ACFAS 2022 BREAKS RECORDS IN AUSTIN! The Annual Scientific Conference in Austin broke records once again as being the most attended conference with over 1,900 attendees, and it didn’t stop there. The Exhibit Hall was not only sold out — it was the largest number of exhibitors ever at an ACFAS conference! THE MANY FACETS OF “FELLOWSHIP” May joy and good fellowship reign…—Pierre de Coubertin, father of the modern Olympic Games. It’s been only a short time since I returned to my office after a truly memorable experience at the 2022 ACFAS Annual Scientific Conference in Austin. I imagine a lot of you feel as recharged as I do after an ASC, but that’s been especially true this year, following the most attended ASC to date. No doubt, so many of us were ready to get back together again. Through the pandemic, we’ve found easier ways to access our continuing education virtually. In place of exhibit halls, we’ve had to find alternative methods to engage with our industry partners on whom we depend in our daily practices. As sure as necessity is the mother of invention, the technology world stepped up to our needs, and we will continue to benefit from optimized virtual platforms. Nevertheless, when the opportunity finally presented itself, more of us got together in Austin than at any previous ASC. Throughout the meeting and since, I’ve done some earnest thinking as to why this has been the case. One of the things I think we’ve been longing for is the sense of fellowship we experience when we are able to congregate in large numbers, with those we’ve known since we started down our shared pathways, with others we’ve since met along the way, and with whom we want to share what we’ve been doing since we last met and what we expect we’ll be doing until we meet again. For foot and ankle surgeons, there’s no better place to do it than the ASC.
“Fellowship” is a term we encounter frequently within the realm of what we do as foot and ankle surgeons. In this particular case, it refers to “friendly association, especially with people who share one's interests.” Most recently, if you were to ask our professional community what the first thing that comes to mind when we hear the word “fellowship,” many would likely associate it with post-residency advanced training. Indeed, as the knowledge base of foot and ankle surgery expands and as new technologies continue to develop, it is quite practical to address this with additional training in subspecialty areas. The growth in the number of fellowship training programs has been substantial, and back in 2010 the ACFAS anticipated the potential ramifications of this with the creation of its Fellowship Recognition Initiative. The intent was and remains to foster high-quality programs by establishing specific criteria that must be met to become an ACFAS Recognized Fellowship. Today, the college recognizes over fifty such fellowship programs. For more information, please visit the Fellowship Center at acfas.org. Fellowship might mean something different to some of our newest members. In Austin, I had the distinct honor of assisting ACFAS Immediate Past President Dr. Thanh Dinh in presenting Fellow pins to those foot and ankle surgeons who this year achieved Fellowship member status in ACFAS. ACFAS, as is typical of medical specialty colleges, represents considerably more than just being a member. Another specialty college, the American College of Surgeons, describes being a fellow thusly: The letters FACS (Fellow, American College of Surgeons) after a surgeon’s name mean that the surgeon’s education and training, professional qualifications, surgical competence, and ethical conduct have passed a rigorous evaluation, and have been found to be consistent with the high standards established and demanded by the College Likewise, the post-nominal letters FACFAS indicate that the foot and ankle surgeon has met the threshold of high standards demanded by our own College; in our case, these include board certification specifically by the American Board of Foot and Ankle Surgery (ABFAS), the only board recognized by the Joint Committee on the Recognition of Specialty Boards certifying Doctors of Podiatric Medicine in foot and ankle surgery. Fellowship, in this sense, represents a brand that the public and referring health care professionals can depend upon when seeking surgical care of the foot and ankle. Anywhere you look, there was ACFAS everywhere within the Convention Center.” This year’s International Symposium was also a hit and the Program Committee was happy to see not only a number of international speakers, but also international attendees who flew in from all over the world to see their colleagues speak in the US. “We were very happy to see this trend as our intention was to make our conference

Whether it was the manuscript session, poster exhibits, International Symposium, or the past manuscript winners presentation, we had a lot of data-driven presentations.” He was especially thrilled to see a lot of involve-
ment from younger members, “I was happy to see so many young, motivated members being part of this year’s conference.” Dr. Shibuya credits the Conference Program Committee for this year’s success. “I've never seen anything like this. There was a lot of traffic and activities at the poster and manuscript sessions. People were interested in the latest research! Our committee did a great job attracting people to these “scientific” venues at ACFAS 2022.

Over the duration of my 25 years as a Doctor of Podiatric Medicine, I have frequently heard about real or presumed differences and conflicts between professional organizations, many with other medical specialties but also others within our own. More recently, there has been an earnest recognition that in many cases, those that have differences with us oftentimes share much more in common with us. There is no

better illustration of this than the collaboration that has been fostered by the Joint Task Force of Podiatric and Orthopaedic Surgeons, composed of the ACFAS, the APMA, the American Academy of Orthopaedic Surgeons (AAOS), and the American Orthopaedic Foot and Ankle Society (AOFAS). Representatives within the Task Force will continue to convene to work on mutually-experienced challenges surrounding issues such as reimbursement, CPT code valuation, pre-authorization practices, and unilateral exclusions of specific procedures, modalities and surgical devices by payors, to
name a few. Significant progress has already been made in these endeavors which never would have been possible without the collaborative efforts of the Task Force. So “fellowship” means a number of things to the College, all of which it constantly takes quite seriously. As I enter into my term as President, to me it reflects firstly the common bond all ACFAS members have of being Proven Leaders and Lifelong Learners who are positively impacting the lives of our patients. Per the College’s stated mission, I look forward to doing my part to help empower you as a member of the College.

This year’s manuscript competition received 58 submissions for the 2022 Annual ACFAS Manuscript Awards of Excellence competition. All submissions were blind reviewed by a panel of ACFAS Fellows to select 42 to be presented at ACFAS 2022 in Austin. Winners were announced during the ACFAS Honors and Awards Ceremony on February 25 and split $10,000 in award money funded in part by Organogenesis and the Podiatry Foundation of Pittsburgh. Manuscript submissions for the 2023 Annual Scientific Conference in Los Angeles are due August 3, 2022.

You Can't Afford to Miss ACFAS’ Coding Workshops! Over 150 attendees took home valuable reimbursement information from ACFAS 2022's coding and billing courses, including the newest course—Coding Fundamentals, to help change the way they are paid for patient care in their practices. Coding Fundamentals Coding Fundamentals made its debut in Austin as a foundational course designed to teach or remind attendees the basics of coding and billing—all specific to foot and ankle surgeons. Attendees filled the room to hear from expert faculty Danielle Butto, DPM, FACFAS and Sean Grambart, DPM, FACFAS on terminology, E & M Coding, modifiers as well as some basic procedure coding scenarios – all while earning 2.5 CME. “Coding is hard for a new practitioner, and we’ve heard from members and past attendees that a course where we really break down definitions and the basics of coding would be reassuring and helpful.” Dr. Butto explains. “So, we are really excited to be bringing Coding Fundamentals to life as a first step or refresher of the basics before taking ACFAS’ regular Coding and Billing course to help attendees start at the beginning and walk through the basics before moving on to the next course.” Faculty member Dr. Grambart explains many of the attendees this year were residents or fellows who are going to start to practice soon.
“During residency or fellowship, our surgeons are concentrating on perfecting their surgical skills. As they get closer to graduation, they start to feel the need to receive more education with coding and billing. Coding Fundamentals is designed to help them feel more comfortable transitioning into their practice,” Dr. Grambart said. Dr. Scot Malay Receives 2022 Distinguished Service Award D. Scot Malay, DPM, MSCE, FACFAS was honored with the College’s highest award, the Distinguished Service Award. Dr. Malay received the award in February during ACFAS 2022 in Austin. “I am honored to know the Board of ACFAS feels my work as the Editor of JFAS warrants their Distinguished Service Award for it has been my pleasure to serve as the editor,” said Dr. Malay. “As editor, I strived to make sure every article we published was scientifically sound and useful to the foot and ankle surgeon profession as well as to the medical community as a whole for the benefit of our patients.” “I am indebted to the expertise and hard work all of our Section Editors and Peer Reviewers put forth by providing authors, and me, with critical appraisals of the manuscripts we considered for publication,” he added. Dr. Malay took over as editor of the Journal of Foot & Ankle Surgery in 2006 and retired as the Journal editor May 2, 2022. Naohiro Shibuya, DPM, MS, FACFAS, former deputy editor of JFAS, is now the new editor-in-chief. “The 15 years Dr. Malay was at the College’s premiere research publication’s helm were some of the most exceptional years in the

life of foot and ankle surgery’s scientific breakthroughs,” said Thanh Dinh, DPM, FACFAS, Immediate Past President of ACFAS. “Under his leadership, 90 journals were produced with the highest quality bringing cutting-edge science directly to those providing patient care. We are all, as foot and ankle surgeons, indebted to Dr. Malay for his excellent work in our Journal.” Dr. Malay also expressed his gratitude for the support and guidance of the College’s Council for Journal Management as well as the authors who submitted to JFAS. “No editor can put together a useful journal without support from one’s peers and a steady stream of meaningful manuscripts to consider from scientific authors dedicated to helping to advance patient care through foot and ankle health research.” Dr. Malay earned his doctoral degree in podiatric medicine at the Pennsylvania College of Podiatric Medicine and received advanced training at The Doctor’s Hospital. Dr. Malay is currently the director of podiatric research at Penn Presbyterian Medical Center, and a physician on staff at Penn Podiatry University City. He is board-certified in foot surgery and reconstructive rearfoot/ankle surgery.

ACFAS Promotes You at the Austin Marathon
ACFAS prominently represented you and the specialty to over 15,000 marathon runners and their families at the Austin Marathon ahead of the February 2022 Annual Scientific Conference in Austin. A countless steady stream of runners stopped by the ACFAS booth to ask questions of ACFAS members, pick up reusable FootHealthFacts.org hot/cold packs and to grab infographic literature on keeping their feet healthy for running. One of those members was Austin-based ACFAS Fellow member Andy Bruyn, DPM, FACFAS who volunteered at the booth to offer tips and tricks to runners and answer any questions about potential foot and ankle issues. Dr. Bruyn believes volunteering at a marathon is a great concept and a unique opportunity from a provider’s perspective. “It’s a great way to meet potential patients in the area who really need foot and ankle surgeons and to get involved in the community,” he said.

The marathon drew runners from every US state, as well as some countries from all over the world, making it great exposure for all ACFAS members as well as the profession. The hot/cold packs were the most soughtafter giveaway at the event and attendees were grateful to “ask the surgeon” their questions as well as for the information they received on ways to keep their feet and ankles healthy so they can keep running. Dr. Bruyn expressed how great it was to give people some quality tips and tricks for the race day. “A lot of them just needed help with simple things like blisters but the most common question was about remedies for plantar fasciitis,” he added. While members don’t need to be runners themselves to host a booth in a marathon or community event, Dr. Bruyn was specifically qualified as a runner to offer some tricks he uses. “I taught of a lot of people how to properly moleskin a blister or keep a patch in their pocket for reapplication during the race. Some people needed to run on a certain side
of the road to take advantage of the slope and reduce medial or lateral stress depending on their complaint.” He adds, “These are just little tips I know personally from being a runner, but also from treating these conditions and it was nice to offer some focused pointers right before the race.” ACFAS encourages members to take advantage of running or walking events in their area to increase their practice’s exposure. The College plans to continue representing the profession at marathons across the US and to give members even more tools to help build grassroots efforts in their own communities with a new Marathon Toolbox debuting this year. RESIDENCY DIRECTORS FORUM
Successful In-Person Collaborations
Despite the lingering effects of the pandemic and flight cancellations due to frigid weather in Austin and surrounding cities, this year’s Residents Day and Residency Directors Forum were huge successes, with the Forum hitting recordbreaking attendance. The sessions for both events were laid out to provide specific advice for each audience, and the speakers were carefully selected to share their expertise with various topics. “This year’s Residency Directors Forum, co-hosted by the Council of Teaching Hospitals (COTH), was a wonderful collection of speakers from orthopaedic surgeons to attorneys, psychologists, and even a DIO of a medical school. The panel discussions were full of great tips and pearls for all attendees,” said Jason Piraino, DPM, FACFAS, Co-Chair of the College’s Post Graduate Affairs Committee. For the first time in the history of ACFAS Residents Day, resident attendees had the opportunity to

meet one-on-one with from the COTH, the Council on Podiatric Medical Education (CPME) and the American Board of Foot and Ankle Surgery (ABFAS). The program directors had a one-hour lecture and discussion on resident remediation processes to include prevention measures. Both residents and program directors were provided guidance on steps to improve resident’s pass rates of the ABFAS Board Certification case review with emphasis placed on appropriate chart documentation. “What I enjoyed the most from this year’s events was bringing the program directors, faculty, program coordinators, and deans of the podiatry schools in one room to discuss and address the challenges we encounter each day within our educational programs and to find ways to improve our processes,” said Roya Mirmiran, DPM, FACFAS, Co-Chair of the College’s Post Graduate Affairs Committee. “Every year, the Residency Directors Forum is designed by
residency directors, for residency directors. We use the comments and feedback we have received from prior year programs to design the future programs,” added Dr. Mirmiran. 2022 Fellowship Breakfast Fair Bursting at the Seams The College’s Fellowship Breakfast Fair, held on Saturday, February 26 in Austin, was host to 42 fellowship program directors and current fellows from across the country who set their alarms to wake early and meet those students and residents who are curious about fellowships and considering taking the leap and applying for a coveted fellowship position. The ballroom was full of tables lining the walls and prospective fellows having coffee while networking with various program representatives. Said Emily Zulauf, DPM, AACFAS, Fellowship Committee Member and host of this year’s fair: “This year's event was an overwhelming success! Candidates in attendance networked with current fellows
and directors from over 40 fellowships across the country.
