BACK TO SCHOOL TIPS FOR PARENTS

Helping Children to Achieve & be Their Best at Home, at School, & at Life

E

Back-to-School Transitions nsuring a new school year is getting off to a good start can influence children’s attitude, confidence, and performance both socially and academically. This transition from August to September can be difficult for both children and parents. Even those children who are eager to return to class must adjust to the greater levels of activity, structure, and, for some, pressures associated with school life. The level and degree of adjustment depend on the child, but parents can help their children (and the rest of the family) manage the increased pace of life by planning ahead, being realistic in expectations, and maintaining a positive attitude. Here are just a few suggestions to help with the transition and promote a successful school experience. The Balance of School and Family Life Plan to schedule doctor and dental checkups early. Discuss any concerns about your child’s emotional or psychological development with your pediatrician. Your doctor can help determine if your concerns are normal, age-appropriate issues or require further assessment. Your child will benefit if you can identify and begin addressing a potential issue before school starts. Review the material sent by the school as soon as it arrives. These packets include important information about your child’s progress at school. Make a note of important dates, PTA meeting and parent/teacher conferences. This is especially important if you have children in more than one school and need to juggle obligations. Keep up with school supplies. Consider buying extra backpacks and lunch boxes in the event that one breaks. Label items such as sweaters, coats, and thermos. Continue to maintain bedtime and mealtime routines (especially breakfast). Breakfast will give them the mental and physical stamina to function at their best. Turn off the TV and be mindful not add too many extracurricular activities as the school year builds momentum. Minimize clothes shopping woes. Buy only the essentials. Check with your school to confirm dress code guidelines. Designate and clear a place to do homework. Select a spot to keep backpacks and lunch boxes. Freeze a few easy dinners. It will be much easier on you if you have dinner prepared so that meal preparation will not add to household tensions during the school week.

Stay Engaged in Your Child’s School Life Clear your own schedule. If possible, postpone business trips, volunteer meetings, and extra projects. Consider how you can become involved in the school community. Ask your child what subject or activity they are really enjoying. These questions may spark ideas of how to get involved. Make lunches the night before school. Ensure healthy choices are made; set alarm clocks. Leave plenty of extra time. Make sure your child has plenty of time to get up, eat breakfast, and get to school. Review your child’s schoolbooks. Send a note, letting the teachers know you want to receive regular feedback on how and what your child is doing in school. Overcoming Anxiety: Let your children know you care. If your child is anxious about school, send personal notes in their lunch box or book bag, this will help to reinforce the ability to cope. Model both optimism and confidence for your child. Do not overreact to situations and remain calm and positive. Reinforce your child’s ability to cope. Extracurricular Activities: Look for activities that will give quality, not quantity. Your child will benefit most from one or two activities that are fun, reinforce social development, and teach new skills. Too much scheduled time can be stressful. Also, consider your family schedule and personal energy level. When Problems Arise: Most children can display a variety of behaviors. They can or cannot contribute to a positive and or productive school experience for the child; it is generally wise not to over-interpret those behaviors. More often than not, time and a few intervention strategies will remedy the problem. Most children are wonderfully resilient and, with your support and encouragement, will thrive throughout their school experience. If the problems seem extreme in nature or go on for an extended period, you may want to contact the school to set up an appointment to meet with your child’s teachers and school psychologist. Less Wrinkles otox® is a highly purified, naturally occurring protein that has the ability to very precisely relax the muscles to which it is applied. In the 70’s ophthalmologists began injecting it into tiny eye muscles to treat “lazy eye”. In the 80’s, the drug was used to correct other facial, eyelid and limb diseases such as tremors and spasms. Get Rid of Baggage Upper eyelid blepharoplasty is one of the most commonly performed surgeries. It can be cosmetic or functional. Cosmetic blepharoplasty can give you back your original youthful appearance and reduce heaviness. Functional blepharoplasty may be indicated when heavy eyelid skin hangs over your visual field, blocking the side or overhead visual field. This condition, sometimes referred to as dermatochalasis, is a common change with aging. The need for functional surgery is documented with photography and visual field testing with and without manually lifting the eyelids with tape. Lower lid blepharoplasty can remove the “bags” under the eyes. It is also used if the lower lid hangs down or is

pulled away from the eye. Functional causes are indicated if the lower lid is not in contact with the eyeball or if the lower lid turns inward intermittently and the eye lashes rub against the eye. BLepharoplasty surgery can be done as an outpatient procedure, taking approximately one hour, using gentle sedation and local anaesthetic. It does not require general anaesthesia. Both types of functional blepharoplasty may be covered by insurance. A thorough exam by an ophthalmologist can determine this. five steps to Keep your Child Safe at School

or parents of children living with diabetes, back to school season includes more than the usual shopping trips for supplies and clothes. Diabetes management is a 24-hour job. During the school year, your child is spending a large portion of their day in class or participating in extracurricular activities. So it’s critical that you feel confident that school staff members are trained and able to support your child’s diabetes care needs. Most of all, you want your child to feel safe while also having access to the same opportunities as their peers. You are not alone. There are nearly 210,000 children living with diabetes in the United States, according to the American Diabetes Association. Here are a few important steps that parents of children with diabetes can take to make sure their kids are safe at school: 1. Know your rights. Get the facts about legal protections at school for your child. Federal laws protect children with diabetes from discrimination. Also, many states have laws in place that include additional safeguards to ensure your child has what he or she needs to be safe and healthy. Discrimination can come in many forms such as not providing your child with access to diabetes care so he or she may safely participate in extracurricular activities or field trips, and not allowing self-management during the school day. Learn more about your state’s laws and whether they offer protections and guidance in addition to federal law at diabetes.org/kidswin. More information about existing legal protections is also available at diabetes.org/safeatschool. 2. Have a written plan. Do you have a Diabetes Medical Management Plan in place for your child? If not, this is a perfect time to ask your child’s health care provider to develop one. Even if you already have a plan, it needs to be reviewed and updated so the school has the correct information. 3. You should also update or develop your child’s Section 504 Plan, which is an accommodations plan developed for students eligible for protections under Section 504, a federal law that prohibits discrimination against people with disabilities. This agreement between the parent/guardian and the school spells out what the school is specifically going to do to make sure your child’s diabetes is safely managed. The plan also identifies who is going to help your child when the school nurse is unavailable. 4. Students who qualify for services under another federal law - the Individuals with Disabilities Education Act - will need to develop an Individualized Education Program (IEP). Both of these plans are developed by a coordinator at your child’s school to create clear lines of communication around individual needs and support for your child, from self-management requirements to

snacks and supplies. You should contact your school’s 504/IEP coordinator, who is often a school principal, guidance counselor or teacher, to initiate the process or to update your child’s existing plan. 5. Get to know the school nurse or health coordinator. The school nurse or school health coordinator is usually the primary provider of diabetes care for your child. Be sure your child is comfortable with the school nurse and that you have a good line of communication with the diabetes care team at your child’s school. Provide the necessary supplies, including insulin, glucagon, a “low box” (with glucose tablets or juice boxes) and food, and make sure these supplies are in an accessible location. 6. Make sure staff receives diabetes training. While the school nurse should always be the main point of contact for your child’s diabetes care during the school day, there will undoubtedly be times when the nurse is not immediately available or not on the premises. It is very important that additional school staff is trained by a school nurse or other qualified health professional with expertise in pediatric diabetes to provide assistance. These staff members should understand the daily needs of a child with diabetes, know how to identify a diabetes emergency, administer insulin and emergency glucagon, and know when to seek emergency medical assistance. 7. Advocate for self-management. If your child is able and your child’s health care provider approves, your child should be permitted to monitor glucose levels, administer insulin, and carry the necessary supplies needed to self-manage. You know your child’s self-management abilities best, so coordinate with your school administrator to make sure your child is as comfortable as possible throughout the day. Follow these tips for a smooth back-to-school transition where you can feel at ease, while your child has the freedom to learn and participate alongside his or her peers. If you don’t feel comfortable and confident in the care your child is receiving at school, if you feel your child is being treated unfairly or if you want help with developing a written plan, contact the American Diabetes Association at 1-800-DIABETES for information and guidance from the Association’s legal advocates.■ August - September 2016

What are the Symptoms of ADHD in Children?

nattention, hyperactivity, and impulsivity are the key behaviors of ADHD. It is normal for all children to be inattentive, hyperactive, or impulsive sometimes, but for children with ADHD, these behaviors are more severe and occur more often. To be diagnosed with the disorder, a child must have symptoms for 6 or more months and to a degree that is greater than other children of the same age. Children who have symptoms of inattention may: • Be easily distracted, miss details, forget things, and frequently switch from one activity to another • Have difficulty focusing on one thing • Become bored with a task after only a few minutes, unless they are doing something enjoyable • Have difficulty focusing attention on organizing and completing a task or learning something new • Have trouble completing or turning in homework assignments, often losing things (e.g., pencils, toys, assignments) needed to complete tasks or activities • Not seem to listen when spoken to • Daydream, become easily confused, and move slowly • Have difficulty processing information as quickly and accurately as others • Struggle to follow instructions. Children who have symptoms of hyperactivity may: • Fidget and squirm in their seats • Talk nonstop • Dash around, touching or playing with anything and everything in sight • Have trouble sitting still during dinner, school, and story time • Be constantly in motion • Have difficulty doing quiet tasks or activities. Children who have symptoms of impulsivity may: • Be very impatient • Blurt out inappropriate comments, show their emotions without restraint, and act without regard for consequences • Have difficulty waiting for things they want or waiting their turns in games • Often interrupt conversations or others’ activities.

ADHD Can Be Mistaken for Other Problems - Parents and teachers can miss the fact that children with symptoms of inattention have ADHD because they are often quiet and less likely to act out. They may sit quietly, seeming to work, but they are often not paying attention to what they are doing. They may get along well with other children, whereas children who have more symptoms of hyperactivity or impulsivity tend to have social problems. But children with the inattentive kind of ADHD are not the only ones whose disorders can be missed. For example, adults may think that children with the hyperactive and impulsive symptoms just have disciplinary problems. What Causes ADHD? Scientists are not sure what causes ADHD, although many studies suggest that genes play a large role. ‘Female Viagra’ work? By Dr. Okasi

any women experience problems with sex at some point in their lives. Yet, it is a topic that many people are hesitant or embarrassed to discuss. Pain, lack of desire, lack of arousal, or failure to achieve orgasm are all signs of female sexual dysfunction. Natural changes that occur after childbirth or menopause can play a role. Additionally, illness, medications, life and relationship stress, and poor self-image can

Several years ago, the discovery of Viagra for men revolutionized the world of male sexual dysfunction and it was hoped that a similar drug would be found for women. The FDA recently approved Flibanserin (trade name “Addy”), a new female libido drug that aims to boost sexual desire in women. Although it is sometimes referred to as the “female Viagra,” it works very differently.

Flibanserin was originally investigated in 1995 by a team of researchers at Boehringer Ingelheim Italia in Milan as an antidepressant. A clinical trial found it did little to alleviate depression, but did seem to have an effect on mood. It just wasn’t the mood the researchers were expecting. These early trials tipped clinicians to flibanserin’s more prominent role in sexual health, as female subjects had higher scores on the Arizona Sexual Experience Scale, a survey that asks


participants to rate their satisfaction on a variety of sexual health topics, like how often participants felt sexual desire and how intense that desire was. In later trials, another group of researchers explored flibanserin as a libido-enhancer in 2008. Their subjects reported higher levels of sexual desire. In spite of these results, the FDA rejected the drug twice concluding that the reported increases in libido were not statistically significant. Regulators were wary of potentially dangerous side effects like dizziness, sleepiness, nausea, and fainting. Subsequently, Sprout Pharmaceuticals did additional testing and development with thousands of women reporting significant improvement in sexual desire. Flibanserin took center stage this summer by winning long-sought approval from the U.S. Food and Drug Administration (FDA) in August 2015. So what turned FDA’s red light green? For one: the parameters of the later trials. According to Sheryl Kingsberg, a reproductive biology and psychology researcher at University Hospitals Case Medical Center in Cleveland, Ohio and Clinical Investigator on the flibanserin trials, the evaluation of sexual desire in the brain is not a particularly clear-cut measurement. As the clinical trials progressed over the course of about 8 years, so did techniques in determining desire. In more recent trials, women reported about one more positive sexual experience per month, and about 10% more of the patients who took flibanserin reported meaningful improvements compared with a placebo group. By these measures, FDA approved Addyi as the first drug meant to kindle desire. Though it’s been nicknamed the “female Viagra,” the two drugs function in very different ways—Viagra aims to enhance performance whereas flibanserin (Addyi) aims to balance chemicals in the brain that enhance

Addyi is much less effective than Viagra so don’t expect to see a guaranteed response in every woman. Unlike Viagra that men take just when they want to have sex, Addyi is meant to be taken on a daily basis and the effects can take up to 4 weeks to notice, and may improve as time progresses. Addyi is meant only

HEALTH & WELLNESS for certain women who have not yet gone through menopause and who have a low desire for sex that is causing them stress. It is not recommended for women whose low libido is due to medical or psychiatric problems or medication, or problems within the relationship. Addyi cannot be used with alcohol or certain medications. For now, FDA has cautioned users with a black-box warning. In essence, a woman taking Addyi daily would have to give up alcohol. Addyi can cause dizziness, low blood pressure and fainting. Although Addyi is available to the general public, Addyi must be prescribed by a doctor with training and certification for patient safety. Women who experience any type of problem with sex are encouraged to talk to their doctor. A thorough assessment of all possible underlying causes is essential. In most cases, doctors recommend trying other things to improve your relationship and sex life before even trying medicine. For some women, Addyi can prove to be an effective solution for a normal healthy sex life. Most importantly, women who feel healthy and happy are more likely to be happy with their sex life.■

Could Vitamin D Deficiency Hasten Cognitive Decline?

V

itamin D insufficiency is associated with greater declines in cognitive ability and episodic memory in older adults, according to a recent study. In order to further examine the association between vitamin D status and cognitive function, researchers conducted a longitudinal multiethnic cohort study of 382 participants between 2002 and 2010. Participants’ vitamin D serum levels were measured at baseline, and at yearly follow-up visits Note: Vitamin D deficiency was defined as serum level less than 12 ng/mL, insufficient as 12 ng/mL to less than 20 ng/mL, adequate as 20 ng/mL to less than 50 ng/mL, and high as 50 ng/mL or higher. At enrollment, 17.5% of the participants had dementia, 32.7% had mild cognitive impairment, and 49.5% had no cognitive impairment. Overall, average levels of serum vitamin D were lower in the dementia group compared with those of individuals with mild cognitive impairment and healthy individuals. Genicular Neurotomy, a Novel

Approach to the Management of Knee Pain

K

nee pain is a common complaint among many patients. It is one of the most common reasons that patients seek medical attention from a physician (general practitioners, orthopedists, rheumatologists, and pain management specialists). The cause of knee pain arises from several factors. These include factors originating from inside the knee joint, those from around the joint, and that referred from nearby structures like the hip, femur bone and tibia and fibula bones. Some of the common causes of knee pain from inside the joint include arthritis, infection, fracture, tumor, chronic dislocation of the patella, joint instability, foreign body, patellofemoral syndrome, and hemarthrosis, which is blood formation in the joint. Causes of knee pain attributable to structures around the knee joint includes bursitis, tendonitis, joint instability, muscle strain, muscle sprain, infection around the joint which does not involve the joint space, and adhesive capsulitis (frozen joint). Knee pain can also be caused from systemic disease and some of the most common cases of these are rheumatoid arthritis, gout, Reiter’s syndrome, collagen vascular disease, among others. Though less common, knee pain can also be caused from other areas of the body. These include lumbar radiculopathy (commonly referred to as sciatica), lumbar spondylosis (degeneration in the spinal discs), fibromyalgia, myofascial pain syndrome, inguinal hernia, nerve entrapment, and pelvic tumors. Knee pain can be acute or chronic. When the pain is acute in the absence of trauma, it warrants seeking medical attention as some conditions associated with acute knee pain can cause serious damage to the joint, such as bleeding into the joint especially in patients on blood thinners. On the other hand, the prevalence of chronic knee pain has increased over the years. Osteoarthritis of the knee is a common disorder among older adults. Chronic osteoarthritis of the knee is often not effectively treated with our current pharmacological and interventional treatments. As an alternative to current treatment options, a new technology has been developed to effectively treat knee pain: Genicular Nerve Radiofrequency Ablation. Genicular Nerve Radiofrequency Ablation is a new cutting edge treatment currently used to treat osteoarthritis of the knee in patients who have not been effectively treated by current treatment management, those who want to delay surgery, and those who have had knee surgery and failed to have relief of their pain. The procedure is performed in two stages. These include a diagnostic Genicular Nerve block and the Genicular Nerve Ablation. The Genicular Nerve block is a test injection that consists of injecting a small amount of local anesthetic to the Genicular Nerves. If the patient has

sufficient response to the test with significant pain relief, then this justifies going forward with the Genicular Nerve Ablation. The nerve radiofrequency ablation is the therapeutic treatment in which a specialized type of radio wave is applied to the nerves to effectively relieve the pain, provide long term relief and restore function to the patient. The radiofrequency ablation works by shocking the nerves that supplies pain to the knee and stop them from sending signal to the brain. While this is a neuro destructive procedure, the nerves do grow back. The same technology has been used for years in treatment of neck and back pain with excellent success. One of the advantage of this procedure is the fact that it is performed outside of the knee joint. This procedure is indicated for a number of conditions including osteoarthritis, degenerative joint disease, chronic knee pain, patients who have undergone total or partial knee surgery, patients who are not candidates for knee replacement, and those who want to avoid surgery totally. Meanwhile, some traditional watchmakers - such as Tag Heuer - have developed their own high-tech timepieces. Top designers are also collaborating with high-tech developers to create smart rings, bracelets, necklaces and other fine and fashion jewelry. FitBit, for instance, offers a line of fitness trackers conceived by Tory Burch, a well-known women’s wear designer. The technology is embedded in embossed metallic pendants and bracelets. Protect smart investments Since smart jewelry could be a major jewelry investment - second only, perhaps, to an engagement ring - it’s important to adequately protect it with insurance.

r. Hillard has been an exceptional resource parent serving the District’s teens in foster care for nearly 30 years. Dr. Hillard is no stranger to foster care. Dr. Hillard shared, “I was introduced to the idea by my parphotographybymadison.com ents who fostered numerous children for many years. It became a collaborative family mission to care for children and youth in need and as a result, I initially became involved by serving as a backup support and respite to my parents.” Dr. Hillard also shared, “I too wanted to give back so that I could contribute in a positive way to my community. Dr. Hillard decided to eventually become a foster parent and serve specifically, youth over the age of 13. “I felt that this group was often vulnerable and difficult to place says Dr. Hillard.” Dr. Hillard feels that she has made a difference for youths in care in so many ways. “I have exposed them to theater, travel, educational activities, as well as career opportunities. I also feel that in addition, I have encouraged spiritual growth and development. It has been so wonderful to witness some of the youth pursue college or other vocational paths.” Currently, she is fostering a young lady who is 18 years old who will be attending college this fall. D

onna Flenory has been a foster parent with the D.C. Child and Family Services Agency for 16 years and exclusively cares for teenagers. “I began fostering when a friend, who was a CFSA employee, suggested that I get involved with a program that was only a six-month obligation,” Flenory says. “Sixteen years later, fostering has been one of the most rewarding things I have done in my life. The teens have actually enriched my life far more than I think I have enriched theirs. My foster children have been a joy to be around.” In caring for teens, Flenory strives to create a safe place for them to be themselves. “My approach and attitude are to be open and understanding of their needs and not to take all of their words and actions personally,” she says. She does not engage in power struggles but is both loving and firm. She wants the youths to settle in and feel safe and at home. Flenory is still in touch with the first two teens she cared for years ago. “I am considered a grandparent to three children born to two former young people who lived in my care,” she says proudly. One young woman formerly in care lives within walking distance of Flenory’s home, an ongoing connection that definitely feels like family. In fact, Flenory has been “extended family” for a number of young people in care, helping them enroll in college and transporting one to her freshman dorm at Alabama A & M University. “Teens never outgrow the need for family and secretly desire guidance and support even when they become young adults,”Flenory states. “I hope other District residents will step up and take on this important work. Building entrance and access ramps, widening entrances and doorways, adding a first-floor bedroom and bath, and other structural accommodations can be costly. But many other options that add both convenience and safety don’t require major expenditures. Replacing slippery tile or vinyl flooring with slip-resistant surfaces is also a must as falls are among the leading cause of injury to seniors.

Universal Design guidelines call for increasing both task lighting and general room illumination as we age. Research published in the British Journal of Ophthalmology suggests that natural light may be better for aging eyes than artificial light. The health benefits of natural light are well documented, from boosting your body’s ability to produce vitamin D to helping improve mood and fight seasonal affective disorder (SAD). Natural light also enhances virtually any decor, while helping reduce dependence on artificial light and lowering electricity bills. Increased ventilation Indoor air quality, according to an EPA study, can be worse than the air outside your home, and many people spend as much as 90 percent of their time indoors. Poor indoor air quality has been linked to respiratory problems, eye irritation, headaches and even depression. Adding skylights is one of the easiest, most cost-effective ways to achieve balanced, glare-free natural light, as well as passive ventilation, in virtually any space. Recent research confirms most Americans eat a combination of fruit and fruit juice. In fact, according to another study published in International Journal of Child Health and Nutrition, children who drink juice also eat significantly more fruit than those who don’t. Farro Salad with Veggies in Basil-Grapefruit Dressing Ingredients / makes 6 servings 1 cup/6 ounces farro 2 cups/8 ounces asparagus, cut into 1-inch lengths 4 ounces ruby red grapefruit juice 1 teaspoon grated garlic 2 teaspoons Dijon mustard 3 tablespoons extra virgin olive oil 1/2 cup/1/2 ounce basil, sliced or very roughly chopped 2 cups/8 ounces cherry tomatoes, halved 2 cups/2 ounces arugula, roughly chopped Preparation Place farro in a large pot with plenty of salted water. Bring to a boil and cook until al dente, about 40 minutes. One minute (depending on the size of the asparagus) before the farro is done, add the asparagus to the pot. Drain. While the farro is cooking, make the dressing. Whisk together the grapefruit juice, garlic, mustard and olive oil. Stir in the basil. When the farro is done and while it’s still hot, toss half the dressing with the farro. Taste and season with salt and pepper. Let sit for 10 minutes to allow the flavors of the dressing to absorb. Stir in the cherry tomatoes and arugula along with the remaining dressing. Taste once more and serve. Nutrition Information: Makes 6 (1-cup) servings. How can I get more vitamin E in my diet? The US Recommended Dietary Allowance (RDA) for vitamin E in individuals 14 years and older is 15 mg. Instead of taking supplements to get the recommended daily dosage, however, save your money and add vitamin-rich foods to your diet. “The simplest way to increase your vitamin E intake is to follow a healthy diet that focuses on eating vitamin E-rich vegetables, fruits, healthy fats and lean forms of protein,” says Lyssie Lakatos, a registered dietitian. Lakatos lists these five foods as

Spinach: When preparing salads, toss some spinach into the mix. This dark, leafy green is also high in calcium.
