Water Aerobics: When It Comes to Easing Arthritis Pain, Wetter Is Better For people with arthritis, water exercise can be a safe, effective and fun exercise option. In fact, people with chronic joint conditions often say they feel free from their condition when in the water. And one study published in the American Journal of Physical Medicine and Rehabilitation found women with rheumatoid arthritis (RA) who did water-based exercises three times a week for sixteen weeks saw significant improvements in disease activity, pain, and functional capacity compared to those who did land-based exercises. Think of the pool as “nature’s body weight support system.” The water can unload up to almost 90 percent of your body weight, according to experts at the American Physical Therapy Association. So it’s no wonder water workouts are good for your joints. Moving in the water is much easier on the joints, allowing you to perform movement with an arthritic joint that would be painful on land – which then helps improve strength and flexibility in that joint. Water exercise also helps build muscle and also burns more calories to help you lose weight faster. Before you try any water exercise program, talk to your doctor or physical therapist to make sure pool exercises are right for you.

NUTRITION

The Fight Against Inflammation Starts in the Kitchen Chronic inflammation in the body can cause serious, long-term health problems, such as heart disease, stroke and arthritis, and can make it difficult to lose weight. And the biggest reason many people suffer from inflammation is due to their diets. While there is no single anti-inflammatory diet, but a diet that includes plenty of fresh fruits and vegetables, whole grains, and healthful fats may help manage inflammation. Best of all, an antiinflammatory diet can be just as tasty as it is healthy!

A

s long as everything is going all right, many of us don’t give much thought to our overall health. Feeling good is usually good enough. When we get sick though, we have to figure out the best way to react – whether it’s just taking it easy, seeing a doctor or even going to the ER for a medical emergency. But at AdventHealth, we’re here for our communities in sickness and in wellness. We want everyone to understand there are many things you can do to prevent sickness and even to improve your health, no matter what your current health status may be. In other words, you can not only react to sickness. You can be proactive and take steps to prevent some illnesses and improve your overall health. “Our programs and services are designed for our patients and

6 | AdventHealth | SPRING 2021

community members to have the knowledge and resources to make healthy lifestyle choices and live life to their fullest potential. At AdventHealth, we are committed to providing wellness support at all stages of patient care: including inpatient, outpatient, home health and community settings,” said Amanda Nunamaker, BHS, RRT, Community Wellness, AdventHealth Shawnee Mission.

The facts are clear. Four out of the five leading causes of death are caused by chronic diseases that are either preventable or likely to be manageable: 3 Heart disease 3 Cancer 3 Chronic lower respiratory disease 3 Stroke. When patients regularly access preventative care, their chronic

conditions are more likely to be discovered and managed. This lowers the likelihood both of emergency room visits and more expensive treatments for diseases that have progressed past regular management.

Whole-person health care offers a whole different perspective. As a leader in whole-person health care – which means treating the entire person – AdventHealth is focused not only with treating the body but offering care and resources to address the mental and spiritual side of healing, such as education on mindfulness, improved nutrition and mental health and spiritual resources. Of course, all age groups should receive regular checkups with their primary care physician (PCP). These appointments help your doctor identify potential problems

Wellness activities can improve your health regardless of your current health status. as well as determine if family history or lifestyle choices could affect your health in the future. “We understand that addressing all sides offers the greatest opportunity for treating the whole person. A good example of whole-person treatment includes asking our primary care patients to complete a fourquestion assessment that would be followed-up by optional free spiritual care,” said Nunamaker.

As you’re getting older, you can also get healthier. Of course, wellness doesn’t become less important as we age. In fact, it’s just the opposite. Investing in the health of your body at age 50 or 60 can equal big payoffs later in life. So it’s no wonder an increasing number of mature adults want lifestyle solutions that address their health concerns. “Our Community Wellness and

Behavioral Health Departments offer wellness and therapeutic fitness classes, massage therapy and outpatient mental health resources and support groups to address the self-care and mental side of healing and recovery,” said Nunamaker. And partnering with community organizations, like Merriam Community Center, allows AdventHealth expertise to reach even more people. “Warm Water Arthritis ClassPowered by AdventHealth is offered in Merriam Community Center’s beautiful aquatic facilities. Partnering with a facility that has a pool allows us to further our reach to populations that can benefit from our therapeutic expertise,” said Nunamaker. To learn more about the wellness programs we offer that are designed to strengthen you mind, body and spirit, visit AdventHealthKC.com/Wellness.

AdventHealth offers fitness programming specifically for patients with significant health challenges. The AdventHealth Move 4 Life program is a monitored fitness membership for chronic health patients. We have found that members of this program with good attendance stay out of the hospital and are better equipped to age in place. We will meet you where you are and help you to meet your health goals.

From Amanda Nunamaker, Fitness Program Supervisor, AdventHealth Shawnee Mission As Fitness Program Supervisor, Amanda and her colleagues design workout programs for patients and community members and works with them to achieve their health goals. Nunamaker earned her Bachelor of Health Science – Respiratory Therapy degree from the University of Missouri – Columbia, and has worked at AdventHealth Shawnee Mission since 2015.

Home Is Where the Healing Is For a growing number of joint replacement patients, outpatient surgery is now “in.”

8 | AdventHealth | SPRING 2021

M

ake no mistake. Getting a hip or knee replacement is a serious health event, whether it’s because of advanced arthritis or the result of an injury or other condition. When less invasive therapies don’t relieve the pain or dysfunction, a joint replacement can improve mobility and provide welcome pain relief by removing damaged or diseased parts of a joint and replacing them with new, man-made parts. No wonder there are now more than one million joint replacement surgeries performed in the U.S. each year – with nearly four million each year expected by 2030. But these days, there’s been another important breakthrough. Thanks to recent advances in medical technology, technique and pain management, it’s not only possible for some patients to have knee or hip replacement surgery on an outpatient basis – it’s preferable. “For most patients, being at home, sleeping in their own bed, not being woken up on strange schedules for vital signs, etc. allows patients to focus on their healing while motivating them to be up and moving around an environment they are comfortable in and familiar with,” said Amy Williams, RN, AdventHealth Shawnee Mission Administrative Director of Surgical Services. After all, who wouldn’t want to spend their recovery in the comfort of their own home? But there are actually many other benefits to people who qualify for this outpatient procedure, including a quicker recovery time.

“Outpatient total joint replacement surgery is the first step in a protocol called ‘rapid recovery joint replacement’ that helps patients get back to normal in a shorter period,” said Christopher Eckland, DO, Orthopedic Surgery Specialist at AdventHealth Orthopedic and Spine Care. There’s also an added measure of patient safety. “Anytime we can provide surgical care in the setting that has little to no biologic burden, it is beneficial to the patient,” said Eckland. Of course, not all patients

Qualities of a good candidate for outpatient joint replacement surgery 3 Motivated: A positive attitude and willingness to follow your doctor’s post-surgery instructions. 3 Mobile: If you’re active and independent before surgery, you’ll respond better to therapy and resume your daily activities more easily. 3 Healthy: The fewer ailments you have and medications you take, the better outpatient candidate you’ll be. 3 Supported: Having a group of family members and/or friends who can help you after surgery will make your recovery faster and better.

qualify for outpatient surgery. “To be a good candidate for an outpatient total joint procedure, a candidate must have well controlled medical conditions, be motivated for rapid recovery and maintain a relatively healthy weight. Patients with more complex medical problems, significant obesity and physically weaker patients are likely best suited for a traditional inpatient model,” said Eckland. Many consider the outpatient option from an emotional and social standpoint. “Those considering outpatient joint replacement surgery need to have a strong and consistent support person or team with them at home who will encourage them to be moving per surgeon instruction despite some potential discomfort,” said Williams.

The more prepared you are, the better. Whether it’s outpatient or not, the biggest concern is simply fear of the unknown. After all, most people have never been through a joint replacement procedure. “However, outpatient joint replacement candidates have another level of unknown, since they are relying primarily on themselves and their personal support team to get them through the immediate postoperative period – when nursing has traditionally filled that role for patients,” said Williams. The fact is, all total joint replacement patients and their families need to be prepared for the discomfort that can come in the days immediately following surgery, while still committing to being up and around and working

on their appropriate therapy. “This is one reason why it is critical leading up to the surgery that the patient attends the Joint Replacement Class,” said Eckland. Just as rehabilitation helps you recover after surgery, this exercise and therapy class helps patients prepare before surgery, to help ensure a speedy recovery and an overall successful outcome. In other words, pre-hab is just as important as rehab. For more information about Outpatient Joint Replacement, contact Orthopedic Nurse Navigator Amber Keller, BSN, RN, ONC, at 913-632-2574 or KCjoint@adventhealth.com.

From Christopher Eckland, DO, AdventHealth Shawnee Mission Dr. Eckland focuses on open and arthroscopic treatment of shoulder and knee, performing numerous knee and shoulder replacements, including unicompartmental knee replacements. He maintains multiple academic memberships in orthopedic societies. Dr. Eckland earned his DO from the Kansas City University of Medicine and Biosciences, and completed his residency in orthopedic surgery through Michigan State University at Botsford Hospital.

From Amy Williams, RN, MSN, MBA, CNOR, NE-BC, AdventHealth Shawnee Mission As the Administrative Director of Surgical Services at AdventHealth Shawnee Mission, Williams collaborates with Senior Leadership to ensure Surgical Services supports the mission, vision and values of the hospital and shows fiscal responsibility while ensuring superior patient care. Williams earned RN degree from the University of Missouri – Kansas City, along with her Master of Science in Nursing (MSN) degree and an MBA from the University of Mary.

ASK-A-DOCTOR

Darren Lovick, MD,

Neurosurgeon, AdventHealth Shawnee Mission Neurosurgery, on arthritis affecting the spine

What is the most common spine condition affecting adults age 50 and over? Arthritis is the source of most back pain in adults over 50. Each of the vertebrae are connected by a disc or pad in front and two small joints in the back. These small joints, very similar to the knuckles of your hands, can become worn or arthritic. If these joints become large enough, they can encroach upon the nerves in the spinal column and lead to pain down the legs.

How do aging and weight affect spine health? Studies have shown a correlation of obesity and low back pain, although we see people in all weight ranges with low back pain. However, as someone becomes less active, they lose strength in 10 | AdventHealth | SPRING 2021

their core musculature, and the spine has less support, which can lead to more low back pain.

Are there lifestyle changes that can improve spine health? Lifestyle habits are the same for those taking care of their heart or joints. Moderate exercise thirty minutes a day for five days a week. Core strengthening exercises and light weight training can improve stability of the spine. Setting up a comfortable work environment can also be helpful.

Can Physical Therapy (PT) be used to avoid spine surgery? The evaluation of a physical therapist is very valuable and includes evaluation of posture and strengthening. PT is the mainstay of treatment for low back pain.

If spine surgery is ever needed, what should a patient know before the procedure? We explain the anatomy and what has happened in their spine causing the symptoms. We then explain the procedure, risks, benefits and expected outcomes. It’s always best to try and do “the least amount of surgery for the most amount of benefit.”

Your pain condition or injury should first be brought to the attention of your primary care doctor, who can determine if you should be referred to a surgeon. For more information, visit AdventHealthKC.com, or to match with the right primary care physician for you, visit MyHealthKC.com.

An Anti-Inflammation Diet Is Pro-Healthy Foods

Broccoli An excellent source of fiber and is considered an ‘anti-aging’ food due to its high vitamin C, betacarotene, vitamin B and vitamin E content.

Science still has a long way to go to fully understand every aspect of how different foods affect our health. But we do know that maintaining a healthy weight through a healthy diet and regular exercise is probably the best way to battle unhealthy inflammation. Although there is no magic anti-inflammation diet, putting the right combination of foods into your meal plan can produce remarkable results.
