Great Expectations Labor and delivery in South Florida NICU View at Memorial West in Pembroke Pines
BY JENNIFER JHON Advances in technology and a return to natural-birth methods have changed the way newborns are delivered and cared for in South Florida. Hospitals have made earnest efforts to reduce C-section rates and refuse to deliver before 39 weeks, except in cases of medical necessity. Many hospitals also have initiatives supporting the importance of family bonding at birth.
Jackson Memorial The Women’s Hospital at Jackson Memorial in Miami puts an emphasis on family togetherness during and after the birth, CEO Geralyn Lunsford said, “so when you have a baby now, it is a wonderful family-centered approach to maternity care.” That means family members – not just dads – are welcome in the labor room. “We are all about honoring the mother’s and father’s choice with birthing experiences,” said Jane McCarthy, director for patient care services. “If mom wants her whole family with her to welcome this baby into the world, as long as we’re under a controlled setting and baby is stable, they’re more than welcome to join in the birthing experience.”
McCarthy said babies are put on mom’s chest the moment they are born at Jackson, whether via C-section or vaginal birth, to cuddle and to nurse for at least 60 minutes. “That helps with thermal regulation for the baby, and with mom and dad bonding with the baby,” she said. Once the family is ready to move, they go downstairs together to the newly renovated Little Miracles maternity suites, with private rooms and a relaxation lounge stocked with coffee, tea, pastries and fruit. “We don’t ever separate for a minute as long as the baby is doing fine, the mom is doing fine,” Lunsford said. “We keep them all together.” Babies aren’t taken to a nursery. “We don’t have a nursery,” she said. “We keep the babies with the parents the entire time, and it’s quite an enjoyable experience.”
South Miami Hospital Baptist Health’s South Miami Hospital Center for Women & Infants also emphasizes keeping the family together, said the center’s chair, Dr. Jorge Perez. Moms and babies room together, and the newborn bath is delayed for up to 24 hours to encourage bonding time. Spouses and families are invited into
South Miami’s suites, he said, which have beds for mom and baby and couches for dads or other family members to spend the night. The arrangement, he said, reduces the stress and anxiety of having a child. “Even in jaundice, the baby stays with the mom,” he said. South Miami Hospital has walking epidurals for moms to encourage vaginal births, Perez said, and the hospital does not allow elective C-sections. To make the birthing process easier on babies during their first hour, which Perez called the “golden hour,” South Miami’s labor and delivery rooms are warmed up to body temperature to ease baby’s transition to life outside the womb. The doctors wear airconditioned gowns to stay comfortable in the heat. They also practice delayed cord clamping, waiting 30 seconds to 1 minute after birth to clamp the umbilical cord, to keep blood flowing from the mom to the baby. South Miami also has a free umbilical blood cord donation program and a screening policy to detect postpartum depression in new moms before they go home, as well as breastfeeding and lactation support for its moms 24 hours a day and support groups for moms for a year after birth. 2017 ~ 2018 • BABY & MATERNITY GUIDE

Memorial Healthcare System Hypnobirth and NICU View – streaming video of babies in intensive care – are the big initiatives at Memorial Hospital in Broward County. The webcams, one for each baby in the Neonatal Intensive Care Unit, have been a big hit, said Gessy Target Johnson, director of nursing at Family Birthplace at Memorial Hospital. Families can log in with a password, and “they can literally watch their baby all day and keep track of their baby’s care,” she said. “So the moms love it.” NICU View is live at Memorial West in Pembroke Pines. “Memorial Regional [in Hollywood] is on their way. Memorial Miramar will be last,” by the end of the year, Johnson said. Dads are getting more time in the NICU at Memorial as well. “They’re allowing dad that ability to be at the bedside with the baby doing kangaroo time. … We’ll let dad take off his shirt and put the baby right on his chest; it’s a little different.” Memorial Hospital Miramar offers a hypnobirth class with a certified perinatal educator. Moms are taught how to hypnotize themselves and refocus their pain during labor. As a result, they’re staying home longer, Johnson said. “We just had a patient, she stayed home, [despite this being her] first baby, until she was 9 centimeters,” Johnson said. “Within an hour and a half, she had a baby. No meds, no epidural, and her comment was, ‘I wasn’t feeling the pain.’ ” Moms who have taken the hypnobirth class have options during labor. “They can walk around, they don’t have to be tied down to the fetal monitors during their entire labor,” Johnson said. “They’re allowed to take hot showers, they’re allowed to be on their birthing ball. “These moms are really prepared, so we’re comfortable having them do those things,” she said. The goal of the hypnobirth class is to decrease the C-section rate, Johnson said. “That’s a huge plus for obstetrics moving forward because now there’s that rule: Once you’re a C-section, you’re always a C-section. And the more C-sections you have, the higher the risks become for hemorrhage and other complications.” Memorial Miramar’s hypnobirth class is also a way to bring natural birth back into the hospital setting. “We want to support that home birth, and this is a good way of doing it,” Johnson said. “You’re allowed to labor at home, no medication, no Pitocin, we’re not rushing your delivery. … We’re giving your body time to flow with the birthing process.”

Natural birth is also easier on the fathers, Johnson said. “Sometimes when you’re watching your poor wife in labor and she’s screaming … that’s very dramatic for the fathers. Sometimes we lose a couple dads – they faint, they can’t take it. The dads, realizing that mom is in control [through hypnobirth] … they can be a lot more supportive.” After a baby is born at Memorial, the baby’s bath is delayed to increase bonding time with mom and encourage breastfeeding. The delay can also boost the baby’s immunity, thanks to the microorganisms captured in the waxy coating on a baby’s skin during the birth process. “We will wipe the baby down,” Johnson said. “But the benefits of having that flora on the baby is a huge plus. The research on that is saying baby’s immune system gets strong by having that flora. … So they’re leaving that on.” Memorial also has a system in place for moms who need more specialized care. Its High Risk Perinatal Navigation system identifies moms who are high risk and plans in detail for her care. The system means Memorial is prepared for moms with hypertension, cardiac conditions, moms with cancer, transplants, paraplegics, previous C-sections and other complications. “Every player that’s going to have a part in that delivery meets ahead of time,” Johnson said. “They plan the delivery, they have the surgeons in the room, everybody who needs to be there is already aware, and the patient’s aware.”
Broward Health The Maternity Place at Broward Health Medical Center in Fort Lauderdale seeks to put mom in control of her birth process, said Tonia Royster, the manager for labor and delivery. “We support birth plans 100 percent,” she said. “We encourage [moms] to speak up and speak out … so we understand their expectations when they come in. It is attached to their prenatal record, so when they come in, we have that information ahead of time.” Several programs help with that planning, including a concierge service in which a physician commits to the client, no matter when baby is born, Royster said. A concierge physician meets the mom, develops a birth plan to meet her needs and stays with her throughout labor and delivery. With the concierge service, moms have greater options in natural childbirth, such as water birth. “Water birth is very popular and common,” she said. A patient must have a physician who provides that service, Royster said. “They bring the device in, the inflatable tub. They will inflate it and add the water. They will deliver the baby in the water, and then they transition back into the bed.” Broward Health is also one of the few hospitals in South Florida to offer nitrous oxide. “It’s a 50 percent blend of oxygen and nitrous, and the moms are using it for pain management.” The gas is delivered though a device that mom holds in her hand, and she breathes

much their baby is eating. Moms do not have to deliver at the hospital to use the services, she said. The hospital has also implemented a postpartum support group. “It’s underdiagnosed, and it’s undertreated,” Yormark said. “We work with our department of psychology and offer it with topics. … And it’s free. There is another place offering postpartum support, but you have to pay.”

the mixture in until she gets the comfort she needs. The benefit of nitrous oxide is that it absorbs quickly, so it has no effect on the baby, she said. Moms at Broward Health usually start with nitrous oxide, and if they change their minds, they can go to an epidural, Royster said. “It gives moms something besides breathing exercises to manage their pain.” Broward Health also offers a Dancing with Birth class, which teaches moms rhythmic movement with music. “So instead of lying in bed, women can get up and usually use some type of a dance motion to help with the laboring of the baby,” Royster said. With the hospital’s wireless fetal monitors, mothers are able to get up, move around and get wet while being monitored, and the activity helps the labor to advance, she said. Family togetherness is also a priority at Broward Health, said Alana Yormark, the nurse manager of Maternity Place. A baby born there is quickly examined and then put back on the mom’s chest, she said. “If mom can’t do that … if the dad is there, we’ve done skin-to-skin with the dad. And that encourages bonding.” Broward Health moves baby and mom as a family unit to the new suites at Maternity Place, which include full-size beds so a family member can sleep with mom, Yormark said. Visitors are allowed to stay 24 hours a day in the center’s private rooms and upgraded suites. Broward Health also offers “Tea and Teach” time every evening for moms who are going to be discharged the next day, Yormark said. “Mom gets a warm beverage, and then the nurses take that time to do one-on-one teaching.” Broward Health has full-service lactation services and a breastfeeding support group, and it is open 24 hours for breast pump rentals. It also rents scales that moms can use pre- and post-feeding to check how
Boca Raton Regional In recent years, Boca Raton Regional Hospital has focused on trying to improve the mother-baby care, said Karen Edlington, director of women’s and children’s services. The hospital has “gentle C-sections” using clear drapes during the operation so moms can see their babies being born, and policies that keep babies with their moms as much as possible. Moms are encouraged to have their baby stay with them instead of in the nursery, and doing as much skin-to-skin contact as they can, even if they are not breastfeeding. Boca Regional also delays bathing of newborns until after 24 hours. “That’s all part of keeping mom and baby together, especially during those transitional hours,” Edlington said. “When you delay the bathing, you really get to do the actual bath much more as a family teaching moment than you do when it’s done in those really early hours after birth. … It’s much more meaningful, too.” Boca Regional has breastfeeding support seven days a week, including nighttime support; a lactation service that includes pump sales and rentals; and a free breastfeeding support group that meets twice a week. The hospital also supports natural birth and allows aromatherapy in every room. It does not allow water births, but it does have hydrotherapy, thanks to a partnership with The Orchid Nest, which rents tubs for laboring moms. Doulas are welcome and wireless monitors allow moms in labor to move around and get in the water. “Whatever kind of delivery or birth that you’re looking for, we can answer to that,” Edlington said. “We definitely support natural childbirth, the birthing balls, aromatherapy, hypnobirthing, all of that. We’re very used to supporting those types of parents.” Boca Regional also offers various classes, such as childbirth, prenatal baby care, prenatal breastfeeding and even a hypnotherapy course taught by The Orchid Nest. There is a breastfeeding group daily for moms on the maternity floor and a formal discharge class for every mom. The NICU at Boca Regional is a Level 2, but all nurses there are Level 3 trained,
and a neonatologist is in house 24 hours a day. Within the NICU, the hospital supplies donor milk for NICU babies, and the hospital is a milk depot, so registered donors can drop milk there for distribution to NICU babies throughout Florida.
St. Mary’s Personal attention and education are important at The Birthplace at St. Mary’s Medical Center in West Palm Beach, said Amy McGuire, director of women’s services. The Birthplace has an OB Navigator who helps expecting moms prepare for birth. “She meets with our mothers prior to delivery and gets personalized support, then she follows them during their stay here,” McGuire said. “She rounds on them every day, and then she also does a followup home visit.” The nurses at St. Mary’s also contribute. “All of our nurses are trained in getting the mom ready to know how to take care of herself and her baby when she goes home,” McGuire said. “And so each nurse will meet privately with their patient and go through and make that teaching session individualized for that patient, so really focusing on what she needs help with.” St. Mary’s also has a pharmacy liaison, who meets with moms before discharge about their medications, and a liaison who meets with each patient to determine if there are any community services they would benefit from, such as Healthy Mother, Healthy Babies, or WIC. “It’s like a one-stop shop,” she said. Moms at St. Mary’s are encouraged to have a birth plan, and wireless fetal monitors allow them freedom of movement during labor, McGuire said. Moms can get up and walk around in the hallways or get in a birthing tub. “You’re not attached to anything,” she said. “We also have birthing balls that patients can sit on, it kind of looks like a big exercise ball. And then they also have a peanut ball, which is something new. It is literally shaped liked a peanut, and if you lay on your side, you kind of put one leg on top of it and that helps with the rotation of the baby. “It’s just something that most women find very comfortable, and it will help their labor to progress.” St. Mary’s also encourages breastfeeding within the first hour following delivery and encourages skin-to-skin contact for at least three hours after birth, with the bath delayed until after that time. “If she doesn’t want to do skin-to-skin or she can’t, her partner can,” McGuire said. “Even our moms in recovery after a C-section can do skin-to-skin.”

Its hard to mess up BY DANIELLE BRAFF, CHICAGO TRIBUNE Children don’t come with an instruction manual. And as every new parent learns, it can be a terrifying job – with the feedback erring on the negative side: Your child will scream and cry for hours. Once he can talk, he’ll probably tell you how much he hates you. He may even tell you that he wants a new parent. We compiled advice from seasoned parents, ranging from must-have baby items to emotional support that’ll get you through those early years. Take time for yourself before the baby arrives. Make plans with friends, have a manicure, see a movie, said Lindsay Pinchuk, CEO and founder of Bump Club & Beyond, an organization connecting parents and parents-to-be with resources and products through events in more than 25 cities nationwide. The last three movies she saw in a theater were “Moana,” “Sing” and “Trolls.” So before your viewing experience transforms to G-rated only, take advantage of your adult time. “Having a baby is the best thing that will ever happen to you, but your life will change, and some of the things you are used to doing every day _ or every week, simply won’t happen anymore because you’re now responsible for this little being,” Pinchuk said. It’s hard to mess up. There are many shades of gray when it comes to things like how to get a baby to sleep, to eat and to develop in the best way possible, said Scott Goldstein, pediatrician with The Northwestern Children’s Practice, instructor at Northwestern University Feinberg School of Medicine and father of three. “This means that by doing what you feel comfortable with, you _ and your baby _ will almost always be just fine,” Goldstein said. “The issues your child will have when they are an adult will most likely not be traced back to anything you did in the first year.” Connect with your partner. Set aside time each week to connect with your partner, said Janeen Hayward, founder of Swellbeing, a parenting resource in Illinois that helps with everything from potty training to sleep issues. “This is time to talk about what you need, what’s working and what’s not working.” She suggested that couples set a weekly date, even if you don’t leave the house _ to stay connected during this transition and appreciate that you’re both going through a major identity shift. “And when you’re ready, it’s so nice to be able to
go out together, even if you talk about the baby the entire time,” she added. Let your child know you. Bring him into your world, and don’t be a mystery to him, said John Badalament, author of “The Modern Dad’s Dilemma” and director off programs at The Fatherhood Project, a program empowering fathers at Massachusetts General Hospital in Boston. “Tell them stories about yourself,” Badalament said. “Raise the verbal bar.” Don’t let guilt rule you. If you are like most people, you will second-guess your parenting decisions and feel bad about perceived mistakes, Goldstein said. “I often suggest to parents that they commit to memory a few examples of when they felt like really great parents and, when they are having tough moments, to recall those bright examples, so that they have the confidence to know that they do a good job.” A baby doesn’t need much. There are plenty of books, apps and products designed to help your baby, but your baby requires very little, Goldstein said. The Finnish government provides parents a box for newborns containing everything they need: a box doubling as a bassinet, a sleeping bag, outerwear, clothing, toiletries, bedding and diapers. “Some people find the experience of gearing up for a baby to be fun, but if you find it overwhelming, then skip it,” Goldstein said. “A place to sleep, clothes to wear and something to eat _ that’s about all your baby will need initially.” He added that you may want some diapers unless you want to redo your home. Get a white noise machine. Hayward said she recommends this item for all new parents. “Since babies are used to constant white noise in utero, this sound is very soothing to them and can help with sleep,” said Hayward, who recommends the Marpac Dohm ($49.95). Purchase diaper pails. Keep a diaper pail on every floor of the house if you have multiple levels. “No explanation necessary – just trust me,” Pinchuk said.
Snag a free breast pump. Insurance plans are required to cover the cost of a breast pump. Pinchuk recommends getting your pump through a durable medical equipment center. Often insurance companies will provide a preferred list. “At no charge, they facilitate the entire process between you and your insurance company to get you the best pump possible,” Pinchuk said. “The time that they save women is priceless.” Understand that nursing is hard. Yes, it’s supposed to be the most natural thing in the world, but getting the baby to breastfeed can be difficult and even impossible for some mothers. So all expectant moms should have the number of an international board-certified lactation consultant (iblce. org) on hand if they plan to nurse, Pinchuk said. Your best is good enough. “We can be very hard on ourselves and compare ourselves and our babies to others,” Pinchuk said. “Don’t forget to do your best, what works for you and your family, and to trust your gut.” Ask for help. We are wired to want to do it all and to think we can do it all, but you can’t do it all right now. When friends and family ask what they can do to help, make sure you have an answer ready, whether it’s picking up coffee, holding the baby so you can shower, or throwing in a load of laundry. If you say, “Nothing,” the help is going to go away, and you’re going to regret it, Pinchuk said. 2017 ~ 2018 • BABY & MATERNITY GUIDE
