Wedding Bells! Points of Interest
Happy summer, everyone! It’s been a HOT one! I couldn’t believe how warm Memorial Day weekend was. I heard it hadn’t been that warm for close to 100 years. I don’t know if that was true, but it was a great way to kick off the summer. I hope all of you have had a chance to do some serious relaxing. It seems that everyone I talk to is running crazy—all of the time. My daughter, Carissa, and her family are no exception. We were trying to plan get-togethers for the summer, so everyone brought out their phones to see what would work. Between Carissa and her husband’s schedules, vacations, and the girls’ camps, there were just a few days that were available. Crazy! On another note, I am happy to announce that there will be a wedding in the Van deWalle family in September. My son, Adam, and his girlfriend, Sam Taylor, are going to tie the knot! Sam has been part of this family for over 10 years. She and Adam met in high school and have been together ever since. Congratulations, guys! Sam and Adam recently rescued a Newfoundland mix puppy—a very cute dog. They named him Doug. I think that it’s always a good idea to test out parenting by adopting a puppy. Sam and Adam already have two cats, but puppies are different. You learn a lot about each other when someone has to let the dog out in the middle of the night! Sam shared with me the other day that it has made them realize that they might not be ready for parenting quite yet. Sam searched online and found that Doug will likely weigh more than 120 pounds at adulthood. Wow! That’s another person! The important things are that they love Doug and that the experience is helping them learn more about one another. Sam and Adam are planning the wedding with the help of Sam’s mom, Kathy. They’ve chosen an outdoor venue on a farm, and I’ve been told that it’s going to be very casual. I looked at the pictures, and it just seemed to “fit” them both. I admit, it’s kind of nice to just be a bystander and watch the thing unfold. I’ve got to plan the rehearsal dinner…I have a few ideas I’m going to run by them soon. Before we know it, summer will be gone, and the wedding will be just around the corner! Let’s all make a decision to take some time to relax this summer. Everyone I know works so hard, even those friends who are retired. Stop and smell the roses! Spend time with your families and create some great summer memories! We’ll talk next month!

We’ve been waiting all year for this—vacation season is finally here! Let’s make packing less intimidating by keeping it simple. I will never forget a trip I took a few years ago, before the extra charges for luggage were implemented. I took seven suitcases! One was filled entirely with shoes. Needless to say, it was a big hassle toting all that around, and my husband won’t forget it, either! The worst part? I only wore about a third of what I packed. Luggage restrictions force us to streamline and be savvy travelers. As it turns out, I actually find it less stressful to travel when I have fewer clothing choices...plus, if I fit all my clothes in one carry-on, I don’t have to wait around at baggage claim! When you’re choosing clothes for a trip, the first thing to consider, of course, is your destination… and I don’t just mean dressing for the weather. For instance, people in Europe and people in America dress quite differently. There are differences within America, too. Californians dress considerably more casually than New Yorkers do. The last thing you want to do is stand out as a tourist. The first time I visited San Francisco, I packed sundresses. It’s sunny California, right? Wrong! Northern California attire is very different from Southern California attire. I certainly stood out in my yellow sundress while the natives were sporting black leather jackets. It’s important to do a little investigative work on your location before you start making a packing list. Yes, I said a packing list. Making an actual, physical list is important, even if you think you will remember everything (you won’t). Here is a list of priorities that will remain similar no matter what your destination. Just use this as your starting point and customize it for your own travel! 1. Bring comfortable walking shoes. This is first on the list for a reason! Forget those cute heels—comfortable, flat walking shoes will make your trip more pleasurable, I guarantee it. If you’re anticipating dressier occasions on your trip, bring along a cute

We do custom shoots as well. pair of neutral (skin-toned) sandals with a small wedge, some ballet flats, or platforms. 2. Bring flip-flops or slides, even if you are not going to the beach. You will be glad you have them to wear in your hotel room and while going to the spa or whirlpool. 3. To make sure you don’t forget any toiletries, you should mentally take yourself through your morning and bedtime routines and list each item you use, including medications. Don’t forget the medications you only use occasionally, such as aspirin, allergy medications, stomach ailment medications, sunscreen, and so on. If you are flying, be sure to check the airline’s guidelines for carry-on items. 4. You’ll be glad to have an all-purpose jacket or sweater that can work with everything. Even if you’re going to the desert, it gets chilly at night—not to mention those air-conditioned restaurants. A jean jacket looks cute with anything and can be worn on the airplane to save room in your suitcase. 5. Figure out a color scheme and stick to it. Black with white or one color is always good. To get the most use out of the fewest clothing items, you need to be able to mix and match everything you bring. For every set of bottoms, bring two tops. For dressier occasions, I suggest a little black dress or a little white dress. A gathered skirt with an elastic waist can be pulled up under your arms and used for a bathing suit cover-up! 6. Keep jewelry to a minimum and leave the expensive stuff at home. Jewelry is so easily lost, broken, or stolen! One necklace that goes with everything, along with a pair of earrings and a watch, is all you really need when you’re traveling. A long, rectangular, multipurpose scarf is a useful accessory on vacation. Use it as a bathing suit sarong, shrug it around your shoulders with a sleeveless top, tie it around your waist for a belt, or toss it around your neck instead of the necklace. Let’s pack like champions so we can implement a vacation mindset. Let’s beat the heat and leave behind the stressors of home and work. Let’s slow down, take a moment to breathe, and smell the flowers, or the surf, or the forest, or the jungle. Are we there yet? GRILLED SHRIMP
Grilled Shrimp In a large bowl, mix one-fourth cup of olive oil, 4 cloves of minced garlic, 2 teaspoons of lemon zest, 1 teaspoon of red pepper flakes, and salt and pepper as desired. Mix well. Add 2 pounds of peeled and de-veined shrimp. Stir gently to coat the shrimp and then refrigerate for 20–30 minutes. When ready to grill, skewer the shrimp (remember to soak the skewers if they are wooden). Grill the shrimp for 3–4 minutes per side (or sauté in a skillet) until pink. Serve hot or cold. Holiday Morning Muffins Mix 2 Tablespoons of boiling water with 2 Tablespoons of powdered instant espresso; set aside. In a large bowl, combine three-fourths of a stick of soft butter with two-thirds cup of sugar and beat until fluffy. Add 1 egg, 1 teaspoon of vanilla, and the espresso mix. In another bowl, mix one and three-fourths cups of flour, one and one-half teaspoon of baking powder, and one-half teaspoon of salt. Add the dry mixture to the wet, alternating with one-fourth cup of milk. Fold in three-fourths cup of semisweet chocolate chips. Mix. Spoon batter into a lined cupcake pan. Bake at 375 degrees for 15–20 minutes, until muffins are golden brown and an inserted toothpick comes out clean.

You may also top them with cooked shrimp, thinly sliced cooked tuna, onion jam, thinly sliced steak, or chicken. Favorite Hot Beef Dip Blend two packages (8 ounces each) of softened cream cheese with one and one-fourth cup of sour cream, 2 minced scallions, and one-half teaspoon each of garlic powder and Old Bay (or several dashes of hot sauce). Add 2–4 ounces of minced dried beef (in the meat aisle) and 3 slices of crisp, crumbled bacon. Mix well and pour into a baking dish. Top with 1 cup of chopped toasted pecans. Bake at 350 degrees for 20 minutes. Serve with toasted breads, crackers, or veggies.
Red, White, and Blue Cake Pops For the cake pops: Bake your favorite cake in a 9x13-inch pan. Let it cool and then put the cake in a large bowl. Add about one-half cup of canned frosting and stir until you get a mixture that sticks together. Roll into balls (about the size of golf balls) and push one cake pop stick into each one. Refrigerate for 1 hour. For the frosting: Melt 1 package of white almond bark and divide into 3 bowls. Leave one bowl white for white frosting and use food coloring to make the remaining bowls red and blue. Dip the cake pops into the white frosting and then drizzle with the red and blue frosting. Sprinkle with holiday sprinkles or sugars. Refrigerate until set. Makes 24.
Best Baked Beans Cook 1 pound of hamburger or turkey and drain off the grease. Cook 5 or 6 strips of bacon until crisp, then dice. Place the meat in a slow cooker with 1 can of drained butter beans, 2 Tablespoons of brown sugar, 2 cans of prepared pork and beans, 1 can of drained garbanzo beans, 1 can of drained black beans, and 1 can of drained kidney beans. Pour in three-fourths cup of ketchup or chili sauce, 1 Tablespoon of Worcestershire sauce, and one-half cup of molasses. Add half of a chopped onion and a clove of minced garlic. Add salt and pepper to taste. Cook for 4–5 hours on low in the slow cooker or for 2 hours in a 325-degree oven.
Holiday Apple Pie You will need 2 pie pastries, either homemade or purchased. Peel and thinly slice 6 apples (a combination of Granny Smiths and reds). Mix together three-fourths cup of sugar, one-fourth cup of brown sugar, 2 Tablespoons of flour, and 1 teaspoon of ground cinnamon. Pour the mixture over the apple slices and toss. Put one pie pastry in the pie pan and pour in the apples. Dot the apples with 3–4 cubes of butter. Place the second pastry on top, flute, and vent. Put the pie on a cookie sheet. Bake at 375 degrees for 50–55 minutes, until golden and bubbly. WE

Most women share the miraculous ability to bear children. Some of us have blessedly passed through our childbearing years, some of us are still in the thick of it, and some of us are just entering this wondrous phase. Even those of us who do not bring any children into the world never fail to be awestruck by maternity, pregnancy, and the wonder of giving birth. Let’s be honest, though—not all of us mothers recall those nine or 10 months as a fairy tale that offered nothing but a happy ending. Pregnancy, delivery, and the postpartum period can be very challenging. Our bodies are asked to perform tasks which are absolutely miraculous, and, of course, only women can perform this duty. For many of us, there is the interminable morning sickness (which is certainly not limited to the morning hours alone!). For some of us, there is the inescapable heartburn or that terrible taste of metal in the mouth. We were expecting that our clothes wouldn’t fit, but even our shoes don’t seem friendly. Our skin and hair can become unruly, and our moods can swing from “Zip-a-Dee-Doo-Dah” to Hagatha the Witch. No matter. In just a few short months, a brand-new person will arrive and change everything forever. Throughout human history, we have accepted pregnancy as a

matter of fact. It is, after all, part of life. This most natural of human functions has been viewed differently by different societies, ranging from nothing special to placing the mother-to-be or the new mother on a pedestal. There are different ways of thinking about pregnancy, childbirth, and early motherhood. Today, childbirth seems to be experiencing a “rebirth.” At one time, in some societies, pregnant women were kept from the public eye. Once they had reached a certain stage of pregnancy, they went into “confinement.” I suppose they spent their time knitting booties and making layettes. More recently, pregnancy was viewed as a medical issue, compared to an illness or a temporary disability. Currently, pregnancy is enjoying being what it is, and that is life renewing itself. Today, we see pregnant women everywhere, and what a joyous sight it is! Pregnancy wardrobes have become

more flattering and easier to live with. During the 1950s and 1960s, maternity clothing was often festooned with lace, frills, ruffles, and other doodads. For many of us, these garments did nothing to add to the joy of the moment. Thank goodness someone finally realized that being pregnant does not mean having to look like a circus act! Many maternity dresses show off rather than hide that growing bump and help us celebrate the fact that there is someone soon to be born. Although I can’t cite any scientific evidence of the “feel good” factor, we all know that looking good helps us feel good. When senior women encounter a pregnant woman, all kinds of memories come rushing back as we take time to remember our own pregnancies and deliveries. It’s a special gift to those of us who have passed through our childbearing years. In the face of increased technology and care, it’s a pretty good time to have a baby. Today’s expectant mothers can be assured of their baby’s health and critical development even before the baby has been born. New methods of childbirth are available to make labor and delivery easier. We are seeing a resurgence of midwives, and doctors and their teams offer the best support available for both mother and baby. This wonderful time in a woman’s life now can be less frightening and uncertain. What about after the delivery, when the support team has gone home? The new mom is left physically exhausted, experiencing at least some degree of pain and, in some cases, challenged by postpartum depression that is sometimes mistaken for “just” the blues. Although it is not generally well known, there are services available for expectant mothers and new moms to help them cope with problems and challenges accompanying pregnancy, childbirth, and the postpartum period. While waiting for baby to be born, it’s a good idea to spend time gathering information about the birth experience and what to expect. Studies do show that women equipped with knowledge before childbirth experience less fear, fewer drugs, and fewer forceps deliveries. Services such as physical therapy, classes designed especially for women who need a little extra support, and other methods of help are now sometimes covered by insurance plans. Be sure to check them out. After all, the more knowledge you possess and the more confidently you can advocate for yourself and your baby, the better you will feel as you begin this new chapter of life. WE

A stylish mane has been an important component of self-image since ancient Greek and Roman times. It has long been associated with wealth and royalty, and it still greatly impacts self-confidence today, particularly among females. Let’s face it—first impressions count, and being well-coiffed certainly influences that first reaction. Many women will acknowledge that a bad hair day can mean a bad day overall, especially for those of us with thinning hair or bald spots. Luckily, when it comes to hair loss, many treatments exist that will help preserve those beautiful tresses. There are several factors that can lead to hair loss. Stress, pregnancy, an overactive thyroid gland, lack of protein, sudden weight loss, hormonal changes, and certain medications can all cause hair loss. Fortunately, these effects are usually temporary. What can we do about permanent hair loss? Men may begin to notice bald spots or receding hairlines as early as their late teens or early 20s, with two thirds of men experiencing what is referred to as male pattern baldness by age 60. Hair loss in women usually manifests itself as thinning hair or a widening part, typically at age 40 and over. One in five women will be affected by female pattern hair loss, or androgenic alopecia, but options are available to help slow
down the process. One of those options is medication. Propecia for men can slow down hair loss, and some may even experience new growth. It’s typically administered in pill form. For women, hormone replacement therapy may decrease or even stop hair loss. If an autoimmune disorder is suspected, a corticosteroid may be prescribed to stop the inflammation responsible for the problem. Platelet-rich plasma therapy (known as PRP), a minimally invasive treatment that has been around for the last decade or so, has demonstrated favorable results in many patients. PRP therapy is a process whereby blood is drawn from the patient and placed into a centrifuge that isolates the platelets. Following this separation, a physician injects the platelets into areas of the scalp where hair loss has occurred to stimulate new growth. The treatments are usually administered monthly for the first three to four months, and then every three to six months after that. Results may be noticeable in as little as two months. Doctors have reported that a majority of their patients experience hair regrowth of 30–40 percent. This procedure is approved for both men and women. Low-level laser therapy has also shown promise in combating hair loss. Laser treatments increase circulation and stimulate hair follicles, promoting growth. A bonus is that there are virtually no side effects. It may also improve the strength and thickness of existing hair. Outcomes vary, but many patients experience promising results. Surgical options are also available to combat hair loss. Hair transplant surgery has been around since the 1950s, but techniques and technology have greatly improved over the years. Gone are the days of hair plugs that looked like tiny bushes sprouting out all over your head! Two widely-used hair transplant methods are follicular unit transplantation (FUT) and follicular unit extraction (FUE). FUT involves removing a long, thin strip of hair from the back or side of the scalp. The wound is then closed, and individual follicles are removed from the strip and reinserted into balding areas. The FUE procedure harvests hair follicles over a larger area of the donor region, so wound closure is not required. The reinsertion process is similar in both procedures. The physician numbs the area and creates tiny slits using a scalpel or needle. A graft is placed in each opening. The procedure can take up to 10 hours. The area may be bandaged for a couple of days. The transplanted hair may fall out in two to three weeks, but don’t be alarmed! New growth appears in a few months. Touchups and multiple sessions may be needed. It may take up to two years for full results to be visible, but most patients are pleased with the results. Sometimes, more than one treatment is utilized to help patients restore their crowning glory. Hair loss can feel defeating, but selecting a treatment doesn’t have to be a hair-raising experience. Benched by Sticking Joints? Get Moving Again With Orthopedic Procedures By Kylie Gumpert
One of the unfortunate truths about life is that joints wear out over time. Maybe it’s a knee that gets a little stiff, or a shoulder that hasn’t been the same since you injured it, or arthritis that’s making it hard to type. Joint pain can be a real bummer, and it can sneak up on you. Even something like a little excess weight can start to cause trouble with your hips, which makes it difficult to exercise as much as your doctor recommends, which makes you gain more weight—it’s a vicious cycle. If any of these situations sounds like yours, it might be time to consider a joint replacement procedure. Joint replacement is an orthopedic surgery that can involve removing worn or damaged joint material and replacing it with a prosthetic joint made from plastic, metal, or ceramic. Surgical procedures to treat painful, worn-out joints can include total joint replacement, partial joint replacement, or smaller, less invasive procedures that repair soft tissue or cartilage within the joint. Women are significantly more likely than men to undergo joint replacement surgery, often because of joint inflammation, pain, and damage caused by arthritis. However, according to a study presented at the annual meeting for the American Academy of Orthopaedic Surgeons (AAOS), younger people and more men are now getting this type of surgery. Surgeons speculate that the changing demographics are due to increasing obesity rates as well as more active populations who want to maintain their mobility as long as possible. Invasiveness ranges from total replacement surgery, where joints are completely replaced, to partial replacement, where joints are partially replaced, to more minimal procedures to repair, replace, or remove tissue and cartilage that surround or are part of joints. Even total replacement surgery varies in invasiveness—depending on your particular situation, techniques that require a smaller incision and less muscle detachment may be suitable. Most popular is knee replacement surgery, which is actually considered “resurfacing” because only the top layer of the femur and tibia bones are replaced with metal parts. They are then separated with a spacer made from plastic. In some cases, the back of the kneecap is also resurfaced to fit with the new parts. These replacement parts come in different sizes to fit different patients. A study from the Archives of Bone and Joint Surgery found that shoe size is one of the best predictors for determining how large or small a knee replacement part should be. With that estimate, surgeons can order parts that will fit and lead to better healing. Over 90 percent of patients report reduced pain and increased mobility, according to the AAOS, but it’s important to remember the limitations that surgeries like this can have. A knee replacement should last for several years and should allow you to walk, hike, bike, dance, or swim like you used to, but surgeons don’t recommend that you take on the highest-impact activities. Within three

to six weeks, you should be able to resume activities such as walking or climbing stairs as well as special exercises meant to increase mobility. The same goes for total hip replacement surgery. Since the hip is a ball-and-socket joint, total replacement surgery involves removing and replacing both the ball, or femoral head, and the socket, or acetabulum, which is part of the pelvic bone. For smooth movement, the surgeon will also insert a spacer made from metal, ceramic, or plastic between the new ball and new socket. There is debate over whether a posterior approach (from the back of the hip) or anterior approach (from the front) is more effective. Some surgeons say patients tend to recover more quickly and experience less pain when they undergo anterior surgery, but posterior surgery does not involve cutting through as much muscle. The general consensus seems to be that surgeon skill matters most. Many people report decreased pain and increased mobility with this surgery. The shoulder is another balland-socket joint that, depending on the damage to the bone, may only need to be partially replaced or resurfaced. In cases where a patient has severe damage to the arm muscle (specifically, the rotator cuff), regular replacement surgery will not relieve the pain. In these cases, reverse total shoulder replacement, where the prosthetic ball and socket are switched, is used. Patients will need to use a sling for up to four weeks after shoulder replacement surgery. They can resume activities such as dressing unassisted after a couple of weeks as their healing continues. The wrist is a more complex joint because it’s made up of several bones. It can be replaced with two metal pieces that are inserted into the radius and carpal bone, then cushioned with a spacer. A patient will need to wear a cast for the first few weeks and a splint for up to eight weeks after that. If a patient is suffering from issues related to the soft tissue around the wrist joint, other treatment may

be needed. Carpal tunnel syndrome causes pain, tingling, and numbness in the hand and arm as a result of pinching of the medial nerve, which travels down the arm and through a space called the carpal tunnel in the wrist. If nonsurgical options like splinting and medication don’t work, carpal tunnel release surgery may be used. A surgeon will cut the ligament that forms the roof of the carpal tunnel to allow more space for the nerve, relieving pressure. Trigger finger causes pain and locking of the finger and is often caused by arthritis, diabetes, or repetitive hand movements that involve grasping. Bands of tissue that help attach forearm muscles to fingers swell or sometimes thicken from disease, making it difficult for tendons to slide back and forth as they should. As a result, the finger gets stuck in a bent or curled position. Trigger finger release surgery cuts the tissue that is preventing the tendon from moving smoothly. In total ankle replacement, a surgeon will remove damaged bone and cartilage, attach a metal piece to both the end of the tibia and the top of the talus, and then insert a plastic spacer. Less intensive is arthroscopic debridement, or cleansing, where troublesome bone spurs and loose cartilage are removed from around the joint. Another option is arthrodesis, where the joint bones are completely fused to prevent painful motion from occurring. Full recovery for these procedures can take up to nine months. Technology isn’t perfect yet, but by using a combination of CT scans, MRIs, and other 3D imaging tools, surgeons can capture more accurate renderings of bone and create better replacement parts. This reduces the chance of misalignment and the need for revision surgeries or procedures to further replace replacement parts that become worn over time. Minimally invasive procedures that are showing promise as
alternatives to surgery are stem cell therapy and platelet-rich plasma therapy. When concentrated stem cells or plasma derived from your own body are injected into a damaged joint, your cells can more quickly and effectively regenerate and heal damage such as torn ligaments or injury to cartilage and bone. Some injuries or joint issues simply require surgery, but, as we learn more about using the body’s own healing systems to our advantage, fewer people may need to go under the knife. For osteoarthritis in particular, several clinical trials have shown stem cell therapy to be a simpler and less invasive (and, therefore, less stressful) treatment, according to a study from the World Journal of Stem Cells. Experts note that more study is needed to establish the best standards for these treatments, such as how many cells are sufficient to stimulate more effective healing. If you think you may be a candidate for orthopedic surgery, begin by asking your primary care doctor to refer you to a boardcertified orthopedic surgeon. The AAOS also has an online directory where you can look for specialists in your area who are board-certified and have the necessary experience in the procedure you need. As these procedures have gotten better and better over time, they’ve brought significant pain relief and increased mobility for the vast majority of patients who decide to try them. Life’s too short to live in pain or miss those special moments because of joint trouble. Homestead Rehabilitation Center Expansion Into Neurological Rehab By Jodi Fuson
According to Homestead Rehabilitation Center administrator Matt Romshek, the number-one cause of hospital admissions in Lancaster County is injury to the brain or neurological disorders. To meet this need, Homestead has decided to commit its time, effort, and energy to specialize in the treatment of neurological disorders like Parkinson’s disease, strokes, brain injuries, and Alzheimer’s disease. As one of four skilled nursing facilities in Nebraska to earn the 2017 National Silver Achievement in Quality Award, Homestead is in a great position to help even more patients with this new focus. To meet this need, Homestead is dedicated to the commitment to treatment of all brain injuries. Homestead therapy provider is collaborating with RehabCare, a premier provider of physical, occupational, and speech-language rehabilitation services. “We’re going several steps further to specialize in this,” Matt explains. Homestead’s 11 therapists will be receiving their movement disorder training through RehabCare so they can more effectively assist clients with a brain disease or those who have suffered a

stroke or brain injury. Depending on their diagnosis, patients with brain disease or injury may need short-term or long-term rehabilitation services; both are available at Homestead. The majority of rehab patients are referred to Homestead’s Stroke and Brain Injury Specialty Program by doctors after being discharged from the hospital. Others arrive straight from their own homes or from other assisted living facilities that could not provide the specialized treatment required. Patients

who are living in Homestead’s long-term skilled facility are screened quarterly to assess their need for neurological services. All of Homestead’s patients benefit from access to on-site physical, occupational, and speech therapists. A nurse practitioner is available seven days a week, including weekends and holidays. Patients are evaluated within 24 hours of admission. A plan of care is developed with an interdisciplinary approach. Members of the team primarily include the patient, family members, nursing staff, and therapists. Neurologists, dietitians, and psychologists may also be included. An orthotics vendor is available to provide shoe lifts, braces, splints, and ankle and foot orthotics. Landon Berg is a physical therapist and director of the Homestead Stroke and Brain Injury Specialty Program. He explains that therapy sessions are scheduled as often as the patient needs them. In many cases, physical therapy sessions are scheduled seven times a week, occupational therapy sessions five times a week, and speech therapy sessions three to five times a week. For many patients, the primary therapy goal is to reduce the risk of falling. That can mean helping a patient reclaim the ability to ascend and descend stairs or independently handle toileting and transfers to and from furniture. For Parkinson’s patients, improving gait is a goal. Landon notes that he often uses a metronome or music to aid therapy for those with gait issues. “It helps if it’s music that the patient is interested in,” Landon shares. “We have a Roku, and we ask the patients what music they like.” For patients who have had a stroke or some other brain injury, therapists might work on head movement, eating, decision-making, and memory. VitalStim, an electrical stimulation device, can be used to help patients with swallowing practice.
Most patients in the specialty program at Homestead need all three types of therapy—speech, occupational, and physical. “We care about getting each patient home as soon as possible,” Landon says. He is proud of the great therapy team they have assembled. Several of the staff members have 10 or more years of experience in the therapy field, and the rehab technician has more than 35 years of experience, Landon shares. Landon’s favorite thing about his job is seeing patients progress toward independence. He recently worked with a stroke patient who couldn’t stand or walk when she was admitted but was moving independently when she left six months later. Whether patients are completely independent when they leave or still require some support, Landon says, the Homestead team collaborates to get patients discharged with the least restrictive environment possible. When patients are discharged, they leave with a wellness program designed to keep them strong and help them maintain their function, Landon explains. The plan typically includes daily exercises assigned by the occupational, speech, and physical therapists. Some examples include cognitive and balance exercises. After discharge from Homestead, patients can still access outpatient therapy services. Many prefer to return to Homestead so they can continue working with the same therapists, Landon states. Sportsmanship at Kids’ Sporting Events What Do They Learn From Watching You?
I wrote my first Kids Comments column for Women’s Edition nearly 20 years ago. While reading over some of those early columns, I found a recurring theme: Parents, YOU are the models for your children’s behavior. I spent 20 years volunteering with youth sports. In that time, I moved from working with local teams and leagues to directing a district comprising a third of our state. I had the opportunity to work with some talented and well-behaved kids, eager-to-help parents, and a bevy of volunteers. Most of those interactions were positive. Those are the ones I remember. Not long ago, I attended a youth sports game for the first time in years. Following an experience at that event, I’ve decided it’s time to reiterate my statement: Children learn from the example that adults set. The game was proceeding nicely, even with young and easily distracted kids. Most of the fans on both sides were positive and supportive. Comments like “Good job!” and “Nice try!” were heard from both sidelines. Then, the trouble started. A parent overheard a coach from the opposing team tell one of his players to be more aggressive. That parent berated the coach and then hollered at his own kid to do the same thing. Those adults (and I use the word loosely) started yelling at each other, with name-calling and four-letter words not usually heard in mixed company.

Many of the remaining parents from both teams seemed embarrassed. Someone went to find an official who might intervene. Some tried to shush the argument. Still others joined in the yelling, screaming, and general mayhem. The poor kids stood by, looking confused and asking each other what they should do. Other onlookers and coaches urged the players to continue the game. Eventually, the original offenders were convinced to return to their sidelines and encouraged to be quiet. The game continued. So did the feeling of unrest and discomfort. Fortunately, the whole fiasco remained verbal and didn’t end up in fisticuffs. I hope someone talked to those involved and explained why what they did was wrong. In fact, I hope that was pointed out to all the teams—and fans—involved in the league, no matter what their age. Fans are expected to root their teams onward. Kids are expected to play aggressively, but there should be no intent of hurting another player. The language used was offensive to me and, I hope, to other adults at the game. We need to teach these kids to be good sports…graceful winners and good losers…respectful of their opponents. The best way to get this across is to set the example you want the kids to follow. What example do YOU set? Recognizing Sleep Apnea Are You Getting a Good Night’s Breath?

Recognizing and treating sleep apnea can significantly improve your quality of life. In fact, it may even save your life, since sleep apnea is strongly associated with dangerous medical conditions like high blood pressure, heart arrhythmia, and stroke. An estimated one in five adults has sleep apnea, a condition in which breathing repeatedly stops and starts during sleep. Common symptoms of sleep apnea include a pause in your breathing (observed by another person), waking up gasping for breath, waking with dry mouth and throat, morning headache, frequent waking in the night, loud snoring, excessive daytime sleepiness, attention problems, or irritability. Men are more prone to sleep apnea, but a significant number of women experience symptoms, especially after menopause. The signs tend to be different for women—they are more likely to have headaches, fatigue, depression, anxiety, insomnia, and sleep disruption. The causes of the two main types of sleep apnea are quite different. Obstructive sleep apnea (OSA) occurs when your airway is blocked or narrowed. Central sleep apnea (CSA) happens when your brain does not send the signal to breathe. The obstructive variety is far more common than central, though some people exhibit both types of sleep apnea. The good news is that sleep apnea can be successfully treated, regardless of the root cause. Factors contributing to OSA might include chronic nasal congestion, large tonsils or adenoids, or muscles in the throat that relax and allow tissues to collapse, which narrows or blocks the airway. Being overweight can contribute to sleep apnea when fat deposits put excess pressure on the upper airways. Smoking, alcohol, and some medications can also increase a person’s risk of OSA. In the less common CSA, breathing stops not because of obstruction but because the brain doesn’t send the proper signal to the muscles involved in breathing. CSA may be attributed to aging, neurological disease, heart issues, opioid pain meds, or stroke. Snoring is less common in CSA. A common diagnostic tool for sleep apnea has been the overnight sleep study in a hospital or clinic setting. (Now, there is also a simpler test that some people can do at home—ask your doctor.) In the clinic or hospital sleep study, machines monitor heart rate, breathing, brain activity, and oxygen levels in the blood. In the sleep lab, a technician applies sensors or electrodes that monitor sleep stages and cycles of REM and non-REM sleep. Vital signs will be recorded while you sleep. Data will then be evaluated by a physician, who will look for disruptions in such indicators as oxygen levels and breathing and heart rates.

Once a diagnosis is made, treatment options are considered. Continuous positive airway pressure (CPAP) machines are often prescribed for sleep apnea. The machine delivers air pressure through a mask over the nose to help keep airways open during sleep. Other simpler devices may treat mild forms of OSA. Oral devices fitted by the dentist may help keep the airway open. Such appliances prevent airway obstruction by holding the tongue in place or by advancing the lower jaw forward. A dental specialist will determine the best option for each patient. Various surgical procedures, such as removal of excess tissue, jaw repositioning, or insertion of rods might be options, depending on the individual. With CSA, underlying causes such as heart or neuromuscular issues would be addressed by a cardiologist or neurologist. Lung disease may be treated by a pulmonologist. Supplemental oxygen or the newer airflow machine hybrids are sometimes prescribed. The seriousness of sleep apnea complications cannot be overstated. Untreated, it can cause high blood pressure, with resulting damage to the heart and kidneys. In some patients, sleep apnea is believed to contribute to heart attack, glaucoma, diabetes, and cognitive and behavioral disorders. Several friends of mine have been successfully treated for sleep apnea, and all are feeling rested and happier since being fitted with CPAP machines. The almost immediate improvement in quality of life is obvious, and a healthier and longer life is a probable long-term result. The best place to begin is with your family doctor or internal medicine physician. Referrals might be made to a specialist in sleep medicine, pulmonology, neurology, or dentistry. Don’t worry about the strangeness of a sleep study—they’re noninvasive, and you can wear your own pajamas! For mild cases, lifestyle changes might be sufficient or might be combined with other therapies. Eating healthy foods, getting regular exercise, losing excess weight, and quitting smoking can have a positive impact (on both sleep apnea and your general health). It’s as if life sweeps us up in a tornado, and off we go. We’re traveling a million miles an hour, and maybe we’re afraid to get off? I don’t know what I was getting out of it. All I know is that it was making me crazy. I could feel the toll it was taking on my health and relationships.
