By Angela Caswell-Monack, DO I often get questions about when young women should start coming in for gynecologic care. I love this question because it means someone is thinking ahead. I recommend a first OB/ GYN visit anytime in the early teen years. Our governing body, American College of Obstetricians and Gynecologists, recommends establishing care with a women’s health provider between the ages of 1315. Though it may seem intimidating, this visit is intended to be a “getting to know each other” visit and an opportunity to talk about confidentiality, wellness, normal menstrual cycle, and to outline future gynecologic care. I go through family and personal medical history to find any red-flags such as history of blood clots, cancers, and bleeding disorders. We screen for depression and eating disorders. I talk about safe sex and contraception. I stress the importance of communication with trusted adults and also with one’s romantic partner (if there is one.) I am careful to support each young woman, and I talk about sexuality and sex while trying to make her feel empowered and have self-confidence. We review the menstrual cycle, normal physiology and assess individual needs. And, we talk about how to stay healthy. One of the things I want to promote is that an OB/
GYN can be another trusted adult who will care about your child. We try to be nonjudgmental, and we encourage our patients to talk to their parents. I know some parents fear that once their daughter enters the realm of OB/GYN, that she will be encouraged to have sex and that all innocence will be lost. This is not the case. There are laws in Delaware to protect her confidentiality, even from her parents (i.e. testing for sexually transmitted infections, pregnancy testing, or contraception.) But, we really try to encourage responsible behavior, selfrespect and self-esteem. Statistics show that one-third of high schoolers are sexually active, and it is important to arm our young women with real knowledge and access to information and care. She does not necessarily need to have an invasive exam until she is 21 years old. Personally, I work hard to keep the visits comfortable and limited to talking. I will do a pelvic exam if the conversation reveals a concern, but a pap test is not done until age 21. I instruct my patients to do self-collections in the office for some of the sexually transmitted infections. Usually, my patients are relieved and leave with a smile and new knowledge. This non-scary experience helps them come back regularly for follow-up care. Many young women see their primary care provider or pediatrician for all of their needs. This is usually perfectly great. For some

young women, it may be helpful to establish care with an OB/GYN if they want to talk about the more “uncomfortable” private concerns with someone different. Here are a few examples of when I would recommend a gynecology consult: • You are 15 years old and have not had a period. • Your periods were regular each month and then they stopped being regular. • Your period comes more often than every 21 days or less often than every 45 days.
• Your periods come 90 days or more apart. • Your periods last more than 7 days. • Your periods are so heavy that you have to change pads or tampons more than every 1-2 hours. • You have bad cramps that keep you from doing your regular activities and pain medicines don’t help. • You cannot get a tampon into the vagina. • You have an abnormal vaginal discharge (other than a clear-white) or a bad odor. • You have a discharge from your breasts or a new

With time, age and gravity, our facial structures tend to lose some of the youthful volume it once had. “Broken Heart Syndrome” Is a “broken heart” a true medical condition or is it just a figure of speech to describe someone’s emotional state after some great personal loss or stressful situation? It may come as a surprise to many people but the answer is both. This condition is also known as “stress-induced cardiomyopathy”, a rapid and severe heart muscle weakness, typically as a consequence of sudden emotional or physical stress: unexpected illness, injury or loss of a loved one. It can also be from a severe medical condition, surgery, an accident, intense fear, domestic violence, severe pain, financial loss, even public speaking or a surprise party. The exact cause is unknown, but it is clear that most of the affected individuals are postmenopausal women between the ages of 58 and 75 years, in about 90 percent of cases. The symptoms are very similar to those of a heart attack, with chest pain or pressure and shortness of breath being the most frequent ones. It is not completely understood why this condition affects mainly women, but most investigators believe that after menopause the decline in estrogen levels renders patients more susceptible.
It was first observed in Japanese women in the early 1990’s and the name of takotsubo cardiomyopathy was used originally. This name describes the characteristic appearance of the left ventricle (heart’s main pumping chamber) as seen in cardiac studies that resembles the traditional Japanese octopus trap. There is ample evidence to suggest that the massive release of certain hormones in the bloodstream, such as adrenaline and noradrenaline, as a natural response to stress, under certain conditions causes the heart muscle cells to stop working properly, especially in the lower part and the “tip” of the left ventricle. Stress-induced cardiomyopathy can be serious medical condition because it involves the sudden onset of congestive heart failure due to the heart muscle weakness, low blood pressure and potential shock or life threatening heart rhythm problems; it is, however, rarely a fatal illness. Furthermore, it is almost always reversible with most patients recovering completely within 2 to 4 weeks. The main differences between a heart attack and “broken heart syndrome” is

that heart attacks occur as a consequence of blockages or blood clots in the coronary vessels (heart vessels that supply blood flow and oxygen/ nutrients) to the heart muscle cells that cause those cells to die or to suffer permanent damage. In the stress cardiomyopathy patients, the coronary vessels are open and the blood flow is usually normal. It is the “stunning” of the heart muscle cells by adrenaline and other hormones that creates the muscle weakness and this process is, for the most part reversible. Another interesting finding in this broken heart syndrome
is that it does not tend to recur in the same patient. In other words, once somebody experiences this syndrome, even if other stressful events come along in their lives, the same phenomenon doesn’t tend to happen again. Learning to cope better with life stresses can prevent it in the first place. The management of this medical condition is similar to the treatment of congestive heart failure with medications such as diuretics, betablockers and ace-inhibitors as well as aspirin and in some cases blood thinners on a temporary basis. So yes, it is quite possible
to have a “broken heart” literally, as a true medical illness that is brought about by a heightened physiological response to stress, especially in women. The good news is that in almost every case it is a temporary condition and full recovery is the expected outcome. There is no way to differentiate this situation from a heart attack, before hand. Therefore, it is very important to seek immediate medical attention for symptoms of sudden chest pain or pressure and shortness of breath. It might be the case that your heart doctor could tell you, in the end: “Just a broken heart, already on the mend”.

These symptoms can include: • Low Back Pain • Pelvic Pain • Urinary incontinence (leakage of urine) • Constipation • Diastasis Recti (separation of abdominal muscles) • Pubic Symphysis Separation • Wrist and Hand Pain Frequently, women are told, “These symptoms are a normal part of pregnancy,” “Ignore it and it will go away once you give birth.” This response is not adequate and is not the case!!! All these issues are treatable through physical therapy by a women’s physical therapy specialist. Additionally, after birth, these problems may not resolve. For example, if a woman experiences incontinence during pregnancy, it is unlikely to spontaneously resolve on its own postpartum. Some minor leakage for 3-4 weeks after delivery is normal, but overall involuntary loss of urine is not normal and should be addressed by a physical therapist specially trained to treat pelvic floor muscle dysfunction. In France, women are automatically referred for postpartum physical therapy. In the United States, mothers typically receive one follow up visit to their referring practitioner. This one visit is not necessarily enough to recover from birth and to thoroughly assess a women’s return to her pre-pregnancy state. This is where pelvic health physical therapy can help! A pelvic health therapist is trained to assess the function of your pelvic floor muscles, check your abdomen for diastasis recti, and to assess any perineal or abdominal scars you may have from tearing, episiotomy, or cesarean birth. Physical therapists are fortunate to have the time to spend with patients to provide a lot of the education regarding changes that occur during pregnancy and after delivery. You can expect to spend 40 minutes to an hour at your appointment with your physical therapist. Your PT can help you learn safe lifting mechanics for your new baby, positioning for breastfeeding, talk to you about returning to intercourse, and what to expect in regards to your body’s changes after you’ve had a baby.

Delaware Eye Optical Like any retail company products change over time and Delaware Eye Optical has seen a change recently regarding the lenses we dispense. Throughout my career in the optical business we have fit and dispensed various lens designs by various manufactures. Manufactures of lenses are all hoping to be the lens of choice with opticians throughout the world and recently we have come full circle. Varilux was one of our most successful lens brands throughout the mid-eighties and through the 90’s when the progressive lens market really took off. Early on as independent opticians in an eye doctor’s world progressive lenses were not well received especially early on when opticians were asked to increase the add or bifocal power by +.25 diopters. Doctors perceived this as opticians over stepping their bounds. Those days are well behind us now and we have been working with doctors for over twenty-seven years. Varilux is back with us and improved in many ways. The design of their upper tier lenses is much improved by adding new technologies to their digital design manufacturing as well as material additions like Trivex. Caring for the memory impaired… Helpful Hints By Patrice Schellinger, LPN, Operations Specialist Taking care of someone with Alzheimer’s or dementia can be very difficult. Here at Always Best Care, we strive to educate people in how to better take care of loved ones and yourself when dealing with this disease. We understand that it may make the caregiver feel overwhelmed and isolated. We want you to know that these are very normal feelings. Here are some tips to help you better understand the disease and how to manage it as best you can. Typical Behaviors • It is important for people with memory impairment to maintain as much independence as possible. This will help their overall wellbeing and attitude. • Expect tasks to take longer. Short-term memory is greatly affected in this disease. It is normal for them to forget what they were doing in the middle of performing a task. Just be sure to redirect them back to the task at hand. • Aggression and agitation are a normal reaction with these diseases. They may curse, scream, insult and become violent. You need to know this is not your fault and you aren’t doing anything wrong. They aren’t doing it on purpose. Remain calm, don’t argue and try and redirect them. Tips • Establishing a routine and sticking to it can reduce frustrations. It also helps them feel independent and in control, so you can experience less agitation. For example, showering upon waking up and then going to eat breakfast. If you try and switch up the routine, it may confuse them and result in negative attitude changes. • Alzheimer’s makes it hard to focus. It is important to reduce distractions. For example, if you are sitting down to eat dinner, make sure you turn off the tv or radio so that they may focus on just eating. • Stay away from open ended questions. Always try and ask a question that would require a yes or no answer. If you are unable to do so, ask a question giving two choices. For example, “would you like scrambled eggs or oatmeal for breakfast?” • Limit background noise. Loud, abrupt noises can cause agitation. Soft tones and a quiet environment are best to keep them relaxed. • One step instructions. When assigning a task for them to complete, try using one step instructions. For example, don’t say “Get dressed.” Instead, instruct them step by step. “Put your socks on.” And when they have completed that task, instruct the next step. “Put your shoes on.” Maintain a safe environment in your home. • Avoid extension cords, area rugs and clutter to avoid falls. Make sure there are nonskid mats in showers/tubs. Use locks. Make sure you lock away medicine, fire arms and anything potentially dangerous. Take fire safety precautions. Make sure you hide lighters and matches. Keep fire extinguishers in an accessible location and make sure smoke detectors have batteries. Make sure electrical outlets that aren’t being used are covered with appropriate hardware. Music therapy has shown to be extremely helpful. Music that was popular when a person was between the ages of 18 to 25 has proven to be most effective in promoting positive responses. Here are some examples of its uses: Help awaken memories, Reduces agitation and improves behavioral issues, Motivates them to participate in exercise, Relaxing music can provide comfort and is soothing to help calm their mind. Don’t get burnt out. No one ever said it was easy taking care of someone with Alzheimer’s disease. It can be physically and mentally draining. If you feel anxious, exhausted, or depressed make sure you take appropriate measures to take care of yourself. How does CoolSculpting Work? The technology behind CoolSculpting was developed by Harvard scientists who noted that fat was destroyed by cold under certain conditions. The practice of medicine has changed and will continue to change. With the advent of electronic medical records (EMR), physicians and providers have been asked to use EMR in order to document and keep track of the patients visits. This has taken away from the providerpatient connection during visits. The time spent with patients has been gradually decreasing due to administrative demands placed on providers by Insurance companies and Medicare. As a response to these changes, we at Delaware Arthritis believe that patient education, community support and human connection between providers and the community can greatly impact a patient’s outcome. We have incorporated modules of health education (in the areas of selfcare, movement, mindfulness and nutrition) as part of the classroom work. During group visits, each patient will be seen by a provider individually while also participating in the group didactic and experencial work. Who can participate in a group appointment? Anyone can participate once they have become an established patient in our practice. A patient is eligible to participate in a group appointment
series if they commit to themselves and to the group work that will be covered.
In order to become an established patient, you will need to be seen by one of the Delaware Arthritis providers prior to be beginning of the introduction class/visit. The groups are organized according to diagnosis. For example, you may participate in an inflammation group (patients diagnosed with Rheumatoid Arthritis, Psoriasis, Psoriatic Arthritis) or other diagnosis such as Osteoarthritis, Osteoporosis or Fibromyalgia. How often do you meet and what is the format? The group meets monthly for one, 2 hour visit. We discuss a predetermined topic presented by guest speakers, demonstrations may be offered and participation of the attendees is required. The group appointment setting is interactive, educational, collaborative and FUN! Why would you participate on a group appointment? You should join a group appointment if you want to learn more about your diagnosis and ways in which you can help yourself. In most cases, patients that have chronic conditions feel powerless and isolated. Really Understanding Women’s Health & Medical Marijuana Mark Lally, Delaware’s medical marijuana pioneer is CEO of First State Compassion. The news is spreading. Women and medical marijuana use Did you know that more women than men are likely to suffer from diseases such as Alzheimer’s Disease and arthritis? Medical marijuana treats these and other conditions that are prevalent among women. So, it’s no surprise that recent surveys show women are steadily narrowing the gender gap of patients using cannabis as part of their health and wellness plan. Here are a few of the common diseases that qualify for medical marijuana: 1. Alzheimer’s. Almost two-thirds of Americans with Alzheimer’s Disease are women. ut for those suffering from the ailment, it’s important to know about the relationship between medical cannabis and Alzheimer’s. Cannabinoids and other components of medical marijuana can offer anti-inflammatory properties to treat symptoms, and recent research by the Salk Institute for Biological Studies confirms that cannabis can decrease amyloid beta accumulation in the body’s cells, which is critical for slowing progression of the disease.
The State regulates the process of qualifying for a Medical Marijuana card; you must meet the following qualifications: • 18 years of age or older, or a pediatric patient with parental consent • Diagnosed with a qualifying medical condition – the list of conditions can be found on our website • Have a Physician Certification from a Delaware-licensed MD or DO • Delaware resident with proof of residency (DE Driver’s License or DE State ID)

Cancer is the second most common threat to a woman’s health, accounting for 22 percent of female deaths in the United States each year. La Red Health Center To Provide Health Care Services at a 2nd CHEER Activity Center! Partnership Brings More Health Care Options To Sussex County By Carolyn M. O’Neal CHEER Community Relations Director August 14 marked the one-year anniversary of La Red Health Center providing health care services at CHEER’s Ocean View Activity Center. Coming soon will be the opening of La Red Health Center’s 2nd Family Practice site at a CHEER Activity Center. This one will be established in the Pelican Cove CHEER Activity Center at Long Neck located at 26089 Shoppes at Long Neck. CHEER and La Red Health Center have teamed up to improve access to adult and senior healthcare services throughout Sussex County. A ribbon cutting ceremony will be held at Long Neck to mark the continued collaboration. Watch local media and the CHEER website www.cheerde.com for the exact date. “Both of our agencies are about bettering the lives of the people of Sussex County,” said Brian Olson, La Red Health Center Chief Executive Officer. “We are much stronger working together than independently.” Sussex County is federally designated as a medically underserved area, a low-income health, mental health, and dental professional shortage area. The vast majority of the people moving to Sussex County are senior citizens who are retiring and relocating to live here. “A large percentage of our population (seniors), don’t have medical care,” said Barbara Williams, a CHEER member at Long Neck. “I think this will be good for preventative care.” CHEER member Mary McCurnin is one of those retirees who moved to Ocean View from Wilmington in 2003. Mary had owned a cleaning business and her husband, Richard Michael (Mike) worked for the railroad for 40 years. They had vacationed in Fenwick Island for a number of years and enjoyed the beach life. Mary joined CHEER in January 2011 and likes the activities offered at the Ocean View center. She usually attends at least three times a week and volunteers to help in several areas. She especially enjoys working in the gym at the center. Mary was a physical education teacher before her children were born and volunteers to set up personal training programs for seniors at CHEER. Mary says when she first moved to Sussex County she found there were not a lot of options for medical care in the area. She has to have an injection once a month and had to travel to Wilmington to get it. Now, she can visit La Red Health Center at her CHEER center and the La Red Health Center staff can give it to her. She likes that convenience. “I have family in Wilmington I could visit when I went to the doctor,” Mary explained, “but now I can get the shot at

The staff is wonderful too.” The services provided by La Red Health Center include, health screenings, nutrition counseling coordinated with CHEER’s nutrition program, acute illness treatment, physical exams, and vaccine and prescription administration. La Red Health Center staff are at the Ocean View site on Thursdays. Walk-ins are welcome but appointments have priority. Patients must be 12 years of age and over and do not have to be a CHEER Member. The days and hours of operation that La Red Health Center will provide health care services at the CHEER Long Neck Activity Center will be announced soon. Nancy Emplit, a CHEER member at Long Neck, is looking forward to the convenience of having medical care right at her activity center. “I think this will be productive for us (the members),” said Nancy. “I will most likely take advantage of it.” The goal of the collaboration is to establish primary care for an underserved area. CHEER’s Ocean View Center Director Yolanda Gallego also transplanted to Sussex County in 2008 from the Shenandoah Valley area of Virginia. She agrees that health care options are not good in Sussex County. “Health care was great in Virginia,” Yolanda said. “But there are many more retirees here and it is challenging to find medical help in this area. La Red Health Center and CHEER are really helping the community by this partnership.”

Helping Children Cope with Loss — Before it Happens
When a loved one is in decline, everyone in that person’s family and their circle of friends is affected—whether it is their six-year-old grandson or their ninety-six-year-old college buddy. And each person needs the space and the opportunity to process the grief of their pending loss in their own way. But quite often, we shield our children from the details of a loved one’s condition in the name of protecting the child— only to make things more difficult for them. Children can intuitively sense that something is amiss. It’s already stressful enough watching a loved one die. But the experience can become even more confusing for a child without the chance to understand what’s going on and to talk to someone about it. Children experience grief, too. By the time they’re 18, one in five children will lose someone close to them. There’s a common misconception that children don’t grieve, when in fact, children grieve in many of the same ways adults do. It’s just that children’s developmental way of handling the grief might manifest itself differently. For children, grief often comes in waves. After all, their attention spans are different from an adult’s. It’s normal for a child to go from talking about death to asking to play outside in the span of 20 minutes.
Children may also ask blunt questions about exactly what’s happening—because they expect direct answers. While this often makes adults uncomfortable, such candidness has nothing to do with a lack of sensitivity. It’s a very age-appropriate response to a child’s way of processing what’s going on.

Sometimes that means opening the door for the patient and the child to spend more time together, or enabling the child to have important conversations with the loved one who is dying.
As children watch a loved one decline, their worries may include things such as: When will my loved one die? Will I be there when they die? Will something happen to my other parent? Who will take care of me?
At Delaware Hospice, our counselors are adept at helping children open up and understand that their feelings are normal, whether by asking them encouraging questions or engaging them in an art project or a game.
Leaving such questions unanswered can often do more harm than good. Children dealing with grief are at risk of anxiety, isolation and depression, which can lead to things like poor school attendance, behavioral problems, drug use or even suicide.
Anticipatory grief support is one of the primary services offered through Delaware Hospice’s nationally recognized New Hope program, which helps children and young people ages 6-17 sort through the maze of emotions they’re experiencing before and after their loved one dies. In addition to anticipatory grief support and bereavement support, New Hope also offers a school support group program and a week-long camp for children who have experienced loss.
Grief support for children in anticipation of a loss helps kids understand and process what’s to come. Children need someone to walk this difficult journey with them just as much as adults do. Through anticipatory grief support, children have the opportunity to ask questions and to talk with family members and a professional about what’s happening around them. It allows them to confirm what they can see with their own eyes—that their loved one is dying. It enables them to hear concrete information about their loved one’s condition—and to know that their loved one’s illness is not their fault. Anticipatory grief support also helps a child understand that dying is a part of life, while providing time for the child to start processing what that means and what they’re feeling. Anticipatory grief support is the first step on the child’s grief journey. After a loss, a child will still have much to process. Anticipatory grief counseling can ease that transition by establishing a trusting relationship with a professional in advance, giving the child a sense of stability as they continue on their grief journey with a familiar face. As children move through different developmental stages, they’ll continue to have feelings of grief. For example, a child who has lost a loved one at age four will often boomerang back to grief when they turn age thirteen. Even though the loss has happened over nine years ago, as the child matures, they
gain more insight into their loss and increase their ability to engage in abstract thinking. By helping children start processing their loss as early as possible, we can help children move forward with their grief in the healthiest way possible, giving them the tools

MEMORY CARE: Where do You Turn for Help?

t’s never easy deciding that a loved one can no longer live independently due to Alzheimer’s disease, dementia or other forms of memory impairment. There may come a time when escalating needs are unfortunately more than you and your family can handle. Sudden mood swings, disorientation, delusions and even aggressive behaviors are just some of the signs that indicate that your loved one and you, their caregiver, need to seek help.

The holidays are upon us, and soon our screens will be filled with all the feel-good holiday classics. Loving families, festive feasting, unbridled fun and the hap-happiest season of all, right? Not exactly. To reference just a few, George Bailey is overcome with grief to the point of considering suicide in “It’s a Wonderful Life.” Clark Griswold deals with so much misfortune to the point of considering homicide in “Christmas Vacation.” And Kevin McCallister from “Home Alone” gets introduced to severe abandonment issues at an early age. When it comes to holiday cinema, art really does imitate life. According to a survey by Healthline, a consumer health firm based in San Francisco, 62 percent of respondents described their stress level as “very or somewhat” elevated during the holidays, while only 10 percent reported no stress during the season. In another study by the American Psychological Association, nearly a quarter of Americans reported feeling “extreme stress” come holiday time, and statistics show that up to 69 percent of people are stressed by the feeling of having a “lack of time,” 69 percent by perceiving a “lack of money,” and 51 percent are stressed about the “pressure to give or get gifts.” Perhaps most alarmingly, 45 percent of Americans would prefer to skip Christmas altogether, according to a survey from Think Finance. It’s an all-too-common problem, and one that doesn’t discriminate. No matter geographic location, income level, gender, ethnicity or relationship status, people can feel sad, depressed, anxious, lonely, overwhelmed, bitter or worse during times that are typically billed as joyous and carefree. The stats above identify some of the reasons why – time pressures, financial pressures, gifting pressures – and there are many more. Writer Maud Purcell described several of those reasons in her recent article on holiday stress: • Pressure to feel merry: Do you feel joyous when holiday decorations go up and store
windows fill with gifts? If you don’t, take comfort in knowing that you are not alone. The disparity between how you actually feel and what you think you are supposed to feel can cause you guilt and confusion. • Remembrances of holidays past: Consciously or unconsciously, you have a mental record of previous holidays. Your mood may be contaminated by the specter of sad holidays past. If your current life circumstances are unhappy, however, you may long for the happy holidays you once enjoyed. • Reminders of loved ones lost: Holidays are a time for reflection. All too often your thoughts turn to beloved family members and friends who have passed away. • Loneliness: Holidays can be dreadfully lonely if you don’t have a significant other. Additionally, separation from family members (emotional or geographic) can be particularly painful at this time of year. • In search of sunlight: Many people are adversely impacted by the relative loss of sunlight they experience during the winter months. This phenomenon even has a name: seasonal affective disorder or SAD. Of course, when it comes to sunlight, our team at SUN Behavioral Health is uniquely qualified to help. We frequently encounter these tough emotional triggers in our patients as the fall and winter months approach. Here are a few time-tested strategies we recommend:

A Permanent Fix for Chronic Sinus Sufferers

Sinus disease, including recurrent infection requiring antibiotic along with chronic congestion, pressure and post nasal drip year round, is the most common health problem in the United States; affecting 1 in 5 patients per year. Most sinusitis patients suffer facial pain, pressure, headache, nasal discharge, constant post nasal drip with discolored mucous and fatigue. Sinusitis has significant effects on a patient’s quality of life according to quality of life studies. Overall, the chronic, recurrent sinusitis patient’s quality of life is WORSE than that of patients with chronic heart failure, chronic obstructive pulmonary disease and Parkinson’s disease. Historically, chronic sinusitis patients were limited to two treatment options: continued medical therapy, including antibiotics and topical nasal steroids, or conventional surgery such as Endoscopic Sinus Surgery (ESS). This all changed approximately 8 years ago with the introduction of an innovative technology called Balloon Sinuplasty. The Balloon Sinuplasty devices, originally designed developed, and commercialized by Acclarent, utilize a small, flexible balloon catheter that is placed through the nose into the blocked sinus passageway. The balloon is then inflated to gently restructure and open the sinus passageway, restoring the normal sinus drainage and function. Since the beginning of 2011 Balloon Sinuplasty has been offered as an in-office procedure without the need for general anesthesia. This innovative office procedure typically allows patients to return to normal activity within 48 hours. Acclarent’s Balloon Sinuplasty devices are safe and effective and have been used in more than 375,000 in-office procedures. Locally, Dr. James R. VA Study Finds Chiropractic Linked to Lower Opiate Usage Dr. Tracy Hudson

Dr. Hudson is a ’95 Cape Henlopen High School graduate and continued her educational career at the University of North Carolina at Wilmington where she completed her Bachelor’s degree in Physical Education. While attending UNCW, Dr. Hudson suffered an injury playing softball and she sought treatment with a local chiropractor. After a few adjustments, Dr. Hudson was back on the field. Now being a true believer, she decided to further her education in the natural healing art. She attended Life University in Atlanta, GA for 3 years, and then transferred to Texas Chiropractic College in Houston. After graduating in 2004, she moved to Clearwater, FL, where she was in private practice for 8 years. During that time, she also became a Fellow of the International Academy of Medical Acupuncture. She has now moved back to her home town to share her love of chiropractic with our community.
We’re all aware of the serious problem in the US with opiate addiction. According to the National Institute on Drug Abuse, 115 Americans die from opiate overdoses each day. Many people who become addicted to opiates are first introduced to them by doctors who prescribe them for chronic pain. A recent study in the American Journal of Psychiatry found that 60% of opioid overdoses first used these drugs when given a prescription for chronic, non-cancer pain: 59% of those patients were prescribed painkillers for chronic back pain and 24% were for chronic headache. In summary: 36% of people who died from opioid overdoses were first given a narcotic because they had back pain. Chiropractic: Working with the Root Cause of Pain Chiropractic takes a different approach to pain by working to help the body repair the root cause of the problem rather than simply masking the symptoms, like opiates do. Remember: painkillers don’t repair injuries or damaged tissue; they simply stop the brain from processing pain, leaving the underlying problem. If the problem isn’t treated and the normal function isn’t restored to the body, the pain will return. Numerous studies have found that chiropractic care is equally (or even more) effective than medical care for a variety of pain conditions, including back pain, sciatica, headache, and scoliosis. And since

chiropractors don’t prescribe drugs or perform surgery, patients who get adjustments don’t have to worry about the negative side effects that come with these treatments…including addiction. Even the American Medical Association has acknowledged that chiropractic adjustments should be a course of treatment before surgery is considered for back pain. The US Department of Veterans Affairs has also seen the benefits of chiropractic care in the treatment of pain and, over the last few years, dramatically increased the utilization of chiropractic for veterans. Chiropractic Patients Use Fewer Opiates Now a new study1 from the VA looked at the relationship between chiropractic care and opioid consumption in returning veterans. In this study, the researchers looked at the health records of 14,000 individuals who had received at least one chiropractic adjustment. The authors found: • One third of these patients were prescribed an opioid within six months of their chiropractic visit. • Patients were more likely to be prescribed an opiate if they had moderate to severe pain. • They were also more likely to receive an opiate if they had PTSD or depression. • The chiropractic patients, though, had a significantly lower usage of opiates after their chiropractic visits compared to non-chiropractic patients.
This is not the first study to show that chiropractic patients are less likely to use opiates. • A 2016 study2 found that in areas where there are more chiropractors per capita, younger, disabled Medicare beneficiaries were less likely to obtain an opioid prescription. • Another 2018 study3 found that chiropractic patients had a 55% lower chance of using opioids than did medical patients. It’s clear from our public health crisis that we need healthy, non-addictive solutions to chronic pain in the United States. Many people with pain are prescribed opiates when there are solutions that are just as effective and safer than drugs. According to the literature, it seems that chiropractic can play an important role in helping these patients. Medical References 1. Dr. Fabricio Alarcon MD, FACP
Dr. Fabricio Alarcon completed his Internal Medicine/Primary Care Residency training at The Cleveland Clinic Foundation in Cleveland, Ohio in 1998. He was elected Best Senior Medical Resident in 1997. He was subsequently elected Chief Medical Resident and served as such from 1998 to 1999. Dr. Alarcon became a diplomat of the American Board of Internal Medicine in 1998 and he was subsequently honored as a Fellow of the American College of Physicians. Dr. Alarcon moved to Delaware in 1999, where he started his primary care practice. In 2012 he became board certified in Obesity Medicine by the American Board of Obesity Medicine, and he scored in the top 10% in the nation in his Obesity Medicine Board exam. He is the founder and owner of Atlantic Family Physicians, a primary care practice. During his professional practice, he has dedicated his career to address serious problems that affect our community. This is reflected in other positions he holds. He is the founder and owner of Healthy Outcomes Medical Weight Loss Center that provides a comprehensive approach to our obesity crisis via proper nutrition and healthy lifestyle modifications; he is the Medical Director of La Red Health Center that provides access to full range of primary care, behavioral health, prenatal, and dental services to uninsured and under-insured patients; and he is currently pursuing training and board certification in Addiction Medicine by the American Board of Preventive Medicine. CEO & President of Delaware Hospice For over three decades, Susan Lloyd has provided national, regional, and state leadership and support within the hospice community. As President and CEO of Delaware Hospice, she has upheld the organization’s mission while expanding its services, from one program in 1987 to ten programs and services in 2018. As the leading nonprofit hospice agency, Susan has developed the service area from New Castle County to include Kent and Sussex Counties along with Chester and Delaware Counties in Pennsylvania. Susan is currently serving on the National Board of the Hospice and Palliative Care Organization’s Hospice Action Network. She is also a member of the Public Policy Committee. Locally, Susan has led the way for Delaware in hospice care. Susan was the co-founder of the Delaware End of Life Coalition (now Delaware Quality of Life Coalition) and the Delaware Cancer Consortium. She is past president of the Delaware Association of Home and Community Care and Delaware HIV Consortium. Susan has held several national rolls including serving on the National Hospice and Palliative Care Organization’s Board of Directors as the Northeast Geographic Area Director. Susan received her undergraduate Nursing Degree from Bloomsburg University, and her Master’s Degree in Nursing from Villanova University. She enjoys running, Pilates, and spending time with her golden retriever, Maverick.
Amy Wood
Chief Executive Officer, SUN Behavioral Health Delaware Amy Wood has nearly three decades of senior leadership experience and is a recognized expert in mental health and substance use disorders. She has overseen the development of new psychiatric hospitals, launched startups for partial hospitalization and intensive outpatient programs, and led quality and risk management assessments to ensure the highest standards of patient safety and quality of care. Wood currently serves as Chief Executive Officer for SUN Behavioral Health Delaware, after joining the organization in 2017 as Vice President. As CEO, she leads a team of professionals committed to solving the unmet behavioral health needs that exist in neighborhoods throughout Delaware and across the region. Wood previously served as Corporate Director of Intake and Utilization Management at Springstone. Prior to that, she held leadership positions during a 23-year career with Laurel Ridge Hospital in San Antonio, Texas. While there, she opened four partial hospitalization and intensive outpatient facilities in Central Texas and helped launch a 72-bed, inpatient program for active duty service members dealing with PTSD and substance use rehabilitation. A Licensed Professional Counselor, Wood earned a bachelor’s degree in psychology from Texas A&M and a master’s degree in education with a concentration in counseling from the University of Texas – San Antonio. She serves on the Board of Directors for the Georgetown Chamber of Commerce and is a member of the Access & Treatment Committee for Delaware Lieutenant Governor Bethany Hall-Long’s Behavioral Health Consortium. Penny Short, RN, BSN Chief Operations Office and Chief Nursing Officer Nanticoke Health Services
Penny Short, RN, BSN, joined Nanticoke Memorial Hospital in 1983 and has over 30 years of experience in the health care field. She has been Chief Nursing Officer since 2008 and in 2012 was also named Chief Operating Officer for Nanticoke Health Services. Ms. Short led the hospital to accreditation as the first Primary Stroke Center in Delaware, Level III Trauma Center, ACOS Accredited Cancer Center, as well as being recognized by Healthgrades for Critical Care, Pulmonology, and Emergency Medicine and recent recognition by the AHA GWTG for both stroke and hear failure. She takes special pride in Nanticoke being recognized by CMS as the first 4-Star hospital in Delaware for both the patient experience and quality and the recent acknowledgement from Healthgrades for Patient Safety and Patient Experience. Under her direction, Nanticoke has fully implemented a robust Cerner EMR allowing them to recently receive Meaningful Use II, HIMSS Level 6 certification, and Hospital Most Wired status. Ms. Short serves on the Board of the La Esperanza and the Sussex County Montessori School. She is a member of the American Organization for Nurse Executives and the American College of Healthcare Executives. In 2012, Ms. Short was honored with the Distinguished Alumni award from Delaware Technical and Community College-Owens Campus. She was also recognized in Delaware Today as Top Women in Business 2015.
Henry Childers, IV, MD, FAAO
Henry Childers, IV, MD, FAAO was a professional sailor and member of the America’s Cup winning team, before completing his medical training at the University of Pennsylvania. He specializes cardio-thoracic surgery and is a former cardiothoracic surgery department chairman. Sports injuries and a chronic degenerative shoulder condition led to my life-changing medical decision. After years of chronic pain I had to make a decision, “Do I have my shoulder and knee replaced as multiple doctors had recommended? Is there a better way.” I began investigating non-surgical treatments. Fortunately, I choose the non-surgical approach. This not only led to my physical recovery but also to an academic interest in “cellular medicine” an integrative approach to treatment. This discipline employs innovative and novel techniques including Frequency Specific Microcurrent and Ortho-Mitochondrial Medicine using ozone, prolozone, platelet rich plasma and stem cells to heal and regenerate tissues, reduce inflammation, and detoxifying cells. After experiencing the amazing result on my shoulder and knees I realized that non-surgical treatments for most skeletalmuscular conditions have come of age. They are used throughout the world, especially in Europe and play an essential part in the treatment of professional athletes. But despite ample clarification in leading scientific and medical research, these treatments are not yet mainstream in the United States.

Do I need to purchase a “women’s” The Perfect Christmas Gift. bike? The theory of “one size fits all” is convenient for many things. They stretch, cling, sag or bag depending on your size and shape. Somehow we accept the differences. However, the same theory does not work so well when selecting a bicycle! Or does it? Saddle a little low or high...oh well, back pain is normal...right?! Women have specific needs when it comes to bicycle sizing and performance. So much so, that Trek has researched and adapted Women’s Specific Design (WSD) technology. Trek was the first major bicycle company to develop a full line of bikes designed specifically for women. Here’s the reality: every woman is unique. Women want different things from a bike. Veterinary Services Peninsula Veterinary Services is a full-service, AAHA-accredited hospital located in Long Neck, Delaware. Owner Dr. April Reid moved from Brooklyn, New York in 2015 to start her own practice in this area after spending summers in Rehoboth growing up Dr. Reid is a graduate of Ross University School of Veterinary Medicine. She previously owned a thoroughbred racetrack practice for 14 years before making the move to small animal practice. Dr. Reid works closely with several local rescue organizations. She is currently pursuing board certification in veterinary practice as well as finishing an MBA degree. In her spare time, Dr. Reid enjoys being with her two Great Danes, Ambrose and Amelia, and her three cats.
Winter Pet Tips Winter is just around the corner and preparation is key with the unpredictable weather. For pet owners, there are a few additional things to keep in mind. Holiday Food and Decorations Black Friday is the busiest day of the year at most veterinary hospitals. The combination of rich foods, extra guests, and the holiday spirit of sharing are the perfect storm for our pets to be fed things they shouldn’t. This invariably causes gastric upset and in the most severe cases, pancreatitis. The recommendation is to not feed pets ANY table food, but especially fatty foods, garlic, onion, raisins, grapes, turkey skin, or bones. A little white meat turkey is usually fine for all but the pets with the most sensitive stomachs. Guests should check with their hosts to make sure it is okay before feeding table scraps. Pets should also be considered when planning and decorating for the holidays. Extra lights, low-hanging ornaments, tinsel and candles are all risky for curious cats and dogs. The Great Outdoors A lot of pets love the colder weather! However, don’t assume because they have fur coats that they are fine in frigid temperatures. Larger dogs and those with thicker coats typically do fine but puppies, older pets and small dogs are more at risk. Frostbite is a serious issue in the winter. Snow and ice between paw pads is painful and all pets should have their feet, stomach and
legs checked and dried after being outside. This is especially important when ice-melting products are used. It is not recommended that any pets be left outdoors. It is a state law in Delaware that anytime the temperature drops below 35 degrees, shelter must be provided for outdoor dogs. When there are hazardous weather warnings from the National Weather Service, it is illegal to leave dogs outdoors. Random Things to Consider Stray cats get cold too. They tend to seek warmth on car engines and wheel wells. Checking under the hood or honking the horn helps move them out of harm’s way. Speaking of cars, it is no safer to leave a pet in a freezing car than a hot car. Unless someone will be staying in the vehicle with the pet, it is safer to leave him or her at home.
All pets should be microchipped as a basic rule anytime of the year. In the winter, it gets dark much earlier and it is important to remember to have reflective gear on both the pet and the owner. Finally, remember your pets when planning an emergency kit. Pet food, water and medications should be included in your kit.
And the Great Indoors Unless pets spend a great deal of time outdoors, it is not necessary to increase the amount of calories they are fed. Indoor pets tend to gain weight in the winter because of this common fallacy. Drier air inside homes may cause pets’ skin, noses and throats to be drier. Humidifiers and fatty acid supplementation can help. Typically, it is not necessary to bathe pets as much in the cold weather as this can also cause dry skin by removing the natural oils of the coat. Heat sources attract pets and they should be monitored closely when using space heaters and fireplaces. Older pets may show more arthritic pain in the winter due to the drop in air pressure. Pets having trouble getting up or down, using the stairs or jumping should be seen by a veterinarian and evaluated for pain. There are a variety of antiinflammatory medications as well as nutraceuticals that can be given to pets to make them more comfortable.

‘Tis The Season For Safe Driving Susan D’Alonzo Ament is a partner with Morris James LLP and has practiced for 35 years representing personal injury victims. Over the years, Mrs. Ament has found that some women don’t understand the crucial need to have adequate automobile insurance coverage in the event of a serious accident. Susan Ament educates women on these needs every day in her practice and in seminars that she presents throughout the year. The next few months can bring with them some of the most challenging driving conditions in Delaware. Whether you are traveling locally or planning a trip to visit friends or family out of state this holiday season, it’s important to take some extra precautions when driving during the holiday season as well as into the winter months. To best avoid unpleasant situations, follow these safety suggestions: Perform routine maintenance. One of the easiest ways to avoid car mishaps is to drive a safe vehicle. Check and rotate your tires, get a tune-up, and have your oil changed regularly - at least every six months. Keeping up with these tasks ensures that your vehicle will be in top driving condition for Delaware’s oftentreacherous winter weather. Prepare for inclement weather. Due to the unpredictability of East Coast weather conditions, it’s always a good idea to keep an emergency kit in your vehicle. Emergency kits include items that can help signal others in times of distress, enable you to make minor repairs, or keep you protected from excessive temperatures while stalled. These kits can be purchased for as little as $30.00 or you can make your own. Some items that you might want to include are blankets, water, road flares, and tools. For more ideas on what to include in your vehicle emergency kit, speak to your auto insurance agent or visit The Red Cross online.

Sadly, every day in our law practice, we see victims seriously injured in automobile accidents due to another person’s negligence or reckless driving. Be sure to follow these guidelines to protect yourself and others during one of the most wonderful times of the year. WHAT TO DO IF INVOLVED IN A MOTOR VEHICLE COLLISION 1. Stop your Vehicle. Never drive away from the scene of a collision, even if it’s a minor collision. The vehicles should remain where they are unless they interfere with traffic. Turn your hazard lights on, and set out cones or other warning devices if you have them. 2. Call 911 or the Police. Even if there are no serious injuries, call to report the collision. An officer will be sent to file necessary reports and exchange information, and will call for medical personnel if needed for injuries. 3. Take Photos. Use your camera or phone to photograph the collision scene, document damage to the vehicles, and depict how the collision took place.
Drive defensively. Just as the old adage goes, sometimes the best offense is a good defense. When driving, be on the lookout for erratic drivers, slippery roads, snowdrifts, and other unusual conditions. Never drive at speeds that would prevent you from taking necessary corrective actions, particularly in the evening when hazards are more difficult to see. A little extra vigilance and a lot of common sense go far in keeping everyone safe during the holiday season and throughout the winter. If we all take these simple protective measures, we

4. Retrieve Relevant Information. Record the names and contact information of the other drivers or witnesses; license plate numbers; the make, the model and the colors of other vehicles; insurance information; and the exact location of the incident; 5. File a Collision Report with the Police. In some circumstances, the police will not come to the scene of a collision. If this happens, you should still contact the police to report the collision. A police report may help you make a claim with the insurance companies. 6. Contact your Insurance Company. Report the collision to your insurance company - even if the collision was not your fault. 7. Do not give a Recorded Statement over the Telephone. Do not give a recorded statement to the other driver’s insurance adjuster without an attorney present. 8. Seek Medical Attention. Often injuries are not readily apparent in the hours, or even days, following a collision. Unless you are absolutely sure you were not injured, you should be examined by your family doctor or emergency medical professionals.

9. Keep a File. Keep a file with all of the relevant information regarding the collision in one location for easy reference. 10. Social Media. Refrain from using any social media, as this may create evidence which can be used in Court in the future. 11. Know your Automobile Insurance Policy. The insurance process will be much easier if you know and understand your policy benefits and coverages before a collision. Never wait until after an incident to learn this information. For example, do you know if you have a deductible, rental car coverage or enough wage loss coverage? If not, call your insurance agent or insurance company directly and find out! By: Jeanine O’Donnell With the holidays upon us I always struggle with the comments I hear this time of year. “I just have to get Susie/Johnny the game they want-even though I don’t have the money.” “I just got out of last year’s debt-- and here we are again!” “I just don’t know what to get these kids-they get everything all year long anyway!” I think this season brings with it many stressors-and while we all want to give our loved ones a token of some sort to “show our love” I always like to recommend some other things that may be a bit more valuable. Parents and partners, give each other the gift of a deep conversation. This conversation should be about protecting each other and your children if something were to go wrong. I can’t help but recall a comment that one of the passengers who was on the cruise ship that recently caught fire said—“it seemed like the crew wasn’t really prepared for what happened.” Are any of us really thinking that something could go horribly wrong everyday when we get up? Of course not—or who would want to face each day! However, avoidance is not the answer either. Do you have wills in place for guardianship? Do you have life insurance in place that would continue to provide the standard of living you are used to if one of you were gone tomorrow? Is there an emergency savings in place in the event one of you was to lose your job? Are your disability benefits at work really enough to keep you going if you were ill or injured long-term? OK, it’s not a diamond necklace, but to be prepared is worth its weight in gold. And if you are fortunate enough to be able to get a beautiful piece of jewelry-make sure you know your homeowner policy limits-you may need an additional policy! When grandparents or godparents ask what the kids would like this year, why not tell them the gift of life insurance or a college savings plan. While your children may not understand the value of these gifts, you can make it exciting for them by explaining how they are going to be able to do things many other children may not be able to afford. They are going to have protection for their families when they grow up because Grammy or Pop-Pop took such good care of them. It is one less thing to break, outgrow or clean up! I have seen many people change their priorities over the years. Tough decisions have been made based on loss of jobs, investment income

and tragedy striking. What good is cable TV if something happens and you can’t make the mortgage payment? How important is that “game” to your children if you are in debt the next 12 months paying it off? You hear it said-- remember the “true reason for the season” and try to explain to your children that the meaning of the holidays is love, family, and being all together. Give gifts that will create a legacy and protect your family no matter what may happen. It is a memory they will cherish at a price you can afford!

Winter is coming! It’s that time of year again and there are several things to consider as the cooler temperatures are approaching. HVAC System Maintenance Have your HVAC system inspected and cleaned by a professional service technician. Routine maintenance will help your system operate more efficiently, lengthen the life of your equipment and ensure it’s reliability through the winter season. Deciding on which treatment to cover a normal window with can be difficult enough but sometimes you run into an opening that requires a little more effort. Sliding glass & French doors can be troublesome, arches & eyebrows over windows along with ovals, circles, octagons, triangles, rhombuses, and trapezoids, are all out there and may be letting in too much sun! That long skinny window next to your front door that we call a sidelight can also throw a monkey wrench into the works. Besides letting in natural light and providing you with a look to see who is at the door, it can also sometimes let passersby have a clear look right into your home. Through the years we have installed many different types of shades and blinds onto these openings but the best thing we have ever done, for this, was to start putting shutters on them. Plantation shutters are the most versatile answer to these issues because they really are the only covering that has a treatment for everything mentioned. For sliders there are both bypass style and bi-fold style which now have the ability to be folded over to lie against the wall next to the slider instead of perpendicular to the wall as in the past. This greatly reduces the projection into the room while at the same time letting you completely open up the glass to the light and the view beyond it. It is an improvement over the bypass because while the bypass doesn’t invade into the room when opened it does always have at least one panel’s width covering the glass. For French doors we can design a shutter that fits right over the glass. We attach the frame to the door and can even design the frame around the door handle for that stunning custom look. The panel can swing completely open so that you may clean the glass and for those instances when you need maximum light but most of the time you will just operate the louvers for light control and privacy. This covering is also very functional for a door as the entire covering is attached so that there is nothing to swing away from the glass as it moves. It’s a good choice for a door that will be used often.
Arches, eyebrows, and other specialty shapes such as ovals and triangles are another chance for shutters to standout. Almost always, in this category, a template is required. It doesn’t get much more customized than that and it shows. What you end up with is a gorgeous and unique piece that was made just for that opening. Often homeowners struggle with whether to let the difficult dictate the basic or to let the basic dictate the difficult and the best way to deal with this is to be armed with the facts. A few months later after signing contracts with them, we experienced a bed bug problem, immediately they were finding a solution, Dean Bennett spent many hours researching how to handle this problem in a library where the heat treatment could not be used and within days had our situation under control and found a maintenance program for regular prevention. Dean, and all Bennett employees are professional, knowledgeable, and trustworthy. Recently I experienced an issue with bed bugs. I called around to several different pest companies in Baltimore, MD and I came across Bennett Termite & Pest Solutions. I spoke with Bruce for over 40 minutes, he answered every single question I had. He was very informative and detailed about the process not missing a beat. Directly after graduation I enlisted in the Armed Forces as a Seabee builder in the United States Navy. In the nine years I served as a Navy Seabee I learned most of the skills I have used for the past 25 plus years in the painting industry. Including drywall repair, finish trim, and light carpentry.
