A Calcium-Rich Diet Can Be Just What the Doctor Ordered When we’re young, calcium is essential to building strong, dense bones. But as we age, calcium is just as important to keep bones strong and healthy. “Too many Americans fall short of getting the amount of calcium they need every day and that can lead to bone loss, low bone density and even broken bones,” said Diane Harrington, RD, LD, nutritionist at AdventHealth Shawnee Mission. For women age 51 or older, the recommended daily allowance (RDA) is 1,200 mg. of calcium. For men age 70 or younger, it’s 1,000 mg of calcium, rising to 1,200 mg for men 71 and older.

Baked Macaroni and Cheese

“Food is the best source of calcium, since our bodies cannot produce their own,” said Harrington. “And this Baked Macaroni and Cheese recipe is both calcium-rich and delicious!”

Directions: 1 Preheat oven to 350 degrees. Cook pasta according to package directions; drain. 2 Combine cooked pasta with remaining ingredients. Place in 13 x 9 inch pan that has been sprayed with vegetable or olive oil cooking spray. 3 Bake uncovered for 45 minutes to one hour, or until bubbly and the top is golden brown.

Fighting Chronic Pain Takes a Team Effort They say that into each life, some rain must fall. But at some point in our lives, especially as we get a bit older, the chances are that we’ll have to deal with something much more serious than some precipitation. In fact, according to the American Association of Neurosurgical Surgeons, just about every adult in the U.S. will suffer from back and other nerverelated pain including pulled muscles, degenerative disease and trauma. As the spine ages, some people develop diagnoses such as a herniated disc or spinal stenosis in which case they will experience back, neck or extremity pain as well as symptoms such as arm or leg pain or weakness or sciatica. The question is: what’s the best way to address these painful situations? For most people, the first step is to be seen by their primary care physician. “Whether the patient needs surgery or not, the primary care provider is an integral component in helping the patient experiencing pain in the back, neck or limbs. This role can range from arranging physical therapy or simply changing expectations that a condition 6 | AdventHealth | FALL 2019


cannot be cured but can be managed effectively,” said Darren Lovick, MD, neurosurgeon, AdventHealth Medical Group Neurosurgery at Shawnee Mission. If the pain that does not subside or has fluctuated for an extensive period of time, your primary care physician may refer you to a neurosurgeon. A neurosurgeon is a Board-certified physician who specializes in preventing, diagnosing and treating problems related to the nervous system. This includes the brain, spinal cord, spinal column and also the nerves that branch out from the spine – the peripheral nervous system. Of course, if someone is referred to a neurosurgeon, it’s only natural to assume they need an operation. Fortunately, that’s not the case. Most patients will benefit from non-surgical treatment, and appropriate referrals can be made to those specialists such as pain management. Neurosurgeons evaluate and recommend treatment for a variety of conditions, including: 33 Cervical spine disorders 33 Herniated disc 33 Lumbar spinal stenosis 33 Sciatica. Neurosurgeons are equipped with the latest technologies to diagnose, treat and improve the lives of patients – from evaluating their health, medical history and symptoms, to diagnosing the issue and working with the patient to create a personalized treatment and recovery plan. “The goal is to try and determine the cause of the pain and what is the next best step in trying to resolve it,” said Lovick. “In fact, most of the patients we see do not need an operation, or we refer them for less invasive options for initial treatment.” While it can be difficult for a patient in severe pain to comprehend that they will likely improve with time and physical therapy, most neurosurgeons will typically recommend 12 weeks of physical therapy before surgical treatment.

If a patient fails physical therapy, or can’t perform physical therapy because of severe pain, a physician may recommend other non-operative measures like epidural steroid injections, for example. This outpatient procedure can help blunt the pain by reducing the swelling around inflamed nerves. Often, a steroid injection can reduce pain enough for the patient to start a course of physical therapy. If a patient is still in pain or experiencing neurological symptoms after trying both physical therapy and injections, then it may be time to start considering surgery. “For back and neck pain, surgery is often used to decompress a nerve that is causing pain radiating down an arm or leg,” said Lovick. hether it’s stubborn low back pain, an aching neck or sciatic nerve pain shooting down your legs, chronic back pain can sideline you from work and your favorite activities. In fact, low back pain in particular is responsible for 2.6 million visits to the emergency room each year and is the leading cause of disability worldwide. But most people with chronic back pain can avoid surgery or pain pills by implementing specific healthy lifestyle choices, many of which are easy to do at home, with no doctor’s prescription necessary, including: Eat an Anti-Inflammatory Diet | Foods that fight inflammation include brightly-colored fruits and vegetables, fatty fish (such as salmon), nuts and seeds, green, leafy vegetables and monosaturated fats such as avocado and olive oil.

Keep Moving | Taking a break from all physical activity when your back hurts can actually make the pain worse. A combination of strength training, flexibility exercises and aerobic activity can effectively prevent and control chronic back pain.

Get Enough Calcium | Depending on age, most adults should get between 1,000 to 1,200 mg per day. Too much calcium can cause heart problems and increase the risk of bone fractures.

Quit Smoking | Smoking inhibits blood flow and prevents tissue throughout the body from getting oxygen and nutrients, which can cause the spine and back muscles to weaken.

Achieve a Healthy Weight | Losing weight can help control your back pain, prevent the condition from getting worse and even potentially eliminate the need for pain medication or surgery.

Of course, there are times when surgery is the only option. However, anyone considering surgery for low back pain should carefully discuss the risks and benefits with their health care team before deciding to proceed.

Practice Yoga | Regardless of the type and severity of your back pain, yoga can significantly reduce your symptoms. Some patients are able to avoid pain medication and surgery altogether.

Consider These Natural Anti-Inflammatory Supplements for Back Pain Magnesium | For relief of back pain, getting tense muscles to relax is critical. Taking the right type and dose of magnesium can help. For muscle relaxation and back pain relief, look for magnesium glycinate or magnesium citrate. Avoid magnesium oxide, as this magnesium isn’t well absorbed in the digestive tract. Dose: 200 to 500 mg, either combined or can break up the dose depending on how your digestive tract is reacting. Magnesium can also help those with mildly elevated blood pressure, mild anxiety, mild sleep disturbances, constipation, menstrual cramping, migraines and asthma.

Omega-3 fatty acids from fish oil | A 2006 study showed that 80 percent of non-surgical back pain suffers experienced relief from back pain symptoms after taking between 1200 mg and 2,400 of DHA/EPA, Omega-3 fatty acids for an average of 75 days. Find a brand with a higher than industry standard quality assurance protocol. “Third party testing” can be a good sign of a higher quality fish oil, too. And never buy fish oil in a bottle that you can easily see the fish oil capsules. Fish oil does have blood thinning action, so discontinue five days prior to any surgical procedure.

Curcumin, extract from herb turmeric | In a review of studies, use of curcumin significantly decreased pain severity in patients with osteoarthritis. Curcumin also showed little to no gastrointestinal side effects, which is a common side effect of NSAID pain relievers. One of curcumin’s limitations is its poor gastric and intestinal absorbability. But bioavailable forms called Meriva or Theracurmin have been shown to have the highest blood levels of curcumin after oral ingestion. Dose depends on the form/ absorbability potential, so talk with a knowledge provider on dosing.

Anesthesia and Pain Medicine, on what injections can do for spine pain What are the most common conditions that cause pain in the spine? Nerve pain typically will radiate down an arm or a leg and can be associated with numbness, tingling or the feeling of arm or leg weakness. Arthritis pain is usually an achy pain in the low back or neck with a feeling of stiffness in the morning and can be worsened with bending the spine backwards. Muscle pain is often very tender to touch close to the skin surface.

Do injections help some patients avoid surgery for pain relief? The majority of patients with spinerelated pain don’t need surgery. In many cases, the back pain can improve with time and more conservative measures. Injections 10 | AdventHealth | FALL 2019

can give individuals a window of comfort to remain functional and active while the body is healing.

Why do injections help relieve pain more than pills? If you inject a potent anti-inflammatory directly at the site of inflammation, you can achieve a greater degree of pain relief which can last much longer compared to medicines by mouth.

How do you determine the proper placement of the injection? Needle placement is usually guided by imaging such as low dose X-ray (fluoroscopy) or ultrasound where one can visualize the target and guide the needle to it. Once the needle is in the correct position, it is common to inject contrast dye to ensure the medicine will spread where it needs to be before injecting the treatment drug.

How long does it take for an injection to take effect? A steroid typically takes two to four days to reach peak effect but can take up to seven to 10 days in some individuals. Radiofrequency ablation involves burning very small nerves through the tip of a needle that conducts electricity. Nerve ablation commonly takes two to four weeks for the patient to achieve pain relief.
