Our fall issue profiles the subject of Cancer and the survivorship of this terrible disease. With every industry feeling the strain, my colleagues in pubic relations, marketing, advertising, and design all admit to feeling the pinch from clients who think these services are the easiest to cut. After all, they have to pay their electric bill, but they don’t have to pay for marketing. Unfortunately, this way of thinking is all too common, and according to the experts, just the opposite is true. Customers are being more selective than ever when deciding where and how to spend their money. So it’s imperative that business owners are putting the message out about why they are the best, biggest, greenest, most affordable, most experienced, or whatever the niche in their industry. So take a tip from the experts, and think before you cut off the marketing lifeline. Hopefully, we’ll all soon see a change and will not need to make these tough decisions. Remember, when the going gets tough the tough get going. Be selective with your advertising dollars. Be sure to educate the public. A Permanent Fix for Chronic Sinus Sufferers

Innovative Office Procedure for Chronic Sinusitis Now Available in Salisbury!

Sinus disease, including recurrent infection requiring antibiotic along with chronic congestion, pressure and post nasal drip year round, is the most common health problem in the United States; affecting 1 in 5 patients per year. Most sinusitis patients suffer facial pain, pressure, headache, nasal discharge, constant post nasal drip with discolored mucous and fatigue. Sinusitis has significant effects on a patient’s quality of life according to quality of life studies. Overall, the chronic, recurrent sinusitis patient’s quality of life is WORSE than that of patients with chronic heart failure, chronic obstructive pulmonary disease and Parkinson’s disease. Historically, chronic sinusitis patients were limited to two treatment options: continued medical therapy, including antibiotics and topical nasal steroids, or conventional surgery such as Endoscopic Sinus Surgery (ESS). This all changed approximately 8 years ago with the introduction of an innovative technology called Balloon Sinuplasty. The Balloon Sinuplasty devices, originally designed developed, and commercialized by Acclarent, utilize a small, flexible balloon catheter that is placed through the nose into the blocked sinus passageway. The balloon is then inflated to gently restructure and open the sinus passageway, restoring the normal sinus drainage and function. Since the beginning of 2011 Balloon Sinuplasty has been offered as an in-office procedure without the need for general anesthesia. This innovative office procedure typically allows patients to return to normal activity within 48 hours. Acclarent’s Balloon Sinuplasty devices are safe and effective and have been used in more than 375,000 in-office procedures. Locally, Dr. James R. Gaul now offers Balloon Sinuplasty in their office as a possible alternative to radical sinus surgery in the operating room, which may include cutting and removal of bone and tissue, general anesthesia, and an extended recovery period. Dr. Gaul states, “This Balloon Sinuplasty: essentially angioplasty of the sinus, is a safe, very simple, patient friendly procedure that takes 30 to 45 minutes in the office under local anesthesia that has revolutionized the treatment

With September around the corner and kids preparing to return to school buying clothes, backpacks, paper and pens, parents should consider vision as part of this preparation. Eye exams are routine for many children whose visual problems were caught early and corrected with eyeglasses or contact lenses depending on their age. Early in children’s eyecare frames are designed with safety, comfort and durability in mind. Preschool, kindergarten and grade school ages often find frames that are colorful, flexible and in shapes that fit the typically narrow pupillary distances as well having pads for the bridge of noses that are less developed. Today’s children are more aware of style and brands they are exposed to at an early age such as Fisher Price, Adidas, LINDBERG Kids, Solo Bambini, Rec Specs, and dilli dalli to name a few. Even though parents are responsible for paying the bill and making sure their children receive proper medical care, in this case eyecare, it is very important for the child to be part of the frame selection process. This will go a long way toward the child wearing and taking care of their new eyewear and overall success of improving the child’s quality of life. The prescription is always the main consideration when choosing a child’s eyewear. When selecting frames the optician should be able to read the doctor’s prescription and have suggestions on frame styles that best fit the child but also lenses that provide safe, clear vision. Lenses, depending on the prescription, can be thicker either in the center (plus lenses) or the outer edge (minus lenses). Frame selection can cosmetically help children

feel more comfortable in these cases. Frames with wider temples can hide some of the edge thickness on a minus prescription. Those with the plus lenses can benefit from having a frame where the pupil fits as close to the geometric center of the frame as possible. The lens with less decentration (moving optical center) creates less thickness and better peripheral vision. There are many lens materials in the market place and each have different aspects from safety, to cosmetic, weight, clarity or specific applications that can be added. For children polycarbonate has been the standard for many years due to its characteristics of light weight, absorbing UV, thinness and primarily safety. It is a very durable impact resistant material long prescribed for children which had its origin in industrial eyewear. Trivex or Phoenix (Hoya’s brand) are more recent materials that qualifies for children 18 and under, we feel is better than polycarbonate. The two materials have similar characteristics but Trivex or Phoenix is more scratch resistant and has a higher abbe value so vision is slightly better. Obviously safety is primary for children’s eyewear along with the prescription and fashion. Eyewear today can help them through the early years when development is so critical. Sports eyewear is critical for children involved in almost any sport. Polycarbonate and Trivex can provide impact resistance from balls or flying elbows on a basketball court for example. This area of eyecare has grown and offers much more variety than in past years. We begin fitting children with contact lenses around the age of 12 or 13 depending on the child’s personal habits and maturity. Parents are usually involved in the decision process for obvious reasons. Contact lenses require more discipline and responsibility on the

child’s part due to the health implications if directions are not followed. For those who are able to wear them successfully they offer an alternative and can help those whose prescriptions are burdensome due to the lens power. What You Should Know About Oral Cancer

Detecting oral cancer early nearly double your chance of survival. Therefore, it is important that you now what factors put you at risk of developing the disease and what symptoms to watch for. Approximately 3 % of all cancers diagnosed in 2015 involved the mouth or the back of the throat. When certain types of skin cancers were excluded, these oral cancers accounted for most head and neck cancers.

RISK FACTORS

Some risk factors for developing oral cancer are beyond you control. Men tend to be at greater risk than women for oral cancer, which is diagnosed most often in adults between the ages of 55 and 64 years. However, you can control other risk factors. For example, some behaviors that may put at risk include: • using tobacco products; • drinking alcohol heavily (more that 4 drinks a day);

• using alcohol and tobacco products together (significantly increases the risk); • using betel quid (Paan); • eating a diet low in fruits and vegetables; • spending long periods in the sun which is associated with lip cancer.

WHAT CAN YOU DO?

Another risk factor on the rise is infection with human papilloma virus (HPV), which sexually transmitted. Specifically, HPV is linked to cancers classified as oropharyngeal. Oropharyngeal cancer involves tissues near the back of the mouth and throat, including the back and base of the tongue, and the tonsils. People who have HPV-related oropharyngeal cancer tend to be 4 to 10 years younger than people with oral cancers that are not related to HPV.

• Check your mouth and neck regularly for any of the above symptoms, and tell your dentist if you notice any of these or other changes; • Avoid using tobacco or drinking heavily; • Eat a healthy diet rich in fruits and vegetables; • Avoid spending extended periods in the sun and use sunscreen; • If you are sexually active, practice safe sex; • See your dentist-as part of your dental visit, he or she will examine your mouth and neck for signs of oral cancer.

ORAL CANCER SIGNS

Some signs you can watch for include: • a sore on lips or in the mouth that will not heal; • red or white patches in the mouth; • pain, tenderness, or numbness on the lips or in the mouth; • a lump, thickening, a rough spot, crusty area, or eroded area on the lips or in the mouth; • difficulty chewing, swallowing, speaking, or moving the jaw or tongue; • a change in the way your teeth fit together when you close your mouth; • a lump or growth in your throat or neck; • cough or sore throat that will not go away; • earache; • trouble swallowing; • hoarseness or other changes in your voice.

“BROKEN HEART SYNDROME”

He completed his post-graduate medical education at the University of Pennsylvania Graduate Hospital in Philadelphia with a residency in internal medicine (1991-1994), Chief Medical Resident (1994-1995), and cardiology fellowship (1995-1998). Dr. Rosa has practiced cardiology in Sussex County since July 1998. He has served as Medical Director of the Cardiac Catheterization Laboratory and as Chief of the Department of Cardiology at Beebe Medical Center in Lewes, DE. Dr. Rosa is trained in non-invasive cardiology, Nuclear Transesopageal Echocardiograms, as well as invasive diagnostic procedures and pacemaker implantations. Dr. Rosa is board certified by the American Boards of Internal Medicine and Cardiology. He is also a Certified Aviation Medical Examiner and is Board Certified in Nuclear Medicine.

Is a “broken heart” a true medical condition or is it just a figure of speech to describe someone’s emotional state after some great personal loss or stressful situation? It may come as a surprise to many people but the answer is both. This condition is also known as “stressinduced cardiomyopathy”, a rapid and severe heart muscle weakness, typically as a consequence of sudden emotional or physical stress: unexpected illness, injury or loss of a loved one. It can also be from a severe medical condition, surgery, an accident, intense fear, domestic violence, severe pain, financial loss, even public speaking or a surprise party.

The exact cause is unknown, but it is clear that most of the affected individuals are postmenopausal women between the ages of 58 and 75 years, in about 90 percent of cases. The symptoms are very similar to those of a heart attack, with chest pain or pressure and shortness of breath being the most frequent ones. It is not completely understood why this condition affects mainly women, but most investigators believe that after menopause the decline in estrogen levels renders patients more susceptible.

patients, the coronary vessels are open and the blood flow is usually normal. It is the “stunning” of the heart muscle cells by adrenaline and other hormones that creates the muscle weakness and this process is, for the most part reversible. Another interesting finding in this broken heart syndrome is that it does not tend to recur in the same patient. In other words, once somebody experiences this syndrome, even if other stressful events come along in their lives, the same phenomenon doesn’t tend to happen again. Learning to cope better with life stresses can prevent it in the first place.

It was first observed in Japanese women in the early 1990’s and the name of takotsubo cardiomyopathy was used originally. This name describes the characteristic appearance of the left ventricle (heart’s main pumping chamber) as seen in cardiac studies that resembles the traditional Japanese octopus trap. There is ample evidence to suggest that the massive release of certain hormones in the bloodstream, such as adrenaline and noradrenaline, as a natural response to stress, under certain conditions causes the heart muscle cells to stop working properly, especially in the lower part and the “tip” of the left ventricle.

The management of this medical condition is similar to the treatment of congestive heart failure with medications

such as diuretics, beta-blockers and ace-inhibitors as well as aspirin and in some cases blood thinners on a temporary basis. So yes, it is quite possible to have a “broken heart” literally, as a true medical illness that is brought about by a heightened physiological response to stress, especially in women. The good news is that in almost every case it is a temporary condition and full recovery is the expected outcome. There is no way to differentiate this situation from a heart attack, before hand. Therefore, it is very important to seek immediate medical attention for symptoms of sudden chest pain or pressure and shortness of breath. It might be the case that your heart doctor could tell you, in the end: “Just a broken heart, already on the mend”.

Stress-induced cardiomyopathy can be serious medical condition because it involves the sudden onset of congestive heart failure due to the heart muscle weakness, low blood pressure and potential shock or life threatening heart rhythm problems; it is, however, rarely a fatal illness. Furthermore, it is almost always reversible with most patients recovering completely within 2 to 4 weeks. The main differences between a heart attack and “broken heart syndrome” is that heart attacks occur as a consequence of blockages or blood clots in the coronary vessels (heart vessels that supply blood flow and oxygen/ nutrients) to the heart muscle cells that cause those cells to die or to suffer permanent damage. It is her own personal experience and survival that is what led her to the path of Grief Counseling at Delaware Hospice. Midge started her path on Grief Counseling long before she knew it; when she suddenly became a single mother after the death of her husband. However it was not until after the loss of her 23 year old son to suicide that really started Midge on her counseling journey. Soon after the loss of her son and while living in Atlanta, Midge became involved in the American Foundation for Suicide Prevention (AFSP) and SPAN- GA. It is during this time that Midge had her revelation that she just understood grief, “I get grief.” She trained to become a Survivor’s of Suicide (SOS) group facilitator in Georgia. It was after doing this training that Midge decided to pursue Grief Counseling in order to help a larger group with their losses. From that time on, Midge wanted to be that person for others who they could lean on along their journey. “Working with the bereaved was what I was meant to do with my life,” comments Midge. Being a “beacon of hope” at Delaware Hospice, Midge has been able to help over 1,000 clients along their journey. There is no prescribed method of helping someone through their sorrow but with her personal and professional experience around death, Midge has become a resource for Sussex County. Midge has found her home with Delaware Hospice’s Bereavement Department. “I love what I do each day; there is nothing sad about helping someone with grief. Putting on workshops for individuals and working with nurses and case managers are always a great way to round out my day. I have learned so much from my clients and I hope that they have learned something from me.” Midge and the rest of the Delaware Hospice Bereavement Team offer over 20 different bereavement workshops and meetings throughout the month in a variety of locations across the county.

But what is 3-D mammography? How is it different from traditional mammography? What is 3-D mammography? Approved by the FDA, 3-D mammography uses multiple images, or X-rays, to recreate a 3-D picture of the breast. Using these multiple images, a 3-D mammogram provides more detail than traditional mammography, which uses a single image. During the 3-D exam, an X-ray arm sweeps in a slight arc over the breast, taking multiple images. A computer then converts the images into a stack of thin layers, allowing the radiologist to review the breast tissue one layer at a time.

Who should have a 3-D Mammogram? Any woman who needs breast screening should consider 3-D mammography. In particular, women with dense breast tissue may benefit from 3-D technology because of the layered images that make it easier for doctors to catch breast cancer early. 3-D also helps catch more cancers and helps doctors see the cancer size much better. If you have a family history of breast cancer, regardless of your age, talk with your provider about having a mammogram. What are the risks associated with having a 3-D Mammogram? A 3-D mammogram releases the same amount of radiation as a traditional mammogram so there is no increased risk to a patient than a traditional mammogram.

Why 3-D mammography? There are several reasons why finding a provider that uses 3-D mammography is important. Because this technology uses multiple images, doctors have a

Protecting Seniors from Scammers

By Carolyn M. O’Neal CHEER Community Relations Director “I’m so glad my sister is flying in from California to help me convince Mom to let me handle her finances. Having her electricity shut off last month proved to me that she is no longer capable of taking care of her money. Last week I went to her house to help go through her paperwork. She gave me a bundle of “bills” and asked me to take them to the post office. When I got in the car, I went through the bundle and there were three envelopes to be sent to unfamiliar questionable addresses. Forgive me, but I opened them and was shocked when I saw twenty dollar bills inside each envelope. The money in one envelope was for a so-called “charity” that I never heard of; the money in another was to help feed “starving” children in Africa, but it was being mailed to Mexico; and the money in the other was to enter Mom in a million dollar sweepstake. Taking control of her finances is the only way I know to protect her from the telemarketers and all that junk mail that comes to her house.” Thousands of adult children have the same frustrating plight while trying to take care of their declining elderly loved ones. Senior citizens grew up when trusting and being trustworthy were normal behavior. Because of this, they are easy prey for unscrupulous people through various forms of fraud. Telemarketing and junk mail are the two most common avenues for conducting elderly scams. Telemarketers use 800 or 900 numbers. Toll-free numbers like 800, 877, 866, or 822 are long distance. Numbers such as 900 and 976 are caller paid long distance services that allow consumers and businesses to retrieve information over the telephone. Watch out for unfamiliar area codes.

Illegal telemarketing is big business that robs thousands of seniors daily. Telemarketing fraud is a crime and punishable by law. Illegal telemarketers do not care about the pain that they inflict on seniors. A call is generally a scam if the caller asks for a social security number, a credit card number, or a bank account number. These callers should be reported to the state attorney general’s office, a local consumer protection agency, and/or to the National Fraud Information Center. Junk mail can cause the same heartache. It is not uncommon for seniors to receive dozens of pieces of junk mail every day. It comes in the form of 1st class mail, 2nd class mail, 3rd class mail, and bulk mail. Bulk mail or 3rd class and 2nd class mail will not be returned to the sender so don’t bother trying. There is a postal form you can fill out to have bulk mail stopped, but it doesn’t always work. The predators seem to always find a way around it. If you order something from any of them, they do sell your name and address to others. “So what can I do? After balancing her check book, I learned that she has given over $9,000 to these scammers last year. No wonder her electric was cut off, she didn’t have the money to pay her bill. Mom lives only on her monthly Social Security check, how can I protect her from these thieves?” That is a difficult question to answer. Mom still possesses her basic right of making her own decisions, and most elderly parents are reluctant to turn over control of their finances to their children until it is too late. As responsible adults who care about our loved one’s welfare, we must remain diligent about keeping an eye on that decision making. Know the warning signs of your loved one being targeted by scammers: • Watch for unusual amounts of junk mail, particularly if it is asking for payment of some sort. • Be aware of frequent calls making offers for alleged valuable awards or asking for charitable donations. • Watch for evidence of many cheap items (costume jewelry, small

appliances, etc.) that had to be purchased to win something or receive a prize. • Watch for secretive or guarded behavior regarding telephone calls. • Notice if a private courier stops by the senior’s home to pick up something. • Observe whether the senior is having unexpected problems paying bills or buying food or other necessities. • Watch for the receipt of calls from someone or an organization stating they can help seniors recover money paid to telemarketers – for a fee. It will be a long, frustrating process to protect your loved one from scammers, but continue to help them. Stress the importance to them of reporting this crime. Help your loved one learn how to identify scams. Encourage them to hang up on telephone solicitations that seem too good to be true. Have a heart-to-heart,

calm discussion with your loved one about their finances and try to come to an agreement about the best way to handle them in the future. If all else fails, change your loved one’s telephone number. If possible, go through their mail before they get it and pull out all the junk mail. This situation requires daily scrutiny. But, be sure not to blame or accuse your loved one of being stupid or foolish. Swindlers take advantage of a person’s honesty. And, don’t threaten to take away the person’s financial and physical independence. This may only make them secretive and resentful. Most importantly, don’t give up! The Weight is Over: To Sleeve or Not to Sleeve? Thomas P. Barnett, M.D., F.A.C.S.

Dr. Barnett graduated from Princeton University in 1983 and obtained his medical degree from George Washington University in 1987. He completed his surgical training at Thomas Jefferson University affiliate Lankenau Hospital in Philadelphia in 1992. He served in the U.S. Air Force at Dover Air Force Base (DAFB) from 1992-1996 where he started the Laparoscopic/ Minimally Invasive & Ambulatory Surgery programs. Dr. Barnett was Chief of Surgery at DAFB Hospital in 1995 and 1996. He has continued to practice surgery in Dover, Delaware ever since being honorably discharged and he founded his private practice, Surgical Associates, P.A., in 1999. Dr. Barnett is Board Certified in Surgery and is a Fellow of the American College of Surgeons. He is also a member of the American Society for Metabolic Bariatric Surgery (ASMBS) and the Medical Society of Delaware. Dr. Barnett has special interests in weight loss surgery. He currently serves as the medical director of bariatric surgery at BayhealthMilford Memorial Hospital and the Delaware Surgery Center in Dover.

Weight loss surgery has evolved over the last 30 years. Laparoscopic Sleeve Gastrectomy (”Sleeve surgery”) or taking out a portion of the stomach (see diagram 1) has emerged as the most common weight loss procedure in America over the last couple of years. It is estimated that in 2017, 70-80% of the weight loss surgeries done in the US will be the sleeve gastrectomy. This surgery is done through a series of incisions, most of which are the diameter of a dime. The Sleeve surgery basically converts the stomach from football size to banana size. The decreased size of the stomach

causes a restriction feeling to the patient when they eat and therefore they will eat less. In addition, the surgery removes a part of the stomach that produces a hormone called ghrelin, which is secreted by the oxyntic cells of the fundus in response to fasting. Ghrelin stimulates appetite. The Sleeve decreases the amount of ghrelin produced and therefore patients are able to better control their hunger. Patients who have weight loss surgery have significant positive metabolic effects as well. Type 2 diabetes, high blood pressure, and sleep apnea improve and therefore decrease obesity related morbidity and mortality. Many patients will be able to decrease or completely get off medications for high blood pressure and diabetes. Back and knee pain also improve with weight loss. In a recent study from the Netherlands on 1000 patients undergoing sleeve surgery, they found on average, an excess weight loss of 86.6% in the first postoperative

year, 84.2% after two years, and 84.5% after 3 years. Pre-existent comorbidities appear to improve or even resolve after sleeve surgery. Resolution of type 2 diabetes occurs in 60-96% of the patients. Dr. Sandra Costa is not only a family physician, but a sleeve surgery patient of Dr. Barnett’s herself. She has lost 108 pounds since starting her “sleeve journey.” She had her surgery in November 2015 at Milford Memorial Hospital. “I consider that day my rebirthday. I was born obese at almost 12 pounds and put on weight every year until my sleeve journey began. I feel much better and more energetic.” Francis Fooks, another patient of Dr. Barnett’s, has gone from 318 to 183 pounds 10 months after her Sleeve surgery. “I was able to get off all my medications and get rid of my sleep apnea machine. This surgery has given me my life back! Dr. Barnett, Nicole Fisher ,PA-C, the team at Surgical

Associates, and Milford Hospital have been extremely supportive throughout my weight loss journey.” There are risks with all weight loss surgical procedures. These include, but are not limited to bleeding, infection, obstruction, leaks, and the need for reoperation. Death has occurred in rare cases. Fortunately, with the sleeve gastrectomy, safety profiles have been proven to be high with few serious complications. Sleeve surgery causes less nutritional deficiencies post-operatively compared to the gastric bypass. Complications after Sleeve surgery in the literature vary from 2.9 to 9.5% which appear to be lower than the complication rate after gastric bypass, varying between 4.6 and 20.5% in the literature. In redesigning its app a year or so ago, Walgreens found the perfect prescription for success: simplicity, valuing the ecosystem and understanding the native capabilities of the mobile device. Deepika Pandey, GVP of digital marketing, product and innovation, and Ray Kim, senior manager of user experience for mobile, shared their journey at the Shop.org Digital Experience Workshop. Lesson One: Simplicity always wins. “It seems very simple, very basic,” Pandey said, “but it’s about taking friction out of the experience.” The first “killer” piece of functionality, for example, was refill by scan, which allows users to scan in a photo of a prescription bottle bar code. More from the Shop.org Digital Experience Workshop “We like to call this ‘refills in 17 seconds or less,’” she said. “As you can imagine, most people don’t think about refilling a prescription as something that’s enjoyable … but mobile actually showed us that you can change the way that a customer thinks about an experience.” The proof was in the pudding as Pandey shared her favorite feedback: “That was so much fun, I wish I had more prescriptions to fill.” Mobile added a gamification aspect to something that traditionally had been dry. Since then, the company has introduced refill by text, taking the process from 17 seconds to less than three. The user receives a text message, and only has to reply “refill.” Lesson Two: Mobile is the bridge between the digital and physical worlds. “When you are a retailer that has over 8,000 stores, this is really, really important,” Pandey said. Knowing that consumers were already using the company app while in-store,

Walgreens has been a frontrunner in mobile payments, too. Lesson three: The ecosystem matters Walgreens launched an application programming interface program, starting with its photo experience. The company knew no one could do fulfillment better, but understood partners could improve other elements and experiences. Memory Loss Memory loss is easily the most associated symptom of Alzheimer’s disease although it isn’t often how people think it is. Many are under the impression that with the onset of Alzheimer’s that the memory simply begins to fade, and while that is true in some cases, and in most over time, at the beginning, it is new information that is usually the type that suffers. Alzheimer’s changes usually first develop in the part of the brain that affects learning. Thus it is usually new learned information that can be difficult to retain while older, more ingrained memories are usually left more or less intact. However, it is important to note that with no cure for Alzheimer’s and the progressive nature of the disease, that in time; even older memories are likely to begin to fade. Decreased Activities A common symptom among people suffering from Alzheimer’s disease is a general decrease in all activities Many suffers of Alzheimer’s become lethargic and loose interest, exhibiting levels of depression and anxiety. While it might not be acknowledge by someone suffering from Alzheimer’s just having a person in their presence

One of the hallmark aspects of Alzheimer’s disease is the fact it not only affects the person behind the illness, but is also devastating to witness, and thus can be difficult for the family. However, it is near impossible to battle this disease alone. This is why it is so important to be aware of the signs and symptoms, so you can have a better idea of what to do if Alzheimer’s should rear itself in your life. Emotional Another area of the brain that is commonly affected by the onslaught of Alzheimer’s is the hippocampus, or the area of the brain that deals with emotions - more specifically emotional maturity. Again, this is often likened to childlike behavior, as a suffer from Alzheimer’s disease may often overreact to situations or on the inverse, might not fully grasp the severity of others. This disconnect directly impacts many other symptoms of the disease, from poor judgment and diction making skills, to trouble speaking and employment problems. It is also worth noting that a great deal of family members of people affected by Alzheimer’s report that their decline in emotional maturity is one of the hardest symptoms to bear witness to. That is because we normally think of these people in their prime; the peak of their lives, in which physically and mentally there was little they couldn’t accomplish, at least in our eyes. However, seeing them likes this, child like, stirs great feelings in many of us, often ones that are hard to deal with. It

is unnatural for many people to see that, especially in their parents or loved ones. Poor Judgment This sign of the presence of Alzheimer’s disease can be likened to regularly of exhibiting child-like behavior. Many report that their loved ones with Alzheimer’s often demonstrate a disregard for consequences, even referring to the behavior as brazen. Difficulty can be had in explaining instructions, especially as we know the ability to learn and retain new information often being unfortunately one of the first parts of the mind to be ravaged. Severity of these symptoms varies greatly on a patient by patient basis, but it is often exhibited in those who have suffered from the disease for a prolonged period of time - give the degenerative and progressive nature of the disease. The more severe the illness becomes, the likely grows that your loved will require 24-hour care. Losing Items Now, obviously this one is connected to several other symptoms on this list. Memory loss, and confusion mainly, but this one is especially important to the family of someone affected, as memory loss and confusion can often go months or years without be noticed - if they

ever are at all. By keeping eye out on your loved ones, especially the aging ones, you increase your chances of noticing a problem in its infancy - that is of course if you know what to look for. The best way to tell is to give your entrust your love ones with small, seemingly meaningless tasks. This can be anything from asking them to remember an item when you go to the store to holding onto a set of keys. The point is to keep just as much track of the item as you hope they would. That way, you can get an accurate picture as to where their mental faculties are. Alzheimer’s care is often as important as healthcare, particularly when it enables a person with Alzheimer’s disease to remain at home. Know Where You’re Going … And Why! (It could save you money)

About Dr. Chao Dr. Philip Chao graduated Phi Beta Kappa from Yale University and was Scholar of the House. He continued his studies at the University of Rochester School of Medicine and Dentistry, receiving his medical degree in 1983. After a transitional internship year at the Mary Imogene Bassett Hospital in Cooperstown, NY, Dr. Chao did his residency in Radiology at Brooklyn’s SUNY Health Science Center (1984-1987), where he became Chief Resident. He stayed on to complete a fellowship in Neuroradiology (1987-1988). Dr. Chao has been interested in MRI technology since its very beginning – the first MRI scan took place in 1981 while he was pursuing his medical studies. The University of Pennsylvania, where Dr. Chao was both a Body MRI Fellow (19881990) and a Neuroradiology Fellow (1989-1990), was at the heart of the development of this technology and Dr. Chao was able to work with pioneers in MRI on the first GE Signa 1.5T (tesla) scanner. His advanced research and specializations complete, Dr. Chao left the University of Pennsylvania for a position as Director of MRI in Wilmington, DE – a position he held for 14 years. Dr. Chao eventually left that position to create the best MRI Center in Delaware which uses the very latest technology: 3T MRI. While 3T (tesla) scanners have been used for scanning research patients since 2002, these advanced, more powerful MRI scanners only became available for clinical use in 2004. Dr. Chao worked hard to bring 3T technology to Delaware and in February 2007, MRI Consultants, LLC began operating the first 3T scanner in the State. 3T is the newest horizon for MRI. By: Dr. Philip Chao Did you know you can save $400 to $600 on your next MRI study simply by choosing where your MRI study is done? If your MRI study is done at a hospital or clinic-based imaging center associated with a hospital, you may have to pay additional fees (co-pay or coinsurance). Certain insurance companies routinely charge these extra fees without mentioning them before a study is done— even if you have your study “authorized.” It can be several hundred dollars. Insurance companies do not charge this extra fee if you go to an independent facility—such as MRI Consultants. In these facilities, your MRI is billed as an Office Visit. In such instances, your co-pay can be significantly less. Just ask your insurance company. Quality is important. So few people (including doctors) realize how important it is to have your study done on newer 3T or multichannel 1.5 scanners instead of older scanners. The older scanners just don’t “see” as much! If your study is done on an old scanner with just 1 channel, you can be pretty certain subtle things will be missed. We recently found a tibial non union which was only 1mm in size! This can cause a great deal of pain. See image. The resolution on older scanners and low-field OPEN MRI is insufficient to detect such small fractures. It just can’t see it. Higher definition is extremely important. We recently scanned a patient for a possible superior orbital vein thrombosis. The ORBIT study we performed at 3T definitively showed this isolated finding. 3T high quality pictures are like HDTV (as compared to blurry old snowy cathode ray tube pictures). 3T is just better. And, despite what you may

have been told, 3T only costs more for the person who purchases the scanner! For the patient, the price is the same. Quality seems to be unimportant to insurance companies (and even to some doctors). For them, an MRI is an MRI. Your co-pay is the same regardless of quality. Read the NY Times article: “The Scan That Didn’t Scan.” The author did not seem to know that the ACR, which accredits MRI units, has different criteria for different scanners. Weaker MRI scanners are not required to “see” as much as stronger scanners. And, surprisingly, there is currently no minimum level required in order for your insurance company to pay. None. Even the weakest scanner can be “accredited.” You may have to have your study repeated on a stronger machine (frequently at your own cost), but the medical profession and insurance companies are largely unconcerned. Understanding your pictures is important. At our facility you get to discuss your images with the radiologist (upon request). I do not know if this is even possible at other facilities. At MRI Consultants, patient reports are frequently done within an hour— sometimes before leaving the office. Also, all patients leave our office with a

CD of their images. We offer a quality and speed of care which is not often given elsewhere. At MRI Consultants, we do studies other facilities will not (or cannot) do. We do indirect arthrogram studies which frequently show MORE than direct arthrograms. IV contrast is used to see if there is a tumor, infection or inflammation. At 3T, the fat saturation is better and we are able to do thinner “slices” (or sections), which allows us to see very small tears. Even a small tear in your shoulder can be very painful.

Finding such small tears is one of the great things about indirect arthrography. A recent patient of ours had two labral tears with two paralabral cysts. According to “the literature,” these paralabral cysts are rare, yet I see them even without contrast in many of my patients at 3T in the hip and shoulder. This just another example of why 3T quality is better. Most doctors and orthopedic surgeons do not know this. 3T MRI simply shows more. Seeing more can reduce the need for surgery or, at the very least, make it safer. Be smart. Be certain. Use 3T.

I enjoy reading what others in my field have to say. I recently read an article by Ruth Lera, meditation teacher, energy healer and creator of The Self Healing Academy. Although Energy Medicine is one if the oldest healing professions, it hasn’t been recognized as a valid method of healing for our bodies and minds by conventional medical practitioners. In recent years it has made a well deserved comeback. The tradition of laying of hands as a means of healing the physical and emotional woes goes back thousands of years, people are now finding that seeking out Energy Healers is a fantastic addition to conventional healthcare choices. CAN A LOW FAT DIET REALLY BE HIGH FAT DIET? First, let’s understand that there are 3 main groups of foods that provide us with energy: carbohydrates, protein, and fat. They are known as macronutrients. They provide us with the main substrates for our bodies to produce energy for all our daily activities, from just sitting down and thinking to strenuous exercise. For the majority of people, carbohydrates are the main fuel source for our bodies. Almost all carbohydrates eaten are eventually converted to a single simple molecule (monosaccharides: glucose, fructose, galactose) so they can enter the cells and be metabolized (go through a series of chemical reactions) to decide their final fate: produce energy, produce glycogen (storage form of carbohydrates), or be used to produce other substances such as ketones or cholesterol. All cells in the body can use glucose for energy production. An alternative fuel source is ketones, small molecules produced in the liver if carbohydrates are scarce or absent. In order to produce ketones, the body has to burn fat. Another important concept to understand is where fat in our fat cells come from. Carbohydrates and fats provide most of the dietary energy. Proteins do not really provide a significant source of energy, but are crucial for structural functions. Therefore, body weight and fat tissue size maintenance is primarily determined by the intake and utilization of carbohydrate and fat. The fat cell (adipocyte) can expand and shrink considerably, but once formed, their number cannot be reduced by dieting, only their size. The main source of fat to our fat cells comes from dietary fat. Therefore, it seems only logical to think that we

could lose weight by reducing the amount of fat we eat. Any excess carbohydrates we eat gets converted to fat in the liver through a process known as “de novo lipogenesis (DNL)”. However, the amount of fat produced from carbohydrates is insignificant in relation to the amount of fat we get from diet. However, carbohydrates have major indirect effects on fat metabolism and utilization. This becomes important when we chose a low fat diet, since for fat is usually replaced by carbohydrates, which will provide better flavor and texture, than replacing fat with protein. Higher carbohydrate intake can have the following metabolic effects: raise insulin levels, promote fat deposits in the liver (fatty liver), cause body inflammation, increase cravings, fluid and salt retention, brain resistance to Leptin (hormone that controls appetite, cravings, food reward/pleasure), elevate triglycerides, lower HDL (good cholesterol), shrinks LDL (making bad cholesterol more prone to stick to your arteries), and eventually can lead to food addiction. Probably the most significant effect is elevation of insulin levels. Insulin promotes storage of nutrients, but most importantly, it stimulates the metabolic pathways and enzymes necessary to store fat. At the level of the fat cell, it facilitates fat release from blood stream and entrance into the fat cell (stimulates an enzyme called lipoprotein lipase), stimulates conversion of free fatty acids to triglycerides (storage form of fat), and blocks fat burning, by blocking the ability of fat cells to breakdown fat (inhibits an enzyme called hormone sensitive lipase) and release it back to the blood stream to be used and burned somewhere else. At the level of the liver, insulin stimulates conversion of carbohydrates to fat (de novo lipogenesis) and cholesterol. It also blocks ability to produce ketones as an alternative source of fuel.

Therefore, a low fat, high carbohydrate diet can act as a high fat diet by inducing liver conversion of carbohydrates to fat, promoting fat storage through higher insulin levels, blocking ability to burn fat, and stimulating cholesterol synthesis. For all these reasons, a low fat diet, can really have worse effects than a high fat diet, if not designed carefully. It is very important that when you chose any dietary intervention for weight loss, you do it with the guidance and monitoring of a physician that specializes in weight loss. Dietary changes in macronutrients have to be designed to promote not only weight loss but also good health. Many times what we think is healthy might not be.

Chiropractors and Exercise Are Better than Drugs Says New Study Dr. Tracy Hudson Dr. Hudson is a ’95 Cape Henlopen High School graduate and continued her educational career at the University of North Carolina at Wilmington where she completed her Bachelor’s degree in Physical Education. While attending UNCW, Dr. Hudson suffered an injury playing softball and she sought treatment with a local chiropractor. After a few adjustments, Dr. Hudson was back on the field. Now being a true believer, she decided to further her education in the natural healing art. She attended Life University in Atlanta, GA for 3 years, and then transferred to Texas Chiropractic College in Houston. Chronic pain is an exceedingly common condition impacting an estimated 76.5 million Americans, one-third of whom describe their pain as severe and “disabling”. Among them, many suffer from neck pain, which is the third most common type of pain according to the American Pain Foundation. It is estimated that 70 percent of people will experience neck pain at some point in their lives but research into effective treatments is surprisingly limited. If you visit a conventional physician for pain, there’s a very good chance you’ll leave with a prescription for a medication, as nonsteroidal anti-inflammatory drugs (NSAIDs), acetaminophen (Tylenol) and even opioids (OxyContin, Vicodin, etc.) are the go-to treatment for pain in the modern medical world. However, there are better options than drugs for neck pain, not only in terms of pain relief, but also in helping to treat the underlying cause of the pain so that healing can truly occur.

New Study Shows Exercises and Chiropractic Care Beat Drugs for Neck Pain

According to a new study published in the Annals of Internal Medicine and funded by the National Institutes of Health, medication is not the best option for treating neck pain. After following 272 neck-pain patients for 12 weeks, those who used a chiropractor or exercise were more than twice as likely to be pain free compared to those who took medication. Specifically: • 32 percent who received chiropractic care became pain free • 30 percent of those who exercised became pain free • 13 percent of those treated with medication became pain free

What is osteoarthritis (OA)? Characterized by joint pain, swelling, and stiffness, osteoarthritis is the most common form of arthritis, affecting approximately 27 million US adults. If you are living with osteoarthritis, it is important to stay informed about advances in treatment, while focusing on information you can trust. Here are some facts I offer my patients to help them manage their osteoarthritis pain: Nonsteroidal anti-inflammatory drugs (NSAIDs) NSAIDs are the most frequently used medicines to help manage pain and inflammation. NSAIDs are found in nearly 550 over-the-counter and prescription medicines, including aspirin, Aleve® (naproxen sodium), and Advil® (ibuprofen), to name a few. Other NSAIDs are only available as a prescription. The higher the dose, the higher the risk While NSAIDs can be effective, using them can also put you at risk for serious heart, stomach, and kidney problems. In fact, 16,500 people with rheumatic diseases (osteoarthritis and rheumatoid arthritis) are estimated to die each year due to NSAID-related gastrointestinal complications. The US Food and Drug Administration (FDA) recommends that NSAIDs be used at the lowest possible dose and the shortest time needed for your treatment.

Dr. Christine D. Hannaway is a General Surgeon practicing in Seaford, Delaware. In addition to a broad scope of general surgical care, Dr. Hannaway evaluates and treats a wide array of breast disease. This includes benign conditions such as breast pain and lumps as well as the diagnosis and surgical treatment of breast cancer. Dr. Hannaway works closely with Nanticoke Health Services’ mammography department and the Cancer Care Center as part of a multidisciplinary breast care team. She utilizes advanced techniques such as 3D mammography, MRI mammography and stereotactic biopsies to diagnose breast cancer, all of which are available at Nanticoke Memorial Hospital. Every aspect of each patient’s surgical breast care is managed by Dr. Hannaway, from breast biopsies to long-term follow-up. As a woman, Dr. Hannaway understands the anxiety and fear that every woman experiences when faced with the evaluation and treatment of her breast symptoms. Kelly Yoder‘s desire to help people led her to choose a career in the medical profession. Upon graduating from college, she started working with Nanticoke Health Services as a Radiologic Technologist in July of 1992. In 1995, Kelly was asked if she would be interested in learning mammography and stereotactic breast biopsies. “I said yes without hesitation, and have been very pleased with my decision,” says Kelly. “I have seen quite the change over the past 24 years transitioning from analog to digital and now to 3D tomosynthesis.” “Although no one looks forward to getting their mammograms, they do enjoy talking to us,” explains Kelly. “With patients coming year after year, you get to know them on a personal level.” “When not doing mammograms, I am working alongside the surgeons performing stereotactic breast biopsies, which is something I thoroughly relish. I like the fact that I am with the patient every step of the way beginning with the mammogram, next the biopsy, and, if needed, surgery. It makes them feel more at ease when they see a familiar face. I have had patients say ’you saved my life’ and that does my heart good knowing I am making a difference.” “Some say I was meant to do this job or that this job was meant for me,” Kelly adds. “Breast cancer hits very close to home: When I was 8, my mother succumbed to breast cancer at the young age of 40. She was 38 when she was diagnosed and fought hard for those two years. Her sister, at age 67, was also diagnosed with breast cancer, but thankfully defeated it. With advancements in technology and treatment, the survivorship is much greater today than years ago.” When not at work, Kelly enjoys crabbing in the summer and snowmobiling in the winter with her husband, John. “Yes, I love snow! All year long I take pleasure in caring for the elderly, walking and baking. I am also quite the animal lover, especially for my dog, Noble.”

Senior Mammographer, Nanticoke Diagnostic Imaging

Terry Persolio, is the Senior Mammographer at Nanticoke Diagnostic Imaging in Seaford, Delaware. She constantly strives to make sure her department is offering the highest quality imaging possible for women in Sussex County. Terry began her rewarding twenty-seven-year career with Nanticoke Health Services in 1989 in the employed physicians’ network. Terry is a native of Sussex County, Delaware and a graduate of Indian River High School in Dagsboro. After completing high school, she earned her associate’s degree in radiologic technology in 1989 at Delaware Technical Community College. She earned her mammography certification in 1993 and obtained her quality management certification in 1999. Terry officially took over the Senior Mammographer position at Nanticoke in 2005. “In this position there are many challenges,” explains Terry. “Mammography is one of the mostly highly regulated departments in radiology. I face the challenges of accreditation by the American College of Radiology and yearly onsite inspections by the State of Delaware and the Food and Drug Administration/MQSA.” In Terry’s career, she has experienced three technology changes in mammography from analog, or film mammography, to digital mammography, and most recently to the 3D tomosynthesis. Her research and belief in the new 3D technology gave her the drive to help Nanticoke see the need to offer this high level of service to patients. Terry possesses a unique combination of strong supervisory and patient care skills. In addition, she believes in building a long-lasting and trusting relationship with patients. When Terry is not at work, she enjoys spending time with her husband, Steve, son, Mark, and daughters, Tia and Kiley. She also enjoys tanning on the beach, cruising on her boat and traveling to places such as Punta Cana, New Orleans and Las Vegas.

Julie Abel is the Director of the Nanticoke Cancer Care Center in Seaford, Delaware. As Director, she manages operations that continue to provide lower Sussex County with quality cancer care. She has a Master’s Degree in Healthcare Administration and has been working within the world of Oncology for over fifteen years. “The care of people who have been diagnosed with cancer is complex,” explains Julie. “I have been blessed to work with the dedicated physicians, nurses and support staff at three cancer centers and I find them to be a unique group of amazing, compassionate people.” Until June of 2015, Julie lived in the Lancaster area of Pennsylvania for nearly her entire life. She and her husband had wanted to move to Delaware. When the opportunity at Nanticoke presented itself last year, her ever patient husband, Patrick, and their two cats packed up, moved to Delaware and couldn’t be more pleased. When she is not at work, Julie and her husband enjoy outdoor activities on the Delmarva Peninsula including kayaking and spending time at the beach. She also enjoys photography, reading and attempting to paint. She has a son and daughter who still live in Pennsylvania, and twin grandchildren.

The cancer coalition offered me a mentor, who gave me support and knew what I was going through at the time. My faith was a big part of my recovery, through faith I had an inner strength to get through the treatments. I’m a two and a half years survivor of cancer. My hair grew back, my energy returned, I was able to return to work, and continue to enjoy my family and friends. I’m still knitting scarfs and enjoying long walks. Since becoming a survivor I have been able to be a mentor to someone else through the cancer coalition, someone who was just starting their journey.

I had a second mammogram and ultrasound, which also showed there were abnormal cells in my breast. Shortly after, biopsies confirmed that I had breast cancer. In March 2015, I had a lumpectomy and the pathology report showed the cancer had gone into my lymph nodes. Over the next several months, I underwent 16 rounds of chemotherapy followed by 30 radiation treatments. My doctors also suggested that I should have my ovaries and fallopian tubes removed as part of my treatment. My breast cancer journey has surely changed my life but more than anything I believe it has saved my life. Life changed for my family immediately. My husband and I had always worked side by side on our family grain & poultry farm, where we have no employees other than each other. Our son & daughter pitched in along with other family members & friends to help during my treatments. This life change was not easy for any of us. It was very difficult to maintain the farming chores that are necessary on a daily basis. I had always been very healthy, strong and an all around busy person. During my treatment that all changed. I’m happy to say that today, a year and a half later, I am back. Back to my life as a wife, a Mom and a farmer. I also trained and have been active as a peer mentor with the Delaware Breast Cancer Coalition. Feeling good, feeling healthy and feeling blessed.

Breast cancer is more than a pink ribbon. It is a mother, a grandmother, a sister, an aunt, or a friend. Breast cancer is about 100 times less common among men than among women. For men, the lifetime risk of getting breast cancer is about 1 in 1,000. Women Supporting Women has been helping Delmarva families affected by breast cancer since 1993 when three local women saw a need for a breast cancer support group. Originally meeting once a month in a hospital waiting room, the group grew to what is now the 501(c)3 nonprofit that helps hundreds of families a year. “With no national ties or funding, Women Supporting Women reaches out to the person just diagnosed with breast cancer before we even meet them.” Emily Rantz, PR & Marketing Director for WSW said, “We provide the surgeons in the area with informational tote bags filled with literature and information about breast cancer. They are given out to the survivor the day of their diagnosis. From there, it is up to them whether or not they would like us to contact them.” In addition to the Informational tote bags, WSW also provides chemotherapy and radiation bags which include some comfort items for someone going through those treatments. Handmade “Piglet Pillows” are given to someone having a single or bilateral mastectomy and are meant to go under the arm for comfort after surgery. Located on Belmont Ave, next to the Community Foundation of the Eastern Shore, they have an entire room filled with wigs all different colors, cuts and shapes. A woman undergoing chemotherapy can come to be fitted for the perfect wig. WSW relies on grants, donations and fundraisers to continue offering all of their services at no cost. CoolSculpting Non-Surgical Fat Removal fat. This non-invasive procedure represents an alternative to surgical liposuction by using a novel cooling technology that permanently removes fat cells from treated areas.

How does CoolSculpting Work? The technology behind CoolSculpting was developed by Harvard scientists who noted that fat was destroyed by cold under certain conditions. CoolSculpting is a result of their efforts to harness and refine this application of cold to selectively remove unwanted fat deposits in a safe manner. During a treatment, your unwanted fatty area will be pressed and chilled to a temperature 12 degrees above freezing. At this temperature the fat within fat cells crystallizes. When the fat within the cells crystallizes, it damages and kills the fat cell. Skin and other tissue is not damaged as the water in other cells is not crystallized, only fat. The body will then eliminate the killed fat cells over time. The best news is that we do not make new fat cells. Fat cells merely get bigger or smaller as they store more fat or lose fat. So in essence the fat removal from CoolSculpting is permanent. And we have seen instances of patients who if they gain weight, the weight will not return to the treated areas. What are the most common areas to be treated? Many people on a weight loss program continue to have stubborn hard to lose areas, no matter how much dieting, sit ups or weight they lose. Such stubborn areas are typically around the abdomen, the love handles and inner thighs. Once you are over forty, losing fat in these areas is extremely difficult. These are the areas that CoolSculpt can address. Basically if there is an area you can pinch several inches, that area can be treated and eliminated

by CoolSculpt. Most of our patients are interested in losing fatty areas in the upper and lower abdomen, the muffin top, waist, brat fat, back fat, love handles, and “mommy pooches.” We can also treat arms, thighs and saddle bags.

Are there any side effects from CoolSculpting? The side effects from a CoolSculpting treatment are minimal. It is possible Before to have bruising after the treatment, which resolves in a week. It is common to have numbness or decreased sensitivity in the treated area lasting 1 to 2 weeks. In some instances patients will develop a burning tingling sensation 5 days after the treatment which can be controlled with a Lidoderm patch. There are virtually no long term negative side effects. What does CoolSculpting feel like? Our treatment room is very comfortable where you will relax on a padded recliner. When the CoolSculpting is applied to the fatty area to be treated there is a sensation of pulling that feels slightly uncomfortable for the first 5 to 10 minutes. Gradually this sensation lessens to the point during the treatment any uncomfortable sensation is minimal. There seems to be a natural endorphin release during the treatment so patients are very relaxed during the treatment, which takes an hour. Some people have fallen asleep during the treatment! A typical treatment session lasts between 1 and 3 hours, depending on how many areas are treated. During the treatment, we have Netflix available so you can watch a movie or catch

Prior to joining La Red Health Center, Ericka served as Family Nurse Practitioner, Director Clinical Services for Connections Community Services Program in Georgetown, Delaware, where she provided primary and urgent care services to patients fifteen and older. Ericka also worked as a Staff Registered Nurse at the Bayhealth Medical Center in Milford, Delaware and was a member of the Delaware Technical and Community College Adjunct Faculty Terry Campus. Patty Walton, Family Nurse Practitioner-BC La Red Health Center is delighted to announce that Patty Walton, FNP-BC has rejoined La Red Health Center on June 1, 2016 as a Family Nurse Practitioner. Patty has had significant experience as a Family Nurse Practitioner, Registered Nurse, Home Health Nurse and Labor and Delivery Nurse for various Sussex County Health Care organizations. Patty initially joined La Red Health Center in September of 2007 and worked as a Registered Nurse until March of 2011, providing direct patient care for primary care and obstetrical patients as well as interpreting services for medical practitioners. For the past five years, Patty has worked as a Family Nurse Practitioner at Lewes Medical and Surgical Associates, providing primary care services (assess, diagnose, prescribe and treat) for diverse patient populations aged 11 years and older. Patty received an MSN- Family Practice Nurse Practitioner from Wilmington University in December of 2010, a BSN from Salisbury University in December of 2005, an MA in Latin American and Caribbean Studies from Indiana University in December 1997, and a BA in Spanish from Washington College in May 1996 Patty is credentialed as an ANCC Family Practice NP, a Certified Medical Examiner by the National Registry of Medical Examiners, US Department of Transportation and a certified Medical Interpreter in Spanish. This year alone, more than 1 million Americans are expected to be diagnosed with cancer. In a report written by Bharat B. Aggarwal, Ph.D., a pioneering Biochemist of the M.D. Anderson Cancer Center, he states that only 5 – 10% of all cancer cases can be attributed to genetic defects; whereas the remaining 90 – 95% have their roots in the environment and lifestyle. Ovarian Cancer...What All Women (and Those Who Love Them) Need to Know! Each year more than 21,000 new cases of ovarian cancer are diagnosed in the United States, resulting in over 15,500 deaths. It is the deadliest of the gynecologic cancers, affecting approximately 1 in 70 women. Because there is no reliable diagnostic screening, it often goes undetected or is misdiagnosed until it has advanced to the later stages. Less than 20% of cases are discovered in the early stage. Due to late diagnoses and a very high recurrence rate (70-90%), less than 50% of women make it to the 5 year milestone.

Ovarian cancer does not discriminate and can affect females of any age and ethnic background. It continues to be one of the most underfunded cancers and the lack of public awareness and education is of critical concern. Although the symptoms of ovarian cancer often are not intense in the early stages, they are NOT silent. Once thought to be a ‘silent disease’, research indicates that 95% of women DO have vague but persistent symptoms. These symptoms are not always useful for diagnosis since they may be indicative of other less serious conditions. If any of the following single or combination of symptoms persist for more than 2 weeks, consult your physician and insist that a manual exam and transvaginal ultrasound be performed. If any abnormalities are detected, a referral to a gynecologic oncologist may be in order. • Bloating • Pelvic or abdominal pain • Difficulty eating or feeling full quickly • Urinary symptoms (frequency or urgency) Other symptoms may include: • Extreme fatigue • Indigestion, heartburn, or upset stomach • Shortness of breath • Lower back and/or leg pain • Change in bowel habits (constipation, diarrhea) • Unexplained weight gain or loss • Menstrual irregularities (including post menopausal bleeding) • Pain during intercourse

Did You Know… Even if your ovaries have been removed, it is possible to develop Primary Peritoneal Cancer (PPC), a rare form of cancer, which is closely related to ovarian cancer. The peritoneum is a thin sheet of tissue that protects and supports the abdominal organs. When certain cells transform into cancerous cells, the result is PPC. The cause is not known. Symptoms are more gastrointestinal in nature and may include: · abdominal bloating · changes in bowel habits · an early feeling of fullness after eating Nausea and vomiting may occur when bloating is severe. Medical evaluation and treatment is the same as for ovarian cancer and recurrences are common. There is no screening for PPC.

Prior to his Gastroenterology fellowship, Dr. Arrigo was in practice for 2 years (2011-2013) with the Liver Transplant program in Newark, NJ. There he managed patients with severe liver disease, Cirrhosis and all its complications. Through the outpatient clinic encounters, he experienced managing Fatty Liver disease, Hepatitis C, Hepatitis B, Alcohol related liver disease, auto-immune related liver conditions, and metabolic liver conditions such as hemochromatosis and drug-induced liver injury. Dr. Arrigo likes to think his approach is different from others. His bedside manner is very important to him. When taking a social history, he likes to know personal details about his patients so that he can see the big picture. Most importantly, he LISTENS. He truly wants to get an understanding of what his patients are telling him and he wants them to know that he is truly hearing them. He tries to explain things in a way they can understand. Dr. Arrigo hopes to give his patients an answer to their problem and perhaps a cure or a way to control their chronic medical condition so they can live a better quality of life. He hopes to give his patients confidence that they are in good hands. He loves spending his spare time with his wife, two young children and two dogs. Bye Bye Hepatitis C, Hello Fatty Liver Disease! You may have recently seen many more television commercials for new medications for Hepatitis C virus. The CDC is now recommending that everyone born during 1945 through 1965, also known as baby boomers, get a blood test for Hepatitis C. Advances in modern medicine is and has resulted in significant changes to the most common causes of Liver related disease and Cirrhosis (scar tissue or fibrosis). As Hepatitis C will soon fade away in the coming years with a simple pill, the new threat is the epidemic of obesity in the United States and significant fatty liver disease. Nearly 78 million adults and 13 million children in the United States deal with health issues related to obesity. An entity in the medical field called the Metabolic Syndrome, is a cluster of common medical problems including Obesity, Type 2 Diabetes, elevated cholesterol and triglycerides, hypothyroidism and hypertension. This syndrome can result in significant fatty liver disease and liver injury.

What is NAFLD?

• Avoid heavy alcohol use. • Lower your cholesterol and triglycerides. • Control your blood pressure. • Manage your hypothyroid condition. • If you are diagnosed with NASH, control and manage the above tightly. • See a trained specialist with expertise in gastrointestinal and liver diseases as certain medications and anti-inflammatory agents are available.

Non-alcoholic fatty liver disease is the build up of extra fat in the liver cells that is not caused by alcohol. The liver may contain some fat, which is normal. However, if more then 5-10% of the liver weight is fat, then it is called a fatty liver (steatosis). Fatty liver disease is very common and may go undiagnosed for many years. An estimated 15 to 20% of all Americans have some degree of fatty liver disease, making it the most prevalent liver disease in this country. Some people develop NAFLD even if they do not have risk factors such as the metabolic syndrome, rapid weight loss and poor eating habits.

What is NASH?

Fat in the liver alone could become a worrisome condition and requires evaluation by a specialist in gastrointestinal and liver disease. Elevation of certain liver function tests with fatty liver may lead to significant inflammatory changes in the liver seen on a biopsy and this entity is called NASH (nonalcoholic steatohepatitis: fat plus inflammation). NASH is a concerning condition and is on the rise as a result of metabolic syndrome. NASH can lead to cirrhosis of liver. This may ultimately result a liver transplant and cirrhosis is a risk factor for liver cancer, which unfortunately is also on the rise.

How is NAFLD/NASH Diagnosed?

NAFLD is suspected initially if blood tests show elevation of specific liver enzymes. Other etiologies require investigation and need to be ruled out, however. There may be no symptoms at all. When symptoms occur they include fatigue, loss of appetite, weakness, nausea and abdominal pain, a dull achy sensation in the right upper quadrant of the abdomen. A simple abdominal or right upper quadrant ultrasound can be used to confirm diagnosis. CT scan and MRI abdominal imaging can also be performed. NASH is a histological diagnosis, which means it requires a liver biopsy to see inflammation of the hepatocytes (liver cells) with steatosis (fat) under the microscope. Some newer blood testing is also available and helpful. Advanced liver disease or cirrhosis from NASH may present with jaundice (yellowing of skin or eyes), new onset lower extremity edema or abdominal swelling (ascites) and even mental slowing or confusion.

What can you do to prevent and treat fatty liver disease?

Eating a healthy diet and exercising regularly can help prevent liver damage from starting and can even reverse changes of fatty liver once diagnosed. • Control your Diabetes and work closely with your primary physician or endocrinologist to lower your A1C level. • If you are overweight or obese, lose weight.

It took Julie Harrigan about three years to realize she chose the wrong career. She joined the corporate world after graduating from the University of Delaware, and realized it wasn’t for her. She moved back to Delaware, landed a job at Stingray in Rehoboth Beach and fell in love with the area, the people and Mike Zygmonski, in particular. When Zygmonski, now the wine director and sommelier for SoDel Concepts, decided to move to New York for training, she followed. Harrigan got a job at one of New York’s most popular juice bars, acquiring vital knowledge of the industry and getting hooked on the green juice movement in the process. When the couple came home for good, things had changed. Harrigan finally knew what she wanted to do. Blinds, Shades and Child Safety……

Do you remember all the news stories, a couple years ago, concerning child safety and window coverings? The subject sticks out in my mind obviously because it is related to my work and because I have small children. The concern was mostly over mail order and or stock items that were bought off a shelf and installed, usually, by the homeowner. There were tragedies, huge recalls and lawsuits but fortunately the custom window covering trade was not really involved. I would say that the overall higher quality of product and the experienced sales and installation that comes with a custom retailer helped to keep the trade uninvolved however the custom window covering community definitely took notice and many positive changes have taken place. There is much more awareness about the subject compared to a couple of years ago and that in its self is very helpful but the industry has been very proactive in making their products more child safe as well. On the awareness side of things, it is good to note that there are many products out there that have been around for a long time like shutters and draperies that already are or can be made without child safety’s #1 enemy which is strings. Draperies, when set up to traverse by hand, present no problems for toddlers or even pets who like them because they can easily move the fabric aside to leave wet nose marks on the glass. Shutters are another great choice. They are naturally string free, simple to use, even for children, and are one of the best insulators against both heat and cold. With their timeless look and classic feel they a great child safe choice in window coverings. One way that the industry is changing to become more child safe is the introduction of cordless control into a wider variety of products. Cordless roller shades and cordless honeycomb shades have been around for a while but lately we are seeing the industry really improving and trying to mainstream the option. The string free lift systems are getting better and more reliable plus now we are seeing the option more and more on roman shades and wood and faux wood blinds which was mostly an unavailable option in years past on these popular window coverings. Window covering manufacturers are also making improvements on shades and blinds that do have strings with breakaway mechanisms that will separate when too much stress is applied and they have made it so that small children can’t get their heads between the back of the shade and the strings found there on roman style shades. These are two very important changes. Keep the elements out– Enclose your screen porch! With the return of the fall/ winter season, you may be battening down the hatches of your screen porch. Many homeowners are familiar with the popular sunroom additions on the market today, and those who have purchased one enjoy the extra living space available for family gatherings or evening relaxation as well as high return on investment value. However did you know that existing screen porches can be enclosed as well? Fall is the right time to think about this project. Many of today’s homes have decks or are now often being built with screen porches as a standard feature. Exposed to the elements, decks and patios are often simply too hot or too cold to use much of the year. Screen porches offer shade and some bug protection; but, summer heat, dirt, rain, and winter cold with snow and ice again restrict comfortable usage to a short while out of the year. Studies and testimonials indicate that a sunroom addition, or conversion of an existing porch, is very likely to become the most enjoyed and well used room in the house. They offer protection from bugs, dirt, pollen and bad weather while preserving the outdoor feel of a deck or patio. Sunrooms maximize the seasons you can enjoy your back yard and your view and are today can be more sophisticated in terms of thermal efficiency and ultraviolet protection.

The building industry is strongly committed toward pre-fabricated, panelized, or modular enclosure systems. This is due to the many advantages of making the product indoors in a controlled environment, to close tolerances, using modern equipment and techniques. The jobsite work is greatly reduced for a quick installation to eliminate the stress and mess of a stick framed construction project at your home. While all factory made sunroom additions offer a degree of standardized quality control, there are so many variations of the old 60’s Florida room, that it must be extremely confusing to a homeowner to select the best product for your money. Inexpensive aluminum screen or glass enclosures may install quicker and have less maintenance than a stick built addition, but they conduct heat and cold, the finish can scratch or the kickpanels can dent easily. Better rooms with aluminum structure are now being built by many fabricators using vinyl clad insulated panels and powder coat finishes (like a car) for added durability. The best have thermally broken wall mullions, insulated foam core walls and can be purchased with insulated UV blocking glass. Over the past few decades, many products formerly made of wood or metal are now made of vinyl. Look at what happened to aluminum siding, cast iron drain pipes, and alls sorts of car parts,

and today you see vinyl everywhere. While there are some vinyl clad sunroom products on the market, they tend to be expensive with bulky frames since the vinyl is not structural and must have aluminum internal support. These rooms are similar to “stick building” and must be assembled from parts one piece at a time on site. This places the quality control in the hands of the installer and workmanship can vary. Don’t let the corners of your pantry go to waste or things hide deep in the back of a shelf where they may never be seen again. Corner shelves are a great use of space, but items that find their way to the corner are very difficult to find and get.

What to Do if Involved in a Motor Vehicle Collision Susan D’Alonzo Ament is a partner with Morris James LLP and has practiced for 33 years representing personal injury victims. Over the years, Mrs. Ament has found that some women don’t understand the crucial need to have adequate automobile insurance coverage in the event of a serious accident. Did you know in 2015, 24,066 traffic collisions occurred in Delaware resulting in 8,058 people injured and 133 fatalities? If you or a loved one is ever involved in this unfortunate circumstance, do you know what to do? The following is a list of steps you should follow to protect yourself or loved one involved in a motor vehicle collision: 1. Stop your Vehicle. Never drive away from the scene of a collision, even if it’s a minor collision. The vehicles should remain where they are unless they interfere with traffic. Turn your hazard lights on, set out cones or other warning devices if you have them; 2. Call 911 or the Police. Even if there are no serious injuries, call to report the collision. An officer will be sent to file necessary reports and exchange information as well as medical personnel for injuries; 3. Take Photos. Use your camera or phone to photograph the collision scene, damage to the vehicles and depict how the collision took place; 4. Retrieve Relevant Information. Record the names and contact information of the other drivers or witnesses, license

plate numbers, the make, model and colors of other vehicles, insurance information and the exact location of the incident; 5. File a Collision Report with the Police. In some circumstances the police will not come to the scene of a collision. If this happens, you should still contact the police to report the collision. A police report may help you make a claim with the insurance companies; 6. Contact your Insurance Company. Report the collision to your insurance company even if the collision was not your fault; 7. Do not give a Recorded Statement over the Telephone. Do not give a recorded statement

to the other driver’s insurance adjuster without an attorney present;

benefits and coverages before a collision. Never wait until after an incident to learn this information. For example, do you know if you have a deductible, rental car coverage or enough wage loss coverage?

8. Seek Medical Attention. Often injuries are not readily apparent in the hours or even days following a collision. Unless you are absolutely sure you were not injured, you should be examined by your family doctor or emergency medical professionals for injuries;

Train, Remembering Safety First, Management and How Vital it is to set your dog up for success Training your dog to ‘listen’, ‘respond’ and ‘perform’. Always keeping Safety First by Managing your dog/puppy will ‘Set your dog/puppy up for Success!’ in the Training of your dog/puppy. There are three rules a dog owner needs to remember while Training their dog. One: Safety First! If in doubt always use precaution in order ensure that your dog, other dogs and even the humans are safe from any ‘uh-ohs’ that could take place. Know your dog/ puppy. Know the trigger points by recognizing what will set your dog off and cause a serious behavior problem or a bad situation. When you know the trigger points you will recognize at best your dog’s body language where you will now be able to redirect your dog back to you. You are now able to keep him/her under Your control and out of an unwanted situation. Better yet, before your dog/puppy is able to respond to a situation ‘Set him/her up for success!’ keeping the environment positive. You now are able to keep your dog from reacting or even having to protect him/herself. Train yourself to recognize what the trigger points are, look like with your dog. Two: Management! Do understand that Management is OKAY! When you ‘manage’ your dog/puppy you are using the ‘tools’ needed to keep unwanted behaviors from arising. Now you

can reward the wanted, ‘Good’, behaviors. This simply means that you are putting your dog in a positive situation where your dog/puppy is unable to perform an unwanted behavior. Or worse cause your dog to ‘protect’ him/ herself. With the ‘management’ of your dog, you may need some, but not all, of the tools mentioned below. These tools can be crates, leashes, baby gates, kennels, Treats!, etc. These are not to be used to punish but help with the training and success of your dog/ puppy. By implementing the use of the tools you are ‘Setting your dog up for success!’ while training the ‘good’ responses to a situation. Three: ‘Set your dog/puppy up for SUCCESS!’ By keeping Safety First, implementing the Management tools you are ‘Setting your dog/puppy up for SUCCESS!’ All dogs and puppies earn the privilege to have more freedom inside and outside your home. Mark the good behavior with REWARD, REWARD, REWARD and Mark the ‘Bad’ with at least ‘Nope’, ‘No’, or ‘Eh’. Using the tools necessary will assure success with the Training of your dog/puppy! Train, Remembering Safety First, Management and How Vital it is to Set your Dog Up for SUCCESS.

“ GIVE US ONE TRY, AND YOU WILL KEEP COMING BACK ” A conversation with Shawn Xiong of Confucius Chinese Restaurant

Confucius Chinese Restaurant was opened in the spring of 2004 in downtown Rehoboth Beach. Shawn Xiong, the owner, had previously owned a Chinese restaurant in Wilmington prior to relocating to the beach area. Since then, Confucius has been named the best Chinese restaurant in the Delaware Today magazine and favorably mentioned in various newspaper and magazines. We had a chance to talk with Shawn XIong and the following is the summary of our conversation. Q: Why do you call your restaurant “Confucius”? X: When I started Confucius, I wanted to offer a menu that most “generic” Chinese restaurants offer. Wherever you go, you probably will

find a Chinese eatery in every shopping center that offers pretty much the same items such as lo meins & chow meins. Our menu is more focused on items you don’t usually find in those places. So I decided that I will not name my place “ wok” or “great wall” or “palace”, yet I want people know it is Chinese. And everybody knows who Confucius is. Q: What made you decide to relocate to Rehoboth? X: When I had my previous restaurant in Wilmington, I was working 360 days a year, 12 hours a day. After I had my son, I decided that I wanted to spend more time with him, more time with his education. Coming to a resort town, it allows me to take some time away from work without feeling guilty about it. Q: What was your biggest challenge coming to Rehoboth? X: There were plenty of challenges

I have faced. The biggest of all was not knowing the market when I first opened. The other challenge was (and continues to be especially during the summer) to convince people that Chinese restaurant IS INDEED more than just Fried Rice and Eggrolls. There are more than 40000 Chinese eateries in this country (more than McDonalds’, Pizza Hut and all other fast food restaurant COMBINED). While this has made Chinese food popular in this country, it has made Chinese food a stereotype (think “$5.99 eat all you can eat,” “free deliveries” for example). I have to explain to people what we do, but once people give us a try, they keep coming back. Q: What are your most popular dishes? X: This is a hard question to answer. There are different popular dishes to different customers depending on their own personal taste. If you like spicy food, we offer dishes

such as Hot Pepper Chicken, Cumin Beef or Kung Pao Shrimp. If you like fish, Salmon with Black Bean Sauce, Steamed Halibut or Crispy Whole Black Sea Bass are all excellent choices. If you are a first time customer, we encourage you to tell our server or what you do or do not like, and we will offer the right recommendation for you. I believe every customer deserves special attention at my restaurant. If they order something they don’t quite like, they are welcome to send it back for exchange AT NO COST. I want customer to have a pleasant dining experience at my place. Q: What are your hours and Specials? X: Confucius is open 7 days a week. Call for hours and reservations.

“Shasta Shutterbug” Has Arrived!

BY: MIKE BAKER I always like to do things with my photography just a little different or something more my way. My “Shasta Shutterbug” camper photo booth was an idea I saw online from California and Florida. My niece, Ashley, had sent a couple of pictures to me, and I started to research. It’s also something popular in Europe. Most of those are from the back of a VW bus or a similar style vehicle. I initially spotted this 1969 Shasta Compact Camper for sale about a year ago alongside a local road. Not knowing what I could do with it, I somewhat shelved the idea of buying it. After I had seen some ideas online, I knew I needed that “Shasta.” I was sure it was already sold. I took a 15-minute drive where I saw it previously. There it was still sitting there waiting for me. I immediately jumped on the idea. I purchased the camper and started looking at different ones all over the US. There are not many “camper photo booths” out there. I never realized how big photo booths are becoming. I shoot many weddings throughout the year and started noticing the appeal. I figured photo booths would be more of a short-term fad and dwindle away. Not true! They are getting more and more popular. So this was my winter project! So ideas started rolling through my mind. I had my daughter, Courtney, listen to my ideas. Not good. Seems like she had better ideas then I did. Most 21 year olds know more about what attracts the younger crowds. She’s the designer, I did the engineering and labor. My 21-year-old daughter knew what was “in” more than I did. She was attending UD while I worked on it and came home to check on it (and my work). I was blessed with a relatively warm fall and winter. One never knows what’s underneath until you start pulling it apart. I replaced some walls and floors from 47 years of weathering. Put new ceiling and wall coverings with a hardwood floor. It is decked out with a stereo system inside and outside, a photo booth and seating. Sorry no room for rest rooms. After Courtney placed it on Facebook, it was booked instantly the next day for 2 bookings. It was an instant hit at the 2016 Heart Ball in Dover. We also made a 2 day appearance at the Dover International Speedway on the recent race weekend. We have gotten many bookings already for weddings, gala events and other outdoor events. We also have the standard photo booth, but I think this one has gotten much attention. After all, this is the first one in the First State!
