WISDOM FOR THE SENIOR GARDENER How to stay fit and avoid injury

Lifting, digging, raking, bending, and walking are all part of gardening. We use and abuse our hands as we weed, prune, and hand-dig. We challenge our knees and hips as we move up and down, carry pails, drag hoses, and pull wheelbarrows. When we were younger, it was much easier.

As a younger gardener, I only stopped when I had to make dinner or attend to other work (and I went back to the garden as soon as I could). Now, my battery runs out; I have to stop. I don’t like it!

It’s time for some guidance, including instruction from a physical therapist, fresh thoughts from a selfproclaimed geriatric gardener, and some old-fashioned common sense.

Get a smart start

Surveys from several garden and landscape groups tell us that one and a half to two million Americans began gardening during the pandemic. Most report that they have continued gardening, even when family life and work returned to some form of normal. The following guidelines

work for these new gardeners as well as those who need to change their existing bad gardening habits.

• Warm up before going to the garden (or starting any physical project), every time. Whether your preference is stretching and calisthenics, yoga, Tai Chi, or a gym workout—do it. The idea that “I’ll get enough exercise in the garden” is not helpful. In gardening, you are using many muscles, including some rarely used otherwise, and they all need to be warmed up.

• Start an exercise program well before the season begins. What you do in March will make your hands, knees, and back much better partners in May.

• If it hurts, stop. Gardeners tend to

push through discomfort or even pain. If you feel an ache or strain, if you have a cut or bump, do something about it immediately. That choice may make it possible to continue tomorrow.

• Protect yourself. Use sunscreen and wear hats and sunglasses. Wear protective glasses when you are pruning, cutting, fertilizing, or using any products that could do damage. If anything pokes or splashes your eye, wash it thoroughly, or seek medical attention if that doesn’t offer relief. Use ear protection when you are using lawn mowers, chain saws, or any other noisy equipment. Wear gloves and a mask any time you are working with potting mix,

soil amendments, or any other products that produce dust or fumes. Finally, drink water often, and remember to eat well.

• Just be careful. Don’t use pruners or chain saws to cut anything above your head. Don’t do work that an arborist should do. Don’t lift more than you should (and you know what is too heavy). Don’t climb ladders on your own.

What the physical therapist taught me

I recently graduated from physical therapy, having approached it with a crooked and painful back. David Manzella (PT, DPT) straightened me out (literally), developed a long-term exercise plan, and taught me a lot about what to do and not to do, especially as a gardener.

In gardening, we do almost all our work bending forward, using arms and hands in front of ourselves.

Sitting, standing, or kneeling, the movement is out front, with the back curved and often not supported.

Manzella advises the following:

• Work on core strength, developing abdominal muscles to support your back. Stop whenever you feel pain.

• Stretch frequently and arch your back often to counter the forward movements. (Particular exercises may apply for individual needs.

• When lifting, keep objects centered and close to your body—no reaching off to the side to pick up a rock or container. Lift with your legs, not your back. To lift weights, such as pails, balance the weight on both sides and keep your spine straight. Rather than push, pull the wheelbarrow or garden cart, with a straight back.

How long we can garden as we once did, and how much we cut back on our projects, are personal

choices. Everyone has limitations, whatever our ages and physical conditions. Knowing ourselves— observing and listening to our bodies—is an important life skill.

Our attitudes and priorities are equally important. In the early 1990s, I knew a Master Gardener and rosarian named Steve, who worked in the Delaware Park Rose Garden. As his physical condition deteriorated, causing much pain, he would lie on the ground on his side to reach the roses with his pruners. While I suspect that his wife, doctor, or physical therapist would have advised otherwise, gardening meant that much to him.

Let’s each find our limits and follow our passions without hurting ourselves. Start your warm-up; spring is nearly upon us. FY

THE DIVESTING SIDE OF DOWNSIZING

Diana Augspurger has felt the pain of downsizing both through her own experience and the experiences of countless clients. As founder and CEO of Creative Storage in Buffalo, she’s spent decades helping people pare down and she’s developed some strategy along the way.

Augspurger is well aware of the emotion and guilt that trap people in a crazy cycle of clutter. When her mother, legendary Buffalo interior decorator Florence Cooper, died, Augspurger was faced with the task of disposing of her extensive furnishings and possessions, including eleven sets of china. The items all had value, not the least of which was sentimental, but Augspurger knew she couldn’t keep them all. What she did is something she now suggests to clients: “I created a digital library of my mother’s possessions by photographing them. They were things I did not want to forget.”

It isn’t always that easy, of course. “It’s hard for people to get rid of things like their grandmother’s chair,” Augspurger maintains. “It’s like saying goodbye all over again.” But what happens when you don’t have room for the chair, or it doesn’t

match your décor? Bad feelings can lead to a lot of clutter but, according to Augspurger, there is a way to minimize both. Here are her best tips:

1) Look on the bright side. “People tend to look at the negatives of downsizing, focusing on losing something,” says Augspurger, who says people of a certain age bracket tend to see downsizing as the next step before the last step. “Try a positive outlook. A lot of the big responsibilities of your life, like putting kids through college, are behind you. You can focus again on being yourself.” And with less stuff.

2) Let others appreciate the items. Some folks keep extra furniture in storage for the day their grown kids might need it. Some hold onto one of the many dolls in Mom’s collection and give the rest away. After sheltering it in her Buffalo home

for years, Augspurger finally shipped off her sister’s favorite— and big—stuffed animal to the sibling’s West Coast home and told her to do with it what she would. Vintage collectibles might be of interest to local history museums. Suggets Augspurger: “It’s easier to say goodbye to something when you know it’s going to someone who will use it, enjoy it, and take care of it.”

3) Take advantage of modern disbursement solutions. No one needs to have a cupboard full of mismatched plastic containers and lids wasting valuable kitchen space; that’s what recycle bins are for. Consider eBay or amazon. com as sales outlets for other stuff—books, toys, dishes—that accumulate in our homes. “It takes a little energy,” says Augspurger. “Enlist your kids to help you if your

computer skills are minimal. If that doesn’t work, give items to charity; let them reap the benefit, and you take the tax write-off.”

4) Be creative. The evergrowing organizing industry is full of clutter-reducing solutions, but sometimes just thinking

about what you really need and want can help you come up with your own ideas. Augspurger says her particular problem was magazines, but now she stores them in binders designated for garden, house, and projects, with specific articles in clear protectors,

a solution that enabled her to get rid of the magazines, but not the information she wanted to save.

4) Cultivate habits to break the clutter cycle. “I am too tired at the end of the day to sort through my mail immediately, but I do have a place to put it all near the door so I can tackle it later,” shares Augspurger, who says having a collection point will keep things from spreading all over the kitchen. Try to shop out of need, not want. Buy items that are multi-purpose, such as generic candles as opposed to holiday-specific ones. “There is this feeling of complete and total freedom when you’ve made the tough decisions and lightened the burden,” says Augspurger. “It is so energizing!” FY

KEEPING ROMANCE ALIVE

Spring is almost here, and romance is in the air. Young, starry-eyed lovers seem to be everywhere. For them, maintaining the spark seems effortless, but how do we keep romance alive when we’ve been married or in a relationship for many years? Some may think a romantic cruise or expensive vacation will get the fire burning again, but several couples we surveyed said it’s the “little things” that make all the difference.

Here are some examples:

• One partner says expecting leftovers for dinner and discovering instead a nicely

set table, dimmed lights, soft music, and a favorite meal waiting enhances the mood.

• One wife loves it that her

husband stands up when she is leaving or returning to the table at a restaurant. Garnering similar appreciation

are gestures like holding the car door or running to bring the car around when the weather is frightful.

• Husbands who travel regularly for business share they are thrilled to return home with nothing planned for them other than comfort food and a chance to unwind.

• One wife who had been working late several days in a row came home to find her husband had done laundry, right down to folding the towels. She says it meant more to her than receiving an expensive gift.

The theme? Showing your partner you care. If you’re not sure where to start, consider the following: Take over responsibilities that your partner dislikes. This cannot be overrated. He hates doing taxes, so she gets them done. She isn’t interested in cars, so he makes sure

her tank is full and the inspection sticker is current. These are not complicated tasks, but a relief for whoever dislikes doing them.

Enjoy time spent together—and apart. Most couples have at least some common interests, but if he wants to spend hours watching the playoffs, she will be just as happy to round up a group of girlfriends and go see a foreign film replete with subtitles. Time spent together is great, but now and then time needs to be set aside for individual interests and hobbies. Give each other some space rather than being constantly joined at the hip; it’ll help you value your quality time even more.

Create sacred spaces. Speaking of space, creating places in the home where each partner can retreat, uninterrupted, can prove blissful. It could be anything from a “man cave” to a quiet place for her to read or do yoga.

Let go of pet peeves. Everyone has them, and partners always know which buttons to push. So what if he leaves the seat up? Put it down. She drops her briefcase by the door? Pick it up. Allow your partner some grace.

Sometimes learning the fine points of romance means looking no further than your own family. One of my uncles—a former boxer— was a big, formidable man. When I stopped by to visit one day, I saw him standing outside the door, puffing on his cigar, and waving around a handful of doilies. When I asked what he was doing, he said his wife worked just as hard as he did all week, and that he was happy to help with the cleaning.

I never heard my aunt complain that her fine linens smelled of cigar smoke, but she did smile a lot. FY

STRESS-FREE TRAVEL

Use TripIt, LoungeBuddy, and more for your next getaway

When I’m not tearing my hair out over a website that won’t load or wracking my brain for a forgotten password, I marvel at technology. As a frequent traveler and directionally challenged person, GPS has changed my life. (Perhaps that’s a bit of an exaggeration, but it’s certainly made road trips less stressful.)

The latest travel technology to excite me is a planning app called TripIt. I recently started using it after a friend and a fellow avid traveler raved about it. She showed me her phone, which displayed a detailed itinerary of an upcoming trip: flights, hotels, rental car, and restaurant reservations.

“Cool! How does it work?”

I asked, thinking of the time it takes me to organize addresses, reservations, flight numbers, etc. in a word document. I was surprised to learn that you don’t have to input any information; simply forward travel confirmations to plans@tripit. com.

Impressed, I downloaded the app, signed up, and sent my flight confirmation and hotel reservations for an upcoming trip to California. Instantly, I had my itinerary without having to copy/paste or write anything down.

TripIt offers the following features

for a smoother and safer trip:

• Interactive airport maps with step-by step instructions to get to the gate

• Points of interest like nearby restaurants, gas stations, hospitals, and pharmacies

Taking advantage of airport lounges is another way to ensure a more relaxing airport experience. LoungeBuddy tells you which

lounges you can access at any given airport based on your credit cards, airline status, and lounge membership. And if you have none of these, check out free or day-pass lounge options.

Once you’ve reached your destination, AllTrails lists nearby walking, hiking, and biking trails based on difficulty, distance, and rating. (I would have missed so many scenic hikes without this trusty app.) Once I’m on the trail, I use the iNaturalist app to identify plants that pique my curiosity. And if I hear an unusual bird song but can’t spot the feathered warbler, I open BirdNET for instant recognition.

Make your next trip a quick one to the app store, and you’ll be traveling stress-free in no time. FY

TOWARD OPTIMAL BONE HEALTH

The important relationship between vitamin D, vitamin K, magnesium, and calcium

Osteoporosis and osteopenia are among the most common diseases affecting people over age sixty-five— particularly women. Both are caused by low bone density and can leave those afflicted at greater risk of injury and pain. So what is bone density and how can we maintain it to reduce susceptibility to these diseases?

When people think about nutritionally maintaining healthy bones, calcium is often the first word that pops into their heads. This is because bone tissue comprises mostly calcium, along with another mineral, phosphorous; these nutrients give bones their hardness and rigidity. The amount of these minerals in the bone is referred to as bone density, so the lower the amount of calcium in the bone, the lower the bone density. The lower the bone density, the weaker the bone, and the higher risk for pain and injury.

There’s no debate that calcium is vital for bone health but unfortunately, it’s not as simple as just drinking more milk, eating more dark, leafy greens, or taking calcium supplements. The body is full of complex systems that require different minerals that all work together to keep things healthy and

balanced. Three key nutrients are crucial to keeping a healthy amount of calcium in the bones: vitamin D, vitamin K, and magnesium.

Up first: vitamin D. (Fun fact: despite the name, vitamin D is actually a hormone!) Vitamin D has been getting a lot of attention recently. In fact, it’s commonly listed as a nutrient deficiency in older adults, second to calcium. This isn’t surprising, as the two are related; it is vitamin D’s job is to absorb the calcium and phosphorous from the gut. You can consume all the calcium you want, but without Vitamin D, it’ll never get absorbed into the bloodstream! People with a vitamin D deficiency therefore have a higher risk of bone loss. While you can obtain vitamin D through diet with foods such as wild caught salmon or other fatty fish, oysters, tuna, egg yolks, and mushrooms, diet only provides

our bodies about ten percent of its vitamin D needs. Your skin manufactures the other ninety percent through sun exposure. So make sure to get outside and enjoy sunlight for ten to forty minutes a day (necessary amounts vary based on pigmentation)!

Vitamin D might do the bulk of the work in getting calcium where it needs to go, but it can’t do it alone. It works hand in hand with the lesser known vitamin K. Vitamin D gets the calcium from the gut to the bloodstream, but that isn’t where we want it! That’s where vitamin K comes in; it transports the calcium from the soft tissues and binds it to the bones. Too much vitamin D without enough Vitamin K can be harmful because not only does it prevent calcium from getting to the bones, but it also can create excess calcium levels in the bloodstream, which can lead to heart disease,

atherosclerosis, kidney stones, high blood pressure, and more. Good whole food sources of vitamin K are pasture raised grass fed beef, milk, and cheese, as well as fermented foods such as nattō.

Now we understand how vitamin D, vitamin K, and calcium connect, but there is one more player in this game: magnesium. Magnesium converts vitamin D into calcitrol, the active form necessary promote

the absorption of calcium from the intestines.

Chocolate, coffee, coconut water, cooked greens, and some fruits are all great sources of magnesium. Because magnesium is a mineral and not a nutrient, supplementation may be necessary beyond these sources.

The synergistic magic of vitamin D, vitamin K, calcium, and magnesium work together to build higher bone density! To focus on your bone health, partake in weightbearing exercise, avoid low-calorie diets, eat sufficient amounts of protein, maintain a healthy weight, and be aware of your intake of these elements. If you’re concerned about levels, ask your doctor about supplements. FY

Our medical providers are specialty trained to not just diagnose the root cause and type of each patient’s dizziness or tinnitus, but also implement a specifically tailored protocol to address and treat the problem.

The diagnostic and vestibular therapy team provide diagnostic tools to pinpoint the cause of each patient’s symptoms. Additionally, it provides a full-service vestibular therapy service to rehabilitate patients. We are proud to be one of three sites in the United States which provides state-of-the-art Advanced Vestibular Treatment program. Dr. Lixin Zhang’s research team is dedicated to bring innovative medical research and techniques to our patients. Its primary focus is to improve our patient’s well-being and treatment in dynamic new ways. We also treat Meniere’s Disease, Migraine Associated Vertigo, Post-Concussion Syndrome, and many more.

About two-thirds of Americans have prescription medications, and around 50% of them do not take their medications as prescribed – this is known as medication non-adherence. It is a major cause of avoidable physical and emotional suffering, financial loss, and premature deaths that cost people precious time that could be spent with their loved ones. Today, medication nonadherence causes over 125,000 preventable deaths annually making it a significant problem that should be addressed by the healthcare system.

WHAT ARE SOME CAUSES OF MEDICATION NON-ADHERENCE?

The reasons for poor medication adherence are often multifactorial. Most common of which include forgetfulness, regimen complexity, high cost, side effects, and incomplete understanding of the medications needed.

HOW DO YOU INCREASE MEDICATION ADHERENCE?

Patients can discuss the reasons for poor adherence with their pharmacist or provider and come up with a personalized plan to increase adherence. Increasing adherence to prescription medication not only increases physical and mental health, but also productivity and overall quality of life.

WHAT IS VITAL PHARMACY DOING TO INCREASE MEDICATION ADHERENCE?

Medication non-adherence is an enormous problem that is still largely unaddressed by the healthcare system. At Vital Pharmacy, we welcome new patients who want a team focused on their adherence. Most people do not intend on becoming non-adherent. It often starts with having an overwhelmingly complicated regimen which discourages patients from taking their medications. Having a pharmacy that specializes in helping caregivers and patients with complex regimens is a huge benefit. One study found that 1 in 2 people missed a dose regularly,

1 in 3 forget if they took their medication and 1 in 4 did not get a refill on time.

Vital Pharmacy helps each patient come up with a personalized plan to increase their adherence. We call patients monthly and utilize several new technologies to improve adherence rates. This includes:

Free delivery and mail: offered to all patients to overcome transportation barriers.

Compliance refills: we call patients monthly prior to medications running out so they are ready for pickup by the time patients run low on their prescriptions. We will also call their provider if a new prescription is needed or needs to be synced.

Medication synchronization: all your prescriptions are filled at the same time each month, decreasing the number of trips made to the pharmacy and simplifying refills.

And most importantly, adherence packaging, either in pouches or blister packs, where we organize medications by day and time of administration. No more sorting pill boxes! (as shown on the bottom of this page)

Safety: In addition to the pharmacist verification check, we have a robotic check assessing every medication for size, shape, and color.
