Wedding Time! Happy fall to all! When I was outside on my deck the other night, a wind came up, and it suddenly felt just a little bit like autumn. It was a very bittersweet moment for me! I love the sun, and I love the heat, but I also love the changing color of the leaves and the cool crispness in the air. I have lived in Nebraska my entire life. I think that we “natives” stay because of the variety of the seasons. Yes, winter can be brutal, but I wouldn’t give up the spring, summer, or fall! Maybe down the road…down many, many roads… when I retire, I’ll become one of those “snowbirds!” I am very excited that my son, Adam, will be getting married on September 22. His fiancée, Sam, has been there with him since he was 17 and she was 16. They will have been together almost 11 years when they get married. They have planned the ceremony close by, at a farm-type house, so they are hoping for good weather. They do have a Plan B in case of rain, but I have a feeling they won’t need it. When they were looking for a location, Adam told me they wanted a place that their friends could relax and camp if they wanted to. The house has a large lawn, and a number of friends will be taking Sam and Adam up on this idea. I think they will have a splendid wedding and reception! Weddings certainly have changed from years ago. Adam and Sam will be married by a close friend, who has become a “minister.” The last four weddings I have been to, this has been how the ceremony takes place. The first time I saw this, I found it a little disconcerting, if I am honest with myself! The more I see it play out, however, the more I am coming to believe that it is a very good thing. Let’s face it—if you have a friend who has known you for years perform the ceremony, it can be very personal. The four weddings like this that I have been to have been just that! The “minister” shares stories of the couple’s life together and is likely to know plenty of funny anecdotes for the assembled guests to enjoy. It is just a more personal experience. I don’t know what Adam and Sam’s “minister” has in mind, but Sam told me that this friend has been planning for years to perform their ceremony. I’m looking forward to it! Enjoy the warm days that we have left. I looked at the long-range forecast, and September is supposed to be warm. Let’s milk summertime for all we can get! We’ll talk next month!

Fall for Fashion in Autumn 2018 Nostalgic Trends Rule the Runway By Holly Bell
What were your favorite decades for fashion? Mine were the 1980s and 1990s. I am in luck, as designers have decided to focus on reworking the fabulous fun fashion of those years. From Dad sneakers to Mom jeans, history will be repeating itself—but with new twists. After a long period of minimalism, people are ready to have some fun with fashion again. Referencing the past in fashion, hairstyles, and makeup will turn this year’s fall and winter into seasons of exuberant spirit and indulgence. Here are some of my favorites: High-waisted skinny jeans. Taking the stretch of a legging and mixing it with a high waist was pure genius. You have the slimness of a narrow leg but the comfort of a high waist, all done in stretch denim. Adding to denim’s already fashionable destructed looks will be acid-washed fabrications. Remember those acid-washed jean jackets? They’re back! Animal prints. Animal prints come and go, but this fall they are here to stay. Who doesn’t love a leopard print? Adding to the wildcat trend will be eye-catching tiger prints. Fabulous in fall coats, designer Victoria Beckham leads the way with this look. The newest animal print looks are done in Technicolor and rainbow hues. It’s officially time to get wild! Padded shoulders. Trends are showing exaggerated shoulders, but don’t get too carried away—for most women, a little padding goes a long way. A smaller, squared-off shoulder is flattering to all women, bringing a crisp line to the silhouette and slimming the torso. Designers are even revisiting the “power suit” of the 1980s, with fitted jackets, prominent shoulders, and slim skirts. Jewel tones. The ’80s and ’90s brought us those gorgeous tones of brilliant-blue, purple, scarlet-red, fuchsia, emerald-green, and mustard-gold. These bold colors are a welcome break from the paler colors we’ve been seeing. Faux fur. Dyed in every color imaginable, faux fur will be everywhere. At low price points, faux fur is also fun fur, as women collect unique pieces and wear them for both casual and dressy occasions. The look of shearling will be introduced as a new fabrication that works well for women and men. Plentiful plaid. Tartan plaids in colors from bright yellow to red and black are back, but in new ways. Designers are showing different plaids worn together for a fresh take on the look. That might be a little too daring for the average person; most women will
6

stick to a single plaid pattern, paired with a solid. Classic plaids, such as herringbone, glen plaid, and the ever-present houndstooth check, look richer than ever this fall. The Matrix. This 1999 movie featured Keanu Reeves in a dystopian future where machines controlled human perceptions of reality. He flaunted an all-black maxi leather coat along with wraparound sunglasses throughout the film and made it a fashion craze. Welcome back to the world of Goth, where all black is a mysterious yet unrelentingly slimming and sophisticated look. When wearing all black, bright red lipstick breaks up the somber tone and may be the only accessory you need. It will be fun to explore the darker side of glam. Luxe athleisure wear. Remember the glitz of embellished t-shirts and workout pants with matching jackets? The aesthetic is being brought back by designers who loved the Rodeo Drive trend of embellishing with sequins and rhinestones. Disco ball glitter is all over the runways this season, on apparel, handbags, scarves, and statement jewelry. This is a trend to have fun with! Fall florals. Small-printed florals are everywhere this fall, mostly done on a black background. The trend this year is mixing these florals with other prints. Surprisingly, you can mix plaid or animal print with a floral pattern, as long as they have the same colors in them. It’s time to experiment and come up with some new ways to wear your prints. This season, anything goes! If you are a person who hesitates to try a brand-new style, this is your year! Everything will look familiar this season, but with a few twists and giggles. The fashions we loved in the ’80s and ’90s (assuming you’re old enough to remember them) will be back in full force. It’s time to welcome back this “new vintage” and incorporate all the things we used to love into our current wardrobes. You get to enjoy the familiar looks but with the new fabrications we have come to love for their easy care and lower price points. Fall for fashion this year! What’s not to love? Editor’s Note: Holly has been in the fashion industry for over 30 years as a buyer, boutique store owner, visual merchandiser, and fashion show producer. She is currently the owner of a modeling agency. Holly regularly appears on local TV doing fashion segments. WE For more information, visit www.cartise.com

Good-bye, Summer—Hello, Game Days! By Kate Beiting
Peach-Raspberry Crisp Mix 4 cups of peeled and sliced fresh peaches with 1/4 cup of sugar, 1/2 teaspoon of ground cinnamon, and 2 Tablespoons of fresh lemon juice. Place the mixture into a baking dish and top with 2 cups of fresh raspberries. Make the crisp by crumbling together 1 cup of flour, 1/4 teaspoon of nutmeg, and 1 stick of cut-up butter (use your fingers or a fork). Sprinkle the crisp on top of the fruit and bake at 375 degrees for 40–45 minutes. Ricotta Frittata Whisk 4 large eggs with 1 cup of ricotta, 1/4 cup of grated Parmesan cheese, 3 Tablespoons of fresh minced parsley, and salt and pepper to taste. Add 1 Tablespoon of olive oil and 1 Tablespoon of butter to a large skillet over medium-high heat. Add the egg and cheese mixture, cook for a minute, and then use a spatula to lift the cooked portion up and let the uncooked eggs flow underneath to begin cooking. Cook for 3–4 minutes until the eggs are done (or put the skillet into a 325-degree oven for 5 minutes). Flipping the frittata over to finish the other side is optional. Serve with freshly sliced tomatoes.

(or mix it in with the cream cheese). Top with 24 cooked and deveined shrimp, 1/2 cup of chopped green or red pepper, 1/3 cup of chopped green onion, and 1 cup of shredded cheese. Refrigerate for 1 hour. Cut into 24 squares and serve. Hot Ham Appetizers Mix together one 13-ounce can of pineapple chunks and its liquid, 1/2 cup of water, 1/2 cup of brown sugar, 2 Tablespoons of cornstarch, and 1/4 of a diced green pepper. Cook on low for 5–10 minutes, until thickened. Put 1 pound of sliced cooked ham in a slow cooker. Pour the pineapple mixture on top. Heat on warm for 2 hours. Corn Salad (Adjust ingredients and seasonings as desired.) Mix 1/2 cup of fresh lime juice with 1/4 cup of olive or canola oil, 1 teaspoon of minced garlic, a dash each of salt and pepper, and 1/2 teaspoon of cayenne pepper. Set aside. Mix together 2 cups of cooked corn (fresh, canned, or frozen), 1 diced red pepper, 1 cup of chopped red onion, 4 chopped avocados, 1 seeded and diced jalapeño, and 3 cups of sliced cherry tomatoes. Pour the dressing over the vegetables and stir. Refrigerate until ready to serve. Savory Lemon Chicken (A quick pan sauce and protein—perfect!) Slightly flatten (or butterfly) 4 boneless chicken breasts. Place breasts in a greased skillet and cook for 10 minutes on mediumhigh heat, until browned. Remove and set aside. To the skillet, add 1 can of cream of chicken soup (or make your own white sauce), 1/4 of a chopped red pepper, 2 Tablespoons of milk, 1/2 teaspoon of paprika, a dash of hot sauce, and 2 Tablespoons of lemon juice.
Heat to boiling, reduce heat to low, and add the chicken back to the skillet. Cover and cook for 5–10 minutes, until chicken is no longer pink. Taste the sauce and adjust seasonings as desired. Garnish with 4 fresh lemon slices and fresh parsley. Can be served over cooked rice, spaghetti, egg noodles, or quick biscuits. Tutu’s Grilled Pineapple Chicken For the marinade: Mix together 1 cup of pineapple juice, 1/2 cup of brown sugar, 1/2 cup of soy sauce, a dash of onion powder, 1 minced clove of garlic, and salt and pepper as desired. Put 4 chicken breasts in a zip-top bag and pour half of the marinade over the chicken (reserve the other half for the sauce). Seal the bag and refrigerate several hours or overnight. For the grill: Grill the chicken over medium-high heat (discard the used marinade). Peel, core, and slice a fresh pineapple and grill along with the chicken (3–4 minutes on each side). When ready to serve, take the reserved marinade and add 1 Tablespoon of cornstarch and 1 Tablespoon of water. Whisk and microwave for 1–2 minutes, until thickened. Glaze the cooked chicken with the sauce and serve with the pineapple. Key Lime Cake For the cake: Prepare a yellow or white cake mix according to package directions, except substitute 1/2 cup of fresh lime juice for the water and add the zest of 1 lime. Pour the batter into a greased and floured Bundt or tube pan. Bake at 350 degrees for 35–40 minutes. Cool the cake and invert onto a serving platter. For the icing: Mix 2 ounces of soft cream cheese with 1 Tablespoon of soft butter, 1 cup of powdered sugar, 2 Tablespoons of lime juice, and a dash of vanilla extract. Frost and garnish with more lime zest. WE

Tufly’s Shoe Center
Promoting Happy, Healthy Feet By Roxy Orr
Fitness experts urge us to take 10,000 steps a day to stay fit and healthy. That’s about 3,650,000 steps a year—no small task, especially if your feet hurt. Remember the saying “my dogs are barking”? If you are experiencing foot pain, come visit Alan and Dorothy Tufly of Tufly’s Shoe Center. They fill doctors’ footwear prescriptions for diabetes, plantar fasciitis, arthritic deformities, and other problems. They are specialists at solving foot problems with great-fitting, stylish shoes. We tend to focus on many other parts of our bodies to maintain health—we moisturize, we exercise, we tone, we diet—but we often ignore the important platform upon which our skeleton balances. If you have ever had a foot problem, you know it’s true: If your feet hurt, everything hurts. “We have solutions to foot problems, and the trust of our customers is so important to us,” Dorothy says. “That is what we strive for with each customer.” Alan and Dorothy are passionate about helping their customers find the right shoes for their individual needs. The Tuflys opened their shoe store 25 years ago and have been at their present location at 16909 Lakeside Hills Plaza for 12 years. Alan and Dorothy have a combined 55 years of experience in the business. While I was in the store, looking at the different styles of shoes, a young woman came in with a special request—she had small, wide feet. She was a special education teacher, and she knew that she needed comfortable shoes with good support. Dorothy asked her key questions about her needs, her style, and what types of shoes she had liked in the past. After listening carefully to her customer and zeroing in on what she needed and desired, Dorothy suggested several shoes. Ask yourself if this is the type of treatment you might get at a big box, self-serve shoe store. There is a certified pedorthist on staff at Tufly’s Shoe Center. Dorothy calls pedorthists “pharmacists for the feet.” Pedorthists have completed additional educational and clinical training in the mechanics of the foot; they work with other healthcare professionals to ensure that each customer is evaluated correctly regarding foot needs. One of the solutions for balance or gait difficulties is molded orthotics. At Tufly’s Shoe Center, your orthotics are fitted and molded to your own foot. Orthotics are fit specifically for each customer and are made in the in-house lab. Orthotics can help customers who are suffering from pain in their feet, knees, hips, and back. Shoe manufacturers are listening to men and women who are demanding great style, fit, and comfort in their daily footwear. The Finn Comfort line is known as the ultimate therapy shoe, improving posture and muscle function. Several years ago, I had a colleague who was going to Italy for three weeks. She

asked me what kind of walking shoes to take with her. I referred her to Tufly’s Shoe Center, where she purchased a red pair of Finn Comfort sandals. She later told me that those shoes were the best investment she made for that trip. Finns have removable insoles, allowing you to put in your own custom insoles, if needed. That way, you can have great arch support and a stylish shoe. Other brands available at the store include Brooks, Think!, Dansko, Aravon, Alegria, Earth, Vionic, Dunham, Saucony, P.W. Minor, Drew, and Naot. Danskos are popular with professionals in the medical, culinary, and teaching fields. Finn Comfort’s Finnamic line of stability rocker sole shoes are a very important line for people who need relief from plantar fasciitis, pain in the metatarsal of the mid foot, and arthritis problems. The Tuflys are particularly impressed with the Naot brand. This line from Israel (“Naot” means “oasis”) has been around since 1942 but is relatively new to the United States. Tufly’s carries all styles of shoes, including workout, casual dress, business casual, and special occasion shoes. The office collections are especially popular with male and female young professionals. Young or old, working or retired, active or relaxed—no matter what your style, you’ll find shoes at Tufly’s that fit your needs. Alan and Dorothy are happy to place special orders, and they keep notes about your purchases for future reference. They’re eager to help you find a shoe that keeps those dogs from barking. Shoes that address your foot pain can still have a “Wow!” factor. Remember that 10,000-step journey? It begins with a first step to Tufly’s Shoe Center. Visit Tufly’s Shoe Center at 16909 Lakeside Hills Plaza, in Omaha. For more information, call 402-6142221 or visit www.TuflyShoeCenter.com. WE

A Woman’s Work
Everyone Needs a Hero
They May Be Closer Than You Think By Sharon Knierim
A friend of ours was making a pretty good talking point last week when she asked, “Where have all the heroes gone?” Nobody really knew the answer to her question. Nor did we know the answer to why they had all disappeared from our daily consciousness. Was it our age group? Do people our age feel that we no longer have to seek out and believe in heroes? No, we decided, we all need heroes. Our group did take note that we greatly admired heroes from the past. These are characters who molded and shaped who we finally became as adults. We tried to emulate their ideals and aim as high as they did. Who is your hero? Is there a person, real or imagined, who you admire, whose traits you turn to when it’s decisionmaking time or when you lack guidance, knowledge, or experience? Well, of course we all wonder what others would do or would have done in particular instances. People seek heroes for different reasons. Matinee idols were once thought of as heroes. Their acting, lifestyles, and overall good looks made them heartthrobs for more than a few young people. Athletes are often thought of as heroes because

of their fantastic physical attributes and accomplishments. World War I and World War II, as well as other military conflicts, brought us lots of heroes as men and women fought on behalf of others. They dedicated and sometimes gave their lives as a result of their resolve and dedication to true human principles. Really, though, what is a hero? What qualities does a man or woman bring to heroism? Long ago, heroes were typically brave men who did things such as going on the Crusades or chasing away the Mongol hordes. Over time, heroes were found among religious leaders or educators or

in the ranks of the most ordinary of people. In all probability, nobody sets out to be a hero loved and admired by millions. Nobody wakes up some morning thinking, “Today’s the day I become a hero!” I imagine that becoming a hero happens most often when circumstances arise and a human (or even an animal) performs a miraculous task that saves lives or changes the course of history in a positive way. Like everyone else, I love and admire heroes, but I would hate to be one myself. Being a hero must be overwhelming and sometimes burdensome. My personal hero is Eleanor Roosevelt. To this day, I have a photograph of her hanging in my office. Beneath her photo is one of the most profound statements attributed to her: “Nobody can make you feel inferior without your consent.” By golly, that is true. Like Mrs. Roosevelt, I spent a lot of my youth thinking my feelings had been hurt. As a young adult, I began to study her and her life, and I learned that she and I shared many inferior feelings, which are crippling unless they are conquered. Who was Eleanor Roosevelt’s hero? I wish I knew. I bet she had at least one hero. When we search for heroes, we may discover that they are closer to us than we think they are. A hero does not need to have attained worldwide fame or amassed a large fortune. A hero does not need to carry a royal title or command a large army. It is quite possible that many heroes do not even know that they carry such a status because no one has ever told them, “You are one of my heroes.” Maybe we should tell people more often how much we admire them or appreciate them for the people they are and the things they do. Let’s speak up when even the smallest or seemingly most insignificant act has a powerful impact on us. If you stop and think about it, heroes are all around us. They are interwoven in our daily lives, and we rub elbows with them often on a daily basis. They are the people who are always ready to pitch in. They are the people who know just the right thing to say during a difficult time. Go find these heroes and let them know how important they are to your life. Who knows? You just might be someone’s hero, too. WE

Vantage Point
Home Is Where the Hearth Is Claxton Fireplace Center By Loretta McCollum
Immediately upon entering the showroom at Claxton Fireplace Center, you want nothing more than to pull up an over-stuffed chair, don a comfy robe, and settle in for the duration in front of one of their more than 20 fireplace displays, many featuring active flames. There’s a style for every taste, from traditional to primitive to contemporary. Perhaps you’re looking for a little backyard bliss? Claxton has you covered. They carry firepits, grills, and smokers. They can also design and install an outdoor fireplace or an outdoor kitchen. Need some shade? The store offers a selection of retractable awnings. Sooty chimney? No problem. They employ the latest technology to ensure that your chimney is functioning properly. In other words, Claxton is everything fire. Dan Claxton has been in the hearth industry since 1986, opening family-owned Claxton Fireplace in 1994. His background is in sales and management, but when he met his future wife, his career path changed. “My fatherin-law was in the insurance business, selling policies on race horses, but he also sold glass fireplace doors at home shows,” Dan explains. “My wife thought I would do insurance, but he convinced me to get into the fireplace business.” Dan’s son, Brandon, is following in his father’s footsteps as a key member of the Claxton staff. Their niche is full-service fireplace production and installation, but a good portion of their business involves remodeling existing fireplaces. “We can give your fireplace an extreme makeover,” says Dan. Converting wood-burning fireplaces to gas is a common choice. For increased efficiency, fireplace inserts are popular. Inserts can be electric, gas-powered, or wood-burning. Some come with fans that reheat the air in your house, decreasing your use of the furnace. Claxton can give your fireplace a fresh look with a refacing. This practice utilizes materials such as ledger panel stone, a natural stone veneer that’s economical and easy to install. Other options are available, and their experienced staff members help customers make the best choice. This business prides itself on providing oldschool customer service while employing modernday procedures. For new installations or renovations, customers receive an in-home project evaluation. Either Dan or Brandon serves as a single point of contact, asking questions and making sure they understand their client’s needs. Computer renderings of the designs are also provided. Ninety percent of the work involved in each job is performed in-house. They do their own stone work and run their own gas lines. Typically, their own employees, not subcontractors, install the products. The same cabinet shop has been creating mantels for the store since 1990.

At Claxton, they truly care about their customers, and they approach every job as if they’re doing it for a family member. Over the years, many customers have become lifelong friends. They also take care of their employees, several of whom have been with the company for years. Dan likens their staff to “old Nebraska football—a consistent team, where you always know what to expect.” He tells his employees, “The customer writes your paycheck, not me.” This approach has served the company well—just ask John and Susan Natterman. The Nattermans hired Claxton to install an outdoor fireplace in their backyard. “I would have them back in a heartbeat,” John says. “I give them the highest marks.” He says that Dan came out two or three times to personally check on the project. “They are a very professional group of people; they’re artisans. The guys who came out to do the work cleaned up after themselves and even planted grass seed around the fireplace.” Claxton also performs routine maintenance and can diagnose problems. Their chimney sweep service is one of only a few that utilize cameras, enabling their technicians to see exactly what’s going on inside the chimney. Dan is an expert when it comes to identifying and resolving fireplace issues. “I can get a good idea of the problem based on things like their description and the neighborhood,” he explains. Enjoying time on the deck can be difficult in the Nebraska heat, particularly if it’s not a covered space. Claxton provides professional service and installation of a variety of awning and shade systems. Customers can even view retractable awnings in action inside the showroom. They also carry a selection of gas grills, pellet grills, and smokers to peruse. Commercial projects are also part of Claxton’s resume. Both the outdoor and inside lobby fireplaces at the downtown Omaha Marriott are beautiful examples of their handiwork, as is the fireplace at Firebirds restaurant. Their designs can be found all over Omaha. Claxton is your one-stop fireplace shop. Visit the showroom at 709 North 132nd Street, in Omaha. Fresh Concept
Treating Chronic Pain Alternatives to Opioids By Kylie Gumpert
People like to say, “no pain, no gain” when they’re talking about tasks like building muscle. However, people with chronic pain—that is, pain that lasts longer than 12 weeks—are all too familiar with pain that offers nothing to gain. Because pain is a subjective feeling that can differ dramatically from person to person, it’s hard to accurately describe, and it’s often impossible to see. People who suffer from chronic pain may also face stigma from friends, family, and even healthcare providers who may not understand the pain someone is experiencing. This stigma can make it difficult for patients to get the help they need, and it’s one of the reasons that chronic pain is notoriously difficult to treat. Around 25 million Americans say they experience chronic or persistent, daily pain, according to a survey the National Institutes of Health conducted in 2012. Nearly 23 million more Americans say they experience serious pain, which, like chronic pain, can increase healthcare costs and a person’s risk of developing an addiction to pain-killing medications and drugs. Chronic pain can stem from injury, disease, infection, disorders, surgery, or even unknown sources that cause pain signals to fire over and over. Common examples of chronic pain, which can vary in intensity, include headaches, back pain, pain from cancer and cancer treatments, and arthritic pain. Fibromyalgia is a particularly frustrating cause of chronic pain, as it impacts the body’s nervous system by sending pain signals to the brain that are very real but don’t correspond to any physical bodily harm. Chronic pain creates discomfort that can be debilitating for weeks, months, and even years. It shares a special relationship with depression and stress—chronic pain can lead to depression and stress, which, in turn, make chronic pain worse. For many years, medications to mask pain were the main treatment options for patients. Opioids are very effective at relieving pain, and some patients do need them to live comfortably, at least temporarily. However, opioids are meant to be used for short periods of time. It’s very easy for patients to become dependent upon opioids if they are used to treat chronic pain. As the United States makes a more coordinated effort to control opioid use and prevent addiction, doctors are focusing on understanding why a patient’s pain is occurring, what makes it better or worse, and how to make changes or explore other treatments, when possible, rather than prescribing opioids to numb it. Today, our understanding of the most effective ways to treat chronic pain include more holistic and non-pharmaceutical approaches, such as acupuncture and biofeedback, as well as more invasive procedures, including neurostimulation and steroid injections. Acupuncture, which involves inserting thin needles into certain points along the body, stimulates nerves
20
September 2018
and can help increase endorphins (the body’s natural painkiller). Biofeedback involves attaching electrical sensors to a patient that measure the body’s electrical response to pain. This makes those responses more concrete through visual and auditory cues—when the patient’s body responds to a painful movement, a machine can make a sound or flash a light. Because some pain can be caused by stress and taut muscles, biofeedback can help patients recognize when they are tense. Myofascial release is a manual therapy technique in which the practitioner identifies and relaxes portions of the myofascial tissue (the membranes that wrap throughout the body to connect and support muscles). The idea is that, because these tissues are interconnected, a stiff area in one part of the body could be referring pain to another part of the body. When the stiffness is relieved, the pain should reduce. Massage therapy is another manual therapy, in which the practitioner exerts pressure on the muscles to relieve tension. There are many types of massage; discuss your pain with your massage therapist to find out which type might be most helpful for you. Other noninvasive treatments include mindful practices such as yoga and meditation. These practices focus on deep breathing, relaxation, clearing the mind, and body awareness. The effect that these practices have on chronic pain likely depends on the source of the pain, but anything that promotes relaxation is likely to reduce stress and muscle tension, which often contribute to chronic pain. Physical therapy, which might include biofeedback, can be effective at increasing strength and flexibility and improving posture. Physical therapists will work with you to develop movements and exercises that are doable and that, over time, can help you regain mobility. It might be hard to imagine moving a particular part of your body when you are in pain, but movement, not rest, is often what will bring you relief. If noninvasive measures and over-the-counter medications aren’t working, injections of various types may be suggested. Botox injections have muscle-relaxing and anesthetic effects that can help relieve pain for longer periods of time. An epidural steroid shot is injected directly into the epidural space along the spine to help reduce inflammation and increase mobility. This is used
www.womensedition.com
to treat back pain or inflammation in the back that is causing leg or arm pain. A nerve block is a combination of an anesthetic like Botox and an anti-inflammatory like a steroid that is injected into or near a nerve that is causing pain. It may take several injections to be effective. Relief is temporary but can last up to several months. Infection, allergic reaction, and nerve damage are possible, as well as “steroid flush,” which involves body temperature increase and flushing that can last several days. Trigger point injections, which might consist of Botox, steroids, or both, can be used for myofascial pain syndromes where a tight knot is causing pain. These injections cause the knot to loosen, bringing relief. Radiofrequency ablation involves a medical professional inserting a microelectrode into troublesome nerves, then sending a current through the electrode to heat up and damage the nerves. It’s longer-lasting than other treatments, with most patients experiencing relief for six months to a year or even up to several years. Side effects are mild and might include leg numbness or irritation or bruising around the insertion area. Neurostimulation involves microelectrodes that stimulate nerves along the spine. These microelectrode medical wires are inserted (through the skin or surgically) into the epidural space. They simulate a regular, tingling sensation that helps mask the feeling of pain. Risks from this procedure include infection, bleeding, and movement of the microelectrodes, which can cause discomfort or pain. New technological advances might mean that neurostimulation can occur noninvasively, through wearable devices that also send pain information to a phone app, where a patient can monitor effectiveness. Chronic pain can be a traumatic experience that leaves patients
feeling hopeless, so giving them information and strategies to take more control over their situation can make a huge difference in their ability to cope. Self-care or self-management is an approach to pain management that emphasizes educating the patient about how pain is perceived by the body and brain as well as strategies for managing related symptoms like depression or frustration, which can make pain worse. As our understanding of chronic pain and treatments broadens, some medical professionals are voicing their concerns about not feeling adequately prepared to help patients navigate treatment options. Thankfully, there are specialized clinics where pain management specialists focus on specific treatments, including surgery, interventions and injections, long-term opioid management, and rehabilitation. You can also ask your doctor about noninvasive methods to treat pain and find out which ones might be good for you to try. Chronic pain can have physical, psychological, and social implications, so it’s important to arm yourself with as many ways as you can to cope, relax, and feel supported. It may take some time to find specialists or treatments that work for you, but it’s important to build a support network of professionals who can help you navigate all of the different ways that chronic pain can impact your life. If you’re feeling helpless in the face of long-lasting pain, know that there are people out there who understand, who will listen, and who will do what they can to get you the treatments you need to live a fulfilling life that is as pain-free as possible. Dentistry Comes to the Old Market By Jackie Byers
Omaha dentists Dr. David Schrad and Dr. James Polerecky have long known each other as neighbors in Dundee. A few years ago, they expanded their relationship to become practice partners in the Old Market. That’s right—a dental office in the Old Market! City View Dental is unlike any dentist’s office in Omaha. It’s located at 1415 Harney Street, and the practice is designed for the comfort and convenience of people who live and work downtown. “I’ve been in private practice since 1996,” Dr. Schrad explains. “My second practice was in Syracuse, Nebraska. I love the feel of the kind of small town where I grew up. Jim, who set up City View Dental, was looking for more time to travel. Joining him here four years ago has worked well. We sometimes covered each other’s patients, so it seems natural that we wound up in practice together.” The capable support staff is praised by their clientele. Jessica, Taylor W., Alix, Kim, Taylor P., and Marquita manage the office, provide dental hygiene, and assist the dentists, making sure that patients receive excellent care.

Heartland Center for Reproductive Medicine Meet Dr. Elizabeth Constance By Elizabeth French
This month, Heartland Center for Reproductive Medicine welcomes Dr. Elizabeth Constance to the practice! Originally from Lawrence, Kansas, Dr. Constance completed her training and OB-GYN residency at the University of Kansas before relocating to Ann Arbor, Michigan. Focusing on reproductive endocrinology and infertility, she completed a three-year fellowship program, consisting of half clinical medicine and half research. During her fellowship, she researched new methods for fertility preservation for women and girls going through cancer treatments such as radiation and chemotherapy, which can severely impact fertility. “Infertility is a very difficult situation for individuals and families to walk through,” she says. “When you add a cancer diagnosis on top of that, it can add extra stress and uncertainty. Anything we can do to take the worry off future fertility, so the woman can just focus on the immediate treatment and healing, is a huge weight off of her shoulders.” For adult women going through chemotherapy or radiation, doctors can harvest healthy eggs from the
ovaries and freeze them before the patient begins treatment. This helps preserve the woman’s chances for fertility in the future, after cancer treatment. However, for young girls who haven’t gone through puberty, there is currently no way to preserve eggs that have not matured enough for fertilization. During her fellowship, Dr. Constance dedicated her research to extracting those premature eggs from young women and preserving them outside of the

Good Looks
Mane Attraction The Latest Hair Trends
By Loretta McCollum
Casual summer has stepped aside to make way for its more sophisticated cousin, autumn. The changing of the seasons is the perfect time to try a new hairstyle or color. Contemporary styles have something for everyone, from bobs, bangs, and braids, to pixies and ponytails. Headscarves and headbands have made a comeback, as have retro waves and bobby pins. To achieve a flattering look, you need to consider your face shape, body type, age, and hair texture, as well as what styling you can do at home. Faces come in lots of shapes. For long faces, a short style with volume or curls makes the face appear wider. A simple blowout can also add width. To elongate a round face, try a high ponytail or a below-chin cut with a deep side part. Bangs and side volume help diminish the heart-shaped face’s characteristic prominent forehead, and side volume creates the illusion of a wider jawline. Soften a square face with wispy bangs, choppy ends, and textured curls. An oval face provides the most versatility. Almost any style will flatter this classic shape. We may not think of body type when we’re making decisions about our hair, but we should. The slim and straight lady should avoid a style that’s too big or too long. Choose a cut with long bangs, layers, and angles that add body. A layered, mediumlength cut with a side part and bangs will bring a soft look to complement a full figure. A short-to-medium cut with layers and
angles that add volume and height is an excellent choice for a petite figure. A tall, athletic build works with most cuts and styles. These ladies can experiment to see which styles flatter their hair texture and face shape. We may not want to admit it, but that fab ’do we totally rocked in our 20s likely won’t look as good at age 40 and beyond. As we mature, the texture of our hair changes, becoming dry and wiry, forcing us to adapt. However, that doesn’t mean that women “of a certain age” can’t still achieve a contemporary look. The old “rules,” like no long hair after 40, no longer apply, and gray is most certainly okay. Flattering styles for mature women will avoid harsh lines and colors, focusing instead on warm, natural tones; layers that soften the face; wispy, feathery textures; and soft curls. Deep conditioning treatments and less frequent shampooing help keep your locks looking younger. ›››

The texture of your hair is perhaps the most important factor to consider when selecting a style, and you’ll need to choose your styling tools and products accordingly. Thick, wavy hair delivers many style options but demands proper product choice to keep those tresses tamed. Loose waves and layers create balance and add dimension while avoiding too much volume. Your stylist can eliminate extra weight with specialized scissors called thinning shears. A longer or curly pixie allows for simplified styling. Avoid over-shampooing thick hair. It takes longer for natural oils to travel from the scalp to the ends of the hair, so wash your hair once every few days, and don’t use a volumizing conditioner. Let thick hair air-dry a bit before using the hair dryer; then use a diffuser and a round, boar bristle brush to control frizz and add shine. A detangling brush, deep conditioners, and hot oil treatments are essential for very coarse, kinky hair. Fine hair is typically limp, calling for a style that adds volume and dimension. A blunt cut adds weight and bulk around the perimeter, and an asymmetrical cut provides an illusion of depth. A layered bob or a long pixie are good choices. Use a heat protectant and a volumizer when styling fine hair, and choose mousse over gel. A good wax spray will hold your style in place. The latest color trends feature iridescent and neon hues, deeper reds, and blacker blacks. How about rainbow hair for brunettes? The oil slick technique applies layers of dark, iridescent rainbow tones throughout the hair, mimicking the colors in an actual oil slick. For a sunlit look, the floodlight process applies highlights directly on top of a tint without rinsing in between. For an ombre effect, where the bottom of the hair is lighter than the top, the balayage technique paints highlighter into the hair in an upward, sweeping motion, for gorgeous results. Hair stylists are professionals who know what cuts and styles will work with your hair type. It’s important to trust your stylist and be honest about how much product you’re willing to use and what daily maintenance you have time to do. There’s no use in getting a great cut that takes 20 minutes to style each morning if you’re a wash-and-wear gal! This fall, have an open conversation with your stylist to select the right color, cut, and style for your fresh look. Discover Your Passion Take a Different Path I was visiting with a friend the other day, and she complained that her life was getting stagnant. Her kids were grown and out of the house, her job was the “same old, same old,” and her husband always wanted to go to the same old restaurants, always order the same thing, and never try anything new. She felt like she was in a rut. She loudly exclaimed, “There is no passion in my life—I think I’m going to die of boredom!” “There is no greater power in the world than the zest of a postmenopausal woman.” —Margaret Mead Finding something—anything—to become passionate about can challenge even the most positive of us out there. After all, you can’t just pick a topic and decide it’s going to be your next passion, right? “Follow your heart, your instincts. People might try to dissuade you from your passion, but no one can live your life but you.” —Olympia Dukakis As I continued my conversation with my friend, I discovered that she had a passion that she had decided to throw away years ago. She loved to paint when she was a child. When she went to college, everyone around her convinced her that she couldn’t “make a living as an artist.” Back then, she made the decision that

they were right—although the only time that she felt really alive was when she was painting. “One big reason I do what I do is there’s no two days alike. It’s not work for me. It’s my life. My passion.” —Emeril Lagasse She did what so many of us do. She decided to take the safe path, the path that would pay the bills and eventually help pay for the kids’ college. Yes, this worked for quite some time—she actually felt fulfilled doing what she had decided to do. But now? It wasn’t working anymore. She had come to that fork in the road. “If I had to choose my driving force, it would be passion.” —Anita Roddick So, what would you do? Would you stay on the straight and narrow and continue your life as it has been? Or, would you decide that perhaps today is the day to take a different path? How does a person decide? The process should be no different than any other decision that we make in our lives.Yes, it is bigger.Yes, it has the possibility of shaking up the status quo. But, it all comes down to this: We simply ask ourselves, which path feels better? “I’d rather not sing than sing quiet.” —Janis Joplin

House Guests and House Rules How Do You Handle a Drop-In? The summer is nearly over. As I write this, the only summer holiday left is Labor Day. My property abuts a state park and is on a lake, so the end of summer means it is nearly time for a final cleaning of the boat and outdoor furniture for winter storage. It also indicates a decrease in the number of drop-in visitors. This is a good thing. I like having visitors…as long as I know they are coming. We bought this property for its ambience. We have room, both indoors and outdoors, to entertain. Family is always welcome. I encourage the kids to invite their friends…as long as they let me know they are coming. Sometimes, people abuse the privilege of being invited and assume that, if they were invited once, they are automatically invited every weekend or holiday. At a recent “gripe gathering,” several good friends shared similar feelings. “I love having people over, and I expect it when we’re at the lake,” Jenny commented, “but when we’re having a private gathering, and someone just shows up? It’s annoying!” Connie chimed in, “What can you do when you have planned for a certain number—food, drink, snacks—and people drop in unexpectedly?” “I know what you mean,” Toni added. “We had family over after a funeral, and a bunch of people showed up in swimsuits to party on the water.” “I can top that,” Mari claimed. “We came home from being out of town and found casual acquaintances making themselves at home on the patio. They even got the lawn games out!”
"Raising kids went exactly the way I planned." —Nobody Ever
Don’t Give Up On Your Teen!
By Marge Shoemaker, BS, MS
Most of us would make the best of the situation. Really good friends would recognize the gaffe, apologize for interrupting, and leave...or you could tell them, frankly, that it’s not a good time. We agreed that there are times we really want a quiet weekend and don’t appreciate drop-in guests. We also agreed that, even on those days, we would probably smile and ask them in. Roni asked about house rules. We agreed that it’s our house, so it should be our rules. I iterated my two main rules. The first: This is a peaceful place…leave your problems at home. The second: Parents, watch out for your own kids. Carol proclaimed, “If they don’t like your rules, they don’t have to come? I like that!” Suzy mentioned a guest who always imposed her rules on everyone else. That makes no sense! Our house, our rules. We all concurred that really good friends usually don’t come empty-handed or unannounced. They usually come for a relaxing time and don’t make a fuss. At that, we declared our gripe session over, changed the topic, and poured another glass of wine. How do you handle unexpected guests? What are your rules? I’m like a duck: quiet and serene on the surface but paddling like crazy underneath to make things work. Editor’s Note: Marge has three children, two grandchildren, and numerous nieces and nephews. She holds advanced degrees in education. Her life’s mission is to teach everyone that kids are people, too. WE

Designing With Fire Fun, Fashionable, and Energy-Efficient Fireplaces
By Robyn V. Powell
During the Victorian era, it was common for homes to have a fireplace in every room. After central heating became the primary source of heating in the home, modern homes were typically built with just one fireplace. Fast forward to 2018, and the fireplace is once again taking center stage, showing up in bathrooms, bedrooms, kitchens, and outdoor living spaces. More and more homeowners are recognizing that few design elements can transform the character of a room with the same dramatic impact as a fireplace. Home buyers list fireplaces as one of their most sought-after features, and realtors tell us that having one in a home has a strong positive effect on selling price. Whether you prefer traditional, transitional, or contemporary styling, adding a fireplace will enhance your lifestyle and add to your home’s value. A fireplace is often the most commanding feature in any room. Naturally, you want this element to complement the rest of your home. The large, outdated brick surrounds in some older homes are great candidates for makeovers. Remodeling a fireplace wall by wrapping it with granite, tile, or stone can totally change a room’s character. For older fireplaces that suffer from chimney draft, installing a wood, pellet, or gas insert adds instant warmth and style, and it reduces heating costs. If you’re building a home or plan to introduce a fireplace into a new space in your current home, options include gas, electric, and
wood-burning fireplaces. Gas units are efficient and can be used anywhere in your home where there is access to a gas source. (If you don’t currently have access to gas in your home, a professional can install a gas line.) A gas fireplace doesn’t produce smoke or odors, but the flames do produce pollutants such as carbon monoxide. The safest and most efficient method is the dual vent system, known as direct vent, where pipes go up a flue or straight through an opening in an exterior wall, sending harmful emissions outside. Vent-free models have no flue, allowing them to be placed on an interior wall. Because both heat and pollutants remain in the room, ventless units are equipped with an oxygen sensor to shut them off if the air quality drops below a safe level. Electric fireplaces can be installed almost anywhere, including as an insert in an existing fireplace. They work like an electric stove or space heater—power runs through a heating element, ›››

which produces the heat. The logs and flames are simulated but look better than ever. No heat is lost through a flue, and they are safer for kids and pets because the heating element is not exposed. Perhaps most notably, they produce no harmful emissions in the room. Typically, gas and electric fireplaces are more energy-efficient than wood-burning units. However, as manufacturers continue to make technological improvements in combustion engineering, the efficiency of wood-burning fireplaces has improved. Wood burns longer and more completely, allowing owners to save on wood costs and spend less time stoking and cleaning the ash drawer. Installing glass doors reduces heat loss and keeps sparks safely inside. Another efficient wood-burning option is a freestanding or mounted stove that uses either natural gas or propane. Some models are vent-free and do not require a chimney or flue for safe operation, eliminating heat loss through a chimney. Some of this year’s most popular design trends include a linear wall of fire burning up through a bed of glass or stones. A sparkling bed of tempered glass chips brings a modern look, while calico-colored stones provide rustic appeal. These products have been designed to withstand prolonged exposure to high heat without cracking, fading, or producing unpleasant odors and are ideal for indoor and outdoor use. Traditionalists can choose from various log styles, including popular driftwood. Stylish built-in wood storage is one of the year’s hottest trends, even popping up next to gas fireplaces. Extending stone, concrete, or tile to the ceiling or across an entire wall creates a contemporary aesthetic. Thanks to technology, you can achieve this clean, seamless styling with flammable materials such as wallpaper and wood. These products eliminate the need for a heat barrier mantel, allowing artwork, flat screens, and fire to live together in harmony. For the ultimate in luxury, a see-through gas fireplace above the master suite bathtub enables you to enjoy the flames from the adjacent bedroom. Use a remote to control flame height and room temperature without lifting your head from the pillow. Adding a fireplace creates a warm and inviting ambience. For inspiration, visit your local fireplace store. When the first chilly night of winter arrives, you can cozy up in front of your new blazing hearth. Oncology Associates Caring for People When They Need It Most
By Roxy Orr
When your life is turned upside down by a cancer diagnosis, how will you feel? Lost, scared, wondering who you can trust with your treatment? I recently sat down with Dr. Irina Popa, the sole proprietor of Oncology Associates, P.C., to understand what makes her practice special to her patients and staff. The team at Oncology Associates, she says, understands
how essential it is to see new patients as quickly as possible. At Oncology Associates, they strive to make sure their patients are cared for physically and emotionally from that very first assessment. The practice was created in 1985 by Dr. Herbert Hartman, who was committed to creating a practice built on compassion. He was the sole practitioner covering Methodist Hospital and was one of several specialists to start outreach clinics in Nebraska at the request of Methodist. Dr. Popa recalls many stories told of Dr. Hartman’s adventures getting to and from the clinics, especially in Nebraska’s winters. “One time, the ice and snow were so bad that he was driving with his head out the window to be able to see the road,” she shares. Dr. Hartman continued serving these clinics even after Methodist ended the program. Today, Oncology Associates still has outreach clinics in Norfolk and Blair. Dr. Popa has been a partner at Oncology Associates for 13 years. She credits Dr. Hartman with teaching her the business side of the practice, which has been extremely beneficial. She reviews every case to make sure she is familiar with each patient’s history and treatment. “Each patient is an individual and reacts differently to disease and treatment,” she notes. “I think I work 85 percent of my waking hours,” she says. So much of Dr. Popa’s time is devoted to the practice, that her husband and two sons come up to the office on the weekends just to have the family together. Dr. Popa makes every effort to keep her patients’ well-being in mind. This is one reason that it’s so important to screen new patients as soon as possible— people who are newly diagnosed with cancer don’t want to wait for a consultation. Dr. Popa is often able to see patients within a day or two; sometimes, even the very same day she gets the referral. The next step is for the staff to collect data as quickly and completely as possible, so a treatment plan can be developed. Honest communication with patients is vital to make sure they understand their treatments. Once a treatment plan is in place, the team meets as a group to make sure that everyone

understands the plan. One of the reasons that the patients at Oncology Associates do well during treatment, Dr. Popa asserts, is the attention that the staff pays to the adverse reactions that chemotherapy can cause. “We don’t just write the order without looking into what the toxicities are,” she explains. “We are very proactive and give certain treatments up front to avoid adverse reactions.” Dr. Popa is triple-boarded in oncology, hematology, and internal medicine. It is a great advantage, she says, to have all of these perspectives when dealing with complex cases. She is proud of her knowledgeable and caring team of advanced practitioners and nurses. These professionals make Oncology Associates a caring and compassionate place. Dr. Popa is excited to announce that Dr. Tim Dorius joined the practice in August. He and his family are happy to return to the Omaha area, which they grew to love while he was advancing his medical training at UNMC. Dr. Dorius did a residency in internal medicine and is board-certified in oncology and hematology. “He is a wonderful physician,” Dr. Popa says. “He’s respected in the Omaha medical community, and I’m looking forward to having him as part of the practice.” If a cancer diagnosis has turned your life upside down, consider Oncology Associates, P.C. These caring practitioners are committed to using their multidisciplinary expertise to provide the most advanced treatment and care with empathy and compassion. Professional Holiday Decorating Have Your Dream Display Installed by a Pro
By Anne Yankus
It feels like we only just finished back-to-school shopping, so winter holiday decorating is probably not at the top of your to-do list. Before you flip to the next page, though, ask yourself this: Do you like to have a spectacularly decorated house for all your neighbors and holiday guests come December? If the answer is yes, then you may want to consider saving yourself time and energy by hiring a professional holiday decorator. What are the benefits of hiring a professional holiday decorator? Not only will you be freeing up additional time to spend with all the friends and family you plan to host over the season, but you’ll avoid the surprisingly common risk of injuring yourself while putting up said decorations. Every year, more than 12,000 people are rushed to the emergency room for various decorating-related incidents that range from falling off roofs and ladders to electrocution and burns. Hiring a professional is also helpful if you have a multiple-story house or tall trees in your yard and maybe don’t have a 20-foot ladder lying around in your garage. Hiring a pro also means you won’t have to hold the annual debate over whether to risk getting on the snowy roof to take down your decorations after the holidays or being “those” neighbors who leave their decorations up until after the spring thaw in March! Professional decorating services both set up and take down decorations in a timely manner.
Another great benefit of hiring professionals is that they can save you the cost of buying, storing, and replacing decorations in the long run, as many decorating companies also rent out decorations in addition to their services in setting them up and taking them down. This can give you a huge leg up on keeping up with the latest and most popular holiday decoration styles when they change from year to year. Once you’re sold on the idea of hiring a professional decorator, you need to start looking for a local service right away. Since the decorating season is so short, professionals tend to book up quickly and are usually full by mid-November (if not earlier). You’ll want to pick your pros and book them as soon as possible. Keep in mind, when choosing a service, that some professionals will only install their own decorations, not any electrical decorations you may already own. This is for liability reasons. for decoration rentals, make sure to pick a service that is willing to work with what you already have. The price of professional decorating services can range from a few hundred to several thousand dollars, so you’ll want to choose a service that will operate within your budget. While some services do offer indoor decorating, what we all really want to know is what is involved in getting a top-notch outdoor lighting display. As I mentioned previously, some companies will only work with their own rental equipment. At the same time, this means that most companies can guarantee that all lights will function for the whole season. These are often high-performance, commercial-grade bulbs. Some companies can even set up advanced displays that can be controlled through your smartphone or tablet. However elaborate a setup you choose, most companies do offer an all-inclusive package: They will provide all necessary cables, extension cords, lights, and greenery, and they will travel to your house for full setup and teardown. With exterior decoration services, you don’t even need to be home for them to start working! As with any professional decorating service, it helps to have at least a vague idea of what sort of decorating you want to have done. There’s always the simple, traditional string of colored or icicle lights across your roofline, but why do that when you can go all out? Garlands made of real (or at least realistic) greenery and sometimes interwoven with a simple light string have become popular in recent years, and pineapple mini-Christmas trees are growing in popularity, as well. For outdoor decorations, things like solar lights (which turn on immediately when the sun sets), light-up gift box decorations (for your yard or porch), snow-splashed garlands (strung loosely along your eaves or over your door’s archway), oversized ornaments, balls of string lights, and mason jar lights are what more people want. Rustic accents are in! Hopefully, you are now fully prepared and excited to get a running start on decorating for this holiday season. Remember, work smarter, not harder— hire a professional to get the best holiday display your guests and neighbors have ever seen! Prostate Care
Help Men Help Themselves
By Connie Keck
Just as women are encouraged to get regular health screenings, men also need to think about their health. Some men, like some women, would rather put off visiting with a physician until they feel it is absolutely necessary. As many of us have learned, however, it is best to be proactive about our health. The same goes for the men in our lives; after all, as we age, so do our husbands, sons, and brothers. Sometime during their lives, most men can count on a prostate condition—more than 30 million men currently have one. The primary function of the prostate is to produce and secrete some of the alkaline seminal fluid during ejaculation. It also pumps semen through the penis, filters and removes toxins for protection of the sperm, and plays a valuable role in protecting the urethra from urinary tract infection. According to the National Cancer Institute, the prostate-specific antigen (PSA) blood test shows the quantity of a specific protein present in the blood. This test is used for prostate cancer screening and to monitor prostate health. PSA is made by the prostate, and elevated levels in the blood can indicate prostatitis, benign prostatic hyperplasia, or prostate cancer. The first is more common in younger men; the last two are more common as men grow older. Prostatitis, an inflammation, can be a bacterial infection or a noninfectious condition. Both cause pain with urination and ejaculation. Bacterial infection can also cause fever, chills, and muscle aches. Treatment for a bacterial infection is antibiotics.

About one in nine men will be diagnosed with prostate cancer in his lifetime. Prostate cancer is rare in men younger than 40 years. It develops mainly in older men; about 60 percent of cases are diagnosed in men aged 65 years or older. Rates of prostate cancer are much higher in men of African descent than in other ethnic groups. Risk factors besides age and race include family history and obesity. Doctors know that prostate cancer begins when some cells in the prostate become abnormal, but it’s not clear what causes it. A few of the most common symptoms of prostate cancer are fatigue, incontinence, erectile dysfunction, and bone pain. Keep track of your symptoms and share how you’re feeling with your doctor. Though prostate cancer is the second leading cause of cancer death in American men (lung cancer is the first), most men who develop prostate cancer do not die from it. In fact, it is one of the most treatable malignancies if it is caught early. Routine screening is essential. Another testing option for prostate cancer is a non-invasive 3T MRI scan. 3T MRI has a stronger magnet and produces signifigantly better images of the body’s organs than other types of MRI. If your PSA level is elevated, you may want to schedule a 3T MRI. The MRI prostate
www.womensedition.com
scan lasts approximately 20 minutes. This technology has made the painful endorectal probe virtually obsolete. The results can be sent directly to your urologist to help determine your diagnosis and treatment plan. More than 99 percent of prostate cancers develop in the gland cells. This type of prostate cancer is called adenocarcinoma. There are several strategies for treating prostate adenocarcinoma, and more than one may be used in a single patient. The Cancer Treatment Centers of America describe the following treatment strategies for prostate adenocarcinoma: Active surveillance: Patients are monitored closely for changes in the progression of their cancer and are tested at regular intervals. Surgery: The prostate is surgically removed through a lower abdominal incision or, if done laparoscopically or robotically, through several small incisions in the lower abdomen. The surgeon may also remove lymph nodes around the area of the prostate. Chemotherapy: An option for patients with advanced prostate cancer. Radiation therapy: A machine focuses radiation on the prostate to kill cancer cells. Treatment generally involves 15-minute sessions, five days a week, for several weeks. Brachytherapy is a form of radiation therapy. It places tiny pieces of radioactive material, called seeds or pellets, either in a tumor or in the surgical cavity left after a tumor has been removed. Hormone therapy: Hormones are sometimes used, along with surgery or radiation, to treat patients with higher risk early-stage prostate cancer. Immunology: Also known as a “cancer vaccine,” this therapy may be used to treat patients with advanced prostate cancer who are no longer helped by hormone therapy. Post-treatment therapy will vary depending on the treatment options used. When consulting a physician, it is important to discuss not only treatment options but also the associated post-treatment therapy, including the length of time involved for the therapy, the expected recovery period, and what temporary or lasting side effects may result with each treatment option. The prostate is a powerhouse—a remarkable gland with huge repercussions in a man’s quality of life. Encourage the men in your life to take care of themselves! Have them ask their physicians about prostate health. An Inside Look
Feel the Need to “Go” Too Often? Adult & Pediatric Urology is Here to Help By Elizabeth French
The practitioners at Adult and Pediatric Urology and Urogynecology have made it their mission to enhance their patients’ quality of life. Featuring nine partners, including Dr. Amandeep Mahal, the prestigious clinic treats all manner of urologic conditions in women and men. Serving the entire metro area, the practice includes pediatric providers that visit clinics throughout Omaha. A vast portion of Dr. Mahal’s patients are in their 30s and 40s and have bladder issues (such as leaking) when going about their daily activities. He also treats a lot of women going through menopause, which may cause problems like constipation and pain. Sometimes, symptoms that are caused by a hernia or prolapsed bladder can be treated with simple procedures that (sometimes) take less than 20 minutes. In fact, according to Dr. Mahal, national studies show that 11 percent of women will eventually undergo one of the procedures he offers. When treating new patients, Dr. Mahal makes a point of getting to know their lives before diving into a treatment regimen. He wants to sit down with each patient individually, especially the patient’s first time in the clinic, to truly understand the full picture. “Being a male gynecologist, you’re always hyper-aware,” he points out. “It’s an honor and a privilege to take care of women during a very vulnerable time, in very vulnerable positions. That’s always been my approach. The more they are comfortable discussing with me, the more I’ll know about their disease state and how I can help.” One of his patients, Deb Hallstrom, was extremely impressed by his bedside manner as well as his expertise. She saw him in August of 2017. “The first time I went in, he spent an hour with me, gave me three options of what I could do, and told me about the new surgery they have,” Deb says. He made sure that she had the information she needed to decide which option was best for her. She chose the new procedure and had it done within two weeks. “He came that night to check on me, and he came the next morning. I saw him again after that and he was just a great doctor. Very personable.” Since then, she has given his name to many friends and family, as she has “nothing but good things to say about him.” Born in California, Dr. Mahal attended medical school at Creighton University, where he fell in love with the city of Omaha. After studying OB-GYN for four years at the University of Iowa, he completed a fellowship at Stanford, where he continued developing his craft. Now, he is innovating new ways of treating bladder and pelvic floor issues. While Omaha is a hub for medical education, Dr. Mahal explains that most clinical trials in the city are conducted by smaller to medium-sized practices. “We’ve
48
September 2018
opened our own research center, in which Dr. Amandeep Mahal five staff members and all of our doctors participate,” Dr. Mahal says. “We’re usually running anywhere between four and 12 clinical trials. We feel that it’s important to be on top of the new technologies that are coming out, which offer novel therapies for people.” One popular treatment has been vaginal laser resurfacing. “Essentially,” Dr. Mahal explains, “it’s like skin resurfacing, when it becomes damaged by scar tissue or acne or aging. We do micro-pore incisions, making small incisions across the entire area, but it’s really lasers heating these areas.” Think of the tissue as a grassy field: When the dirt is packed tight, seeds can’t be planted and essential nutrients can’t be absorbed. The area needs aeration for normal nutrients and water to get in. “The same idea happens with the skin’s surface. It’s opening those areas, stimulating the growth of healthy skin tissues. The idea is to help with scar tissue, pain, or dryness in the vagina, especially after menopause. This is a great therapy for women who are breast cancer survivors, where estrogen therapy is not an option.” Adult and Pediatric Urology and Urogynecology also uses InterStim, a device that uses a small wire attached to the nerves in the bladder to stop them from firing. Dr. Mahal attests that stimulating the nerve every few seconds has been shown to help reset the bladder and alleviate the constant urge to urinate. While this minimally invasive procedure has been performed for over 25 years, Adult and Pediatric Urology and Urogynecology is one of the few clinics in Omaha to offer it. If you have issues with your bladder, Dr. Mahal can offer a multitude of advanced therapies that are extremely effective. Contact Adult and Pediatric Urology and Urogynecology today for a consultation at 402-3997817. For more information, visit adultpediatricuro.com. The Omaha office is located at 10707 Pacific Street, Suite 101. The Council Bluffs office is located at 3434 West Broadway, Suite 102. WE

Stock Market Indexes What a Wonderful World
By George Morgan
By the 1890s, the American stock market had developed into what could be considered a legitimate industry. There was a formal organization of traders, and written agreements were in place directing how business was to be conducted. In 1886, Charles Dow and Edward Jones had started to put out a short newspaper every afternoon that discussed the day’s market activity. Even with this information available, traders and investors craved a mechanism that would give them a better sense of what the market was doing. In 1896, to meet this need, Dow and Jones developed what we know today as the Dow Jones Industrial Average (the Dow, for short). At inception, the Dow contained only 12 stocks. During the last few decades of the 19th century, railroads were the dominant sector of the American economy, and they were extremely capital-intensive. Thus, all 12 companies included in the initial Dow were railroad companies. Over the next decade, however, the economy changed significantly—all the railroads in the index were replaced by manufacturing firms like General Electric and Standard Oil. In the beginning, the math was simple: the prices of the 12 stocks were added up and divided by 12. However, as time passed, price movements provided distorted results, and a divisor was developed to alleviate the problem. The Dow is what is known as “price weighted,” which means that the companies with the highest-priced stock have the greatest influence over the index. The Dow currently consists of 30 stocks. A committee decides which stocks to include in the index. The selection criteria are that the companies must be of major size and be representative of a diverse cross-section of the economy. You would recognize almost all the stocks; a short list includes Disney, JPMorgan Chase, Boeing, Coca-Cola, and IBM. When the committee feels that a company is no longer of sufficient quality or that it represents a declining business sector, the company is removed and replaced. Recently, General Electric was removed from the Dow and was replaced by Walgreens. The other stock market index that is recognized by most Americans is the S&P 500 (S&P is short for Standard & Poor). As the name implies, the index contains 500 companies. The index came into being in its present form in 1957. It was developed because some brokers were concerned that the Dow was too narrow in its scope and did not contain the medium and small companies that make up the bulk of the American economy. The S&P 500 is specifically designed to blanket a larger portion of the business world. The S&P 500 is weighted by market capitalization. The market cap of a stock is simply the current price of the stock times the number of shares outstanding. Thus, the biggest companies in the country determine the direction and speed of the index. Currently, these
50
September 2018
companies are the tech giants Amazon, Apple, Alphabet (Google’s parent company), Microsoft, and Facebook. The change of the S&P 500 index value is determined by how the stock prices of these companies move. This is important to individual investors because millions of Americans own index funds that hold the same stocks as the S&P 500 and in the same proportions. For these investors, their life savings may depend on how well these companies perform. The 500 companies represented in the S&P 500 come from all sectors of the economy. They represent companies that manufacture cars, provide life insurance, keep the American public entertained, and create drugs to cure our ills. The value of the companies in the index represents 85 percent of the total value of the American stock market. This provides a measure of safety for investors who own S&P 500 index funds. If the bulk of the companies in the S&P 500 were to declare bankruptcy at once, there would be nothing left of the American economy. (Don’t worry—the chance of that happening is quite small.) The stocks in the S&P 500 are determined using a formula that includes such data points as revenue, profitability, and other financial measures. This ensures that the index will contain a broad and strong crosssection of companies. During discussion of stock market indexes, the issue of relative performance usually comes up—that is, which of the indexes is performing better. The short answer is that these two indexes tend to move in very close tandem. The main reason for this is that all the stocks in the Dow are in the top 50 of the S&P 500. There are over 100 indexes currently available to the investing public, and most of them consist of small companies or stocks in a single industry. Their main function is to provide marketing gimmicks for brokers; most are so narrow in scope as to be of no value to the average investor. To keep an eye on the market, keep an eye on the Dow Jones Industrial Average and the S&P 500. Editor’s Note: Professor Morgan has over 40 years’ experience in the investment field, both as a university professor and as a financial advisor. He currently serves on the faculty at the University of Nebraska at Omaha, where he directs a program designed to educate 401(k) plan participants on how to improve their investment strategy. Unhappy Feet? Modern Podiatry Can Help
By Linda Barnes
We humans relish our mobility as people who must be on the move. Whether we walk, run, pedal, or drive, healthy feet get us where we’re going every day. Our miraculously engineered feet help keep us stable, act as shock absorbers, and are the engines of our mobility. A painful foot or ankle can seriously cramp our style. You’d think we would give the most excellent care and consideration to our precious feet. Instead, we hardly give them a thought until they cause us pain. When that happens, podiatrists come to our rescue. Podiatrists complete four years of training in a podiatric medical school and three years of hospital residency training to achieve the status of DPM (doctor of podiatric medicine). Additional training, clinical experience, and passing a board examination earn them certification in foot and ankle surgery. Every foot is constructed of 26 bones, along with joints, muscles, nerves, tendons, and ligaments. Skin and nails complete the picture. With so many intricate parts and the punishing use we inflict on our feet, is it any wonder that we end up with fractures, sprains, bunions, and bone spurs? According to the National Institutes of Health, we are likely to walk 75,000 miles by age 50. That’s approximately equal to walking around the world three times! It is no surprise, then, that 77 percent of adults say they have had a foot ailment and half say they have foot pain. Older people tend to blame their foot pain on the aging
52
September 2018
process. Podiatrists warn, however, that pain is a signal of a problem and that it is better to deal with such issues sooner rather than later. With early treatment, foot pain can often be addressed with more conservative measures, and invasive measure like surgery can be avoided. Since podiatrists specialize in the treatment of the foot, ankle, and related structures of the leg, they are your go-to doctors for foot pain. From a fungal infection of the skin or nails to a serious injury, a podiatrist can diagnose and treat a myriad of conditions. Treatment might include prescribing meds for skin issues or performing surgery for a fracture or deformity. One of the most common culprits of foot pain is improper footwear. Arthritis, excess weight, and diabetes are also possible factors in foot problems. Some of the most common issues include athlete’s foot, bunions, neuropathy, plantar fasciitis, bone
www.womensedition.com
spurs, corns, joint inflammation, and ankle sprain. Women seem particularly prone to corns and bunions. A bunion is a prominent bump on the joint at the base of your big toe. Shoes that are too tight, too narrow, or too pointed contribute to the formation of these painful bumps, as does inflammatory arthritis. Hereditary factors may also come into play, since bunions tend to run in families. Bunions are progressive, so early treatment for bunions is best to slow or stop the development of further damage to the joint. Conservative measures may provide relief and might include proper footwear and custom inserts (orthotics). Surgery may be an option if simpler treatments fail and the pain is chronic. Those with diabetes need to be especially vigilant where foot issues are concerned. Since diabetes affects blood vessels and can result in poor blood flow, foot injuries are more challenging to heal. In addition, diabetes can cause nerve damage (neuropathy), so pain from a cut or scratch might not alert a person to possible danger. Daily self-inspection for cuts or other problems is recommended, with early treatment a must. Plantar fasciitis involves heel pain from inflammation of the ligament that runs under the foot. It is often seen in people in the 40–60 year range (or in younger people, if they are runners). The condition is quite painful and may be caused by running, being overweight, or standing for long periods of time. Conservative treatment of plantar fasciitis may include physical therapy, orthotics, overthe-counter pain medications, and night splints. Imaging technology may rule out other causes. If conservative measures fail, the podiatrist might use an injection into the tissue to provide temporary pain relief. Other treatments stimulate healing or remove scar tissue. Injuries and fractures bring many of us to the podiatrist for the first time. Ankle sprain involves ligaments that have stretched or torn. Arthritis also may affect the ankle just as it does other joints. Arthroscopy using small incisions may be used to diagnose and treat ankle issues without being too invasive. Really, aren’t there way too many exciting things out there to let foot pain sideline you? If treated early by a podiatrist, most conditions need not put a damper on your mobility. Ask your family physician or a trusted friend for a recommendation and get back on track, free from pain! Sources for this article included: apma.org, ncbi.nlm.nih.gov, and aarp.org. WE

Sidelined by a Sports Injury? Orthopedic Treatment Gets You Back in the Game
By Leslie Byrne
There is no denying the physical and mental benefits of participating in sports at any age, but the more active we are in sports, the greater our risk of sports injuries. Don’t think these only happen to professional athletes! Sports injuries can happen to anyone, from teens on school teams to adults playing pick-up games on the weekend. Fortunately, increased treatment options mean that most athletes are sidelined for only a short period of time. Some of the most common sports injuries are injuries to knees, shoulders, and ankles. Injured athletes have several treatment options to discuss with their healthcare providers. A common treatment plan for serious joint injuries is replacement surgery, though a total joint replacement is never the first option. A visit to an orthopedic surgeon will provide you with a full description of the latest replacement techniques. Although joint replacement surgery has been around for more than 50 years, new procedures mean the incisions are smaller and recovery time is shorter. Even with improvements in surgery, recovery time for a joint replacement can be up to six months, and physical therapy during recovery is required. Partial joint replacement or joint resurfacing is a viable option for some athletes. Because the procedure is not as extreme as total joint replacement, the recovery time is shorter. Many times, an injured tendon, muscle, or ligament can be
repaired with a smaller surgical procedure. Arthroscopy is a surgical technique in which small incisions permit a tiny camera to enter the body. This allows surgeons to see and fix tears in soft tissues around the joint to repair damaged cartilage without having to make a large incision. Injections into an injured joint can provide pain relief for several months and must be repeated for continued relief. Corticosteroid injections are often the first attempted treatment for osteoarthritis symptoms; relief usually lasts two to three months. Hyaluronic acid injections, better known as gel injections, can provide up to six months of relief before having to be repeated. They are usually used when corticosteroid injections do not provide enough pain relief to the injured area. The gel is chemically similar to natural joint fluid and works as a lubricant to surround the joint. Most insurance companies only approve these injections every six

months, so be sure to check with your insurance company regarding coverage. Some doctors are beginning to use cold injections to freeze nerves around an injured knee before knee replacement surgery to decrease pain and speed up recovery. Cold injections are also being used as a treatment for osteoarthritis for patients who do not qualify for (or do not desire) knee replacement surgery. A new and exciting area of sports injury treatment is stem cell therapy. Although stem cell therapy is becoming more popular, it is not considered standard treatment by sports medicine doctors and is not covered by most insurance companies. Don’t let that stop you from finding out how it might help with your injury! Be prepared to ask a lot of questions. At this time, doctors use stem cell treatment to treat damaged tendons, ligaments, muscles, and cartilage. Surgeons may introduce stem cells into the body in different ways. A surgical procedure can place the stem cells directly onto a torn ligament, tendon, or bone. The doctor may instead stitch an injury with a thread-like material, coated in stem cells, that dissolves over time. Another option is to inject stem cells directly into the affected joint. Though it’s not as flashy as stem cells and surgery, it is important to not dismiss physical therapy. Most surgical treatments require physical therapy during recovery, but physical therapy alone is often the only treatment an athlete needs to recover from injury. Physical therapy helps rebuild strength and movement in the injured area. It can also help manage pain and can prevent permanent damage or recurring pain. Physical therapists teach exercises and stretches that promote healing and strength. It can take a few days to several months to resolve a sports injury with physical therapy. Proper training, a good warm-up routine, and sport-specific conditioning can help athletes avoid many sports injuries. It is a good idea for an athlete to consult with a physical therapist before an injury occurs. A trained physical therapist can teach athletes techniques to avoid fatigue or overuse of a joint before it becomes a problem. Don’t let a sports injury stop you from playing the sport you love! With all the treatment options available, an injury shouldn’t keep you out of the game for long. Keep On Rollin’ Tire Maintenance Keeps You Safe and Saves You Money By Brennan Hallock
Our vehicles are complex machines. Sometimes, we focus so intently on the moving and computerized parts that we neglect some of the most fundamental pieces of equipment: the tires. Of course, you flat-out can’t drive a car without tires, but that’s not the only reason to pay attention to them. Proper tire selection and maintenance also impact handling, safety, and fuel economy. To avoid ending up on the side of the road with a flat tire (or worse!), there are a few things you should do to keep your tires in good working order. Keeping your tires at the recommended air pressure goes a long way toward extending the life of your tires, keeping you safe, and improving your fuel economy. This is an easy task, and you should do it yourself once a month—there’s no need to hire a mechanic to determine whether your tires are under- or overinflated. To check the air pressure in your tires, you can use a pressure gauge that can be purchased for less than $10 from any auto parts store. Simply compare the pressure reading on the gauge to the recommended pressure listed on your tires or on the inside of the door on your vehicle (if the recommended pressures are different, use the one listed on your vehicle). Tires tend to gain pressure when they are hot, so make sure to check your tire pressure when your tires are cold (that is, when you have not been driving on them).

Next, it’s time to head to the mechanic for tire rotation, balancing, and alignment. Tire rotation means moving tires from one location to another (like rear tires to the front and front tires to the rear). Tires wear differently based on their location. For example, if you have a front-wheel drive vehicle, your front tires will wear faster, as they are the ones transferring power to the road. In order to have your tires wear as evenly as possible, they must be rotated. Typically, you should rotate your tires about every 5,000–7,500 miles, although there are exceptions. It is best to check your owner’s manual, as it will likely have a recommendation. Having your tires balanced is also important for even wear. A mechanic will remove each wheel and use a machine that spins the wheel, determining whether it is balanced evenly around the axle. Have your tires balanced about every 3,000–6,000 miles.

Balancing is often done at the same time as rotating, which works well for most vehicles. Tire alignment involves aligning your steering to the direction of the tires. If your car begins to drift even when your steering wheel is in the center, your tires are likely out of alignment. This also causes uneven wear on the tires. How often you should have your tires aligned varies, based on your vehicle, but a good rule of thumb for checking alignment is annually. This can be done by a mechanic while you are getting your tires balanced and rotated. Of course, even properly maintained tires wear out eventually. On average, tires should be replaced about every 50,000 miles. This varies, though, depending on the kind of driving, kind of tires, and kind of vehicle. You should refer to your owner’s manual to get a better idea of how long tires should last on your vehicle. You can do the “penny test” to get an idea of whether your tires are worn out. Place a penny into the center of the tread, between the ridges, with the image of Lincoln’s head upside down (into the tread). If you can see the very top of his head,
you should replace immediately. If some of his hair is visible, you should replace soon. If some of the forehead is covered, you still have adequate tread. Once you decide it’s time to replace your tires, it’s important to buy the right tires for your vehicle and your driving conditions, as this affects handling, wear, fuel economy, and safety. For example, touring tires are some of the most commonly used tires because they’re good for city driving or typical highway driving. If you spend most of your time on the highway, though, tires with rib tread may be better, as they provide stability for heavier loads and channel water well. Keep in mind that you get what you pay for—the difference between a bottom-of-the-line tire and a top-of-the-line tire can be as much as 50,000 miles. As we all remember from Michelin’s iconic 1980s ad, “so much is riding on your tires.” Don’t neglect your tires! With regular rotation, balancing, and alignment, you can get the most out of the tires on your vehicle. The Good Life
Bringing Hospitality Home Entertaining Like a Pro By Holly Tierney-Bedord
With autumn come bountiful harvests and crisp evenings perfect for entertaining friends. In our busy, tech-heavy world, mindfulness and simplicity are more important than ever, and that mindset is carrying over into the way we entertain. Family-style dining that incorporates healthy ingredients and encourages all ages to dine together is becoming the new norm. This kind of communal, casual-yet-elegant dining is common in Europe and elsewhere, but it may feel strange to those of us who grew up sitting at “the kids’ table.” To get the look and the warm, embracing feel of this style of eating, start with a long table (or push a couple of same-height tables together and cover with tablecloths to fake it). Use your good dishes! If you’ve got a matching (or mismatched but beloved) set of dishes or chairs, this is the time to bring them out. If you’re short on supplies, contact a local party rental company. They can provide everything from gorgeous linens to tables, decorations, and more. Once your table is ready, add some fresh bouquets of flowers and unscented taper candles, and the scene is set. This will set the mood for your party, and then you can focus on the star of your event: the food. Family-style dining is big right now, and it lends itself to a potluck meal. For a more elegant (and, let’s face it, easier on the host or hostess!) event, have your dinner catered. Depending on your budget and desires, this could mean anything from having chafing pans of food delivered to having a staff that decorates the house, cooks and serves the food, and cleans up afterward! Many restaurants have on-staff event planners whose job it is to execute your party plans seamlessly. They work around your budget and suggest ways to make it stretch a little farther by altering your menu or cutting out unnecessary frills. As long as you and your caterer have communicated the terms ahead of time and you’re in agreement about the contract, you can likely choose from many different levels of service. Popular food trends right now are poke bowls and anything else Hawaiian-inspired, vegan food, deepfried food, and food that is locally sourced. Alcoholfree drinks are also very popular, and they’re a way to make everyone around the table feel welcomed and included. Drinks that incorporate fresh juice, exotic spices, tea, and mineral water are great ways to add something special to your gathering. If your party has a theme, create a signature cocktail to match it. Cocktail parties will always be in style and require very little planning compared to sit-down dinners, especially with the popularity of catered beverage services. You heard right—the bartender and the bar can now come to you! All you need to do is clean your house, put on your favorite outfit, crank up the music, and you’re ready to party. If your home is small or you’d prefer a change of scenery, don’t forget that restaurants are literally made

for entertaining. When choosing your venue, consider the atmosphere, the menu, your budget, and any dietary restrictions your guests may have. Definitely eat in the restaurant as a regular guest once or twice before planning a party there, so you’re sure you like it. If you’re planning a large party, it’s never too early to contact the restaurant of your choice to talk about booking a private space. Popular restaurants can have holiday parties booked months in advance. If you’re having a smaller party, a few weeks in advance may be enough notice. In general, the more notice you can give, the better your event will go, since the restaurant will have time to make sure they get all the details right for you and are properly staffed for your event. When thinking about your budget, a few things to consider are the cost per guest and what I like to call the “memorablenessfactor” of the event. This is why people spend a lot for weddings: They’re events that are never forgotten (at least for the couple getting married). Special events push the budget. A more everyday event may allow you to include some big-ticket items but decline others. There are many budget planning cheat sheets online that can help you figure out how much you should spend on an event and how you’re likely to spend it. Don’t let a tight budget, a small home, or any other concerns hold you back from having a party. There are inexpensive ways to celebrate with the people you care about, and the holiday season is the perfect time to make it happen. Your local party professionals can help you plan a party that’s right for you and your guests. Rome—The Eternal City You Will Want to Visit Again
By Jackie Williams
Nearly everyone who visits Europe books an Italian destination, and Rome should be on the list to visit more than once. Why? Rome is one of the world’s greatest historic cities, that’s why. The city has about 3,000 years of history still intact. If you don’t already know how Rome has influenced the rest of the world, you will after a visit to this amazing city. Rich in culture and architecture, there is a lot to see, and you will never be able to catch it all in one or two visits. You can travel to Rome several times and have a completely different itinerary each time you go. Join a tour and pick a theme: history, art, churches, underground, foodie, modern day, parks, shopping, museums—the list goes on and on. The hardest part is narrowing it down to fit into your vacation time. Let’s start by hitting the most popular landmarks in Rome. I can think of seven off the top of my head: Colosseum, Forum, Pantheon, Trevi Fountain, Vatican City, Piazza Navona, and Villa Borghese. How can you cover them all? I recommend staying in an area near one of these and within walking distance to the Metro (the subway). Buy a multi-day pass at the Metro station and get unlimited rides anywhere the train goes. (Certain passes also

The main sites are just the beginning of your adventures! Neighborhoods like Trastevere, Colosseo, Old Rome, and Modern City are ideal, as well. I have always visited Rome in August and September. The weather is still sunny and warm and, though there will always be crowds of tourists in Rome, the crowds are a little smaller toward fall than they are in the summertime. The first time I went to Rome, I backpacked with a friend and walked everywhere. We explored on our own, which led to some great experiences and memories. Now, I am older and want to be more comfortable, so I allow for some relaxation by not packing in so many excursions each day. Small tours were in order for my second visit, along with some selfguided walks through the city. ›››

I have come to love foodie walking tours, and The Roman Guy tour my husband and I took was fantastic! I prefer not to be carted around in a big bus group; meeting up with about eight other people was more fun and intimate, and we didn’t stand out so much. The tour guide was enthusiastic and knowledgeable, and we went to some places that larger tour groups may have missed. To prepare for your trip, download maps and apps to your smartphone before leaving. If your phone has a removable SIM card, you can buy a local one once you arrive in Rome, so you can always have access to phone and internet. You will also be able to see the current train and bus times, which makes it easier to coordinate your travel. We use WhatsApp to stay in contact with our traveling buddies, XE Currency Converter to learn the prices of things in dollars, and Google Translate for the language barrier—you can even take a picture of a sign and it translates it (though most signs are in English). Pin landmarks, your hotel, and other places you want to go on your Google Maps ahead of time, and then type in “OK Maps” to download directions even without an internet connection. Rome is generally a safe place to visit, but you have to watch your personal belongings when you are in crowded areas, just like in the United States. Having a crossbody purse with closed pockets is the best. Another item to consider when packing, especially if you are visiting during hot weather, is that many of the Vatican sites have a conservative dress code. Your shoulders and knees will need to be covered, so be sure to pack a pair of long pants or a longer skirt for the day you visit Vatican City. A wrap that you can fold into your bag or tie around your waist for the rest of the day will do nicely for the shoulders. The best thing to remember about a trip to Rome is that you cannot see it all in one or even two trips—just pick a few things and enjoy yourself! Make time to sit at a piazza with some local treats and coffee. Bringing the family would be a wonderful experience for your kids, opening them up to a lifetime of travel and exploration themselves! The Friendship Force Transcends Boundaries Promoting Goodwill, Respect, and International Understanding By Jackie Byers
If you enjoy traveling, have hope for a more peaceful world, and want to learn more about the citizens and cultures of other countries, Friendship Force of Greater Omaha is for you. Since 1977, Friendship Force Ambassadors (members), have been traveling the world and making friends by exchanging home stays with other ambassadors. The international organization was founded by Wayne Smith and introduced to the nation by President Jimmy Carter. It is still expanding its outreach. Currently, 63 countries have chapters. Each chapter offers at least one trip each year. Many chapters, including Omaha’s, participate in the Open World Leadership Center sponsored by the Library of Congress. This program shares Friendship Force’s goal of fostering cultural understanding. Emerging Eurasian leaders, brought here by Open World, enjoy a home stay with Friendship Force members. The home stay promotes friendship, understanding, and appreciation of other cultures. According to Joy Schaal, the president of Friendship Force of Greater Omaha, the home stay is an essential part of the organization. “Home stays provide personal guides who know the best places to visit. Equally vital is the practice of breaking bread with guests, letting them experience your culture. Language differences disappear.” Joy and her husband, David, have hosted many guests from Europe and Asia over the years, and the internet makes it easy to stay in touch and maintain long-distance friendships. “I remember some visitors from Japan,” Joy recalls. “As we planned their stay, I learned that they were interested in quilting—my hobby! I contacted Bernina Sewing Center. They arranged a tour and a day of quilting on their machines. Our guests loved it.” When you travel, Joy explains, it is customary to bring hostess gifts that represent your home. “Your hosts will gift you with something that represents their home in return. These small keepsakes keep wonderful memories fresh.” Thanks to a home stay hostess who gave Joy a quilting bundle filled with native fabrics, Joy decided to make a quilt to commemorate each trip. “I made a quilt as soon as we got home,” she says. “Now, I am working on my fifth Friendship quilt. I show them to all our home stay guests.” Another important custom is taking your hosts out to dinner. This practice further enhances your cultural learning experience. Joy declares, “We have so many friends of all faiths in many lands. I have learned that people are so much alike, no matter where they live.” This holds true, she says, even in the face of language barriers. “In Brazil, our hosts had no English; we had no

Portuguese.” David Friendship Force adds, “My high school Spanish was no help, though Ambassadors Joy and computers translate a bit. We still bonded, met the whole David Schaal, and Margie and John Stolte family, and reconnect often.” Joy and David and their good friends Margie and John Stolte are longtime Friendship Force Ambassadors and have wonderful stories of the friends they have made on their travels. “On a tour with even the most professional guide, you don’t get to know the people. You see the castles and cathedrals, and that is great. But you don’t get in-depth looks about how life works for the people who live there. Sitting down to dinners, seeing yards and gardens, meeting families—these make the culture of that country come alive.” For a successful international exchange, planning is essential. Joy and David start making arrangements two years in advance. “Because International does not structure the visits, the many ways to learn about other countries are full of possibilities,” David says. “We do check the International website for trips,” Joy notes. “If a group has a trip we want to join, and we ask early, they may have room for us. We meet lots of great people that way, too. Even non-members can ask to join a trip by any group.” In addition to fabulous travel opportunities, Friendship Force of Greater Omaha has local events to keep members in contact. Margie explains, “We have LEOs—Let’s Eat Out. I have been planning these for eight years. We have never eaten in the same restaurant twice. With LEOs, we can keep up with each other’s travels. We also have ‘Inbound’ and ‘Outbound’ exchanges each year. Sometimes we hike, bike, or motor. We like to get around.” John adds, “I enjoy taking guests around locally as much as I enjoy visiting other lands. We often take guests at the right time of year to view the Sandhills Cranes. It is always a thrill.” John was especially moved by visiting Nagasaki. “Japan was wonderful. Great host family. Bathing customs were unique—bucket shower, followed by a soak in a warm tub. It was so relaxing, I fell asleep.” To learn more about this amazing, friendship-building travel organization, visit friendshipforcegreateromaha. org, call 402-630-5089, or send a message to friendshipforcegreateromaha@gmail.com. The Green Thumb
What the Garden Needs
Proper Watering, Feeding, and Weeding By Sharon Knierim
It’s not surprising that proper and regular watering and feeding need to occur in order to maintain a beautiful and successful garden. Also important is keeping the garden plot neat and clean. Pulling weeds, deadheading, and removing spent plants is a great opportunity to be on the lookout for trouble. If disease or pests are present, get busy right away to correct the problem. Problems in the garden can bring a sudden and disastrous end to what had been a successful year, so regular maintenance of the garden is absolutely essential. We all know that plants need water, but what are we specifically attempting to achieve when we water plants? Basically, we need to soak the root systems or root areas of plants (not the leaves), but do not do this more than twice a week. Watering daily is a no-no unless your garden is the victim of a hard drought. The first rule of watering is to water deeply. Shallow watering or light sprinkling may be more harmful than not watering at all. Not applying enough water causes root systems to develop too close to the soil’s surface. This can become problematic if a drought does occur because the roots are not deep enough into the soil to get what moisture they can find down there. Plants will be in trouble. Water slowly and steadily, making sure to soak the soil well. Some gardeners water while counting to 20 or 30 before they move
on to another area. Keeping a deep dish in the garden and watching the water level in that dish is one measure of how much water is being offered to plants. Because annuals have shorter, shallower root systems than perennials and bulbs, they should be watered (and mulched) more often but not as deeply. Do not water the garden under a blazing hot sun. This method can actually burn plant leaves and stems. If possible, water plants in the morning so that they can dry themselves before sundown. Plants that remain wet overnight are most likely to develop fungus problems. In addition to watering plants, we also need to feed them. The most highly recommended natural fertilizer is manure. Even compared to today’s modern chemical fertilizers, good old-fashioned

manure works fast and works well. Don’t worry—you won’t have to cozy up to a farmer to get enough manure for your garden. It is easily purchased in its dry form, by the bag, at a garden center. If you’re squeamish about touching manure (even in its driest form), wear gloves. I guarantee you’ll love the results. If you are fortunate enough to have a compost pile or a composter, enjoy its benefits. Although it does take some time to turn scraps and shredded newspaper into compost, it is certainly worth the wait. When those kitchen scraps, old paper, dead or wilted plant material, and even weeds (though not in seed-setting stage) are finished “cooking,” it’s like finding black gold to be added and mixed into your garden soil. Allowing weeds to grow in a garden is promoting unwanted competition for your hard-working plants. Vegetable gardens and flower gardens do not appreciate sharing their soil, their water, or their food. Hand-pulling weeds is a tried-andtrue but time-consuming method of weed control. Many master gardeners have discovered that the best way to control weeds is to use a sharp hoe—just hoe between rows. Another popular weed control strategy is chemical warfare. Preemergent weed killers are available at any garden supply store and are well worth their small cost. Weeds often appear before seedlings emerge. Do not wait until weeds are ready to claim the garden! Instead, start weed control at the very beginning of the growing period. Weeds are most vulnerable between the hours of 11:00 a.m. and 2:30 p.m. If possible, that is the time to get out there and start eliminating them. As weeds appear, keep up your defenses, but don’t overdo it. During hot and dry spells, the soil will lose moisture more rapidly if the ground is disturbed too much. Did I mention how useful a hoe can be in the garden? Whether you’re weeding, deep-spading animal or green manure, or blending leaves and compost into the soil, a good hoe is a vital gardening tool. Learning and attending to the needs of your garden will bring you years of wonderful harvests! She lives on a small farm in western Iowa and looks forward to sharing her passion for gardening each month with her green-thumbed reader friends. WE September 2018

Cuddly Critters
Pocket Pets Tiny Critters with Big Personalities By Elizabeth French
Gone are the days of Paris Hilton and Elle Woods with their chihuahuas in their handbags; now, true “pocket pets” are all the rage in the form of hamsters, rabbits, guinea pigs, and even chinchillas. Pocket pets are typically some form of rodent and vary in their care. Here are a few of the most common pocket pets and how to care for them so you can determine which one is right for your household. If you have a household with older children, you might opt for a rabbit. Living between 8 and 10 years, rabbits come in a variety of breeds. They love interacting with humans and are easily potty-trained. They require less care than many other pocket pets and require no vaccinations whatsoever. However, they do need a good amount of time outside of the cage and at least 12 square feet of enclosure space, depending on their size and weight. It is essential that bunnies have access to chew toys because they are lagomorphs, which means that their teeth continuously grow throughout their lives. Chew toys are essential to whittle their teeth down to a comfortable length. Rabbits eat a varied diet, including greens, pellets, veggies, and hay (for the digestive system). An important thing to remember with rabbits (and most other pocket pets) is that they are instinctually prey animals, so they’re easily startled. It is essential to approach them slowly and let them come to you, as they can be highly sensitive at first. If you get down to your bunny’s level and let the animal make the first move, you’ll be playing and cuddling together in no time. Chinchillas are very energetic but are not good for households with small children because they are extremely sensitive to light and sound. Living on a crepuscular cycle, these soft critters are most active at dawn and dusk and may literally bounce off the walls of their enclosures! A single chinchilla needs a minimum of 5 or 6 cubic feet in its cage to roam, and 10 to 12 cubic feet are needed if you opt to purchase a playmate. They play with a variety of chew toys and live between 15 and 20 years. These rambunctious rodents need regular dust baths, an exercise wheel, and a very specific diet of hay and alfalfa pellets to maintain a healthy digestive system. However, with the right amount of time, money, and care invested, these soft creatures can be wonderful companions for years to come. The greatest pocket pet for any household with children has to be the tried-and-true guinea pig. With a short lifespan of about 5 years, these cuddly critters are fantastic with children, easily potty trained, and have been kept as pets since the 16th century! Needing a cage of only about 8 square feet, these portable piggies are sweet, loving, and require a simple diet of hay, vegetables, and pellets. Like rabbits, their teeth constantly grow, so they need plenty of chew toys to keep their teeth ground down. They have poor

eyesight, so handle them slowly and deliberately. It is also recommended to keep at least two guinea pigs at a time because they are happiest when living in groups. However, be sure to get two of the same gender to avoid potential conflict and unwanted offspring. Keep in mind that a pair of guinea pigs will need a larger cage than a single pet. As with any pet, be sure to see a veterinarian regularly. Different vets specialize in different kinds of pets, so be sure to ask before you make an appointment. These tiny creatures age far quicker than other animals, so it is essential to keep their nutrition levels as high as possible. Be sure to bring them to the vet if you notice any changes in behavior or appetite. As most pocket pets are prey in the wild, they will instinctually hide signs of illness until it is sometimes too late; as soon as any symptoms show, the situation needs to be dealt with immediately. A seemingly healthy pocket pet may deteriorate and expire in a matter of hours if symptoms are ignored or left untreated. It is essential to make sure an animal will be a good fit in its environment to ensure the happiness of both owner and animal. Depending on the level of commitment you want to invest, the bottom line is to really learn about an animal’s needs before purchasing one. (Also note that not all pocket pets are legal in every state. If you’re looking to get a hedgehog or a chinchilla, you should check your local laws.) You can still enjoy cuddling with a pet if cats and dogs are not for you! Consider a pocket pet as a fun and entertaining companion. Positive Perspective
Stop Lying to Yourself Live the Life You Want
By Janet Van deWalle
Are you living a life built on lies? I’m sure most of us would answer with a resounding “no.” Recently, I came across some information—a short video from a man by the name of Vishen Lakhiani—that gave me a new perspective. In this video, he spoke about how many of us have built our lives based on information we have been taught. Makes sense, right? None of us entered the world knowing how to read or tie our shoes. Of course, if you break it down, it goes much deeper than that.
“Become aware of your beliefs and automatic default settings. Bring them into the light of your present, adult knowledge. Gently acknowledge that they are what they are. Then accept that they constitute what you’ve believed until now, and that you can transform them into beliefs that allow you to fully express who you really are. Without judgment, patiently begin working to change subconscious and limiting beliefs into true expressions of your authentic self.”—Sue Patton Thoele
“You are what you are by what you believe.”—Oprah Winfrey
I challenge you to identify those truths that you are living with and examine if they are true or not. Ask yourself questions such as: Does this “truth” make me feel good? Does it allow me to treat others the way I want to be treated? Does this “truth” help serve my overall happiness? Then ask yourself where you learned it. Was it a rational choice you made or something I’ve learned?
We have learned about religion, relationships, work, and basically every aspect of our lives through teachers, parents, media, and other major influences. Those beliefs, those “truths,” are what have helped bring us to the point in our lives where we are now. The problem is, some of these things we’ve learned are not necessarily based on truth. Think about it for a moment. What beliefs or truths do you hold as true that aren’t serving you well? For example, many people feel that hard work is the only way to be successful and that relationships have to be hard. If you take a moment and step back, you may find that you are living with many “truths” in your life that are simply based on other people’s realities, and you have allowed them to shape your own.
