column on September 9, and it is darned cold out there. The long-range forecast doesn’t look a whole lot better! I am a huge fan of autumn, but not this early. My husband, Manny, dug out a winter coat for me before we went to grab some dinner last night, and it felt so good! I’m hoping this sudden chill is not a preview of the winter we will be heading into. Just this past weekend, it was in the 90s. Manny and I hosted a gettogether at our house for my daughter, Carissa, her husband, Ken, and my granddaughters, Savannah and McKenna. We had a terrific time. Ken provided a smoked salmon that he had prepared on his Traeger grill. Manny and I provided the garnishes: cocktail-size rye bread, sour cream, chopped hard-boiled egg, capers, chopped red onion, chopped parsley, and caviar. We added the salmon and topped it off with a squeeze of lemon juice and a little bit of freshly ground black pepper. Now, many kids wouldn’t even try this wonderful meal, but not our girls! This is the second time we have all shared this meal. We all love it…including the girls. Ken does a terrific job smoking the salmon. Then we all sit around and build little sandwiches, talk, laugh, and enjoy one another’s company. It is very a very “communal” experience. In addition to that, it is one of the tastiest meals I’ve ever had. I’ve been doing this recipe for decades! It came from the Silver Palate Cookbook series. It’s always a huge hit when I bring it to parties. Years ago, I took it to a Christmas Eve get-together. My Aunt Bea, who was 90 years old at the time, told me, “This is the best thing I have ever eaten!” Now, that’s a testimony you can’t beat. I sat by my sweet Aunt Bea, who passed away several years ago. I couldn’t believe how many salmon sandwiches she ate. She was a tiny lady, but I kept making her the tiny sandwiches, one right after another. It is a very fond memory! Give it a try; I really think you’ll like it. To build the sandwich, Carissa strongly recommends that you put the sour cream on the cocktail rye and then “put anything that rolls on the bread next!” She’s right. I hope you all have taken some family time throughout this year. There is nothing like sitting around a kitchen table with our loved ones to make life worth living. Nature has been an inspiration for clothing designers for centuries! In the 1700s, wearing animal prints became popular as a sign of influence and elevated status. Animal print has become a classic since then and continues its reign in the fashion jungle this fall.You can find these versatile prints in subtle, bold, and sassy designs. Don’t be afraid to invest in this ongoing trend—it will be on point for years to come. Based on what we’ve seen on the runways recently, zebra print is going to be the most popular animal print this fall, with tiger stripes, snakeskin, and leopard spots not far behind. The usual way to style animal prints is to pair them with neutrals or black and white. This year, consider some new ideas to keep the look fresh and a little wild! Try choosing an animal print that incorporates a bright color. For example, instead of a black-and-white zebra stripe, designers are offering zebra stripes with bright blues, yellows, and brilliant greens. If you love wearing a pop of color along with a wild print, this kills two birds with one stone! You could also choose to pair your zebra or leopard print with red. This combination looks surprisingly chic! For the more conservative fashionista, fabulous animal print accessories can top off any look. As you may have noticed, belts are back in a big way. Turn a ho-hum outfit into a trendy one by adding a leopard belt. If you like basic black, add a little zest to your outfit by rocking some animal print shoes, boots, or a bag! Have fun creating unique, stylish combinations with animal print accessories. Along with the animal looks, the natural combination of fur comes into play this fall and winter. Faux fur done in creative ways is perfect for those of us who live in the polar vortex! On the coldest days, you cannot beat faux fur for warmth and style. If you’re in the market for a winter coat this year, take special note of the oversized fur collar. The fall classics we know and love are still with us this year. Preppy looks, along with Scottish plaids, are very much in style. These exuberant patterns have been part of the fall fashion world for decades and settle nicely into a vintage feel. All-black is back in a big way this year (that is, if it ever left). Embellished with ruffles, netting, faux fur, and glitz, the all-black outfit this year is anything but boring! We will also see lots of bold reds this fall. Not since the heyday of Valentino red in the 1990s has the color felt more relevant. For an uplifting boost to your wardrobe after a difficult year, a red dress may be just what you need. Red paired with black is also a vibrant combination sure to give anyone a lift, including the people wearing it and the lucky folks who get to look at it! Entertaining details are looking fresh again this year. On handbags, vests, and skirts, fringe is always fun! Pronounced shoulders and mutton sleeves will be found everywhere. These bold shoulders and sleeves remind us of a bygone era of glamour and fantasy—the look conveys a feeling of power.

My favorite trend this year is in shoes. High statement strutters are out of the picture, as the focus is on more comfortable, lower-heeled shoes. Loafers that have that Gucci vibe are perfect with the animal prints and plaids coming out this year. For those occasions when you want a more feminine look, Mary Janes or sleek sling-backs can be the perfect choice. Boots are featuring low-heeled looks, also. The rain “welly” will be seen everywhere this year. Practical and stylish, the welly is a great all-weather boot. This tall boot, usually made of a seamless weatherproof material, is named after the first Duke of Wellington. Wellies have been popular in England for a long time and have made their way across the pond. It is a stylish boot that is truly weatherproof. Good news for the bling lovers out there: After a summer of minimal jewelry, we can indulge in more statement pieces this fall. Gold is the expensive look and a chic choice. Silver is relegated to a more casual vibe. Multi-purpose gold chains are perfect for fall and can also be worn as belts! After a year of challenges, we look forward to a vibrant fall, with beautiful colors and patterns to create a warm, comforting message. Find something to bring out the exotic in you! Fall Harvests and Celebrations—Let’s Eat! By Kate Beiting
Four-Cheese Fettucine Cook a 1/2-pound box of fettucine according to package directions. Drain and return to the pot. In a large saucepan, melt 2 Tablespoons of butter and stir in 1 and 1/3 cups of Half-and-Half or light cream. Add 1/2 cup of grated Fontina cheese, 1 cup of grated mozzarella cheese, 1/3 cup of grated Romano cheese, and 1/2 cup of grated provolone. Cook and stir for 3–4 minutes until the cheese melts. Add 1 Tablespoon of parsley and 1/2 teaspoon of black pepper. Add the cheese to the fettucine in the pot. Stir until combined. Easy Nachos Fry up and crumble 1 pound of ground beef or chicken strips, seasoning with 1 teaspoon of cumin or chili powder, a can of green chilies, and 1/2 diced onion. Spread a bag of your favorite tortilla chips onto a large cookie sheet. Spoon the meat mixture all over the chips. Sprinkle with 2 cups of your choice of grated cheese. Bake at 350 degrees for 20 minutes until bubbly. Keto-Friendly Skillet Quiche In 1 Tablespoon of butter, sauté 5 sliced scallions for 5 minutes. In a bowl, mix 6 eggs, 2 three-ounce packages of soft cream cheese, and 1 cup of milk. Stir in 1 cup of grated cheddar cheese, a few

dashes each of salt and pepper, and 1/2 teaspoon of dried or fresh chopped basil. Pour the egg mixture into a skillet and cook for 2–3 minutes, until the mixture is set. Cover and turn heat to low. Cook for another 15 minutes, until a knife inserted into the center comes out clean. (If you’d rather bake, prepare the mixture in an oven-safe skillet and bake for 10–15 minutes at 375 degrees.) Corn Muffins Mix together 1 cup of flour, 1/4 cup of sugar, 4 teaspoons of baking powder, and 1/2 teaspoon of salt in a large bowl. In another bowl, blend 1 egg, 1 cup of milk, and 1/4 cup of vegetable oil. Mix the wet ingredients with the dry. Portion the batter into a paper-lined 12-

cup muffin tin. Bake at 400 degrees for 20–25 minutes. Brush the tops with a bit of melted butter when you remove them from the oven. Bread Dough Pizza Thaw 1 pound of frozen bread dough. In a large skillet, heat 2 Tablespoons of olive or vegetable oil and sauté a mixture of 1 diced small eggplant, 1 or 2 diced onions, 1 diced pepper, and 1 clove of minced garlic. Sprinkle with salt and pepper and fresh or dried oregano. Sauté for 3–4 minutes. Pat the defrosted bread dough into an oiled pizza pan or cookie sheet and form a rim. Bake for 10 minutes at 450 degrees, then remove from oven. Top the dough with a jar of pizza sauce, spices that you like, and the sautéed veggies. Top with 2 cups of grated mozzarella cheese (or your choice of cheese). Return the pizza to the oven and bake for 5–10 minutes, until the cheese is hot and bubbly. Curly Mac Salad Cook 2 cups of curly pasta noodles (cellentani, corkscrew, elbow, or rotini). Add 1 medium chopped onion, 1/2 chopped cucumber, 1/2 chopped green or red pepper, and 1 chopped tomato. Mix everything together. In a Mason jar or a bottle, add 3/4 cup of canola oil, 3/4 cup of sugar, and 3/4 cup of vinegar. Close the jar and shake until mixed. Add 2 Tablespoons of fresh chopped parsley, 1 Tablespoon of dry mustard, and 1 and 1/2 teaspoons each of salt, pepper, and garlic powder. Add a dash of Lawry’s seasoned salt, if desired. Pour the dressing over the salad and mix well. Refrigerate for 24–48 hours. Ghoulish Goulash in a Pot Sauté 1–2 pounds of ground beef or ground chuck along with
as much diced onion, green pepper, and garlic as you like. Put into a slow cooker along with 1 can of diced tomatoes, 1 small can of tomato paste or pasta sauce, 1 cup of beef broth, and 1 small can of drained mushrooms. Season as you wish with salt, pepper, and paprika. Cook on low all day or on high for 4 hours. During the last 30 minutes of cooking, add 1–2 cups of cooked pasta—elbow macaroni, small shells, noodles, or whatever you have on hand. Serve topped with shredded cheese and along with a salad, garlic toast, or corn muffins. Halloween Pumpkin Bars For the pumpkin bars: Whisk together 2 cups of flour, 1 and 1/2 teaspoons of baking powder, 1 teaspoon of baking soda, 1 and 1/2 teaspoons of ground cinnamon, and 2 teaspoons of pumpkin pie spice; set aside. In a large mixing bowl, mix 1 cup of vegetable oil, 3 large eggs, 1 cup of light brown sugar, 1/3 cup of white sugar, a 15-ounce can of pumpkin puree, 1 and 1/2 teaspoons of vanilla extract, and 2 Tablespoons of pure maple syrup (NOT pancake syrup). Add the wet ingredients to the dry until combined. Pour the batter into a greased jelly roll pan (10x15 inches). Bake at 350 degrees for 25–30 minutes or until a toothpick inserted in the center comes out clean. Cool on a wire rack for 45 minutes. For the cream cheese frosting: Mix an 8-ounce package of soft cream cheese with 1/4 cup of soft butter, 3 cups of powdered sugar, 1 teaspoon of vanilla, 1/4 teaspoon of pumpkin pie spice, and a dash of salt. Mix until creamy. Spread onto cooled bars. Decorate with an assortment of Halloween sprinkles or sliced candy pumpkins. Cut into bars. Store in refrigerator. WE

Receiving Visitors Again! Lancaster Rehabilitation Center

Jen Graff’s excitement is visible when she speaks of Lancaster Rehabilitation Center (LRC) opening its doors to visitors after nearly six months of closure. On March 7, LRC closed its doors to comply with pandemic regulations designed to keep staff and residents safe. However, Jen shares, its excellent services to residents didn’t slow down. “Day-to-day operations did not change, but responsiveness did,” Jen says. She is associate administrator at LRC. “Guidance was changing and shifting every day, every hour. It was a scary time for a lot of people.” Getting the correct message out to the public, providing guidance for the 300 staff members, and ensuring safety for the 220 residents was a challenge, but Jen knew it could be done. With cooperation from staff and residents, LRC had great success navigating the changes. They also worked with local agencies like Lincoln/Lancaster Emergency Management, the Public Health Department, and Health & Human Services to ensure quick response and to provide personal protection equipment, as needed. When the doors shut to visitors, the dining room closed down and group activities ceased, the residents and the staff of LRC handled it in stride, Jen notes. “The most critical element was maintaining as much normal as we could,” she says. “Our residents are the most vulnerable population, and they will always be our first priority.” Since day one of the shutdown, staff have worked to make the residents’ days bright by bringing in window visitors, inviting sidewalk chalk artists, and facilitating video calls so residents could communicate with family. Jen thanks the community’s abundant volunteers, who give so freely of their time. She also can’t give enough credit to the flexibility of their residents’ family members. “We have the best family members,” she raves. “They’re so understanding and appreciative.” LRC’s dedicated staff members work hard to maintain some normalcy during the constant changes, Jen notes. “We try to have a good time around here. We didn’t want to stop our day-to-day. We keep doing what we do best while keeping our residents safe at the same time.” Safety has always been the top priority in the state’s largest skilled nursing care facility—that’s one thing that the pandemic hasn’t changed. The staff at LRC are proud that the facility passed the baseline testing process with flying colors in midAugust and was able to progress into Phase 3 on August 17. This meant that the residents could begin enjoying one-on-one visitations in a controlled atmosphere for 20-minute intervals. Jen is ecstatic that this is happening. She says the

interaction of residents with others is important not only to family members but also to her. Working in the facility for 11 years makes it almost home. “It is very important for me to have interaction with the residents every day,” Jen says. “It’s something I just have to do.” Even amidst the additional logistics and anxiety that the pandemic has brought, staff members at LRC find time to offer residents the high levels of care and consideration that they have come to expect. LRC has three divisions with different levels of care. The short-term rehabilitation division helps residents of all ages regain a full range of functional abilities while preventing complications. The long-term care division provides 24-hour health care with certified nursing assistants, culinary services, physician services, and social and therapy services. The memory care unit provides safety and engagement for residents with dementia and also helps their family members stay involved in their loved ones’ lives. The care at LRC is top-notch, and luxuries for the residents make the facility a prime location for loved ones. They offer an outdoor gazebo with a secure courtyard, a bistro, on-site physician services and dietitians, individualized activity programs, a beauty and barber shop, cable television in each room, free Wi-Fi, a library, a theater, and a Husker lounge, to name just some of the amenities. “We want our residents to feel at home and to be able to go home successfully after rehabilitation,” Jen says. “We want our residents to have what they need.” The staff members at LRC are well trained and also bring a sense of loyalty to the residents. As associate administrator, Jen believes in giving everyone the utmost dignity and respect. The resolve of the entire LRC Team has made them one of the most successful skilled nursing homes in the state, pandemic or no pandemic. Accessibility-Safety-Convenience Aging in Place

Many seniors are proactively deciding where and how they’d like to live out their later years. They choose and prepare for as much comfort and independence as possible. With the number of Americans aged 65 and over projected to reach 70 million by 2030, businesses and services are gearing up to facilitate aging in place. What is “aging in place”? The CDC defines it as “the ability to live in one’s own home and community safely, independently, and comfortably, regardless of age, income, or ability level.” Essentially, it means living in a place of your choosing that is set up to handle whatever issues aging might bring. For some, aging in place may mean making modifications to their existing home in order to remain there. Others may choose to downsize to a smaller home or apartment with special features designed for seniors. In some cases, these apartments or condos may be in communities designed specifically for aging residents. Aging in place means anticipating the inevitable changes that come with age and putting in place solutions to ensure safety and accessibility. Other considerations may include convenience to needed services and opportunities for social interaction. We never know just what the future holds, but some changes that accompany aging are predictable. Mobility issues, declining vision, diminished agility, and decreased hearing are common examples. Few existing homes offer accommodations needed to face these challenges.
Luckily, a host of professionals are specially trained to assist with planning and implementing needed modifications. Many builders and remodelers possess expertise in universal design, which makes homes attractive, accessible, safe, and usable by people of all abilities. Certification as an Aging in Place Specialist involves advanced training and proficiency for occupational therapists, builders, and contractors. It enables them to assist seniors in making choices that allow them to age in place. Modifications to an existing home or features to consider when building a new home include zero-step entries, additional lighting, main floor laundry, grab bars in bathrooms, walk-in showers or tubs, and doorways, hallways, and kitchens wide enough to accommodate a walker or wheelchair. Covered entrances, raised toilets, and easyto-use knobs and switches are age-friendly, as well. As strength and balance deficiencies develop, seniors are more likely to fall…and they’re more likely to experience fractures if they do. a stairlift can provide safer navigation. Even with the cost of remodeling, remaining in one’s home often involves less expense than moving to a retirement facility. Many seniors wish to remain in their family home and can do so with modifications and assistance with yard and home maintenance. Others may be happier in a retirement community without the responsibilities of maintaining a house and grounds. Some seniors will eventually need the skilled nursing care they can only get in an assisted living facility, and that may not be the best moment to move to a new home. Here, the concept of continuing care might be beneficial. In this situation, a facility provides different levels of care in the same location: independent living, assisted living, and long-term care. Independent living works well for those who can manage their own care. This is often a comfortable apartment with a compact kitchen. Residents may choose to do their own cooking or have community meals with other residents. Amenities, planned activities, and events help keep everyone engaged. When more support is required with daily activities, an assisted living arrangement will provide for those needs. Accommodations are also typically apartments, but trained staff provide help with such things as bathing, reminders, toileting, personal care, and housekeeping. Many of the same activities and amenities are available as with independent living. Such communities are likely to include plenty of outdoor space. Activities may focus on hobbies or personal interests, while others may include transportation to local shopping or entertainment. Spiritual needs are addressed, as well. The great advantage of a continuing care retirement community is that one can move from independent to assisted living while staying within the community. It might be just down the hall or to another floor. In a similar way, if skilled nursing becomes necessary, one moves to a specialized wing or perhaps another building on the same campus. It provides a sense of stability and supports social contact to stay within a community that is familiar. We can’t prevent aging, but if we plan ahead for it, we can maximize our independence and our ability to live as we choose. No matter where you want to age, a little foresight and planning can ensure that you get to choose the place. Non-Invasive Fat Reduction A Slimming Solution

We’re all looking for spiritual, emotional, and physical well-being in one way or another. Physical therapy connects these three concepts, shining a light on our best selves. When physical therapists help alleviate physical pain, they’re also helping us to be the best we can be and to overcome things we thought we couldn’t. Functional movement is key to our maximum health—mind, body, and soul. Physical therapy (PT), also known as physiotherapy, is a health profession that treats chronic pain, physical deficiency, and dysfunctional movement, all of which usually result from an illness, injury, or trauma. Whether you’re being treated for cancer, recovering from a heart attack or stroke, rehabilitating after a sports injury, or hoping to avoid your family’s propensity toward osteoporosis, physical therapists can help. They are highly trained to help people of all ages live their healthiest and most active lives. They develop individual treatment plans to restore function and reduce pain. They even incorporate fitness goals for prevention, independence, and overall wellness. In the United States, a physical therapist earns a bachelor’s degree before advancing to a doctorate in physical therapy (DPT). Study for this advanced degree is approved by the Commission on Accreditation in Physical Therapy Education, and the course of study usually takes three years to complete. Even after obtaining these degrees, graduates must pass a state licensing exam before they can practice. Biology, anatomy, exercise physiology, pathology, kinesiology, and neuroscience are only a fraction of the curriculum studied by physical therapists. PTs are also well-versed in behavioral science, communication, and clinical reasoning so they can identify and take on each patient’s challenges. Physical therapy practices like massage, manual therapy, and hydrotherapy have been around for thousands of years. In the 18th century, musculoskeletal physicians (now known as orthopedists) developed gymnastic-like machines to help treat gout and other joint diseases through systematic exercise. Modern physical therapy was better established in the early 20th century during global events like the 1916 polio outbreak. During the First World War, women trained in physical education were recruited to help restore function to wounded soldiers. Physical therapists were standard in hospitals around the time that polio vaccines were developed in the early 1950s. By the end of the decade, they had moved into their own clinics, schools, nursing homes, and rehab centers. Today, physical therapists can specialize in just about every minor ache or major trauma, neurological rehabilitation, or wellness program to help prevent loss of mobility before it starts. Sprains, strains, fractures, and dislocation are just some of the damage you can experience after a sports injury. These injuries are not uncommon among those with an active lifestyle. Whether you developed shin splints running long distances, twisted your knee in a fitness class, or tangoed your way to hip flexor pain, your suffering warrants the attention of a physical therapist. Manual therapy helps decrease the pain, dizziness, and motion problems that can accompany a concussion. All that said, physical therapy isn’t just for athletes. It can help people with a surprisingly large range of difficulties. A receptionist who fights lower back pain from sitting at a desk all day, a warehouse stocker with a pinched nerve, a senior who needs to improve balance to avoid a fall, or a mom who is tired of putting up with incontinence after childbirth—all of these people could benefit from the help of a physical therapist. There are many specialized types of physical therapists, but they are all trained to analyze your situation and teach you how to improve it with exercises, stretches, and, sometimes, complex equipment. Orthopedic physical therapy helps those who want to stop hurting and start healing after surgery. The musculoskeletal system in our bodies includes muscles, bones, connective tissue, and all the parts of moving joints, like tendons and ligaments. Some common orthopedic physical therapy treatments include joint and spine mobilization, therapeutic exercise, muscle re-education, neuromuscular treatment, electrical muscle stimulation, and dry needling (similar to acupuncture). Physical therapy can even be helpful before surgery, to make sure the muscles and bones are in the best possible shape, giving patients a better chance for a complete and timely recovery. Some therapists specialize in women’s health, solving problems related to the reproductive system and pelvic floor. Some women experience unexplained pelvic pain, the cause of which remains elusive even after extensive testing. Experts say that one of the underlying causes of chronic pelvic pain is myofascial, which means the body’s soft tissue is inflamed and painful. Pelvic physical therapy can help alleviate such pain.

The pelvic floor is a bowl-shaped muscle that holds your bladder, bowel, rectum, and uterus. It’s an important body part! After childbirth, and as we get older, the pelvic floor can weaken. You have probably heard of Kegels, the exercise of contracting and relaxing your floor muscles. This is only one way to work this important muscle system. Your physical therapist can guide you through measured breathing coordinated with exercises like squats, bridges, and lunges to help with pain management, urinary incontinence, pregnancy pain, and constipation. Breast cancer treatments like chemotherapy and radiation can cause physical and mental stress. Scar tissue; pain in the shoulders, neck, and back; neuropathy; bladder and bowel changes; and lymphedema are just some of the challenges that a physical therapist can address. As a matter of fact, physical therapy can help during all stages of cancer treatment: before surgery, during medical treatment, and after surgery. Your therapist will assess baseline measurements of your shoulders and spine (to help identify impairments during treatment and recovery) and establish an exercise program to keep you on track. After surgery, and during medical treatment, manual therapy helps ease joint pain and muscle stiffness and increases your range of motion. Some physical therapists specialize in lymphatic drainage techniques. Lymphedema is often provoked by a lymph node removal during cancer treatment. The lymphedema blocks the lymphatic vessels, and this can lead to troublesome fluid retention. Manual lymphatic drainage, compression, and specific exercises can help relieve swelling in the arms and legs by moving the lymphatic fluid from your limbs to help keep you comfortable. You may not think of headache as a condition that responds
to physical therapy, but it is! Stretching exercises can help to loosen the tense neck and shoulder muscles that often trigger tension headaches. Therapists also use relaxation techniques, deep breathing, and manual therapy for neck function and correct posture. Even jaw dysfunction can cause a headache—if you’re grinding your teeth, seek help. Lower back pain is a common problem in our stress-filled lives, and physical therapy is an effective way to address it. Though a heating pad on the lower back before exercise or ice packs afterwards are tried and true, they’re not the only ways to go. Electrical stimulation to the lower back muscles can be a good solution, especially to prevent further lumbar problems after surgery. A physical therapist can also develop an exercise program to strengthen your core. Your lumbar spine is dependent on lower back muscles and their support from abdominal muscles. The muscles in the front and back need to be equally strong so that less stress is placed on the spine. Experts think of this as developing a “belt” of muscle around the spinal column, and a physical therapist can show you the best way to go about it. Last, but not least, our feet bear the shock of everything we do all day long. Physical therapy can address everything from plantar fasciitis to collapsed arches so that we can focus on keeping our feet healthy. Strengthening, range of motion, and flexibility exercises help correct what we might not know is wrong until it hurts. Breast Cancer Care Advancements Are Saving Lives

Ongoing research offers new ways to detect breast cancer early and treat it successfully. That’s great news, since statistics show that early detection is decreasing breast cancer deaths. However, the numbers are still sobering: In the United States, one in eight women will develop invasive breast cancer. Although the death rate from breast cancer has decreased, over 42,000 American women are expected to die from breast cancer in 2020. It’s important to stay abreast of the latest guidelines for detection options. As researchers learn more about the disease, best practices change to incorporate new information. For example, you may have heard that women must start having annual mammograms when they turn 40. Is that true? The answer is: Maybe. The best advice depends on an individual woman’s risk level for breast cancer, and current guidelines break down risk further than just by age. For screening purposes, a woman is considered to be at average risk for breast cancer if she has not had breast cancer in the past, does not have a genetic mutation in a BRCA gene, has not had chest radiation therapy, and does not have a strong family history of breast cancer (which means a first-degree relative like a mother, sister, or daughter with breast cancer or a family member who had breast cancer before age 50). A woman is considered at high risk for breast cancer if gene testing confirms a BRCA1 or BRCA2 gene mutation; if she has a first-

degree relative with the gene mutation but has not undergone gene testing herself; if she underwent chest radiation therapy between the ages of 10 and 30 years; if she has Li-Fraumeni syndrome, Cowden syndrome, or Bannayan-Riley-Ruvalcaba syndrome; or if she has a first-degree relative with one of those syndromes. Women who are at high risk for breast cancer should talk to their doctor about doing a breast MRI and a mammogram every year, starting at age 30. Women at average risk have the option to start annual mammogram screenings between age 40 and 44. Women of average risk between 45 and 54 years of age should undergo annual mammograms. You may be surprised to learn that women 55 and older in the average risk category can opt to have a mammogram only every other year (or they can continue with annual mammograms). Women diagnosed with breast cancer today have several

treatment options, usually chosen based on the type of cancer and how advanced it is. Potential treatments include surgery (when the cancer is physically removed), chemotherapy (when medication shrinks or kills cancer cells), radiation therapy (when high-energy rays kill cancer cells), or hormone therapy (which blocks cancer cells from getting to the hormones they need to grow the cancer). Newer treatments include biological therapy or immunotherapy (when the patient’s immune system helps fight cancer cells). Unfortunately, the side effects of current treatments can make the cancer journey very unpleasant. Common side effects include nausea, vomiting, diarrhea, constipation, pain, arm swelling, shortness of breath, and skin irritation. In some cases, immunotherapy can be used to control the side effects of other treatments. Work on a breast cancer vaccine is promising. The Mayo Clinic is actively developing a vaccine and reports that could be available within eight years. The vaccine would work by stimulating the body’s immune system to attach to and kill cancer cells. If successful, this vaccine would not only stop the recurrence of breast cancer but might prevent cancer from developing in the first place. As anyone who has undergone surgical treatment for breast cancer can tell you, the removal of the tumor is just the beginning of the healing process. Many women go on to have reconstructive surgery. For earlystage breast cancers, breast-conserving surgery like lumpectomy or partial mastectomy may suffice, though they often result in breasts of different size or shape. To address this issue, many doctors use a technique called oncoplastic surgery. This procedure combines the cancer removal surgery with the reconstructive surgery to restore the breast or breasts. Options for reconstruction include implants or flap reconstruction, in which tissue from another part of the body is used to reconstruct the breast. While most women choose to have some type of reconstruction, some do not. After surgery, physical therapy can improve range of motion and strength, help with the healing process after lymphedema, and reduce pain. Although a diagnosis of breast cancer is always challenging, and a lot goes into recovery, ongoing innovations in early diagnosis and treatment are helping more and more women survive the disease! Medicinal compounding is not new. Mass manufacturing of drugs does provide the convenience of having a large supply of widely prescribed medications in the most common doses available at all times. There are many reasons that compounded drugs are essential for doctors or veterinarians to have as an intervention option for their patients. For example, Jennifer points out, “Sometimes, the best medication to treat a health issue in a child is only mass-manufactured in adult doses.” In that instance, a compounding pharmacist would be called upon to custom-create the desired medication in a pediatric dose. The experts at Pharmacy Solutions routinely generate medications to address issues such as hormonal imbalances or dermatological problems. Some patients have allergies or sensitivities to the fillers or binding agents that are often found in commercial drugs. In these cases, a custom medication that does not contain the offending ingredients can be compounded. Pharmacy Solutions takes the same customized approach with each feline or canine client in need of pharmaceutical intervention. Jennifer grew up on a farm, so caring for animals was something she learned early on. Before joining the team at Pharmacy Solutions, she worked at a pharmacy in Omaha where the staff included a veterinarian. It was there that she learned about compounding specifically for animals. “Sometimes, pets (cats in particular) have trouble taking oral medications or tablets,” says Jennifer. “We are often able to create an ear paste that allows the medication to be absorbed transdermally (that is, through the skin).” The flavor or texture of a medication can also be problematic when it comes to giving pills to pets. Pharmacy Solutions can address this by adding flavor to medication to make pills and tablets more appealing to

Did you realize that protecting your clothes can be as simple as going to your dry cleaner? In fact, experts say that dry cleaning can make a big difference in keeping your clothes looking newer longer. That’s because dry cleaning technology removes stains and ground-in dirt that can cause fiber abrasion, yellowing, and other problems. Today’s professional dry cleaners can be considered your own personal clothing care specialists. They’re highly trained to keep your cashmeres and silks looking beautiful. They’re also skilled at cleaning your suede and leather garments. You can have your shirts laundered and pressed too. Plus, many dry cleaners offer minor repairs and alterations. They can even take care of your household items like draperies and feather pillows. In order to protect your wardrobe investment, it pays to know that dry cleaning helps dissolve grease and oil in a way that water cannot. Natural fibers such as wools and silks dry clean beautifully, but if you wash them in water, they might distort. shrink, or lose color. Experts agree that newer technology has made dry cleaning more effective and affordable than in the past. One organization that’s dedicated to industry advances is the Drycleaning & Laundry Institute (DLI), an international trade association for garment care professionals. It has been around since 1883, and represents more than 10,000 retail dry cleaning facilities in the United States alone. According to the DLI, the practice of dry cleaning dates back to ancient times. During the evolution of early laundering, lye, and ammonia were used, along with a type of clay, to absorb soils and grease from clothing. Credit for the first dry cleaning business goes to a Paris store in the 1840s.

Since fluids are used in dry cleaning, the process isn’t actually “dry” at all. All dry cleaning machines function basically the same and consist of the holding tank, pump, filter, and cylinder or wheel. To begin, the garment is placed in the cylinder or wheel. The solvent, which is stored in the holding tank, is then pumped through the machine. Solid impurities are trapped by the filter. When the cycle is complete, warm air is circulated through the cylinder to vaporize any solvent that’s left on the clothes. Many of us opt to dry clean our professional or specialoccasion types of clothes. Those items are typically made of finer fabrics and finishes than perhaps our weekend at-home clothes. Although you may be tempted to wash your “dry clean only” items at home, please resist. If you ignore the fabric care tag, you run the risk of permanently ruining your clothing and shrinking or snagging your beautiful fabric. Once fabrics have been damaged by incorrect cleaning, there’s usually very little your professional dry cleaner can do to fix the problem. Since stains can be divided into two major categories, solventsoluble stains and water-soluble stains. it can be very risky to experiment at home with an “all-purpose” stain removal product. In addition to cleaning our skirts, shirts, pants, and dresses, dry cleaners stay busy with commercial services as well. Businesses often need dry cleaners for things like upholstery and draperies, linens (such as tablecloths and napkins). rugs, and more. Some cleaners offer on-site visits to companies so they can better clean items like chair pads and pillows. Many cleaners also specialize in the pres ervation of wedding gowns, christening gowns, and other family heirlooms. Opening the door for others is a good place to start. When I first met my husband, Manny, he had to “train me” on waiting for him to open doors for me—store doors, car doors, my home door. I literally have seen him race ahead of someone so that he can be the gentleman that he is and be there to open the door. Waking up in the morning and setting the intent to be a kind person really isn’t that difficult. A great starting place is to walk out the door with a big smile. This sets the tone to project a feeling of kindness and caring that cannot happen if we’re scowling. Yes, we have lots on our minds, lots to do. It is still easier on the body, mind, and spirit to smile instead of scowl. “A warm smile is the universal language of kindness.” —William Arthur Ward A challenge for a lot of us is that, for the time being, we are all wearing masks when we are out in public. What’s a smiling person to do? Stand in front of the mirror and practice smiling with your eyes. My mother told me years ago that the eyes are the windows to the soul. Yes, we have to dig down a little deeper than usual to project a smile with our eyes, but it’s worth it. The benefit for us in all of this is that we will all feel better. Communication, Respect, Trust The Building Blocks of Relationships We’re all in relationships. Whenever we deal with other people—with spouses, family, kids, friends, coworkers, neighbors, even shopkeepers—it’s a relationship. What can we do to build and maintain positive relationships? To answer that question, I gathered a few friends for a discussion. We agreed that the most enduring relationships are based on a combination of communication, respect, and trust. It’s important to remember that good communication means different things to different people. Some people prefer to talk in person; others prefer something less direct like written notes, phone calls, or texts. The group agreed that communication is letting others know what you want or how you feel…as plainly as possible…by whichever method you are most comfortable. If we expect others (our kids included) to do something, we should tell them. If you’re unhappy about someone’s behavior, talk to that person about it. People aren’t mind readers! We can’t fix what we don’t realize is broken. Remember, communication goes both ways. We also agreed that respect given merits respect in return. One of the teachers I subbed for left a note saying she thought all the “trouble-makers” were in her class. As soon as our day started, the students raised objections: Mrs. Smith does this first; we don’t do it that way; and so on and so forth. I stepped to the front of the desk. “Because I’m not Mrs. Smith,” I started, “I will probably do a lot of things differently. I expect you to be

cooperative. What do you expect of me?” Within five minutes of letting them “speak their piece,” we were all working together. Once I listened to what they expected and they knew what I expected, we could respect each other. It’s easier to meet expectations when we know what they are. Open communication and respect work together to build trust. Our friendly group defined trust as not only keeping your word but also expecting the other person to do the same. When we bring our kids up to behave in a certain way, we’ve communicated our expectations. We trust them to act in this manner even when we aren’t around. One of the things I’ve always advocated is teaching our kids to accept responsibility for their choices. When they make mistakes—and they will; that’s how people learn—they will trust that their family is still behind them. In this group of friends, which includes singles, parents, grandparents, and retirees, we share a mutual respect for different ways of thinking. We have different backgrounds and experiences and are at different places in our lives. We listen. We agree to disagree—it keeps things interesting. With open communication, mutual respect, and shared trust, we have strengthened our relationship. We’re good friends! How are your relationships? Safety Takes the Top Prize Choosing Your 2021 Vehicle

Safety has always been a high priority for drivers and vehicle manufacturers. In fact, many vehicles are designed with safety as the highest priority. This may sound obvious, but it can be more challenging than you think. For one thing, safety regulations change almost every year, and that’s just in the United States! Manufacturers sell their vehicles in numerous countries, and these governments each have their own safety regulations. In order to maintain a customer base in each country, manufacturers have to keep up with continually tightening restrictions to make sure their vehicles are highly recommended for keeping drivers and passengers safe. It’s not like there’s no reason to enforce safety regulations in the auto industry—we all want to be able to commute to work or go on a road trip without spending undue time worrying about injury. We want to know that our vehicle will prevent driver error from causing an accident in as many scenarios as possible. Even more importantly, we want to feel confident that if we do end up in an accident, the vehicle will do everything it can to make sure we are able to walk away. This is why we should take into consideration several specific safety features when purchasing a new vehicle. Some of the top safety features that you should look for in a new vehicle include adaptive cruise control, which locks on to any vehicles ahead and keeps a safe distance between the two cars; automatic emergency braking, which brakes in the case of a potential collision; lane departure warning/ lane keep, which warns you if the car wanders outside the designated lane; blind spot detection, which warns you if something enters your blind spot; and rear-view camera, which shows a 180-degree backward view for when you are backing up. Some relatively new safety features are already starting to become standard, including safety exit assist, which prevents doors from being opened while the car is moving, and facial recognition software, which monitors the driver and sends an alert when the driver doesn’t appear to be paying attention to the road. There are a number of ways to determine whether your vehicle rates high in the safety department. These include steering responsive headlights, rear seat belt reminders, traction control system, ABS, blind spot detection with lane change assist, rear cross-traffic alert, and reverse automatic braking. Honda brings us the Sensing safety features, which are now standard on the Honda Odyssey ($33,000) and across the lineup. Features include automatic emergency braking, pedestrian detection, traffic sign recognition, lane departure warning, lanekeep assist, and adaptive cruise control. Toyota has updated their safety features for the upcoming Camry model ($25,000), and some of these features are impressive. They help the vehicle detect a bicyclist in daytime and a pedestrian even in lowlight conditions, help detect cars and pedestrians at intersections when the driver is making a left turn, include automated emergency braking in certain conditions, and include adaptive cruise control. Safety features that were once restricted to high-end Chevrolet models are now being added as standard to all models. Such features include automatic emergency braking, front pedestrian braking, lane-keep assist, forward collision alert, auto high beams, and following distance indicator. You’ll find these features on the 2021 Chevy Traverse ($30,000). The Kia Telluride ($32,000), which is the top-rated 2021 vehicle overall by several auto websites, is not lacking on safety features. These include automatic emergency braking and warning, blind spot detection, rear cross-traffic alert and avoidance, lane keeping and lane following, safe exit assist, and rear occupant alert. As you can see, vehicle manufacturers know we have auto safety in mind when we consider our new purchases. Their 2021 lineups demonstrate that they’re stepping up to meet the safety challenges of today’s drivers.
