problem in the United States; affecting 1 in 5 patients per year. Most sinusitis patients suffer facial pain, pressure, headache, nasal discharge, constant post nasal drip with discolored mucous and fatigue. Sinusitis has significant effects on a patient’s quality of life according to quality of life studies. Overall, the chronic, recurrent sinusitis patient’s quality of life is WORSE than that of patients with chronic heart failure, chronic obstructive pulmonary disease and Parkinson’s disease. Historically, chronic sinusitis patients were limited to two treatment options: continued medical therapy, including antibiotics and topical nasal steroids, or conventional surgery such as Endoscopic Sinus Surgery (ESS). This all changed approximately 8 years ago with the introduction of an innovative technology called Balloon Sinuplasty. The Balloon Sinuplasty devices, originally designed developed, and commercialized by Acclarent, utilize a small, flexible balloon catheter that is placed through the nose into the blocked sinus passageway. The balloon is then inflated to gently restructure and open the sinus passageway, restoring the normal sinus drainage and function. Since the beginning of 2011 Balloon Sinuplasty has been offered as an in-office procedure without the need for general anesthesia. This innovative office procedure typically allows patients to return to normal activity within 48 hours. In this position, Ms. Gardner manages operations that continue to provide lower Sussex County with quality cancer care. Ms. Gardner has been an employee with Nanticoke Health Services for the past 35 years. During her time at Nanticoke, she has touched almost every nursing area throughout the organization either providing direct care or leadership. She has served in multiple disciplines including charge nurse, mentor, manager, and director. Ms. Gardner received her Associate’s Degree in Nursing from Delaware Technical Community College in 1982 and received her Bachelor of Science in Nursing from Chamberlain School of Nursing in 2015. She is a member of numerous infection prevention, quality and safety committees within the organization. Lisa’s 28 years of experience in the nursing profession, provides her with the insight to embrace the entire continuum of health beginning with outreach to planning survivorship. An important role of the Cancer Screening Nurse Navigator is to be part of the community. Lisa understands the importance of educating the public about cancer to empower an individual to improve their health and seek access to medical care. She wants everyone to know that cancer screening tests save lives. Lisa lives in the town of Laurel, Delaware and enjoys spending time with her three grown children, who all attended schools in Sussex County. Profiles Misty Allen Misty Allen is a stay at home mom in Ocean City, Maryland. She lives with her husband John, her daughter Samantha and her mother in law Evelyn. She loves to cook, read, watch NASCAR and is an avid Disney fan. Also, she enjoys taking part in Girl Scouts with her daughter. Misty was diagnosed with stage 3C Breast Cancer on December 20, 2016. That Day, her life changed forever. She had to decide quickly on what course of treatment she was going to take, move and close her Childcare Business. With the support of her husband and doctors on February 14, 2017 Misty had a double mastectomy and shortly after began two rounds of chemotherapy and a round of radiation. During that time, she lost her hair, some toe nails but never her will to keep fighting. At a loss for where to find answers to help her keep fighting, Misty turned to Women Supporting Women. There she found someone to listen, give advice when needed and help deal with her emotions and other problems caused by cancer treatments. She started attending WSW support group with her mother in law, also a breast cancer survivor, where she felt welcomed and found others who understood. This is her motto “Around here, however, we don’t look backwards for very long. We keep moving forward, opening up new doors and doing new things… and curiosity keeps leading us down new paths.” – Walt Disney
Chrystal Chon My husband and I were married in 2009. We were living in Pennsylvania and moved to Delaware for business many years ago, and enjoy living here. July 2017, I became an active stage IV breast cancer patient. At the time of my cancer diagnosis, it was devastating. I started to pray to God, Day and Night. I also gave my prayer request to family, church, friends and people who I met at the Nanticoke Cancer Care Center. A few months later, I started to feel better about cancer. Stage IV Breast Cancer was not a death sentence and after I read an article from American Cancer Society, I started to have hope. Through Delaware Breast Cancer Coalition, I started to go to support group, talking with survivors and mentors. I started to see REAL hope. Praise the Lord! My body was responding well to treatments. My family and friends were very happy to see improvements also. My children (My nephews and niece) wrote me a card “ stay strong hope you feel better. I am your #1 fan “ and I really did not want to disappoint them in anyway. I decided to fight cancer and win! I started to drink lots of fruit and vegetable juices and trying to eat well with real foods. My case has soy and flaxseed restrictions. I have to find all kinds of food without soy or flaxseed. It is not easy, however I found some substitute for soy which was Coconut Aminos and ChickpeaMiso made out of chickpeas. I am living each day with hope and thankful heart. I can be here with my husband and family who are always my home. I really appreciate Delaware Breast Cancer coalition and all the people who truly prayed for my healing. Without them, I would not be here.

Faye C. West I was born and raised in Georgetown. I met my husband while a senior in high school. We married four years later. We have been married for 39 years. I have two daughters and they both live out of state. I have one granddaughter that is 2 months old. I graduated from Delaware Tech and Wilmington University with degrees in Business Administration. I followed in my mother’s footsteps and went into banking. I started working at Sussex Trust which is now M & T Bank. I have been there for 42 years. I was diagnosed January 2017 with triple negative breast cancer. I was shocked with this diagnosis. I had been going every year for my mammograms. I had chemo, lumpectomy, and radiation. I remained positive and kept my faith through it all. It’s hard to understand someone who has cancer until you have gone through it yourself. Your first thoughts are “why me with a lot of emotion about it being the end of the world.” Then you get mad and think I’m going to beat this. At this point your support staff (family and friends) get behind you and make your outlook a whole lot better. My husband was my rock through everything and more. He stopped his part time job to be there to help me. He had to do everything at home for 9 months. As of 2018, I have been cancer free and enjoying life. The one thing I will always remember is the love and support of the Cancer center staff. The first day walking into the center I felt I was with family. The staff was very understanding and helped Bill and I get through these tough times. My husband is the Mayor of Georgetown since 2014. We both enjoy being involved with community. My husband and I volunteer each year for the Breast Cancer Coalition car show.
Tamara (Tammy) Donohoe BSN, RN, CCRP Cardiopulmonary Rehabilitation Coordinator, Nanticoke Health Services My journey with breast cancer started in August 2017, after a yearly mammogram detected breast changes. After a biopsy and lumpectomy it was determined the cancer had spread to my lymph nodes. I was scheduled for twenty chemotherapy sessions and thirty radiation treatments. I could not have gotten through it without my faith, family, and friends. I was blessed to have made new lifelong friendships because of my cancer journey and have been able to share my story with others. My life prior to cancer was full of amazing moments shared with my husband, family, and friends. Being a wife, mother, and nurse of 29 years completed my life. Life after cancer has been full of “life moments” where every day is a gift. Every sunrise and sunset is more majestic and relationships with others are treasured. I am approaching my one year anniversary and pray I can use this journey to encourage others in some way.

The Best Worst Day of Your Life or inconvenient due to the procedure preparation. Many fear “finding something” wrong.
Imagine being in a room waiting anxiously to see your doctor for your test results. Your doctor somehow convinced you to get a colonoscopy to screen for colorectal cancer. You hadn’t had one and were years past due. After a greeting with a handshake and a smile, your doctor’s demeanor changes as he explains the results of the biopsy of the polyp he removed; pre-cancerous.
Such barriers to a colonoscopy can keep people from this important preventative procedure. Other barriers include providers who may be anchored with busy patient schedules, leaving less time to encourage reluctant patients toward the procedure. Patients may have a combination of emotional distress since they perceive a colonoscopy as painful or embarrassing combined with a lack knowledge about the importance of being screened. Language can also become tremendous barriers to delivery of healthcare, including preventative procedures such as a colonoscopy. Sometimes rural communities struggle with access to sub-specialists to meet the needs of a community. There may also be health insurance related issues.
The world stops for a moment. He pauses allowing you to grasp the word; precancerous. He explains the colonoscopy was helpful in preventing cancer; he found the polyp early and removed it. You avoided a possible devastating life event. This is often the conversation I have with patients during followup appointments. Patients often avoid a colonoscopy, focusing on the procedure as invasive and painful
Most often patients are unaware about how important
this preventative procedure is and how many times a colon cancer screening has helped a patient avoid cancer. For those with a known family history of colon cancer or patients knowing someone with cancer helps them understand this is important. Most importantly a healthcare provider encouraging patients and having important conversations with patients is key to patient completion of a colon cancer screening. You should also know, as a patient, you can start the conversation. With the Nanticoke Physician Network, the use of care coordinators who work to assist the doctor in identifying and following up with those needing cancer screenings has also proven to be very helpful. Colonoscopy still remains the most useful screening exam in early detection and prevention of colorectal cancer. The screening is recommended for men and women ages 50-75 without a family history of cancer or pre-cancerous polyps.
Those with a family history should talk with their healthcare provider. This inconvenience once every 10 years could save your life.

Lung Cancer By: Görgün Akpek, MD, MHS, Hematologist & Medical Oncologist, Nanticoke Cancer Services & Lisa Schirtzinger, Cancer Screening Nurse Navigator, Nanticoke Health Services Lung cancer occurs when cancer cells develop in the bronchi and alveoli (tiny airways and air sacs) of the lungs. Lung cancer is the leading cause of all cancer deaths with two main types: Non-Small Cell Lung Cancer (NSCLC) and Small Cell Lung Cancer (SCLC). While smoking causes most lung cancers, non-smokers also develop lung cancer. NSCLC tumors may grow slowly over a period of time before symptoms develop. Tumors may interfere with air passageways and spread into lymph nodes or elsewhere in the body, bringing about other conditions. SCLC usually starts

near the center of the chest in the bronchi. It is fast-growing, spreading in its early stages; growing and spreading much faster than NSCLC. It is rare in nonsmokers. Early symptoms of lung cancer may be a slight cough or shortness of breath depending on which part of the lung is affected. As the cancer develops, symptoms may become more severe or intense. Like many other types of cancer, lung cancer may also cause symptoms like loss of appetite or fatigue. Over six percent of people will be diagnosed with lung and bronchus cancer during their lifetime. More than 541,000 people are estimated to be living with lung and bronchus cancer in the U.S. In Sussex County (2011-2015), 8,273 new cases of lung cancer were reported or 505 cases for every 100,000 people. Over those same years,
2,781 people in Sussex died of lung cancer (166 for every 100,000 people). LUNG CANCER SCREENING For people at high risk, screening for lung cancer using low-dose, computed tomography (LDCT) saves lives. Screening may be offered to patient’s age 55-80 years and who have at least a 30 pack/year smoking history and either continue to smoke or have quit within the past 15 years. Nanticoke Health Services offers lung cancer screenings and most major insurance companies including Medicare and Medicaid pay for this screening for at-risk individuals that meet the above guidelines. Buying a New Bicycle...
Do I need to purchase a “women’s” bike? The theory of “one size fits all” is convenient for many things. They stretch, cling, sag or bag depending on your size and shape. Somehow we accept the differences. However, the same theory does not work so well when selecting a bicycle! Or does it? Saddle a little low or high...oh well, back pain is normal...right?! Women have specific needs when it comes to bicycle sizing and performance. So much so, that Trek has researched and adapted Women’s Specific Design (WSD) technology. Veterinary Services Peninsula Veterinary Services is a full-service, AAHA-accredited hospital located in Long Neck, Delaware. It is owned by Dr. April Reid, who moved from Brooklyn, New York to Delaware in 2015 to pursue her dream of owning her own practice. Current research and advances in anesthetic safety has allowed veterinarians to be able to perform spay and neuters earlier in the majority of animals. However, the recommended age for some breeds, especially the giant breeds, is much later than this so please consult with your veterinarian specifically for your pet. Spaying and neutering early is beneficial both for inadvertent overpopulation issues as well as your pet’s health. Unspayed females are at risk for mammary tumors and pyometra, a life-threatening infection of the uterus. Unneutered males can develop prostatic hyperplasia and infection as well as unwanted behaviors. Do my dogs need flea/tick/ heartworm prevention yearround? Absolutely! Because of urban sprawl and warmer temperatures, external parasites and mosquitoes are able to survive easily year-round and thus we need to protect our pets (and ourselves) all year. Fleas can survive the colder temperatures indoors in your carpet or on your pet. Ticks become less active in the winter, but can also live year-round

in your home or kennel. Mosquitoes can also live indoors and if the temperature is above 57 degrees for a few days, the heartworm larvae in mosquitoes can develop to the infective stage and be transmitted to your pet through a mosquito bite. What is best food for my pet/How much should I feed my pet? The best food for your pet is a high quality food that he/she enjoys. Always look for a food that has animal proteins as the first ingredient. While cats are strict carnivores, dogs are not and dogs need and can obtain necessary nutrients from fruits, vegetables and grains. The most important thing to look for on the packaging is the statement “complete and balanced” as the Association of American Feed Control (AAFCO) has strict requirements to make sure food labeled as such contain the minimum amount of necessary nutrients ie the “guaranteed analysis.” As to the amount to feed your pet, always go by the amount on the bag as each food has different serving sizes due to differing ingredients. In addition, how much to feed your pet is also determined by how active your pet is, your pet’s individual metabolism, and your pet’s age. Should I brush my pet’s teeth? Yes! The more often the better but generally aim for 3-4 times per week. Never use human toothpaste, baking soda or salt as these can be harmful to your pet if swallowed. If your pet will absolutely not allow you to brush his/her teeth try dental bones and dental water additives. A
yearly professional dental cleaning that includes full mouth radiographs is recommended for all pets although some breeds of dogs and small dogs may need more frequent attention. Regular cleanings help prevent gum disease that can lead to heart and organ failure due to circulating bacteria. How often should I bathe my pet? Unless your pet has a medical
condition that necessitates more frequent bathing, once monthly is usually adequate. Bathing more frequently can dry out your pet’s skin and increase the risk of bacterial or fungal skin infections so it is preferable to spot clean in between bathing with pet wipes. Always use a gentle shampoo formulated for pets when bathing as human shampoo is normally too harsh for pets’ skin.

Blinds, Shades and Child Safety…. Do you remember all the news stories, a couple years ago, concerning child safety and window coverings? The subject sticks out in my mind obviously because it is related to my work and because I have small children. The concern was mostly over mail order and or stock items that were bought off a shelf and installed, usually, by the homeowner. There were tragedies, huge recalls and lawsuits but fortunately the custom window covering trade was not really involved. I would say that the overall higher quality of product and the experienced sales and installation that comes with a custom retailer helped to keep the trade uninvolved however the custom window covering community definitely took notice and many positive changes have taken place. There is much more awareness about the subject compared to a couple of years ago and that in its self is very helpful but the industry has been very proactive in making their products more child safe as well. On the awareness side of things, it is good to note that there are many products out there that have been around for a long time like shutters and draperies that already are or can be made without child safety’s #1 enemy which is strings. Draperies, when set up to traverse by hand, present no problems for toddlers or even pets who like them because they can easily move the fabric aside to leave wet nose marks on the glass. Shutters are another great choice. They are naturally string free, simple to use, even for children, and are one of the best insulators against both heat and cold. With their timeless look and classic feel they a great child safe choice in window coverings. One way that the industry is changing to become more child safe is the introduction of cordless control into a wider variety of products. Cordless roller shades and cordless honeycomb shades have been around for a while but lately we are seeing the industry really improving and trying to mainstream the option. The string free lift systems are getting better and more reliable plus now we are seeing the option more and more on roman shades and wood and faux wood blinds which was mostly an unavailable option in years past on these popular window coverings. Window covering manufacturers are also making improvements on shades and blinds that do have strings with breakaway mechanisms that will separate when too much stress is applied and they have made it so that small children can’t get their heads between the back of the shade and the strings found there on roman style shades. These are two very important changes. Important New Changes Relating to Delaware’s Motor Vehicle Safety Statute Susan D’Alonzo Ament is a partner with Morris James LLP and has practiced for 35 years representing personal injury victims. Over the years, Mrs. Ament has found that some women don’t understand the crucial need to have adequate automobile insurance coverage in the event of a serious accident. Susan Ament educates women on these needs every day in her practice and in seminars that she presents throughout the year. The Personal Injury Group Attorneys of Morris James LLP would like to dedicate this article to educating Delaware residents on the newest amendment to Del. Code Ann. Tit. 21, § 3907 (West), which is Delaware’s Motor Vehicle Safety Statute. On July 1, 2018 the General Assembly passed House Bill 413, which is an amendment titled “[a]n Act to amend Title 21 of the Delaware Code Relating to Disclosure of Information Related to Automobile Insurance”, which will benefit Delawareans who have suffered serious personal injuries as a result of a motor vehicle accident through no fault of their own. As of the date of this article, the bill is anticipated to be signed by Governor John Carney.

The reason why this amendment is so important is because an injured individual will no longer have to incur the costs of filing a lawsuit just to obtain the information of the atfault driver’s liability coverage. This early disclosure will also allow the injured individual or their attorney to determine whether the at-fault driver was underinsured for the accident they caused. This determination is key in that it will allow the injured individual or their attorney to proceed with additional claims under the injured individual’s own underinsured motorist coverage without the delay and extra costs that come with filing a lawsuit.
What is the Purpose of This New Amendment?
What is the Process of Obtaining Liability Coverage Information?
Prior to this amendment, automobile insurance companies were not required to disclose liability coverage information to an injured claimant or their attorney. Liability coverage is the coverage available that an injured individual may recover from the atfault party for their pain and suffering, outstanding medical bills and wage loss claims. Currently in Delaware, all drivers are required to carry a liability coverage of a minimum of up to $25,000 to one person involved in a single accident or $50,000 to all people involved in a single accident, with no more than $25,000 to any one person. Previously, automobile insurance companies were required to disclose this information only once a lawsuit was filed. The purpose of this amendment is to “reduce the number of lawsuits filed by requiring automobile insurance companies to disclose liability coverage information prior to the time that a lawsuit is

an injured person, or an individual injured in a motor vehicle accident who is not represented by an attorney, may request that the insurer disclose the bodily injury limits of liability of any motor vehicle liability policy that may be applicable to the claim, prior to filing a lawsuit. The request must be in writing and must provide the insurer with the date of the motor vehicle accident, the name and last known address of the alleged liable party, a copy of the police report, if any, and the claim number if available. In addition, the requesting party must also submit to the insurer the injured person’s medical records, medical bills, and wage-loss documentation, pertaining to the claimed injury. Depending on the amount of medical bills and wage loss, the insurer will be required to respond within 30 days of receipt of the request. What Can You do to Protect Yourself and the Ones You Love? The Brick’s History Originally known during the Civil War era as The Union Hotel, the existing structure was built in 1836 by Joshua S. Layton and Caleb B. Sipple, builders of the Sussex County Courthouse across the Circle, and it actually replaced a frame Public House that stood on the site. Brick structures like this one were very rare prior to the 19th century and, in fact, less than a dozen remain in the State of Delaware! Our building is one of the few early commercial buildings remaining in Southern Delaware, and the brick used was actually made from a kiln near Georgetown. The first innkeeper was a gentleman by the name of Burton C. Baker, and history tells us that he was personally responsible for making the Brick Hotel one of the most popular hostelries on the Delmarva Peninsula. It was a favorite lounging spot for staunch supporters of the Union. The Brick Hotel operated as a hotel for over 100 years. During various periods of time, however, it also served other purposes. For a few years soon after it was built, it housed the county court house while a new building was being erected; following that, the post office was housed in the building from 1841-1844. In the 1950’s, the property was sold to and significantly remodeled by the Wilmington Trust Company, a banking industry that continued to service our community until the mid-1990’s. The property was sold to the State of Delaware so that they could replace the structure with a new court house. Word of the destruction of the building, however, put the Georgetown community into action. The efforts of a group of town citizens committed to preserve this piece of Georgetown history with their “Save the Brick Hotel” campaign resulted in the State finding an alternative site and selling the property to the H. P Layton Partnership. Long story short, Ed and I became owners of the property and our major restoration project returned the building to its original use as an inn and restaurant, complete with a quaint tavern! Since this is a National Historic Registry property, we had an obligation to preserve the historical elements of the building, including staircases, mantelpieces, original window and door trim and more. As you visit with us, please feel free to stroll into the lobby and throughout the first floor to enjoy all the history surrounding you! (Our 2nd & 3rd floors are restricted to Inn Guests or escorted tours.)

Celebrating 25 Years Women Supporting Women has been in existence for 25 years, originating in 1993 from the minds of three local women; Harriet Fine, Carol Prager and Sue Revelle. Their forethought brought about this nonprofit organization to help breast cancer survivors on Delmarva. Services began with WSW's signature Pink Bags in the basement of Sue Revelle’s house. It then morphed into the plethora of free services Women Supporting Women offers today. WSW celebrated their 25th Anniversary with a beautiful ribbon cutting ceremony on July 26, 2018, unveiling their new logo. Ending the nag of knee pain Terry Gorlich is full of spunk. She recently relocated to Lewes to enjoy retirement closer to some of her grandchildren. But nagging knee pain was getting in the way of living her life to the fullest. That’s when she asked Bayhealth Orthopaedic Surgeon Stephen Manifold, MD, what she could do to alleviate the pain.
“I’ve been a patient with Dr. Manifold for years. When my knee pain first came on we treated it with medication, which worked for a while,” said Gorlich. “But I knew the pain was getting worse and I needed to do something about it.” Gorlich remembers she finally had enough when she found herself going to the grocery store several times a week because she couldn’t finish a full trip in one visit because of the pain. There would be times when she would have to stop shopping because the pain was so unbearable. “I remember it was a dramatic change for me when the pain was just too much,” said Gorlich. “I was active and then suddenly I couldn’t be. I mow my own grass, but then it was taking me three times as long. I loved to garden and then I felt like I couldn’t. I even used to do a boot camp group workout. I loved the atmosphere. But I definitely couldn’t do that anymore. That’s when I knew it was time for a change.” Further tests confirmed the osteoarthritis in Gorlich’s knee had worsened. After another appointment with Dr. Manifold, they both decided Gorlich needed a total knee replacement. During a total knee replacement, an orthopaedic surgeon removes damaged cartilage and bone, and then positions the new implants to restore the alignment and function of the knee. Most patients who undergo a total knee replacement are able to reduce their pain while also improving their ability to do daily tasks again. Gorlich had the surgery earlier this year at Bayhealth Milford Memorial. She started a physical therapy regimen a few weeks before the

surgery and continued to go for several weeks after. She did both traditional physical therapy as well as aquatic therapy. She loved the aquatic therapy so much; she still goes several times a week as part of a maintenance program. “One of the reasons Terry has done so well after surgery is her positive attitude and goal-oriented thinking,” said Dr. Manifold. “Terry knew what she needed to do to get the best outcome and she put in the work to achieve that. How a patient approaches that type of surgery is as important as the procedure itself in achieving a good outcome.” Just weeks after her knee replacement surgery, Gorlich felt like her old self. She’s back to doing the things she loves, even planting flowers in her garden. “I love having my life back. Now I can go downtown and shop or see friends. I can go to the grocery store without any trouble,” said Gorlich. “I’m mowing my grass again. I’ve even planted flowers in my garden. And I can’t wait to do even more in the garden next year. I did this for a better quality of life, and I’ve achieved it. It’s wonderful.”
