We’re delighted that John Noseworthy of the Mayo Clinic is joining us on the keynote stage. The clinic, renowned at age 151 as a leading light in both patient care and research, is a great model of an institution adapting aggressively to a brave new world. And we’ll also be hearing from Andy Slavitt, Acting Administrator of the Centers for Medicare and Medicaid Services, about what to expect from his agency in the future. CCN: HOSPITAL ACQUISITIONS OF PHYSICIAN PRACTICES AND FUTURE TRENDS New risk-based payment models, including bundled payments and accountable care organizations (ACOs), incorporate robust incentives for effective utilization of post-acute care. Mounting evidence suggests that the road to success under these models depends upon better management of the postacute care provided to Medicare beneficiaries. Development of a continuing care network (CCN) is often a key part of a hospital’s strategy to manage risk under these new models. Participants will discuss the benefits of a CCN and the
practical steps to creating a CCN as a strategy for achieving the goals of accountable care. Avalere Health experts will provide real-world experiences and lessons from working directly with hospitals and health systems participating in both bundled payments and ACOs in the development of a CCN. Presented by: POPULATION HEALTH STRATEGIES FOR EFFECTIVE AND EFFICIENT CARE DELIVERY Population health takes into account factors that traditional health care delivery is less focused on, such as preventive care services, health behaviors, and engagement with the community and other advocacy organizations. Much needs to be done to improve the training and education of providers, to deploy the right kind of labor mix, and to take advantage of the explosion in informatics capabilities and technology. Improving population health is a journey that focuses on populations, health communities, social determinants and social support networks, and the importance of education, environment, and changing behaviors. The way forward will require multipronged approaches, better information, changing incentives, better coordination of care, a focus on high-cost patients with chronic diseases, and measurement. Nobody knows the right formula, which affords an opportunity for experimentation. This is going to require retraining of health care professionals to do a job that’s slightly different from what they were trained for in school. Health care delivery is only one of the determinants of the health of a population. Moving forward, it will be exciting to see how collaboration with other sectors of society within population health management develops and matures. CCJR: CMS’ NEW MANDATORY BUNDLED PAYMENT MODEL: WHAT DOES IT MEAN FOR YOU? In July 2015, CMS announced a new bundled payment model: the Comprehensive Care for Joint Replacement (CCJR) model. As proposed, beginning in 2016, hospitals in 75 metropolitan statistical areas (MSAs) nationwide will be required to participate in this bundled payment model for major joint replacement of the lower extremity cases. Presented by: CASE STUDY: PATIENT ENGAGEMENT IS CRITICAL TO THE SUCCESS OF A VALUE-BASED HEALTH CARE SYSTEM Room 3 With the implementation of the Affordable Care Act, large numbers of newly insured patients are expected to enter the health care system. The challenge for hospitals and health care providers is to meet the needs of these increasingly diverse patient populations with escalating levels of comorbidities (obesity, diabetes, heart disease and arthritis). The burden of musculoskeletal conditions increasingly impacts all segments of the U.S. population, including working adults ages 45-64, and these conditions are dominant drivers of health care costs and personal disability. While no one is immune, women and members of the African American and Hispanic/Latino communities are disproportionately impacted. This unequal disease burden makes these groups more susceptible to experiencing disparities. Current methods to engage patients to adopt healthier behaviors have been largely unsuccessful. We created an innovative application of a knee arthritis cost model that will allow patients and providers to estimate the impact of treatment choice on quality adjusted life expectancy, direct health care expenses and lost wages based on gender/racial/ethnic characteristics of the individual. We postulate that the effectiveness of a patient engagement tool is highly influenced by the ability to “make the patient aware of their own future” as influenced by their health and medical choices. This tool will be piloted to evaluate

its effectiveness in promoting dialogue between patients and providers, with the ultimate goal being use of a cost model framework to help patients and providers understand holistic care benefits, drive patient engagement, promote shared decision making and optimize value based health care. The Center for Medicare and Medicaid Innovation was created by the Affordable Care Act partly to test whether ACOs work, and the jury is still out. Controversies also swirl over whether ACOs are instigating consolidation in the health care sector.
