Genicular Neurotomy, a Novel

Approach to the Management of Knee Pain

K

nee pain is a common complaint among many patients. It is one of the most common reasons that patients seek medical attention from a physician (general practitioners, orthopedists, rheumatologists, and pain management specialists). The cause of knee pain arises from several factors. These include factors originating from inside the knee joint, those from around the joint, and that referred from nearby structures like the hip, femur bone and tibia and fibula bones. Some of the common causes of knee pain from inside the joint include arthritis, infection, fracture, tumor, chronic dislocation of the patella, joint instability, foreign body, patellofemoral syndrome, and hemarthrosis, which is blood formation in the joint. Causes of knee pain attributable to structures around the knee joint includes bursitis, tendonitis, joint instability, muscle strain, muscle sprain, infection around the joint which does not involve the joint space, and adhesive capsulitis (frozen joint). Knee pain can also be caused from systemic disease and some of the most common cases of these are rheumatoid arthritis, gout, Reiter’s syndrome, collagen vascular disease, among others. Though less common, knee pain can also be caused from other areas of the body. These include lumbar radiculopathy (commonly referred to as sciatica), lumbar spondylosis (degeneration in the spinal discs), fibromyalgia, myofascial pain syndrome, inguinal hernia, nerve entrapment, and pelvic tumors. Knee pain can be acute or chronic. When the pain is acute in the absence of trauma, it warrants seeking medical attention as some conditions associated with acute knee pain can cause serious damage to the joint, such as bleeding into the joint especially in patients on blood thinners. On the other hand, the prevalence of chronic knee pain has increased over the years. Osteoarthritis of the knee is a common disorder among older adults. Chronic osteoarthritis of the knee is often not effectively treated with our current pharmacological and interventional treatments. As an alternative to current treatment options, a new technology has been developed to effectively treat knee pain: Genicular Nerve Radiofrequency Ablation. Genicular Nerve Radiofrequency Ablation is a new cutting edge treatment currently used to treat osteoarthritis of the knee in patients who have not been effectively treated by current treatment management, those who want to delay surgery, and those who have had knee surgery and failed to have relief of their pain. The procedure is performed in two stages. These include a diagnostic Genicular Nerve block and the Genicular Nerve Ablation. The Genicular Nerve block is a test injection that consists of injecting a small amount of local anesthetic to the Genicular Nerves. If the patient has

sufficient response to the test with significant pain relief, then this justifies going forward with the Genicular Nerve Ablation. The nerve radiofrequency ablation is the therapeutic treatment in which a specialized type of radio wave is applied to the nerves to effectively relieve the pain, provide long term relief and restore function to the patient. The radiofrequency ablation works by shocking the nerves that supplies pain to the knee and stop them from sending signal to the brain. While this is a neuro destructive procedure, the nerves do grow back. The same technology has been used for years in treatment of neck and back pain with excellent success. One of the advantage of this procedure is the fact that it is performed outside of the knee joint. This procedure is indicated for a number of conditions including osteoarthritis, degenerative joint disease, chronic knee pain, patients who have undergone total or partial knee surgery, patients who are not candidates for knee replacement, and those who want to avoid surgery totally. Safe & Effective Bio-identical Hormone Replacement

ormones to a large degree determine vitality, youthfulness, how we feel and how we look. Because of this, Bio-Identical Hormone replacement (BHRT) is a hot topic. While hormone replacement is not for everybody, for those who do choose to use them a very big difference can be made in one’s health. And this is an important issue for both men and women. Simply said, hormones are chemical messengers that are created in one part of the body and sent elsewhere to produce an effect. Without hormones, we could not live, could not function, and could not reproduce. Examples of hormones include testosterone, estrogen, progesterone, DHEA, growth hormone, and many others (like insulin!) As we age our hormone levels decline. However, another way of viewing this relationship is that as our hormone levels decline, the aging process is accelerated. Bio-identical hormone replacement is an approach to optimize the natural hormonal status of a man or woman to allow for health and well being in the safest possible way. This is done by using hormones that are chemically and molecularly identical to the hormones that are already in the human body. It is important to draw this distinction because many of the hormones that have been studied and that (legitimately) aroused concern about the safety of hormone therapy have been hormones that are synthetic and do not necessarily belong in the human body and do not promote optimal health. While there are not as many studies done on bio-identical hormone therapy, due to the enormous cost of medical studies that will only be done when the pharmaceutical companies can make huge profits out of synthetic medications, it makes intuitive sense that the hormones that belong in the human body are the ones we were born with! And there are studies that do demonstrate the safety and benefits of bio-identical hormone therapy. So, what are the uses and benefits of BHRT? Treating the symptoms of menopause with safe and effective doses of estrogen, progesterone, and in some testosterone can reduce the hot flashes, night sweats and vaginal dryness often experienced. As well, judicious use of hormones can improve heart health, cognitive function, and bone health. Men with low testosterone levels are at a higher risk of dying from many causes than are men with normal levels of testosterone. As well, low testosterone can cause fatigue,

loss of muscle mass, low libido, elevated cholesterol and can play a role in the development of diabetes and the metabolic syndrome. Optimizing testosterone levels help men experience heightened vitality and well-being. These hormones can be delivered by creams or by subcutaneous pellet insertion. Hormone pellets can provide a sustained delivery of bio identical hormones to achieve optimal health. any women experience problems with sex at some point in their lives. Yet, it is a topic that many people are hesitant or embarrassed to discuss. Pain, lack of desire, lack of arousal, or failure to achieve orgasm are all signs of female sexual dysfunction. Natural changes that occur after childbirth or menopause can play a role. Additionally, illness, medications, life and relationship stress, and poor self-image can prevent women from experiencing satisfaction from sexual activity. Several years ago, the discovery of Viagra for men revolutionized the world of male sexual dysfunction and it was hoped that a similar drug would be found for women. The FDA recently approved Flibanserin (trade name “Addy”), a new female libido drug that aims to boost sexual desire in women. Although it is sometimes referred to as the “female Viagra,” it works very differently. Flibanserin was originally investigated in 1995 by a team of researchers at Boehringer Ingelheim Italia in Milan as an antidepressant. A clinical trial found it did little to alleviate depression, but did seem to have an effect on mood. It just wasn’t the mood the researchers were expecting. These early trials tipped clinicians to flibanserin’s more prominent role in sexual health, as female subjects had higher scores on the Arizona Sexual Experience Scale, a survey that asks participants to rate their satisfaction on a variety of sexual health topics, like how often participants felt sexual desire and how intense that desire was. In later trials, another group of researchers explored flibanserin as a libido-enhancer in 2008. Their subjects reported higher levels of sexual desire. In spite of these results, the FDA rejected the drug twice concluding that the reported increases in libido were not statistically significant. Regulators were wary of po-

HEALTH & WELLNESS tentially dangerous side effects like dizziness, sleepiness, nausea, and fainting. Subsequently, Sprout Pharmaceuticals did additional testing and development with thousands of women reporting significant improvement in sexual desire. Flibanserin took center stage this summer by winning long-sought approval from the U.S. Food and Drug Administration (FDA) in August 2015. So what turned FDA’s red light green? For one: the parameters of the later trials. According to Sheryl Kingsberg, a reproductive biology and psychology researcher at University Hospitals Case Medical Center in Cleveland, Ohio and Clinical Investigator on the flibanserin trials, the evaluation of sexual desire in the brain is not a particularly clear-cut measurement. As the clinical trials progressed over the course of about 8 years, so did techniques in determining desire. In more recent trials, women reported about one more positive sexual experience per month, and about 10% more of the patients who took flibanserin reported meaningful improvements compared with a placebo group. By these measures, FDA approved Addyi as the first drug meant to kindle desire. Though it’s been nicknamed the “female Viagra,” the two drugs function in very different ways—Viagra aims to enhance performance whereas flibanserin (Addyi) aims to balance chemicals in the brain that enhance sexual desire. Addyi is much less effective than Viagra so don’t expect to see a guaranteed response in every woman. Unlike Viagra that men take just when they want to have sex, Addyi is meant to be taken on a daily basis and the effects can take up to 4 weeks to notice, and may improve as time progresses. Addyi is meant only for certain women who have not yet gone through menopause and who have a low desire for sex that is causing them stress. It is not recommend-

ed for women whose low libido is due to medical or psychiatric problems or medication, or problems within the relationship. Addyi cannot be used with alcohol or certain medications. For now, FDA has cautioned users with a black-box warning. In essence, a woman taking Addyi daily would have to give up alcohol. Addyi can cause dizziness, low blood pressure and fainting. Although Addyi is available to the general public, Addyi must be prescribed by a doctor with training and certification for patient safety. Women who experience any type of problem with sex are encouraged to talk to their doctor. A thorough assessment of all possible underlying causes is essential. In most cases, doctors recommend trying other things to improve your relationship and sex life before even trying medicine. For some women, Addyi can prove to be an effective solution for a normal healthy sex life. Most importantly, women who feel healthy and happy are more likely to be happy with their sex life.■

Making Physical Activity a Part of Your Family Life o improve your overall health and fitness, and reduce your risk for many chronic diseases regular physical activity helps. The 2008 Physical Activity Guidelines for Americans are now more flexible than ever, giving individuals the freedom to reach their physical activity goals through different types and amounts of activities each week. The 2008 Physical Activity Guidelines for Americans do not include guidelines for children younger than 6 years old. Physical activity in infants and young children is, of course, necessary for healthy growth and development. Children who are younger than 6 years old should be physically active in ways which are appropriate for their age and stage of development. Physical activity guidelines for children younger than 6 are included in Caring for Our Children: National Health and Safety Performance Standards; Guidelines for Early Care and Education Programs (3rd Ed.)

1. Benefits of Regular Exercise Everyone can benefit from regular exercise. Children who are active will: • have stronger muscles and bones • have a leaner body because exercise helps control body fat • be less likely to become overweight • decrease the risk of developing type 2 diabetes • lower blood pressure and blood cholesterol levels • have a better outlook on life Make sure your child or adolescent is doing three types of physical activity: Aerobic Activity, Muscle Strengthening and Bone Strengthening. To meet the guidelines for aerobic activity, basically anything counts, as long as it’s done at a moderate- or vigorous-intensity for at least 10 minutes at a time for about 60 minutes a day. Many of the physical activities fall under more than one type of activity. For example, if your child is on a basketball team and practices with their teammates daily they are not only doing vigorous-intensity aerobic activity but also bone-strengthening. It’s easy to fit each type of activity into your child’s schedule – all it takes is being familiar with the Guidelines and finding activities that your child enjoys. 2. Stick With It There are many things you can do to get and keep your child active physically, that can help shape their behavior and attitude toward physical activity. Know the guidelines. Choose physical activities

you and your child enjoy and that match your child’s abilities, it will help ensure that you both stick with them. Encourage them to be physically active for one hour or more each day, with activities ranging from informal, active play to organized sports. Here are some examples. • Physical activity should be a part of the family’s daily routine such as taking family walks or playing active games together. • Set a positive example by leading an active lifestyle yourself. Run, swim, jog, or dance. • Be positive about the physical activities in which your child participates and encourage them to be interested in new activities. • Make physical activity fun. Fun activities can be anything which your child enjoys, structured or non- structured sports. Activities such as walking, running, skating, bicycling, swimming, playground activities or free-time play. • After dinner, encourage your child to find fun activities with friends and family, such as walking, playing chase or riding bikes. • Be safe! Always provide protective equipment and ensure that the physical activity is developmentally and age-appropriate. 3. Children with Disabilities Physical activity is important for all children. If your child has a chronic health condition or other symptoms be sure to talk with the doctor about the types and amounts of physical activity that are right for them. s a rite of passage, the bachelorette party has long gotten a bum rap. Stereotypes persist of the bride’s night-before-the-wedding gathering as a ladies-only version of the raucous bachelor party. Yet the truth is, a growing number of brides crave a tasteful, relaxing and deeply personal send-off for their single days. In fact, a relaxing getaway ranked high as an ideal bachelorette party for 59 percent of brides-to-be in the Ultimate

Cervical Cancer

Screening and Prevention

n the past 40 years, there has been a significant decrease in both the number of cases and the number of deaths associated with cervical cancer. In the 21st century, cervical cancer is one of the fewNmalignancies that is now considered a preventableNdisease. This is possible through early detection programs and/or vaccination. Despite all of this, 12,280 women were diagnosed with cervical cancer in 2007 and 4,021 deaths occurred as a result of this disease. Although this still seems like a large figure for a highly preventable cancer, it is far less than what we have seen in the past and is no longer one of the most common cancers in women in the United States. By contrast, there will be an estimated 226,870 cases of breast cancers diagnosed, leading to 39,510 deaths in 2012. Unfortunately, cervical cancer still remains a significant problem in parts of the world that are less economically fortunate. Due to this, worldwide, cervical cancer still remains one of the most common cancers in women accounting for 275,000 deaths in 2008. We have known for many years that there is a link between various degrees of sexual activity and cervical cancer. Recently we have been able to identify that up to 99% of cervical cancers are associated with certain types of the human papilloma virus (HPV). î “is virus is considered a sexually transmitted disease. There are also other factors that can increase your risk for cervical cancer. These include: smoking, HIV infection, extended birth control use as the only means of contraception, and having given birth to over three children. Symptoms of cervical cancer include abnormal vaginal discharge or bleeding. It is widely accepted that cervical cancer screening has successfully decreased the development of cervical cancer and mortality associated with it. Recent advances in the link between HPV and cervical cancer has helped us further this cause. HPV is a sexually transmitted virus to the genital area. There is a strong association between HPV and development of cervical cancer. One study showed the presence of HPV in 93% of tumor specimens. HPV 16 was present in 50% of the specimens, HPV 18 in 14%, HPV 45 in 8%, and HPV 31 in 5%. HPV 16 was the predominant type in all countries except Indonesia, where HPV 18 was more common.

Based on this information, recent research has shown the benefits of HPV vaccinations. However, only certain age groups qualify for this vaccination. Ideally the vaccine should be administered before there is any exposure to genital HPV through sexual intercourse. î “e benefits of this vaccine decrease with increasing numbers of sexual partners. Routine HPV vaccination is recommended for females aged 11 to 12 years. Of note is that girls as young as 9 years of age may start the HPV vaccination series. HPV vaccination is also recommended for females aged 13 to 18 years to catch up on a missed vaccine or to complete the vaccination series. Although it is still safe to receive this vaccine after the age of 26, there is no evidence thus far of any benefit in this age group. Therefore, as it now stands, the HPV vaccine is not routinely recommended after the age of 26. Based on what we know so far there is also no benefit to men receiving this vaccination. These recommendations may change in the future as more research is done.

Based on the advances in cervical cancer screening, review of years of data on this subject, and recent information regarding the role of HPV in cervical cancer, the screening guidelines were recently updated. In March 2012, the American Cancer Society released these new guidelines for cervical cancer screening. The whole idea behind cervical cancer screening is to detect cervical cancer in its earlier stages in patients who do not yet show signs of cervical cancer. In recent years, due to this early detection, physicians have been able to diagnose and treat cervical cancer at much earlier stages thus increasing the potential for cure. At the present time, there are two different types of screening modalities utilized: a pap smear and the HPV test. A pap smear is a method for detecting cells suspicious for turning into cancer or for detecting cervical cancer in its early stages. The HPV test can detect the certain strains of viruses that are known to eventually develop into cervical cancer. The latest screening guidelines from the American Cancer Society now recommend that all women should begin cervical cancer screening at the age of 21. The most drastic change is that yearly pap smear are no longer required, since we now know that it can take years for cervical cancer to develop. More frequent testing can in turn lead to further unnecessary procedures. Women between the ages of 21 and 29 should undergo a pap smear every 3 years. In this age group, the HPV test should be performed only if the pap smear shows any abnormalities. A pap smear along with the HPV test should be performed every 5 years for women between the ages of 30 and 65. In this age group, if the HPV test is not done, then a pap smear should be done every 3 years. If a woman has had normal screening results, she no longer needs screening for cervical cancer after the age of 65. Despite what we know of the benefits of the HPV vaccine for prevention of cervical cancer, the screening recommendations remain the same in a woman who has received this vaccine. This means that a woman who has received this vaccine must still continue to have screening the same as any women who never received such vaccine.

Despite the less frequent testing recommended in these new guidelines, women who are in the high risk category still need to be screened more often. The high risk category includes women who have had exposure to an agent called DES, those who have an HIV infection, those who have had a history of an organ transplant, and those who participate in excessive promiscuous sexual activity with multiple partners. efore intraocular lenses (IOLs) were developed, people had to wear very thick eyeglasses or special contact lenses to be able to see after cataract surgery. Now, with cataract lens replacement, several types of IOL implants are available to help people enjoy improved vision. Discuss these options with Dr. Bovelle to determine the IOL that best suits your vision needs and lifestyle. Cataract lens replacement: How IOLs work Like your eye’s natural lens, an IOL focuses light that comes into your eye through the cornea and pupil onto the retina, the sensitive tissue at the back of the eye that relays images through the optic nerve to the brain. Most IOLs are made of a flexible, foldable material and are about one-third of the size of a dime. Like the lenses of prescription eyeglasses, your IOL will contain the appropriate prescription to give you the best vision possible. Some who choose monofocals decide to have the IOL for one eye set for distance vision, and the other set for near vision, a strategy called “monovision.” The brain adapts and synthesizes the information from both eyes to provide vision at intermediate distances. Often this reduces the need for reading glasses. People who regularly use computers, PDAs or other digital devices may find this especially useful. Individuals considering monovision may be able to try this technique with contact lenses first to see how well they can adapt to monovision. Those who require crisp, detailed vision may decide monovision is not for them. People with appropriate vision prescriptions may find that monovision allows them see well at most distances with little or no need for eyeglasses.

Presbyopia is a condition that affects everyone at some point after age 40, when the eye’s lens becomes less flexible and makes near vision more difficult, especially in low light. Since presbyopia makes it difficult to see near objects clearly, even people without cataracts need reading glasses or an equivalent form of vision correction.

Multifocal or accommodative lenses These newer IOL types reduce or eliminate the need for glasses or contact lenses. • In the multifocal type, a series of focal zones or rings is designed into the IOL. Depending on where incoming light focuses through the zones, the person may be able to see both near and distant objects clearly. • The design of the accommodative lens allows certain eye muscles to move the IOL forward and backward, changing the focus much as it would with a natural lens, allowing near and distance vision. • The ability to read and perform other tasks without glasses varies from person to person but is generally best when multifocal or accommodative IOLs are placed in both eyes. • It usually takes 6 to 12 weeks after surgery on the second eye for the brain to adapt and vision improvement to be complete with either of these IOL types. Considerations with multifocal or accommodative IOLs • For many people, these IOL types reduce but do not eliminate the need for glasses or contact lenses. For example, a person can read without glasses, but the words appear less clear than with glasses. • Each person’s success with these IOLs may depend on the size of his/her pupils and other eye health factors. People with astigmatism can ask Dr Bovelle about toric IOLs and related treatments. • Side effects such as glare or halos around lights, or decreased sharpness of vision (contrast sensitivity) may occur, especially at night or in dim light. Most people adapt to and are not bothered by these effects, but those who frequently drive at night or need to focus on close-up work may be more satisfied with monofocal IOLs. Toric IOL for astigmatism This is a monofocal IOL with astigmatism correction built into the lens. • Astigmatism: This eye condition distorts or blurs the ability to see both near and distant objects. With astigmatism the cornea (the clear front window of

the eye) is not round and smooth (like a basketball), but instead is curved like a football. People with significant degrees of astigmatism are usually most satisfied with toric IOLs. People who want to reduce (or possibly eliminate) the need for eyeglasses may opt for an additional treatment called limbal relaxing incisions, which may be done at the same time as cataract surgery or separately. These small incisions allow the cornea’s shape to be rounder or more symmetrical.

Other important cataract lens replacement considerations • In some cases, after healing completely from the cataract lens surgery, some people may need further correction to achieve the best vision possible. Their ophthalmologist may recommend additional surgery to exchange an IOL for another type, implant an additional IOL, or make limbal relaxing incisions in the cornea. Other laser refractive surgery may be recommended in some cases. • People who have had refractive surgery such as LASIK need to be carefully evaluated before getting IOLs because the ability to calculate the correct IOL prescription may be affected by the previous refractive surgery. • People with Dry Eye need to be evaluated before cataract surgery. Untreated Dry Eye can affect the calculations needed to choose the specific IOL for you. Hypothyroidism K nown as the master gland, the butterfly-shaped thyroid sits in a notch just below your throat. Its hormones affect many systems and functions, including: • Metabolism • Brain development • Breathing, heart and nervous system functions • Blood cells production • Muscle and bone strength • Body temperature • Menstrual cycle • Weight gain and loss • Cholesterol levels • Skin hydration So it makes sense that thyroid imbalances can make your life seem entirely off-kilter. The most common form of thyroid imbalances is hypothyroidism. It occurs when your thyroid is not producing enough thyroid hormone to support your daily activities. Thyroid-related issues can arise at any age and may not necessarily show up on routine lab tests. Subclinical hypothyroidism is a term used when someone is experiencing symptoms of hypothyroidism but whose blood test results are still in the “normal range” for thyroid hormone production. The signs and symptoms of hypothyroidism vary widely, depending on the severity of the hormone deficiency. But in general, any problems you do have tend to develop slowly, often over a number of years. Childhood Obesity

A

ccording to the Journal of the American Medical Association and the National Center for Health Statistics, childhood obesity has more than doubled in children and quadrupled in adolescents in the past 30 years. Defined by the National Institutes of Health overweight is defined as having excess body weight for a particular height from fat, muscle, bone, water, or a combination of these factors. Obesity is defined as having excess body fat. Overweight and obesity are the result of “caloric imbalance”—too few calories expended for the amount of calories consumed—and are affected by various genetic, behavioral, and environmental factors. Childhood obesity has both immediate and long-term effects on health and well-being. Some of the immediate effects include cardiovascular issues that may develop. Obese youth are more likely to have risk factors for cardiovascular disease, such as high cholesterol or high blood pressure. Overweight children also experience instability in glucose levels which often result to pre-diabetes, a condition in which blood glucose levels indicate a high risk for development of diabetes diabetic conditions. According to the U.S. Department of Health and Human Services, children and adolescents who are obese are at greater risk for bone and joint problems, sleep apnea, and social and psychological problems such as stigmatization and poor self-esteem. Some long term affects include; heart disease, type 2 diabetes, stroke, several types of cancer, and osteoarthritis. Overweight and obesity are associated with increased risk for many types of cancer, including cancer of the breast, colon, endometrium, esophagus, kidney, pancreas, gall bladder, thyroid, ovary, cervix, and prostate, as well as multiple myeloma and Hodgkin’s lymphoma states the Cancer Journal for Clinicians. The Journal of Pediatrics published that Healthy lifestyle habits, including healthy eating and physical activity, can lower the risk of becoming obese and developing related diseases. The dietary and physical activity behaviors of children and adolescents are influenced by many sectors of society, including families, communities, schools, child care settings, medical care providers, faith-based institutions, government agencies, the media, and the food and beverage industries and entertainment industries. Schools play a particularly critical role by establishing a safe and supportive environment with policies and practices that support healthy behaviors. Schools also provide opportuni-

This new paradigm says that the patient’s total health status is now the focus, as evidenced by new policies issued by insurers, Medicare and state governments.

Quality healthcare is now viewed to be based on a longterm relationship between patient and physician, about both long term health efficiencies and acute issues. Remote encounters are usually more efficient and convenient for both patient and physician. Medicare and states are holding hospitals responsible for readmission rates, which requires the doctor and the patient to have an ongoing relationship, and telemedicine facilitates that relationship. Organizations operating Telemedicine programs will need to address questions like shared responsibility for securing and managing the health information generated through a Telemedicine encounter – including responsibilities Privacy and Security considerations related to data breach notification and reporting—to confirm compliance with privacy and security requirements. Telemedicine transmissions also may be vulnerable to interference, signal errors or transmission outages that can result in interrupted communications and the alteration or loss of important clinical information which would potentially be a HIPAA violation. The additional risks of Telemedicine programs require a health care organization’s privacy and security professionals to participate from the start in the design and implementation of Telemedicine programs, and assume responsibility for actively monitoring operations. Telemedicine technology brings new privacy and security issues that could raise concerns about protection of privacy, confidentiality, and security of sensitive patient information.

■■■■They are: A lack of uniform confidentiality and privacy legislation at the state level in terms of the transfer of health information in telemedicine encounters. Telemedicine consultations can take place over state lines, the potential for confusion over which state’s standards should be employed could arise. A long-distance telemedicine consultation typically involves a clinician-patient session that can be videotaped (most are) in its entirety, and this record is maintained as part of the documentation of the consultation. As a result, practitioners have less discretion to remove sensitive items that they might otherwise not record. From the patient perspective, the patient may not be able to “see” who else is viewing the session along with the clinician on the other side of the long distance consultation. The use of telemedicine equipment usually adds additional personnel to the typical provider-patient encounter. For instance, a technical outsider, like an engineer, may be privy to the consultation. From a technical standpoint, there is a higher volume of data and complexity involved in the various communication mediums used during a typical telemedicine consultation. That could make securing the data more problematic. It is now more important and necessary to conduct regular HIPAA/HITECH Risk Assessments under the fast evolving telemedicine landscape. ■■■■Conclusion: However, in this day of ever increasing data breaches, it is imperative that both physicians and patients insist that their telemedicine organization at least meets the minimum standards mandated by HIPPA Privacy and Security Rule. In addition to the importance of HIPPA compliant telemedicine providers, it is just as important that the organization’ s Telemedicine technology companies and business associates be HIPAA compliant. Privacy and Security concerns notwithstanding, Telemedicine enables outcome-based medicine: doctors can maintain the conversation with the patient in a manner that is far more efficient and effective than traditional encounters; they can serve more patients well. It is not always easy for a person to discuss these matters with adult children. While communicating with one’s children ahead of time and expressing wishes about distribution of assets may be the ideal way for an individual to handle the situation, realistically families don’t always follow this practice. Far too often after a parent passes away, the children end up challenging the parent’s decisions. An experienced and knowledgeable estate planning and trust attorney can help an individual prepare for a discussion of theirintentions with their children first before the trust agreement is drafted. A trust challenge can be a long and costly process delaying the distribution of assets to the beneficiaries and legal costs can ultimately reduce the value of the estate. thus, it is a smart planning strategy for the individual to try and resolve any anticipated disputes prior to the creation of the trust. The chances of a beneficiary or heir challenging the trust after the decedent’s death can be reduced when all parties to the trust are aware of the parent’s intentions. Of course, there may still be situations that arise after a parent’s death where family members may not agree on all financial decisions made by the trustee. A typical example of this is when the deceased parent’s house is the major asset of the estate, and it needs to be sold in order to distribute the sale proceeds to the children. Since every person’s financial situation is unique, the attorney will be able to guide the individual in choosing the best suited person or entity to act as trustee. From a financial and practical sense, the best way for a parent to avoid major conflicts between their beneficiaries after their death is for the elderly parent to consult with an experienced estate and trust attorney who can help them sit down with their children and discuss their estate planning choices. s a funeral care provider I am often asked the question “What made you choose this profession.” Which is usually followed up by the statement “I could never do what you do.” I smile on the inside and quietly thank God for the gift of science which He has innately place on the inside of me. My answer to the question is always the same, I didn’t choose it, it chose me. And yes I absolutely love it! The reward of my gift is educating the community up close and personal. There are many options related to end of life decisions. I believe education relieves apprehension which allows you to “Live from a Position of Power” (Pamela Reaves). Several times a year, I am honored to speak to brave people who wish to be empowered and take control of their lives, even to the very end. Lately, the concern has centered on LIFE INSURANCE. Life insurance can be pretty confusing given the number of misleading TV and radio commercials vying for your attention. Please allow me to clarify some of your concerns and arm you with the facts related to funding funerals with life insurance products. The goal of life insurance is to ensure you and your families are protected against financial devastation in the unfortunate event of death. There are a plethora of insurance products on the market. Some of the most popular are: • • •

Whole Life / Permanent (Traditional, Universal, Variable) – Pays a death benefit when you die- even if you live to 100. Term Life – The simplest form of insurance which only pays if death occurs during the term of the policy, which is usually one to 30 years. Final Expense- Is usually a Whole Life product specifically designed to cover funeral cost.

Although any coverage you might have is good, out of the three listed Term Life insurance may not be your best option for end of life coverage. Here is why: We’ve all heard the commercials where the announcer is boasting their ability to offer a 45 year old male, with high blood pressure a 20 year, $50,000., TERM life policy for under $40.00 a month. Sounds good right? Well, at the end

BUSINESS of the term, the male will be 65 years of age. The problem: the older you are the more you’ll pay for insurance because you become a high risk factor. That same $50,000.00 term policy, upon renewal, might now cost you four or five times the original amount IF you’ve managed to remain healthy. Remember, high blood pressure made you a high risk from the start. The pro is you have coverage for a period of time. The con is with age health is likely to decline which makes being able to qualify or afford coverage a huge problem. Whole Life Insurance is great and used by many to amass great wealth. Others plan for it to be used as an inheritance. If you are planning to fund your funeral with it please know, traditional life insurance policies can take as long as 6 to 8 weeks to pay out. Most funeral homes will transfer the policy into a “FUNERAL ASSIGNMENT”. A funeral assignment allows you the benefit of a FINAL EXPENSE policy however, be prepared to incur a fee of up to 6% of the total funeral contract. Fees are paid to third parties, on your behalf, for speedy verifications and quick pay out. Final Expense Insurance policies are designed to fund funerals. Once the benefactor and the amount of the policy is verified your funeral cost can be paid out to the funeral provider within 24/48 hours. You should not be charged a fee for using a Final Expense policy that has been assigned directly to the funeral care provider from your insurance agency. Final Expense policies are not intended to leave inheritance funds. Please be sure to adjust your life insurance policy should you have a major life change, i.e., marriage, divorce, new birth. Failing to do so might prove to be costly. Finally, to the person who think to themselves I DON”T CARE what happens to me after I’ve passed. They can just put me in a pine box. for Newly Married Couples

n most parts of the country, the months of June, August, September and October are the most popular for weddings. Whether recently married or anxiously counting the days until you tie the knot, there are some important tax implications that you and your spouse can’t afford to overlook. “In between the flurry of wedding planning, dress shopping and bridal showers, few engaged couples stop to think about how getting married will affect their income taxes,” explained TaxAct spokesperson, Shaunna Morgan. “While it’s important to understand the tax consequences, keep in mind that you can still do your own taxes. TaxAct, a leading DIY tax solution, asks simple questions about life events like marriage to guide you through the tax implications and help you get all of your deductions to maximize your refund.” Name Changes Your name(s) and Social Security number(s) are critical elements of your tax return. Both identifiers must exactly match the information the Social Security Administration (SSA) has on file. That means if you legally change your name after you get married, the SSA needs to know about it. To get a Social Security card with your new name, you’ll need to provide proof of identity, such as a marriage certificate, fill out an application and either mail it or deliver it in person to your local SSA office. You’ll want to do this long before you file your income tax return.

New Address If you’re moving, remember to report your new address to the Internal Revenue Service (IRS). The fastest way to make the change is to complete Form 8822 available on the agency website at www.irs.gov. New Filing Status Once you’ve said, “I do,” you’ll have the option to claim one of two filing statuses on your tax return: married filing jointly or married filing separately. If you and your spouse each earn an income, you might consider running the numbers to determine if you have a lower combined tax by filing one way versus the other. Some tax software packages do the calculations for you so it’s easier to make an informed decision when selecting a filing status. TaxAct Deluxe, for example, gives users a Joint vs. Separate Analysis report that does all the calculations so you can see your estimated refund amounts for each filing status. For many couples, married filing jointly results in lower income tax liability. Keep in mind that no matter when in the year you get married, you are considered married for the full year. Even if your big day is December 31, you will have a married status in the eyes of the IRS for the entirety of that tax year. Changing Your Tax Withholding Any time you have a life change you should revisit your Form W-4. Getting married is definitely one of those times. If both you and your spouse work, you’ll likely have a higher combined income, which may put you into a higher tax bracket. In order to ensure the right amount of tax is withheld from your paycheck, you’ll want to update your Form W-4 to ensure you aren’t having too much or too little tax withheld. Affordable Care Act Premium Tax Credit If you or your spouse receive health insurance through a government-sponsored marketplace and receive advance payments of the premium tax credit to help offset the cost of your premiums, you’ll want to review your coverage after the wedding, and notify the marketplace of your newly married status. Reporting this change will help you avoid having too much or too little premium assistance paid and ultimately, help you steer clear of owing additional money or getting a small refund when you file your taxes. If you elect to keep your coverage “as is,” separate from your spouse, or wish to add your spouse to your coverage, you should notify the marketplace of your special changes in coverage. If you elect to go under your spouse’s health plan, be sure to cancel your marketplace coverage. If you’re an expecting or breastfeeding mom, it’s even more important to eat fish because it’s a vital staple for optimal brain and eye development in babies, as well as brain and heart health for moms. Eating two to three servings of fish each week has also been shown to have a positive effect on developing babies’ and children’s IQs. Unfortunately, many mothers avoid eating fish based on fears associated with misinformation. “Years ago, there were concerns that trace amounts of mercury in fish could be harmful, but, actually, hundreds of peer-reviewed studies show the health benefits of eating fish are real and the powerful nutrients in fish outweigh any potential concerns,” says Jennifer McGuire, MS, RD. “In fact, at a time when people are told to limit many foods, including popular proteins, seafood is among the handful of foods Americans are encouraged to eat more of for their health. Seafood is a satisfying and affordable source of protein that is easy to incorporate into meals twice a week.” According to the 2015-2020 Dietary Guidelines for Americans, healthy eating patterns should include at least 8 ounces - or two servings - per week of a variety of seafood because it is an important part of eating patterns that contribute to a healthy heart and weight. Seafood also provides powerful nutrients including brain-building omega-3 fatty acids, protein, B vitamins, vitamin D, iron and selenium. In addition, eating seafood during pregnancy contributes to better health for babies. Salad w/Crumbled Feta Ingredients

1/2 cup farro, uncooked

1 (5-oz.) can tuna, drained 1 cup canned chickpeas, drained and rinsed 1/2 large bulb fennel, cut in half, cored and sliced very thin 2 generous handfuls baby arugula 2 tablespoon extra-virgin olive oil 2 tablespoon fresh lemon juice Zest of 1/2 lemon 1/4 teaspoon ground cumin 1/3 cup crumbled feta cheese Salt and pepper, to taste

Preparation 1. Cook farro according to package directions. Drain well if any cooking water remains. 2. Transfer farro to a large bowl. Add tuna (flake with a fork), chickpeas, fennel, arugula, olive oil, lemon juice, lemon zest, cumin and salt. Toss well. 3. Spoon into a shallow serving bowl or small platter and sprinkle feta over the top. That’s it! A quick, easy recipe that serves as a lunch or a light supper for four or as a side dish for six with just 310 calories and 12 grams of fat. Sweet Potato Loaf Ingredients 2 cups gluten-free all purpose flour 2½ tsp. organic cinnamon 1 tsp. baking soda ¼ tsp. baking powder ¼ tsp. sea salt 1¾ cups coconut sugar ¾ cup vegetable oil 3 large eggs *( golden flax seed can be used as an egg replacer) 1 tsp. almond extract 3 cups sweet potatoes, cooked, peeled and grated (about 8 sweet potatoes) Preparation 1. Preheat oven to 350 degrees F. 2. Spray loaf pan with nonstick baking spray. 3. In a large bowl, combine flour, cinnamon, baking soda, baking powder and sea salt. 4. In a separate large bowl, beat coconut sugar, oil, eggs and almond extract. Add sweet potato; mix well. Combine with dry ingredients and mix well. 5. Transfer batter into baking pan. Bake for 1 hour. 6. Remove from oven, cool for 15 minutes, serve and enjoy.■ February - March 2016

1 can chilled, full fat coconut milk 2 tbs Chatfield’s carob powder 2 tbs organic agave nectar 1/4 teaspoon vanilla extract raspberries or strawberries on top

Preparation 1. Chill the can of coconut milk overnight (at least 8 hours) to cause the cream and liquid to separate. 2. Remove can of coconut milk from refrigerator, turn upside down, and pour off the clear liquid at the top. 3. Scoop the cream into a bowl and whisk till it turns to a whip. 4. Blend the coconut cream, cocoa powder, vanilla extract and agave nectar until completely smooth. Refrigerator 1 hour. Top with raspberries or strawberries and delight.■

Are you ready to “take charge” of your money? Here are some tips to help you get started. Cheers to the New Year and to your new financial goals! Understand Where You Are Now - Find your starting point. Take stock of your financial situation, which entails identifying of all your income resources and outstanding debt. At the end of the month, look at where you are spending your money. Look at cash, checks, and automatic transfers from your account. Tracking your spending helps you locate spending “leaks” – areas where money is leaking from your pocket – so you can plug them. Create Your Spending Plan - You know where you are, where you want to go, and where your money is going. Now it’s time to create your spending plan (or adjust your old one) and take charge of your money. Making a spending plan is easy. Just subtract all of your expenses from your income. Make Saving a Habit for Life - Make saving a priority rather than an afterthought. As you take charge of your money, saving money should be considered an expense, just like other bills.
