2012 SFPGA GOLFPASS PROGRAM Please read the following information before using your 2012 SFPGA GolfPass. Important instructions and the terms & conditions of the SFPGA. GolfPass program are provided below.

Course Information: All courses participating in the 2012 SFPGA GolfPass program (as of March 1, 2012) are located within this booklet, organized in alphabetical order, by county. Additionally, courses may be found using the alphabetical listing beginning on page 17.

Length of Program: All SFPGA GolfPass discounted rates apply from May 1–October 31, 2012, unless otherwise noted.

Plays: Each course is listed as either Unlimited Play, or will have a specific number of plays listed under its description, along with an appropriate number of boxes representing those plays. If a course offers a limited number of plays, you must present this SFPGA GolfPass book at the time of participation. The boxes will be checked off or dated by course personnel at the time of play. A facility offering limited play(s) may refuse participation if this SFPGA GolfPass membership book is not presented or if the name found within your GolfPass book does not match your identification (ie. Driver’s License). Unlimited play is available at the majority of the courses and does not require any denotation within this SFPGA GolfPass membership book by course personnel. Unlimited play courses will allow you to use your SFPGA GolfPass as many times as you wish throughout the summer. It does NOT mean that SFPGA GolfPass members can play a course repeatedly on the same day for the listed price. At courses offering unlimited play, the SFPGA membership book or SFPGA GolfPass card are both acceptable forms of member identification. Please note that unlimited play refers to the frequency of play, whereas the “Anytime” or “After” designation refers to the actual time (am or pm) available each day.

Fees: All fees are per person, include cart & greens fees, and are exclusive of tax unless otherwise noted. If a GolfPass facility advertises a rate that falls below their posted GolfPass rate within the dates of the GolfPass program, that GolfPass facility will match the lowest rate published/available to the general public at the time of play.

Tee Time Availability: Please inquire with all courses regarding their policy upon reserving your tee time. You must identify yourself as a SFPGA GolfPass Member when telephoning a course for tee times. CALL FOR RATES / TBD: If a facility is listed with CALL FOR RATES / TBD where an actual GolfPass rate should be listed, the facility was unable to configure a discounted rate for the SFPGA GolfPass program before the printing of this book. This listing means that rates for SFPGA GolfPass members are still to be determined - the South Florida PGA will update the SFPGA GolfPass website and send out emails to the 2012 SFPGA GolfPass membership once a rate has been established. If you wish to play a facility that lists CALL FOR RATES / TBD and no update has been provided at that point in time, please contact the specific GolfPass facility for their GolfPass rate.

Dress Code & Club Policies: Please inquire about proper attire for each course when calling for tee times. Golf course personnel may refuse play if a SFPGA GolfPass customer does not meet proper dress code requirements. There are absolutely no short shorts, no tank tops or tee shirts, no jeans or cutoffs allowed. Golf shoes are required. Golf course personnel at all SFPGA GolfPass facilities are entitled to enforce their club policies at all times. Remember, you are representing the South Florida PGA when utilizing 14
SFPGA GolfPass Membership Identification: Facilities participating in the South Florida PGA GolfPass program may refuse usage or confiscate your SFPGA GolfPass membership card/book if the membership label located on the back of the GolfPass card or found within the membership book has been altered in any manner or if proper personal I.D. is not provided. The SFPGA GolfPass membership book and card are not valid without the South Florida PGA logo on the label. The discounted rates offered through the SFPGA GolfPass program will only be available to SFPGA GolfPas members at the time of play. The SFPGA GolfPass Membership is non-transferable. There is a $10.00 replacement fee is the SFPGA GolfPass book is lost or stolen. SFPGA GolfPass cards will not be replaced. The SFPGA GolfPass program disclaims all warranty as to merchantability and fitness for purpose intended or implied. Availability of facilities, dates and times of play and prices are subject to change without notice. Your South Florida PGA GolfPass membership not only entitles you to discounts at an incredible selection of courses, but it gives you the opportunity to participate in the GolfPass Member-Only Outing Program (additional fee per outing applies). These outings are your chance to play at 12 exclusive courses at a fraction of their normal fees.
FESSIONAL VERIFYING CURRENT HANDICAP INDEX AT EACH OUTING. NO EXCEPTIONS! THE MAXIMUM HANDICAP IN THE HANDICAP DIVISION IS 24.
RULES & REGULATIONS
SFPGA GolfPass Outings are held in a friendly, informal golf environment with three skills divisions available, each with its own format to help make the event competitive, yet enjoyable for golfers of all skill levels. Prizes are awarded to the top finishers in each of the divisions. Sign up as an individual, with a friend, or with a group and play in any or all of the outings held in 2012!
ELIGIBILITY:
All participants must be 18 years or older.
FORMAT:
All competitions are based on individual scores unless otherwise noted at the event. Competitors can only compete in one of three divisions: HANDICAP, CALLAWAY or SCRATCH. IF COMPETING IN THE HANDICAP DIVISION, THE COMPETITOR MUST PRESENT AN UPDATED HANDICAP INDEX CARD (WITHIN 45 DAYS OF THE EVENT) OR A LETTER FROM A PGA PRO66
Failure to produce a current USGA Handicap Index Card automatically puts the competitor into either the Callaway or Scratch Division. SFPGA staff retains the right to adjust competitor handicaps based on past GolfPass outing performance. Divisions will be established on-site.

Outings are for South Florida PGA GolfPass Members only, unless otherwise noted. Players will be paired together, but the format of the event will be individual stroke play competition. GolfPass members must call in the name of their guest to register, or send in his/her name along with a mailed entry form - no online registration is available for GolfPass members who wish to play with a guest.

Soft Spikes only and proper dress required at all outing sites. No short shorts, jean-shorts, cutoffs, t-shirts. If you do not cooperate with the dress code, you will be asked to leave the premises or purchase clothing in the Pro Shop of the host facility.
ENTRY FEES:
Outing entry fees include cart fee, greens fee, range balls (if applicable) and prizes to division winners. ALL PAYMENTS WILL BE PROCESSED ONCE RECEIVED BY THE SFPGA. THE SFPGA WILL NOT HOLD CHECKS OR BE RESPONSIBLE FOR RETURNED CHECKS!! THERE WILL BE A $25.00 SERVICE CHARGE ON ALL RETURNED CHECKS.
OUTING REGISTRATION:
You may register for GolfPass Outings either online at www.sfpgagolf.com or by sending in a hard copy entry. Processing of SFPGA GolfPass Outing entries (Hard copy by mail and online) opens on March 1, 2012. Participants are encouraged to register at that time. ONLINE REGISTRATION INSTRUCTIONS:
Visit www.sfpgagolf.com and click on GolfPass on the top navigation toolbar, then click on GolfPass Outings. All of the outings will be listed. Select the outing in which you wish to play. When the pop-up screen appears click the link that says REGISTER ONLINE next to your designated outing. You will be prompted to enter your LAST NAME and CUSTOMER ID NUMBER. The 8-digit Customer ID number can be found on your GolfPass card or on the inside front page of this book. If you purchased your GolfPass at a retail store, golf course facility, or other, please call the SFPGA to retrieve your customer ID. You will then follow the onscreen instructions. Your credit card will be charged before the 68

PAIRINGS:
all outing rounds on the scheduled day. If inclement weather suspends play and play cannot be continued, the outing will be deemed complete and division winners will be announced unless otherwise decided by the SFPGA Staff.
Every effort will be made to grant special request for pairing twosomes, threesomes and foursomes. ALL PLAYERS ARE ASKED TO PLEASE DOUBLE CHECK YOUR TEAM’S ARRANGEMENTS BEFORE YOU SIGN UP!! Pairing requests may be made during online registration or should be written on the hard copy entry form prior to submission to the SFPGA. Once pairings are completed, NO CHANGES will be accepted. All changes prior to the deadline must be made by written correspondence via mail, fax, or email (see contact information listed above). No tee time or pairing changes will be accepted over the telephone.

Tee time requests may be made in writing by contacting Meredith Schuler at mschuler@pgahq.com, but no tee time requests are guaranteed. GolfPass members may request a tee time in one of two ways: EARLY or LATE. Tee time requests may be granted via these two time segments, but no specific time requests will be taken For example, If a tee time profile begins at 7:30AM, the “early” segment would range from 7:30-8:30AM, the “late” segment from 8:30-9:30AM. Available outing tee times will be split in half to designate early or late.
If a participant misses his/her tee time, there is no guarantee that the participant will be able to play in the event & NO REFUNDS will be given. Be sure to confirm your tee time prior to the event and arrive on-site at least 30 minutes before your starting time. Slow play during events will not be tolerated. Warnings and penalties will be given to those participants deemed out-of-position.
DIRECTIONS TO OUTING SITES:
It is recommended that all players contact the host club prior to the event to re-confirm directions. All up-to-date outing addresses will be listed on the SFPGA website. It is the player’s responsibility to get directions to each outing site. No refunds will be given to tardy players.
ON-SITE REGISTRATION:
On-site registration is required at each event, and you must present you GolfPass card or membership booklet in order to play.
INCLEMENT WEATHER:
SFPGA GolfPass Outings are conducted in South Florida’s rainy season and all participants are expected to continue to play in the rain unless the SFPGA Staff and/or host PGA Professional signals a horn that lightning or other inclement weather is nearby. Play will then be suspended until further notice by the SFPGA Staff and/or host PGA Professional. It is the SFPGA’s intent to complete 70
