Calming Out Points of Interest
I have torn my attention away from the hurricanes to write this column. Irma is the latest one. I have been watching the planet for the last several weeks and have had to take multiple deep breaths to find my balance in all of this. The question that keeps coming to my mind is: What can I do? Of course, we can donate to the American Red Cross. We can pray, each in our own way. But what can I do to help calm this situation? Then I realized that I can recite these song lyrics: “Let there be peace on earth, and let it begin with me.” Now, I realize that some may think this is a strange way to deal with weather issues. But one thing I have noticed in my life is that when the planet is stressed, we humans seem to be stressed, as well. There seems to be a correlation between when we humans are experiencing turmoil and when we are experiencing hurricanes, tornadoes, fires, and earthquakes. Call me crazy…you won’t be the first one! My mother, Anne Albracht, who passed away in 1993, taught me this. She was a very wise woman. She was a Pisces who would sit in her chair, take a look at the “big picture,” and then listen, talk, and teach all six of us kids. Boy, do I miss her! What I learned from Mom is that everything on this planet is energy. Everything. The chair I am sitting in. The computer I am typing on. My kitten, Little Stinker, who finds great joy in unplugging my laptop! And let’s not forget about the weather. I have energy, as well. I can sit and obsess, watching the latest video on Weather.com. The wind, the storm surge, the destruction. OR…I can lie in my bed, take some deep breaths, and know that I can have a positive impact—no matter how small—on calming things down. I ask myself: What feels better? To watch the destruction or to try to make a little bit of difference in making things better? I know the answer. I hope that, by the time this issue is out, things will have calmed down with the planet. I am appreciative of remembering what my wise mother taught me. What I do know is that I’m turning off the Weather Channel and breathing. Imagining peaceful thoughts. Calming out. If I calm out…I can help the planet. What do you all think? Let me know!

The Breathtaking Colors of Fall It’s Not Just the Foliage! By Holly Bell
In the fall, the fabrics are luxurious, the colors are rich, and the styles are flattering. It’s my favorite fashion time out of all the six seasons. That’s right—in the Midwest, we really have six seasons. In the winter, we’re all bundled up and layered from head to toe. Then comes early spring, when we’re tired of our winter attire, but the weather is not yet warm enough for pastels and bare shoulders. At this time, we put away the heavy gear and go for lighter sweaters. Then we have glorious spring, when we are jumping for joy over everything turning green again, but we still need light sweaters and the occasional leggings. Of course, then comes wonderful summer, when we can burst forth in beautiful florals and can finally bare our shoulders, arms, and legs. Come September, we feel just a little chill, so we put away the sandals and flip-flops and get out cute booties, darker colors, and slightly heavier fabrics. When October blows in, it is finally time for those rich, luxurious colors and fabrics that I’m wearing right now! Enjoy it while you can, before we have to bundle up again. The colors for fall 2017 are some of my favorites. All the shades of red will be gorgeous, from light pink to dark wine tones. Red is particularly flattering when worn next to the face, as it brightens your complexion. Rich olive green is the next color featured this fall. This color is beautiful and fresh when mixed with the next fall favorite: navy blue. In fact, all of these colors can be worn together, as olive and navy are really great neutrals. Rather than always turning to black for your bottom half, try

navy this year. You will find it is just as versatile and has more of a modern look to it. The go-to fabric trend this year will be velvet! From stretchy smooth textures to crushed velvet reminiscent of the 1970s, velvet in all its soft splendor will be everywhere this fall. From pantsuits and dresses to t-shirts and blazers, velvet creates a gleaming opulence in all the colors of the rainbow. An interesting juxtaposition to the femininity of velvet will be menswear looks. Checks, houndstooth, herringbone, and tweed in sharply tailored classic menswear shapes will look great with a touch of velvet. Picture a menswear-cut, black-and-white tweed jacket with a soft, burgundy velvet tank underneath. Classic and fabulous for all ages! With the menswear trend, military looks will also be important, featuring gleaming buttons and shoulder pads. Leather is an ongoing fashion trend for all the cooler seasons. Invest in a beautiful leather moto jacket, trench coat, or even pants and wear them for years. There is something to be said for faux leather, which is a more costfriendly option. Faux leather is so good these days, the only way you can tell the difference is to do the sniff test! Leather looks best when worn with a softer fabric and style. Too much leather can make you look tough, but mix it up with feminine pieces and it is on trend and flattering. Faux fur is another fun, ongoing trend for winter. Again, it can be hard to tell the real from the faux, and it really is an outstanding bargain. Faux fur trim on jackets, sweaters, capes, scarves, and gloves offers a luxurious look without the luxury price. With all this talk of leather and menswear, you might be thinking that you have to give up your glamorous femininity this season. Don’t worry. While these looks are fun options to work into your wardrobe, there are still great trends this season for the girly girl. Fall florals are hot this season, featuring gorgeous bright florals on dark backgrounds. Lace and sleeve interest will be moving forward with bell sleeves, lantern sleeves, multiple-tiered sleeves, and more innovative cutouts and designs. Thank goodness that shine and sparkle will still be around this season—I could not possibly give them up! Shiny boots and bags are cool with those tailored looks for fall. More femininity abounds, with the soft lingerie looks still popular. Kimonos worn over dresses or jeans will add a fresh vibe to your fall wardrobe. Silky fabrics slink out of the bedroom in satiny blouses, slip dresses, and pajama-inspired pantsuits. This season has so many wonderful offerings, I can hardly wait to turn my closet into fall! I’m planning to add some new divine pieces in the luxurious velvets and rich fall tones of olive, navy, and red. This fall, join me in mixing it all together! I always feel best when I’m dancing to the beat of my own fashion drum. Fall Football Feasts, Goblins, and Shadows! By Kate Beiting
Apple Pie with Dutch Crumb Topping
For the pie: Bake a prepared pie crust (in the pie plate) at 400 degrees for 5 minutes, then remove from the oven. In a large bowl, mix 6 cups of peeled, cored, and sliced apples (a mix of Grannies, Jons, golden delicious, any tasty apples) with 2 teaspoons of lemon juice, two-thirds cup of sugar, 2 teaspoons of flour, threefourths teaspoon of cinnamon, one-fourth teaspoon of ground nutmeg, and a pinch of salt. Mix well, pour into the pie crust, and dot with a few cubes of butter. For the crumb topping: Mix three-fourths cup of flour, one-half cup of sugar, one-half teaspoon of cinnamon, and one-third cup of butter. Rub the ingredients with your fingers until crumbly. Pour over the pie. Bake on a cookie sheet at 375 degrees for 60 minutes. Festive Red Peanuts Heat 2 Tablespoons of oil in a skillet. Stir in one 12-ounce jar of dry roasted peanuts, along with one-fourth teaspoon each of garlic powder, ginger, and cayenne pepper. Stir to coat. Add a dash of sugar. Cook for 2 minutes. Remove from heat and cool. Store in a covered container. Caramel Dip Mix two 8-ounce cubes of softened cream cheese with one-half cup of brown sugar. Spread mixture out onto a serving platter or plate. Warm a container of caramel apple dip in the microwave for 20

seconds. Drizzle over the cream cheese. Top with crushed peanuts, a bag of toffee or toffee chips—whatever you like for a little crunch. Serve with graham crackers, wheat crackers, or apple wedges. Brats and Chopped Cabbage In a large skillet or stockpot, cook 4–6 slices of bacon until crispy. Add a chopped onion and 2 cloves of minced garlic. Add 1–2 pounds of chunk-cut kielbasa or brats, and sauté for 5 minutes until brown. Add 1 cup of chicken broth and one-half head of chopped cabbage (throw in a cubed apple for interest). Cover and simmer for 15–20 minutes. Salt and pepper to taste, and add more broth, if needed. ›››

Fried Green Tomatoes (You still have tomatoes on the vine, don’t you?) In one bowl, mix 1 cup of white cornmeal, one-half cup of flour, 1 teaspoon of sweet paprika, 1 teaspoon of Lawry’s seasoned salt, and a dash of salt. In another bowl, mix 1 egg and one-third cup of milk. Core two large green tomatoes, cut off the ends, and slice into four slices. Dip the tomato slices in the flour mixture, then in the egg mixture, and then back into the flour mixture. Heat onefourth cup of oil in a cast iron skillet. Fry the tomatoes until golden brown. Drain on paper towels. Quick Family Pork Chops Brown 4–6 pork chops in a skillet for 4–5 minutes (the browning creates another layer of flavor). Place the chops in a casserole dish (or use the same skillet, if it can go in the oven). Sprinkle the chops with ground pepper and 1 teaspoon of poultry seasoning. Pour 1–2 cans of chicken and rice soup over the chops. Add a few peeled and cut-up sweet potatoes around the chops. Add 2 sliced apples on top of the chops, and cover with foil. Bake at 350 degrees for 1 hour or until the chops are done. Zucchini Brownies Melt 2 Tablespoons of butter and 1 cup of semi-sweet chocolate chips (on the stove or gradually in the microwave— careful not to burn it!). Add 1 cup of sugar to the melted chocolate, then add 1 egg plus 1 egg yolk. Add 1 teaspoon of vanilla and 1 cup of grated zucchini. Mix together one-half cup of flour, one-half teaspoon of baking powder, and one-half teaspoon of salt. Add the wet ingredients to the dry until combined. Grease an 8-inch baking pan and line with parchment paper. Pour
the mixture into pan. Bake at 350 degrees for 25 minutes. Let cool for 1 hour. Frost with chocolate frosting, if desired, or dust with powdered sugar. To make ganache frosting, heat 3 Tablespoons of cream until bubbly. Remove from heat, add three-fourths cup of semi-sweet chocolate chips, and stir until melted. Baked Cinnamon Apples (You can control the amount of sugar in this dish.) Peel, core, and slice 6 large apples. Place them in a buttered 9x13-inch casserole dish. Mix together 3–6 Tablespoons of brown sugar, 3 teaspoons of ground cinnamon, and 1 teaspoon of ground nutmeg. Sprinkle the mixture over the apples. Mix 1 teaspoon of vanilla into one-fourth cup of water and sprinkle over the apples. Dot the apples with 6–7 cubes of butter. Bake at 350 degrees for 35–40 minutes until tender. You can add red hots for some zip. Pumpkin Cookies Mix one stick of soft butter with one-half cup of vegetable oil. Blend in one-half cup of canned pumpkin, 1 cup of sugar, one-half cup of powdered sugar, 1 teaspoon of vanilla, and 2 room-temperature eggs. Mix together 4 cups of flour, one-fourth teaspoon of baking soda, one-fourth teaspoon of cream of tartar, one-half teaspoon of salt, and one-half teaspoon of pumpkin pie spice. Add the dry ingredients to the wet and mix well. Scoop by Tablespoons onto a prepared cookie sheet, lightly flattening the dough. Bake for 9–10 minutes at 350 degrees. Cool. Glaze with a mixture of 3 cups of powdered sugar, 4 Tablespoons of milk, and one-fourth teaspoon of pumpkin pie spice. Makes 3–4 dozen cookies. WE

Nancy’s Boutique on the Move
Style and Quality in Women’s Clothing By Marge Shoemaker
Tired of shopping at the big department stores and never feeling like they care you are there? My suggestion is to take a short drive to Fremont to experience Nancy’s Boutique, nestled in the historic commercial district in downtown Fremont. Nancy’s Boutique has recently moved and has a fresh, new look. Nancy’s hasn’t moved far—only across the street and down a block from the previous location. The new boutique has a charming, chic atmosphere, where you will find top designer lines that are of the utmost quality and style. When owner Nancy Hoshor initially found the new location, the building was in pretty poor condition, but she felt it had possibilities. Nancy says she is thrilled that the renovations of the store are just as she had envisioned and were ready August 1, as promised. Nancy shared with me she will be hosting a re-grand opening in the middle of September to celebrate the new location and the fact she has been in business for 14 years. The new space is bright, light, and airy. Nancy has yet to tint the front windows, and a couple of signs still need to be hung. She held a soft opening, giving her and her staff time to get new wall racks hung and fall merchandise displayed. “We’ve already had several people drop by and have made several sales,” Nancy comments. Even though the space is new, Nancy’s attitude about fashion remains the same, and she carries many of the same top designers she has relied on over the years, including Simon Chang, Cartise, Bandlino, and Bylyse to name a few. “I look for well-made clothes in classic styles,” she says. Her clients are usually a little older, she points out—not young people in their 20s and 30s who change their wardrobe often. “My clients are business, professional, or established women who appreciate style and quality.” She helps them build a wardrobe with classic, neutral pieces that offer versatility yet go well together. Such a wardrobe can be jazzed up with color in jewelry and accessories. Although she sticks with the classics, Nancy still keeps up with the modern fashion that her clients enjoy, including the ever-present legging. “Anyone can wear tights or leggings…as long as her top comes down to cover her bottom,” she advises. “My clients aren’t stodgy, but even the skinny ones should cover their derriere. Few women look good in short tops.” Among the fall merchandise at Nancy’s Boutique is a selection of sweaters by different designers, all of whom reflect Nancy’s preferences. Some sweaters by Angel have yarn-ties at the neck with poofs on the ends. Others are color-blocked and reversible…with pockets, both front and back! A new line from Tricotto includes pants in basic

colors and graphic tees and sweaters. Owner Nancy Hoshor, The Hello Mello line includes comfy lounge pants and with her dog, Toby tops. Also on display is a collection of lightweight, classically tailored jackets and coats. Meant more for style than midwestern winter warmth, the coats pair well with pants and light tops for indoors, or with sweaters for a quick jaunt out and about. Especially noticeable is an area of bright “Nebraska red” clothing. A colorful array of red and white in sweaters, jackets, pants, tops, bottoms, and scarves offers a selection to tempt the most avid Big Red fan with classic lines and design. Most of the clothing does not have a Nebraska logo on it, which adds to its versatility—you can put your team loyalty on display even when you’re not in the mood for athletic wear. Nancy’s Boutique isn’t like a big box store, where racks upon racks hold every style in every size and color. “Most of the things I buy are with my regular clientele in mind,” Nancy explains. “I believe in trying on clothes to determine fit rather than going by the size on the label.” All women know how much sizing can differ between designers! Fright Night Why We Scare Ourselves Silly

Things are getting scary—it must be Halloween. Do you jump when someone shouts, “BOO!”? Do you feel uneasy in the dark? How about those moving shadows at the edge of the yard or in the dark corners of rooms? What was that loud THUD upstairs? You thought you were alone in the house, didn’t you? BOO! Gotcha! For a very long time, Halloween was a holiday enjoyed (or dreaded) mainly by children, who dressed up as fictional characters and visited every house in the neighborhood, looking for treats. Many adults felt trapped when youngsters sang out, “Trick or Treat!” The only way to get those kids off the doorstep was to bribe them with candy or treats. For the most part, these little spooks were good-natured and would never play a trick on someone who was giving them candy and goodies. Children and adults alike enjoyed this oncea-year festivity while creating long-lasting and highly treasured memories. The focus of Halloween and the methods of celebrating it have changed over time. Many schools no longer encourage classroom parties during this season, so the thrill of dressing up, parading, and garnering treats at school has gone by the wayside. Instead, organizations, shopping malls, and churches often host Halloween parties featuring costumes and treats. Some of these highly organized functions even feature games and prizes. Of course, adults are welcome to participate…if you

watch closely, you’ll see these grown-up and overgrown kids having lots of fun right alongside the children. One of the mainstays of long-ago Halloween parties was bobbing for apples. Today’s healthcare workers shudder when they learn how this was done. A large tub was filled with cold water and apples were added. Two by two, children and adults attempted to “capture” an apple with their teeth while their hands were held behind their backs. Does that sound healthy? Because we are now much more sophisticated about germs, there probably isn’t an apple bobbing contest held in the civilized world these days. Can you imagine the germs and bacteria floating around in that water and on those apples? If an apple left in the water already had teeth marks in it, a contestant could still attempt to bite into it as it remained in the water. Now, that’s scary!

Today, Halloween is all about scaring ourselves enough to want to leave the lights on for a few nights. Movies certainly have the ability to make our hearts beat faster. Characterizations of absolutely revolting humans, animals, insects, and otherworldly creatures abound. Modern special effects leave nothing to the imagination; for some of us more squeamish folks, it is almost more than we can bear. Still, we are spellbound by images too horrible for most of us to imagine on our own. We seem to have evolved from scary to shock and revulsion. Really, though—why do we expose ourselves to such frightening images and ideas? Professionals who deal with this type of phenomenon tell us it’s part of our nature to do so. Intrinsically, we seem to need a little excitement in our lives and an opportunity to question reality’s boundaries. It’s a good thing most of us only do this once a year! Recently, we viewed the movie “The Exorcist.” By 2017 standards, this is a pretty mild movie, but it really frightened a lot of people when it was first released. That fright affected even people who were known to be logical and not given to suggesting otherworldly activity. But there is no reason to lose heart if you relish the entertainment value of fright. It can be fun, especially if you’re watching a movie with friends who are willing to leave the lights on. On the other hand, if you are faint-hearted (and many of us are), you should not place yourself in this situation. The Halloween costumes of yesterday were primarily cowboys and pirates for little boy spooks. Of course, little girls liked to dress up as princesses and angels. In addition, there were plenty of sheet-clad ghosts out on Halloween night. Occasionally, a witch would show up with a pointed black hat, or Frankenstein might come lumbering up the sidewalk, but there were surprisingly few monsters. Fast forward to 2017, and complicated costumes have become an integral part of the Halloween scene. Much planning is put into assembling outfits to scare the daylights out of us. What we would not accept on any other night of the year is apropos on Halloween. Luckily for many of us quieter souls, these scary images are usually attending a private party rather than haunting neighborhoods. Have fun on Halloween, but leave the lights on, just in case. BOO! Precision Compounding Custom Prescription Medication By Brittany Nicolle Willis
Nancy Johnson and Jodi Peterson are the co-owners of Precision Compounding in West Omaha, a compoundingonly pharmacy that has specialized in producing high-quality prescription medication since 2010. I recently sat down with Nancy, who explained the incredible benefits of compounding. Prescriptions can be customized for the specific needs of each patient in any number of ways—by combining multiple medications into a single dosage, by providing an alternative method of ingestion, or by adding a flavor to make the medicine more palatable. As they note on their website, the goal at Precision Compounding is to “make the medication fit the patient, rather than make the patient fit the medication.” The FDA requires that compounding prescriptions are prescribed by a licensed practitioner for a specific patient and are compounded by a licensed pharmacy. At Precision Compounding, they compound medications in many areas of medicine, including dentistry, dermatology, chronic pain management, sports medicine, infertility, hospice, pediatrics, ophthalmology, neuropathic treatment, wound therapy, podiatry, gastroenterology—even veterinary medicine. The vast majority of their patients (about 90 percent) are there for hormone replacement therapy. Other people seek out customized medication because they can’t swallow pills, because their children dislike the taste so strongly that they refuse medication, or because they have had adverse reactions to the inactive ingredients in massproduced medications. Nancy is excited about the company’s growth and notes that they have expanded their business to accommodate the demand they are experiencing. “We have increased our retail and work space an additional 1,500 square feet and have grown from one full-time employee to six,” Nancy states. “The extra space provides us more room to make and store compounding materials and to expand our nutritional line. People really like to get high-quality nutritional supplements, and we love to serve our customers in this way.” Precision Compounding is unique as a pharmacy, Nancy explains, because of their professional pharmaceutical knowledge and background in chemical and medical prescriptions. “That is a big plus,” Nancy notes. “It makes a big difference compared to other places or walk-in pharmacies that do not have someone available to address questions and issues.” Nancy describes their resident pharmacist, Suanne, as an invaluable resource. “A lot of times, the pharmacist is the guide to a prescription,” Nancy says. Drug-Free Treatment Chiropractic Helps So Much More Than Back Pain By Ellen M. Walker
Most people assume that chiropractors are good for a stiff neck or lower back pain. Chiropractors do indeed treat those problems, but there are so many other things they can do! The central nervous system pretty much rules the roost regarding the whole health of your body. This is why chiropractic care, which focuses on the nervous and musculoskeletal systems, is recognized as a holistic approach to total body health. Chiropractic consists of hands-on, drug-free methods that can provide health benefits for people of all ages, even infants and children. The best-known procedure performed by doctors of chiropractic (DCs) is spinal manipulation, also known as a chiropractic adjustment. Chiropractors refer to misalignment of the spine as a subluxation. Think of the spine as the main highway for our nerve impulses to travel through. When there is a fender bender, the road becomes blocked, and everything that was being transported along the nerve system highway on its way to the brain gets stuck in traffic. To clear the road, a chiropractor adjusts the spine so the central nervous system can get back to making the whole body function properly again. Chiropractic focuses on maintaining your health naturally to help your body resist disease, rather than simply treating the symptoms of disease. Your chiropractor will work in partnership with you to ensure your optimal health and wellness. She will look at many factors that affect your health, including exercise, nutrition, sleep, environment, and heredity. So, what else do chiropractors treat? They may help you find relief from all sorts of ailments, including sciatic pain, joint pain, headaches, migraines, temporomandibular (TMJ) disorders, shoulder and arm pain, foot pain, plantar fasciitis, digestive issues, and pregnancy-related pain. As a child grows and becomes more active, chiropractic care can help keep her in top physical condition. Many professional athletes and sports teams get regular adjustments to keep themselves in good competitive form. If injuries arise, chiropractic care can reduce or eliminate the need for any drug usage during recovery. Do you sit in front of a computer all day long? With the now-ubiquitous use of handheld digital devices, many people are developing neck issues at younger and younger ages. They are required to obtain their education from a nationally accredited institution. Upon completion of a fouryear doctoral graduate school program, chiropractors must pass national board exams to get a license to practice, and they must maintain their license annually by earning continuing education credits. As technology advances, so do chiropractic techniques. Chiropractors are using sophisticated technology to help better treat their patients. For example, a thermal scan can get a snapshot of how your nervous system looks. Then, your chiropractor can make more accurate care recommendations and have better clarity regarding your overall care plan. A newer instrument called ArthroStim can also be used on patients to produce the same “thrust and recoil” motion a chiropractor would use with her hands to adjust the spine. The instrument can produce this motion very quickly (12 times per second), so it saves the chiropractor a lot of strength. (Ironically, many chiropractors suffer injuries in the wrist, back, and shoulder from treating patients with these forceful and repetitive motions.) Because the instrument focuses on a very small treatment area, its force radiates through the surrounding joint and tissue, which may reach the trigger points that often cause muscle spasms. The rise in opioid abuse in the United States makes this a great time to explore the benefits of chiropractic that are natural and offer benefits to your whole body—without drugs. In response to the opioid epidemic, the Centers for Disease Control and Prevention, in March of 2016, released guidelines
for prescribing opioids that also promote non-pharmacologic alternatives for the treatment of chronic pain. In 2017, the American College of Physicians (ACP) updated its own guidelines for the treatment of acute and chronic low back pain to recommend first using noninvasive, non-drug treatments before resorting to drug therapies. These revised guidelines were based on a review of randomized controlled trials and observational studies. They include spinal manipulation as an option for noninvasive, non-drug therapy for low back pain. You don’t need a referral to make an appointment with a chiropractor, but it may be wise to consult with your primary care physician about your pain or limited range of motion. The American Chiropractic Association (ACA) encourages patients and healthcare providers to first exhaust conservative forms of pain management, when appropriate, before moving on to riskier, potentially addictive treatments such as opioids. While seeking relief, your chiropractor may recommend multiple visits per week, for two to four weeks, or until your pain is reduced. As you get back to full motion, she may suggest maintenance visits for routine care, along with a balanced diet and exercise, to keep your body in optimum health. Just as a fever is the body’s way of destroying a virus that invades the body, chiropractic care allows the body to self-heal by keeping the nerve pathways in the body clear. Regular chiropractic adjustments have improved overall health and wellness and quality of life for many patients. Kim sees it, are a clear assessment and a clearly written plan, independent of hospitals or school districts. “School therapies may be in a group setting, once a week. Private therapy can target the needs of the individual child, assess those needs, figure out a plan, and tackle it intensely. Many times, private therapy need not be a long, month-after-month treatment,” Kim explains. But how can you tell if an infant is progressing properly toward speech? What are some of the behaviors that might indicate language or speech challenges in a person who can’t yet talk? Kim understands the confusion, and she can help your family navigate it. “If you do not hear any babbles or utterings by about nine months, you may want to have a professional evaluate your infant,” she advises. “The earlier we can identify a behavior, the earlier we can begin therapy.” By the time your child is two years old, you should be hearing some words, short sentences, snips of nursery rhymes, or short phrases. If not, it’s time for an evaluation.You may also notice difficulty with feeding or with the eating habits of your toddler. Although picky eating is common with toddlers and young children, serious food aversion is not. Food aversion can have many causes and needs to be addressed to maintain proper nutrition for your child. Kim has experience in a variety of reluctant speaking situations. Don’t Suffer Through Urological Problems Call Dr. Mahal By Marge Shoemaker
Nationally, approximately 15 percent of post-menopausal women (that is, those aged about 40 to 70 years) experience some kind of urological problem. That’s according to Aman Mahal, MD, FACOG, the recently hired surgeon and urogynecologist at Adult Pediatric Urology and Urogynecology. As the name of the clinic suggests, they offer urological and gynecological treatment for men, women, and children. Dr. Mahal specializes in surgery, and he treats urological difficulties such as painful urination, urinary tract infections, incontinence, and uterine prolapse. Dr. Mahal grew up in central California. He attended the Creighton University School of Medicine, where he met his wife. After attaining his medical degree, he completed his residency in obstetrics and gynecology at the University of Iowa. He continued his studies at Stanford University, and he is a fellow of the American College of Obstetricians and Gynecologists. Throughout his education, Dr. Mahal has studied and trained with some of the best pelvic surgeons in the country. He
specializes in female pelvic medicine and has been with Adult Pediatric Urology and Urogynecology since August. Dr. Mahal understands that incontinence and other urogynecological problems have been around for so long that many women assume they are difficulties that all women must suffer. Not so, he says. “I want women of any age to know they do not have to put up with these kinds of problems,” Dr. Mahal says.

“Incontinence isn’t necessary. We can do something about it!” Even though he is a surgeon, Dr. Mahal knows that surgery is not the best course for every patient. “There are so many options for the patient to choose from, including muscle control exercises, biofeedback, physical therapy, and various surgical options,” he points out. “I want the patient to have control. The method of treatment should be a decision made by both the patient and the physician…it’s the individual’s decision, not just the doctor’s.” Dr. Mahal notes that most of the surgery he performs involves reconstruction of the pelvic floor to treat a prolapsed uterus. This condition generally affects women who have experienced difficulty when bearing children vaginally. Additional causes may include low estrogen levels, delivery of a large baby, and repeated heavy lifting, among other maladies. Traditionally, urogynecological surgery leaves a large scar across the abdomen. Dr. Mahal explains that that is no longer necessary. “Surgical techniques have been refined and can be as noninvasive as the patient wishes,” he says. “Surgery can be accomplished going through the vagina or the abdomen. Robotic surgery and laparoscopy leave only tiny scars—almost pinholes compared to what was once common—and they are much less invasive.” Another option to consider is the installation of transvaginal mesh to hold the pelvic organs in place. This procedure can be done through either the abdomen or the vagina; Dr. Mahal prefers the vaginal approach for mesh implantation. Although transvaginal mesh is generally more permanent, it can also increase the risk of complications, which may include painful intercourse, bleeding, or damage to the bladder and other pelvic organs. Dr. Mahal cautions, “The FDA recently reclassified the use of mesh to treat prolapse from moderate to high risk.” He believes each patient
should make an informed decision about this and any treatment she receives. Though most of his patients do not choose mesh implantation, he notes, “I’ll implant the mesh if that is her desire.” This demonstrates his attitude that a patient’s course of treatment should not be determined by her doctor alone—each patient must make informed decisions about her own treatment. In addition to Dr. Mahal’s duties as a surgeon at Adult Pediatric Urology and Urogynecology, his responsibilities include training young doctors and interns and doing research. He has privileges at CHI-affiliated hospitals and those associated with Methodist Hospital. The office also partners with the VA hospital on special cases. Adult Pediatric Urology and Urogynecology has been in business in the Omaha area for nearly a quarter century. Mirror, Mirror Fighting the Signs of Aging With Injectables By Monica L. Davis
We all age, whether we’re ready or not. Although many of us would like to age with a graceful attitude, we sometimes catch a glimpse of something in the mirror that we don’t particularly like. Many visible signs of aging are brought on by volume loss as our facial tissue thins out. This, combined with years of facial expressions, allows wrinkles and fine lines to form—even if you spent more time smiling than frowning! Both women and men may feel frustrated with the visual effects of aging that can make a person appear tired, unhappy, or even irritated! Thankfully, dermatologists and plastic surgeons offer many options to help fight those signs of aging. One such option is known as cosmetic injectables. Some injectables work by paralyzing muscles; others are dermal fillers, adding volume underneath the skin. Botox is used cosmetically to remove wrinkles. During the treatment, tiny amounts of botulinum toxin are injected. The injections block signals from the nerves to the muscles in the targeted area. When the muscles can’t contract, the associated wrinkles relax and smooth. The number of injections needed depends on many factors, including the extent of the area being treated. Botox is commonly used for the forehead, the area between the brows, and fine lines around the eyes. The effects from Botox last three to six months. As muscle action gradually returns, the lines and wrinkles begin to reappear. They often appear less severe over time because the muscles are shrinking.
Normal activities may resume immediately after the procedure. Dysport and Xeomin are similar to Botox. Xeomin, the newest form of botulinum toxin, is used to erase dramatic wrinkles caused by repetitive facial muscle movements. Dysport is also used to smooth fine lines and wrinkles between the eyebrows, on the forehead, and around the eyes. Xeomin or Dysport can be administered in about 30 minutes, and most patients resume regular activities the same day. Results are typically noticed within a few days of treatment and will last three to six months. A dermal filler is a gel-like substance that is injected beneath the skin to add volume. The skin over the treated area stretches to accommodate the increased volume, which smooths lines and creases and enhances facial features. Many of these fillers are made with hyaluronic acid (HA), which causes the plumping and hydrating effects. These fillers are typically infused with lidocaine to minimize ›››

discomfort during and after the injection. The Juvederm product family offers many variations of these injectables, which vary in concentration based on the location and depth of the injection. Volbella is the latest FDA-approved injectable in the Juvederm family. Volbella is designed to plump the lips and minimize the appearance of fine lines around the mouth. Volbella has a lower concentration of hyaluronic acid than some fillers, which decreases the chances for swelling. It can be used with other noninvasive injectables such as Botox or other Juvederm fillers. Patients can resume normal activities the same day as treatment. Teosyal Redensity combines dermal filler techniques and mesotherapy. Mesotherapy uses multiple tiny injections of vitamins to restore the skin’s light-reflecting properties. Combining hyaluronic acid and components naturally found in your skin, the serum provides an antioxidant protection to treat dehydrated skin and acne scarring. The treatment is done in three sessions at three-week intervals. Results last nine months to a year. Radiesse works by adding volume under the skin, which, over time, stimulates the body’s natural collagen development. Because it works with your body’s own collagen to diminish fine lines and wrinkles, the injection provides natural-looking results that last for at least a year. For more dramatic outcomes, Restylane injections are gaining popularity. Restylane has a success rate for reducing or eliminating wrinkles around the mouth and nose that less invasive treatments may not reach. Restylane contains no lidocaine; therefore, patients may experience some pain and discomfort during the procedure. Restylane is typically done in a physician’s office under local anesthesia. Selphyl is a platelet-rich plasma treatment that uses your own blood cells to improve facial volume. In one 30-minute procedure, your blood is drawn and processed to separate the platelets. The platelet-rich plasma is then injected into the targeted area on the face. Multiple treatments may be required. Gourmet Food for Thought Recognizing When Something Feels Good I went out to dinner with my brother and his wife last night. It was a spur-of-the-moment decision that ended up being a night full of laughs, stories, and great food. When I left the restaurant to drive home, I realized that I was really quite happy. I always like to “take note” when I’m feeling happy. Recognizing when something feels good and being appreciative of it will bring more of those types of experiences into our lives. Turn Left… Break Out of Your Rut My brother called the other day. We’ve both been traveling, and he was checking in, as we do at least once a month. “How are things?” he asked. “Same old, same old,” I replied. “You need to make a change,” he advised. “Turn left! I did, and I like the difference it has made.” At first, I wondered what he meant. When driving, it’s easy to turn right…no traffic lanes to cross. It takes a bit more effort to turn left, especially from a two-way street. You also can’t get out of a rut without turning the wheels. I decided he was saying I was in a rut (same old, same old). I agreed that I should change things up…try something different. When I go to visit my daughter, I usually take the same route. It’s familiar. I know where to stop and what to expect. It’s safe. This time, I decided to turn left and follow a different path. Las Vegas, New Mexico, was only about 50 miles out of the way. I took a two-lane highway…no traffic (I saw two other cars), no trucks, and an abundance of scenery and wildlife. I visited a state park nestled between red rock bluffs and the Pecos River. I drove through a small town where people waved as I passed (driving slowly because of children and pets along the road). The Las Vegas in New Mexico was built when the railroad was built and that path was being considered for Route 66. A branch of the railroad still comes through town, but the Mother Road crossed further south, where the terrain

is flatter. After that adventure, I decided that turning left is a good idea. Routine has its place in our lives. We all have schedules to keep and deadlines to meet. Ball games, dance lessons, meetings, practices, classes—these things require schedules because they affect many people. Day-to-day activities could never be maintained if our lives were in chaos. Our schedules are usually hectic enough without the added stress of inconsistency. But sometimes, getting out of that routine is like taking a vacation. Get out of your rut. Try something new. Develop a sense of curiosity and a desire for learning. Encourage appreciation of adventure and daring. Show the kids the way…they’ll learn by following the example you set. If the kids have studied something in school that is close by, take a weekend trip to visit that location. It makes what’s learned in school stick…it becomes something concrete. When festivals are staged in your city, take the family. You all might learn something new. Once the family experiences the fun of learning or doing something different, they’ll experience success. Midwest Pain Clinics At the End of Your Rope? Tie a Knot and Hang On! By Julianne Dunn RN, BSN, JD
For most of us, pain management amounts to aspirin for a headache, an ice pack for an athletic injury, or, in the more serious cases, two ibuprofen and rest for back pain. But what if your pain is not relieved by these tried and true methods? What happens if pain interferes with your daily activities and even impairs the quality of your sleep? Is there any hope for relief? The physicians at Midwest Pain Clinics want you to know that the answer to that question is a resounding: Yes! A relatively new field of medicine has emerged in response to the need for more efficient and effective relief of pain. That field is called pain management. Practitioners must be graduates of medical or osteopathy school, must complete a residency in one of four specialties, and then must complete a fellowship in pain management. The physicians at Midwest Pain Clinics are either board-certified or board-eligible and are qualified both to prescribe medication and to perform interventions that can address ongoing pain issues. All licensed physicians who have a DEA (Drug Enforcement Administration) number can prescribe controlled substances for the relief of pain. However, only those with specialized training can perform the sophisticated interventions that can bring more effective and long-lasting relief while reducing the risk of addiction to painkillers. The local Midwest Pain Clinic is located at 825 North 90th Street in Omaha, with satellite locations in Lincoln, in Columbus, and in Dakota Dunes, South Dakota. Physicians, nurse practitioners, and physician assistants offer care to patients in a collaborative atmosphere, where the providers call upon skills and knowledge from a variety of sources to relieve pain. Dr. Rafal Krejza has recently joined the group after completing osteopathy school in Des Moines, an anesthesiology residency at the University of Nebraska Medical School, and a pain management fellowship at Cook County Hospital in Chicago, Illinois. He is board-eligible. Dr. Krejza describes pain management as multimodal, which means it is made up of different methods that work together to manage pain. Among the methods he uses to ease the pain of his patients are prescription medications (often called opioids) enhanced with adjuvants; injections of lidocaine and steroids into the epidural, lumbar, and facet areas of the spine; spinal cord stimulation; physical therapy; acupuncture; and radiofrequency ablation. He has treated patients with nerve blocks and ultrasonography in order to interrupt the pain loop. Dr. Krejza says that pain management specialists are

very aware of the opioid crisis that is affecting patients across the United States and elsewhere. He sees patients who are referred to him by other doctors who have reached the end of their skills in treating chronic pain. There is no doubt that the patients have real pain. There is also no doubt that the dosages of opioids some of them have been prescribed exceed the limits of safety established by the Centers for Disease Control and Prevention. Some patients seek prescription drugs not solely for pain relief but rather for secondary purposes: for resale, maintenance of an addiction, or distribution to family and friends. This is a real danger of pain management that cannot be ignored. However, there are multiple safeguards in place to protect both practitioners and patients from improper prescription and use. One of these safeguards is an accurate assessment of both the source and the intensity of pain. Correct assessment of pain is the greatest challenge for the practitioner. Because pain is purely subjective, doctors must rely on their assessment skills to determine the level of pain and the appropriate treatment. Medical management of pain is a complex discipline that not only relies on prescription medication but also offers an entire menu of treatments to provide relief from acute and chronic pain. Sometimes, neither the patient nor the doctor can identify the source of the pain that the patient is experiencing. That does not make the pain any less real or debilitating. Dr. Krejza and his colleagues view pain itself as the disease, and they endeavor to treat it with all the tools at their disposal. Safe, effective pain relief is their ultimate goal. Breathing Easy Optimize Your Indoor Air Quality

Air quality is essential to clean and healthy living. To get the most out of your indoor air, you need to make sure your air quality appliances are efficient and cost-effective. Even if you’re comfortable with what you have, it’s worth taking a look at your current HVAC, humidifiers and dehumidifiers, and air purifiers to make sure they’re right for your home. Deciding when to replace your HVAC unit can be tricky— after all, if it’s not broken, why fix it? First of all, if you wait until your furnace or air conditioner breaks during peak usage, you’ll probably end up paying more because you won’t have time to do any research. According to Energy Star, a good rule of thumb is to consider replacing any heat pump or air conditioning unit after 10 years, and any furnace or boiler after 15 years. When shopping for new models, keeping track of all the efficiency ratings and scores can be daunting. Keep in mind that the highest rated air conditioning units for 2017 have SEER (Seasonal Energy Efficiency Ratio) scores of 20 or higher. Of course, you don’t choose an HVAC system on energy rating alone. Single-stage units, which operate at full capacity all the time, are popular in the warmest or coldest climates. For those of us living in between, multi-stage units provide a more efficient option. Zoned systems heat or cool selective areas of your home using valves and dampers within your ducts.

New “smart home” technologies allow you to access the HVAC system through your smartphone or other device while you’re not at home. As with any major purchase, it’s natural to be concerned about being oversold when buying a new HVAC. The best way to avoid this is to do some research. First, ask yourself the following questions: Do my utility bills seem too high? Is my current HVAC system noisy? Is there a problem with excess humidity in the summer or dry air in the winter? Are there rooms that are too hot or too cold? Do any family members have allergies? Does the heating or cooling unit constantly turn on and off? Then, talk to your contractor. It’s fine to ask, flat-out, which type and size of system is right for your home (a good contractor will be honest). Ask how the unit he recommends will feel different from what you have now. Get at least three bids to make sure the first opinion isn’t an outlier. Find out if there’s anything you need to do in your home or yard to accommodate the installation. rebates, and be sure to get the warranty in writing. Controlling the humidity in your home may seem like a superfluous task, but it can be important to both your breathing comfort and skin health. High humidity makes the air feel hotter than it is and can encourage the growth of mold. Low humidity makes the air feel colder and can exacerbate dry skin. Ideally, your in-home humidity level should stay between 30 and 50 percent. To keep the humidity down in your house, you can use a portable dehumidifier and take advantage of exhaust fans when cooking and bathing to remove the steam. To dampen the air, portable humidifiers are popular and easy to use. There are also wholehome units, which work in conjunction with your HVAC and control the humidity levels in your house with optimal efficiency. Regular cleaning and maintenance for any humidifier is essential—a dirty humidifier runs poorly and can contaminate the air. Cleaning a humidifier is relatively easy and can be accomplished with white vinegar or bleach, a soft-bristled brush, and plenty of water. Another item to consider is an air purifier. While there is little to no definitive scientific evidence that air purifiers will relieve respiratory symptoms caused by asthma or allergies, the best air purifiers can indeed filter out pollutants such as dust, smoke from candles or fireplaces, and pollen. An air purifier is not meant to be a substitute for regular vacuuming and HVAC filter maintenance, but it can give you an extra boost toward cleaner air. Most air purifiers work by sucking in ambient air and blowing it past a filter. Models that don’t use a fan (they filter air as it naturally flows) are quieter but less effective. Air purifiers need to be cleaned and have filters changed regularly to maintain efficiency. Most models are portable and can be moved from room to room. Beating Breast Cancer Odds Are Up With Community Support

Breast cancer is something no one wants to experience. Thankfully, the odds are higher than ever that a woman will leave breast cancer in the dust for good. Five-year survival rates for women with stage one breast cancer have increased 30 percent since the 1970s. Today, according to the American Cancer Society, these women are 99 percent likely to live. Even those whose breast cancer is more advanced have many more options and experienced specialists to choose from. Breast cancer diagnosis and treatment have become more specialized and accurate. Three-dimensional (3D) mammography is considered a more accurate method than a standard 2D mammogram for examining breast tissue for cancerous cells or masses because it can capture more detailed images. A study out of the University of Pennsylvania found that 3D mammography is better at diagnosing breast cancer and that it helps patients avoid unnecessary biopsies. Most biopsies turn out to be negative, which is good news, but they still result in stress and discomfort for patients and can increase healthcare costs. In some cases, using multiple screening methods can be more effective at analyzing breast tissue for abnormalities. This may include coupling 3D mammography with optical imaging, which uses light to capture images. According to the American Cancer Society, women should start getting annual mammograms at age 45. When they’re 55, they should go in for testing every other year. Women had long been told that
they should undergo annual mammograms starting at age 40—and for women with other risk factors for developing breast cancer, this is still recommended. However, experts have more recently agreed that the benefits of this more rigorous testing schedule weren’t clear. For many women, the unnecessary fear instilled by false positives from mammograms can end up doing more harm than good. For many years, chemotherapy was hailed as the ultimate treatment for breast cancer, even though it has its drawbacks. But as we’ve learned more about how breast cancer can spread and what can prevent it, there are many treatment options to explore outside of chemotherapy. Certain medications can more specifically target what is causing breast cancer to develop or spread. For cancer cells to multiply, they need blood vessels to form around them. This process is called angiogenesis. Blocking this process may stop those cells from spreading. Estrogen-production inhibitors are also being studied to prevent breast cancer. oncoplastic surgery means a surgeon will remove the cancerous parts of the breast while also reshaping or reconstructing the breast tissue that remains. This type of surgery will help maintain the symmetry of the breasts, which otherwise could be widely different in size and shape. In some cases, double mastectomy (complete removal of both breasts) is required to remove all the cancerous tissue. Many women subsequently undergo reconstructive surgery with breast implants. These days, some women are opting out of reconstructive surgery and “going flat” instead. For many years, this wasn’t considered an option. However, as women share their experiences with reconstructive surgery, which may result in complications like infection or implant failure, a supportive group of survivors has formed to talk about why breasts are not necessary to living a happy life as a woman. There are also complementary methods that may be used in conjunction with the treatment a doctor recommends. These may include massage, acupuncture, or eating certain foods to help manage stress and pain. Especially after surgery, it’s important to remain active to reduce swelling and maintain full range of motion. Your doctor will share simple exercises that you should do that will help your healing process. You may also work with a physical therapist to better understand your body’s needs and capabilities. After treatment or surgery, ask your doctor about a survivorship plan, which will lay out when you need follow-up appointments, exercises you should do, possible side effects, and suggestions for support groups. As any doctor will attest, a woman’s self-esteem and positive outlook are vital to her quality of life after breast cancer treatment. Breast cancer is brutal, and it can seem to take away the things that make a woman feel beautiful. But a woman’s beauty is not her breasts and it’s not her hair: it’s her. At Pedodontics, PC, staff members work hard to start each child’s journey of good dental habits in a happy, non-threatening, even fun way. “We talk to our little patients about brushing their teeth, washing away brown spots, ‘pushing’ their teeth to sleep, and ‘painting vitamins’ on their teeth,” says co-owner Dr. Lisa Strunk. “We praise their bravery, and we reward them with prizes from the treasure tower, stickers, and helium balloons.” Nervous patients who need a little more distraction can concentrate on the music and Disney movies playing in the large, open treatment room. Unfortunately, many of the problems that may be easily corrected at a younger age cannot be fully corrected if treatment is delayed until the teenage years. “Just as a younger tree can be encouraged to grow in a particular direction, the jaws and facial bones of children may also be encouraged to grow in more desirable directions,” she says. “Today, we know that younger school-aged children can benefit in many ways from early or interceptive orthodontic treatment.”

Dr. Strunk describes The Pedodontics, PC team orthodontic treatment as divided into two phases if it will benefit the child’s development. The first phase is used to create a better environment for teeth to erupt. This phase of treatment is about the big picture: jaw problems, horizontal and vertical bite problems, modifying growth, expanding the arches, and controlling habits. These things can be very difficult to treat once children have reached 12 or 13 years or when growth is almost complete. “Growth modification as part of early orthodontic treatment is about preventing more serious problems from occurring at a later age,” Dr. Strunk points out. “Growth modification is possible when the bones are still growing and is most effective during a child’s growth spurts.” In many cases, expansion or “developing the arches” can be used to avoid extractions in younger children with crowded teeth. Although expansion may be accomplished in children 12 years or older (and even in some adults), it is generally less effective and much less stable than arch development done at a younger age. A less tangible reason for initiating treatment early is that children in grade school are becoming aware of their own appearance. Children establish ideas about themselves at this age that are based on what others say and notice. Early treatments can have a dramatic effect on a child’s self-esteem, especially a child who has an obvious orthodontic problem. Phase two of orthodontic care is the application of oral fixed appliances. It is indicated most often for children with a large overjet, large underbite, or crowding or rotated teeth. The treatment usually lasts about 18-24 months, and the goals are to try to prevent extractions and to decrease the severity of the overall case. In many cases, Dr. Strunk notes, “if all goes well with phase one, phase two may not be needed.” Dr. Strunk, Dr. Dana, or Dr. Snyder will evaluate your child’s needs and help you decide if a first phase of treatment will benefit your son or daughter. Market Predictions How to Handle the Upcoming “Crash”

With the market hitting all-time highs, a growing number of discussions are popping up in the media explaining how and why the stock market is about to crash. They often include instructions on how the investing public can avoid financial disaster. Several times a day, I am approached by people asking me when the market will collapse and how low it will go. Let’s begin with the fact that cars “crash;” markets simply “decline in value.” Financial professionals know that, but the phrase “decline in value” just doesn’t have the same emotional impact that “crash” does. Whenever someone offers you a market prediction, you should turn and run the other way. The most destructive market decline in my lifetime was the subprime crisis of 2006 to 2009. During that period, the market lost 43 percent of its total value, and dozens of billion-dollar hedge funds went bankrupt. Ben Bernanke, Alan Greenspan, the Federal Reserve, the President’s Council of Economic Advisers, Merrill Lynch, and Jim Cramer are the cream of the financial profession—and they all failed to see the crisis coming. Prior to that, we had the dot-com bubble, when billions of dollars were lost on start-up companies that had no money and no future. Again, the gurus of Wall Street failed to see it coming and came up with an explanation only after the dust had settled. With all their experience, trillions of data points, and the

biggest and fastest computers in the world, why can’t the financial professionals give us any real guidance on the future of the market? The answers are varied but not all that complex. The first reason stems from the fact that 80 percent of all the transactions made daily on the major markets are the results of interactions between giant computer server farms scattered about the city of New York. The only decision-making criteria these trading computers have is that a transaction must result in an instantaneous profit. Their actions dominate the short-term outcome, while their long-term impact is minimal. But, when you think about it, the long run is just a series of short runs. A second hurdle facing market prognosticators is the emotions of the investing public, who own and control 80 percent of the total market value. Now, emotions did not start the subprime crisis. It was started by the greed of financial professionals who saw a way to make billions of dollars by granting the federal government’s

wish that everybody own a home. This led to a decline of the housing market, which created a sense of panic in the investing public. The public thought the only way to save their life’s savings was to sell their mutual funds…after the market had already dropped 25 percent. This led to months of continuous selling. At the end of the day, the market lost 43 percent of its value, while the 500 companies that are a part of the S&P 500 faced little or no change in the value of their business. The point is: The subprime crisis resulted from a sense of fear among the public and was not the result of rational financial analysis. Emotions and fear are difficult to predict. Given current market conditions, discussions about a market decline are not totally without merit. Markets are like humans—after an extended period of exertion, they need to pause and rest before moving ahead. In financial jargon, this situation is called a “correction.” A correction follows an extended period during which the market has seen a series of new highs. To qualify as a correction, the major indexes need to decline by double digits (20 percent is considered the norm). As I am writing this column, the major indexes are only off the record highs by less than 2 percent. A 20 percent correction would require the Dow to drop to 17,600 and the S&P 500 to 1920. We have a long way to go before we can talk about a correction. If you have a 401(k) account and do a little homework, you can make money during a correction. Invest in a major market index fund and have your account set up to automatically invest all incoming money into that fund. As the market declines, you will be purchasing new shares at a lower price, bringing down your average share cost. When the market starts to go back up again (as it always has), you will begin to make money because of your lower average share cost. My final tip? Turn off the financial news shows and ignore all market predictions— even mine. Energetic seniors have amazing opportunities now to simplify their lives while remaining active and engaged in the things they enjoy most. Whether aging in place in their own homes or moving to a retirement community, today’s seniors have terrific alternatives. Never before has this age group been as robust or as determined to make their remaining years full and satisfying. For many seniors, the time comes when the large family home, lawn, and garden become inconvenient. The choice becomes whether to modify the home and hire help or to find a retirement community that is more accommodating. Happily, we have many excellent options. An upscale retirement community for active seniors near my home offers beautiful apartments, attractive grounds, and ample activities to please people with widely varied interests. The apartments range from small but efficient one-bedroom units with kitchen to spacious suites with two or more bedrooms and a lovely terrace. Laundry and housekeeping services are readily available. This community, similar to many around the country, offers
healthy, active seniors many of the most popular services and amenities. Apartments may be rented or purchased. The community calendar is full of activities like bridge, quilting, Wii bowling, and walking or exercise clubs. Meal plans are available, with restaurantstyle dining on site. Since many seniors are still quite active and driving, underground parking is available. For those not driving, transportation is provided on a regular basis. An ample lobby area affords opportunities for social interaction. In addition, there is a meeting room with fireplace, a rec room with billiards, a fitness room, and pleasant green spaces outdoors. A library and theater are available, as well. Health services are provided regularly, including such things as blood pressure checks, foot and ear care, and on-site physical therapy. Residents may opt for such activities as book clubs, live music, movie nights, and social hours on Friday and Saturday evenings. Writers, pitch players, and those who enjoy Tai Chi will make friends with similar interests at regularly scheduled activities. In addition, the community is located near golf courses, entertainment venues, and churches.

For seniors whose financial means dictate a smaller apartment, it is good that many independent living communities offer guest accommodations for out-oftown visitors. Friends and family can enjoy the same amenities, rivaling those of a quality resort. Safety and security are top concerns, with round-the-clock staff on site. Apartments have a host of safety features designed with seniors in mind, including extra lighting, grab bars in bathrooms, and non-slip floor surfaces. While this particular community is limited to those able to live independently, other communities will include many of the same features but with on-site accommodations for assisted living and skilled nursing, should those services become necessary. These continuing care retirement communities (CCRCs) provide the advantage of making just one move. As more care is needed, it is available at the same location. CCRC residents will typically enter into a long-term contract that provides housing, health care, and services for their remaining years. Contracts may be full-life (the most expensive), modified life care, or fee-for-service agreements. It is important to understand all of the terms to make the best decision for one’s own circumstances. Another popular route for today’s seniors is to make modifications to the home you’re already living in or to buy a newly constructed home specifically designed for what has come to be called “aging in place.” If you’re considering this option, check out the National Aging in Place Council website (AgeInPlace.org) for ideas and resources. When opting to stay in one’s own home, some changes to the structure may be necessary. Updates may include things like additional lighting, wider doorways, no-step entries to curb-less showers, easy-to-reach cabinets, or comfort height toilets. The idea of aging in place, whether modifying one’s home or constructing a new home, is to accommodate the common challenges of aging. Reduced mobility, diminished eyesight, and safety issues are paramount. Ask around—some homebuilders and remodelers specialize in home design or renovation for aging in place. Wide doors and hallways, bathrooms with safety features, and special kitchen design are some of their focal points. There are many exciting options out there for active seniors to discover. The holidays are nearly upon us! The holiday season can be a whirlwind of events. Attending one gathering after the next is stressful enough, but having to plan one of your own can really get your tinsel in a tangle. The good news is that there are many places to turn for help. A great place to start, especially if you’re planning a large event, is a local party planner or event service. They will be able to help you through the planning process and can refer you to reputable venues and vendors. Such services vary in price and usually depend on the size of the event and the amount of work involved in the planning. If you want to plan the event yourself, you must first decide what kind of party you want to have, including the level of formality, the budget, and the guest list. Consider a date that is convenient for both you and your guests. If it’s a work function, opt for a Thursday or Friday night after close of business. If you’re just inviting close family or friends, you can probably snag a Saturday or the days around major holidays. The next step is to choose a venue. If your home is large and would be an appropriate setting, that’s always a great free option. However, if you are expecting a crowd of 25 or more (or you just don’t want all your colleagues at your house), you may want to consider an alternative venue. Some good options include private party rooms at restaurants or spaces that cater to receptions. Keep in mind that some venues may require that you book a certain number of hours. And book early—dates close to the holidays usually go fast. Ask the staff at each potential venue if they provide catering services or allow food to be brought in, and whether or not you’re allowed to decorate. Lastly, if necessary, determine if alcohol is allowed and if they provide bartending services (and at what cost). Once you know what your venue provides, you’ll know what other vendors you need to find. For a formal event, you can’t beat professionally designed invitations. Local retailers of fine papers are a good place to start; they can probably refer you to a good printer, if they don’t do printing themselves. Invitations should be mailed at least two weeks before a holiday party. Ask guests to respond a few days in advance so you can get a good head count for the food. Ah, the food—the center of any party. Whether you’re planning a formal dinner or a casual spread of hors d’oeuvres, you should carefully consider who is going to make the food. Even if you’re hosting an event at your own home, you might want to hire a catering or delivery service. You can select prepared platters of the items you want to serve, and you won’t be stuck in the kitchen during the whole party! Caterers can help you determine a menu and figure out how much food you’ll need.

The same goes for the beverages—don’t spend your party stuck behind the bar! Go for self-serve drinks or hire a bartending service. One advantage to using a service is that you can limit the type and number of drinks that are served. Don’t forget to remind your guests not to drink and drive! Hire a valet service to park cars at the door, and have a cab company’s number handy in case someone needs a ride home. If you’re inviting more than a dozen guests, you’ll probably need extra table service. Disposable dishes don’t have to be paper plates—today’s party rental stores carry designer plastic ware as well as fine china. They can also help you with any additional seating, glassware, serving dishes, or linens that you might need. Party rental stores can provide decorations, centerpieces, and party favors. If you want the touch of fresh flowers, a local florist can create anything you need, plus deliver it and set it all up on the day of the event. It isn’t a party without some kind of music. For a small gathering at home, you can make your own playlist. For a larger group, go bigger! Take a formal event to the next level with a string quartet. If there’s room to dance, book a DJ or a live band. Musicians typically charge by the hour and may be booked weeks in advance, especially around the holidays, so start looking around now. extreme (hot stone massaging seats and aromatherapy?) to interiors that could be displayed in a modern art museum, luxury cars offer a dizzying array of fine details, and they are done with attention to long-lasting quality. While mainstream vehicles lose most of their value the moment they are driven off the lot, some European luxury cars (including certain models from Alfa Romeo and BMW) retain their value and may eventually sell for even more than their initial purchase price. In order to increase their customer base, European luxury automakers are now making a variety of models at many price points, from less than $40,000 to $100,000 and more. European luxury auto brands include Aston Martin, McLaren, Jaguar, Bentley, Rolls Royce, Land Rover, Ferrari, Alfa Romeo, Porsche, Lamborghini, and Maserati; the 10 top-selling luxury cars in 2016 (in the U.S.) included Mercedes-Benz, BMW, and Audi. In first place, the top-selling Mercedes-Benz C-Class Sedan offers a lot of power (a standard turbocharged engine

with 241 horsepower, or a turbo/plug-in hybrid with 275 horsepower), intuitive interior controls, touchpad navigation, and a smartphone app for remote starting and more. Their own description speaks loudly to prospective buyers: “Sporty, spacious and sleek, the C-Class proves there’s nothing as elegant as a true athlete. Muscular lines trace a body that’s lean, confident and capable.” The BMW 3 series came in second place, according to U.S. sales for 2016, and is available as a sedan, sports wagon, or Gran Turismo. Its powerful engine offers up to 320 horsepower, comes in a hybrid option, and can go from zero to 60 miles per hour in as little as 4.6 seconds. BMW notes that “near-perfect 50/50 weight distribution gives it a tighter grip on every turn, and its advanced suspension system ensures renowned road-hugging handling.” BMW offers many different packages and options for this model, allowing buyers to customize their own vehicle. Audi made it into the top 10 luxury cars sold last year at number eight with its A4 model, which its website describes as “a moving work of art.” A 190-horsepower turbocharged engine is standard, but a 252-horsepower turbocharged engine is available as an upgrade. driving experience, Audi offers a “virtual cockpit” option with a 12.3-inch digital display and endless customizable informational features. Millions of American women (about one in five) report that they own and use guns, and many more don’t own a gun personally but live in a household where someone else owns a gun. According to the National Shooting Sports Foundation, women are a fast-growing group of gun owners in the United States. Common reasons cited for gun ownership include personal protection, sport shooting, and hunting. Even amongst a single type of firearm, guns are not all the same (or the same size), and gun selection depends on a variety of physical considerations. Of course, women are not all the same size, either, but anyone selecting a gun should consider things like how much upper body strength she has, how big her hands are, and the length and strength of her trigger finger. It’s important to actually handle some guns to determine what model best fits your hand and feels comfortable. Sporting goods stores carry a wide variety of equipment and are great resources for people who generally know what they need. However, a novice selecting her first gun is better off at a local gun club or shooting range, where she can likely get more time and personal attention from staff. Whether or not you own a gun (or are planning to purchase one), anyone who lives in a gun-owning household should take an interest in gun safety. Gun safety classes are available through sports clubs and indoor and outdoor shooting ranges; some training programs may be limited to women only. Many gun clubs and ranges offer personal instruction and mentoring. Even seasoned gun owners may seek additional training, especially if they’re trying out a new type of firearm. You may also find gun safety clinics available through your state’s Game and Parks Commission. The permitting and purchasing of most handguns is controlled at the state or local level. In Nebraska, a prospective purchaser must obtain a handgun purchase certificate from her county sheriff or local chief of police. The certificate is valid for three years and does not limit the number of handguns the buyer can purchase (so long as she continues to meet the legal requirements). The process in your area may be significantly different; your county sheriff or local police can help you get the process started. All U.S. states allow concealed weapons, though most states require a particular permit to carry one (your state’s Attorney General or State Patrol will have this information). In Nebraska, for example, gun owners must complete the Nebraska Concealed Carry Handgun Course to earn a concealed carry permit. The version offered by the Omaha Gun Club includes “information critical in making good decisions while carrying a concealed handgun as well as situational awareness, safety, proper storage of firearms, and other legal considerations.” The course includes both a written

exam and a practical shooting test. You can find courses at a local gun club or shooting range in your area. Safe gun storage must provide an adequate level of protection to prevent unauthorized persons from accessing the firearms and ammunition. Devices available today are designed to provide some of these important storage functions: some can prevent unauthorized operation of the firearm, some can protect a firearm from physical damage, and some can act as a deterrence against theft. Your choice will be determined by your actual needs concerning the storage of a weapon or weapons. One vital consideration is whether or not there are children in your home. Many gun owners take their firearms with them when they travel. Generally speaking, if your gun is unloaded, locked in its case, and stored in the trunk of your car (where it is not accessible to the occupants of the car), you will not run afoul of state or local regulations while you’re traveling with your gun. However, laws regarding the transportation of firearms, especially if you’re carrying them on or about your person, vary widely from one state or municipality to another. When traveling with firearms, it’s best to check with the states you’re going to be traveling through before you head out. When flying, you may transport unloaded firearms in a locked, hard-sided container as checked baggage only. You will need to declare the firearm and/or ammunition to the airline when checking your bag at the ticket counter. TSA personnel may request the key to the firearm container to ensure compliance with TSA regulations; otherwise, passengers should retain the key or combination to the lock. Gun ownership is a serious responsibility that many American women are taking on. With the proper education, training, and permits, you, too, can join the ranks of responsible gun owners. While New York City is the most popular NY destination for visitors, we shouldn’t forget that the state of New York has many other vacation regions to explore. Among them are the Adirondacks, Greater Niagara, the Finger Lakes, and Hudson Valley. Consider combining an outer region or two on your next trip to the city to see more of this great state. Historic landmarks and gorgeous outdoor scenery are located all over New York, waiting for your visit. With a fantastic foodie and farmers market scene, it’s the oldest wine-producing area in the United States and includes more than 100 wineries, breweries, distilleries, and cideries. Beautiful gardens beckon you to walk through them, as does the Walkway Over the Hudson pedestrian bridge—check out those river views! Then, into the city we go! New York City can be seen on foot, on a bike, by subway, or in your car. Take a taxi, a bus (even a double-decker), a boat, or even a helicopter! It’s also a great place to start or end a cruise trip. Broadway shows starting in October 2017 include Bruce Springsteen’s “Springsteen on Broadway,” Clive Owen in “M. Butterfly,” “Meteor Shower” by Steve Martin, and “The Parisian Woman” with Uma Thurman. For the younger set (and the young at heart), “Spongebob Squarepants” and “Frozen” will be coming out toward the end of the year. Or see classic shows like “Phantom of the Opera,” “The Lion King,” or “Cats.” As soon as you decide which shows you want to see, call your travel agent so she can get you some good seats before they sell out. I highly recommend taking a Biblical history tour at the Metropolitan Museum of Art—they are fascinating! I’ve done it twice already, and I wouldn’t visit New York City without going again. Actually, I always recommend taking a tour at a large museum. They can be overwhelming, and you might not know where to start. On a tour, a professional guide can show you to specific artifacts that keep within a theme, making your visit enjoyable and informative. What are some other must-sees in NYC? The view from the Empire State Building, for one. Going across the Brooklyn Bridge and taking a ride on the Staten Island Ferry for shots of the Statue of Liberty is another must-do. Visit Central Park—exploring what’s going on there can take up an entire day, if you want it to. Trace your roots at Ellis Island, take a jaunt around Times Square, hang out at Grand Central Terminal, and visit the 9/11 Memorial and Museum. Your schedule can be as full as you want it to be! What makes New York City so different from everywhere else? Its iconic city skyline alone is unlike any other, but you should also consider what an amazing melting pot it is, with its blends of culture, history, music, fashion, and art. Think about the influential effects it has on international commerce and finance, too. Have your travel agent book you a trip in a historic landmark hotel in the city or in a quaint bed and breakfast near a winery—when you travel to the state of New York, you can choose any type of accommodation. It’s a terrific place to visit as a family, with friends, or even alone— there is something for everyone to love about New York, both in the city and outside of it. R & G Service Center Where Auto Repair Customers Get the Best

The business philosophy at R & G Service Center is: “Our customers are entitled to the best service and convenience.” That is what they provide at each of their locations—635 South 75th Street and 143rd and C Circle. The two automobile repair businesses are individually owned and operated, but they enjoy cooperation. Founded in 1986 by Mike Gordon and Ron Redding, they honor each other’s warrantees and, importantly, they speak highly of each other’s service. Bruce Peterson and his wife, Lori, own the location at 14305 C Circle. Both are quick to praise their entire team. “The success of our business is the result of everyone here being committed to our philosophy and doing their jobs right,” Bruce says. Their dedication to customer service begins in the front office when customers walk in the door. “Rob Bohl, who has been here since 2001, and Chad Buhrman, who has been here since 2006, are part of our combined 228 years of experience,” Bruce continues. “They are our first customer contact, and we really rely on them getting it right at the outset.” As we can all attest, most people are not at their happiest when their cars aren’t running well. “Rob and Chad offer courtesy and the assurance that we will solve our customers’ problems quickly,” Bruce says. “They know how to explain needed repairs and prepare estimates. The best part of their job, they say, is contacting customers with the welcome news that their problems have been solved. Judging by our number of loyal customers, Rob and Chad do their jobs really well.” Bruce oversees the shop, assigning repairs to the right technician and making sure the repairs are done right and in a timely fashion. “Getting the car running well and back to the owner quickly is essential for customer satisfaction.” Bruce notes that the field of auto maintenance and repair went unchanged for many years, but that time is over. “From the Duryea in about 1900 to 1973, cars stayed the same,” he points out. “In 1973, automakers began to change the way cars work. They found ways to make cars more powerful, faster, economical, and more comfortable. And the changes keep coming.” The most recent changes have been technological. Today’s autos can practically talk to the driver about problems large or small, potential or immediate. “Cars have become computer-reliant in more and more ways,” Bruce continues. “Today, anything in a car that runs with a motor is computer-operated. When we repair or replace a part that works on a motor, we

have to teach the computer to communicate with the new module. If the computer cannot talk to the repaired part, it will not work. We have frequent training to keep our technicians savvy about the technology, so we can and do repair every make and model of car and light truck—even the newest and most tech-loaded.” Technology also makes cars safer. All those signals from the dashboard are essentially computergenerated. A certain one means you need to tighten the lid to your gas tank. Another is the familiar warning that it is time to stop at the pump and fill up. “If the warning light is red,” Bruce advises, “it means to stop immediately. Serious damage to your car may result if you don’t. That’s why we offer 24-hour towing service.” (Call 402-681-5443 if you need a tow.) Of course, not all repairs involve computer technology. The technicians at R & G Service Center are also well-trained in mechanical repair. “We still do brake jobs and wheel alignments on everything from vintage cars to the most modern.” With 10 employees and plenty of room, they can complete necessary repairs quickly, often the same day you bring your car in. Jumping Into Fall A Sure Cure for End-of-Summer Blahs! By Sharon Knierim
Sometimes, we find ourselves feeling sad toward the end of summer. Soon to be gone are the exciting colors and hues of summertime blossoms and the wild and wonderful constant growth of those glorious summer months. For gardeners and gardening enthusiasts, summer is the heyday of nature’s wonders. But don’t be downhearted. A sure cure for those end-of-growingseason blues is the opportunity to enjoy the more subtle shades of autumn (not to mention the refreshingly cooler weather). Nature’s color palette is different during the fall months, true, but maybe that is a good thing. After all that riotous color, we need to relax and tone down the color scheme in our gardens. Of course, if you’re someone who particularly enjoys more subtle color, this is definitely your time of year. Now are the weeks to enjoy tones of reds, oranges, yellows, and browns. While it is impossible to pass through a summer without the rampage of color, so too is it unthinkable to not enjoy the subtle palette of nature’s more calming shades. If a garden has been planted with all seasons in mind and trees and shrubs have been placed for maximum time to “show off,” September, October, and even November can be absolutely beautiful and very satisfying. Pumpkins planted in the vegetable garden or in the compost are now impressive in their different shades of orange. Whether
harvested and displayed or left in the field, these orange-brown delights add plenty of interest to any landscape. Remember, pumpkins are relatively easy to grow and maintain. A special bonus in growing pumpkins is the vine’s ability to cover up some not-so-attractive areas in the garden. During the summer, pumpkin vines and their blossoms grow beautifully and lustily. As autumn visits us, the vines begin to wither, leaving the big orange spheres that we all enjoy. Trees can be real showstoppers when autumn arrives. We all notice that some species give us a hint of what’s going to happen. Beautiful autumn show trees have leaves that range from brilliant red and fabulous yellow to tawny shades of brown and copper. In some areas of the country, these trees provide the reason for flocks of tourists who enjoy this display covering the landscape. You can have the same beauty in your own garden by planning and planting the correct tree or trees for exceptional autumn beauty. Consult a garden expert to be sure you select the right tree

to grow and enjoy. A fellow gardener and I have had a long-standing debate regarding the Autumn Blaze Maple. This species is the most beautiful autumn show tree, but—being a maple tree, it has its drawbacks, including shallow roots and weak limbs that are susceptible to storm damage. Needless to say, the debate continues. Is it worth it to plant a stunning tree that might become problematic? That decision has to be made by each individual, but observing the beauty of those scarlet leaves during autumn is my argument. It is absolutely breathtaking! Autumn is the perfect time to plant a tree in the Midwest. A favorite plant for many fall gardeners is the chrysanthemum. We have discussed the mum here before. This plant is hardy, sturdy, and truly reliable. Many of them can withstand drought and the searing heat of summer. Not much maintenance is required, with the exception of watering and the separation of plants every few years. By separating mums and replanting clumps here and there, the number of mums can be increased significantly. The color palette for mums is unbelievable, including white, yellow, pink, gold, and purple. Be sure to keep them trimmed back until at least the Fourth of July to avoid premature bloom, and just wait for their display of pleasing colors. Mums are usually the last bloomers in the garden. Along with asters, they will provide welcome color until at least the first hard freeze. Covering them at night with a light blanket when frost or freeze warnings are issued will sustain them and keep them blooming longer. For the most part, many gardens, yards, and landscapes naturally decorate themselves for their fall showing. Mother Nature has a way of revealing her beauty without too much human manipulation. However, because we are human, we enjoy enhancing what is already available and we are full of curiosity. It is instinctive, then, that we love to plant and experiment in league with nature. Dog Rescues Volunteer, Adopt, Love By Kathy Glow
An estimated six million dogs are currently living in shelters or with rescue organizations, needing a home. With so many furry friends that need placement, more and more people are turning to rescue organizations to find their forever friends. Animal rescues and animal shelters are often thought of as interchangeable terms, but they are not. There are two major differences between shelters and rescue groups. Shelters are usually funded by local government and staffed by employees, whereas rescue groups are funded mainly by donations and staffed by volunteers. While some shelters put animals into foster homes, many are housed on site in kennels. An animal rescue group is dedicated to pet adoption. These groups take unwanted, abandoned, abused, or stray pets and work to find forever homes for them. Rescue groups exist for many pet types but are most common for dogs and cats and may specialize in specific breeds. A rescue can be a private citizen or a non-profit organization. Dog rescues typically take only a certain number of dogs and use foster homes or boarding kennels to house them. They may work closely with animal shelters to place adoptable dogs in homes, avoiding a stay in a shelter. Rescue organizations are often looking for volunteers to staff offices, care for animals, or be a foster family to a furry charge. Generally, you will need to set a schedule with them rather than popping in whenever it is convenient for you. Whether you are there for clerical work or animal care, they may ask you to complete certain types of training. They will ask you a variety of questions, such as the types of tasks you are willing to perform, whether you have allergies or live with someone who does, and—most importantly—if you are willing and able to foster a dog. While there is generally no monetary compensation for volunteers, a rescue organization often pays for all expenses and equipment needed during the duration of the foster dog’s stay. If you are interested in volunteering, particularly in fostering a dog, you need to ask yourself certain questions: Are there tasks you are unwilling to do (after all, someone has to clean up after the dogs)? Are you able to handle the potential heartbreak of getting close to a dog and then seeing it go to another owner? Are you willing and able to care for dogs that may have medical or behavioral needs? If the answers are yes, you are likely a good match for a dog rescue. Find rescue groups by visiting sites such as VolunteerMatch.org, PetFinder.org, or AKC.org (the American Kennel Club’s website). These sites can help you find organizations in your area that are looking for volunteers. No matter what kind of dog you might want—large or small, purebred or mixed, old or young—there is an adoptable dog out there, waiting for you. Rescue organizations acquire dogs from breeders, from shelters,

or from owners who, for one reason or another, had to give up their animal. Many of these organizations will have “meet and greet” times at local pet stores or pet supply stores so that the community can get to know them, see the types of dogs they have available, and learn about the adoption process. Most also have websites on which they post the dogs’ pictures and information about them, such as their breed and age and any background information that potential owners might wish to know. Because dog rescue organizations pour so much time, love, and money into their pets, adoption fees generally range from $150 to $450; many times, at least a portion of that money is tax-deductible. The organization will have you fill out an adoption application, conduct a phone or in-person interview, and ask to visit your home to evaluate it for the safety of the dog you’re interested in adopting. This may seem like a lot, but it’s the organization’s job to make sure pets and owners are matched carefully and lovingly. In return, you get a pet whose behavior and medical history are well-known (and may already be trained, microchipped, vaccinated, and spayed or neutered). You will also receive post-adoption support and a solid return policy, should the pet surprise everyone and not be a match for your home. Since each rescue is so different, make sure to ask about their policies before deciding to adopt. Adopting a dog from a rescue group means that you are saving an animal’s life and giving it the wonderful home it deserves. You are choosing a dog simply because it needs you and you need it.
