Our Focus We Take Safety, Security, and Environmental Protection Seriously The safety and health of our associates, our customers, and those who visit our facility, in addition to protection of the environment, are fundamental values of Sysco Columbia, LLC. We strive to achieve and sustain a worksite that is free of accidents, injuries, environmental impacts, and security events. Once you step into our facility, you become a part of our Zero Incident safety culture. We expect that every individual will go home in the same condition that brought them to Sysco Columbia. We are dedicated to impeding unwanted events by preventing unsafe acts, eliminating or mitigating unsafe conditions, and responding properly to near misses and emergency situations. Rather than just reacting to occurrences and correcting contributing factors, our objective is to be proactive and prevent them from happening in the first place.

Our Roadmap to Zero Incidents To achieve our goal of daily excellence in the protection of our associates, our customers, our visitors, and the environment, we apply an aggressive approach to integrated safety and security management. To support this process, we ask that you understand the following: • Clearly understand the information presented to you in the Visitor Safety Orientation Pamphlet. • Practice and promote safe behaviors and proper work habits. • Recognize Hazards. • Use good judgment. • Comply with applicable regulatory standards and guidance defined by your host during your visit. • Provide notification of unsafe work areas and work processes to your host. You also have the right to know: • The potential hazards associated with the areas to which you are exposed (if applicable). • The measures to be taken to protect you from the dangers these hazards may impose. You play a meaningful role in contributing to our success.

Onsite Accountability Please ensure that you sign in and out on the Visitor Logs located in the lobby. Ensuring that everyone is accounted for during an emergency situation is paramount. For this reason, we ask that you remain with your host during your visit at Sysco Columbia. A copy of our evacuation plan can be located throughout our facility and on Page 7of this pamphlet.

Parking Lot Safety The speed limit on Sysco Columbia’s property is 10 mph. Please drive responsibly while maneuvering through the parking lot as there may be pedestrians walking around other vehicles. The back parking lot is reserved for truck traffic only. There are designated Visitor Parking spaces that are provided for your convenience. Handicap Parking spaces are also available along the front of the building.

Key Visitor Responsibilities

1. Respond to all building alarms per the guidance of your host. 2. Inform your host of any medical restrictions or physical limitations that may prevent you from being able to evacuate in the event of an emergency. 3. Obey all posted signs and rules at Sysco Columbia. 4. Wear your name badge conspicuously above the waist and in plain view on your outer-most layer of clothing. 5. Do not tamper with sealed product in the warehouse. 6. Notify your host immediately if you identify a potential hazard or problem. 7. All minors must be accompanied by their guardian while visiting Sysco Columbia. Warehouse tours including minors may be abbreviated to ensure a safe environment is maintained. 3


Occupational Safety and Health In order to ensure compliance with all applicable safety and health standards set forth by South Carolina Occupational Safety and Health (OSHA), Sysco Columbia has developed a comprehensive written Safety and Health Manual. Our Safety and Health Manual includes the following programs: • Access to Employee Medical Information • Accident Reporting and Investigation Policy • Bloodborne Pathogens Program • Confined Space Entry Program • Electrical Safety Program (including arc flash awareness) • Emergency Action Plan • Fall Protection Program • Fire Prevention Plan • Hazard Communications Program • Ladder Safety Program • Lockout/Tagout Policy (including Subpart S) • Material Handling – Sysco Perfect Lift • Medical Services and First Aid • Powered Industrial Truck Program • Process Safety Management Program • Respirator Protection Policy • Welding, Cutting, and Brazing Policy • Workplace Violence and Security Plan Sysco Columbia recognizes that the safety and health standards are only a ‘minimum’ level of compliance. Therefore, it is our expectation to not only meet the standards, but in many cases exceed the expectations set forth in the standards. The Safety and Health Manual is located in the Safety office.

Sysco Emergency Preparedness Plan Sysco Columbia recognizes the need to be prepared in the event of an emergency and/ or disaster. Therefore, we have developed an extensive emergency preparedness plan in the event of certain emergency and/or disaster situations. Our Sysco Emergency Preparedness Plan Manual (SEPP Manual) is located in the Safety Office and includes sections regarding the following situations: • Ammonia Compressor Failure • Ammonia Spill • Bomb Threat • Building Collapse • Computer Failure • Earthquake • Evacuation Plan • Fire • Flood • Freezing Weather • Fuel Spill

• Hostage Situation • Hurricane • Labor Disruption • Media Response • Medical Emergency • Power Outages • Product Recall • Robbery • Sprinkler Trip • Tornado • Vehicle Accident

In addition to the above listed sections, Sysco Columbia has a written Flu Pandemic Plan that covers events such as the H1N1 Virus and Avian Flu. We realize that our customers will be counting on us to provide them support in the event of an emergency and/or natural disaster.

General Facility Safety Hazard Communications Program – Material Safety Data Sheet (MSDS) are available for any chemical that you may be exposed to. The information for the chemicals that we store to sell is located on all Operations computers in the office, and on the loading dock podiums. MSDS Online is the program used for all Material Safety Data Sheets. There is also ‘area specific’ MSDS binders located in the: • Fleet Shop • Facility Maintenance Shop Bloodborne Pathogens Program – ‘Sharp Containers’ are located in each restroom in the event that needles are used to regulate sugar levels. Please 5 do not throw ‘sharps’ in the regular trash receptacles.


Emergency Action Plan - Maps of our evacuation plan are located throughout our facility and Page 7 of this pamphlet. Remain with your host in the event of an evacuation. Powered Industrial Truck Program – Operators of forklifts, pallet jacks, and other types of powered industrial trucks at Sysco Columbia are licensed and certified to operate the equipment in our warehouse. Please DO NOT attempt to operate any powered industrial trucks - regardless if you have had training at another facility. Process Safety Management Program – Sysco Columbia utilizes anhydrous ammonia (NH3) as a refrigerant in order to maintain the correct temperatures in our cooler and freezer spaces of the warehouse. Over exposure to anhydrous ammonia can lead to serious health effects. If you would like further information, refer to the MSDS for specific health hazards. An MSDS regarding anhydrous ammonia can be found in the Safety Office or on any Operations computers. Please follow the guidance of your host in the event of an emergency situation involving anhydrous ammonia.

Tobacco Use Your host can direct you to the only authorized ‘Smoking Area’ at Sysco Columbia. Smoking or chewing tobacco is not authorized inside the facility at Sysco Columbia.

Concealed Weapons and Firearms Concealed weapons and firearms are NOT ALLOWED in the facility.

Cell Phone and Camera Usage in the Warehouse In order to maintain your safety awareness around powered equipment, we ask that you do not use cell phones during the warehouse tour. Also please do not take pictures of the warehouse during tours. 6

Good Manufacturing Practices (GMP) Sysco Columbia recognizes the importance of protecting the integrity of our stored product. Therefore, the following Good Manufacturing Practices have been developed for our distribution facility: • Eating, drinking, and smoking are not allowed in the warehouse. • Separate facilities are provided for employees to take breaks and lunch. • Smoking is allowed outside the building in one designated area. • Employees must wash hands prior to returning to work after smoking. • Restroom facilities are provided upstairs and employees are required to wash hands before returning to work. • The ice machines are supplied with city potable water and back-flow prevention devices are installed and maintained at the city main feed. 7
