2014 is about to begin: it is the right time to take stock of the past year and to start making plans for the new one. 2013 has undoubtedly and generally been a very difficult year for our economy. The crisis has obviously involved the publishing world, too, and this has led magazines to diminish the number of their pages and/or to reduce their rhythm of publication. This is in part also due to the short-sighted policy of many companies opting for the internet, which certainly charges them lower costs but also makes them lose sight of the quality of the contacts which I believe is fundamental for the companies offering quality products. Besides, we must not forget that real culture and knowledge are fundamental for any form of real communication aiming at being deep and long lasting. What we buy prompted by our emotions and curiosity does not have a long life. Fortunately our magazine, which stands by its principles, is still able to resist and its circulation has even increased thanks to its coherence and to the action of spreading by AFFWA (Accademia Funzionale del Fitness, Wellness e Antiaging). In 2013 AFFWA gave up a project in order to support an initiative of one of our collaborators, Doctor Alessandro Gelli, who had managed to become coordinator of the Education and Higher Education Course “Antiaging and Antistress Methologies” at the Sapienza University in Rome. AFFWA readily supported this course through all its media and even “sponsoring” the enrolment of some of its members. In fact, a whole advertising page was dedicated to the course at the Sapienza, associating it to the logo of AFFWA and EURODREAM (2013 sponsor of AFFWA) believing, in good faith, not to be acting improperly, but perhaps making a
mistake. Instead, this decision has caused many problems to the course managers, who invited me to a disclaimer. Therefore, I hereby present my disclaimer by specifying that AFFWA and EURODREAM have never been official sponsors of the course since they have not paid any economic contribution as direct sponsors of the course at Sapienza University, nor to any of its organisers and management staff. They simply paid for the registration of some AFFWA members. Having duly cleared this, as requested, I also sincerely express my bitterness for this situation, since what I did was simply take the trouble to register to the course over 15 people, many of whom either belonged to the team of teachers of the Accademia or are my close collaborators. This, together with some different opinions concerning the didactic approach to the course in “Antiaging and Antistress Methodologies”, has driven me to withdraw my candidature as a teacher for 2014. As a consequence, our energies will be devoted to promote the activities of AFFWA in the coming year. I can already anticipate the following information: on 7th and 8th of May, on the occasion of Pianeta Nutrizione we will present the First International Convention of Antiaging Functional Medicine of AFFWA “Nutrition and Exercise” ; besides, we have resumed that old project we gave up, thus permitting the course “Antiaging Advisor Certification” to begin. Do not miss these very important appointments : “we preach what we practice and viceversa”. Massimo Spattini Accademia del Fitness President

HEALTHY FOOD FOR HEALTHY SKIN The skin is not just a mere covering: it is a real organ whose size and surface are second only to the intestine. Like the intestine, the skin acts as a selective barrier between the internal and external part of the body. In fact, through the digestive tube (stomach - oesophagus- mouth) it communicates with the outside. It acts like a filter, allowing the substances we need to pass and stopping the harmful ones, favouring their elimination. Obviously, due to anatomical reasons, the skin is particularly exposed to mechanical attacks coming from the external environment. Yet, its health can be damaged by the metabolic alterations of the internal environment as well. The health of our skin depends on proper lifestyles which include moderate sun exposure, environmental pollution, electromagnetic pollution, microwave radiations (see mobile phones), the use of particularly aggressive soaps and detergents. Eating healthy food is fundamental: it is important to limit the intake of simple sugars and saturated fats, as well as hydrogenated fats which are above all found in sweets, soft drinks and snacks. Moreover, it is important not to smoke, limit the consumption of alcoholic drinks and do one’s best to manage stress. Going to bed early and sleeping at least eight hours a night is essential, too. Sleeping well in the first hours of the night favours the natural secretion of the GH hormone, which is known as the antiaging hormone used by Hollywood stars in their antiaging therapies.
6
This hormone gives back the skin tissues tone and elasticity, causing a sort of lift effect, while the lack of sleep causes a higher production of cortisol. Cortisol, also defined as stress hormone, has a negative effect on skin proteins such as collagen and elastin, causing the process of skin relaxation. Excessive stress and wrong lifestyles increase oxidative stress, consisting in a raise in the production of free radicals which, if in excess, damage the cells and tissues. The formation of free radicals also increases when the “cleaner” system, naturally present in our body to keep free radicals under control, experiences a crisis. This is often due to an excess in “toxins” which our body cannot eliminate. This is why the intake of antioxidants – substances which are able to neutralise toxic free radicals – is fundamental for the health of our skin. The most famous and most powerful antioxidant substances are vitamins A, C, E, betacarotene (provitamin A), lipoic acid, acetylcysteine, Q10 coenzyme. Vitamin C and Q10 are fundamental for collagen production. Lipoic acid stimulates the activity of an enzyme which digests damaged collagen and favours its regeneration. In particular lipoic acid and acetylcysteine, besides their direct antioxidant effect, also help the detoxifying function of the liver, since they are the precursors of glutathione, the most important detoxifying molecule in our body which cannot be taken orally because it is dismantled by digestive enzymes.

L’Accademia del Fitness Other less famous yet very interesting substances with high antioxidant and regenerative power are deutrosulfazyme and silicon dioxide. Deutrosulfazyme is a saline form of deuterium (composed of oxygen and hydrogen), more specifically by deuterium sulphate enriched by amino acid minerals and enzymes. This formulation favours the fission of water into oxygen and hydrogen, allowing higher tissue oxygenation since, as doctor Artur Guyton said, “any pain, suffering or chronic disease is caused by insufficient cell oxygenation”. Silicon dioxide is very important for the connective tissue. As the Nobel prize Metschnikov (1845-1916) said, “man is as old as his connective tissue”. The age of the connective tissue is determined by its content of silicon dioxide, which decreases with age. The effects of such decrease are forehead wrinkles, reduced skin elasticity, opacity, hair loss and nail weakness. The lack of silicon dioxide causes the loss of the capacity to link water to the connective tissue and this has visible consequences on the face, which loses compactness. This is why it is possible to use subcutaneous injections of hyaluronic acid, a mucopolysaccharide rich in collagen which retains water, giving the tissues more turgidity. If the use of antioxidant vitamins and integrators is certainly a good way to prevent skin aging, it is also important to consider one’s diet. After listing the food to avoid - that is, sweets, soft drinks, snacks, alcohol, excessively elaborated food rich in saturated fatty acids, excessive consumption of bread and pasta, cold pork meat, preserved and refined food – it is now time to consider the food which is particularly effective in protecting our skin. Among this kind of food are various types of berries which are particularly rich in antioxidants. For example, Goji berries - which have been used in Tibetan medicine for thousands of years – are very rich in betacarotene and vitamin C. Acaj berries, which grow in the Amazon forest, are very rich in polyphenols (very powerful antioxidants). Elderberry is rich in mineral salts and vitamin A. Also very common berries such as blueberries, raspberries and blackberries are rich in antioxidants. Actually, it is reductive to mention as beneficial effects only the most powerful and famous active principles of this food, since the great effectiveness of natural food is characterised by a perfect mix which only nature can generate: minerals, vitamins, phytonutrients which act synergically and in a unique way with the active principle itself.
Another exceptional food which helps avoiding skin aging is salmon, which is particularly rich in Omega 3 essential fatty acids, extremely useful to fight inflammation – one of the major causes that leads to the production of free radicals – and contains Astaxanthin. This carotenoid has high antioxidant, antiinflammatory and antiestrogenic power. It is contained in micro seaweed eaten by salmon, hence its typical colour (we are talking about wild salmon, since farm-raised salmon is not fed on seaweed but on specific colouring agents in order to obtain that typical colour). Another substance which is richly found in salmon is DMAE (dimethylaminoethanol), a powerful compound which is the precursor of the neurotransmitter acetylcholine, also responsible for skin tone. Try and eat salmon and vegetables as your main meals for three days, together with berries and dried fruit as a snack... your face will become at least 10 years younger... first try and then trust!

THE TREATMENT OF
HYPERTENSION
IN ANTIAGING MEDICINE (PART ONE) Introduction According to the latest figures in Italy 21% of men and 24% of women suffer from hypertension. This means that over 10 million people suffer from this pathology. Pharmacological treatment of artery pressure is less effective than we think and a great number of studies have shown that a large number of treated subjects are nonetheless exposed to high pressure levels as well as to the side effects of the drugs they take. These figures take 140/90 mmHg as target values, although at present it is well known that optimum pressure levels, for which a person is less likely to develop cardiovascular damage, are around 120/80mmHg. If we should consider people with higher values than these as hypertensive, most of the adult population would have to be considered hypertensive. What is hypertension? By blood pressure we mean the measurement of the blood’s pumping power through the arteries. High pressure is the consequence of the excessive power along artery walls which, with
10
the passing of time, can cause damage to the endothelium, the internal vessel layer. Blood power is measured in two phases: when the heart contracts (systolic or maximum pressure) and when the heart releases (diastolic or minimum pressure). Pressure increase can be due to some underlying pathologies among which, for instance, hyperaldosteronism, Cushing’s syndrome or pheochromocytoma. However, over 90% of hypertension cases are considered essential, namely not based on any organic pathology. The main problem of hypertension is linked to damage to the endothelium. Damage to the internal vessel layer causes thickening of the artery which gradually forms an artheriosclerotic plaque. Hypertension and endothelial dysfunction are therefore intimately connected and lead to stiffening of the arteries which are no longer able to contract and release properly. This rigidity of the system leads to further heart stress with possible left ventricular hypertrophy and damage to various organs. The limits of traditional hypertension treatment A doctor nowadays can choose among a long list of drugs to re-
L’Accademia del Fitness duce artery pressure. These drugs in the best cases can reduce pressure, but do not restore the proper regulation. In fact, there is a pressure decrease due to drugs, but the pressure regulation system is as a matter of fact turned off. As we can see in the above table, none of the existing drugs intervenes improving pressure regulation but, on the contrary, they suppress it. They can be necessary and indispensable, but the fact that a doctor chooses a diuretic, an agent for the reduction of heart power or for vessel dilation does not make much difference in physio-pathological terms in that each of these drugs triggers multiple responses which can have many effects besides the desired one. Some recent studies have underlined that the main mistake of an exclusively pharmacologic approach to hypertension is that pressure variations are non simple alterations from the norm, but parameters which increase and decrease owing to a complex web of signals regulated by the
central nervous system according to the needs it foresees the body will have. Therefore, if a signal is simply suppressed by the drug, the brain will compensate by activating others. For instance, when hypertension is treated with a diuretic, the response compensation is heart rate increase and vascular constriction. These compensation effects lead the doctor to begin multiple treatments which unfortunately complicate the situation even more. Blocking a parameter and its regulating agents by means of a drug has its cost, since it makes that originally flexible variable rigid and completely insensible to the needs generated by the external environment and the body itself. In short, it deteriorates the body’s entire regulation capacity. For instance, beta blockers cause exercise intolerance due to the body’s inability to increase heart rate when necessary. For these and many other reasons less than 25% of hypertension cases are actually kept under control by prescribed drugs. Some researches carried out in Scandinavia some years ago un-
High pressure is the consequence of the excessive power along artery walls which, with the passing of time, can cause damage to the endothelium, the internal vessel layer.
TABLE 1: Drugs used in the treatment of hypertension DRUG CLASS
ACTION MECHANISMS AND SIDE EFFECTS
Thiazide diuretics
They act on the kidney increasing the elimination of sodium and water through urine. With less fluids in the body, the blood volume decreases and the blood pressure lowers. The side effects are impotence, hyperinsulinemia and glucose intolerance, gout, hypopotassemia, hyponatriemia.
Beta blockers
Beta blockers act on beta adrenergic receptors reducing the action of adrenalin and noradrenaline, they relax vascular muscles, reducing heart rate and oxygen need. The side effects are hyperinsulinemia, glucose intolerance, triglyceride increase, HDL reduction, intolerance to exercise, impotence.
ACE inhibitors
Angiotensin is formed when the kidney receives the signal to raise pressure. ACE inhibitors prevent or reduce the production of angiotensin and stop its vasoconstriction activity. Side effects are hyperkalemia, coughing and angioedema.
Angiotensin II receptor blockers (ARBs)
They are similar to ACE inhibitors but instead of blocking the enzyme which forms angiotensin, they block the effect of angiotensin itself tying themselves to specific receptors (AT1). Side effects include headache, dizziness, diarrhea, metallic taste.
Calcium antagonists
They act on the transport of calcium to heart and vessels causing vascular relaxation. They must not be used in patients who have suffered from infarction or cardiac decompensation. Side effects include constipation, swollen legs, flushing, headache.
Alpha blockers
They selectively block alpha receptors of vascular smooth muscles making them insensible to catecholamines. Their main risk is the loss of orthostatic pressure regulation (on standing up) and must be avoided in patients with risk of congestive cardiac insufficiency.

derlined that there might be an excess of insulin behind hypertension. Besides regulating glycaemia, insulin also represents an important vascular growth factor and enhances the reabsorption of sodium at renal level. Therefore, it has a potential hypertensive effect. Our modern diet, rich in sugars and refined carbohydrates (bread, pasta, white rice, sweets, etc), has the consequence of causing higher insulin levels in an increasingly greater number of people, due to a so-called phenomenon of insulin resistance. In short, a greater amount of insulin is necessary since cells become more and more insensible to its action and, with time, this leads to the metabolic syndrome and to diabetes. Hyperinsulinemia is still greatly neglected in medicine and its central role in the development of hypertension is too often ignored in clinical practice. At present it is even well known that type II diabetes can be due to the anti-hypertensive therapy. In fact, the appearance of hypertension tends to occur before the manifestation of diabetes and, since the role of insulin in the appearance of hypertension is not recognized, the latter is treated for instance with thiazide diuretics and beta-blockers increase, respectively 4.6 and 6.1 times as much, the risk of developing diabetes. The subjects who take both drugs have 11.5 times a greater risk of developing diabetes. It is therefore evident that, in spite of the researches which are published, there still remain fundamental errors in the treatment of special pathologies which have hyperinsulinemia as a common base. The treatment of hypertension in antiaging medicine Complex pathologies due to various factors cannot be properly

treated in elementary approaches such as for instance giving the patient a single drug or a combination of drugs. In most chronicdegenerative diseases more significant and lasting results are obtained by means of a so-called multi-modal approach, that is, a combination of integrated interventions which do not necessarily exclude drugs, but which, as well as drugs, include physical exercise, stress reduction, a healthy diet and the use of natural and phytotherapeutic integrators of proven value. A multimodal approach allows the doctor to keep in due consideration the incidents of body adaptation and to adopt a philosophy of intervention which, rather than attempt to block blood pressure regulation by means of a drug which would make it rigid and static, is able to stimulate its use and flexibility. Arterial pressure is controlled by a complex interaction of factors among which one’s diet, genetic predispositions, response to stress and so on. Consequently, regimens including physical exercise, body weight loss, a low glycaemic index diet with less sodium and more calcium, potassium and fibres and no smoking have proved to be more effective than drugs to keep artery pressure within adequate levels. CELLFOOD: OXYGENBASED SUPPLEMENT
AN INTRODUCTION TO OXYGEN-BASED INTEGRATION AS FOOD SUPPLEMENT IN ONE’S DIET (PART TWO) Where do cells obtain their energy from? Doctor Otto Warburg carried out many studies on the reason why cells need oxygen to create the ‘energy’ necessary to life. Thanks to his discoveries on the importance of oxygen for cellular life, he was awarded the Nobel Prize for Medicine. The healthy cells of the body decompose all the carbohydrates consumed in one’s diet, even when they are complex, into simple sugars and finally into molecules of glucose, sugar or carbohydrate which are then accumulated in the cells for energy purposes. When the cells need energy for their functions (reproduction, heat, etc.) they take the accumulated glucose and, thanks to a series of chemical reactions (glycolysis) with oxygen, they form ATP (Adenosine triphosphate) which becomes, according to doctor Warburg’s description, the “pure energy of the cell”, that is, the molecule which accumulates cellular metabolic energy. If there is lack of oxygen at cellular level, the vital processes cannot take place, therefore the cells dramatically decrease their activity up to their death.
16
Yet, doctor Warburg also discovered that scarce oxygen intake also damages cells. When cells lack the right amount of oxygen, glucose starts fermenting in a different way (state of anaerobic glycolysis). This causes a series of undesired and harmful effects. The collapse of the immune system Doctor Stephen Levine (a praised molecular biologist and an expert in genetics) and doctor M. Kidd have carried out a study which confirms that “...oxygen is the source of life for all the cells”. The constant abuse on our body caused by what we eat and drink, together with lack of physical exercise, deprives it of precious oxygen. This is made even worse by pollutant agents, toxic preservatives contained in water, food and in the air we breathe. As doctor Kidd has written: “Oxygen has a fundamental role in the functioning of the immune system...” above all as for its resistance against bacteria and viruses. Doctor Levine adds: “We can consider lack of oxygen as the first cause of all the diseases”. According to consistent research, the lack of oxygen in the blood may cause the collapse of the immune system.
number 12 / 2014 According to doctor Levine, a natural diet with the right amount of oxygen allows “proper oxygen intake”. In other words, an amount of oxygen proportional to the “density of the food consumed” is the key to good cellular metabolism. Doctor Levine has also demonstrated that complex carbohydrates are rich in oxygen. Complex carbohydrates include vegetables, wholemeal cereal and seeds (fruit contains too many sugars to be classified as complex carbohydrate). Since doctor Levine is a chemical researcher, he defines a complex carbohydrate as composed by 16 parts of oxygen and only 14 parts of carbon and hydrogen. He has declared that “over half of the weight of complex carbohydrates is given by oxygen, but the percentage of oxygen in fats is lower than 10% or it is 15% at its maximum, so fats have little oxygen. In fact, they steal oxygen. Proteins are 0 to 50% composed of oxygen, according to the specific feature of each amino acid. No nutrients – may they be proteins, fatty acids, vitamins or minerals – carry out their function in their original form. The nutrients which are present in our diet are only substances which are mechanically necessary to turn the potential energy of food into chemical energy which can be used for life. In order for this transformation to take place, oxygen must be present anyway. In his research doctor Warburg points out these discoveries: he said that if the oxygenation of tissues and cells is lower than what is necessary, as in cellular hypoxia, diseases such as cancer may develop from this condition, as well as a certain predisposition to degenerative diseases. The lack of oxygen is the main factor of immune-depressive diseases. Therefore, the three researchers agree in concluding that an increase in oxygenation of the blood flux and the cells is essential to favour and restore the general health of the body.
Oxygen is the cellular detoxicant necessary for the cells’ life. When the levels of oxygen in the body are insufficient, toxins increase and may end up devastating the organic functions and reducing the energy necessary for life. Without oxygen there cannot be nourishment. And without nourishment there are neither heat nor energy, so the body cannot purify itself. A body with excellent oxygen intake, where the development of the metabolic processes of oxidation and oxygenation have no obstacles, can be considered fit and strong, that is, healthy.
The constant abuse on
Free radicals, enemies or allies? In the last thirty years, over 6,000 scientific studies have recognised the activity of free radicals as one of the main causes of all the degenerative diseases. The same studies also indicate very clearly that there is an effective way to control and, in most of the cases, cancel the damage produced by free radicals. Unfortunately, the science of free radicals is totally unknown to the general public and this topic is often dealt with misinformation on part of the media and of the very manufacturers of supplements and nutrients which should solve the problem of free radicals. What is the truth? How are free radicals generated? Are all the free radicals harmful for the body? Is oxygen the real responsible element in the production of free radicals? The simplest definition of free radicals is the one made by doctor Kurt Donsbach in his book Oxygen-Oxygen-Oxygen: “It is an element, or a compound, with a free electron (not bound): this lack of balance makes the substance particularly prone to reactivity. However, it must be said that these free radicals have a very short life span, in general a tenthousandth of a second. Yet, within this short period of time they are able to cause damage if combined with other chemical substances which modify their features, sometimes even producing a chain reaction through which new free radicals are made”. In his work Free Radicals, Stress and Antioxidant Enzymes Doctor Peter Rothschild says that “Because of their hyper-reactive nature, free radicals can be extremely toxic and are the direct consequence of primary stress factors which affect the immune system and which menace our health. Anyway, this does not mean that free radicals are always harmful or dangerous. Small amounts of free radicals are necessary for many functions of the immune system and for other vital cellular activities. For example, the immune system creates free radicals to be used in the process of elimination of viruses or bacteria. Only in presence of high concentrations of free radicals, or when the levels of free radicals prevail over the ability of the body to eliminate them, our health is in danger. Keeping the
our body caused by what we eat and drink, toge-
ther with lack of physical exercise, deprives it of precious oxygen. This
is made even worse by pollutant agents, toxic
preservatives contained
in water, food and in the air we breathe.
The oxidation process The cells use oxygen in many processes where toxic substances in excess in our body are decomposed. The process by which a substance is combined with oxygen at cellular level is called ‘oxidation’. Doctor Levine describes oxidation in this way: “Oxygen represents the spark of life. The nutrients are its fuel. Good health needs fuel and oxygen in the right proportions”. The lack of a proper amount of oxygen makes it impossible to have oxidation and oxygenation, two processes which energize the cells for biological regeneration. These processes are at the basis of life and death. In a normal cellular environment, the cell does not lose its growth and reproduction potential. Insufficient oxygen intake causes scarce oxidation, which in turn causes an increase in cellular pollution.
17
L’Accademia del Fitness balance between the activity of free radicals and the intake of antioxidant enzymes is one of the main functions of the body”. Therefore, free radicals lack electrons, and in this atomic quantum state, they will do anything to steal an electron from another source. In very simple words we can say that there are electrons orbiting around every atom inside the so-called ‘shells’. These shells (or sub-orbitals) are placed at various distance from the centre of the atomic nucleus. Every sub-orbit can contain no more than two electrons, each of which ‘turns round’, that is, generates what quantum physicists call ‘wave forms’ or ‘wave packets’ in many directions and in its own. Thus, a magnetic field of parallel and anti-parallel spin is created. This means that it is opposite, since the electric charge (negative) is identical and in order to coexist in the same orbital space, or better sub-orbital space, it must at least have a different magnetic charge from the other electron. If one of these electrons is missing in the external orbit, it will look for the suitable electron to reach energy stabilization. This very reactive and unstable atom is called free radical. Free radicals can be of various size, from an only atom to more complex molecules. They can be beneficial or harmful, but above all they can be controlled by the molecules produced in the body,
18
such as the superoxide-dismutase (SOD) enzymes and glutathione peroxidase or other physiological molecules such as bilirubin and uric acid, or natural vitamins and nutrients that we can daily find in sources such as vitamin E, C, beta-carotene and bioflavonoids. In his book Freedom from Disease: How to Control Free Radicals, Maharishi Ayur-Ved writes: “They are a fundamental characteristic of all life based on oxygen. Free radicals, as part of the vital processes of every single cell, are created as toxic waste substances. When the immune system sends its special forces to fight infections, free radicals act as weapons. If the body takes in pesticides, industrial chemical substances, preserved food, cigarette smoke or alcohol, free radicals are their natural direct consequence. Also when the mind and the body are under stress free radicals are mass-produced”. PLANTS AND VEGETAL EXTRACTS WITH “ADAPTOGENIC” ACTION: HEALTHY ANTI-AGING AND ANTI-STRESS EFFECTS (THIRD AND FINAL PART) Suma or Pfaffia paniculata and its extracts: It is an Amazonian plant. It was widely used in the past by the natives as a tonic, energizer, rejuvenating and sexual tonic. Recent studies have attributed many adaptogenic properties to the root of Pfaffia paniculata, also called Suma or Brazilian Ginseng. It seems to have been widely used by Russian athletes in order to increase their muscular mass and resistance. Its effect is said to be important in complements which are useful for muscles, resistance and physical strain. Its properties are based on the presence of many molecules: not only Beta ecdysterone but also amino acids, electrolytes, minerals, iron, magnesium, zinc, vitamin A, B1, B2, E, K and pantothenic acid. Its root also contains pfaffic acid, saponins (over 11%), glycosides and nortriterpens and ecdysteroid glycosides. In phytotherapy plant saponins are known for their wide range of action, including the general decrease of cholesterol. Ecdysterone, the active molecule on metabolism, is also said to have a positive effect on

the building of lean mass, as a light “natural anabolic steroid” as well as on keeping high levels of sport performance. It is advised as sexual tonic, adaptogenic, energizer, cellular oxygenator, immunostimulant. It reinforces the muscular system and memory, too. Maca (Lepidium meyeni) and its extracts Also known as “Ginseng of the Andes”, this Tuber was cultivated by the Incan population for its energizer properties and its high nutritional property. The tuberous root from which the drug is taken is useful for its proteins (10-18%) with a certain biological value and for its contents of essential amino acids (leucine phenylalanine, lysine) which are not easily found in the vegetable kingdom and which are particularly valuable in the Andes, where there is lack of animal proteins. Branched amino acids (valine, isoleucine and leucine) are particularly requested by muscular masses in order to favour protein synthesis. In 100 g of dry product it is possible to find mineral salts such as Fe (16.6 mg), Mn, Cu (5.9 mg), Zn (3.8 mg), Na, K (2,050 mg), Ca

L’Accademia del Fitness (150 mg) as well as vitamins of the B complex. There is a balanced proportion between saturated fats (about 40%) and unsaturated fatty acids (about 54%) with linoleic essential fatty acid. There is also a sterolic component (in descending order it is formed by beta-sitosterol, delta- campesterol, ergosterol, brassicasterol, delta-ergostadienol), alkaloids, triterpenes, flavonoids, glycosides. There are some molecules such as macaridines (benzilate derivatives of 1,2-dihydroxypyridine) and macamides (benzilate alkamides). Macamides and macaenes are considered the marker components of the species. The essential oil taken from the aerial parts of the maca mainly contains phenyl acetonitrile (85.9%), benzaldehyde (3.1%),3-methoxyphenyl-acetonitrile (2.1%). Some extracts which can be made available by international suppliers of raw materials offer extracts of Maca titrated in Beta-ecdysterone, the same molecule which can be found in Suma, even if Maca contains a lower percentage. The indications are the same as in Pfaffia Paniculata.
ing factor (PAF). Its activity on venous, arterial and above all cerebral circulation has been confirmed. Clinical research has demonstrated the effectiveness of ginkgo leaves at all levels of blood circulation: - It improves circulation: by increasing the afflux of blood to the brain it improves mental acuity, concentration, short term memory and cognitive ability. Therefore, it is useful in case of memory loss due to old age, and in problems connected with short term memory. It also helps in case of tinkle (buzzing in one’s ears) and vertigo. Better peripheral circulation helps to reduce the feeling of cold limbs, intermittent pain in the legs and cramps. It is also useful in the treatment of diabetic neuropathies, macular degeneration and other circulatory problems. - Antioxidant: it is a scavenger of free radicals, it inhibits lipid peroxidation in the membranes, thus protecting the brain and the nervous system from the damage caused by free radicals. This may help to control the effects of aging. - It protects the blood-brain barrier (anatomic barriers and transport systems which control the types of substances which enter the extracellular space of the brain) - It reduces blood viscosity, inhibiting the platelet activating factor (PAF) - It reduces tissue damage during circulatory stress and it increases circulation in the limbs - It helps in the distribution of oxygen and glucose to the brain - It protects the arteries, veins and capillaries from damage, and helps regulating their tone and elasticity - It also seems useful in slowing down the progression of Alzheimer’s disease. Even if Ginkgo Biloba has been fairly analysed, many of its qualities are probably still unknown. They may help today’s man who, despite chronic stress, wants to live a longer and healthy life.
Also known as “Ginseng of the Andes”, this Tuber was cultivated by the Incan population for its energizer properties and its high nutritional property.
Ginkgo Biloba and its extracts Rightly called the “living fossil”, it is a legendary tree, over 250 million years old, which has survived despite the worst forms of pollution in the twentieth century, in particular radioactive waste due to the atomic bomb in Hiroshima. The Ginkgo Biloba is also said to have inhibition properties on 5-alpha-reductase. It contains many molecules, among which , B ginkgolide with inhibitory action of Paf-acether, intercellular phospholipid mediator implied in platelet aggregation, thrombus formation, atherogenesis and capillary hyperpermeability; , flavonoids (ginketol, isiginketol, bilabetol, ginkolide), with action at all levels of the circulatory system: vasodilator on the arteries, increase in venous tone, increased cerebral circulation. It has many active principles, called ginksenoids. Ginketol, isiginketol and bilabetol act on cellular membranes stabilizing them. Ginkolide stops lipid peroxidation and the formation of free radicals, it inhibits the platelet activat-

L’Accademia del Fitness to help the regularization of gastric ph (one of the main targets of stress), making it increase when it is low and vice versa. It stimulates the central nervous system, causing an improvement in reflexes, resistance and work ability in healthy people. The extract of schizandra allows lower glycogen degradation and higher lactate reconversion synthesis, which leads to better cardiovascular and respiratory resilience. Intense physical activity determines the hyperproduction of free radicals and an increase in circulatory hepatic transaminases. Thanks to its antioxidant and protective properties –it protects the hepatic cells from toxins -, schizandra may be useful to reduce the effects caused by intense physical activity. Like other adaptogenic plants, it contains lignans and its use in Chinese traditional medicine (CTM) is quite known. Fenugreek (trigonella foenum graecum): an adaptogenic plant with natural anticatabolic functions. The components of its phytocomplex can be useful in menopause, andropause and during recovery. This extract can also support hormonal synthesis in our body. Among its active components there are furostanolic saponins, which can also lower the plasmatic levels of cholesterol. The most important saponin isolated in fenugreek is diosgenin. Furostanolic saponins have immunostimulant ac-

tivities and they stimulate the LHRH hormone with consequent increase in the levels of testosterone and spermatogenesis. This causes a positive effect on the lean mass, in keeping high levels of performance and in the increase of libido. Note: “Adaptogenic” plants, compared to the typically stimulating ones (Guaranà, Muira Puama, Panax Ginseng etc.) may cause, in case of health-conscious intake, fewer undesired effects. However, it is always necessary to see a doctor who is an expert in the Pharmacognostic field, too, and who can take into consideration the pharmacologic interactions of the molecules contained in the plants taken by the subject. Moreover, since a combination of plants is normally taken, for example Rodiola Rosea and Pfaffia Paniculata, it is important to consider their global effect as well as the drugs and/or other natural substances taken by the subject. PSYCHO-BIO GOOD MANNERS: LEARNING BY DOING
(PART ONE) Imagine a terrace overlooking the sea, the full moon, a light breeze blowing and an elegantly-laid table: a perfect scene! Eating does not simply mean getting our food supply. It is above all an enjoyable experience which stimulates our senses. It is a joy-sharing feast, in which we can enjoy with our friends one of the greatest pleasures of life, a prelude to other pleasures… Then, suddenly the enchantment is over: what has destroyed it? The expected guests arrive: overlapping voices, hasty introductions, affected attitudes and conventional words. What a pity, with the appropriate tone of voice and good manners the enchantment would not have vanished. If this is not often the case, it means that the matter is more complex than it appears. My participation to conventions inherent to my profession as a psycho-therapist has always ended up being a good starting point for concrete observations concerning such complexity. Formal dinner parties, which turn out to be the typical representation of a paradox of style, usually take place at the end of these conventions. Those very people who only few moments before were standing on the speaker’s platform issuing their high-sounding speeches on behaviour deviations and thaumaturgic remedies reveal their inability to hold proper conversation at the dinner table. It has always been surprising for me to find how communication experts, image and marketing professionals, staff selectors and writers of genius, just like most other people, seem to forget their Good Manners at the dinner table; this also involves those very people who teach how to study a person by decoding their body language. At a closer look this lack of discipline at the table is unbelievably common throughout all social classes, all cultural, professional and generational levels. It is something that brings all kinds of people to a common level, which would be just unimaginable in any other context. It is a sort of livella, as Totò would say.

L’Accademia del Fitness In the light of these considerations, I have decided to organize a course on Psycho-bio good manners, considering it as an opportunity for people to reconsider and reflect on their behaviour. Being aware is the underlying leitmotiv of the course. Its aim is to make us aware that through our behaviour during meals - one of the most common activities in our daily life when we are almost as unconscious as when we simply look or walk around, engage in conversations and are in relation with others - we involuntarily reveal our real nature as perhaps we would rather avoid doing: so this might turn into an opportunity to get to know ourselves better and to regain control over ourselves. As a matter of fact, when we are having our meals more than in any other situation, we unconsciously lower our defence levels, we let ourselves go and reveal our most hidden nature and true soul to any careful and trained observer. During the day we tend to adopt a dignified attitude, we build up an image of ourselves, we interpret a role and somehow we manage to play our role until the end of the performance. But this is not true at the dinner table. The dinner table gives evidence of who we really are and unmasks us. We must keep in mind that, according to cognitive neuro-scientists, five percent of our daily behaviour is consciously controlled by our mind, whereas the remaining ninety-five percent is controlled by our subconscious. Therefore, our subconscious mind is the most powerful source in our daily life. Our subconscious is the recorder tape which controls us without our being aware of it. Being aware of this natural state of ours, we could consciously decide to place under control that undisciplined part of us.
28
Hence my invitation to sit around the table, to estrange yourselves from your role of table companions and place yourselves in the position of invisible observers even of yourselves. Even if we are very hungry, our robust appetite often turns out to be the trap which easily reveals to what extent we respect the rules of good manners (temperance and moderation are signs of respect for our table companions, for the food, for the love and the hard work of the person who has prepared and offered it). It is at this point that the curtain rises and we start that representation of ourselves: most of the times a tragicomic and embarrassing representation where we give full view of our faulty behaviour and reveal our cultural origins and our level of evolution. I have given myself the task of finding a modus operandi as a consequence of this general and demoralising picture so that we might become protagonists of the scene rather than simple puppets moved by our emotions‌ Even if we are at the table, let us not be commanded by our stomachs! Consciously choosing to concentrate on the way to behave while we are eating, deciding to remain concentrated on the actions we are performing, and remaining in a state of attention makes it possible for us to be in control and to begin a journey of self-knowledge. I have often been asked to select candidates for new jobs and after the interviews, the curricula, the Minesota and Luscher tests and whatever, if I have the opportunity, I invite the candidates to lunch and I engage in a conversation. I get to understand them and I draw much more information in that situation than through any other test because, as we know, there is a body language, and the body, unlike words, never lies.

L’Accademia del Fitness Believe me, that test provides unexpected and useful information. The dinner table provides a valid means to understand if there is any real truth in our description of ourselves. A meal is one of those many moments in which we inexorably disclose our instinctive selves. If we do not like what comes up to the surface and are ashamed of it, why don’t we come to terms with it and try to correct it? When we become conscious that our liberty and growth imply that we are completely responsible for what happens to us, then we can find the solution to our problems or to the ones we believe are our problems and at this point we have command over our life and stop expecting other people to free us from our problems. In most cases this act of self awareness does not obviously take place automatically, it occurs only in rare occasions and is not often our case. It usually occurs after wishing strongly to experience a deep change. Often we are our own worst enemy. It is therefore important to understand that any meal we have, even in a fast food restaurant, is a way to enhance this process of self awareness. We need to become careful observers of ourselves and ask ourselves what annoys us while doing so. Most probably we will realize that we will have to deal with that still not integrated part of us which does not make us feel comfortable. It is time to ask ourselves, in order to understand the origin of our sense of dissatisfaction, what kind of relationship we have, for instance, with the observance of the rules. Why does it annoy me if I am asked to follow the rules of good manners? Why should I be obliged to control myself even at the table? With regard to this, I am often told: let me live in peace at least there! … it is really disarming for everybody to come to terms with the irritation we feel when we are asked to underline how we do not cope. We feel we are perfect, so nobody can judge us. Not even ourselves? A repenting judge. Do you have Albert Camus’s formidable novel “The Fall” in mind? The main character and narrator of this novel, the lawyer JeanBaptiste Clamence, symbolizes a man who lives in an absurd condition, a philosophical state which Camus utilizes to analyze the human condition. Clamence is a man who is resigned to an absurd life, he does not fight for anything and multiplies the lack of sense of his life through the repetition of senseless actions which only increase his constant state of anxiety and sense of estrangement with his life.
Is this the view of life we want to share? I really hope not! Man is a sociable creature, he goes on living because he recognizes himself in a set of ethical and moral values, which, if well tended, makes him mature in a personal growth which connects him to the wise part present in us all. Mine is an invitation to work on oneself in order to learn to submit our instinctive side and this is an enormous task which takes a long time. Therefore, I have identified in the act of eating an effective psychophysical technique which can be applied constantly and frequently as eating is an indispensable daily act. This is a demanding task as we tend to be lazy and do not like to change. The fundamental rules of the table are not recognized and respected even by people who should “naturally” do so owing to their social role, exhibited culture and origins. The particularly pleasant moment of our meals, both alone or in company, offers the opportunity to turn customary and wrong habits into a constructive effort which can improve us, elevate us from the state of psycho-biological machine to our real self, our self awareness. It is the same difference between the simple act of breathing and the act of living.
At a closer look this lack of discipline at the table is unbelievably common throughout all social classes, all cultural, professional and generational levels. It is something that brings all kinds of people to a common level, which would be just unimaginable in any other context.

BOOK REVIEW "ANTIAGING E LO STILE DI VITA INTEGRATO" The following considerations are the outcome of my reading of the interesting book “ANTIAGING AND AN INTEGRATED LIFE STYLE” that Dr Spattini was so kind to give me. What does anti-aging mean? Does it perhaps mean that it is possible to stop aging? No. In its introduction Spattini makes it clear that this is not the meaning of the term. By anti-aging we intend a way which allows man’s nature to express itself and to develop at its best along the itinerary that Medical Science has been investigating for many years concerning the physiological duration of life and human health. In the last decades Medical Science has succeeded in making human life much longer (allowing us to reach the maximum level of survival possible according to man’s genetic code). It has not only succeeded in curing a great number of mortal diseases which lead to precocious death, but it has also managed to treat those diseases which are typical of old age, limiting their consequences. Of even greater importance we must consider the possibility to prevent a number of diseases with lethal or invalidating consequences. Obviously, the processes leading to aging have not been defeated: aging is in fact an inevitable genetic phenomenon involving the life of each living creature. However, there
32
is evidence - not only theories -that it is possible to influence, direct, delay it and to make it take place in the best of ways, above all reducing its negative consequences on the quality of life. It is by now evident that it is possible to mitigate, delay and give relief to many of the features of old age. Besides, it is also possible to control and sometimes eliminate or at least reduce the negative effects already caused by old age. In other words, we can prevent, contrast, treat and fight back the worst aspects of old age: the lack of physical and psychic independence. Two aspects are strictly connected: good mental efficiency which is definitely the most important one to maintain good quality of life - depends on good physical efficiency and vice versa. When a person loses mental efficiency, the ability to organize oneself and to be useful to oneself and to others is lost, too. When we lose physical efficiency, also our ability to live independently more or less rapidly declines, together with the ability to establish lively social relations and therefore the possibility to receive motivations which allow us to maintain an active and lively mental life. This is why independence, mental efficiency and fitness become inseparable. These concepts are well expressed above all in the articles

L’Accademia del Fitness written by Spattini, who is clearly not only the main author of the book, but also its real animator. Also the chapter written by Cau contains interesting ideas on the same issues. Both authors combine their personal positive experience with their passion for these themes and their intention to found them on precise scientific data. The book contains many quotations and a wide selection of literature supporting the above mentioned concepts. Literature which in these years has been expanding and becoming more convincing for the authors who, however, in some cases judge perhaps too benevolently the results of experiences carried out on a too restricted number or too selected subjects. Such experiences need to be verified on a much wider population. However, the authors’ attempt to corroborate their theses (also as a result of their wide experience) with data which are as scientifically valid as possible is praiseworthy. Spattini’s descriptions are particularly detailed and convincing, both when dealing with general and specific aspects such as, for instance, the problems concerning the hormonal system. I find Cau’s way of dealing with the complex problem of sexuality in old age very useful and encouraging. I think his insistence on confuting many prejudices and stereotypes is very constructive as well as his way of dealing with the real aspects of the problems concerning both sexes. In the same way, I think it is very useful to take into consideration the possibility of preventing (at least partially) the losses that are inevitable at this age, but not as tragic as we tend to believe nor as irreparable as depressed patients often believe them to be. The possibility to improve the situation with specific techniques for women and men, the possibility of recovery, the need for integrated action which is feasible by associating the psychological aspects with the physical activities of fitness is well explained. The contributions of the other authors are in the same line and constitute a sort of manual for consultation in which important information can be found. VALERIA GUERRA, for instance, rightly underlines the need to fight against the inclination to detach oneself from friends, closing oneself into a restricted world, overwhelmed by fear and distrust and the refusal of accepting oneself. It is important to insist on the need to rediscover and give value to one’s creativity. Also interesting is the thesis of SAPOROSO, who underlines the importance of smiling (and therefore also of oral hygiene) and of posture, as the necessary expression of the joy of being alive. This concept is underlined by SILVIA IORIO, too, in her chapter on “beauty and wellbeing”, in which the importance of looking after one’s aspect is given the right value. This can also be achieved through certain activities such as the ones explained by GELLI (stress, yoga, anti-stress methods) or by COZZINI (qigong methods).
34
The activities described in the latter chapters, as well as the ones described by MONTAGNA, dedicated to the suitable diet in a fitness programme, or by CLAUDIA BONINI on integrators (in which it is also possible to find useful advice on antioxidants and the need to avoid illusions) are of great interest and can be taken as guidelines. However, the chapters just mentioned, as well as the ones concerning metabolic fitness (ANTONIELLA), training (NOTARNICOLA), the evaluation of the situation (PEDRAZZINI) might result too technical, supplying advice which is in part too difficult to follow. We must keep in mind, under this respect, that this book is written by a group of valid professionals with technical skills who have actually put into practice their advice. Let us say that they have perhaps put it into practice in an intensive way and, I hope they will forgive me for saying so, in a fanatical way. They are enthusiasts indeed. Yet, it is not believable that anybody can reach their levels. However, anybody can find useful advice and information in the book. Guidelines and advice can be certainly found in the chapter by DI CRISTINO dedicated to posture: a problem which too often is not given the right attention and that instead is of fundamental importance for one’s balance, the maintenance of a good articular mobility, resistance etc. In short, as already underlined, I consider this book as a book for consultation, showing many useful ideas and methods against the drawbacks of aging, in order to prevent and treat them. Advice which is certainly useful to everybody: to the young for prevention; to the elderly for the maintenance and improvement of their condition. All presented in an psycho-physical integration of real fitness. The main concepts should be learnt and kept into consideration by medical students. They are concepts that draw us closer to mankind. This is not what happens at present in medical education, which is mainly concerned with technical aspects. Eventually, I wish to underline that this book also represents a contribution to hope: the hope to be able not only to live a longer, but a more pleasant and enjoyable life.

CELLULITE AND TRAINING
Cellullite, more properly called liposclerosis, is a problem not only for a great number of women (8-9 women out of 10), but also for men (up to 4 out of 10). Actually, it concerns over half the population. It is useful to make a distinction between Cellulite and aesthetic Cellulite. Cellulite, which in medical terms is more properly called edematofibrosclerotic panniculitis (EFP), refers to a localized alteration in the dermis or in the hypoderm involving: • The adipose panniculus defined panniculosis • Extravascular fluid accumulation (oedematous features) • Increase of reticular fibres (fibrosis) • Increase of elastic fibres and scar tissue (sclerosis) Basically, one of its consequences is progressive toxicosis of tissues due to the fact that the vessels are unable to carry oxygen to the tissues and therefore to free them from toxic substances. A therapy based on detoxifying diets and consisting of water supply and antioxidants is one of the first remedies against it. Other various causes are also to be considered, such as for instance high estrogen levels, contraceptive pills, smoking, preg-
nancy, genetic factors, excessive saturated fats in one’s diet and more. All these factors lead to the pathology. Aesthetic cellulite refers to an altered condition of the subcutaneous tissue, which is rich in adipose cells. It is situated under the skin and is characterised by the hypertrophy of the adipose cells, where liquids in excess accumulate in the intracellular spaces (residues of the body’s biochemical processes). The balance of the venous and lymphatic system (the lymph is a liquid which flows in the lymphatic vessels and collects the body’s waste materials) is modified, thus causing a slowdown in the blood flux and fluid retention on part of the tissues. Aesthetic cellulite can be divided into three growth stages: • Oedematous: it creates an oedema, that is, fluid accumulation and it is present in the adipose tissue, above all around the ankles, calves, thighs and arms. • Fibrous: it creates fibrosis, that is, an increase in connective tissue, which hardens the adipose tissue. It is characterised by little lumps and by orange peel skin. • Sclerotic: It creates a sclerosis, so that the tissue becomes hard and big lumps are generated. It is very painful. The factors which cause cellulite can be divided into: • Primary factors • Secondary factors • Behavioural factors In short, primary factors are all the congenital factors which make the subject prone to the formation of cellulite, especially women and above all Caucasian women. In women the action of estrogens is responsible for the “pear” body shape, or gynoid. In this case, fat is concentrated on the hips and in the lower part of the body, with consequent water retention and blood microcirculatory problems. Due to the hormonal upsetting typical of puberty, the problem of cellulite becomes evident. Another primary cause for the formation of cellulite is familiarity with this problem, that is, hereditariness. The secondary non congenital factors, in women, are the menstrual cycle and pregnancy. Actually, before the cycle the irregular aspect of the skin is accentuated due to an increase in fluid stagnation. Instead, in pregnancy the levels of estrogens increase, and this causes water retention, blood circulation difficulty and increase in hunger.

L’Accademia del Fitness Because of hormones, however, the problem of cellulite improves in menopause. Behavioural factors are all those factors that can worsen the pre-existing condition. Once we know them, it is better to avoid them, at least in order not to worsen the situation. Some examples: • Frequent consumption of fat, caloric and very salty food favours water retention, as well as localised adipose accumulation. • Overweight is a cause and an aggravating factor of the orange peel skin. • In case of too quick and unbalanced weight loss the muscular tissue tends to collapse due to ptosis, with consequent skin relaxation. • Excessively sedentary life. The lack of physical exercise favours fluid stagnation and water retention in the tissues, with consequent swollen aspect and orange peel skin. • Wrong posture, above all the habit of crossing one’s legs, favours venous and lymphatic stasis, because the blood has difficulty in flowing up due to vessel compression. • Excessively tight-fitting clothing compresses the vessels and makes it difficult for the blood to flow. • Likewise, tight shoes or excessively high heels, obstruct the mechanism of natural blood pumping from the heel upwards. • Circulatory stasis is also caused by standing up for long periods without moving. • Chronic constipation and bad intestinal motility cause pressure on the blood vessels and, as a consequence, are an aggravating factor of cellulite. • Smoke acts as a vasoconstrictor, so the microcirculation is compromised, causing fluid accumulation and contributing to the tissues’ early aging. • Stress modifies the hormonal level, favouring cellulite. In order to prevent and treat cellulite it is useful to follow a healthier lifestyle based on: regular physical exercise, healthy and proper diet and proper rest, trying to sleep at least 8 hours a day and lowering stress levels as much as possible. Both in case of Cellulite (pathology) or of mere localised adiposity in excess (aesthetic cellulite), the pieces of advice on physical activity are generally equivalent. The programme that follows is quite general, since it does not take into consideration personal and unique characteristics typical of a specific subject. Alternate a session of gym exercises, circuits and cyclic cardio sessions. Example of a session of gym exercises: • 10’/15’ BIKE WARM-UP OR UPHILL WALKING • SUMO SQUAT 3X12 • FRONT THRUSTS 3X12 • CRUNCH 5X20 10” REC. • FRONT LAT MACHINE 3X12 • TRICEPS PULL DOWN 3X12 • SIDE LIFTS 3X12

• The consumption of water is very important because it allows the optimal elimination of toxic and waste substances. Drink a lot and don’t forget to bring a bottle of water with you, especially when you go to the gym. It is advised to drink water low in sodium in order to favour diuresis. • The market also offers many creams which can help eliminate cellulite, even if only superficially. • As for aesthetic treatment, all the treatments based on lymph-draining, pressotherapy, hydromassage, circulatory massage, etc. are all useful.
Cellullite, more properly called liposclerosis, is a problem not only for a great number of women (8-9 women out of 10), but also for men (up to 4 out of 10).
USEFUL ADVICE: • In case of cellulite it is not advised to perform high-intensity physical activity, since it causes the formation of lactic acid. This metabolite is a cellulite ally, since the formation of muscular toxins has negative effects on circulation and on tissue oxygenation. • In order to fight cellulite it is fundamental to combine a proper diet to regular physical activity. • Therefore, it is important to follow a diet rich in vegetables and fibres, have at least 3 main meals a day, plus two little snacks during the morning and afternoon, drink at least a litre and a half/two litres of water a day, avoid food rich in fats, alcoholic drinks and excessive consumption of coffee (more than 2-3 cups a day). Moreover, it is important to try and avoid salt, since the salt contained in food is enough to cover the daily need.
In conclusion, if followed sporadically and not as a whole, these pieces of advice will not lead to great results. Instead, if followed as a whole and combined with a high dose of determination and perseverance, they will certainly lead to a clear improvement of the situation and perhaps they will even solve the problem! HYPERACTIVITY OF STRESS SYSTEMS AND IMMUNOENDOCRINE RESPONSE.
The role of physiological regulating medicine Pnei, as it is by now well-known, is the science which studies stress systems. When a word becomes ordinary, it risks losing its original meaning. This is what happened to stress! The progress made in research has opened many new perspectives, thus turning the theories on bad stress and the need to fight it into something obsolete. Among those who contributed to this change in perspectives, the American Bruce McEwen stands out. The general public knows him thanks to his popular essay The end of the stress as we know it (with Elisabeth Norton Lasley, National Academic Press 2002). McEwen has devoted his career to Rockefeller University in New York, where he is currently the head of the neuroendocrinology laboratory.

Rather than talking about stress, he says, we should talk about challenges. Our body is programmed in order for us to react to these challenges, to the things that happen to us and to environmental changes. The body goes back to its original balance through a process of “allostasis”, that is, stability achieved through change. The problem occurs when this physiological mechanism of response becomes overloaded due to excess of stress or for insufficient management of allostasis. In other words, due to the inability to reduce tension when necessary. This condition is called “allostatic overload”, which is a condition of constant frustration or anxiety and causes an increase in those chemical mediators which end up damaging the cardio-

L’Accademia del Fitness vascular, endocrine and immune system. The British epidemiologist Michael Marmot and his so-called Whitehall Studies on state employees has demonstrated that the people belonging to lower social classes are more vulnerable to diseases even if they take care of themselves. Many factors which are pivotal for the excess of stress are linked to personal interactions, to the little control on one’s life and occupation. The first studies measuring what the British scientists call “wear and tear” took into consideration the following parameters: • -Maximum systolic arterial pressure • -Minimum diastolic arterial pressure • -Haematic and urinary cortisol • -Urinary adrenaline and noradrenaline • -Dhea-s (dehydroepiandrosterone sulphate) in serum • -Glycosylated haemoglobin • -HDL cholesterol • -Ratio between total cholesterol and HDL cholesterol • -Waistline/hip measurement Then, a study published on Pnas (Gruenewald,T e al, Combinations of biomarkers predictive of later life mortality, Pnas 2006;103:14158-14163) also used inflammation markers: PCR, Interleukin-6, fibrinogen, albumin. The integrated assessment of the “wear and tear” degree of the body through endocrine, immune and metabolic markers is relatively easy to apply. Other levels of stress and inflammation markers indicate high cardiovascular risk and not only. This risk can be mitigated by other factors such as for example HDL. This perspective does not consider cardiovascular risk as something connected to single parameters such as cholesterolemia. According to some studies carried out by the national institute of Health, low cholesterolemia in old age may even be considered something bad! The degree of “wear and tear” is what determines the risk of diseases. So many of us sometimes are just unable to... take a break! The hyperactivity of stress systems characterises many pathologies representing the main target of a Pnei therapy.
On the contrary, the holistic paradigm recognises many possibilities of non-pharmacological therapeutic interactions with the Pnei network. Diet, physical activity, antistress techniques, meditation and respiration techniques, physical and music therapies are powerful modulators of the Pnei network! In the light of these reflections, the last few years of my life have been all devoted to the setting-up of a holistic and integrated therapeutic model based on four pivotal pillars: P psyche, N nutrition, E exercise, I therapeutic iter. The Pnei4U® method is taught in base and advanced courses. Its most recent development is the innovative Pneisystem software. The software is based on the Pnei4u method, which offers a therapy based on antistress techniques, nutrition, postural physical activity and integrated therapeutic iter. It is a handy and practical software which enhances the talent and personal qualities of the operator, contributing to the creation of a data base through which algorithms turn the therapeutic result and therapy progress into objective data. It is a big challenge for patients, who are motivated to improve their score (whichever therapy/needs they may require), while for us it represents a surplus value, which is to make a therapeutic result objective in an epistemological frame which is not only based on “linear” measurements but also on the measurement of “qualitative” parameters, too, such as energy, vitality, oxidative stress, Ph, saturimetry... In a 2004 review published in BrJSports Med 38: 536-541 entitled “Endocannabinoids and sport activity” Dietrich and Mc Daniel suggest that the feeling of wellness and the deep changes in mental state after practising sport – in particular analgesia, sedation, anxiolysis – could be associated to an increase in seric concentrations of Endocannabinoids, among which anandamide (ananda is the Sanskrit word for bliss). The scientific literature has widely proved that physical exercise causes a deep alteration on one’s consciousness: such change has been labelled as “runner’s high”, subjectively described as “pure happiness, activation, sense of unity, peace, interior harmony, uncontainable energy and reduction in the perception of pain”!
Pnei, as it is by now well-known, is the science which studies stress systems. When a word becomes ordinary, it risks losing its original meaning. This is what happened to stress!
Now I would like to make another remark! I have already had the occasion to reassess the concept of “placebo”. An analysis of literature data demonstrates that a placebo meant as lactose tablet - used for randomized double-blind controlled clinical trials - hardly ever has therapeutic effectiveness (apart from some modest results on pain). Actually, the reductionist model inaccurately defines “placebo” as any form of non-pharmacological therapeutic intervention according to its standard meaning.
44
What else do we need to give physical activity a key-role in the regulation of PNEI network? It is at this point that the integration between Pnei and Accademia del Fitness begins. Massimo Spattini is a world icon in the Pnei vision of sport and I am particularly honoured to write for this prestigious magazine. Physiological regulating medicine which makes use of decimal dilutions of cytokines and neurotransmitters uses products based on catalysts of the citric acid cycle, low-dose hormones, pools of essential amino acids which contribute to muscle

L’Accademia del Fitness anabolism, stimulate oxidative phosphorylation and reduce the synthesis of lactic acid, accelerating its disposal. The quality of sleep is another essential element to achieve optimal performance. Insomnia is one of the most common disorders treated by Pnei. There are two forms of insomnia: initial insomnia, characterised by trouble in falling asleep, and middle insomnia, characterised by trouble in maintaining sleep. The first one depends on the lack of balance of neuronal activity in the brainstem where the synchronizer neurons – or sleep neurons (serotonin, adrenaline) – and desynchronizer neurons – or wakefulness neurons (acetylcholine, GABA) – lose their coordination, interrupting the sleep starting line. In low-dose therapies we use products based on Eschscholtzia californica, Melilotus officinalis, Citrus aurantium, Gaba and Serotonin, which optimise the mechanisms of sleep induction. I also suggest three useful oligoelements: aluminium, magnesium and manganese. The most innovative therapeutic proposal involves pidolate minerals, the absorption of which is said to be optimal. In case of insomnia with early awakening, the prescription of Melatonin 4CH is advised (the night peak of melatonin synthesis on part of the epiphysis and bone marrow is at around three a.m.) Stress frequently leads to a decrease in sexual desire and sport performance. Among the possible causes, a frequent endocrine cause both in women and in men is hyperprolactinemia, also
borderline (values around 25-30ng/ml). The subsequent physiological and compensatory increase of the dopaminergic tone inhibits the gonadial activity through GNRH. Hyperprolactinemia may also indicate thyroid pathology, mainly on autoimmune basis. The prolactin dosage must be repeated at least twice at rest and in women it is advisable to measure it just after the menstrual cycle. The increase in prolactin is frequently associated to the hyperactivity of stress systems and to the decrease in sexual desire. The pharmacological response is also based on the intake of dose Serotonin, Prolactin and ACTH low dose. We have tried to explain some of the fundamental concepts of Pnei physiopathology and, above all, to present them under the scope of systemic therapy. L’Accademia del Fitness The aging process, together with wrong lifestyles and stress, cause an inevitable decline in the hormonal levels of men and women. This phenomenon is called endocrine aging and is one of the causes of aging. It is by now ascertained that keeping good hormonal levels gives vitality and energy. Besides, it improves one’s mood. The lack of steroidal hormones such as pregnenolone, estrogens, progesterone, dhea and testosterone causes chronic fatigue, mood shifts (the steroidal hormones tie themselves to the GABA receptor), decrease in libido and slowdown in metabolism. This is why, in order to face these problems, above all for menopausal women, in the last few years the hormonal substitutive treatment has been introduced. It consists in the integration of synthetic hormones to fight some typical disorders of climacteric. Yet, the patients quite often tend not to change their lifestyle at all. They approach the treatment with the typical pharmacological attitude: the medicine treats the symptom, with no need to change one’s diet and physical activity. Moreover, the hormonal substitutive treatment is only advised for a limited period, usually 5 years, otherwise its risks may exceed its beneficial effects. For this reason, considerate people have started look-
ANDROGENS ARE WOMEN’S FRIENDS: THEY IMPROVE MOOD, METABOLISM AND VITALITY! ing for more natural methods to fight female disorders, such as the use of products based on phytoestrogens, medicinal plants which mime the activity of estrogens in the body, for example soy and hop. This is certainly a step forward, yet it may become useless if women do not adopt wider antiaging strategies which include changes in one’s diet, in practising sport activities and in one’s thoughts – it is important to remember that the excess of stress causes an increase in cortisol and a decrease in the other steroidal hormones. When we talk of modern antiaging strategies for women, however, we cannot focus the problem on the mere lack of estrogens and/or progesterone, since physical efficiency is linked above all to another class of steroids which are regularly produced by women. That is, androgenic hormones such as testosterone and DHEA, which in blood reach higher levels than the very estrogens, the typical women’s hormones. The androgenic hormones have a fundamental role in women’s wellness, above all DHEA: it increases sex desire, improves strength and sports performance, it increases basal metabolism, favouring the reduction of adipose tissue, it reinforces

L’Accademia del Fitness the immune system, stimulates neurological functioning and improves one’s memory. It also prevents osteoporosis and cardiovascular pathologies. These reasons are valid enough to understand that good DHEA levels should be kept as long as possible, considering that they briskly decrease after 40, above all in conditions of stress. The experts in the field of aging are convinced that the lack of balance between the production of cortisol and DHEA is responsible for many symptoms connected to the aging process, while many studies demonstrate that DHEA produces a general “sense of wellness”. Researchers of the Jean Hailes Foundation - which focuses on women’s health – carried out a study that was presented yesterday in New Orleans, USA at the 86th annual meeting of the Endocrine Society. The study, based on the examination of 1423 women aged between 18 and 75, proves that low levels in DHEA, which is produced in the suprarenal gland, are significantly connected with problems in sexual excitement and desire in women under 45. Low testosterone levels, instead, do not show any link to the lack of libido in women of that age. By now you will all be asking yourselves what to do in order to keep good DHEA levels. The first step is certainly to radically cut down stressful situations in order to avoid the lack of balance between cortisol and DHEA, practise intense physical exercise regularly, avoid refined substances and simple sugars, consume enough good fats, above all those contained in fish, and help the physiological hormonal activity
through medicinal plants rich in steroidal molecules such as Tribulus terrestris and Dioscorea Villosa. Even if these vegetal extracts cannot be considered substitutes for DHEA, they can act on many levels to fight the symptoms caused by the lack of the hormone itself. According to several researchers, the phytohormones -plants containing active principles similar to the hormone produced by the body – can act through several mechanisms: • Tie to the receptor of the hormone itself, yet the response is weaker and not as long-lasting • Favour the production of hormones on part of the body, for example with an increase in the LH and FSH levels. • Mime the hormone functionality, causing the same physiological response in the body even without tying themselves to the receptor. In conclusion: stress reduction, sport, diet and medicinal plants help the action of DHEA and represent a good antiaging mix which improves women’s psycho-physical condition. Let’s keep in mind that this kind of hormonal lack are often multiple and borderline. Consequently, they cannot be highlighted through the traditional lab analyses. Therefore, the therapist will have to be very careful in the analysis of the patient’s symptoms. In particular, the symptoms of lack in DHEA are both physical and mental: reduction of lean mass, dry skin and hair, cellulite, decrease in libido, anxiety, depression, fatigue.
Considerate people have started looking for more natural methods to fight female disorders, medicinal plants which mime the activity of estrogens in the body, for example soy and hop.
