here are times in our lives that give us pause, and we think to ourselves: “What happened to the happy-golucky college graduate out to show the world? I’m struggling with strange feelings, moods and emotions that I don’t understand.” Others may say, “How did our relationship change? Mental health counseling or “therapy” is a process where you work with a professional, licensed counselor towards addressing problems that

When you find a counselor and explain your concerns to them, they will first develop a Treatment plan— steps that need to be taken to deal with the issue at hand. Sometimes a therapist may recommend a psychiatric evaluation or a rehabilitation program. Once therapy begins, the therapist may guide your journey, but in most settings the “client” or “patient” defines the journey and is generally asked to set an agenda for each session. As you are guided through a process of self-reflection, you will learn more about yourself and the behaviors, conflict or situation that you would like to change. You may discuss when or how your problem began, the skills you used to cope before you began therapy and your strengths and areas for growth. Often times, therapy helps a person to understand that while they may be struggling, they are competent in many areas and may even have skills or strengths that they never realized or tapped into. At times painful, therapy will lead you to a better understanding of yourself and provide you with a set of tools with which to tackle your unique situation. In 2012, my aunt Deborah Dozier-Hall also passed away from this dreaded disease. Pancreatic cancer like so many other cancers is usually diagnosed in the late stages, which makes it harder to find a cure. My mother and my aunt’s willingness to fight inspired me to continue the fight for others with pancreatic cancer. I was determined to find a way to make a difference in their honor. I researched organizations that supported efforts to beat pancreatic cancer and found The Lustgarten Foundation. All the money raised goes to research, and walks are held across the country; however there wasn’t one in my area. My co-founder Michelle and I organized a walk in the honor of my mother and other family members. The Lustgarten Foundation is making great strides towards a cure via research funding from charitable donations. Your donation and participation in the walk would help us to continue and expand upon these vital activities. Pancreatic cancer is the fourth leading cause of cancer deaths in this country. This year, an estimated 48,960 adults (24,840 men and 24,120 women) in the United States will be diagnosed with pancreatic cancer. It is estimated that 40,560 deaths (20,710 men and 19,850 women) from this disease will occur this year. The Lustgarten Foundation is a not-for-profit organization whose mission is to change these statistics. To date, we have provided more than $110 million in research support. In addition, we provide in-depth information to patients and their families. We are hopeful that increasing awareness and raising funds to support the research will help advance the early detection and treatment for pancreatic cancer.

Drinking Too Much Iced Tea Led to Arkansas Man’s Kidney Failure: Doctors

D

octors traced an Arkansas man’s kidney failure to an unusual cause — his habit of drinking a gallon of iced tea each day. They ruled out several potential causes before stumbling on a reason for the 56-year-old man’s kidney problems: He said he drank about 16 8-ounce cups of iced tea every day. Black tea has a chemical known to cause kidney stones or even kidney failure in excessive amounts. “It was the only reasonable explanation,” said Dr. Umbar Ghaffar of the University of Arkansas for Medical Sciences in Little Rock. She and two other doctors describe the case in Thursday’s New England Journal of Medicine. The unidentified man went to the hospital last May with nausea, weakness, fatigue and body aches. Doctors determined his kidneys were badly clogged and inflamed by the food chemical called oxalate. The man is now on dialysis, and likely will be for the rest of his life, Ghaffar said. Besides black tea, oxalate is found in spinach, rhubarb, nuts, wheat bran and chocolate. In rare cases, too much oxalate can lead to kidney trouble, but often there’s also a contributing intestinal problem. That didn’t seem to be the case for the Arkansas man, and he had no family or personal history of kidney disease. At 16 cups of iced black tea each day, he was taking in three to 10 times more oxalate than the average American, Ghaffar and her colleagues reported. Federal studies suggest that, on average, U.S. adults drink a total of 10 or 11 cups of beverages per day — that includes water, coffee, soda and all other liquids combined. Ghaffar didn’t know if the man drank sugar-sweetened iced tea — the way it is usually served in the South. While he’d had diabetes, that didn’t cause his kidney problems, she said. The Arkansas case appears to be very unusual, said Dr. Randy Luciano, a Yale School of Medicine kidney specialist who has treated people with kidney damage from too much oxalate. “I wouldn’t tell people to stop drinking tea,” said Luciano, who was not involved in the research. What the man drank “is a lot of tea.”■

What are the Symptoms of ADHD in Children?

nattention, hyperactivity, and impulsivity are the key behaviors of ADHD. It is normal for all children to be inattentive, hyperactive, or impulsive sometimes, but for children with ADHD, these behaviors are more severe and occur more often. To be diagnosed with the disorder, a child must have symptoms for 6 or more months and to a degree that is greater than other children of the same age. Children who have symptoms of inattention may: • Be easily distracted, miss details, forget things, and frequently switch from one activity to another • Have difficulty focusing on one thing • Become bored with a task after only a few minutes, unless they are doing something enjoyable • Have difficulty focusing attention on organizing and completing a task or learning something new • Have trouble completing or turning in homework assignments, often losing things (e.g., pencils, toys, assignments) needed to complete tasks or activities • Not seem to listen when spoken to • Daydream, become easily confused, and move slowly • Have difficulty processing information as quickly and accurately as others • Struggle to follow instructions. Children who have symptoms of hyperactivity may: • Fidget and squirm in their seats • Talk nonstop • Dash around, touching or playing with anything and everything in sight • Have trouble sitting still during dinner, school, and story time • Be constantly in motion • Have difficulty doing quiet tasks or activities. Children who have symptoms of impulsivity may: • Be very impatient • Blurt out inappropriate comments, show their emotions without restraint, and act without regard for consequences • Have difficulty waiting for things they want or waiting their turns in games • Often interrupt conversations or others’ activities.

ADHD Can Be Mistaken for Other Problems - Parents and teachers can miss the fact that children with symptoms of inattention have ADHD because they are often quiet and less likely to act out. They may sit quietly, seeming to work, but they are often not paying attention to what they are doing. They may get along well with other children, whereas children who have more symptoms of hyperactivity or impulsivity tend to have social problems. But children with the inattentive kind of ADHD are not the only ones whose disorders can be missed. For example, adults may think that children with the hyperactive and impulsive symptoms just have disciplinary problems. What Causes ADHD? Scientists are not sure what causes ADHD, although many studies suggest that genes play a large role. Like many other illnesses, ADHD probably results from a combination of factors. Why Is Losing Weight So Difficult?

W

e have all been on a diet at sometime in our life. We deprive and discipline ourselves for days, weeks and even months to achieve a certain goal, only to see our results sabotaged because of our inability to maintain the level of strictness. Few individuals can make drastic changes and have lasting success. The body is always striving for health and it attempts to achieve that by continuously cleansing itself of waste material. Toxicity is the primary cause of excess weight, particularly in people who have problems losing weight and keeping it off. Toxins are produced in the body from the byproducts of foods that are not properly digested. This is increased with the ingestion of processed and chemically altered foods. We are not biologically adapted to deal with these forms of “food.” The more toxic waste that builds up in your system, the more this translates to excess weight. Three reasons why we are unable to keep weight off or lose weight are poor digestion, poor elimination/ constipation and a sluggish liver. Food cravings and hunger are closely related to digestive problems. Enzymes help us digest our food. They boost our energy, boost metabolism, detoxify and aid in weight loss. You can get enzymes by eating more raw fruits and vegetables, drinking water with lemon, making a tea with apple cider vinegar or taking a supplemental enzyme formula. Chronic Pelvic Pain Caused by Vascular Conditions

y some estimates, chronic pelvic pain in women accounts for 10-15% of all gynecologic visits. There may be several causes or contributing factors to chronic pelvic pain; vascular conditions are often overlooked. Vascular conditions that cause pelvic pain in women can be diagnosed easily and in a non-invasive manner. Once identified, they can be further evaluated by venography and treated successfully with outpatient-based endovascular techniques.

Causes For Pelvic Pain in Women Pelvic pain is most often caused by a variety of non-vascular conditions: Gastrointestinal, Urinary, Reproductive or Musculoskeletal/other After these more common causes for pelvic pain have been ruled out, one should consider further vascular evaluation. Pelvic Congestion Syndrome (PCS): PCS is most commonly characterized by chronic pain in the pelvic region, caused by venous reflux and poor blood flow in the pelvic veins. May-Thurner Syndrome (MTS): MTS is caused by a compression of the iliac vein. MTS may or may not have pelvic pain, but generally will have varicose veins and/or swelling in one leg (typically the left leg). Signs and Symptoms Many patients with PCS and MTS will experience no symptoms. However, chronic pelvic pain is the most common. Because pelvic pain can present with other conditions, it is important to rule those out prior to the diagnosis. The PCS and MTS common symptoms include: • Chronic pelvic pain, experienced for six or more months. • Pain that is constant or intermittent. • Pain that is dull, achy, in the lower abdomen, pelvic region or lower back. • Pain that generally worsens as the day progresses, particularly with pro-longed sitting or standing. • Pain during sex Both PCS and MTS are more common in women who have had multiple births. How Is Pelvic Pain Diagnosed? Diagnostic testing is minimally invasive and generally pain free. Several techniques may be used: Pelvic ultra12

sound, Venography (x-ray of the veins), and Intravascular ultrasound (IVUS) Treatment For Chronic Pelvic Pain The most conservative and least invasive treatments are most desirable to restore optimal blood flow and relieve pelvic pain. There is no one-size-fits-all solution. Treatment options may include: Embolization therapy – generally involves injecting a sclerosing agent into the vein. This results in the closing of the affected vein, with blood flow being naturally rerouted into healthier veins. Treatment is very similar to sclerotherapy in the legs. Venoplasty and Stenting – a majority of women diagnosed with MTS will undergo a minimally invasive procedure to place a stent in the compressed iliac vein. Using intravascular ultrasound, the physician will guide a catheter through the vein in the leg and into the pelvic region allowing precise measurement. If necessary, a balloon and stent may be placed into the vein to keep the vein open. Bypass surgery – in very rare cases, a bypass surgery may be the best option to restore proper blood flow in the pelvic region.■

When Might I Notice Changes In My Oral Health? During puberty, fluctuations in hormones can make gums more susceptible to gingivitis. As a result the gums may appear red and swollen, and they can bleed. During menstruation, women who have a tendency to develop canker sores and cold sores may develop a pattern in which these sores recur during every menstrual cycle. During pregnancy, gingivitis may develop. In fact, gingivitis is the most common oral condition associated with being pregnant. Sometimes, however, women will avoid dental checkups for fear that treatment might harm the developing baby. In truth, untreated gum infections and decayed teeth can put a mother and her baby at risk. Dental infections may be responsible for as much as 5% of low-birth-weight preterm babies. Pregnant women need routine checkups. Always tell your dentist and his or her staff if you are pregnant. Some women also experience dry mouth while pregnant. Taking fre-

quent sips of water and chewing sugarless gum or candy can help alleviate this symptom. Women who experience morning sickness need to brush their teeth more frequently than twice a day. Women who use birth control pills may also be more prone to healing problems or dry socket after tooth extraction. our doctor’s name, your past diagnoses and medical treatment history—even the kind of medicine you take— are examples of the types of personal information for sale in a billion dollar underground market called Darknet. The underground Darknet contains a collection of websites where health-care information, credit card numbers, weapons, social security numbers, illegal drugs and personal financial information can be found for sale. Cyber-criminals target healthcare providers, insurers and pharmaceutical manufacturers and other organizations for personal and medical information. One of the ways in which cyber-criminals are committing healthcare fraud is by filing false patient claims to insurers and government agencies that provide health services. They also fraudulently file tax returns, open loans and drain home equity and retirement accounts. Credit card and account data have a limited lifespan; the bank or the victim can cancel the card numbers and accounts instantly but the information contained in medical records has much more information. The value of healthcare data to a cybercriminal is much higher than a credit card or bank account information. Healthcare organizations are at a disadvantage when it comes to protecting itself from cyber-attacks. The Importance of

Beneficiary Designations

I

f you have an employer-sponsored retirement plan, life insurance policy, annuity, or other retirement account, you probably specified a beneficiary to inherit the proceeds upon your death. Beneficiaries can also be named on brokerage accounts, certificates of deposit, mutual fund accounts, and checking or savings accounts. These accounts become known as payable on death, or transfer on death accounts. Unfortunately, once the account is established and funded, many of us forget to revisit the beneficiary designations. What if you divorce or have children? What if one of your beneficiaries passes away? All of these are important reasons to review your beneficiary designations every few years or when there is a significant life event. In Maryland, divorce revokes all provisions in a will relating to the spouse. However, divorce would not affect the beneficiary designation on an insurance policy or other account. It is important to review all policies and accounts to ensure the beneficiaries are updated following a divorce. Otherwise you may be leaving a windfall to an ex-husband or wife. If your beneficiary passes away before you and you don’t update your beneficiary designations, the secondary beneficiary will inherit, if you’ve named one at all. But if you did not have a contingent beneficiary and your primary beneficiary predeceases you, in most circumstances the account will be paid to the estate and the proceeds will need to go through probate. If you do have a secondary beneficiary named when the primary beneficiary passes away, the paperwork

should still be revisited so that both primary and contingent beneficiaries are designated. If you rolled over an employer-sponsored plan, such as a 401k or 403b upon separating from the company or retiring, your beneficiary designations do not carry over to your IRA. Whether you’re setting up a new IRA or rolling over the plan into an existing retirement account, the rollover is a good time to ensure that your beneficiary designations are updated. Other considerations are whether you named a minor child or special needs beneficiary to inherit the account. A minor child should not be designated as a beneficiary, because if they inherit, a guardian or custodian would need to be appointed to manage the property. Often, it is better to establish a trust for the child’s benefit and designate the trustee of the child’s trust as the beneficiary of your account. If a special needs beneficiary inherits money outright, he may become disqualified from receiving public benefits. To avoid jeopardizing the beneficiary’s eligibility, a special needs trust should be established and the trustee of that trust should be designated as the beneficiary of your account. Perhaps you named beneficiaries before you had children or grandchildren. Though your spouse may be designated as your primary beneficiary, you should determine if adding contingent beneficiaries, such as children or grandchildren, would help achieve your estate planning goals. An estate planning attorney can assist you in creating or reviewing your estate planning documents, including updating beneficiary designations on your accounts.■

Open Enrollment and Maximizing Your Employee Benefits Part 2 of 2

want to spend this section helping people understand the benefits of lower healthcare cost through the Affordable Care Act (ACA). Many people assume the Affordable Care Act approved health insurance only for Small Businesses with 100 employees, Uninsured Individuals, Underinsured Individuals, and Individuals with preexisting medical conditions. After helping several clients—who are self-employed contractors—research health insurance plans for their small business or themselves, I realized the significant savings that are available. There were some assumptions my clients and I made before we researched health insurance. The assumptions were as follows: All insurance was the same 1. We discovered many different ways to modify your out of pocket expenses. Insurance provided by an employer doesn’t give the design flexibility. You can pay a higher premium and have a lower deductible to prevent higher cost if there are existing medical conditions. One client had diabetes and irregular heartbeat and was able to reduce their annual out of pocket cost because they could choose a higher premium and lower out of pocket deductible. Health insurance cost wouldn’t decrease significantly 1. If you work with one of the approved 15 health insurance brokers, you can qualify for a tax subsidy to lower your monthly payment up to $600/month. Some clients were able to pay a

premium of $50-$150/month because of the subsidy. I was able to save $550/month by switching from my wife’s employer plan to a tax-subsidized health insurance plan through one of the ACA plans and qualify for the subsidy. Many people assume because they are offered health insurance by their employer, that it is the best option for them. Don’t make the $550/month mistake I almost made, find out your options for your situation. Applying on the website would be a hassle 1. People were having technical problems applying online to the healthcare websites, which has now been corrected. 2. The insurance premiums do not reflect the tax subsidies and made it difficult to see what was the benefit of the ACA. When you work with one of the 15 insurance brokers they can help bring clarity to your health insurance needs. We were pleasantly surprised at the following benefits of the Affordable Care Act: We weren’t aware of alternative Tele-med health plans designed to lower out of pocket cost 1. Alternative health plans such as Tele-med services that give you access to nurses and doctors 24/7 to address your medical needs. Obviously, if your health needs are severe, you would seek emergency room care. But for symptoms for common ailments like sore throat, rashes, itching, and

other non-emergency ailments, they can be diagnosed by a nurse and doctor and a prescription can be picked up at fortune 500 drugstores. This prevents long wait times at doctors’ offices and interruption in work/school hours to seek medical attention. The cost is also cheaper than what you would pay for a co-pay. Studies say that 80% of emergency room visits are for non-emergency ailments, and they could have been diagnosed and treated by a medical professional over the phone. This Tele-med plan keeps your out of pocket cost down by offering substantial discounts on prescriptions and gives you greater access to medical professionals 24/7 from the comfort of your home. Lower cost Tele-med health plans available for Small Business Owners to offer employees satisfies IRS compliance 1. There are requirements for small businesses, that have 100 or more employees, to offer some type of healthcare plan (traditional health care/Tele-med plans). If they don’t, there can be fines. Some employers are converting employees to part-time and contractors to fall under the limits. A Tele-Med plan can be an option to allow small business owners to offer access to medical care for a significantly lower contribution towards a health insurance plan. Recipe: Veggie Lasagna Ingredients 1 tbsp cooking oil 1 clove garlic, minced 1 medium eggplant, diced 2 10 ounce packages frozen chopped spinach, defrosted 1 64 ounce jar pre-made tomato sauce (Organic)

1 pound lasagna noodles (Gluten free or non) Preparation Heat the oil in a medium frying pan over medium heat. Sautee the garlic for two minutes, then add the eggplant and stir. Cover the frying pan and cook until the eggplant is just tender, about 5 to 7 minutes. Cover the bottom of a 9x12 inch baking pan with 2 cups of the tomato sauce, then cover the tomato sauce with 4 or 5 uncooked noodles. Cover this with a thin layer of sauce, then add the cooked eggplant and another thin layer of sauce. Add another layer of noodles followed by a thing layer of sauce, then the spinach and another thin layer of sauce. Add the remaining noodles and sauce. Cover tightly with foil and bake for 45-50 minutes. Noodles are cooked when they can be pierced with a fork. Makes six servings of eggplant and spinach lasagna. altimore Teens In Transition Program offers a continuum of educational, clinical and therapeutic placements for adolescents with emotional and behavioral challenges. Programs include therapeutic foster care, supervised independent living, preemployment training services, and GED services. Our approach in serving youth includes a two-phase approach in providing services. Phase 1 of our program focuses on placement of a young person in a loving foster home environment with a wealth of support services (transportation, vocational programming, etc). Phase 2 of our program is a celebration of a youth’s progress in your home by transitioning them into independent living. In serving teens who are transitioning into adulthood, it is imperative that we provide guidance, structure and opportunities for them to become self-sufficient. The training that the young people receive with transitioning to independent living include: balancing school and work, continued support with daily living skills, maintaining their own apartment and responsible money management.■

Requirements: -Be at least 21 years old -Have a spare bedroom -Have a High School Diploma/GED Our foster parents receive: -24 /7 Support -Stipend up to $1,400 per month
