instruction The purpose of this diary study is to understand your daily experiences with managing your current chronic heart condition and the difficulties you experience implementing and maintaining your treatment plan. This diary study will last 7 days. If you happen to forget or skip a day, please continue with the study. You may choose to not disclose any private information. You may skip any questions you are uncomfortable answerings and you may stop participating at any time during the diary study. We will anonymize any identifiable and private information. Information collected during this diary study will only be shared with members of our research team.
