S

cience tells us that a body at rest tends to stay at rest. And that can be a problem. In fact, research shows that obesity increases the chance of suffering from COVID-19 by 40 percent. No wonder millions of people all over the world are jumping at the chance to take online fitness challenges — exercise programs designed to maximize results in a short period of time, often just one month. The challenges can range from beginner to expert, and they encourage you to: 3 move and exercise throughout the day 3 maintain a healthy weight by being mindful of your habits 3 feel part of a community 3 set goals and work toward those milestones.

Welcome to the Work(Out)-From-Home Generation Here are just a few examples of the many fitness challenges you can find online: Step It Up Challenge If you’re not used to a lot of activity, a step challenge can be a great way to get moving. The typical activity goal is 10,000 steps a day. Stand Challenge Simply standing up on a regular basis can have a powerful, positive impact on your health. Office work can force you to spend up to 85 percent of your time sitting — which has been linked to an increased risk of heart disease and diabetes. Targeted Muscle Group Challenge These challenges help you focus on one specific part of the body where you’re looking for improvement — toned arms, a stronger core — there’s a challenge for just about everything and every body. Talk to your doctor before you start an exercise program. Visit MyHealthKC.com to find the right primary care doctor for you.

Y

ear in and year out, back pain is one of the most common reasons for visits to see a doctor. Aging, faulty body mechanics and trauma can all harm your spine, leading to back pain and other symptoms. Many spinal problems occur in the lower lumbar portion of the back — which carries most of your body weight — and in the cervical spine (neck). Both areas are susceptible to wear and tear because of amount of motion they support. “Most of the spinal patients we see are either suffering from the degenerative process that can cause stenosis (a chronic narrowing around the nerves) or from a sudden change, like a ruptured disc or cyst, for example,” said Steven Hess, MD, Neurological Surgeon, AdventHealth Shawnee Mission.

Whatever the cause, are you a candidate for spinal surgery? Fortunately, many spine problems can be treated non-surgically. Physical therapy, home exercises, medication and spinal injections are often recommended prior to considering surgery. “Time cures a lot of problems, along with rest, ice and heat. Physical therapy may strengthen the surrounding area, which can also relieve pressure and pain,” said Hess. If the problem isn’t resolved by these conservative measures, then surgery can be a good option. For example, if someone has pain in the extremities and non-surgical management has not provided relief, then surgical intervention can be the best decision — especially for those with significant pain, numbness or weakness in an arm or leg.

8 | AdventHealth | FALL 2021

Thanks to advances in minimally invasive technology and training, spinal surgeons are now able to maximize the benefit for the patient and minimize the ordeal of surgery. “With minimally invasive spinal surgery, we are able to disrupt as little tissue as possible, while accomplishing the task,” said Hess. Minimally invasive spine surgery is aided greatly by microscopic techniques to reduce the exposure needed for spinal implants. “Microscopic minimally invasive spine surgery offers better visibility and light, so we can operate more precisely and with smaller incisions,” said Hess. More recently, navigationbased spinal surgery allows surgeons to utilize computerguided techniques, which help make the surgery safer and the placement of spinal hardware more accurate. And navigationbased techniques are now being combined with robotic surgery. Whether the surgery is minimal or a more fully open procedure, the goals are to fully decompress neurostructures and minimize tissue damage. “When a patient’s life is seriously disrupted by the pain and disability they’re experiencing — and when we have a very good chance of helping them feel better and get back to their life — this is when spinal surgery is most appropriate and valuable,” said Hess. Your pain condition or injury should first be brought to the attention of your primary care doctor, who can determine if you should be referred to a neurosurgeon. When Your Back’s to the Wall

What are the most common spinal procedures? 3 Laminectomy: A thin bony plate on the back of the vertebrae known as the laminae is removed to create more space within the spinal canal and ease pressure. 3 Laminotomy: A part of the lamina (vertebral arch) that covers the spinal cord is removed to create more room to relieve pressure. 3 Discectomy or Microdiscectomy: Herniated discs are removed to relieve pressure from the compressed nerve.

How to Know if You Might Need Spinal Surgery

3 Foraminotomy: Removal of tissue or bone at the passageway known as the neuroforamen, where the nerve roots branch off the spinal cord and leave the spinal column. 3 Spinal Fusion: Two vertebrae are fused together using a bone graft and, frequently, instrumentation. 3 Disc Replacement: The damaged disc is replaced with an artificial one as an alternative to fusion.

Steven Hess, MD, Neurological Surgeon, AdventHealth Steven Hess, MD, is a Board-certified neurological surgeon with AdventHealth Medical Group Neurosurgery at Shawnee Mission. After earning his undergraduate degree in chemistry at Fort Hays State University, Dr. Hess earned his medical degree from the University of Kansas School of Medicine. Before graduating, he completed a clinical clerkship at Barrow Neurological Institute in Phoenix, Arizona. After graduating, he went on to complete an internship in general surgery and a residency in neurosurgery at the University of Kansas Medical Center. Dr. Hess has long been on the medical staff at AdventHealth Shawnee Mission (formerly Shawnee Mission Medical Center) and he joined AdventHealth Medical Group in 2021. Previously, he had been in private practice for nearly 30 years.

ASK-A-DOCTOR

Andre Jackoi, MD,

Orthopedic Surgery Specialist, comparing lower back pain to hip pain

What are the main symptoms of lower back conditions? Usually, it’s pain across the small of the back, just above or at the waistline. Sometimes, the pain is linked to pain that may travel down the back or side of the leg into the buttock or all the way down to the foot (or anywhere in between). This may affect one or both legs.

What are the main symptoms of hip conditions? Typically, the pain is located in the groin area, and includes an inability to lift the thigh, limited range of motion of the leg (particularly difficulty with crossing your legs), difficulty with going up and down stairs, walking abnormalities and pain.

10 | AdventHealth | FALL 2021

Why can it be so difficult to tell the difference between hip and lower back pain?

How can lifestyle and exercise changes affect lower back and hip health?

Both low back (lumbar) and hip pathology may cause buttock pain, lateral hip pain or even groin pain. Sometimes, the lumbar may cause weakness with hip flexion just like hip pathology. They both may cause gait disturbances and a patient will often have both a hip and lumbar issue.

Obesity has been found to cause an increase in degeneration of the hips, knees and lower back. Lifestyle that combats sedentary routines does well to combat this. Diet certainly can be a component in the overall picture, but activity on a daily basis helps prevent many unhealthy components linked to a sedentary lifestyle.

What are the best tools to diagnose these types of issues? X-ray imaging is the gold standard for diagnosis. A detailed conversation with the patient about their complaints and injuries is always required. Lastly, and most importantly, a thorough physical examination is required for all patients to differentiate between the two areas of concern.

Your pain condition or injury should first be brought to the attention of your primary care doctor, who can determine if you should be referred to a surgeon. For more information, visit AdventHealthKC.com, or to match with the right primary care physician for you, visit MyHealthKC.com.

Strain or Sprain? Either Way, It’s a Pain Know what the difference is and know what to do

O

ur bodies do a lot of activities each day, whether it’s exercising or playing a sport, lifting heavy objects or doing a repetitive motion for an extended period of time. Of course, there is also the unplanned tripping or fall and — especially in winter — slipping on a slick surface. So it’s no wonder that most of us have to deal with an occasional strain or sprain. Strains affect the tendons or muscles, while sprains affect the ligaments. Both are connective tissues and both can range from being mild to severe.

For mild strains and sprains, treatment is pretty much the same. It’s a technique known as RICE, which stands for Rest, Ice, Compression and Elevation.

R

Should you visit your doctor? If you experience any of these symptoms, you should see your primary care physician about your sprain or strain: 3 difficulty standing or walking without pain 3 inability to move or flex the affected joint 3 numbness or tingling around the joint Visit MyHealthKC.com to find the right primary care doctor for you.

14 | AdventHealth | FALL 2021

I

REST: Give the joint time to heal by staying off it as much as possible. ICE: Apply ice to the affected area for the first 24-48 hours — 20 minutes on and then 20 minutes off. Caution: never apply ice directly to your skin. Wrap a thin towel or piece of clothing around a bag of ice.

C

COMPRESSION: To help reduce swelling, wrap the affected joint in a bandage or trainer’s tape. Caution: wrapping too tightly can reduce the blood supply.

E

ELEVATION: Keep the affected joint above the level of your heart — this will help reduce swelling. If that’s not possible, keeping it parallel to the ground is also okay.
