eHealth – dieser Begriff bewegt seit einigen Jahren vor allem die Politik und IT-Industrie; er ist zum Hype geworden, wenngleich bezogen auf den HypeZyklus aus Sicht der Praxis das Thema sich nun vielerorts im Tal der Enttäuschung bzw. in der Phase der Desillusionierung befindet mit Anzeichen des Übergangs zur Phase der Erleuchtung bzw. realistischen Einschätzung.

Wunderbar kann dies am Großprojekt des Aufbaus der nationalen Telematikplattform (Synonym: Einführung der Elektronischen Gesundheitskarte) – aber auch an vielen lokalen Projekten – beobachtet werden. Dies hat sicherlich eine ganze Reihe von Ursachen, aber ganz wesentlich ist dabei, dass von vielen Akteuren die Komplexität entsprechender Anwendungen unterschätzt wurde und noch wird, das Thema viel zu technisch und zu wenig soziotechnisch gesehen wurde und wird, die Facetten sehr breit gestreut sind bzw. eine einheitliche Begriffsdefinition schwierig ist und die Erwartungen der verschiedenen beteiligten und betroffenen Personenkreise höchst unterschiedlich sind. eHealth – übersetzt mit „elektronisches Gesundheitswesen“ – umfasst erst einmal alle ITAnwendungen im Gesundheitswesen, wird aber im engeren Sinne vor allem für die Vernetzung von Anwendungen – also für telematische Anwendungen im Gesundheitswesen benutzt.

Bei telematischen Anwendungen „handelt es sich immer um einrichtungsübergreifende und ortsunabhängige vernetzte Anwendungen zur Überbrückung von Raum und Zeit, um damit betriebliche oder überbetriebliche Geschäftsprozesse jeglicher Art zwischen Unternehmen oder diesen und/oder ihren Kunden abzuwikkeln und/oder ganz oder teilweise zu automatisieren.“ (Haas 2006) In der Regel werden hierfür betriebliche Informationssysteme verschiedenster Institutionen wie Krankenhäuser, Arztpraxen, Pflegedienste, Krankenkassen usw. oder spezielle Informationssysteme wie elektronische Patienten- oder Gesundheitsaktensysteme mit-

einander verbunden oder aber Funktionen solcher Informationssysteme in einfacher Weise in der Fläche für viele Nutzer verfügbar gemacht – Beispiel KIS und Einweiserportal.

Das wesentliche Ziel von Telematikanwendungen ist die Erhöhung der Wertschöpfung dieser überbetrieblichen Geschäftsprozesse. Dabei stehen aber nicht nur die vorzufindenden traditionellen Prozesse im Mittelpunkt, sondern Telematik selbst ermöglicht die Schaffung neuer Prozesse und Geschäftsmodelle und damit neuer Wertschöpfungen und Wertschöpfungsketten. Beispiele für letztgenannte Anwendungen sind die Teleüberwachung von

Patienten z.B. nach Herzinfarkt bzw. -operation, spontane und bedarfsgerechte interdisziplinäre Telekonsile und Verfahren wie Teleradiologie, Telepathologie usw.

In diesem Sinne ist Telematik auch zu einem wesentlichen Gestaltungsfaktor gesundheitspolitischen Handelns geworden. In einer Studie zur Zukunft der Gesundheitssysteme in den Ländern Europas (Coopers & Leybrand 1997) heißt es treffend: „…Trotz großer Unterschiede im europäischen Gesundheitswesen gibt es viele Aspekte, die den Reformprogrammen der verschiedenen Länder gemein sind. Sie sind Reaktion auf den überall herrschenden Druck und die durch zeitgemäßes Management und moderne Informationssysteme gegebenen Möglichkeiten. …Verbesserte Entscheidungsfindungsprozesse basieren auf jeder Stufe auf Information. Es kann gar argumentiert werden, dass bessere Informationssysteme und bessere Verwendung von Informationen die Grundlagen für jede wirksame Reform von Gesundheitssystemen sind.“ Gesundheitstelematische Anwendungen sind vielfältig und in Haas (Haas 2006) wird eine zweiachsige Klassifikation vorgestellt, die einerseits Anwendungen für die Patientenversorgung, die Lehre und Fortbildung sowie die Forschung unterscheidet und andererseits Anwendungen der Telekommunikation, Teledokumentation, Telekooperation und Entscheidungsunterstützung innerhalb dieser erstgenannten Klassen. Was erwarten nun die verschiedenen Gestaltenden und Betroffenen von eHealth, welchen Wertebeitrag kann die Technologie leisten?

Die Politik in allen Industrienationen hat das große Potenzial der Gesundheitstelematik erkannt und erhofft sich einerseits eine Effektivierung der Versorgung, Kosteneinsparungen, eine Qualitätssteigerung der Versorgung, die vollständige Transparenz des Leistungs- und Behandlungsgeschehens, zeitnahe Steuerungsmöglichkeiten sowie die Möglichkeit der Erschließung neuer Märkte durch neue Anwendungen und damit auch eine Stärkung der Gesundheitswirtschaft. Andererseits aber wird auch – und dies ist wohl der wesentlichste Wertebeitrag aus Sicht der Politik, auch wenn dies kaum offen kommuniziert wird – durch die Verfügbarkeit einer Gesundheitstelematikplattform die Basis zur Einführung eines differenzierten Versicherungsrechtes gelegt, das nur Sinn macht, wenn Leistungserbringer zeitnah überprüfen können, auf welche Leistungen ein Versicherter bzw. Patient aktuell Anspruch hat. Vor diesem Hintergrund ist es verständlich, dass die Online-Versichertendatenabfrage (im gematik-Jargon VSDD: Versichertenstammdatendienst) eine Pflichtanwendung ist und die Onlineanbindung derzeit der Zankapfel schlechthin zwischen Ärzteschaft und Kassen

ist. In vielen Industrienationen werden derzeit erhebliche nationale Investitionen getätigt, um nationale Gesundheitstelematikplattformen aufzubauen. Dabei determinieren aber die historisch geprägten Einstellungen zur informationellen Selbstbestimmung und zum Datenschutz ganz erheblich Art und Weise sowie Kosten dieser nationalen Strategien und Implementierungen.

Der Versicherte bzw. Patient hat heute kaum Vorstellungen bezüglich der möglichen Anwendungen und deren Nutzen für ihn, was an der mangelnden Aufklärung und den fehlenden flächendeckenden Anwendungen, die Kontakt und Erfahrungen damit ermöglichen, liegt. Verschiedenste Umfragen zeigen aber deutlich, dass die Bürger vor allem die schnellere und bessere Dokumentenübermittlung (Arztbriefe, Bilder etc.) sowie den Einsatz elektronischer Patientenaktensysteme als äußerst positiv ansehen und sich wünschen.

Vor dem Hintergrund der demographischen Entwicklung und der Zunahme chronisch kranker und multimorbider Menschen werden aber perspektivisch – auf Basis entsprechender Anwendungen der Telekommunikation und Teledokumentation – vor allem zwei Anwendungstypen einen wesentlichen Wertebeitrag für die Patientenversorgung und den Patienten leisten: Zum Einen sind dies Anwendungen der Teleüberwachung und damit verbunden der Telebetreuung, zum Anderen Anwendungen, die unter Einbezug aller am Behandlungsprozess Beteiligten ein effektives Case Management ermöglichen. tionen der Gesundheitsversorgung, deren Umsetzung heute aber noch an tradierten Verhaltensweisen und Reflexen der Versorger, rechtlichen und erstattungsbezogenen Hürden scheitert. Auch wird mit diesen Anwendungen die geographische Ausdehnung der Leistungserbringung für Betreuung und Nachsorge ermöglicht, was internationale Aktivitäten ermöglicht – Beispiel Operation von Patienten aus dem Ausland hier vor Ort mit nachfolgender Telebetreuung unter telematischer Kooperation mit Versorgern im Heimatland.

Letztendlich aber möchte sich der Patient nicht mit technologischen oder sonstigen Diskussionen belastet wissen, sondern erwartet zu Recht vom Versorgungssystem als Leistung, dass er die bestmöglichste Versorgung bei optimaler Kooperation und Koordination aller am Behandlungsprozess Beteiligten und die bestmögliche Unterstützung bei persönlichen Defiziten erhält. Er erwartet ernstund wahrgenommen zu werden in seiner bio-psycho-sozialen Gesamtheit, dass die Wahrung des Patienten-Arzt-Vertrauensverhältnisses sichergestellt bleibt und er die Möglichkeit hat, seine informationelle Selbstbestimmung praktikabel wahrzunehmen und dass er einen einfachen Zugang zu verständlichen medizinischen

Sachinformationen hat. Heute erlebt er die Realität aber zumeist noch ganz anders in einem in Sektoren und einzelne spezialisierte isolierte Einrichtungen fragmentierten Versorgungssystem.

Auch der Arzt bzw. Heilberufler ist in einer ähnlichen Situation wie der Patient: Auch er hat heute kaum Vorstellungen bezüglich der möglichen Telematikanwendungen und deren Nutzen für ihn, was ebenfalls an der mangelnden Aufklärung und den fehlenden flächendeckenden Anwendungen und dem Kontakt und der Erfahrung damit liegt. So werden also viele Ärzte empfänglich für die Parolen einiger weniger aggressiver Angstmacher, die die Gesundheitstelematik als Frontalangriff auf den ärztlichen Berufsstand verstehen. Hier wird sich erst dann – und ähnlich war dies bei der Einführung von Krankenhausinformationssystemen zu beobachten – eine positive Einschätzung durchsetzen, wenn der konkrete Kontakt bzw. die konkrete Nutzung durch den einzelnen Arzt durch für diese Zielgruppe nutzbringende Anwendungen auf breiter Front ermöglicht wird. Der unzweifelhaft vorhandene positive Wertebeitrag für die Ärzteschaft in Form einfacherer und effektiverer Kommunikation und Kooperation z. B. im Rahmen von Anwendungen wie eArztbrief, eÜberweisung, eMeldung und eAbrechnung oder Anwendungen der Teledokumentation und Teleko-

operation wird sich aber nur dann realisieren lassen, wenn diese Anwendungen aufgabenangemessen und damit so effektiv wie möglich nutzbar in die einzelnen Primärsysteme wie Arztpraxisund Krankenhausinformationssysteme integriert werden. Dies hat sich bereits bei den Evaluationsergebnissen zum eRezept im Rahmen der Testphase zur Einführung der eGK deutlich gezeigt.

Das Management von Versorgungseinrichtungen erwartet von gesundheitstelematischen Anwendungen im Wesentlichen einen positiven wirtschaftlichen Beitrag bei der Abwicklung einrichtungsübergreifender Geschäftsprozesse, die Möglichkeit sich im Wettbewerb durch für Zuweiser und Patienten interessante Angebote besser positionieren zu können und die Möglichkeit, neue Geschäftsfelder und Angebote erschließen zu können. Schon heute gibt es Kliniken, die ihre spezielle Kompetenz und ihre Expertise telematisch weit über ihr klassisches Einzugsgebiet hinaus mit Erfolg vertreiben.

Zusammenfassung Unzweifelhaft können also die vielfältigen Anwendungen der Gesundheitstelematik einen positiven Wertebeitrag im Gesundheitswesen sowohl in quantitativ-ökonomischer als auch qualitativer Hinsicht beisteuern.

Was aber sind heute die größten Verhinderer gesundheitstelematischer Innovationen, was sind die kritischen Erfolgsfaktoren?

Zum Einen sind dies politik- oder industriegetriebene Lösungsansätze, deren Basis eben nicht solide Analysen des Bedarfs aus verschiedenen Nutzer-/Betroffenen-Perspektiven sind, sondern nur

die theoretische und meist technologiegetriebene Vorstellung, was scheinbar Nutzen stiftet und sinnvoll ist. Warum zuerst eRezept statt eArztbrief, warum zuerst Notfalldaten statt klinischer Basisdokumentation? Des Weiteren sind es Lösungsansätze – wie jeder Besucher einschlägiger Messen nachvollziehen kann – die derart umständlich und aufwändig zu bedienen sind, dass jeder Heilberufler abgeschreckt wird und Telematik mit noch mehr administrativem Aufwand, dem zu geringer Nutzen und keine finanzielle

Aufwandserstattung gegenüber stehen, assoziiert. Sodann sind es aus Markt- und Machtstreben zwar berechtigterweise aber kontraproduktiv implementierte proprietäre Lösungsansätze der einzelnen Hersteller, die ein durchgängiges Miteinander der Produkte in einem offenen verteilten System auf Basis produktunabhängiger syntaktischer und semantischer Interoperabilität gar nicht ermöglichen. Lokale Projekte scheitern oftmals an diesen SchnittstellenMissmatches oder müssen sehr hohe Finanzmittel aufwenden, um diese zu überwinden. Aber nicht nur technische Aspekte sind es, die heute – zumeist unnötigerweise – schier unüberwindbare Hürden darstellen, sondern auch sozio-technische und rechtliche Aspekte erschweren Innovationen. Die tradiert autistisch nach innen gerichtet Arbeitsweise vieler Versorgungsinstitutionen mit minimalistischen Schnittstellen nach außen ist noch kein Nährboden für technologiegestützte intensive Kooperation mit anderen Institutionen, bei der für die Beteiligten auch nicht absehbar ist, wie sich eventuell das Leistungsgefüge verschieben wird. So wird also Telematik oft eher als Bedrohung denn als Chance verstanden.

Auch die Frage der adäquaten Sicherstellung des Vertrauensverhältnisses zwischen Arzt und Patienten durch geeignete Datenschutzmechanismen – die oftmals antagonistisch zur Bedienbarkeit der Anwendungen wirken – ist für eine Reihe von Anwendungen heute noch nicht abschließend mit Blick auf dieses Spannungsverhältnis „Schutz und Bedienbarkeit“ geklärt, ebenso wie haftungsrechtliche Aspekte beim Einsatz von elektronischen Gesundheits- und Patientenaktensystemen. Und Letztendlich fehlt es an

Erlaubnis- und Vergütungsregelungen, die telematische Anwendungen zum interessanten Geschäftsmodell werden lassen.

Wie also deutlich wird, gibt es unabhängig vom notwendigen Aufbau einer Telematikplattform, die eine rechtssichere elektronische Dokumentation, Kommunikation und Kooperation ermöglicht, eine ganze Reihe von Handlungsfeldern – von der notwendigen Standardisierung der Interoperabilität für bestimmte Anwendungen bis hin zu Aufklärungskampagnen –, die rasch, zielgerichtet und professionell bearbeitet werden müssen, um aus der Phase der Ernüchterung in eine konstruktive Phase der breiten und nutzbringenden Umsetzung von gesundheitstelematischen Anwendungen mit frühzeitig vorzeigbaren Success-Stories zu kommen.

Hier sind Politik, Industrie, die Selbstverwaltungsorgane der Kostenträger und Ärzteschaft sowie Patientenvertretungsorganisationen aber auch die Wissenschaft mehr denn je gefordert, gemeinsam zu praktikablen Regelungen, Rahmenbedingungen und Standards zu gelangen. Prof. Dr. Peter Haas studierte an der Universität Heidelberg Medizinische Informatik und war danach 5 Jahre in Großkrankenhäusern in der IT – z. T. in leitender Stellung – tätig.

Danach wechselte er in die Industrie, wo er zuerst bei einem Datenbankhersteller und danach bei einem KIS-Hersteller – dort zuständig für Vertrieb, Marketing und Schulung – tätig war. Seit 1994 ist er Professor für Medizinische Informatik an der Fachhochschule Dortmund, wo er einen entsprechenden Studiengang aufgebaut hat. Seine Arbeitsschwerpunkte sind Medizinische Informationssysteme, Gesundheitstelematik und Wissensmanagement/wissensbasierte Systeme in der Medizin. Prof. Haas ist■ Autor von mehreren einschlägigen Lehrbüchern. Er ist derzeit Sprecher des Beirats der gematik sowie Vorsitzender des Fachbeirats der ZTG Krefeld sowie Präsident des conhIT-Kongresses „Praxis der Informationsverarbeitung im Gesundheitswesen“.

Märkte, Wissenschaft und Technologie treiben die Welt voran. Stärker als im 20. Jahrhundert, das stark von zwei Weltkriegen und der nachfolgenden Bipolarität des kapitalistischen und sozialistischen Systems überschattet war, wird das 21. Jahrhundert durch die Macht und Faszination der Technologie und ihrem weltweit ungebremsten Einsatz geprägt.

Technologie ist der Transmissionsriemen zwischen Wissenschaft und Markt, wobei der Markt selbst wissenschaftlich-technische Erkenntnisse massenhaft in die Gesellschaft bringt. Insofern ist Technologie nach dem Schweizer Wissenschaftler Dieter Ruloff zum Treibsatz sozialen, politischen und wirtschaftlichen Wandels geworden, wie nie zuvor in der Geschichte der Menschheit.

Die Innovation von Technologie und ihre globale Distribution öffnen das Fenster, das uns zeigt, wie Zukunft entsteht. Innovationen sind Neuerungen. Sie veranschaulichen die Kraft des menschlichen Intellekts, im Rahmen wissenschaftlich-technischer Erkenntnisse und Prozesse etwas Neues zu schaffen. Sie bedeuten, bekannte Pfade zu verlassen, neue Ideen zu entwickeln, durchaus auch, etwas Anderes anders zu machen. Sie entstehen durch Wissen, Denken und Handeln. Innovationen sind mehr als nur Erfindungen, oder technologische Verbesserungen. Sie repräsentieren eine Wertschöpfungskette, mithin das Wollen, von einer Idee über eine Erfindung und einen Test ihrer Relevanz bis hin zur konkreten materiellen oder immateriellen Nutzung, erfolgreich etwas Neues zu implementieren. In der Ökonomie bringen sie neue Produkte und Dienstleistungen auf den Markt, erhöhen die Produktivität und schaffen neue oder andere Arbeitsplätze. Quantitativ wie qualitativ fundamental sind sie im Rahmen ihrer industriellen Erzeugung für die Performance eines Wirtschaftsraumes. Länder, Regionen oder Kontinente entscheiden mittels der Innovationskraft ihrer Unternehmen und ihrer Ordnungspolitik über den Grad ihrer Wettbe-werbsfähigkeit und Prosperität.

Politik ist eine der entscheidenden Voraussetzungen, die Innovationen erst ermöglichen. Gleichzeitig fundieren Innovationen in der Politik die wirtschaftliche Performance selbst. Es geht um die Ausformung und Qualität der Sozial- und Bildungssysteme sowie

der Infrastruktur; und es geht im Mehrebenensystem der EU um die Verteilung der Verantwortung zwischen den Mitgliedstaaten und der Kommission selbst. Innovation ist somit ein System, dass in Wissenschaft und Technik ebenso wie in Politik und Gesellschaft Modernität und Fortschritt generiert.

Eine weitere Verbindung zur sprunghaft wachsenden Bedeutung der Gesundheitstelematik ist: in der Marktwirtschaft treten nicht nur kurze und mittlere Wirtschaftsschwankungen auf, sondern auch lange Zyklen, mit einer Periode von 40 bis 60 Jahren. Sie beruhen auf Basisinnovationen, welche die Weltwirtschaft in einen kräftigen Wachstumsprozess führen. Sie gelten als Auslöser ganzer Wirtschaftszyklen, die man als Theorie nach dem russischen Wirtschaftswissenschaftler Nikolai Kondratieff auch Kondratieff-Zyklen nennt. Die Dampfmaschine, die Elektrotechnik, die Chemie, die Informations- und Kommunikationstechnologie sind Beispiele solcher Basisinnovationen, die man bisher in fünf Kondratieff-Zyklen eingeteilt hat. Sie haben das Tempo und die Richtung des Innovationsprozesses über mehrere Jahrzehnte weltweit bestimmt. Mit bahnbrechenden Entwicklungen und Innovationen in der modernen Medizin steht die Weltwirtschaft nach den oben genannten Basisinnovationen mutmaßlich wieder am Beginn eines neuen Zyklus, dem sechsten Kondratieff. Der Megamarkt des nächsten Kondratieff wird dabei nach dem Wirtschaftswissenschaftler Leo Nefiodow der Gesundheitssektor sein. Gesundheit wird hierbei

Telemedizin ist eine sinnvolle Ergänzung klassischer Versorgungsmethoden. Angewendet insbesondere bei chronischen Erkrankungen wie Herzinsuffizienz oder akuten Problemen wie Herzinfarkt steht sie für eine innovative Behandlungsform, die elektronische Mittel für Vorsorge und Behandlung nutzt. In einem telemedizinischen Zentrum mit elektronischer Patientenakte kann so der Kreislauf von ärztlicher Diagnostik und Therapiekontrolle mit einem Fernmonitoring durch die elektronische Übertragung etwa von kardiologischen Vitalparametern medizinisch besser und kosteneffizienter organisiert werden. Ihr Potenzial und weit reichende Erfahrungen in anderen Ländern lassen erkennen, dass sie sich auch in Deutschland langfristig durchsetzen wird. In Israel etwa ist sie flächendeckend im Einsatz. Israel ist Benchmark, da das Land insgesamt ein dicht und kohärent organisiertes Gesundheitssystem mit optimierten Behandlungszyklen und Versorgungsmaßnahmen hat, das vergesellschaftete und private Versorgungskomponenten verknüpft, und weltweit hinsichtlich Technik und Eigenverantwortung als besonders innovativ gilt. Hierbei fließen rund 11 Milliarden US-Dollar jährlich in das nationale Gesundheitssystem.

Das sind etwa 8 Prozent des Bruttosozialprodukts. Diese Mittel unterhalten 4 Krankenkassen, 29 Hospitäler und 21 psychiatrische Kliniken mit insgesamt 14.200 Betten. Das als hoch funktionsfähige geltende System schließt öffentlich-staatliche und private Einrichtungen ein; ein Drittel der Finanzierung ist privat. Private

Finanzierungsanteile sind grundsätzlich beachtlich, gelten sie doch als wichtige Antreiber von Innovationen im Gesundheitssystem.

Die Wirksamkeit der Gesundheitstelematik wird mittlerweile durch zahlreiche Studien belegt. Sie ermöglicht es, Fortschritte in der Medizintechnologie mit ökonomischer Effizienzsteigerung zu verbinden. So bietet etwa das Telemonitoring die Möglichkeit, Rekonvaleszente früher aus der stationären Behandlung zu entlassen und im privaten Umfeld weiter zu betreuen. Aus der Sicht der Beteiligten kann die medizinische Nachversorgung somit für die Patienten angenehmer und für die Akteure im Gesundheitssystem billiger gestaltet werden.

Angesichts der Zunahme des Lebensalters und der Anzahl chronischer Erkrankungen werden medizinische wie gesundheitsökonomische Aspekte der Versorgung immer wichtiger. Alleine in Deutschland leben 1,6 Millionen Menschen mit Herzinsuffizienz und über 5 Millionen Menschen mit koronarer Herzkrankheit, die es heute und in Zukunft gut und ökonomisch vertretbar zu versorgen gilt. Weitere chronische Krankheiten wie Diabetes oder periphere Verschlusskrankheiten kommen zu diesem Potenzial hinzu.

Die Gesundheitstelematik zeigt hierfür praktikable Wege und überraschend gute Lösungen auf. Ein flächendeckender Einsatz der Technologie hat jedoch noch Hürden zu überwinden. Standards und einheitliche Verfahren gehören dazu. Der medizinische Nutzen der Telemedizin ist durch zahlreiche Studien belegt, und sie wird international erfolgreich eingesetzt. Die USA stehen hier neben Israel als gutes Beispiel: Auch dort ist ein starkes Wachstum des Gesundheitsmarktes zu beobachten und Telemedizin gilt hier als probates Mittel, um die Kosteneffizienz der Behandlung signifikant zu steigern. Untersuchungen in Amerika haben gezeigt, dass der Einsatz der Gesundheitstelematik unvorhergesehene Notfalleinweisungen verringern, Liegezeiten in Hospitälern einschränkten und so die Kosteneffizienz steigern kann. Telemedizin ist darüber hinaus ein Mittel, um die strategischen Ziele des „eHealth Action Plan“ für alle Bürger der europäischen Union zu erreichen. Diese von der Europäischen Kommission formulierten Ziele sind besserer Zugang, bessere Qualität und höhere Effizienz hinsichtlich der Gesundheitsdienste.

Der internationale Vergleich zeigt das enorme Potenzial der Telemedizin auf. Das in Europa und Deutschland gewollte Maß an medizinischer Qualitätssteigerung und Kosteneffizienz rückt mit ihrem Einsatz in große Nähe. Mit Blick auf Deutschland aber gilt: Eine Innovation gesundheitstelematischer Instrumente und Prozesse hat die besonderen Eigenheiten des deutschen Gesundheits-

Hierbei spielen die Einbindung der Ärzte, die stärkere Eigenverantwortung der Patienten und die adäquaten rechtlichen Rahmenbedingungen eine entscheidende Rolle. In Zukunft wird der „Public-Private-Mix“ bei einer insgesamt weiter wachsenden Gesundheitssparte auch in Deutschland deshalb immer wichtiger.

Es gilt die Telemedizin nun energisch einzusetzen und beim Datenschutz sorgfältig aber auch pragmatisch zu handeln. In diesem Sinne sind mehr internationale Vergleichsstudien notwendig um Best Practices angemessen studieren zu können.

Gesundheitstelematik und Telemedizin sind Beispiel einer innovativen Spitzentechnologie. Hinsichtlich ihrer Bedeutung im Gesundheitssektor erschließt sie die Möglichkeit, Fortschritt in der Medizintechnologie mit ökonomischen Fortschritten und politischen Zielsetzungen zu verbinden. Sie ermöglicht, medizinischen Fortschritt nicht nur an der Sicherheit, Qualität und Wirksamkeit auszurichten, sondern auch die Effektivität und Wirtschaftlichkeit der Gesundheitsversorgung im Auge zu behalten. Voraussetzung dazu sind Deregulierung des Gesundheitswesens im Nationalstaat und in Europa sowie die Schaffung von Marktstrukturen. Der rudimentäre Markt lässt sich als zentrales Hemmnis für technologische Innovationen und Kosteneffizienz der Gesundheitsversorgung identifizieren. Einen Anstoß zur Marktöffnung und für Innovationsprozesse könnte ein Bericht nach dem Vorbild des Cecchini-

Reports leisten. Er müsste einmal die Kosten der Nicht-Öffnung des Gesundheitssektors für die Wirtschaftlichkeit der medizinischen Versorgung, die Innovationskraft in Europa sowie die gesamtwirtschaftlichen Effekte evaluieren. Insgesamt gilt es zudem, den Kulturprozess telemedizinischer Innovation im öffentlichen Raum zu begleiten und den kommunikativen Herausforderungen im Gesundheitssektor und bei der Einführung der Telemedizin produktiv zu begegnen. Die Debatte um die medizinische Versorgung sollte geöffnet werden, so dass nicht nur die Argumentationsfigur der Finanzierbarkeit und Wirtschaftlichkeit des medizinischen Fortschritts thematisiert wird, sondern dass die Potentiale und Chancen für die Innovation und die gesamtwirtschaftlichen Effekte hervorgehoben werden. Die Diskussionen im Gesundheitswesen müssen in den größeren Kontext einer Debatte um Zukunft, Fortschritt und Wachstum eingebettet sein. ■

1975 folgte die Habilitation im Fach Politikwissenschaft. Von 1975 bis 1995 war er Professor für Politikwissenschaft an der Universität Mainz, von 1986 bis 1988 Professeur associé an der Sorbonne, Paris, und von 1987 bis 1999 Koordinator der Bundesregierung für die deutsch-amerikanische Zusammenarbeit. Von 1992 bis 2007 war Professor Weidenfeld Mitglied des Vorstandes der Bertelsmann Stiftung, seit 1995 ist er Inhaber des Lehrstuhls für Politische Systeme und Europäische Einigung an ■ der Ludwig-Maximilans-Universität München und Direktor des Centrums für angewandte Politikforschung (C•A•P) in München.

Jürgen Turek, geboren am 24. April 1959, ist stellvertretender Direktor des Centrums für angewandte Politikforschung (C•A•P) an der Ludwig-MaximiliansUniversität München und leitet dort zusätzlich die Forschungsgruppe Zukunftsfragen. Davor war er von 1981 bis 1987 Dokumentationsredakteur im Presse- und Informationsamt der Bundesregierung (BPA), von 1988 bis 1991 wissenschaftlicher Mitarbeiter am Institut für Politikwissenschaft der Universität Mainz und 1991 bis 1992 Pressereferent bei der KölnMesse GmbH in Köln. Zwischen 1993 und 1995 war Jürgen Turek in verschiedene Projekte ■ des C•A•P als wissenschaftlicher Mitarbeiter involviert. Vor seiner Berufung in die jetzigen Positionen war er dort von 1995 bis 1998 stellvertretender Leiter der Bertelsmann Forschungsgruppe Politik, ab 1998 Leiter der Forschungsgruppe Zukunftsfragen.

Online Tumor Konferenzen Effizienz für Ärzte und und eine bessere Behandlungsqualität kann zum Beispiel bei Online Konferenzen erreicht werden. So können wöchentliche Tumorkonferenzen eines standortübergreifenden Brustzentrums durchgeführt werden, um die Fälle mit allen Beteiligten der unterschiedlichsten Fachdisziplinen (wie Gynäkogie, Onkologie, Radiologie) zu besprechen. Die eingesetzten Technologien erlauben es, dass auch niedergelssaene Fachärzte und Zuweiser an den Konferenzen teilnehmen, obwohl diese gegebenfalls nicht über die High Definition Auflösung der Pathologen und Onkologen verfügen. Durch die Integration der normalen Arbeitsplatzumgebung der niedergelassenen Ärzte ist es jedoch für diese möglich, wenn auch mit schwächerer Bildqualität. Somit entfallen unproduktive Fahrzeiten. Darüber hinaus können internationale Spezialisten eingebunden werden, die sonst nur unter erheblichen Aufwand, wenn überhaupt teilnehmen könnten. In den Sessions kann parallel eine Fortschreibung der Krankenakte im Beisein aller Beteiligten erfolgen. Die Einberufung derartiger Videokonferenzen wird dabei über die normalen Microsoft Outlock Mechanismen spontan oder als Serie von Terminen gewohnheitsmäßig angelegt.

Damit bewegen sich die Ärzte in der normalen Officeumgebung und eine Umstellung oder das Erlernen einer neuer Umgebungen ist nicht notwendig. Die große Verbreitung der Basisinfrastruktur und die damit geringen Kosten sorgen für eine große Akzeptanz im medizinischen Bereich.

HomeCare statt Hausbesuche HomeCare@Facharzt sichert eine Patientenversorgung außerhalb der Praxisräume. Der behandelnde Arzt verschafft sich einen Eindruck vom Gesundheitszustand des Patienten verordnen Medikamente oder setzen sie ab, bei gleichzeitiger Beratung der Patienten und Angehörigen. Der Kontakt zum Patienten ist ein elementarer wichtiger Bestandteil der hausärztlichen Tätigkeit. Es stellt sich jedoch die Frage, wann ist ein Hausbesuch notwendig oder ist die Kommunikation mit dem Patienten nicht der wichtigere Bestandteil. Letztendlich bestimmen auch Aufwand und Abrechnungsmöglichkeiten die Frequenz der Hausbesuche. Die verfügbaren integrierten Lösungen von HP ermöglichen es niedergelassenen Ärzten sehr viel einfacher als in der Vergangenheit, audiovisuell mit den Patienten in Kontakt zu treten. Dazu ist es nicht mehr zwingend notwendig, dass der Patient einen Computer nutzt, sondern er kann über eine neue Generation von „Videotelefonen“ mit dem Arzt über ISDN oder das Internet (DSL) in Verbindung treten. Die Handhabung der Geräte ist dabei ausserordentlich einfach und liegt auf dem Niveau normaler Telefone.

Durch die hohe Standardisierung ist es möglich, weitere Geräte aus dem Telemedizinischen Bereich einzubinden und die Funktionalitäten zu nutzen, um medizinische Daten zu zusammen mit dem Videotelefonverbindung zu übertragen.

HomeCare statt Klinik Aufenthalt Steigender Kostendruck, neue Behandlungsmethoden und immer kürzere Klinik Aufenthalte bedürfen neue Wege um den Patienten zu Hause anstatt in der Klinik zu behandeln. Immer kompliziertere Eingriffe werden ambulant ausgeführt und aufgrund des starken Kostendrucks wird die Nachsorge bis hin zur Reha immer öfter den Familien des Patienten übertragen. Der Patient muss in der Regel von der Familie zuhause betreut und oft noch täglich in die Klinik zur Nachsorge oder Reha gebracht werden.

Die oben beschriebenen Szenarien zeigen deutlich das Video, Telefonie und Instant Messaging zusammengewachsen sind und heute über die Breitbandversorgung in Deutschland fast allen privaten wie professionellen Benutzern bereitsteht.

Online Kommunikation für Ärzte, Kliniken und Patienten wird rasant zunehmen und in sehr kurzer Zeit einen festen Standard in der Kommunikation zwischen Arzt und Patienten prägen.

In allen Fällen wird der Arzt die Kommunikation zu seinen Patienten durch die visuellen Möglichkeiten wesentlich verbessern und eine weitaus höhere Anzahl an Kontakten haben. Termine sind einfacher zu planen und Fahrzeiten werden verringert. Damit kommt der Arzt in vielen Fällen dem Wunsch nach umfangreicher Betreuung näher und hat zugleich die Möglichkeit, selber effizienter zu arbeiten.

Letztendlich tritt an dieser Stelle die medizinische Versorgung mit anderen Wirtschaftszweigen wie z. B. der Transportwirtschaft in Wettbewerb. Bei einem weiteren Durchdringung der Technologie wird zunehmend die Frage aufkommen, wann der Weg zum Arzt tatsächlich gesucht wird. Und es entstehen neue Verhaltensmuster.

So haben in den letzten vier Jahren die sozialen Netzwerke ungemein an Bedeutung gewonnen. Der Austausch von beruflichen Informationen oder das Pflegen und Kontaktieren von Freunden ist heute über das Internet schon fast selbstverständlich. Was als interessante Idee für Endbenutzer begann ist heute zu einer Vielzahl von interessensbasierter User Groups ausgereift. Die Nutzung von Standards hat diese imposante Entwicklung möglich gemacht. Mit der Integraton verschiedener visueller Technologien basierend auf Standards kann und wird das Gesundheitssystem ebenfalls von Normen profitieren. Die marktreifen Technologien werden für eine erhöhte, eine bessere und freiwillige Kommunikation zwischen Patienten und Leistungserbringern führen. ■

Der Bedarf für neue Versorgungsmodelle – das Zuhause als Zentrum der Gesundheitsversorgung Jens Seeliger

Zum heutigen Zeitpunkt sind etwa über 16 Mio. aller Einwohner Deutschlands über 65 Jahre alt. Das Statistische Bundesamt geht davon aus, dass dieser Bevölkerungsanteil im Jahre 2050 auf ca. 23 Mio. angestiegen sein wird.

Bis zu 50 % aller Kosten im Gesundheitswesen entwickelter Länder entfallen derzeit auf die Versorgung älterer Menschen. So lagen z. B. die Krankheitskosten für Menschen ab 65 im Jahr 2002 bei 6.250/5.830 Euro (weiblich/männlich) und ab 85 Jahren sogar bei 12.660/11.690 Euro pro Jahr (w/m). Für 30–44 Jährige betrugen die Kosten lediglich 2.150 bzw. 1.250 Euro. Ohne die Einführung neuer Modelle der Krankenversorgung bedroht dieser demographische Count-down das Gesundheitswesen Deutschlands und weltweit.

Wir müssen uns von unserer derzeitigen Abhängigkeit von kostspieliger akuter und stationärer Krankenversorgung älterer Menschen entfernen, um ein Modell anzusteuern, bei dem die Versorgung in den eigenen 4 Wänden im Zentrum steht. Wir bei Intel nennen das „Shift Left“, d. h. Reduzierung von teurer, akutmedizinischer Versorgung durch Stärkung der Vorsorge und des selbständigen Alterns mit Hilfe von intelligenter Homecare- und Telemonitoring-Technologie in den eigenen 4 Wänden.

Der Weg zum proaktiven, patientenorientierten Modell

Zwei Drittel aller Europäer über 65 sind von mindestens einer chronischen Krankheit betroffen, die häufige und teure Arztbesuche erfordert.1 Forschungsergebnisse haben jedoch gezeigt, dass Menschen, die körperlich fit und in ihrem sozialen Umfeld aktiv bleiben und gleichzeitig ihre eigene Gesundheitsversorgung mit zunehmendem Alter eigenständig verwalten, in der Regel länger gesund bleiben.2 Wenn es uns gelingt, älteren Menschen den Kontakt und Umgang mit Ärzten, Krankenpersonal und Betreuern von ihrem eigenen Zuhause aus zu erleichtern, könnte damit sowohl ihre

Lebensqualität verbessert, als auch der Bedarf an stationärer Pflege gesenkt werden.

Wir sind der Auffassung, dass neue digitale Technologien für das Gesundheitswesen bei den Herausforderungen des Alterns weltweit eine Rolle spielen können. Der Schwerpunkt dieser Technologien wird eher auf der Vorsorge als auf Behandlung liegen und setzt voraus, dass Einzelpersonen, ihre Familienangehörigen und Bekannten bei der Durch- und Ausführung der Versorgung Verantwortung mitübernehmen.

Ethnografie: Das Verstehen von Menschen in ihrem Umfeld Die Entwicklung gänzlich neuer Technologien für den Einsatz im Alltag erfordert einen sorgfältigen Einblick in die Lebensweise und tatsächlichen Bedürfnisse von Mitmenschen.

Um den Altersprozess besser zu verstehen, haben „Intel Ethnografen“ sehr viel Zeit in über 1.000 Wohnungen und 150 Krankenhäuser und Kliniken in 20 Ländern verbracht.

Mithilfe von Offenen Befragungen, Beobachtungen und wiederholten Besuchen in Wohnungen haben die Forscher das Miteinander von Personen und deren Verhalten im eigenen Umfeld untersucht. Was fiel den Beteiligten leicht, was fiel ihnen schwer? Welches Gerät oder welche Form von Intervention oder Vermittlung wäre am hilfreichsten? Diese Angaben wurden dann an die technischen Fachkräfte und Wissenschaftler weitergegeben, die die neuen Technologien entwerfen und produzieren.

Ethnografie als Grundlage für die Entwicklung neuer Technologie Die Entwicklungstechniker bei Intel haben nun ethnografische Forschungsergebnisse in innovative Entwicklungskonzepte für das Gesundheitswesen umgesetzt. Diese Arbeit läuft derzeit u.a. im Rahmen des TRIL-Centre (Technology Research for Independent Living) in Irland als ein gemeinsames Projekt von Intel und der irischen Industrial Development Agency (http://www.trilcentre.org).

Die Intel Digital Health Forscher arbeiten Seite an Seite mit Forschern aus mehreren Fachbereichen an irischen Universitäten, um technologische Neuerungen zu entwickeln und vorzustellen, die selbstversorgtes Leben im Alter im eigenen Heim unterstützen.

Ihre anfänglichen Bemühungen konzentrieren sich auf die Verhinderung von Sturzunfällen, auf kognitive Funktionen und soziale Kontakte.

Die Bedeutung eines aktiven sozialen Austausches wird mitunter übersehen, obwohl er ein Hauptfaktor für gesundes Altern ist.

Die Teilnahme am Sozialleben stellt einen bedeutenden Motivationsfaktor für körperliche Bewegung dar, die ihrerseits gesundheitsfördernd ist, und auch die Belastung des kognitiven Abbaus vermindert.

Digitales Gesundheitswesen im Heim und darüber hinaus Untersuchungen zeigen auf, dass Patienten es vorziehen im eigenen Heim versorgt zu werden. Es muss ein neues Modell der Gesundheitsversorgung entstehen und diese Art der Versorgung anbieten. Eine erfolgreiche Versorgung im eigenen Heim erfordert verbesserte Kommunikationswege zwischen Patienten und ihrem ärztlichen Personal sowie besseren Zugriff auf Patientendaten.

Technologie wird aller Voraussicht nach eine wichtige Rolle bei der Gesundheitsversorgung im eigenen Heim spielen und muss daher allen zur Verfügung stehen und leicht bedienbar sein. Einfache, benutzerfreundliche Technologien können dazu beitragen, dass Patienten aktiv, engagiert und mit ihrem ärztlichen Versorgungsteam in Verbindung bleiben, ihre Lebensqualität steigern und ihre Unabhängigkeit länger aufrechterhalten.

Die Intel Digital Health Group entwickelt derzeit eine Reihe von Lösungen, mit denen die Gesundheitsversorgung im eigenen Heim durchsetzbar ist und mit deren Hilfe alternde und chronisch kranke Mitmenschen besser versorgt werden können. Über die Entwicklung von Produkten hinaus wirken wir im Rahmen der Continua Health Allianceam Aufbau eines umfangreichen Marktes für interoperable, persönliche Telehealth-Geräte und -Dienstleistungen mit. Das Ziel der Zusammenarbeit von Intel und Continua und weiterer Bemühungen ist es, Übernahmeschwellen, u. a. von Seiten von Zulassungsbehörden, Kostenerstattung und Richtlinien, zu mindern.

B. die mobile klinische Assistenzplattform – auch MCA (Mobile Clinical Assistant) genannt, welche auf ausgiebigen Untersuchungen aufbaut, an denen aktiv tausende ärztlicher Fachkräfte und Softwareanbieter teilgenommen haben, um für Krankenpersonal, Ärzte und andere ärztliche Fachkräfte eine wirksamere Arbeitsweise an unterschiedlichen Standorten zu gewährleisten und durch Zugriff auf Patientendaten am Krankenbett eine bessere Behandlung zu ermöglichen.

Einblick in den Bedarf an Gesundheitsversorgung Es ist unser Ziel bei der Alterungsforschung, Einblick in den Gesundheitsversorgungsbedarf älterer Menschen und ihrer Betreuer zu erhalten und die Erneuerung der Technologie der nächsten Generation einzuleiten, die diese Menschen und ihre Betreuer unterstützt. Indem wir Menschen helfen, bei der Überwachung ihrer Gesundheit mehr Initiative zu ergreifen und Betreuern, Ärzten und Krankenpersonal die erforderlichen Informationen und Geräte zur Verfügung stellen, um ihre Patienten zu jeder Zeit und an jedem Ort erfolgreich zu versorgen, hoffen wir beim Eingehen auf die Bedürfnisse der alternden Bevölkerung eine wichtige Rolle spielen zu können. ■ 3 Motion Computing war der erste Hersteller, der ein MCA-Produkt, das Motion C5, auf den Markt brachte.

Sicher vor Identitätsdiebstahl Thomas Bengs

Was bedeutet Identität? Welche Gefahren sind damit verknüpft? Wie können wir diesen Gefahren mit den heute verfügbaren technischen Mitteln begegnen?

Relevante Fragen unserer heutigen Gesellschaft, die auch weite Bereiche des allgemeinen Gesundheitswesens betreffen. Biometrische Authentifizierung sichert dabei neuartige Lösungsansätze.

Die Enzyklopädie bezeichnet Identität (lat. Idem... derselbe, der gleiche) die beim Menschen kennzeichnende und als Individuum von anderen Menschen unterscheidende Eigentümlichkeit seines Wesens. Im weiteren sozial-psychologischen Sinne wird Identität häufig als die Summe der Merkmale bezeichnet, durch die sich ein Individuum von anderen unterscheiden lässt. Das erlaubt eine eindeutige Identifizierung.

Parallel zu den Entwicklungen von Regularien und Methoden zur Identitätsfeststellung stieg auch das kriminelle Potential, Identitäten zu fälschen, zu imitieren oder zu stehlen.

Zu Beginn des menschlichen Lebens wird die Personenidentität durch eine offizielle Geburtsurkunde beglaubigt. Diese dient im Verlauf unseres Lebens als Grundlage für weitere Identifikationsunterlagen, die unsere Person zweifelsfrei bestimmen sollen, wie beispielsweise ein Personalausweis oder Reisepass. Ohne einen gültigen Personalausweis erhält niemand – zumindest in Deutschland – zusätzliche Identifikationsmittel, mit denen sich die Person für bestimmte Vorgänge und Prozesse authentifizieren kann.

Der Bundespersonalausweis ist letztendlich ein Dokument, der die individuelle Identität sicherstellt. Die Fälschungssicherheit dieses Ausweises wurde in den letzten Jahren drastisch erhöht und dennoch war dieser Identitätsbeleg nie vollkommen sicher vor Falsifizierungen. Bot der Personalausweis dank seinem beinhalteten Personenfoto und der persönlichen Unterschrift zumindest zwei individuelle Faktoren, die zur sicheren Identifikation einer Person verhelfen – wenn auch nicht hundertprozentig, so ist dies bei der Applikation von Identitätskarten, wie im bargeldlosen Zahlungsverkehr oder auch bei Karten von Krankenversicherungen zur Identifizierung eines Patienten beim Arztbesuch nicht der Fall.

Solche Identitätskarten sind, auch mit einer persönlichen Unterschrift, leicht übertragbar. Sie können bei Verlust oder bei Diebstahl von anderen Personen genutzt werden und verursachen somit einen beträchtlichen wirtschaftlichen Schaden.

Bereits weit vor dem neuen Millennium befassten sich Politiker und Wirtschaftsexperten mit der Frage, wie die persönliche Identität besser gegen den vermehrt aufkommenden Identitätsdiebstahl gesichert werden könnte.

Zunächst erweiterte die Finanzwirtschaft die Kartensicherheit auf eine 3-Faktor-Methode (Besitz der Karte + Magnetstreifen+ Eingabe einer geheimen PIN-Nummer). Dies erlangte nur einen kurzzeitigen Erfolg, denn die Karten konnten ausgelesen, kopiert und PIN-Nummern ausspioniert werden.

Sicherlich trugen die Terroranschläge des 11. Septembers in New York wie auch die vielen darauffolgenden Regierungsbeschlüsse weltweit dazu bei, individuelle wie auch biometrische Merkmale in die Reisepässe und nationalen ID-Dokumente zu integrieren.

Hierzu werden vorwiegend der Fingerabdruck und individuelle Gesichtsmerkmale in den Pass digital eingefügt. So können diese Merkmale an den Grenzen jederzeit mit entsprechenden, technischen Geräten überprüft werden.

Aber ist die Biometrie die Lösung für den stetig steigenden Identitätsdiebstahl überhaupt praxistauglich, wirtschaftlich umsetzbar sowie sozialpolitisch vertretbar?

Zu den hervorragenden Eigenschaften der Handflächenvenenbiometrie zählen Fälschungssicherheit, kontaktfreie und daher hygienische Anwendung wie auch ihre unkomplizierte Nutzbarkeit für jeden Menschen - unabhängig von Alter oder Geschlecht.

“

Mittlerweile existieren eine Reihe unterschiedlicher biometrischer Verfahren, die die Sicherheit in der Praxis deutlich erhöhen.

Die bekanntesten Technologien sind die Erkennung des Fingerabdrucks wie auch die Gesichts- und Iriserkennung. Beim Einsatz von Biometrie zur zusätzlichen Absicherung der Identität in den verschiedenen Anwendungsgebieten müssen jedoch Vorrausetzungen erfüllt werden, die nicht alle biometrischen Erkennungstechnologien gleichermaßen erfüllen. So wurde bestätigt, das Fingerabdrücke sehr junger und älterer Menschen von heutigen Sensoren nur schwierig oder gar nicht erfasst werden können. Mit dem Fingerabdruck wird gewissermaßen auch ein krimineller Hintergrund in Verbindung gebracht. Ist ein Finger verschmutzt oder verkratzt, ist eine solcher Erkennung nicht immer gewährleistet. In

Anwendungsbereichen, die eine gewisse Hygiene voraussetzen, ist der Einsatz eines Fingerabdrucksensors sicherlich nicht sehr praxisnah.

Die Gesichtserkennung ist immer abhängig von den Umgebungsbedingungen (z. B. Lichtverhältnisse). Da sich das Gesicht im Laufe eines Lebens verändert, müssen die erforderlichen biometrischen Daten gegebenenfalls öfter neu erfasst werden. Kosmetische Schminke, Bart- und Haarwuchs beeinträchtigen unter Umständen die Ergebnisse. Sogar Fälschungen durch entsprechende Masken sind möglich. Die Werte der Iriserkennung sind von den örtlichen Lichtverhältnissen abhängig, und für Brillen- und Kontaktlinsenträger sind diese Verfahren äußerst umständlich. Hinzu verursacht die Iriserkennung immer noch ein unangenehmes Gefühl bei den meisten Anwendern und generiert die Frage: Was passiert hier mit meinem Auge?

Datenschutzrechtlich ist der Einsatz von Biometrie ein wiederholter Anlass zur Diskussion: Wo werden biometrische Daten gespeichert? Wer hat Zugriff darauf? Sind die biometrischen Merkmale fälschungssicher? Wie kostenintensiv sind biometrisch basierende Sicherheitsanwendungen?

Um es vorweg zunehmen: eine alle kriterienerfüllende biometrische Technologie existiert heute nicht. Biometrie muss einsatzgerecht und praxisnah in bestehende Anwendungsbereiche eingebunden werden können. Als ein Beispiel mag die Handflächenvenentechnologie von Fujitsu dienen.

Auf Grund politischen Drucks und der Neigung der Medien, wurden die japanischen Banken im Jahre 2001 aufgefordert, ihre Bankautomaten und Kreditkarten gegen Diebstahl, Fälschung und

weitere Missbräuche effektiver zu schützen. Schnell konkretisierten die Banken die hohen Sicherheitsfaktoren beim Einsatz der Biometrie im Zahlungsverkehr. Nur welche Art von Technologie?

Viele Bankkunden verknüpften die Anwendung des Fingerabdrucks mit einem kriminellen Hintergrund und lehnten das Verfahren ab. Weder die Durchführung von Gesichtserkennung noch die Iriserkennung an einem Bankautomaten schien den Kunden sinnvoll zu sein. Japans größte Bank – die Tokio Mitsubishi Bank – entschied sich für eine neue biometrische Ausrichtung, die Erkennung der Handflächenvenenstruktur, eine Technologie der Fujitsu Limited.

Dabei erfasst und vergleicht ein Sensor die hochkomplexe und für jeden Menschen individuelle Venenstruktur in der Handfläche mittels Nah-Infrarotlicht. Das sauerstoffreduzierte Blut in den Venen absorbiert das auftreffende Infrarotlicht, eine Sensorkamera nimmt das so sichtbare Bild auf, verschlüsselt es und wandelt es in ein EDV-taugliches Template um. Nach der ersten Erfassung des Venenmusters wird das Template in den Chip auf der Kreditkarte gespeichert.

Möchte der Kunde einen Geldbetrag am Bankautomaten mit der Kreditkarte abheben, muss die Person nicht nur im Besitz der korrekten Kreditkarte und Pin-Nummer sein, sondern sich zusätzlich über einen Handflächenvenenscan identifizieren lassen. Nur wenn das am Bankautomat eingescannte Venenmuster mit dem im Chip der Kreditkarte gespeicherten Muster übereinstimmt, wird das Geld ausgezahlt. Ansonsten wird die Auszahlung verweigert. In Japan sind gegenwärtig circa 150.000 Bankautomaten installiert.

tet. Mehr als sieben Millionen Kunden nutzen diese Systeme ohne Vorfälle von Identitätsdiebstahl. Hinzu haben sich mehrere Banken in Südamerika und im asiatischen Raum für diese biometrische Authentifizierungs-Methode entschieden. In Europa, insbesondere in Deutschland, wurde die Handflächenvenenerkennung im Geldautomaten durch Banken pilotiert und es steht zu Erwarten, dass noch in diesem Jahr mehrere öffentliche Pilotprojekte hinzuzukommen, zumal diese Technologie durch das Bundesamt für IT-Sicherheit (BSI) im November 2008 als eine von sehr wenigen Biometrietechnologien sicherheitszertifiziert wurde.

Die Handflächenvenenbiometrie erlaubt viele Einsatzmöglichkeiten, sowohl in der Erhöhung von Identitätssicherheit im bargeldlosen Zahlungsverkehr, bei Zutrittskontrollen in Gebäuden und sicherheitsbedingten Anlagen und zunehmend auch in der ITNetzwerksicherheit von Unternehmen. Zusätzlich ersetzt sie Passwörter, deren Verlust oder Vergessen erhebliche Kosten und Zeitverlust verursachen und bietet einen besonderen Schutz gegen unbefugten Zugriff auf Daten und Datenbänke.

Zu den hervorragenden Eigenschaften der Handflächenvenenbiometrie zählen Fälschungssicherheit, kontaktfreie und daher hygienische Anwendung wie auch ihre unkomplizierte Nutzbarkeit für jeden Menschen - unabhängig von Alter oder Geschlecht. Selbst oberflächliche Hautverletzungen oder schmutzige Hände lassen eine uneingeschränkte Nutzung zu. Kann ein Fingerabdruck kopiert werden – da sein Muster auf der Hautoberfläche liegt – eine Gesichtserkennung überlistet, wie auch eine Iriserkennung nachgebildet werden, so ist dies bei der Handflächenvenenbiometrie – trotz zahlreicher Versuche – bis heute noch nicht gelungen.

Argumente, die letztendlich auch den amerikanischen Gesundheitssektor beeindruckten. Patienten monierten, dass sie sich zunehmend durch mangelhafte Geheimhaltung ihrer Patientendaten kompromittiert fühlten. Daraufhin empfahl der amerikanische Kongress die Einführung eines Eletronic Medical Record Systems.

Dabei gewährleistet die amerikanische Regierung sowohl die Sicherheit dieses Systems wie auch den Schutz der Patientendaten.

Nachdem alle erforderlichen Maßnahmen den US Health Insurance Portability and Accountability Act (HIPAA) Gesetzesgebungen entsprachen, wurde das medizinische Personal angewiesen, eine

2-Faktor-Prüfung zur einwandfreien Identitätsbestimmung von Patienten durchzuführen, sobald Zugriffe auf Patientendaten wie auch auf Kosten-/ und Kassenabrechnungs- unterlagen angefordert werden.

Das amerikanische Carolina Healthcare Project ist eine der ersten Initiativen im US Gesundheitswesen, das die Handflächenvenenerkennung im Rahmen der 2-Faktor Identitätsprüfung einsetzt. Hier identifiziert sich der Patient mittels einer Gesundheitskarte und seinem Handflächenvenenmuster, bevor der Betroffene in das Sprech- oder Behandlungszimmer zum Termin zugelassen wird. Zwischenzeitlich adaptieren auch andere US Staaten diese Sicherheitskombination. Mit diesem erhöhten Sicherheitseinsatz können nicht nur finanzielle Schäden durch eventuelle Behandlungserschleichung von Patient, Arzt und Krankenversicherung abgewendet, sondern auch die Neuaufnahmen von Patienten und deren Überweisungen zu Fachärzten und erforderlichen Spezialisten vereinfacht werden.

Somit bietet die Handflächenvenenbiometrie, als Beispiel, in Kombination mit einer entsprechenden Identifikationskarte auch innerhalb eines medizinischen Netzwerks von Arzt-/Zahnarzt- und Heilpraxen, Krankenhäusern, Krankenkassen, Apotheken, pharmazeutische Unternehmen, Homecare-Service Anbietern und Rehabilitationszentren, eine äußerst sichere, effiziente und praktische Methode dem Identitätsdiebstahl einen Riegel vorzuschieben – ohne eine zusätzlich erforderliche PIN. ■

Thomas Bengs ist seit 1991 bei der Firma Fujitsu Deutschland Gmbh beschäftigt. Seine Aufgabengebiete waren zunächst der Technische Support für große Magnetbandspeichersysteme, Festplatten und Magneto Optische Speichersysteme. Für letztere war er von 2000 - 2005 als Produktmanager EMEA verantwortlich. 2005 übernahm er zunächst die Position des Produktmanagers EMEA für New Tecchnology Products, im speziellen für Biometrie. Seit 2006 ist er als Senior Account Manager für das Business Development und den Vertrieb der PalmSecure (biometrischer Sensor) in EMEA verantwortlich.

Herr Bengs ist im Umfeld des Marktes für IT-Sicherheitslösungen sehr anerkannt. Daten und Befunde in einer Stunde am Patientenbett Obligatorisches Ziel oder fortwährende Herausforderung?

Alle wichtigen Behandlungsdaten des Patienten frühestmöglich zur Verfügung zu haben ist eine Notwendigkeit für jeden Arzt und eines der postulierten Ziele des AFH. Was nach einer Selbstverständlichkeit klingen mag, entpuppt sich im klinischen Alltag als eine Anstrengung, für die nicht nur im IT-Department und auf den Stationsfluren alle Räder nahtlos ineinander greifen müssen.

In den Asklepios-Kliniken ist, wie in vielen deutschen Krankenhäusern, die Anzahl der in den verschiedenen Abteilungen eingesetzten Softwaresysteme nicht selten dreistellig. Es ist nicht verwunderlich, dass all diese Abteilungslösungen nicht miteinander "sprechen" wollen, und dass es auch im Jahr 2009 zum Beispiel noch keine Selbstverständlichkeit ist, dass ein spezialisiertes Pathologiesystem zumindest rudimentäre Patientendaten erhält. Das AFH mit all seinen Partnern ist sich bewusst, dass nur die Konzentration auf wenige Systeme mit maximaler funktionaler Durchdringung eine Reduktion kosten- und zeitintensiver Kommunikationsstrecken bewirken kann.

Doch Integration von Software ist nicht allein die Lösung, um alle gewonnenen Informationen in immer schnelleren Zyklen an das Patientenbett zu bringen. So steigern sich stetig die Ansprüche an alle Beteiligten: • Alle Stationen sollen über drahtlose Netzwerke ausgeleuchtet sein, um so eine digitale Visite überhaupt erst zu ermöglichen • Biometrische Authentifizierungstechnologie soll Zugänge zu Gebäuden und Anmeldungen an Software sicherer und schneller machen • Geschäftsprozesse sollen über serviceorientierte Architekturen (SOA) orchestriert werden • Klinische Prozesse sollen über Workflow- und Rulesengines getrieben sein • Einweisende Ärzte und Medizinische Versorgungszentren sollen auf dezidierte Terminressourcen oder relevante Teilmengen der Patientendaten Zugriff haben

Und all das in einer "Out-of-the-box-Lösung", deren Implementierung und Wartung einen minimalen administrativen Aufwand erfordert.

Die conhIT ist der alljährliche Marktplatz für Health-Professionals. Das AFH ist erstmalig mit einem eigenen Stand vertreten. Hier werden gemeinsam mit Partnern entwickelte Lösungen präsentiert, die diese Anforderungen erfüllen. Das Asklepios Future Hospital erprobt täglich die Verbesserungspotentiale, die wir alle dringend in unseren Krankenhäusern heben müssen.

So authentifizieren sich Anwender mittels Venenscan am eingesetzten Krankenhausinformationssystem i.s.h.med – ein schneller Benutzerwechsel, äußerst sicher und auch mit einem OP-Handschuh durchführbar. Patienten tragen Barcodes, RFID’s oder ultrasound Armbänder am Handgelenk und sind damit eindeutig identifizierbar. Aber auch durch naheliegende Maßnahmen wie ein in die elektronische Akte eingebundenes Patientenfoto lassen sich schon Medikationsfehler deutlich reduzieren. Beim Eintreffen eines pathologischen Befundwertes besteht die Möglichkeit, den zuständigen Arzt per bidirektionalem Messagingdienst zu informieren – und dieser kann somit die ersten notwendigen Maßnahmen ad hoc einleiten.

Die Visite wird digital am Bett mit Mobile Clinical Assistants durchgeführt, welche die volle Funktionalität des KIS bieten und ggf. eine Wunddokumentation per Foto ermöglichen. Sollte eine OP-Anmeldung notwendig sein, wird sie on demand in den Tagesplan des zentralen Koordinators eingetaktet und sollte der Eingriff mit einem anderen Termin des Patienten kollidieren, wird u.a. die Stationspflege vom System informiert.

Es darf nicht die alltägliche Realität bleiben, dass ein Arzt für eine Konsilanforderung in die Schublade greift, um seinen Kollegen auf einem (hoffentlich noch aktuellen) Vordruck um eine fachärztliche Einschätzung zu bitten. Dieser Prozess wurde im AFH mit der Unterstützung seiner Partner deutlich vereinfacht. Diese Daten nähren sich aus Modalitäten, bildgebenden Verfahren, aus der klinischen Chemie, aus freitextlichen Befunden, strukturierten Dokumentationen aus 3rd-Party-Produkten, Anamnesen des KAS, mitgebrachten Vorbefunden des Patienten, archivierten Altakten aus Voraufenthalten und noch sehr vielem mehr.

Die elektronische Patientenakte ist somit weitaus mehr als das digitalisierte Sammelbecken aller dokumentierten Daten. Sie ist eine klinische Übersicht, in der für den Behandlungszusammenhang relevante Befunde, Labordaten, Medikationen und Vitalparameter in strukturierter Form dargestellt werden.

Dabei soll die Aggregation der Inhalte exakt die benötigte Granularität aufweisen. So ist zum Beispiel keinem Arzt geholfen, wenn er auf dem initialen Bildschirm die Laborwerte eines längst vergangenen Voraufenthaltes des Patienten sieht, diese aber für die aktuelle klinische Fragestellung zunächst keine Relevanz haben. Das gemeinsame Ziel ist, dass der Nutzer nur die Informationen angezeigt bekommt, die für Ihn zu diesem Zeitpunkt und im Behandlungszusammenhang relevant sind. Darüber hinaus sollen seine noch zu erledigenden Aufgaben klar und eindeutig erkennbar sein.

Behandlungszusammenhang bedeutet auch, dass das Personal sich nicht auf die Suche nach Informationen und Aufgaben begibt,

sondern dass die anstehenden Tätigkeiten den Prozessbeteiligten aufgrund von medizinisch indizierten Algorithmen in der aktuellen Situation vorgeschlagen werden. Moderne Informationssysteme wie das im AFH eingesetzte i.s.h.med pushen die benötigte Information zur richtigen Zeit an die beteiligten Akteure.

Hinzu kommt der Aspekt Sicherheit, gerade im Hinblick auf die Sicherheit in der Patientenversorgung, daß das, was ich als medizinisch notwendig erachte und anordne auch so umgesetzt wird. stellen innerhalb dieses Prozesses: Arzt, Pflegepersonal, Labor.

Dabei gibt es immer wieder einzelne Fehlerquellen, und über die ständige Online-Verfügbarkeit der Daten kann ich so überprüfen ob das, was ich iniziert habe auch wirklich stattgefunden hat.“ „Als Neurologe ist für uns ein wesentlicher Bestandteil die Bildverarbeitung, Stichwort Computer- und Kernspintomographie.

Hier habe ich sofortigen Zugriff auf die Untersuchung und in den meisten Fällen auch auf den Befund, weil der in der Radiologie per Spracherkennung sofort in das System diktiert wird. Ich mache mir aber gerne immer eine eigene Meinung über die Röntgenbilder und verlasse mich nicht nur auf den Befund. Mit der Weiterentwicklung der i.s.h.med-Lösung, basierend auf unserer standardisierten ITInfrastruktur, habe ich auch hier bereits kurze Zeit nach der Erstellung der Röntgenaufnahme die Bilder online zur Verfügung. The Evolving Physician Elandscape in Europe How Technology is Changing the Way European Physicians Practice Medicine Meredith Abreu Ressi

From information gathering and education to patient interactions, the Internet, mobile devices, and electronic medical records paved the way for a new e-savvy physician audience who are routinely turning to sources of medical information that didn’t even exist a few years ago.

Across Europe, more than 90 % of physicians access the Internet in some capacity, the vast majority of who also use the Internet or email for professional reasons. Adoption is even higher in countries such as Germany and the United Kingdom, where Internet adoption is nearly ubiquitous.

Reliance on the Internet as a Professional Resource What is perhaps most striking is just how quickly this transformation has taken hold across Europe. In 2005, roughly two-thirds of physicians in Europe agreed that the Internet was “essential” to their professional practice. Today, more than 80 % of physicians

agree with this statement. This mirrors – almost exactly – the percent of physicians in the United States who report the Internet has had this sort of transformational impact on their practice, and illustrates the degree to which online research has become a way of life for physicians in Europe.

Using Technology at the Point of Care Physicians in Europe have even begun to catch up to their counterparts in the United States when it comes to Internet use, technology adoption and the degree to which these sources are used at the point of care.

The presence of technology at the point of care – in the form of electronic medical records, practice management systems, broadband Internet connections and mobile devices – has had a substantial impact on the degree to which these advances have changed the practice of medicine across Europe. Physician Use of Mobile Devices As smartphones, MP3 players, and other handheld devices become commonplace among physicians across Europe, these always-on tools are becoming indispensable resources for physicians during their workday. Physicians are conducting a range of activities including scheduling, email, looking up clinical information, and even accessing downloadable professional applications and resources. Mobile drug reference databases are especially popular, such as Vidal in France, Vademecum in Spain, and U.S.-based Epocrates, which is gaining popularity among physicians in the UK.

Evolving Physician Communication Preferences European physicians are clearly amenable to receiving worthwhile news and information via email. Nearly 80 % of European physicians report that they subscribe to some form of medical or professional email newsletters today, with the average physician reporting more than six newsletter subscriptions. This demonstrates the high penetration and reliance of this population on email as a form of easy information gathering.

Another growing opportunity in Europe is to communicate with physicians via SMS (text) messaging. Nearly half of European physicians report sending or receiving text messages for professional purposes, and over one-fifth report receiving SMS messages from

SMS messaging is also routinely used by physicians to communicate with office staff, colleagues, and even patients.

Eschewing Traditional Sources of Information for “E” Across the globe, the Internet is changing distribution models for information faster than companies can evolve to keep pace with consumer demand. Just as consumers consume media such as newspapers, music, and movies differently today than they did five years ago, physicians, too are changing the way they learn and gather information. In many cases, online proceedings for congresses replace the need to travel to the event itself; online journals provide the same information as their print counterparts, but in a more

searchable and accessible format; and even many textbooks are readily available in a digital format. The majority of physicians in Europe today rely on at least some of these digital sources of information, and a handful have even gone so far as to cancel a journal subscription or decline to purchase a textbook because of the availability of the same information online.

Emerging Online Content Sources The Internet, of course, has also created a crop of entirely new content resources for physicians. Some of the most popular online sources include local (country-specific) websites designed for a physician audience, such as DiarioMedico in Spain and Doctors.net in the UK. One notable exception of a site that has transcended the country divides has been the Univadis portal from MSD (Merck Sharp & Dohme). For years, Univadis has been a leading portal for physicians across Europe by providing content created specifically for the individual country, in said country’s language. The success of Univadis has not gone unnoticed, and pan-European content sources for physicians are continually emerging and gaining traction among European physicians.

In order to successfully develop content to be used by physicians across Europe, companies must devote special effort toward creating local content in the local language. Putting up one site in English and hoping to gain traction across countries is a strategy that has consistently proven to be doomed for failure. With regard to reach and frequency, as the frequency of professional activities online continues to increase year after year, the forecast is that the demand for portals from content providers will continue to grow for years to come.

European Physicians and Web 2.0 User-generated content is all the rage these days, with blogs, microblogs, wikis, and online communities capturing the attention of everyone from marketers to researchers. And this user-generated content is not just contained to teens and Generation Y – physicians across Europe are using communities, listservs, wikis and blogs as sources of medical information and professional consultation.

When it comes to online physician communities, physicians in the UK are the most active, with nearly one-fifth of online physicians reporting they use such online communities – often to discuss clinical issues with their peers. This adoption in the UK is largely due to the communities available to physicians through

More than two-thirds of online European physicians use Wikipedia as a medical resource monthly or more often – and avid users of the site for medical information are even recommending it to patients as an information resource. This underscores the need for monitoring of Wikipedia by members of the physician community, to ensure the content is accurate and up to date.

across the treatment continuum, today’s educated and empowered consumers are shifting the balance of power in the exam room.

For many physicians, this means a shift from the days of a oneway, didactic relationship with the patient, toward more of a collaborative discussion with the patient about treatment options. More than one-third of physicians across Europe are even recommending websites for patients to visit for additional condition information, patient support, and disease education.

Technology has had a profound impact on everything from how physicians in Europe research information to the way they interact with patients. Many of these shifts have occurred only within the past few years, leaving the future wide open in terms of how healthcare delivery will continue to evolve in the face of future technological advances. From virtual house calls to physicians who eschew traditional sources of information altogether in favor of using digital resources, the next few years are poised to see even more substantial changes in the way medicine is practiced in the face of these ever-changing technologies. ■

The Doctor Will See You Online care’s impact on communication between patients and physicians Roy Schoenberg

Introduction: A Healthcare System In Need of Change Through the years, Americans have trusted the healthcare system to be there. This is not to say that we weren’t aware that it was becoming increasingly expensive, got some bad publicity in the news, made employers cry for help, or gobbled up gross domestic product.

But it was always “there.“ Even this may be about to change.

The US healthcare system is now at a precipice. Despite its wealth of medical talent and innovation, it increasingly fails to deliver on the most fundamental service: enabling anyone with a health concern to obtain quality, affordable and timely care from a physician. The numbers tell the tale: nearly 46 million people are uninsured and another 25 million underinsured.

The fundamental problem is a growing mismatch between supply of, and demand for, healthcare services. On one hand, patient demand has increased dramatically during the past decade.

Americans’ health needs have become more intense and complex, fueled by a growing burden of chronic illness and an aging population. By 2023, the Milken Institute projects a 42 percent increase in prevalence of seven common chronic diseases. In parallel, innovation in new treatments, medications and procedures results in more costly, complex care. Demand will only continue to grow as efforts take off to expand coverage via universal healthcare or other government programs.

At the same time, the supply of primary care physicians (PCPs) is shrinking. Physicians burdened by paperwork, malpractice costs and daunting lifestyles are starting to bail out. The numbers are frightening. The percentage of medical school students selecting residency in primary care specialties is shrinking – from 53 % in 1998 to just 15 % in 2006. A 2008 Health Affairs article projects, “By 2025, the nation will be short 35,000 to 44,000 adult care generalists practicing family medicine and general internal medicine.”

Reconnecting Patients and Physicians: The Role of Web-Based Technologies There is clearly a need to restore the balance. Unfortunately, it’s hard to see how thousands of healthcare professionals, most requiring many years of training, will suddenly materialize. It is also un-

likely that people will need less care from physicians. From this point of gridlock, creative solutions are emerging.

Several solutions aim to make existing providers more accessible. Minute Clinic employs nurses and nurse practitioners to allow patients to receive select care at their neighborhood pharmacies. If you call a service offered by TelaDoc with a health question, they will find a doctor to call you back. RelayHealth will facilitate answers to your questions through secure email, while OrganizedWisdom will help you with professional advice. Although these services differ, all represent innovations in giving patients the access to timely healthcare services – access they increasingly lack.

Other approaches employ more advanced technologies to improve access to healthcare information – especially using Webbased technologies. For years, services such as WebMD, Patients LikeMe and Sermo have used the Internet to distribute health information to patients and establish communities. Meanwhile, personal health records, such as Microsoft HealthVault, have improved patients’ ability to store and manage their health information.

Increasingly, however, both patients and providers want to use the Internet to do more. Consulting firm Deloitte’s 2009 Survey of Healthcare Consumers revealed that 57 % of American consumers want a secure Internet site to access medical records, schedule appointments, refill prescriptions and pay medical bills. Another 55 % want e-mail access to their physicians. In parallel, a 2006 Gartner study reported that 85 % of physicians were looking to streamline their operations using the Internet, and 90 % of providers saw the Internet as a way to gain competitive advantage.

This desire is not surprising when one considers the way in which the Internet has changed most patients’ expectations. Today, consumers can go online to manage their financial portfolios (E*Trade), plan travel (Expedia), bank (BankofAmerica.com), purchase or sell goods and services (eBay, Amazon), and even renew their driver’s licenses. These tools share several key attributes: they

are available wherever consumers are, offer choices for shopping around, and provide immediate service. Most importantly, these tools focus on transactions. Thanks to the Internet, interactions with these companies are characterized by convenient access, transparency, responsiveness and informed service.

Patient-Physician Communication in the Online Care Model Enter Online Care. Online Care brings these same tenets to healthcare by offering patients the ability to come together with physicians online and receive immediate healthcare services. In the past, patients relied on Web-based technologies such as portals to obtain and manage information about their health. With Online Care, they receive actual healthcare services directly from physicians, without scheduling an appointment or leaving their homes.

In the US, Online Care is offered through health plans – the keystones that bring together patients and providers for care delivery.

When a health plan offers Online Care, physicians in its existing network can make themselves available for online and phone consultations at any time, from any location, and for as long as they choose. An Online Care system pools these physicians according to specialty and brokers their interactions. As a result, patients seeking care can log into their health plan’s Web site, then find and immediately connect with the primary care or specialist physician who is right for their needs (see Figure 1). They can also search for a physician who fits certain criteria that are important to them and choose based on the results.

During the Online Care visit itself, patients can see and talk to their physician while sharing notes on screen (see Figure 2). The physician can answer questions and, if necessary, write prescriptions or refer the consumer to another physician for a specific concern or second opinion. Importantly, the Online Care interaction is a highly informed one. Physicians have real-time access to the wealth of patient clinical data captured by the health plan, if the patient agrees to share it. This includes, for example, claims infor-

mation, pharmacy, labs, predictive modeling and even care management. Personal health information that resides in Microsoft HealthVault may also be shared.

When the visit concludes, the co-pay is charged to the patient’s credit card, and a claim is processed automatically through the health plan. The patient can share a record of the conversation with his or her primary care physician, ensuring the continuity of care.

All records are stored for reference at any time – and patients retain control over that information.

While the technology is advanced, its goal is simple in that it confronts the gap between healthcare supply and demand headon. Instead of diverting patients to alternative care modalities, Online Care “stretches” the existing healthcare system, so it can cover more ground. On one hand, it makes it easier and quicker for patients to get in front of the same providers they would otherwise schedule appointments with, enabling greater convenience and responsiveness. It does this while upholding care continuity and reinforcing the principles of the medical home.

On the other hand, it makes it much easier for credentialed physicians to offer their time to patients, capturing more “care opportunities” from the same “supply”. But it does this while enabling these physicians to take greater control over their hectic lifestyles, because they are able to deliver care from anywhere they choose – without assuming any administrative overhead.

In this way, Online Care is the first technology that aims to improve healthcare not by distributing medical records, but by distributing actual medical services. It uses the strength of Web-based technology to make this happen in a more efficient and satisfying manner for patients and providers alike.

Online Care in 2009 – A Year of Accelerating Change How fast will Online Care become a universal means of healthcare service delivery in the US? The process is already well underway.

Last year, Lynne A. Dunbrack, program director, Health Industry Insights, an IDC company, predicted that 2009 would prove a pivotal time, saying: “A decade ago, e-commerce allowed consumers to buy medical goods and refill prescriptions; with the launch of Online Care, consumers can now obtain actual healthcare services from their homes. We expect health plans to adopt new technologies in this space over the next 12 to 24 months.”

The first state to offer Online Care was Hawaii, which has a tradition of healthcare leadership. Blue Cross Blue Shield of Hawaii (HMSA) launched Online Care to all residents of the state (1.3 million people) on January 15, 2009 – including its own 800,000 members. HMSA’s full network of physicians is now able to offer services through HMSA’s Online Care. The service is made available to uninsured residents as well.

Several other states will bring Online Care to their residents in 2009, a move which consumers have reacted favorably to. In a recent survey conducted by market research leader TNS Global with American Well, more than 70 percent said they will use Online Care when offered the option. The ability to access physicians instantly drives demand, with nearly half of consumers saying they’d use Online Care to speak with a physician before heading to the ER or scheduling an office visit. The momentum behind Online Care is further accelerated by the arrival of the new administration in Washington. Many in government and industry see IT as a powerful tool to increase access to healthcare services, strengthen physician–patient relationships, and ultimately improve patient outcomes.

In Europe, healthcare professionals may note that pervasive use of Online Care is, in many ways, even simpler to achieve than in the US. The main reason is the greater centralization of healthcare delivery (e.g., the UK’s NHS). Centrality only increases the utility of Online Care – because it is easier to collect and bring together a critical mass of providers and patients, all through a single claims clearinghouse. So, while the US implements Online Care on a state-bystate basis today, we expect to see Europe embrace it on a national level. The need to enlist the latest technologies to improve access to quality, timely healthcare – while actually strengthening the relationships between physicians and patients – is equally urgent on both sides of the Atlantic. ■

• Physicians as knowledge workers – professionals getting the right data in the right format at the right time to provide the best treatment and preventive care.

• New interactions among the key members of the healthcare ecosystem – physicians, patients, pharmacies, researchers, and insurance providers benefiting from a new flow of data to make better, faster decisions.

We all know that information technology is a vital component in improving the healthcare. But simply spending more money on information technology, without considering all the factors driving behavior across the system, is unlikely to lead to better health outcomes. There have been many investments in technology that did

not solve the problems of better quality outcomes, increased access, or reduced costs.

If we think broadly about successful institutions that have invested in and adopted technology, they typically have done so as a means to achieving some institutional goal, not adopting the technology as a goal unto itself. For example, U.S. retail giant Wal-Mart adopted supply chain IT because it enabled a reduction of inventory costs and improved product availability to customers, providing a sustainable competitive advantage and value proposition – low prices and convenience. Likewise, American Airlines adopted computer based yield management software because it allowed them to make more money per airplane segment.

We have to look at Health Information Technology (HIT) the way other successful institutions have – as a means to an end, not an end unto itself. The real lesson of implementing HIT successfully is to focus on the goals first and foremost, and pick the right technology investments to accomplish them. Within industry today, there are many organizations that have been successful in using technology to achieve specific goals. These are organizations whose leadership thought about clear outcomes and embraced technology on many different levels to drive improved efficiency, quality, and a reduction of costs across their systems. In essence, they created patient-centric systems. We believe that these are the kinds of successes that need to be scaled broadly – we should expand on these successes and embrace their core ideals by doing the following: First, We Must Drive the Right Health Outcomes and Payments to Incent Innovation. An industry focused on lifelong wellness and outcomes would reward caregivers when diseases and conditions do not develop.

This problem exist today in the current U.S. healthcare system.

It is one designed to care for people who are ill, not to keep people healthy. For example, we focus on providing episodic treatment and medication to diabetics instead of asking how we can raise aware-

ness of diabetes risk factors and prevent people from developing diabetes in the first place. The system is this way because we do not reward doctors who provide preventive care or innovative services.

Doctors typically receive a flat fee for each treatment they perform, regardless of the quality of the care, and the amount of the fee is set by a bureaucracy of insurers, health plans, and regulators. In this fixed-price system, there is no incentive for providers to improve customer satisfaction. Most physicians are not reimbursed for telephone or email consultations, let alone more advanced uses of technology. Doctors who attempt to innovate – for example, by investing in systems to collect data from patients remotely – end up delivering better care but making less money.

In other health-related areas where prices are set by the market, such as veterinary medicine, dentistry, and cosmetic surgery, providers do a much better job of investing in services that attract customers. For instance, pet owners willingly pay for veterinarians who make house calls, maintain electronic medical records, remind owners to bring their pets in for scheduled vaccinations, call to make sure the pets are taking their pills, and are available for email or telephone consultations. Veterinarians compete on price and quality, so they are constantly looking for innovations that allow them to provide better service and improve customer satisfaction.

Because technology is often a source of innovation, veterinarians are quick to embrace new technologies that fuel better service and better patient care. We need to learn from these examples.

Second, We Must Connect and Share Data Securely Within and Between Health Organizations. We have to recognize that health is on one dimension fundamentally a data problem. Think for a moment about how powerful it would be to have a lifetime record of all tests, prescriptions, and treatments received as well as preventative measures and the responses to treatments help guide future decisions – for consumers and physicians as well as hospitals and health researchers.

Many health systems in countries around the world are built around the idea of a specific provider prescribing specific treatment for a specific condition. Patients’ health data is locked inside each provider’s silo, without being connected or shared. Physicians are forced to either make treatment and prescription decisions without all available clinical data, or else waste time and resources attempting to aggregate data. MedStar Health’s Washington Hospital Center, a community-based network of eight hospitals and other healthcare services in Maryland and the Washington, D.C. region,estimates that 60 % of a clinician’s time is spent searching or waiting for information, with only 16 % spent on direct patient care.2 The right investments in HIT can tear down these silos, offering patients and doctors a holistic picture of a patient’s health history and thereby improving care. Consider chronic diseases, which account for over 75 % of healthcare spending.3 Even though most care for chronic diseases occurs at home, data from at-home care is not integrated with data available at the hospital or at the doctor’s office. Individuals and providers would all benefit if, for example, patients with diabetes could upload their blood glucose readings to a Web site that offered personalized advice and guidance; receive information alerts regarding changes in recommended treatment or behavior; share their results with a supportive community of fellow patients; and securely transmit readings to their clinician. Patients would have more information on managing their condition, would be in a better position to prevent acute incidents, and would need to make fewer trips to the doctor. Treating physicians would have a greater ability to understand their patients’ health over time, allowing them to identify the best treatment for existing patients and to help people who are at risk of developing the disease in the future.

Third, We Must Empower Consumers to Be Stewards of Their Own Health Data. It is often said that people only think about their health when they’re ill or have a chronic disease. Nothing could be farther from the truth. In actuality, most people take actions to optimize some aspect of their health every single day.

How many people step on a scale regularly? How many adjust their fitness regimen based on a target heart rate? How many people count calories or follow a diet of any kind? How many read the ingredients on food labels in order to avoid something they don’t wish to ingest? How many know their waist size, height, and other body measurements, and use that data to guide clothing purchases?

How many have performed a home pregnancy test or an ovulatory cycle test? How many have used a home test for occult blood in the

stool? How many have checked their own blood pressure? How many take a daily vitamin? People engage with their health every day, even it is at a basic level.

That said, in order to manage their health, consumers today must deal with both paper documents and electronic files. They fill out form after form, calling multiple doctors’ offices for appointments. Few people have the resources to keep track of medication lists, vaccination histories, appointment calendars, lab results, diet plans, exercise schedules, and all the other components of health data. Many have little knowledge of how to prevent disease and little, if any, support for managing their healthcare.

We need to empower consumers to manage their health data.

We must help consumers to start building their health data into a lifelong asset, to manage it over time, and to share with those who support them in making key decisions both within and outside of the health system.

With the consumer empowered now imagine how they could connect all their health and wellness data electronically, share it securely from provider to provider, and keep it in one place over time, no matter the doctor or the insurance company. They would have all the relevant data at their fingertips, accessible at any time and any place. They could sign up for services that would provide personalized alerts and information. They could track fitness goals across numerous devices, such as exercise bikes that monitor vital signs, smart watches that record the number of miles run, and scales that measure body fat as well as weight. They could research relevant medical conditions online and interact with support groups so that they would be better prepared and informed for their next visit to the doctor. They could share data with their support systems and make better health decisions for themselves and their families.

We believe technology can make this vision a reality. The Internet and online social networks have already become an everyday resource for consumers seeking information in order to make health decisions, but what is missing is a way to link this information back to the individual’s personal health history. And consumers are ready for it: • 78 % of Americans favor giving doctors the ability to share access to their medical records if done with their permission.4 • 66 % see value in including their own information anonymously in a large database to help researchers.5

MICROSOFT

Providing consumers with access to their healthcare data in a secure and private way, and allowing them to keep it in one place over time and share it from provider to provider, will permit them to make the best daily decisions about their health. It also will enable healthcare professionals to deliver better care.

To enable this connected, patient-centric system, the public and private sectors need to take several key steps, including: Encourage innovation in health IT by setting objective goals and criteria, not by mandating specific technologies or development models. We must remember that technology solutions are a means to an end. The focus should be on setting broad health outcome objectives – for example, increasing compliance in hypertensive and diabetic population up to 90 % by 2012 – and then applying technology to achieve those goals. Hundreds of innovative health IT products and services are available on the market today, and many companies are investing large sums to develop new technologies and solutions. Even as they compete, however, companies are collaborating to enable their products to work together and share information regardless of their underlying development, licensing, or business models.

Reward innovative doctors who make the Internet the foundation of the patient-physician connection. The Internet has created a society that demands access to and up-to-date information.

Patients need information about their conditions, appropriate treatments, pre-procedure instructions, and post-visit follow-ups. The Internet is the most efficient way for doctors to provide this trusted information. But the fixed-price nature of reimbursement means that innovative doctors have no incentive to deliver this kind of service. Physicians should be encouraged to embrace Internet technologies that allow them to communicate more effectively and consistently with patients.

Provide incentives for sharing data. We believe that it is critical to seamlessly and securely connect data and empower individuals to take control of their health. We hope that those in the public sector will facilitate the transformation of health data into a vital asset by removing barriers to data sharing and providing incentives for data exchanges that reduce costs, increase value, and improve the quality. To facilitate this sharing of data, a foundation of trust must also be established – health data is highly sensitive, so consumers and healthcare providers will only share it if they trust that the privacy of the data will be protected.

Focus on making data interoperable today, not waiting for standards, and insist that vendors separate data from applications. Today, data is too often used for a single application or purpose, then thrown away once that purpose is complete. We can use metadata – the details that describe the data and how it has been captured – to ensure that data is kept alive and made available for reuse, no matter what its original purpose. By insisting that vendors supply IT that allows data transfers to and from other non-vendor applications, we can get data moving between different systems today, without waiting for standards that may take years to complete.

Better use of metadata will pave the way for integrating legacy data with standards-based data once these standards are more widely adopted.

Enable the private sector to develop an information infrastructure that connects data, systems, and people. To move from today’s fragmented system to tomorrow’s connected network, we need a technology infrastructure that allows data to flow freely throughout the system and be reused. Without it, we’ll recreate our disconnected paper system in the virtual space. This infrastructure must be (1) flexible, to enable many different players across the ecosystem to do what they need to do; (2) interoperable, to leverage existing standards and infrastructure investments; (3) scalable, to adapt to the rate of medical and technology advances; and (4) secure and private, to foster consumer trust.

To make sense of Health 2.0 you need to understand what Web 2.0 is. Some time in the early years of this decade a number of technology industry commentators, led by Tim O’Reilly got to discussing what had happened in Internet technology. There was a huge boom in the late 1990s followed by a crash that hit Silicon Valley very hard in 2001-3. But underneath the financial meltdown, the Internet was changing.

It was no longer a medium just for publishing. Changes in software – particularly the ability of web browsers to perform more functions without having to make a “call” to a server, and the spread of Broadband and WiFi – meant that more functions could be done more easily on the web. It was also becoming easier to share information, and soon the development of blogging and wiki tools meant that everyone could become a publisher. Similarly the development of early social networks like Tribe & Friendster, meant that anyone could create a profile and an online network.

Then adding video got easier, and soon emerged the giants like MySpace, YouTube, FaceBook and especially Google. Anyone can write, upload video, meet people, share experiences and opinions and use the web to share. One common factor is that users generate the content (UGC) which is making life much more complex for traditional content creators like newspapers. Communities can be simply created and transactions of all kinds have become much easier.

That plethera of tools and experiences was dubbed “Web 2.0” in 2004. Health care is a huge content area for the Internet, and by 2006 several health specific search, community and tool sites began to emerge. That phenomenon was dubbed “Health 2.0”. The movement crystallized at a conference held in San Francisco in September 2007, and the proportion of web users seeking to find information that was created by other users rather than expert medical sources went from a handful to over 35 % by the end of 2008.

How is Health 2.0 changing the way consumers and the healthcare industry collaborate?

Health 2.0 has the potential to fundamentally change the typical doctor-patient relationship. Consumers using health specific communities are able to receive information from thousands of people (known as the “wisdom of crowds” or “crowdsourcing”). Still other tools help them see aggregated opinions, such as the effectiveness of particular drugs or physicians. Additionally, several patient sites reveal information about latest treatments for specific rare diseases that the patients’ physicians may well not know.

It’s far too soon to say that Health 2.0 is having a major impact on every patient, doctor, or health care industry participant, but it is clear that 20-30 % of patients in the US are consulting the Internet to confirm or check on what they are hearing from their physicians, and looking widely for more information.

A small minority are already receiving second opinions or even direct medical advice and services online. But thus far it’s been “information first, transactions later.”

What’s the value for consumers from Health 2.0?

So far the value for health consumers falls in two main areas. First, better information about what particular health issues mean for them. For example, the information generated by a specific site such as PatientsLikeMe (which specializes in ALS among others) has meant that patients have been able to see the experience of other patients, and adjust the amount and type of drugs they are taking to improve their therapies. Several physicians and researchers are using PatientsLikeMe and other sites as virtual Phase IV clinical trials to improve the care options they offer their patients.

These examples are particularly acute in the case of very sick patients such as those with cancer or with severe chronic diseases, but the recent improvement in search tools from sites like

HealthLine, RightHealth and DoubleCheckMD are meaning there is now very easy access to very detailed information for all health consumers – and that information is increasingly personalized to them by the importation of data from other systems, such as insurer or drug information stored in Google Health or Microsoft HealthVault.

The other main benefit is the incredible support that is offered to patients in online communities. Many studies have shown that real-life support groups help patients both emotionally and with their clinical outcomes, and the easy access to large number of fellow-travelers in online disease communities seems to be having a very similar impact. Certainly the conveners of communities like DiabeticConnect, TuDiabetes and dLife – to name just three just for diabetics alone – can show you hundreds of incredibly complimentary comments from their very loyal users who spend considerable amounts of time on their sites, and regard it as an essential part of their lives.

Is the European market ready for Health 2.0?

Does Europe have Internet access, preferably by broadband or 3G mobile? Yes. So Europe is ready for Health 2.0 as it now stands.

Several European countries also have the advantage of being far ahead of the US in electronic medical record use by physicians, which means that it’s easier for them to create connections between local physicians and local patients than it’s been in the US. The only potential disadvantages are that Europe doesn’t quite have the entrepreneurial online tradition of those cavalier Americans. The question is how government or non-profit insurance funds and health care providers will try to use Health 2.0 in Europe.

So how do we transform the – ”healthcare patient from today” to the ”healthcare consumer of the future”?

Much of this change is already happening as people get used to going online or using their mobile phone for all kinds of nonhealthcare transactions. Eventually they’ll start wondering why they can’t do the same thing with their health care issues, and will naturally start demanding these services.

The health care industry can support this by encouraging patient education, helping physicians and other clinicians advise and direct patients to useful online sites, and making sure that it’s not trying to stymie progress because of fears of losing control and/or money.

What should the European health care industry do?

If you’re in an organization that hasn’t already started to use Web 2.0 tools, start now. If you haven’t spent much time studying the phenomenon, start now.

If you’re a little more advanced, start building relationships online with European leaders in Health 2.0. You’ll find them on blogs, on Twitter, in the Linkedin Health 2.0 Group, in the Facebook Health 2.0 group, or contact me at matthew@health2con.com. User Interface Innovation Dr. Ron Ribitzky, M.D.

In 2007, not one smartphone appeared in the top 10 devices used for mobile downloads. In 2008, 6 out of 10 are smartphones. Overall, the audience for downloaded mobile games grew 17 percent year-on-year.

During the 3-month period that ended November 2008, game downloads on smartphones jumped 291 % year-on-year to 2.9M. Game downloads on non-smartphones dropped 14 % to 5.6M. In November 2008, 32 % of iPhone users said that they downloaded a game, exceeding market average by 8X.1 The video games market is the third fastest-growing segment of the entertainment and media market after TV distribution and Internet advertising and access spending.2 What is in video games for the future of the hospital market?

Myca’s advertisement campaign for its Health 2.0 platform that powers the innovative HelloHealth illustrates this fundamental wisdom.

On the other hand, some 40 years after Electronic Medical Record (EMR) systems first came to life at the Massachusetts General Hospital in Boston, the worldwide market penetration of EMR, as measured by EMR systems that are in the process of implementation or are already in production use, is hardly 25 %.3 While certain European countries lead with a remarkable 60-80 % adoption of EMR, less than 20 % of small physician-offices treating about 50% of patients in the U.S. have some form of electronic records.4 Yet having a system in place does not necessarily mean that the intended users actually use it. In fact, clinicians at large are typically ‘last in line’ to use information systems on a routine basis in the process of taking care of their patients.

The inevitable questions are why? how come? and what can we do about it?

Usability is #1 barrier to adoption of hospital information systems A rather broad concept with far reaching implications, usability, or lack thereof, directly impacts the bottom line: Value of I.T. and Return On Investment (ROI) to hospitals as well as hospital information system vendors.

To hospitals, the far-reaching implications of usability include quality of care, patient safety, and workflow optimization. To vendors of hospital information systems, usability affects architectural design decisions, interoperability, and competitive market position.

User Interface (UI) is one’s very first encounter with usability.

For our purpose here, we will extend the scope of discussion about UI to include the user’s experience. And so we will address the role of, and interplay among ‘Presentation’, ‘Navigation’, and ‘Workflow’.

In sharp contrast to the UI of video games, the typical presentation of a contemporary hospital information system immediately after log-in looks a lot like a data entry form, or a structured report.

A couple of clicks away the clinician is often presented with a crowded collection of data-entry look-alike forms and structured reports. Navigating within and among the screens is rigidly confined to pre-programmed structures: windows, tabs, pull-down menus, etc.

As unexciting as it may appear in comparison to a video game, such UI may be a workable approach to support repetitive and highly predictable tasks throughout pre-defined workflows that have finite alternative paths.

UI design of a hospital information system is a formidable challenge Here is why: your task is to deliver a compelling user experience to some 200 different medical specialties, some taking care of patients in more than one venue, where each patient probably suffers from 2 or more co-existing medical problems out of potentially some 15,000 coded diagnoses (as in the ICD system).

When it is time to order a diagnostic or a therapeutic procedure, it should be done in accordance with currently validated evidencebased disease-management protocols and clinical pathways, drawing from around 10,000 coded procedures (as in the CPT system), hundreds of medical devices, and thousands of medications which may or may not be represented in the Orders Catalogue of the Computerized Physician Order Entry (CPOE) system.

Then, you still want to allow thousands of physicians and nurses to personalize their user experience with your system. Finally, most hospitals have dozens of different systems, each covering a ‘fragment’ of the complete (’end-to-end’) patient care workflow.

Your challenge is not over until the UI of your system is interoperable with the others.

Healthcare organizations and industry team up to develop standard UI solutions In 2005, thought leading National Health System (NHS) in the UK and market leader Microsoft teamed up to standardize the user interface across the handful of core clinical systems which make up the nation-wide Connecting For Health system.

The Common User Interface (CUI) program now offers a growing collection of standard ‘UI Controls’ which are designed for clinical application.5,6 CUI is part of Microsoft’s Connected Health Framework that Asklepios Future Hospital helped define during the predecessor eHIP project.

Such pre-developed small software constructs allow developers to ‘assemble’ clinical applications faster and at a lower cost. To the hospitals of the future, CUI-like solutions offer the opportunity to standardize their own UI across the dozens of diverse applications that typically make up a hospital information system, thereby reducing barriers to usability, driving up adoption, optimizing clinicians’ workflow, and increasing effectiveness of patient care.

Necessary and highly valuable nevertheless, CUI-like approaches offer incremental improvements in UI for hospital information systems. Yet the underpinning of the design of these systems, from the database to business logic and workflow continues to be the presumption of certainty.

The practice of Medicine is inherently uncertain “It was not possible for a perceptive man to study medicine in the early nineteenth century without becoming aware of the degree to which patient care was conducted in a pervasive atmosphere of inexactness…” 7 Ironically, the more we know about the translational foundation of disease processes, prevention, and intervention, and the more evidence someone discovers about the correlation between a medical condition, clinical pathway and outcomes, the harder it seems to make that knowledge actionable within a hospital information system during an encounter between a physician and her patient.

Case In Point: Breast Cancer An elusive knowledge gap between life sciences research and discovery and patient care (‘Bench to Bed’) continues to impact the UI design of hospital information systems. Breast Cancer illustrates that point.

Classification and staging of cancers is the foundation for making treatment decisions, developing care plans, and predicting prognosis. Information systems are relied upon to help the clinical care team make these decisions and manage the care process.

Yet in recent years it became apparent that the prior classification of cancers in general, and Breast Cancer in particular, was flawed.

We now know that Breast Cancer is an evolving, multi-stage heterogeneous disease; and that about a 1/3 of the classification of Breast Cancer is driven by genomic factors and the remaining 2/3 on environmental factors.

Thus in the hospital of the future Gene signature, molecular classification, histological information, and clinical manifestations of a patient would instantaneously be examined by evidence-based staging and prognostic models, matched to clinical pathways and mapped to therapeutic agents and targets to optimize the plan of care and monitoring of measurable outcomes.

The discovery, assimilation, and application of new knowledge one patient at a time, one encounter at a time, happen simultaneously in different disciplines by teams and individuals around the world, using a vast array of information systems, databases, knowledge bases, and analytics.

Call to Action: Disruptive UI innovation Facing more complex information and knowledge management environments going forward in light of worrisome low adoption of information systems today, what would make Primary Care Physicians and Oncologists crave the information system of the future hospital?

The inevitable answer is a compelling UI that would meet the following architectural design principles: • User Experience that is fun and which will factor in the inherent uncertainty of the practice of medicine and the exponential cycles of care-flow and discovery.

• UI components that will support interoperable standards for presentation, navigation, and workflow throughout diverse systems and across organizational boundaries.

• It would be downloadable onto and usable from mobile internet devices like one can do with video games.

Global trends of information technology clearly indicate that we are at an inflection point. Breakthrough innovation springs virtually at every level of the ‘architecture stack’, driving convergence of hardware and software platforms, new usage models, and opportunities for cross-industry pollination.

“Gaming interfaces might be the key to the future of health records” 8 A case in point is a patented system and method that I invented (code name DBI).9 It illustrates the kind of foundation technology that can materialize the ultimate UI vision for information systems of the future hospital.

A starting point for a DBI application may be the software representation of ‘Patients’, ‘Problems’, ‘Providers’, ‘Services’, ‘Encounters’, and ‘Genome’ components serving as discrete micro-applications.

Extensive use of ‘Drag & Pop’ would allow the user to explore defined relationships in a multi-dimensional information space throughout the care-flow and discovery cycles.

The user experience would amount to playing a 3-D video-game in a Web 2.0 Social Networking space that is constructed as a Service Oriented Architecture (SOA) environment.

The user-centered system and method of the DBI would guide the software developer to discover the Visual Information Objects (VIOs) that would make up the micro-applications.

Inherent to the design approach, DBI Micro Applications could be downloaded to an iPhone (or any other mobile internet device) like a song, a video clip, a GPS, a restaurant locator, or a calculator.

This is because each micro-application component of a standard DBI-class solution could exist in the information system environment independently of any other DBI component.

Thus a standard-based yet innovative UI would accelerate adoption of hospital information systems, compress the cycle-time from ‘Bench’ (scientific discovery in medicine and life-sciences) to ‘Bed’ (applying new knowledge to patient care), and enable the rapid turnaround of new systems development at lower cost. ■

Primary Care: Transformation by Innovation Nat Findlay

“If you want a glimpse of what health care could look like a few years from now, consider ‘Hello Health,’ the Brooklyn-based primary care practice that is fast becoming an emblem of modern medicine” Health Affairs, March 2009 Why is that?

Put simply, primary care is in crisis, and Primary Care Physicians (PCPs) are in a tailspin.

There is little incentive to maintain continuity of care, through multiple communications channels such as email or instant messaging, or with the range of professionals (“the care team” or “Medical Home”) that potentially surround any patient.

Consequently, the American public is deeply dissatisfied with its Primary Care.

Before Myca, I teamed up with Leonard Schlemm, co-founder of 24 Hour Fitness, to integrate internetready PCs into more than 10,000 pieces of fitness equipment in more than 500 clubs across North America. (I guess it's no surprise that I have a treadmill built into my desk.) Then, I got the opportunity to take that idea farther into the health care world by equipping hospital beds with touchscreen computers connected to the internet, just like the machines in the fitness clubs. A few brave souls and I started a venture called Eon Media Inc, which was quickly acquired by Cardinal Health Inc. one year later.

During my time as VP of Clinical Technology and Services at Cardinal, I found that these hospitals were filled with incredibly dedicated people, working with just as incredibly outdated information and communication systems. I began to wonder how I could enable better communication between patient and doctor outside the four walls of the hospital.

How could I connect mobile consumers to the mainframe of medical computing? That was the inspiration behind Myca, and it still is today. Nach Abschluss der Phase 4 werden dann die Ergebnisse ausgewertet und zusammengeführt. Ziel der Befragungen ist es, den Verantwortlichen im Health Care ITMarkt ein Bild zu vermitteln, welche Trends (Entwicklungen, Lösungen, Technologien) für 2009/2010 erwartet werden, welche Faktoren das Gesundheitssystem beeinflussen und möglicherweise verändern werden und welche internationalen Trends diese Entwicklungen nachhaltig stimulieren und forcieren.

Bitte unterstützen Sie diese Befragung, indem Sie diesen Fragebogen gewissenhaft und vollständig ausfüllen.

Sämtliche Informationen werden streng vertraulich behandelt und der Datenschutz aller Befragten ist uneingeschränkt gewährleistet. Die Fragebögen dienen ausschließlich analytischen Zwecken, werden nicht an Dritte weitergegeben und nach Beendigung des Projektes vernichtet.

Die Auswertung der Befragungen wird Ende Juli 2009 beendet sein. Die Ergenisse werden dann im Rahmen des Asklepios Future Hospital Programms als „HITtrends Studie 2010“ veröffentlicht und können über die Homepage des AFH abgerufen werden (www.asklepios-futurehospital.de).
