WRONG MAN DECLARED DEAD: NOTIFICATION OF DEATH FORM FILLED IN ‘INCORRECTLY’

Richwood resident became a “dead man walking” as he was the one declared dead after his elderly father passed away. Dudley Botha (48) says his father, Douglas Botha (79), who was a resident of Huis Martina in Parow, passed away on Saturday 1 July. The home called to say his father was not doing well. Dudley says his father died at Huis Martina before he could get there. He says there were paramedics and also a representative from Avbob present at the home. They took his father’s ID book, but also asked Dudley for a copy of his own ID. He and his family left Huis Martina before the body was removed as it had gotten very late, says Dudley. They received the death certificate four days later, but did not open the envelope until a week later while preparing for the memorial service, says Dudley. “A lady from our fellowship asked me to send a copy of my dad’s ID number for his date of birth.” Dudley’s wife, Debbie, opened the envelope and made the unfortunate discovery that her husband was officially a “dead man”, as it was Dudley’s name and ID number on the death certificate – and not his father’s. Dudley showed TygerBurger a copy of the BI-1663 form (notification of death), which had been filled in by a medical practitioner (from Parow Clinic) and by a representative from Avbob. It displayed over-writing (see picture on page two). The BI-1663 form states the name of the deceased as “Douglas Bruce Botha”. This is Dudley’s father’s correct first name, but with Dudley’s second name, Bruce. It also shows Dudley’s ID number as that of the deceased. The death certificate, when it came, was in the name of “Dudley Bruce Botha”, as issued by the Department of Home Affairs in Malmesbury on 4 July. TygerBurger made an enquiry to the Parow Clinic regarding the BI-1663 form that
Dudley Botha with a copy of the notification of death form and the death certificate issued in his name. had been filled in incorrectly, as well as to the Western Cape health department that responded on Tuesday. TygerBurger also contacted Avbob Maitland whose representative had filled in section D (particulars of informant – in this case, Avbob) of the BI-1663 form and had submitted the application for the death certificate. The form states: “Section D to be complet-
ed by informant. “Informant is responsible for certifying the identity of the deceased.” Marius du Plessis, spokesperson for Avbob Mutual Assurance Society, responded: “I have investigated the matter of Mr Dudley Botha and have found that the medical practitioner who completed the BI-1663 form entered the wrong ID number on the form, which was used to register the death of Mr
Botha’s late father. “The mistake was therefore not made by us. “However, after Mr Dudley Botha became aware of the error, he approached us for assistance and we offered to assist him to rectify the error made by the medical practitioner by submitting new documentation to Home Affairs on his behalf,” he said.
