EDITOR PAGE Oh well! There will be no Semana Santa to celebrate this year. We can see the light at the end of the tunnel but it seems to stretch just a bit longer. Every corner of the globe seems to be preparing and looking at more freedom of movements from May and June and this is mainly because everybody needs a few good weeks of summer to make up for the weakness in the economy. I only hope that the vaccine and the possible approval of a cure will finally resolve the problem. I was rather hoping that because Corona Virus was made in China that it would not last this long. In June, the IMF forecast a 4.9% contraction in GDP for 2020, the sharpest fall since the Great Depression. Cine Español Acostumbro a aplaudir cuando termino de ver una película que me agita los cimientos y me remueve el alma como si me hallara en un concierto o en una representación teatral. Es la única forma que tengo de expresar mi gratitud, de exteriorizar mi profunda admiración. Esta semana afortunadamente no he tenido más remedio que aplaudir a raudales hasta dolerme las manos ante dos películas inolvidables que ya figuran entre mis títulos preferidos y, precisamente y no es casualidad, ambas eran españolas. Eso sí totalmente diferentes, casi antagonistas me atrevería a decir en sus planteamientos, en sus características técnicas, en sus posicionamientos, pero ambas dignas de figurar con letras de oro en la historia del cine español: La primera de ella es Voces, ópera prima del director algecireño Ángel Gómez Hernández, que aborda el poco cultivado asunto de las psicofonías dentro del más amplio tema de las casas encantadas.

La otra película es El huerto del francés, dirigida por Jacinto Molina, nombre real del actor y director Paul Naschy, en 1977 y rescatada ahora, tras una profunda restauración, gracias a la labor de su hijo Sergio. La cinta cuenta la primera historia de un asesino en serie llevada al cine en España y que narra una historia real, sucedida en un pueblo sevillano y cuenta con un elenco de actores y actrices de primer nivel, ya que, acompañan a Paul tres actrices que hacían furor por aquellos años: Ágata Lys, María José Cantudo y Silvia Tortosa.
Se trata de una película que renueva con sus planteamientos valientes y sus propuestas audaces el cine de terror en España. Es una historia redonda y bien rematada, donde se enlazan el pasado y el presente, una historia anclada en plena Edad Medía que quedó sin resolver es el detonante de unas extrañas voces que parecen proceder del más allá que no dejan descansar en su nueva casa a una familia española.

Maravillosa en su estética y en el esmerado cuidado de todos y cada uno de los detalles técnicos y artísticos es una gran puesta en escena de este talentoso director algecireño, que ya ha sido llamado por Hollywood para trabajar en varios proyectos en la cima del cine mundial.
Una verdadera joya de nuestro cine ahora recuperada. Definitivamente el cine español por muchas razones es, sin lugar a dudas, mi favorito y no entiendo la aversión que todavía sienten muchos de nuestros compatriotas hacia nuestro cine. Las españoladas ya quedaron muy atrás. Ahora se hacen películas y series que sorprenden en todo el mundo.

REFUNDABLE PAY-OUTS The Spanish government recently approved an €11 billion economic relief package which includes €7 billion direct aid to distressed businesses. This is done to stimulate the economy crisis caused by COVID-19 and prevent a wave of corporate insolvencies. The package comes in three separate funds:
1. A €3 billion pool to restructure stateguaranteed loans, to be managed by the banking sector. 2. A €1 billion reserve to recapitalize medium-sized companies, run by the state-owned financing company Cofides.

3. €7 billion in non-refundable direct aid to self-employed workers and small and medium-sized enterprises (SMEs) affected by the coronavirus crisis. (2€ billion are for the Balearic Islands and Canary Islands, two regions badly hit by the crisis.

Eligible companies must prove that their revenues fell by at least 30% in 2020 from 2019, and the cash grants must be used for fixed expenses or debt reduction. The aid targets sectors particularly affected by the pandemic, such as hospitality, wholesale and retail distribution, or culture. The self-employed may collect up to €3,000 or €4,000 depending on whether they file taxes under the módulos system or not. Businesses may be eligible for up to €200,000. Direct aid to struggling businesses has been a longstanding demand in Spain, where restrictions to contain successive waves of the coronavirus have pummelled This is well overdue after the thrashing received by the retail and tourist industry due to the Covid19 restrictions. Lamb Shanks
It’s Easter, lamb is a must and braising it couldn’t be easier
METHOD •

Preheat oven to 350°. Season lamb shanks with salt and pepper. In a large Dutch oven over mediumhigh heat, heat oil. Add lamb and cook until golden all over, about 4 minutes per side. Work in batches as necessary. Remove lamb from pot and place on a plate.

In the pot, melt 2 tablespoons butter over medium heat. Add onion and carrots and cook until soft, 4 minutes. Add garlic and cook until fragrant, 1 minute more, then add tomato paste and stir until vegetables are coated.
•
Pour in broth and tomato sauce, then return lamb shanks to pot. Add rosemary and bring to a boil. Cover and transfer to oven. Cook until lamb shanks are completely tender, 2 hours.
•
Remove lamb from pot and place on a cutting board. Tent with foil to keep warm. Discard rosemary.
•
Return pot to stove over medium heat. Add wine and bring to a boil. Reduce heat and let simmer until thickened, 10 minutes. Stir in remaining 2 tablespoons butter.
•
Serve lamb with mashed potatoes and spoon sauce on top.

Climate change is causing plants to release more pollen in spring and for longer, which could make seasonal allergy cases worse, the effect of this can already be seen this years, this early, in some regions. Spring and summer are increasingly warmer and longer seasons due to the increase in Earth’s temperature because of climate change. This causes plants to release more pollen and for longer than in previous times. As a result, it is expected that the number of people allergic to pollen will grow in numbers. We have already seen the effect of this being announced as a warning in certain UK areas. These are the data from a study led by Lewis Ziska of the United States Department of Agriculture. They analysed data on the variation in the pollen concentration of flowers that cause allergies in humans. They focused on 17 areas of the northern hemisphere and on data recorded over 26 years.

By relating them to the variations in temperature produced by climate change, they found that, in areas with higher temperatures due to climate change, there were higher concentrations of pollen. Of all the areas analysed, 71% showed a very significant increase in the amount of pollen in the environment. In addition, in 65% the pollen production season (between early spring and early summer) lengthened with the increase in temperature. An increase in allergy cases would bring with it higher healthcare costs. More drugs, inhalers and, in the most severe cases, hospitalization would be necessary. In 2018 the US government already allocated 18,000 million dollars just for this disease. In 2020, spring came faster than ever and was much warmer than in previous years.

RADIOTHERAPY AND PROSTATE CANCER It is estimated that prostate in Spain cancer causes up to 7% of cancer deaths in men in Europe.
Localized prostate cancer is radically treated with local therapy, generally surgery and/ or radiation therapy, while advanced/ metastatic prostate cancer is treated with hormone therapy, chemotherapy, and/ or targeted therapies, requiring radiation therapy in some cases to alleviate symptoms or improve. Local control. Radiation therapy is an essential treatment for the multidisciplinary management of this tumor. This treatment has been gaining importance over the years as it presents excellent disease control figures, comparable to surgical treatment1, with the advantage of being a non-invasive treatment. The prevalence of prostate cancer reaches almost 60% in men over 80 years old. Therefore, in this more fragile population of patients, the ideal is to be able to count on a treatment that avoids the patient having to undergo surgery with the risks that this may entail.

RADIATION THERAPY IS GIVEN WITH CURATIVE INTENT: •
In patients with localized tumor, either low, intermediate or high risk.

RADIATION THERAPY TO ALLEVIATE THE SYMPTOMS OF THE DISEASE •
In those patients in whom the disease is more advanced, radiotherapy can improve the symptoms derived from the disease, such as pain.

Prostate cancer tends to have a predilection for bones, which is why metastases are usually located at the level of the bones of the spine and pelvis. Radiotherapy achieves an improvement in pain of up to 70%.
•
When dissemination is limited, radical intention treatment can also be performed at the affected site / locations.
RADIOTHERAPY THAT FOLLOWS THE TUMOR •
The prostate moves up to 12-15mm due to changes in the volume of the bladder or rectum (the organs next to it). Other more conventional radiotherapy systems do not detect this movement, with the risk of not treating the entire area well or of having to widen margins and therefore increase the volume that will receive radiotherapy.

Our radiotherapy has a tracking system known as Synchrony, capable of following the movement of the tumor in real time during each radiation session.

This is achieved after the placement of fiducials inside the prostate, which the system is capable of tracking and thus administering the treatment with high precision.

This precision is ideal for those extreme hypofractionation treatments (SBRT) that manage to eliminate the disease with less treatment sessions.
In summary, radiotherapy treatment is a fundamental part that can be offered in the different stages of the disease and always with a high level of evidence. HC radiation therapy in prostate cancer: IMPROVE SURVIVAL FIGURES: •

The treatment of prostate cancer with tomotherapy achieves excellent control of the disease, reaching 5-year relapse-free survival rates of up to 98.9% without associating severe acute toxicity. In patients at higher risk (high or very high risk) after treatment with tomotherapy together with hormonal therapy, a relapse-free survival at 5 years is observed between 91-99%. Furthermore, tolerance to treatment was good, with 10% or less of the patients presenting long-term moderate-severe genitourinary or gastrointestinal side effects.

MY OPINION
It may just be my opinion due to my upbringing, but there is nothing more satisfying and pleasing to the stomach or the palate than a good plate of Spaghetti. Forget Marco Polo, that is an amusing story, the truth is that he did not bring the idea of noodles to Italy. By the time Marco Polo made his trips to China, at the end of the 13th century, pasta was already very popular in Italy. As a matter of fact there is a belief that it was the arrival of some Arabs in Italy who made this delicious introduction when showing their technique of drying pasta to preserve it as food during their long journeys. This type of early Arab pasta can even be found in ancient Greece. While the Chinese have been and still are using noodles for thousands of years, even before Italian pasta, it took the Italian creativity and good taste to move on from water and flour.

The other main difference is that Chinese noodles are traditionally made with rice flour while Italians use their naturally grown Durum wheat that sets the pasta well apart from the rest. Pasta demands a condiment even if only olive oil and garlic and this is where all sorts of different ideas and condiments came to be, many influenced by the local produce within the individual regions. Spaghetti alla Napoletana, Spaghettri Bolognese, Spaghetti al pesto and so on and on, until it was found that the shape of pasta had a lot to do with improving the taste even further.

LASAGNE is one of the earliest known pasta shapes dating back to Roman times and even ancient Greek times, however the condiments was quite different then because tomatoes were not yet in Europe until the 16th century.
SPAGHETTI is of course the most famous shape of Italian pasta, its origin is traceable to Sicily round about the 1100s. It became the staple diet for Italian immigrant to America that increased their popularity tenfold.

PENNE was invented by Giovanni Battista in 1865, an Italian pasta maker, when he made a machine that cut the pasta at an angle.
MACCHERONI (derived from the word macaria) supposedly introduced to Italy by Greeks when they established a colony in Naples.
The list is of varieties is just as endless as the that of the different sauces, And different ways of preparing them. Outside Italy we are only familiar with just a few varieties on display at the supermarket, but if you were to walk into a grocery store in Italy you would literally be confused with the variety on offer, bucatini, fidellini, fettuccine, pappardelle, fusilli, cavatappi, and….

Gibraltar the end of a tax heaven New Gibraltar Treaty On the 4th March 2019 there was an agreement signed by Spain and the United Kingdom on Gibraltar, it’s the first international treaty signed since the Utrecht Treaty of 1715, when Gibraltar was ceded to the UK. The agreement struck between Spain and the United Kingdom came into force on the 13th March this year, which means that Gibraltar is no longer a tax haven. The idea of the treaty is to eliminate tax fraud and the detrimental effects of a tax system that allowed people to pay corporate tax only on the profits they made in Gibraltar. “The agreement resolves the fiscal sovereignty of the area and ends an anachronistic and tremendously damaging situation for Spain without weakening its position on the sovereignty of the Rock,” said the Minister of Foreign Affairs, Arancha González Laya, in Congress.
The new treaty establishes clear rules on tax and residence and prevents people who are resident in Spain from taking advantage of Gibraltar’s tax system. Legal tax residence will be determined by several factors: the location of the majority of a person’s assets, where most of their income is generated and whether the majority of owners or managers are tax residents in Spain, making it an effective tool in the fight against tax evasion. The new treaty also establishes an increase in Administrative cooperation and reinforces cooperation through a liaison body and a joint coordination committee. Foreign sources have assured that the Agreement does not imply any change with respect to the Spanish or British position regarding sovereignty and jurisdiction in relation to Gibraltar.

La vacuna de AstraZeneca, la píldora anticonceptiva y el riesgo de trombosis ¿Por qué la píldora sí y la vacuna no? Todo empezó con la suspensión temporal de la vacuna de AstraZeneca mientras se investigaban varios episodios de trombosis ocurridos tras su administración. Esto ha llevado a muchas usuarias de este método hormonal, que aumenta el riesgo de sufrir un trombo, a preguntarse el porqué de este sesgo en la administración de dos fármacos con, aparentemente, riesgos similares. Aunque la Agencia Europea del Medicamento ha vuelto a autorizar la vacuna, fueron más de una decena los países europeos que la suspendieron de manera preventiva después de que saltara la alerta de un posible efecto secundario en forma de trombosis. Finalmente, un portavoz de la EMA ha declarado que se trata de “una vacuna segura y muy eficaz” y que “los beneficios superan con creces a los riesgos”.

La Sociedad Española de Neurología ha ayudando al análisis de los tres trombos venosos que se han producido en España. El vocal de su junta directiva, David García Azorín, ha explicado hoy en una entrevista con El País el porqué de la pausa en la administración de la vacuna:

“Cuando se empieza a poner un fármaco nuevo se establece un sistema de farmacovigilancia. Cuando se hace un ensayo, aunque puedan participar muchos sujetos, no es toda la población. Hay un grupo de efectos adversos que pueden ser tan raros que no se vean en el ensayo, pero luego se detecte entre la población”. En el sistema de farmacovigilancia empezó a haber alertas de casos de trombosis de senos venosos (espacios que drenan la sangre del cerebro) y, al coincidir con la administración de la vacuna, se decidió investigarlo. Aquí radica la principal diferencia con el riesgo que presenta la píldora anticonceptiva. “El riesgo de los anticonceptivos está ya establecido pero en el de la vacuna no”. Adrián Valls, neurólogo del Hospital Universitario de Torrejón, nos los aclara así: “el riesgo de los anticonceptivos está ya establecido pero en el de la vacuna no”. Remarca, además, que todo tiene un riesgo y un beneficio en la medicina, así que “hay que tener en cuenta que, si el riesgo de morir por Covid es superior al de hacerlo de trombosis en la franja de edad en la que se están produciendo los casos, conviene vacunar”. La directora de la EMA, Emer Cooke, ha manifestado que no han llegado a la conclusión de que la vacuna esté relacionada con los trombos, aunque tampoco pueden descartar que exista una relación. De este modo, hasta el momento, los datos comparados serían son los siguientes: 30 de los 5 millones de personas a las que se les ha inoculado una dosis de AstraZeneca, es decir, el 0,0006%, ha tenido una trombosis coincidiendo en el tiempo con la vacunación. En lo que concierne a la píldora, esta afección está catalogada como efecto “raro” y puede afectar hasta 1 de cada 1000 pacientes.
La píldora y sus riesgos Se llama trombo a la sangre coagulada dentro de venas o arterias. Se puede producir en las venas y también en las arterias. Así, se distingue entre trombosis arterial (ictus, infartos), que son potencialmente más mortales, y trombosis venosa. Esta última es la que se investiga si guarda relación con la vacuna de AstraZeneca. Médicos consultados por esta revista nos han explicado que la manifestación más frecuente suele ocurrir en las piernas y, en general, no conlleva consecuencias a largo plazo, aunque puede ser potencialmente grave si el trombo va al pulmón. En lo que respecta a la toma de anticonceptivos, las investigaciones señalan que el riesgo de trombosis venosa es de 0,06 por cada 100 personas tomándolos un año. Es decir que, 1-5 de cada 10.000 tendrán una trombosis venosa. Este riesgo, en mujeres que no utilizan un comprimido hormonal combinado y que no están embarazadas se encuentra en 2 cada 10.000. Cuando desplegamos el prospecto de una caja de anticonceptivos como Yasmín o Zoely, vemos que los efectos adversos se dividen entre: frecuentes (pueden afectar hasta 1 de cada 10 pacientes), poco frecuentes (1 de cada 100) y raros (1 de cada 1000 pacientes). Así van categorizados desde los más normales y comunes, como migraña o nauseas, hasta los más graves y menos frecuentes, tal y como asma o la mencionada trombosis.

Beneficios y riesgos La píldora anticonceptiva contiene estrógeno y un progestágeno que se toma una vez al día de forma oral.Tiene como objetivo impedir la ovulación para que este no pueda ser fecundado . Además de ser uno de los métodos femeninos anticonceptivos más utilizados por su alta eficacia, que alcanza el 99.9%, también se ha encontrado "evidencia convincente" de que puede aliviar el dolor menstrual intenso y ha demostrado ser efectiva para el dolor pélvico secundario de endometriosis. Por lo tanto, como sucede con cualquier otro medicamento aprobado, no se trata tanto de posicionarnos en un pastillas anticonceptivas sí o pastillas anticonceptivas no, sino en si están bien indicadas para cada persona. Según los médicos consultados por Trendencias, bien indicadas son seguras, mal indicadas son peligrosas. Y es que los riesgos dependen también de los antecedentes personales. Aunque la posibilidad de trombosis sea baja, entre los factores detectados que aumentan el riesgo de que se produzcan trombos se encuentra que un pariente próximo haya sufrido uno antes de los 50 años, el sobrepeso, 48

fumar a partir de los 35 años, pasar mucho tiempo sin ponerse de pie debido a una lesión o enfermedad, la diabetes o los problemas del corazón, por nombrar algunos. Los expertos recalcan que en los casos de trombosis venosas siempre hay que buscar otras causas subyacentes como trombofilias (ya sea por enfermedades genéticas o por cáncer oculto). Además, los viajes en avión de más de 4 horas también pueden aumentar temporalmente el riesgo de un coágulo de sangre, en especial si se tiene alguno de los otros factores enumerados. Y es que el riesgo de trombosis aumenta cuantas más afecciones tengamos. Es por todo ello que, a la hora de tomar anticonceptivos, es importante informar a nuestro médico de si sufrimos alguna de estas condiciones, aunque no estemos seguras, o si alguna cambia mientras los tomamos. En definitiva, es cierto que las pastillas anticonceptivas aumentan el riesgo de trombosis venosa y arterial pero este es, por lo general, bajo cuando la píldora está bien indicada. Además, la píldora puede ser beneficiosa para el tratamiento de algunas enfermedades, más allá de prevenir embarazos no deseados. Otro tema diferente sería si la industria y los médicos no están prescribiendo actualmente estas pastillas con demasiada frecuencia y a la ligera a mujeres en edad fértil. Pero eso ya lo dejamos para otro día.

WHAT YOU CAN DO TO COMBAT CLIMATE CHANGE IN THE WORDS OF BILL GATES
that as long as humans continued to emit any amount of greenhouse gases, temperatures would continue to rise. So I thought that the world needs to provide more energy so that the poorest can prosper, but we have to provide that energy without releasing more greenhouse gases. Now the problem is even more difficult, in addition to cheap, it also had to be clean. In a few years, I was convinced of three things: •
To avoid a climate disaster, we have to reach zero greenhouse gas emissions.

We need to implement the tools we already have, like solar and wind power, faster and smarter.

The zero emissions target must be rock solid. Setting a goal that only involves reducing emissions, but not eliminating them, will not change anything. The only sensible goal is zero emissions.”
Finally, the book proposes initiatives for citizens. “Thanks to activists around the world, we don’t need to generate demand: millions of people are already calling for action” says Gates. His book encourages you to make calls, write letters, assist town halls demanding improvements. Demand more funding for clean energy R&D, a clean energy standard, or a price for carbon, for example. Actively participate in local politics and, as consumers: “For example, if you can afford to install a smart thermostat to reduce energy consumption when you are not at home, please do so by all means. You will reduce your bill and your greenhouse gas emissions. When you choose an electric car or veggie burger, you’re saying, If there’s a market that offers climate-responsible products, I’ll buy them. If enough people send the same signal, companies will respond, fairly quickly, in my experience. They will spend more money and time making low-emission products, which will lower the prices of those products that will be manufactured in large quantities. It will make investors more confident in financing start-ups that are making progress that will help us reach the zero emissions target.”
