EARLY DETECTION OF BREAST CANCER SAVES LIVES Breast cancer is the most common cancer in women. While breast cancer can occur in men, it is most often found in women. Sometimes breast cancer is found due to symptoms experienced by the patient but many times women with breast cancer do not experience any symptoms. This is why talking with your health care provider about having an annual mammogram is so important. Recommendations by The American Cancer Society for who should be screened have changed slightly but it is still recommended for women with low or moderate risk of breast cancer who are age 40 to 45 years have the option to have a mammogram. By 45 years of age, all women should have a regular mammogram. Women who are at a higher risk for breast cancer may need to have mammograms or other screenings such as an MRI or Ultrasound done earlier in life or more often. When detected early, the survival rate for breast cancer increases dramatically. If you are a woman 40 years or older or if you have a family history of breast cancer, regardless of your age, talk with your provider about having a mammogram. Nanticoke Health Services in Seaford offers the latest in mammograms using Genius™ 3D MAMMOGRAPHY exams.

What is 3-D mammography? Approved by the FDA, 3-D mammography uses multiple images, or X-rays, to recreate a 3-D picture of the breast. Using these multiple images, a 3-D mammogram provides more detail than traditional mammography, which uses a single image. During the 3-D exam, an X-ray arm sweeps in a slight arc over the breast, taking multiple images. A computer then converts the images into a stack of thin layers, allowing the radiologist to review the breast tissue one layer at a time. Any woman who needs breast screening should consider 3-D mammography. In particular, women with dense breast tissue may beneﬁt from 3-D technology because of the layered images that make it easier for doctors to catch breast cancer early. 3-D also helps catch more cancers and helps doctors see the cancer size much better.

Why 3-D mammography? There are several reasons why ﬁnding a provider that uses 3-D mammography is important. Because this technology uses multiple images, doctors have a clearer picture of breast masses, making it easier to detect breast cancer, detect it earlier and reduce false positives. When using 2-D mammography, overlapping tissues is a leading reason why small breast cancers may be missed and normal tissue may appear abnormal, leading to unnecessary callbacks or repeated testing. A 3D mammogram can reduce the tissue overlap effect. A study on breast cancer screening using the Genius exam was published in the June, 2014 issue of the Journal of the American Medical Association (JAMA). Findings include: • A 41% increase in the detection of invasive breast cancers. • A 29% increase in the detection of all breast cancers. • A 15% decrease in women recalled for additional imaging. The Modern Optician

Opticians today play a vital role when it comes to being completely satisfied with your eyewear. I started in the eyewear business almost forty years ago and have seen many changes in the eyecare business. The one constant has been an opticians goal to successfully fit a client’s eyewear to the best of his or her ability. Opticians vary as in any field so as the optical director I’ve worked over the years to educate and train our opticians. Knowledge, skills, tools and experience all enter into the equation for a successful eyewear fitting. Delaware Eye Optical staff is well trained and educated in fitting eyewear using today’s most up to date lenses and a frame collection that is both broad and deep in styling. Lenses we fit are primarily digital and use materials such as trivex with applications of anti-glare, and transitions (photosensitive), mirrors tints and other needed applications. The experience of working with ophthalmology has afforded a vast background of visual needs over the years and prepared our staff to deal with not only day to day requirements, but also more complex issues such as prism, high powers, light sensitivity, and other complex eyewear fitting needs. Knowledge accumulated over the years and the experience we have gained cannot be emphasized enough. These experiences with various optical issues is shared among the staff and allows us to handle any visual needs you might have quickly leading to your satisfaction. These relationships again come from experience in creating niches for various optical needs and style requests. The evolution of fitting digital eyewear has changed how opticians measure an individual once the frame is selected. Medium and higher level digital lenses are custom and require more information to maximize the prescription once the frame is selected. We use electronic fitting with a software application called N-Form. The optician actually takes two pictures one for distance and the other for reading. These two electronic measurements provide

visual axis, facial shape (wrap), optical center heights, and vertex distance so our digital lenses can be designed and manufactured using as worn technology. What does that mean to you? The lenses are personalized taking into account your prescription within the frame you’ve selected. Complete satisfaction is more likely, ease of any change in prescription should be enhanced and comfortable vision in all fields, distance, intermediate, and near. The high-performance category of lenses we provide are our most advanced design and have the highest satisfaction rate of any category. These lenses are “like wearing a single vision lens and seeing all fields clearly and effortlessly” is what many of our clients have said. Recently we ran a report on our turnaround time and it showed our main lab is processing lenses at an average of four days. During a treatment, your unwanted fatty area will be pressed and chilled to a temperature 12 degrees above freezing. At this temperature the fat within fat cells crystallizes. When the fat within the cells crystallizes, it damages and kills the fat cell. Skin and other tissue is not damaged as the water in other cells is not crystallized, only fat. The body will then eliminate the killed fat cells over time. The best news is that we do not make new fat cells. Fat cells merely get bigger or smaller as they store more fat or lose fat. So in essence the fat removal from CoolSculpting is permanent. And we have seen instances of patients who if they gain weight, the weight will not return to the treated areas.

What are the most common areas to be treated?

Many people on a weight loss program continue to have stubborn hard to lose areas, no matter how much dieting, sit ups or weight they lose. Such stubborn areas are typically around the abdomen, the love handles and inner thighs. Once you are over forty, losing fat in these areas is extremely difficult. These are the areas that CoolSculpt can address. Basically if there is an area you can pinch several inches, that area can

Are there any side effects from CoolSculpting?

What does CoolSculpting feel like?

Our treatment room is very comfortable where you will relax on a padded recliner. When the CoolSculpting is applied to the fatty area to be treated there is a sensation of pulling that feels slightly uncomfortable for the first 5 to 10 minutes. Getting to the bottom of an uncomfortable issue. Incontinence is difficult to deal with. For many women, it makes life complicated. They know every bathroom in the mall or every rest area on the highway. And many are just too embarrassed to talk to their doctors about it. “While many women are afraid to open up, the truth is, this is a very common problem and there are treatment options available,” says Elsa Goldstein, MD, of Bayhealth Colon Rectal Surgery with offices in Milford and Lewes.

WHAT CAUSES PELVIC FLOOR DISORDERS?

Patients experiencing problems with urinary or fecal incontinence can now make an appointment at the Pelvic Floor Center. The Pelvic Floor Center has state-of-the-art diagnostic capabilities to accurately diagnose incontinence disorders. Dr. Goldstein sheds some light on pelvic floor disorders and the options available for treating urinary and fecal incontinence.

People should seek treatment when they become symptomatic. Once the problems begin impacting your life, it’s time to look for help. Women will many times make adjustments to their lifestyle to accommodate the symptoms rather than seeking help. Some women will stop traveling or going out because of bathroom concerns. Others won’t eat all day if they know they have to go out.

WHAT IS THE PELVIC FLOOR?

WHAT ARE THE SYMPTOMS OF PELVIC FLOOR DISORDERS?

The pelvic floor is a muscular structure that supports the pelvic organs of the bladder, uterus, vagina, and rectum. WHAT ARE PELVIC DISORDERS?

The most common pelvic disorders involve the weakening of the pelvic floor. That’s when you have bladder, rectal, or uterine prolapse. Prolapse is when the body parts essentially fall out of position. This prolapse causes incontinence with urine or stool. More rarely, some people have pelvic floor problems because the muscles are too tight, but this is not very common. WHO TENDS TO HAVE PELVIC FLOOR DISORDERS?

Women are far more likely to have pelvic floor problems. The most common age group is women over 50. Men can have problems as well, but women are far more likely. Some younger women may also experience pelvic floor disorders if they had a large baby.

The main cause of pelvic floor disorders is a combination of childbirth and aging. That’s why women are more likely to have issues. Nearly one third of women will sustain a tear during childbirth. Once you get older and the aging process is underway, women tend to notice the symptoms. WHEN SHOULD I SEEK HELP FOR PELVIC FLOOR DISORDERS?

The most common symptoms are urine or fecal leakage. Some women will experience leakage when they cough, sneeze or exercise. Other women will have leakage no matter the circumstances. With fecal incontinence, some women may leak gas or stool. There’s also an increased urgency, where you may have limited or no warning that you need to use the bathroom. WHAT ARE THE TREATMENT OPTIONS?

The most common treatment is pelvic floor therapy. Most people respond well to the therapy and do not need any further intervention, such as surgery. We create an individualized plan for all of our patients. It’s not a one-size-fits-all model. We tailor

the plan to the patient to ensure they have the best results. We also work with patients on dietary management, as many people respond well with it too. HOW LONG DOES TREATMENT TAKE?

For many people, treatment is once a week for a few months. It’s very similar to going to physical therapy. There’s nothing preventive that can be done, so it’s important women seek treatment when they notice symptoms. ARE THERE ANY RISK FACTORS?

Women who have given birth are at a higher risk of developing pelvic floor problems. Additionally, anyone who is diabetic, people taking certain medications, and anyone who has undergone radiation treatment to the pelvis are at a higher risk. Those with neurological disorders and a history of rectal surgery are also at a greater risk of developing pelvic floor problems. HOW COMMON ARE PELVIC FLOOR PROBLEMS?

This is a very common problem and women should not be embarrassed. Nearly 20 percent of the population has pelvic floor problems. This is a silent affliction. Women suffer in silence and aren’t telling their doctors or family members. They don’t seek treatment and instead just make lifestyle adjustments. But it doesn’t have to be that way. We work with women to create a specific treatment plan and restore lifestyle and dignity. There’s no longer a reason to suffer in silence. The aligners are changed every two weeks or as often as the doctor prescribes. It’s just like soft contact lenses. And you can also take them out anytime you want to but should be worn as close to 24/7 as you can. Braces Can Hurt More Than They Help Braces can actually cause more damage to your teeth. The reason is that there are certain areas

that you just can’t get to with your toothbrush while they’re in, therefore the risk of tooth decay increases. With Invisalign aligners, you can slip them off any time you need to brush your teeth and there is no risk whatsoever to your perfect smile. Dr. Hudson is a ’95 Cape Henlopen High School graduate and continued her educational career at the University of North Carolina at Wilmington where she completed her Bachelor’s degree in Physical Education. In fact, a 2005 study, published in the journal, Seminars in Oncology Nursing, encouraged the use of chiropractic services for cancer patients, concluding that a review of written resources suggested it appeared to have beneﬁts for relieving their pain and suffering. Your chiropractor will want to be kept up to date on your evolving condition, including the type and location of the cancer, current blood work, most recent imaging and the treatment regimen, in the interest of the delivery of care. We also know that this therapy may not be for every cancer patient. For example, chiropractic services may not be recommended when a bone metastasis is found or the patient’s platelet counts are low. Recommendations for chiropractic care should be made in close consultation with the patient’s oncologist.

Myth: Once you go to a chiropractor, you must keep going for the rest of your life. Continuing chiropractic visits is not mandatory, and some patients stop seeing chiropractors after their pain or other side effects have subsided. Those who continue to receive regular chiropractic care often choose to keep up the treatments because they believe the adjustments contribute to an overall sense of health and well-being. Many patients have reported sleeping better, having fewer to no headaches and experiencing fewer sinus issues, fewer colds and a sense of improved energy and vitality.

Know Where You’re Going … And Why! (It could save you money)

About Dr. Chao Dr. Philip Chao graduated Phi Beta Kappa from Yale University and was Scholar of the House. He continued his studies at the University of Rochester School of Medicine and Dentistry, receiving his medical degree in 1983. After a transitional internship year at the Mary Imogene Bassett Hospital in Cooperstown, NY, Dr. Chao did his residency in Radiology at Brooklyn’s SUNY Health Science Center (1984-1987), where he became Chief Resident. He stayed on to complete a fellowship in Neuroradiology (1987-1988). Dr. Chao has been interested in MRI technology since its very beginning – the first MRI scan took place in 1981 while he was pursuing his medical studies. The University of Pennsylvania, where Dr. Chao was both a Body MRI Fellow (19881990) and a Neuroradiology Fellow (1989-1990), was at the heart of the development of this technology and Dr. Chao was able to work with pioneers in MRI on the first GE Signa 1.5T (tesla) scanner. His advanced research and specializations complete, Dr. Chao left the University of Pennsylvania for a position as Director of MRI in Wilmington, DE – a position he held for 14 years. Dr. Chao eventually left that position to create the best MRI Center in Delaware which uses the very latest technology: 3T MRI. While 3T (tesla) scanners have been used for scanning research patients since 2002, these advanced, more powerful MRI scanners only became available for clinical use in 2004. Dr. Chao worked hard to bring 3T technology to Delaware and in February 2007, MRI Consultants, LLC began operating the first 3T scanner in the State. 3T is the newest horizon for MRI. By: Dr. Philip Chao Did you know you can save $400 to $600 on your next MRI study simply by choosing where your MRI study is done? If your MRI study is done at a hospital or clinic-based imaging center associated with a hospital, you may have to pay additional fees (co-pay or coinsurance). Certain insurance companies routinely charge these extra fees without mentioning them before a study is done— even if you have your study “authorized.” It can be several hundred dollars. Insurance companies do not charge this extra fee if you go to an independent facility—such as MRI Consultants. In these facilities, your MRI is billed as an Office Visit. In such instances, your co-pay can be significantly less. Just ask your insurance company. Quality is important. So few people (including doctors) realize how important it is to have your study done on newer 3T or multichannel 1.5 scanners instead of older scanners. The older scanners just don’t “see” as much! If your study is done on an old scanner with just 1 channel, you can be pretty certain subtle things will be missed. We recently found a tibial non union which was only 1mm in size! This can cause a great deal of pain. See image. The resolution on older scanners and low-field OPEN MRI is insufficient to detect such small fractures. It just can’t see it. Higher definition is extremely important. We recently scanned a patient for a possible superior orbital vein thrombosis. The ORBIT study we performed at 3T definitively showed this isolated finding. 3T high quality pictures are like HDTV (as compared to blurry old snowy cathode ray tube pictures). 3T is just better. And, despite what you may

have been told, 3T only costs more for the person who purchases the scanner! For the patient, the price is the same. Quality seems to be unimportant to insurance companies (and even to some doctors). For them, an MRI is an MRI. Your co-pay is the same regardless of quality. Read the NY Times article: “The Scan That Didn’t Scan.” The author did not seem to know that the ACR, which accredits MRI units, has different criteria for different scanners. Weaker MRI scanners are not required to “see” as much as stronger scanners. And, surprisingly, there is currently no minimum level required in order for your insurance company to pay. None. Even the weakest scanner can be “accredited.” You may have to have your study repeated on a stronger machine (frequently at your own cost), but the medical profession and insurance companies are largely unconcerned. Understanding your pictures is important. At our facility you get to discuss your images with the radiologist (upon request). I do not know if this is even possible at other facilities. At MRI Consultants, patient reports are frequently done within an hour— sometimes before leaving the office. Also, all patients leave our office with a

CD of their images. We offer a quality and speed of care which is not often given elsewhere. At MRI Consultants, we do studies other facilities will not (or cannot) do. We do indirect arthrogram studies which frequently show MORE than direct arthrograms. IV contrast is used to see if there is a tumor, infection or inflammation. At 3T, the fat saturation is better and we are able to do thinner “slices” (or sections), which allows us to see very small tears. Even a small tear in your shoulder can be very painful.

Finding such small tears is one of the great things about indirect arthrography. A recent patient of ours had two labral tears with two paralabral cysts. According to “the literature,” these paralabral cysts are rare, yet I see them even without contrast in many of my patients at 3T in the hip and shoulder. This just another example of why 3T quality is better. Most doctors and orthopedic surgeons do not know this. 3T MRI simply shows more. Seeing more can reduce the need for surgery or, at the very least, make it safer. Be smart. Be certain. Use 3T.

With time, age and gravity, our facial structures tend to lose some of the youthful volume it once had. The cherubic apples in our cheeks fall, leaving us with a drooping jaw line that becomes jowls, and deep lines form around the nose and mouth areas, known as the nasolabial folds. He completed his post-graduate medical education at the University of Pennsylvania Graduate Hospital in Philadelphia with a residency in internal medicine (1991-1994), Chief Medical Resident (1994-1995), and cardiology fellowship (1995-1998). Dr. Rosa has practiced cardiology in Sussex County since July 1998. He has served as Medical Director of the Cardiac Catheterization Laboratory and as Chief of the Department of Cardiology at Beebe Medical Center in Lewes, DE. Dr. Rosa is trained in non-invasive cardiology, Nuclear Transesopageal Echocardiograms, as well as invasive diagnostic procedures and pacemaker implantations. Dr. Rosa is board certified by the American Boards of Internal Medicine and Cardiology. He is also a Certified Aviation Medical Examiner and is Board Certified in Nuclear Medicine. The American Heart Association has released their 2016 Statistical Fact Sheet (1) in women and cardiovascular disease (CVD). Some notable facts are worth highlighting: Cardiovascular Disease • More than one in three women have some form of CV disease • In 2013, there were 398,086 female deaths due to CV disease (49.7% of all CV deaths) • In 2013, CVD caused about 1 death every 80 seconds among females. That represents about the same number of female deaths from cancer, chronic lower respiratory disease, and diabetes combined. There were 40,861 deaths attributable to breast cancer in females; lung cancer claimed 70,542 females. One in 31.6 deaths of females was attributable to breast cancer, whereas 1 in 8.0 was attributable to CHD. For

Stroke • An estimated 3.6 million female stroke survivors are alive today. • Each year about 55,000 more women than men have a stroke. This is because the average life expectancy for women is greater than for men, and the highest rates for stroke are in the oldest age groups. • In 2013, stroke caused the death of 75,287 females (58.4% of total stroke deaths). In 2010, 530,000 females were discharged from short-stay hospitals after having a stroke. High Blood Pressure (HBP) • One in three adults in the United States has HBP. • Among women age 20 and older, the following have HBP: 30.1% of nonHispanic whites; 46.1% of non-Hispanic blacks and 29.9% of Hispanics. In 2013, 38,379 females died from HBP. They represented 53.3% of deaths from HBP • Among adults, 18.3% of white women; 13.4% of black women and 7.4% of Hispanic women smoke cigarettes. Diabetes Mellitus • Of the estimated 21.1 million American adults with physician-diagnosed diabetes, about 10.6 million are women; 6.1% of non-Hispanic whites, 14.6% of non-Hispanic blacks and 11.8% of Hispanics. • Of the estimated 8.1 million Americans with undiagnosed diabetes, about 3.0 million are women; 1.7% of nonHispanic whites, 2.3% of non-Hispanic blacks and 5.0% of Hispanics. • Of the estimated 80.8 million Americans with pre-diabetes, about 34.3 million are women; 28.9% of non-Hispanic whites; 27.8% of non-Hispanic blacks and 26.0% of Hispanics.. • In 2013, diabetes killed 35,737 females. • 319,000 females diagnosed with diabetes were discharged from short-stay hospitals in 2010.

Coronary Heart Disease (CHD) (ICD/10 codes I20-I25) (ICD/9 codes 410-414, 429.2) • About 6.6 million females alive today have CHD. Of these, 2.7 million have a history of myocardial infarction (MI, or heart attack). • Among women age 20 and older, 1.8% of non-Hispanic whites have had an MI; 2.2% of non- Hispanic blacks and 1.7% of Hispanics. • Each year new and recurrent MI and fatal CHD will impact an estimated 405,000 women. • The incidence of CHD in women lags behind men by 10 years for total CHD and by 20 years for more serious clinical events such as MI and sudden death. • In 2013, 161,698 females died from CHD; 50,742 from MI. Overweight and Obesity • An estimated 31.6% of girls age 2 to 19 are overweight or obese; 29.2% non-

Hispanic whites, 36.1% non-Hispanic blacks, and 37.0% Hispanics. • Of these girls, 17.2% are obese; 15.6% non-Hispanic whites, 20.5% nonHispanic blacks, and 20.6% Hispanics. • An estimated 64.7% of women age 20 and older are overweight or obese; 61.2% non-Hispanic whites, 81.9% nonHispanic blacks, and 76.3% Hispanics. • Of these women, 36.0% are obese; 32.5% non-Hispanic whites, 57.5% nonHispanic blacks, and 42.9% Hispanics. These sobering statistics confirm, once again, that heart disease remains the number one cause of mortality and a major cause of morbidity (illness) in American females. Much remains to be done and the role of early childhood. The role of parents is crucial in the effective prevention of many of the above health issues. Starting early helps provide children with the best chances for healthy lives.

TUCK or ROLL? Tummy tuck, or abdominoplasty, is a body contouring procedure that is great for individuals who have loose, excess skin that does not improve with diet or exercise. It will remove loose, excess fat and skin in most cases, restore weakened or separated muscles creating a firmer, more youthful figure. Popular following pregnancy, massive weight loss or whenever a flabby abdomen with weak muscles impairs body contour, this procedure is commonly coupled with liposuction to achieve the most optimal body contours. Traditional Tummy Tuck In addition to the incision across the lower abdomen, a traditional tummy tuck involves an incision around the navel. Excess skin is removed, muscles are tightened and liposuction is performed to achieve the best result. This surgery removes skin from above the navel down to the pubic area that has been compromised by pregnancies, genetic laxity or obesity. The scars normally heal very nicely and are well worth the trade off! Mini Tummy Tuck A single incision is made in the pubic area, which will be placed as low as possible and can range from a few inches to the entire span of your abdomen. Through this incision, Dr. Malek will remove excess skin, tighten muscles and, more than likely, perform liposuction first to further refine the area. The ideal candidate for a minitummy tuck has laxity and protrusion

below the navel and is usually recommended for those who don’t have enough excess skin above the navel to require a traditional tummy tuck. Extended Tummy Tuck If you have excess skin in the love handles or hips, your incision can be made longer to address this problem. This procedure includes a tummy tuck in the front of the abdomen and transfers the lift over the hip to the side of the thigh to improve contour of hip and thigh areas. The incision is usually low so it will not be visible, even when one is wearing low-rise clothing. Recovery Patients will be able to return to a desk job in a few weeks depending on the type of procedure performed, and patients with jobs that require more physical labor, may need more time. A fifteen-twenty pound weight cap is advised for the first six weeks of recovery. Vigorous exercise such as running, swimming, core conditioning and weight training are also advised against in the first six weeks. Walking is recommended as soon as it is comfortable for the patient as it aids in blood circulation and recovery. FAQ: Am I a good candidate? A tummy tuck is suitable for men and women who are in generally good medical health, have realistic expectations and who have excess skin and tissue in the abdominal area. Women who have had several

The need for improved access to health care services in SCD cannot be overstated as SCD is federally designated as a Medically Underserved Area, a lowincome Health Professional Shortage Area, (HPSA), a Dental HPSA, and a Mental Health HPSA. The vast majority of the people moving in to SCD are senior citizens who are retiring and relocating to live here. Overall, in 2015, the SCD population was 212,900 and is expected to grow to 251,900 by 2035. Some signs and symptoms that may indicate a low sex drive include a woman who: • Has no interest in any type of sexual activity, including self-stimulation • Doesn’t have sexual fantasies or thoughts, or only seldom has them • Is bothered by her lack of sexual activity or fantasies

• Power struggles and relationship issues: If relationship issues are not resolved, resentment, anger, frustration may build up, and LD may be the result. • Inﬁdelity: The reason and/or consequence of the incident may be your partner’s lack of sexual desire. • Depression. Depression can change the body’s biochemistry and therefore reduce libido.

What are some of the dynamics that may be attributed to LD? • Hormonal Cocktail: Changes in our hormone levels may decrease your desire for sex. • Aging: The decline in estrogen during perimenopause may wreak havoc on a women’s sex drive, and dry vaginal tissues, resulting in painful or uncomfortable sex may deter her from having sex. • Body Image Issue- If women are feeling unattractive, they are less likely to want to engage in sex. • History of Sexual abuse or abusive relationship: A women may not want to express physical love if she had old wounds or pain to heal. • Fatigue: Fatigue is problematic in women of all ages, even ladies in their 30’s. • Unskilled partner/lover: Maybe your partner skills are lacking in the bedroom department. This problem may continue until she doesn’t want sex anymore.

Dr. Richard Arrigo is also a board certiﬁed gastroenterologist at Hunterdon Gastroenterology Associates. Prior to his fellowship, Dr. Arrigo was in practice with the Liver Transplant program in Newark, NJ where he managed patients with severe liver disease. Dr. Arrigo is trained to perform Endoscopic Ultrasound (EUS), a procedure that uses sound waves to create visual images of the digestive tract, and Endoscopic Retrograde Cholangiopancreatography (ERCP), a procedure that is used to diagnose diseases of the gallbladder, biliary system, pancreas, and liver. Dr. Arrigo is one of three physicians in Hunterdon County trained in the ReShape Weight loss Balloon procedure.

The Effects of Obesity The rising incidence of obesity in both children and adults has gained the attention of health care providers worldwide. While much of the attention has been focused on the association between obesity, cardiovascular disease and diabetes, ongoing research is demonstrating a link between obesity and many other diseases. The American College of Gastroenterology recently convened a task force to assess the impact of obesity on GI diseases. Their findings showed several GI diseases are more common in individuals who are obese. For example, there is a 50% increased risk of gastroesophageal reflux disease (GERD) in an obese individual, a 50-100% increased risk of erosive esophagitis, and a 2-3 fold increase in gallstones. Results from recently published research studies indicate that non-alcoholic fatty liver disease (NAFLD) is one of the most commonly seen diagnoses in patients who are overweight and also have metabolic syndrome. NAFLD can lead to changes in the liver that could ultimately result in more severe diseases, including cirrhosis and liver cancer. Liver cancer is one of the few types of cancer with a rising incidence, attributed to the increasing number of Americans considered overweight or obese. A correlation between

abdominal obesity and pancreatic cancers has been identiﬁed. Endometrial cancer, post-menopausal breast cancer and colon cancer account for 2/3 of cancers attributable to excess body weight. In these cancers, being overweight is one of the most common risk factors, included with smoking. What can you do to decrease your risk of GI diseases and GI cancers, as well as improve or eliminate the symptoms of GI disease you may currently be experiencing?

First, know your numbers. Body mass index (BMI) is the commonly used measure of obesity. BMI calculations are based on height and weight. BMI calculators are available on line on many different web sites, or you can ask your doctor to help you calculate your BMI. Overweight is deﬁned as a BMI from 25-29 kg/m2, and obesity is deﬁned as a BMI of 30 kg/m2 or above. Second, if your BMI places you in the categories of overweight or obese, commit to lifestyle changes that will reduce your risk of GI disease. Just a moderate reduction in calories will enable you to lose 1-2 pounds per week. A slow weight loss is the healthiest approach, and one that is easier to maintain than diets that promise rapid weight loss. Focus on healthy choices and portion control.

For best results, supplement your changes in diet with an increase in physical activity. Long sessions at a gym aren’t necessary. Thirty minutes of moderate activity per day is recommended. Moderate activity can include walking, cycling, dancing, even housekeeping. Anything you do on a consistent basis will provide health beneﬁts.

Proﬁles E. Nancy Richardson I was born in Baltimore, MD and graduated from Infant of Prague High School in 1963. I had quite a few secretarial jobs in the next 8 years before I landed an administrative position at Honeywell in July, 1971 where I stayed for 30 years until retirement. Two weeks later I met the love of my life and married in November, 1973. In 1998 at 53 I discovered a small lump in my right breast which everyone thought was benign and was told to go treat myself to a nice piece of jewelry and stop worrying. It was cancer and I chose a lumpectomy done January, 1999, had four chemo treatments and 33 radiation treatments. I continued to work and tried to maintain my sense of humor and enjoyed the very nice gold bracelet I bought myself. In the next 16 years I decided to enjoy my life more by retiring, moving to Rehoboth Beach, DE, traveling to visit my step children in FL and taking a yearly cruise. In November 2014, 16 years later I was diagnosed with Stage 3 cancer in my left breast. Again I chose a lumpectomy but they didn’t get it all so after much discussion my husband and I thought a double mastectomy was the way to go. In January 2015 I had the surgery and was informed cancer in my right breast would have been back within a year or two. Hopefully I dodged that bullet with my decision. I went onto six treatments of chemo, 34 radiation treatments all of which took there toll. Because I had cancer in 19 out of 20 nodes I was hit with some serious chemo and radiation. 2015 was a rough year but I survived and many will. Being told you have cancer is devastating. No one wants it but it’s a fact of life and you must learn to deal with it. I have made wonderful friends in DE and still have great friends in MD. I have tried to maintain my sense of humor, I read a lot, still take cruises and walk just about every day. I discovered the DE Breast Coalition thru my friend Nancy Mann and it has been quite enjoyable. The statement “it is what it is” is so true and all we can do is help each other, hold onto your faith and do the best we can with what we’ve been dealt.

Amanda Jarmon My journey facing cancer began back in January of 2016. After a self check, and many appointments/scans later I was diagnosed with Angio Sarcoma of the left breast. I underwent a left mastectomy, chemo, & radiation in 2016. Then in March of 2017 I had a reoccurrence in the right breast and went for another mastectomy. I was a wife, a mother to two young boys, I exercised, ate well, and thought that I was doing everything right. Now after being cleared (I have to go every 3 months for scans),I continue to live life as normally as possible. I am back to work at Coastal Home Care doing what I love the most, helping others and trying to make a difference in the community one person at a time. This would not have been possible without amazing family & friend support system, the help of Women Supporting Women, The American Cancer Society, Susan G. Komen getting involved & speaking to other survivors. Even though we may have two different types of breast cancer, the process is the same for us all. I have learned to accept the new me, even if it’s hard some days. I continue to pray & still get anxious when I have upcoming appointments and scans. I’ve come to understand that this will be my new life; my family and I are learning to manage the anxiety and focusing on making each day as positively as possible! I am ﬁnally able to take control of my life again one day at a time. And my family & I are learning to embrace our story and share it with others. Even if it helps just one person going through a similar situation ten it makes it all worth it in the end.

Nancy Cilio Mann I moved to Rehoboth Beach in August of 2000 from New York City. I continued to travel back and forth to New York in order to continue my work as a Project Manager for Corporate Real Estate in the Financial Sector. My work was interesting and I never wanted it to end. The beach kept calling my name and in 2003 my husband Tom and I decided to answer the call. In 2005, I was diagnosed with breast cancer, it was a stage III with lymphatic involvement; at that time I was given a very grim view of my situation. My surgery was done at Beebe Medical Center where I received excellent care. Two weeks prior to the beginning of my aftercare Tunnell Cancer Center opened at the Beebe Campus on Rt24. Being the forever optimist that I am, I returned to work immediately after surgery and chose to do chemo therapy and radiation while I continued working a shortened week. Not only did it seem like the logical thing to do at the time, I also think it gave me hope and encouragement to ﬁght. I was put on many prayer lists and my church started a support group. Not long after that I was introduced to the Delaware Breast Cancer Coalition where I became a mentor. I realized how important it was to recognize the desire to survive and that encouraging others is one way to reinforce your own HOPE! Ten years later I was diagnosed with breast cancer a second time, this time I had a partial mastectomy. I now consider myself a true poster child for success. Having found a great diet and exercise program I continue to be active and enjoy the beach. In just a couple of weeks I will turn 69 years old and I feel blessed to be a survivor and mentor. I have purpose!

Pam Ward I have four children, my daughter is 13 and my triplets boys are 11. I had my first mammogram when I turned 40. I was scared to get it done because everyone says it hurts...it wasn’t so bad. I had a biopsy done at the Breast Center and it came back positive for DCIS. All that I thought was that I wanted it out. I talked to my dr and decided to go to the Breast Center at Mercy Hospital. My doctors office told me to call Women Supporting Women to ask about lending me a electric recliner and it was wonderful, I don’t know how I would have done it with out it. You just don’t know how much you use your arms and abdominal muscle until you can’t. They also gave me pillows to put under your arms to help you keep your arms up and away from your sides. I had a double Masectomy, DEIP flap reconstruction and lymph nodes removed March 2016. Sue from Woman Supporting Women called me regularly to check up on me to see how I was doing. Everyone was so helpful at WSW. I am grateful to my family & friends that all helped me, I couldn’t have done it without them.

Profiles Tina Callahan My survival story begins in September 2016. I had a sharp pain go through my right nipple. When I felt the area to figure out why it was hurting, I felt a lump right away next to my nipple and knew it was not normal. I went in to see my OB/GYN for a full exam. He felt the lump too. But, the way it felt (hard and smooth), he felt as though it was a cyst. To play it safe, he scheduled a mammogram and ultrasound. It was detected right away. Now, it was time for the biopsy. The day of the biopsy, I walked in with confidence that it was just a cyst. And then my surgeon dropped the bomb! The lump was Breast Cancer (Invasive Ductal Carcinoma)! My doctor, Dr. John Riley with PRMC and I started to discuss our options. Right away, I knew I wanted a full mastectomy. My life is worth more than my breast. A double mastectomy surgery was scheduled on November 21st and 3 lymph nodes were removed. After my surgery, I was all bandaged up and it took me about a week to accept the new me. After about a month, I met with my Oncologist, at PRMC to discuss possible chemo. Turns out I scored a 17 which meant I did not need to endure chemo and would instead would be going on the hormonal therapy pill for about 5-10 years. So, starting January 17th, my radiation treatments began. 30 treatments total. My skin is now red, it looks sunburned and I am starting to become sore. I have been tired emotional and physically. But, I refuse to lose this battle. I am a Breast Cancer Survivor. I claimed that every day. I have already won the battle. With this experience, it has changed me. Alfac helped me financially and inspired me to become an Aflac agent. I have a different outlook on life and I have learned to appreciate things and not let the little things bother me anymore. Life is too precious.

Dr. Christine D. Hannaway is a Board Certiﬁed General Surgeon practicing with the Nanticoke Physician Network in Seaford, Delaware. In addition to a broad scope of general surgical care, Dr. Hannaway evaluates and treats a wide array of breast disease. This includes conditions such as breast pain and lumps as well as the diagnosis and surgical treatment of breast cancer. Dr. Hannaway works closely with Nanticoke Health Services’ mammography department and the Cancer Care Center as part of a breast care team. She utilizes advanced techniques to diagnose breast cancer. Every aspect of each patient’s surgical breast care is managed by Dr. Hannaway, from breast biopsies to long-term follow-up. Dr. Hannaway obtained her medical degree at Eastern Virginia Medical School in Norfolk, Virginia. She completed her general surgery residency at INOVA Fairfax Hospital in Falls Church, Virginia. Cancer Care Coordinator, Nanticoke Cancer Care Center

Terri Clifton is the Cancer Care Coordinator at Nanticoke Cancer Care Center. Terri helps guide patients through the complex health care system and informs them of available resources. She assists cancer patients with ﬁnancial issues, transportation needs, coordinating appointments, and providing emotional support. Terri is the Coordinator of Nanticoke’s National Cancer Survivor Day Celebration that is held each year on the ﬁrst Sunday in June. She also helps to organize Nanticoke’s Pink Ribbon Tea every October to honor and celebrate our breast cancer survivors. The annual “pink” tea party is a collaboration between Nanticoke Cancer Care Center, the Cancer Support Community, the American Cancer Society, and the Delaware Breast Cancer Coalition. Terri is very passionate about the work she does for cancer patients and their families.

Radiology Technologist, Nanticoke Diagnostic Imaging

Kelly Yoder‘s desire to help people led her to choose a career in the medical profession. Upon graduating from college, she started working with Nanticoke Health Services as a Radiologic Technologist in July of 1992. In 1995, Kelly was asked if she would be interested in learning mammography and stereotactic breast biopsies. “I said yes without hesitation, and have been very pleased with my decision,” says Kelly. “I have seen quite the change over the past 25 years transitioning from analog to digital and now to 3D tomosynthesis.” “Although no one looks forward to getting their mammograms, they do enjoy talking to us,” explains Kelly. “With patients coming year after year, you get to know them on a personal level. I have had patients say ’you saved my life’ and that does my heart good knowing I am making a difference.”

Terry Persolio, is the Senior Mammographer at Nanticoke Diagnostic Imaging in Seaford, Delaware. She constantly strives to make sure her department is offering the highest quality imaging possible for women in Sussex County. Terry began her rewarding twenty-eight year career with Nanticoke Health Services in 1989 in the employed physicians’ network. She earned her mammography certification in 1993 and obtained her quality management certification in 1999. Terry officially took over the Senior Mammographer position at Nanticoke in 2005. In Terry’s career, she has experienced three technology changes in mammography from analog, or film mammography, to digital mammography, and most recently to the 3D tomosynthesis. Terry possesses a unique combination of strong supervisory and patient care skills. In addition, she believes in building a long-lasting and trusting relationship with patients.

Lisa Schirtzinger, MSN, RN Cancer Screening Nurse Navigator

Lisa Schirtzinger is Nanticoke Health Services’ newest Cancer Screening Nurse Navigator. With 27 years experience in the nursing profession, she is able to meet the challenges of the position. Lisa started her career in nursing at Nanticoke Memorial Hospital. She took a break from nursing for a period of time to raise her three beautiful children. During that time, she pursued other areas including working with developmentally challenged individuals and education. Lisa returned to nursing eight years ago to work on Nanticoke’s Medical Surgical Unit (MSU). She continued to further her career as a Relief Charge Nurse and Administrative Nursing Supervisor. Four years ago she became the Clinical Educator for the Medical Surgical Units within the hospital. While working on MSU, she became certiﬁed to administer chemotherapy medications which started her passion to want to work within the oncology ﬁeld. “I am excited to be in this position to continue to teach and advocate for individuals.”

Deciding on which treatment to cover a normal window with can be difﬁcult enough but sometimes you run into an opening that requires a little more effort. Sliding glass & French doors can be troublesome, arches & eyebrows over windows along with ovals, circles, octagons, triangles, rhombuses, and trapezoids, are all out there and may be letting in too much sun! That long skinny window next to your front door that we call a sidelight can also throw a monkey wrench into the works. Besides letting in natural light and providing you with a look to see who is at the door, it can also sometimes let passersby have a clear look right into your home. Through the years we have installed many different types of shades and blinds onto these openings but the best thing we have ever done, for this, was to start putting shutters on them. Plantation shutters are the most versatile answer to these issues because they really are the only covering that has a treatment for everything mentioned. For sliders there are both bypass style and bi-fold style which now have the ability to be folded over to lie against the wall next to the slider instead of perpendicular to the wall as in the past. This greatly reduces the projection into the room while at the same time letting you completely open up the glass to the light and the view beyond it. It is an improvement over the bypass because while the bypass doesn’t invade into the room when opened it does always have at least one panel’s width covering the glass. For French doors we can design a shutter that ﬁts right over the glass. We attach the frame to the door and can even design the frame around the door handle for that stunning custom look. The panel can swing completely open so that you may clean the glass and for those instances when you need maximum light but most of the time you will just operate the louvers for light control and privacy. This covering is also very functional for a door as the entire covering is attached so that there is nothing to swing away from the glass as it moves. It’s a good choice for a door that will be used often.

Arches, eyebrows, and other specialty shapes such as ovals and triangles are another chance for shutters to standout. Almost always, in this category, a template is required. It doesn’t get much more customized than that and it shows. What you end up with is a gorgeous and unique piece that was made just for that opening. Fall Safety Tips By Regan Roberts Deck, CMI

We may have had what felt like one of the hottest summers ever, but fall is rapidly approaching and that means now is the time to start preparing for the change of seasons. Regular Performance Tune-Ups - like the ones included in many Service Agreements - keep you (and your HVAC company of choice) up-to-date on your equipment. In addition, periodic filter changes keep the air flowing through your home evenly, increasing your comfort and the efficiency of your unit. Finally, the simple act of thinking about your system during these checks and filter changes will help you notice if your comfort level starts dropping. I always recommend having your HVAC equipment checked for proper operation twice a year, but this service becomes even more important when your heating system is fuel burning. Fuel burning heating equipment produces carbon monoxide (CO), a colorless and odorless gas that, if released into your home, can cause headaches and nausea (often mistaken for flu-like symptoms) at low levels and, at high levels, can result in memory loss and even death. The effects of CO exposure can vary greatly from person to person depending on age, overall health and the concentration and length of exposure. But your HVAC system is not the only source of carbon monoxide production in your home: kerosene and gas space heaters; chimneys and furnaces; furnaces, gas water heaters, wood stoves, and fireplaces; gas stoves; generators and other gasoline powered equipment; automobile exhaust from attached garages; and tobacco smoke all release carbon monoxide. The U.S. Consumer Product Safety Commission (CPSC) recommends that every home have a carbon monoxide alarm. For full coverage of your home, you can add one carbon monoxide detector on each level of your home as well as additional detectors in sleeping areas. There are a variety of detectors on the market today from a hard-wired combination smoke/CO detector to a plug-in alternative. For the plug-in detector, all that is needed is an available outlet and the battery backup activates automatically should your power fail. So, during this change of seasons, while your heating system is being checked to ensure proper ventilation and operation, make sure your CO detectors are working properly and that everyone in your family knows the signs and symptoms of a carbon monoxide leak. Hello My name is Richard Lamb. I am a Delaware native,born and raised in Wilmington. Recently retired as a Captain from the Wilmington Fire Department with 35 years of service. The Fire department schedule allotted time off for a 2nd job. I began painting approximately 30 years ago. Parents brought us to the beach when we were very young. Got a taste and new I wanted to live here. Moved to Rehoboth 8 years ago and started up the business. We have now built up an extensive reference list and pride ourselves on quality workmanship. Two years ago we also started Lambs Roof & Exterior cleaning in order to prepare homes properly for our services. We now have our certification as an authorized professional. There are only three companies in the state that are certified. We can clean the dirty black streaks on the roof,remove mildew and mold from siding. We clean stucco and now have started cleaning cedar homes. Some cedar homes turn gray and that looks beachy,others turn black and that is dirt and mold. We can clean your home or business. Since then, C.M.Baker Photography has expanded and evolved just as the photography world has. From ﬁlm to digital, and weeks to develop to instantly printing photos on site, C.M.Baker Photography has many services to offer today’s client looking for photography services. Printing photos onsite started about 10 years ago. We were able to capture the shot, take the card out and print out a 5x7 or 4x6 about 45 minutes after the photo was taken. Boy, have things changed! For events, we now can take photos in front of a background or green screen with limitless choices, and have the photo print out in about 15 seconds. This is great for birthday parties, corporate events or fundraisers. We can even include your logo or text on the 4x6 or 5x7 image. With wedding packages, you will also receive 10, 8x10 photos printed onsite from earlier in your day that will be on display at your reception. Recently our onsite printing experiences have expanded even more as we now have two photo booths! We have a classic black, indoor photobooth that adds a fun experience for your guests to any event. Confucius Chinese Restaurant was opened in the spring of 2004 in downtown Rehoboth Beach. Shawn Xiong, the owner, had previously owned a Chinese restaurant in Wilmington prior to relocating to the beach area. Since then, Confucius has been named the best Chinese restaurant in the Delaware Today Magazine, Sussex County Woman and favorably mentioned in various newspaper and magazines. Wherever you go, you probably will find a Chinese eatery in every shopping center that offers pretty much the same items such as lo miens and chow miens. The Confucius menu is more focused on items you don’t usually find in those places. The name Confucius lets people know it is Chinese, but with a delicate, refined gourmet restaurant and not a fried rice and egg rolls fast food eatery like a McDonalds. There are different popular dishes for different customers depending on their individual personal taste. If you like spicy food, we offer dishes such as Hot Pepper Chicken, Cumin Beef or Kung Pao Shrimp. If you like fish, Salmon and Black Bean Sauce, Steam Halibut or Crispy Whole Black Sea Bass

are all excellent choices. If you are a ﬁrst time customer, they encourage you to tell your server what you do or do not like, and they will offer the right recommendation for you. Confucius believes that every customer deserves special attention, if you order something that you don’t like, they encourage you to send it back and try again AT NO COST. 15 of us were in Rehoboth Beach over the past weekend. We had decided to try Confucius based upon friends recommendations and Yelp reviews. They were very accommodating with the reservation and by providing a nice space, including a high chair. We started with assorted appetizers, including spring rolls, various dumplings, beef sticks and the like. Everything was delicious. So on to the main courses. Eggplant in black bean sauce, Singapore Rice Noodles, Hunan Beef, Chicken with Broccoli, 5 Spice Duck (they will warn you it is very hot-we asked for it to be on the mild side, and still had some heat, but we loved it!) Shrimp and scallops with walnuts (my favorite dish of the night), soft shell crab, chicken fries rice and a few other dishes that never made their way to my end of the table. Portions are large, but the food was so delicious we had very little left over. This is not your typical Chinese restaurant - everything is prepared to order, and they deserve all 5 stars. First time here, and it won’t be our last. Loved the seafood hot and sour soup; chicken hot and sour was excellent as well. Spicy shredded duck was under-spiced, but that was my fault for telling server I did not want it too hot, and she bent over backwards to make sure it was mild. Can’t wait to try other menu items. Our server was outstanding. Could not have been more attentive and accommodating. Co-owner Danielle was a pleasure to talk with as well.
