don’t have a penchant for technology, but I am trying to change.

Smartphones, iPhones, Androids. I hardly know the difference between any of them and had been content with using my Blackberry. The phone had been given to me as a birthday gift by request three years ago. I wanted the phone, for the sake of looking like I was technologically advanced — not because I truly understood how it could benefit me. At first, I only used it for phone conversations, but when I discovered the convenience of texting, I added text messaging to my phone plan. I was beginning to feel more tech savvy, but after three years of having my phone, it began to lose its appeal — the big crack on the frame of the phone didn’t help either. Then I began seeing commercials about new phones with better features, more applications, and faster download speeds. I thought about keeping my phone a little longer, but after much coaxing from my mom, I purchased the iPhone 3GS. Even with my limited knowledge about iPhones, I could understand its popularity. Now, I can’t imagine my life without it. I can use the phone to go online or check my office and personal emails anytime. My appreciation and curiosity about other facets of technology are blossoming. This year, I learned about facebook tagging and how to edit our company’s facebook pages. I also took an introductory Microsoft Excel class and plan to register for the intermediate course next year. I am not interested in becoming a technology guru, but I at least want to know the basics. If technology and computer software scare you, read about some of the fun ways people are using it in our Tech Gadgets They Love feature on page 34. Consider taking a class or if you have grandchildren, let them teach you about it. Once you get started, you might not want to stop.

Now, only an extremely small percentage of prescriptions require a “custom-made” approach. Once again, though, under the direction of a licensed prescriber, today compound pharmacists can tailor a medication to better suit an individual patient’s specific needs. My insurance does not cover it, unfortunately. My hormone replacement runs $42 a month, but it’s worth it because it works. What else can I say? It just works.”

Keeping Customers Safe Compounding pharmacies fill a variety of needs, but some people might question the safety and legality of the resulting product. Martin argues that although the products do not undergo FDA testing (“prescription costs would skyrocket if the FDA were to get unnecessarily involved”), each pharmacy in Kentucky — both traditional and compounding — is regulated by the State Board of Pharmacy. Pharmacies also work closely with professional organizations such as the Professional Compounding Centers of America (PCCA), the Pharmacy Compounding Accreditation Board (PCAB), and others to ensure that their products and procedures are safe and current. These organizations provide consultations, products, continuing education to pharmacists, and other resources. As for questions of legality, the PCCA website reports, “The Food and Drug Administration has stated that compounded prescriptions are both ethical and legal as long as they are prescribed by a licensed practitioner for a specific patient and compounded by a licensed pharmacy.” Compounding pharmacies promote safety by submitting a random sampling of prescription items to an independent lab company for testing, Martin 12

explains. To measure out ingredients, pharmacists use a highly sensitive scale that is hooked up to a computer. Each drug has a bar code on its label that is scanned prior to measurement, and if either the quantity or the ingredient doesn’t match the information filed in the computer, the pharmacist is immediately alerted.

Keeping Customers Happy Many customers aren’t aware of the safeguards put in place to keep them safe, but they are very knowledgeable and excited about the customer service they receive at a compounding pharmacy. Reuling states, “The personalization of it is just wonderful. My pharmacist recognizes me; she knows me by name and that’s the part I really like.” Martin emphasizes this point: “At any pharmacy, you always have the opportunity to ask questions or talk to the pharmacist if you want, but people are in a rush, they’re tired. One of my favorite things about this job is being able to take one of those people aside, talk to them about their medication, and have them leave with a new understanding. This is something that’s missing from the public consciousness — that the pharmacist is there to really talk to you.” t o d a y ’ s

discovered the above quote more than 40 years ago in seminary, and it remains one of my favorites. Why? It addresses one of

the central issues of life. It reminds us that it takes courage daily to find real meaning and purpose in our lives, and this leads to joy. Paul Tillich was a highly revered German philosopher and theologian who spent the last 10 years of his life as a professor at Harvard and the University of Chicago. Among his many books was The Courage to Be, published in 1952. It’s one of those books I’ll never forget. Say Yes to Life. Our earth has meaning and an ultimate purpose. When we come to understand that life is a balancing act between good and evil, and we work our way through the hardship and suffering that comes with it, we eventually discover and appreciate the joy that surrounds us. The most basic requirement is courage. Paul Tillich reminds us that while neither birth nor death are choices, everything in between is. And in spite of life’s many negative elements, we’re challenged to make the most of it under all circumstances. If we don’t have the courage to do this, we’ll never find the real meaning of our lives. And we’ll never become the fully alive people we’re capable of being. He says courage is the vital strength we need to rise above the hardships and sufferings of life and find something ultimately positive and meaningful. Life is hard and not always fair. Whether we like it or not, that’s the way things are. This sometimes harsh reality leaves us with a choice. We can snivel and whine about it and act like wimps when life deals out some hard blows, or we can accept it as a challenge and show the courage to make the most of it. We often equate courage with valor, bravery, gallantry, and heroism. It conjures up images of great martyrs like Joan of Arc, patriots like the people who declared our independence and fought in the American Revolution, reformers like Mahatma Gandhi, civil rights leaders like Nelson 16

“I understand the Courage to Be as the courage to say ‘yes’ to life in spite of all the negative elements in human existence — in spite of man’s finitude, which his coming from nothing and going to nothing, to die . . . It takes courage to see in the reality around us and in us something ultimately positive and meaningful and live with it. Loving life is perhaps the highest form of the courage to be.”

Mandela and Martin Luther King, Jr. and other well-known heroes through history. The courage of these people has always been honored and celebrated. It’s the best known and more admired of many types of courage. But we need to be reminded that there are other types of courage that are equally important. One of them is everyday courage. The definition of courage is the ability to conquer fear or despair. In the past we may have been called courageous because we stayed in circumstances that were difficult or nearly unbearable. We may have felt walking away from family, children, or friends was cowardly or displayed weakness. We may have felt that by holding back our tears we were stronger people. Yet all the things we may have construed as weakness are really signs of courage. All the things we believed to be acts of courage were really not courageous at all. If we walked away from difficult or unbearable circumstances, we would be conquering despair. If we cried, we would have been courageous by letting go of our fear, pain, or sadness. Courage doesn’t mean putting ourselves in stressful or unpleasant situations. Courage doesn’t mean controlling our emotions. Courage is the ability to strengthen ourselves against the fear and despair of life, rather than be drowned by it. In more modern times, a well know advice-giver said something similar: “If I were asked to give what I consider the single most useful bit of advice for all humanity it would be this: Expect trouble as an inevitable part of life and when it comes, hold your head high, look it squarely in the eye, and say, ‘I will be bigger than you. You cannot defeat me.’” — Ann Landers Life is hard and life is often unfair and we can expect trouble. But that doesn’t mean we have to let it defeat us. Digging down, finding strength and standing up to hardship can be one of the great rewards and joys of life. It takes courage — the everyday kind. e’re all concerned about the growing obesity epidemic — especially as it impacts the lives of our grandchildren.

Instead of fast food, give your grandchildren nutritious meals to help keep them healthy and happy.

Childhood obesity contributes to heart disease, diabetes, sleep disorders, high blood pressure, high cholesterol, depression, joint pain, liver disease, social discrimination, and poor school performance. Some of us suffer from the battle of the bulge ourselves, and we don’t want our grandchildren to inherit the same problem. As grandparents, we sometimes believe that food is love. Let’s take a look at how we can make healthy food a way to care for our grandchildren.

A Crash Course in Nutrition 101 Calories come from carbohydrates, fat, and protein. While one gram of fat has twice the calories as one gram of carbohydrate or protein, too much of any calorie source will result in weight gain. It’s easy to tell the difference from carbohydrates, proteins, and fats. Carbohydrates come from plants. Vegetables, fruits, and all wheat and cereal products are carbs. Proteins, on the other hand, are basically everything that comes from a source that at one time or another breathed, as well as certain plants. Calories Count, but Serving Sizes are an Easier Guide to Healthy Nutrition An easy guide to adult serving sizes is that fruits and vegetables should cover half of our plate. We can have a handful of pasta. A serving size of meats is the size of a deck of cards. A serving of cheese is the size of three dice. A thumbnail size portion of fats is all we need and we should limit our sweets to 75 calories per meal. Serving sizes for children: • Children between 6-10: half of adult serving • Children between 2-5: One spoon of each food group/age in years

Fiber is Our Friend A high fiber diet helps to avoid constipation. It also reduces the risk of high blood pressure, heart disease, digestive disorders, and cancer. High fiber foods are also excellent sources of nutrition. Wheat bran, fruit with skin, vegetables, beans, and oat bran are all fiber rich foods. Remember when we were kids, we used to say, “Beans, beans are good for your heart. The more you eat, the more you …” Well, it’s true! Beans are good for your heart. And it’s also true that gas is created by the fermentation of non-digestible fiber. Fiber is good because it delays digestion and gives a feeling of fullness and satisfaction. Fiber delays your stomach’s efforts to extract the sugars in the carbohydrates. It’s Not Just What You Eat, It’s How You COOK The faster the sugar and starches are processed and absorbed into the blood stream, the fatter you get. If you want to lose weight remember, anything that speeds the process of digestion is bad and anything that slows digestion is good. Keep in mind that the process of digestion starts before the food enters your mouth. For example: Raw broccoli is covered in a layer of fiber. If you eat broccoli raw, your digestive system works very hard to remove the fiber and get to the calorie rich carbs. This takes time and energy (calories). Consequently, it slows digestion and it’s a great way to eat for weight loss. If you cook broccoli until it’s hot, but still crispy, the fiber is broken down and digestion takes less time. If you cook the broccoli to death, the fiber along with most of the nutrients are gone and digestion is very quick. Digestion of the broccoli that should take place in the stomach, that is the reduction from a crisp, hard vegetable into mush, takes place in the pot. Bottom line: Limp overcooked vegetables, while the mainstay of Southern cooking, are of no great nutritional or weight loss benefit. Try to redefine “comfort food” when cooking for your children and grandchildren.

When we think of eating, we often just think of solid foods. However, we must remember that it’s often what a child drinks that can sabotage a child’s nutrition. A bottle of cola has 17 teaspoons of sugar. A glass of whole milk has eight grams of fat. If a child drank four glasses of milk a day, she would consume 500 calories in addition to her regular solid food consumption. If a person drinks three glasses of fruit juice, he consumes 450 extra calories. Healthy drink choices are a key element in lifestyle change. Remember, a child over the age of two doesn’t need whole milk. Fruits are more nutritious than highsugar fruit juices. The best drink of all is water. It’s thirst quenching, has no additives, and contains no calories.

Loving Tips to Keep Grandkids Healthy • Let your motto be, “Lead them not into temptation” and get rid of the junk food. • Carry healthy snacks with you in case of a snack attack: trail mix, dried fruit, Cheerios. • Replace soft drinks, sports drinks, and fruit juices with nonfat milk or water. • Serve fresh foods rather than processed foods. • Avoid fast food restaurants. If you must eat at fast food restaurants, look for healthy meals. • Take children on a food shopping adventure. Allow them to choose new fruits or vegetables. • Serve rainbows of food. The more colorful the food, the more nutritious. • Most important, keep “food rules” that parents have established. Staying within these boundaries will maintain healthy family relationships.

Feed your Grandchild’s Soul Value and worth don’t depend on size or shape. If your grandchild has a problem with weight, don’t ever say anything that makes him or her feel unattractive. Shaming or teasing a child about his weight won’t motivate him to stop eating. It may cause him to hate his body and perhaps turn to food for comfort.

Caring for your back

Don’t get backed into a painful corner

ou expect mental exhaustion, and to have some uneasy nights’ sleep. But, who knew you could be carrying the care of your family in your back?

A recent Spanish study found that one of the most common places stress experienced by caregivers goes straight to, is your back. And, that most likely due to postural and psychological factors, the backs of those caring for loved ones — even if you’re not lifting someone in and out of bed or a chair — are taking a beating. Chronic stress-related pain, says Beverly E. Thorn, Ph.D., ABPP, professor of psychology, University of Alabama at Tuscaloosa, and editor of the Journal of Clinical Psychology, is a very real and common condition. “Stress can cause back pain or exacerbate pain originally caused by a previous injury. “The intensity of back pain can range from constant and dull to intermittently piercing,” says Thorn. Regardless of its strength, that pain can affect your ability to provide care to your loved one.

It’s all in your head Your brain, say experts, is the common link between your back and stress usually caused by emotions like frustration, anxiety, and feeling overwhelmed. “That stress opens the floodgate of pain signals to the brain, which heightens your nervous system making it more vigilant for signs of danger,” says Kelly McGonigal, PhD., a health psychologist at Stanford University specializing in the relationship between psychological stress and physical health. This heightened state sends pain signals to your brain faster than normal. “Those speeding signals are also “amplified,” making low-level pain that you might be able to naturally suppress when you’re not under stress, impossible to ignore,” Dr. McGonigal says. It also makes you more sensitive to any kind of pain, including back pain. continued on page 26 24

Physical therapist Elizabeth R. Lombardo, Ph.D., M.S., P.T, in Wexford, Pa., says whether you realize it or not, you pull your shoulders up into a “shrug-like” position when you’re stressed. “That can significantly fatigue the muscles in both the neck and back and lead to pain.” When you’re mentally or physically tired you may be inclined to slouch, something Lombardo says leads to improper body alignment and fatigued muscles. “Standing up straight — using your core muscles to support proper body alignment — eases the stress on your back and reduces pain,” she says. To reduce your risk of developing posture-related back pain, Lombardo suggests checking your seated and standing posture in a mirror. Here’s what she says you should look like in either position:

Standing

• Your weight should be evenly distributed on both feet, head aligned in the middle of your shoulders. Your chin should be in line with your breast bone. • Your shoulders should be pulled comfortably back, lined up with your hips, but not to the point of standing “at military attention.” • To adjust your stance when standing for a while, place one foot in front of the other and shift/ change positions frequently. • Never lock out your knees and always wear shoes with proper arch supports or slip a pair of over-the-counter gel insoles into your shoes for support and shock absorption.

Sitting • Sit comfortably with your back against your chair and your weight evenly distributed on your hips and tail bone. If necessary, sit slightly forward in the chair but avoid slouching. • Just like when you’re standing, keep your head aligned, over neck, with your breast bone or spine. • Hold your shoulders back, not rotated or slouched. Keep them centered above your hips. • Maintain about a 90-degree angle in your knees. Lombardo says any more than 90 degrees puts a lot of stress on your knee joint. If you can’t do this naturally, use a foot stool or phone book under your feet. 26

Physical reactions to stress also affect your back. “Often done subconsciously, your body naturally tenses up in stressful situations,” says McGonigal. “This low-level chronic muscle tension is a major cause of stress-related chronic pain.” The muscles most likely to tighten up and become sore are your back, shoulders, and neck muscles. Constant muscle tension, says Dr. McGonigal, becomes part of the mind-body stress response, so any stressful thought, even one as simple as “what should we have for dinner”, can tense your back muscles, triggering more pain and discomfort.

Give your back a break Before treating back pain, you need to shut off its source of fuel. Dr. Thorn says, “Stress and back pain create a circle of cause and effect. In order to put an end to the effect, you have to stop the cause.” Yoga, meditation, and breathing exercises are some of the easiest ways to blow off stress-filled steam. And, researchers at the University of New Mexico, Albuquerque found you don’t have to dedicate a lot of time to these stress busters, discovering as little as one hour a week can significantly reduce stress levels. Physical therapist, Elizabeth R. Lombardo, Ph.D., M.S., P.T, in Wexford, Pa., says hours of bending, lifting, or carrying things can block the flow of nutrients to the spinal discs and ligaments creating strain in the back muscles. “This is the easiest type of pain to alleviate,” she says. Scheduling a 10- to 15-minute break every hour to an hour and a half to sit comfortably and give your back a rest usually does the trick. “It allows your discs and ligaments to ‘rehydrate and refuel,’ eliminating the strain and related pain,” Lombardo says. Just don’t stop moving altogether. “A common misunderstanding is that if your back hurts, you should stop moving altogether,” Lombardo says, “When in reality, gentle movement is important to recovery.” Lack of movement leads to reduced muscle strength and less muscle tone. “That equals more pain,” says Lombardo, “because your muscles hold your spine in place in a healthy way.”

Prevent pain On those days when you are stressed, a few simple precautions can prevent a back injury, as well as the chance your back will be aching. These steps, says Lombardo, will help you quell aches and pains, and enjoy your day. • Stretch. You wouldn’t start lifting weights or run a mile without jogging. So, don’t start your caregiving day that way, either. “Stretching warms up your muscles and prevents many muscles sprains and strains,” says Lombardo. • Drink water. This hydrates the muscles and the discs between the vertebrae. Says Lombardo, “If you’re dehydrated, the discs will shrink and the nerves become pinched increasing the pain.” • Lift smartly. Always keep the person or object you’re lifting close to your body and lift with your legs (which are much stronger than any other part of your body). Keep your back straight and avoid any twisting or motion that will take your body out of alignment; move or pivot your feet. • Eat right. Excess sugar and caffeine can lead to adrenal burnout which can lead to greater fatigue, and, ultimately to using poor lifting techniques and the risk of injury. t o d a y ’ s

Dick uses both a desktop and a laptop. When he travels, he takes his laptop with him, but he mainly uses his desktop for everyday use. Dick uses his computer for emailing friends and family, and for keeping financial records for his rental properties. Dick surfs the web to look up information and do research; he plays games, such as Spades, where he is partnered with other people from all over the world. He takes photos with his digital camera, uploads the images to his computer, and emails images to friends and family.

Sarah takes classes through the Apple Store to help her organize and alter photos that she takes with her digital camera. Sarah takes photos of things that inspire her for papermaking art. She also is learning programs on the computer to put together flyers for speaking engagements and slideshows of her work. Sarah will have her art displayed at the Firehouse Gallery, 221 S. Because Kentucky is a Certificate of Need (CON)state, which permits counties to license a specific number of Medicaid-covered beds based on that county’s needy population. While this figure varies from year to year, it is speculated that in Kentucky 75 percent of all nursing home residents are paid for by Medicaid. Therefore, costs to our state are exorbitant

given nursing home rates in 2011 average $6,000-8,000 a month for private pays. Kentucky has so many Medicaid recipients in nursing homes that most Medicaid beds are frequently full, especially in good facilities.
