Yoga is Medicine
For more than 5,000 years, yoga has been practiced as a system of healing for both the body and mind. Today, yoga is being employed to help people with virtually any health condition, physical or psychological, using a wide array of tools such as poses, breathing techniques, relaxation exercises and meditation. And right now, with the pandemic still affecting so many people, yoga is becoming a powerful tool to deal with the lockdown’s uncertainty and isolation, as well as to maintain physical well-being. Of course, it may seem a little strange at first, to be alone in front of your computer in yoga clothes, taking yoga classes from a distance. But more and more people are taking to their mats during lockdown to calm their minds and lift their spirits. In fact, for some people, an online class is an easier first step on the yoga path than entering a real studio.
Make Plans Now To Take Our Virtual Yoga is Medicine Class This virtual monthly yoga class will highlight a different health issue every month. This informational yoga will guide you through specific movements and poses designed to improve your symptoms. Class includes a take home guide to help you start a daily yoga practice for long-term relief. Autumn Butternut Squash and Arugula Salad Ingredients: Salad 1 small butternut squash, peeled, seeded and cubed 2 tablespoons extra virgin olive oil ½ teaspoon ground cinnamon ½ teaspoons ground cumin 8 cups loose-packed arugula (or other salad greens) 2 ounces crumbled goat cheese ¼ cup dried cranberries ¼ cup toasted pistachios Salt and pepper to taste Ingredients: Apple Cider Vinaigrette (makes approximately ¾ cup) ¼ cup raw apple cider vinegar 2 tablespoons fresh lemon juice 2 tablespoons raw honey (add more to taste) 2 teaspoons Dijon mustard ½ cup extra-virgin olive oil ¼ teaspoon ground cinnamon ½ teaspoon sea salt ¼ teaspoon black pepper
Directions: 1 Preheat oven to 425°F. Line a large baking sheet with parchment paper. Place the cubed butternut squash on the sheet pan. Spread out evenly on the sheet pan and roast for 35 minutes or until fork-tender and browned around the edges.
2 Make the Apple Cider Vinaigrette by placing the
ingredients in a small bowl in the order listed and whisk olive oil in slowly until well combined. Season with cinnamon, salt and pepper. Set aside.
3 Remove the butternut squash from the oven and allow to cool.

Pelvic Fractures Can Be a Real Pain 2

Christopher Wise, MD, Orthopedic Surgery, AdventHealth Shawnee Mission Christopher Wise, MD, earned his medical degree from Saint Louis University and then completed an orthopedic surgery residency at Michigan State University. He then went on to obtain further sub-specialty expertise in orthopedic trauma at Carolinas Medical Center in Charlotte, North Carolina, and is a fellowship-trained orthopaedic trauma surgeon with special interest in complex fractures of the pelvis, acetabulum and lower extremity. Dr. Wise also has expertise in fractures that have failed to heal, have become infected or have healed with deformity.
6 | AdventHealth | FALL 2020
Anatomy of the Pelvis The pelvis helps anchor the muscles and protect the organs in the lower abdomen. It also serves as an anchor for the muscles of the hip, thigh and abdomen, and consists of a ring of bones located at the lower end of the trunk—between the spine and the legs. The pelvic bones include the: 1 Sacrum (the large triangular bone at the base of the spine) 2 Coccyx (tailbone - at base of sacrum, hidden in illustration) 3 Coxae (hip bones)
T
hanks to technology, there are more ways than ever to get the vital health information we all need. And one of the areas worth exploring is the topic of pelvic fractures. As we age, the chances of pelvic fractures and hip fractures increase significantly, especially for anyone with bone density challenges, otherwise known as osteoporosis. Pelvic fractures are much less common than broken hips, accounting for only about three percent of adult fractures. But they can still cause a great deal of pain and discomfort, requiring medical attention, and possibly surgery, depending on what type of pelvic fracture it is. In general, pelvic fractures are classified either as lower energy or higher energy. “The vast majority of lower energy pelvic fractures are caused by simple falls from a standing height or on stairs. This is usually in an older person with poor bone density. The higher energy, more severe ones are from motor vehicle crashes or falls from a height or being struck by a car as a pedestrian,” said Christopher Wise, MD, Orthopedic Surgery, AdventHealth Shawnee Mission. The seriousness of a pelvic fracture depends on how many bones are broken and how badly, and on what injuries may have occurred to the organs inside the pelvis. In fact, pelvic fractures can range enormously in severity, from fairly minor to life-threatening.
How pelvic fractures are diagnosed A fractured pelvis is almost always painful. This pain is aggravated by moving the hip or attempting to walk. Often, the patient will try to keep his or her hip or knee bent in a specific position to avoid aggravating the pain. Some patients may experience swelling or bruising in the hip area. “Diagnosis usually stems from the history of the recent injury and an X-ray of the pelvis or a CT scan,” said Wise. While X-rays will show most pelvic bone injuries, a computerized tomography (CT) scan builds up a highly detailed, three-dimensional image of the injuries. A magnetic resonance (MRI) scan is an alternative to CT and may give an even clearer picture of damage. Fortunately, many such cases actually are minor. Typically, treatment for a lower energy pelvic fracture does not require surgery, especially when it’s a stable fracture – meaning the broken bone or bones are still properly lined up. “Most lower energy pelvis fractures are able to heal without surgery. A small portion of the low energy type do not heal and become painful insufficiency fractures that are treatable with a simple surgery in which screws are placed across the fracture using X-ray guidance through a small one-to-two inch incision. Many times, this can be done as an outpatient or a single overnight stay,” said Wise.
Treatment for the majority of lower energy fractures is weight bearing (standing and/ or walking) as tolerated and regular X-rays over a period of 10-to-12 weeks to make sure it heals properly. Crutches and walking aids are likely to be used as part of the recovery, and physiotherapy is often an essential part of treatment. Interestingly, you may not even realize you have sustained a pelvic fracture. “I frequently see people who have gone to see a spine doctor for ”back pain“ only to find out that they actually have a sacral fracture (which is part of the pelvis) that never healed,” said Wise. On the other hand, higher energy pelvic fractures (the kind you get from a car/ motorcycle wreck or falling over 10 feet) are a whole, different story. “They often have longterm issues with pain and dysfunction directly related to how severe the original injury was. Thankfully, these are far less common than the lower energy type,” said Wise. Your pain condition or injury should first be brought to the attention of your primary care doctor, who can determine if you should be referred to a specialist.

Mobility Is Vital for Healthy Hips 2020 marks 60 years since Chubby Checker first performed his hip-swiveling classic “The Twist” on “American Bandstand.” And during that time, we’ve learned a lot about why strong, flexible hips are crucial to good overall health. After all, your hips are the center of movement for your body. As a major weight-bearing joint in the body, the hip is responsible for many functional activities such as walking and running, sitting and standing, and climbing stairs. By keeping your hips strong and mobile, hip pain can quickly be eliminated and you can return to normal activity.
8 | AdventHealth | FALL 2020
In short, the healthier and less restricted your hips are, the more potential your body has for strength, power and overall wellness. On the other hand, poor hip mobility can contribute to issues like lower back pain, knee issues and other health problems. “The hip joint has multiple different ligaments and muscles attached – making it one of the most stable joints in the body. The hip joint impacts the entire body's kinetic chain, meaning all other joints of the body can be impacted with decreased stability or mobility of the hip,” said Hannah Arellanes, Physical Therapist, AdventHealth Shawnee Mission. For example, sitting too much
without proper attention to posture is a great way to weaken your glutes and shorten the hip flexors. Range of motion still needs to be accounted for and the lower back often picks up the burden. The lower back is a region meant for stability and thus, when it is called upon to initiate motions such as lifting, you have an injury waiting to happen. “Pain can be a cause of poor mobility. If a joint is painful most people want to favor other parts of the body. Not using the muscles and joint can cause it to stiffen and decrease the mobility. Muscles and joints also need to be stretched and moved to maintain their mobility. If someone doesn't regularly stretch

their joint that can cause it to decrease in mobility,” said Arellanes. The Older You Get, the More Important Hip Mobility Becomes Hip mobility is very important as we age. It can impact everything from getting dressed to your posture to the way you walk. That’s why it’s so important to learn stretching and modified stretching techniques to maintain mobility with aging. “In older adults, poor hip mobility can cause functional deficits. Someone may have difficultly putting on socks if they can't bend over or bring up their foot. It can also impact balance and walking patterns. And having decreased hip
mobility can increase your risk of falling,” said Arellanes. In addition, limited hip mobility is associated with hip or knee osteoarthritis and spinal stenosis for older adults.
Fortunately, manual therapy performed by a trained physical therapist can address these joint restrictions by providing the soft tissue work necessary to release muscles that have gotten used to chronic tension. “Manual therapy can include joint mobilizations, passive and active stretching and massage. A physical therapist is trained in manual therapy and is a great resource to improve hip mobility,” said Arellanes. And to help make sure these changes in hip mobility really last, manual therapy should be combined with exercising on your own (self-mobilization). “A physical therapist is a great resource to improve hip mobility, and can also teach selfmobilization exercises including stretching,” said Arellanes.
Treatment for Greater Hip Mobility It starts with an understanding of how the hip is designed. It’s a large ball-and-socket joint similar to the shoulder. But with the hip, the socket is much larger than the ball – which means there is less mobility for the hip compared to the shoulder’s range of motion. It’s also common for the hip to have added movement restrictions due to muscle or joint capsule tightness.
Hannah Arellanas, Physical Therapist, AdventHealth Shawnee Mission Hannah Arellanes is a physical therapist practicing at AdventHealth Shawnee Mission. Arellanes graduated from University of St. Mary in 2018 and has been practicing in the acute care setting since. She enjoys working with the geriatric population and has special interest in orthopedic, neurologic and vestibular rehabilitation.

ASK-A-DOCTOR
Jonathan Ferns, MD,
General Anesthesiology, Pain Medicine on radiofrequency ablation In simple terms, what is radiofrequency ablation? Radiofrequency ablation is a procedure that uses a finely-targeted probe to ablate or destroy nerves that are originating from painful areas throughout the body.
Which conditions are treated with radiofrequency ablation? We currently use radiofrequency ablation to treat certain types of pain originating from the cervical spine, lumbar spine and knee joints.
What happens during radiofrequency ablation? While the patient is under conscious sedation, we use X-ray guidance to target painful sensory nerves in the affected area. Once proper positioning of the probe has been established, we numb the nerves with a local anesthetic and heat the probe to ablate the affected sensory nerve. In the weeks following the 10 | AdventHealth | FALL 2020
procedure, the sensory nerve loses the ability to transmit pain signals to the brain, and therefore greatly reduces pain perception from the previously painful area.
How long does pain relief from radiofrequency ablation last? Typically, pain relief lasts about 12 months. Pain can recur due to the fact that the sensory nerves will regenerate over time. We certainly have patients that get more sustained relief or their pain does not return at all. In those that have a pain recurrence, we will typically perform another treatment in the same affected area. Many times after subsequent ablations, the nerves will have more difficulty regenerating and the pain will not return.
Is radiofrequency ablation safe? Yes, radiofrequency ablation is considered a safe and low-risk procedure.
Is there anyone who should not get radiofrequency ablation? Radiofrequency ablation is a great tool in providing minimally invasive pain relief. Unfortunately, it is not indicated for every painful area in the body. It is best to be evaluated by a pain management physician to see if your condition warrants this treatment.

Connector My role is then to connect patients with the appropriate providers at the right time along their journey, be a continual pillar of support for patients, and communicate with their care team.
Consultant My regular check ins, desire to know their progress and communication among their care team make it easier to develop a relationship and truly provide the support they need.
