Exercise & Fitness
Interval Training: The Best Way to Work Smarter – Not Longer Staying physically active is a vital part of maintaining a healthy lifestyle. But are some exercise options better than others? You might want to consider interval training – especially if your current routine is feeling stale or doesn’t seem to be helping much. In a nutshell, interval training means alternating short bursts (about 30 seconds) of intense activity with longer intervals (about 1 to 2 minutes) of less intense activity. For example, you might combine leisurely walking with periods of faster walking. If you're walking outdoors, you could walk faster between certain mailboxes, trees or other landmarks. By changing the intensity of effort, you can get maximum cardiovascular benefits – including decreasing your risk of heart disease and high blood pressure.
Here’s How Interval Training Can Help: 3 Burn more calories. The harder you exercise, the more calories you burn — even if you increase intensity for just a few minutes at a time. 3 Make better use of your time. An interval training workout takes less time than a standard cardiovascular workout – without sacrificing results. 3 Improve your aerobic capacity. As your cardiovascular fitness improves, you'll be able to exercise longer or with more intensity. Imagine finishing your 60-minute walk in 45 minutes! 3 No need for special equipment. You can perform intervals walking, running, biking or swimming. Or you can work out on an elliptical trainer, treadmill or exercise bike. 3 Beat workout boredom. Turning up your intensity in short intervals can really spice up your exercise routine. Before starting a new fitness regimen, be sure and talk with your primary care physician. A Helping Hand: Easing the Aches of Gardening
6 | AdventHealth | SUMMER 2021
F
or the avid gardener, tending flowers, trimming bushes, planting a new tree and even mowing the lawn can be a wonderful way to relax and enjoy the outdoors. British horticulturalist and garden designer, Gertrude Jekyll, seemed to say it best: "The love of gardening is a seed that once sown never dies." Too often though, having a green thumb can lead to pain and injury in the hands or arms as the gardening season heats up. “As an occupational therapist, the most common conditions we see in the hand that can cause pain, limit range of motion and decrease someone's ability to participate in daily activities would be tendonitis conditions affecting use of the thumb and the wrist, as well as osteoarthritic conditions affecting the thumb, fingers and wrist,” said Katie Pascuzzi, Doctor of Occupational Therapy (OTD), OTR/L, at PT Solutions Physical Therapy. Also known as "wear and tear" arthritis, osteoarthritis causes the smooth, protective cartilage on the ends of your bones to wear away, causing pain when the bones rub together. “Rheumatoid arthritis – which is genetic – also affects the hands and fingers. This form of arthritis is controlled by the body's immune system. With
rheumatoid arthritis, the body actually attacks its own joints,” said Doug Cusick, MD, Plastic Surgeon at AdventHealth Shawnee Mission. Cusick has performed thousands of hand surgeries over the past 27 years. If the pain is keeping you from doing your everyday activities or if you’re experiencing ongoing tingling, numbness or weakness in the hands or fingers, it’s important to get help early. The AdventHealth Shawnee Mission Hand Specialty Center is the only facility in the greater Kansas City area to exclusively offer a full spectrum of services for the delicate treatment of hands and wrists. “We’re able to see patients within a week when they call for an appointment – so they can receive a prompt diagnosis, followed by treatment,” said Cusick. Therapy can often reverse painful hand, wrist and arm conditions, especially when addressed early. “Occupational therapists often treat patients with these conditions before resorting to surgery. We help them improve range of motion to increase the ability to functionally use the hand and fingers, and strengthen the supporting muscles to offload the large forces placed on these joints,” said Pascuzzi. When these problems become so irritable they are beyond chronic, surgical correction may be required. “Typically, a hand surgery patient should expect a recovery period of three-to-four weeks, depending on the complexity of the outpatient surgery. After the procedure, an occupational therapist can
help make sure the joints stay soft and supple, so the patient can resume normal activities as soon as possible,” said Cusick. What About Trigger Finger and Trigger Thumb? In addition to arthritis and carpal tunnel (which occurs when one of the major nerves to the hand is squeezed or compressed as it travels through the wrist), gardening and similar activities can cause conditions known as “trigger finger” and “trigger thumb” causing your finger or thumb to get stuck or locked in a bent position. Fortunately, diagnosis is usually easy because of an identifiable bump on the palm of your hand at the base of the affected digit. The lump will likely move as you bend your finger or thumb. There are a variety of treatments that can provide long-term relief. For mild cases, antiinflammatory medication can be very effective. But for severe cases, a surgical procedure known as “trigger finger release” may be called for. “With trigger finger, there’s a nodule that gets caught on tendons. The condition can be resolved surgically in an outpatient procedure that simply releases the nodule and doesn’t require cutting or rebuilding any tendons,” said Cusick. If wrist, hand or finger pain is ongoing, call 913-676-8626 to schedule an appointment with our Hand Specialty Center at AdventHealth Shawnee Mission.
Tips to help gardeners avoid wrist, hand and finger injuries: 3 Schedule regular breaks for yourself, especially if you’re performing repetitive motions. 3 Regularly stretch to prevent pain and injury. 3 Perform self-massage on sore forearms, palms and thumb. 3 Wear proper gloves that have good grip and don’t impede your dexterity. 3 Use ergonomic tools that reduce stress on muscles and joints.
From Katie Pascuzzi, OTD, OTR/L, PT Solutions Katie Pascuzzi, OTD, OTR/L, earned her Doctorate of Occupational Therapy from Creighton University in Omaha, NE. She is a Kansas City native and joined PT Solutions in July 2019. Pascuzzi is currently. studying and working towards becoming a Certified Hand Therapist (CHT) and enjoys working with a wide range of patients.
From Doug Cusick, MD, FACS, Board Certified Plastic Surgeon, AdventHealth Shawnee Mission Doug Cusick, MD, FACS, earned his medical degree from Oral Roberts University, completed his General Surgery residency at Loma Linda University Medical Center, followed by an additional Plastic Surgery residency at the University of Kansas Medical Center. Dr. Cusick is board certified by the American Board of Plastic Surgery and is a Fellow of the American College of Surgeons. COPD + COVID-19 = An Additional Challenge Living with COPD can be tough. COVID-19 could make it even tougher.
If you already have COPD, talk to your doctor if you notice any of the following: 3 Worsening breathing problems 3 More coughing 3 New types of coughing 3 Changes in phlegm color or amount 3 More wheezing 3 Lower blood oxygen levels at rest 3 Increased oxygen use 3 More use of rescue inhaler

reathing well – with little effort – is something most of us take for granted. But the American Lung Association estimates that as many as 24 million Americans may be living with chronic obstructive pulmonary disease, commonly known as COPD, a lung disease that gets progressively worse over time. Early symptoms can be shortness of breath and getting tired easily. Later, a cough could develop, and may produce mucus, phlegm or spots of blood. Fatigue and tightness in the chest can become a problem, and physical exertion like climbing a flight of stairs can leave you wheezing or gasping for air. While there are times when COPD limits your ability
to do even simple activities like walking or cooking, the good news is you’re not powerless against COPD, even though there is currently no cure for this disease. In fact, early diagnosis and treatment can often successfully manage COPD symptoms, making it easier to breathe and feel better. In addition, avoiding triggers and staying as healthy as possible go a long way in treating COPD. But now, with the threat of COVID-19, the situation for those with COPD is more serious than ever. “The first step should be to obtain free testing for COVID-19 or contact your physician’s office if you have concerns or difficulties. If you feel like your symptoms are getting worse to the point where you don’t feel you can care for yourself at home, you should seek emergency medical care,” said Derek Low, MD, Pulmonologist, AdventHealth Shawnee Mission. The simple fact is that having COPD makes you more vulnerable to the effects of COVID-19. “Anyone with lung diseases is at higher risk of respiratory infections. And if you have COPD, you have a higher risk of more severe infections,” said Adam Rivers, MHA, EP, Cardiopulmonary Rehab & Community Education Manager, Cardiovascular Services, AdventHealth Shawnee Mission. As nearly everyone knows, the effect of COVID-19 on breathing can be powerful and devastating. “COVID-19 causes inflammation and fluid buildup in your lungs which can result in shortness of breath, cough, fevers, and low oxygen levels. Those with

From Adam Rivers, MHA, EP, Cardiopulmonary Rehab & Community Education Manager, Cardiovascular Services, AdventHealth Shawnee Mission Adam Rivers, MHA, EP, earned his Bachelor of Science degree in Exercise Physiology from Slippery Rock University, PA, and his Masters of Healthcare Administration and Management from Point Park University, PA. He has nearly a decade of experience in Pulmonary Rehabilitation.
From Derek Low, MD, Board-Certified Pulmonologist, Pulmonology, Critical Care and Sleep Services Derek Low, MD, is a Board-certified pulmonologist and critical care physician with AdventHealth Medical Group Pulmonology at Shawnee Mission. Dr. Low earned his medical degree from the University of Kansas School of Medicine and completed his internal medicine residency at the Medical University of South Carolina, where he was named Resident of the Year. He then continued his specialty training in Denver with a pulmonary and critical care medicine fellowship at the University of Colorado and National Jewish Health. He joined Pulmonology at Shawnee Mission in 2021.
COPD may already have limited ventilation due to their chronic condition and could be more prone to developing these symptoms or a worsening of their baseline COPD symptoms,” said Low. Fortunately, there are several simple steps recommended by medical professionals to help COPD sufferers protect themselves from COVID-19. “Anyone who is able should be vaccinated against COVID-19. The currently available COVID-19 vaccines have been shown to be extremely safe and effective. If you are not able to be vaccinated for COVID-19, we recommend wearing a mask or face covering in all public areas,” said Low. Overall, the more you know about COPD – and COVID-19 – the more opportunity there is to avoid getting the coronavirus and maintain your quality of life. “In Pulmonary Rehab, we teach our members with COPD ways they can leave the house, instead of staying in isolation. Wearing a mask when not able to socially distance is a great way to lower risk of exposure while still getting out into the community,” said Rivers. Managing COPD symptoms take a lot of effort and can be stressful at times, but avoiding triggers and staying as healthy as possible will go a long way in treating this progressive lung condition. Ask-A-Doctor
Mark A. Wiles, MD,
Hospice and Palliative Medicine, on the Value of Hospice and Palliative Care
What are the basic differences between hospice care and palliative care? Of course, both palliative care and hospice care provide comfort. Hospice care is provided by a specialized team and designed to help support patients with a terminal disease and a limited prognosis of six months or less. Palliative care, on the other hand, is patientand family-centered care designed to help optimize the quality of life for patients struggling with critical or chronic illness. It can include specialized symptom management like customized pain strategy.
Are there different types of palliative care?
Who can benefit from palliative care?
Because palliative care addresses the needs of patients and families over time and across different stages of illness, it can be offered in many settings including the hospital, outpatient clinics, home-based teams and even within specialized facilities like nursing homes.
Anyone struggling with a lifelimiting illness, including conditions such as chronic lung or heart disease, or irreversible neurologic conditions such as MS and ALS.
When is it time for hospice? It is time when terminally-ill patients – those with a limited prognosis determined by a doctor – are ready to shift their goals from disease-directed care to comfort-oriented care.
Can palliative care help cancer patients? Absolutely. For patients who require aggressive symptom management, having access to palliative care can help maintain their health status so they can continue receiving the cancer treatment they desire for a longer period.

The Facts
These Days, Journaling Is the Write Thing to Do Thanks to decades of TV shows and movies, when we think about journaling, many of us picture a teenage girl, laying on her bed and writing about her hopes and dreams in a diary. But the truth is, journaling is for anyone who can write. In fact, journaling does more than just help you record your memories or find self-expression. It’s good for your health. Here are four important virtues of journaling:
1
2
3
4
Reduces stress by allowing you to work through anxious feelings or worries. Keeps memory sharp by helping keep your brain in tip-top shape and increasing working memory capacity. Boosts mood by giving you a greater sense of overall emotional well-being. Strengthens emotional functions by improving mindfulness and providing a greater sense of confidence and self-identity.
Ready to Start Journaling?
No matter which type of journal you keep, remember – there’s no right or wrong approach. Just choose the type of journal that feels right for you:
Stream of consciousness: Simply capturing your thoughts in action.
Food journal: So you’ll be more mindful about the foods you choose to eat.
Dream journal: A great way of getting in touch with your subconscious.
Fitness journal: Keep track of your workouts so you can stay committed to an active lifestyle.

Day’s events journal: Keep track of your daily experiences.
