Cerner’s Healthe Living Program is designed to help associates “take charge” of their health and provide the tools by which they can achieve optimal health and wellness through health evaluations, preventive care, activities and educational offerings.
The Healthe at Cerner program includes five main services:

Healthe Pharmacies– Provides convenient, competitively priced pharmacy services.
Healthe Fitness Centers– Offers a variety of programs and services to help you keep on track with your exercise needs.

Cerner Health Plan– Progressive benefits that give you more control over your health care expenses. Includes educational resources, decision making tools and easy access to financial information. Cerner’s Health Benefits are managed by Cerner Health Plan Services.

From wellness programs and condition management services to an electronic personal health record, Cerner gives you all the resources you need to achieve the optimal health outcome. Our personal health management tools give you the power to manage your health using information from your providers and hospitals. Use these tools to communicate securely with your doctors about health care matters, request prescription refills, or schedule an appointment online. The personal health management tool also allows you to access your health plan information online including health care expenses and easy to access financial information.

By accessing the tools and services of Healthe at Cerner, such as Cerner Health Plan Services, Price Transparency and the Healthe Clinic, you will create and enhance the connections you need to make the best health care choices for you and your family.
Health & Wellness Healthe at Cerner As a Cerner Associate, you and your family have a new way to experience health care. The Healthe Clinics at Cerner’s World Headquarters (WHQ) and Innovations Campus (IC) offer associates and their eligible dependents convenient, cost effective, patientcentered, quality care as well as a fullservice pharmacy.
Patient Experience The Healthe Clinic is changing the way primary care is being delivered. From the initial contact with clinic staff, to the collection and payment for services, the Healthe Clinic delivers a truly unique experience to associates and dependents. Emulating a service model that is often found in the high end retail industry, where the customer is at the center of activity, the Healthe Clinic space and workflow is designed with this patient centric approach. The Medical Home Model The Healthe Clinic can serve as your Medical Home, an approach for comprehensive and ongoing primary care that focuses on partnerships between patients and their providers. Since opening in 2006, the Healthe Clinic has provided Cerner associates and their families patient-centered, integrative quality care in a nurturing environment. As your Medical Home, you will develop a mindful clinician-patient partnership built around trust, respect and shared decision-making. Your care team will coordinate care for both wellness and illness, a whole person care approach.
Eligibility and Service Pricing Associates and dependents enrolled in a Cerner sponsored health plan are eligible for professional services, inhouse labs, dietitian consultation (with referral) and eVisits at no cost. Out-of-
house labs, pharmacy services, chiropractor services and over the counter medications are the patient’s responsibility. If you are enrolled in a non -Cerner health plan (other employer health plan) or private health plan you are not eligible for the Healthe Clinic plan but may access the clinic services. The associate and dependent will be responsible for full clinic pricing, payment and submission of request for reimbursement from their own health plan. Associates and dependents not enrolled or covered by any health plan are not eligible for the Healthe Clinic. Services include:

Healthe Fitness Centers There are two fitness centers, one located in the Associate Center at the Cerner WHQ campus and the other at our Innovation campus. The fitness centers offer a group exercise studio, cardio equipment, weights and fullservice locker rooms. The WHQ Fitness Center also features a multi-purpose gymnasium, swimming pool with lap lanes, two racquetball courts. In addition, the Healthe Fitness Centers offer a variety of programs and services to help members keep on track with their exercise needs. Degreed exercise professionals use health and fitness goals to create personalized exercise programs for each member. Recreation leagues and group exercise classes are also offered throughout the year that not only enhance your fitness, but also your network of fellow Cerner associates. Virtual associates are eligible for health club reimbursement

Healthe Living with Rewards Research indicates that individuals who actively manage their health achieve better health and lower costs. Eligible Health Plan members and their spouses or same gender domestic partners will have the opportunity to participate in the Healthe Living with Rewards Program and earn premium discounts and Incentives. Price Transparency The price transparency tool is a free tool designed to provide you the cost and quality information you need to make the best health care decisions. This solution will give you access to personalized tracking of your health care costs and notification when lower cost options are available, cost look-up functionality to compare prices of medical services before you need them, information on how to switch prescriptions to the lowest cost pharmacy and quality data on providers. 5

You can engage with a multi-disciplinary coaching team who will work with you to help you achieve your health goals. The

Long Term Disability The Long Term Disability plan provides you with income equal to 50% or 60% of your insured salary in the event you become disabled and are unable to work due to
medical reasons. The premium is paid with after-tax dollars so that the benefit, if ever needed, is non-taxable. Our benefit partner for this plan is The Standard.
Leaves Occasionally associates experience situations that require them to be away from their work. Cerner provides a comprehensive set of absence and leave policies to allow associates to balance personal and work needs. Short Term Disability will typically run concurrent with a Cerner leave of absence. The Short Term Disability plan outlines the pay component of your leave, for the time component please refer to the Cerner Absence and Leave Policy. Long Term Disability will not typically run concurrent with a leave.
Adoption Assistance Cerner recognizes the costs associated with adopting children and supports adopting parents by offering financial assistance for adoption fees and a paid parental leave. You may receive reimbursement up to $5000 per adoption for expenses. This leave may be utilized both pre or post adoption and will be applied to the leave allowed under the Federal Family and Medical Leave Act and similar state family leave laws.

Life Insurance and Travel Plans Cerner provides basic life insurance and AD&D plans to each associate at no cost. Several optional life insurance plans are available to you, at your cost, to expand basic coverage and provide additional protection for yourself and your family. Basic Life Life insurance and Accidental Death & Dismemberment (AD&D) equal to your annual base salary (rounded to the next $1000) to a maximum of $500,000 is provided at no cost to you.
Optional Associate Life and Dependent Life Insurance Optional associate life insurance can be purchased in increments of $10,000 with a minimum purchase of $10,000, up to a maximum of the lesser of $2,000,000 or five times your annual base salary. Rates are based on your age. Spouse or same gender partner coverage, also age-rated, can be purchased in increments of $10,000, with a minimum purchase of $10,000 and a maximum of the lesser of $250,000 or the amount of the Optional Associate Life coverage. Evidence of insurability may be required if you, or your spouse or same gender partner are requesting an increase that will surpass, for the first time, the Guaranteed Issue amount of $500,000 for the associate or $50,000 for the spouse or same gender partner, or you are requesting an increase greater than $10,000.
Child life insurance can be purchased in increments of $5,000 up to $15,000 maximum for each eligible dependent child. Evidence of insurability is not required.
Travel Plans Cerner provides travel life, accident and emergency medical insurance plans for associates and their eligible dependents at no cost. All eligible associates are automatically enrolled. These benefits are insured by ACE American Insurance Company.
Travel Emergency Medical Travel protection insurance is provided to Cerner associates travelling on international business. The plan provides the following types of coverage:

This plan is secondary insurance and will pay covered claims after any other insurance or national healthcare payments are made. The maximum
benefit per accident/illness is $100,000. Coverage begins when you leave your place of residence or employment, whichever occurs last, provided your destination is outside your country and continues until you return to your residence or place of employment, whichever occurs first.
Travel Life and Accident This Plan ensures you up to a maximum benefit of $200,000 if you experience a loss while traveling on business for Cerner*, excluding normal commuting to and from work. Travel Accident Insurance pays the lesser of one times your Annual Salary or $200,000 for a loss incurred while on a business trip.

Supplemental Insurance: Aflac Aflac is different from major medical insurance. It’s insurance for daily living. If you’re sick or injured, Aflac pays cash benefits directly to you to help address out-of-pocket medical costs, everyday expenses—whatever you choose. Aflac voluntary insurance provides the added comfort of being better prepared for whatever life may bring.
Personal Accident After an accident, you may have expenses you've never thought about. It's reassuring to know that an accident insurance plan can be there for you through the many stages of care, from the initial emergency treatment or hospitalization, to follow-up treatments or physical therapy. Group Accident Insurance from Aflac helps with out-of-pocket costs that arise when you have a covered accident such as a fracture, dislocation, or laceration. Benefits: More than 50 events that trigger benefits payments, including Fractures, Dislocations, Ambulance, and Physical Therapy, among others. Critical Illness The Aflac Critical Illness Insurance plan can help with the treatment costs of covered critical illnesses, such as cancer, a heart attack, or a stroke. More importantly, the plan helps you focus on recuperation instead of the distraction and stress over the costs of medical bills. With Critical Illness Insurance, you receive cash benefits directly (unless otherwise assigned)— giving you the flexibility to help pay bills related to treatment or to help with everyday living expenses. Benefits: Lump-sum benefit for a covered critical illness such as: cancer, heart attack, stroke, major organ transplant, and end-stage renal failure Benefit for a recurrence of the same critical illness if separated by at least 12 months or an additional occurrence of a different critical illness if separated by at least 6 months 13
Legal Plan The MetLife Legal Plan offers you and your family value, convenience and comfort in knowing that you can access legal services for almost all personal legal matters. It gives you easy and lowcost access to a wide variety of personal legal services.
Access The Legal Plan provides members with access to a nationwide network of more than 11,000 attorneys from which to choose. Plan attorney assistance includes:

Plus, if you stay within the network, covered legal services are provided with no additional attorney fees. Savings & Retirement Building tax-deferred retirement income is vital to the well-being of all our associates. Our investment plans include a 401(k), with a variety of investment options, and an Associate Stock Purchase Program. This enables our associates to build equity in the company and helps them establish long-term security.

The Cerner 401(k) Retirement Plan allows associates to set aside money on a tax-deferred basis for retirement.
Participants may elect between 1% and 80% of eligible compensation to be deferred into the plan annually 2013 IRS contribution limits:

Associate Stock Purchase Plan The Associate Stock Purchase Plan provides eligible participants with an affordable and convenient method to purchase stock and share in the success of the company. Contributions to the plan are deducted, on a post-tax basis, from your paycheck. Participants may elect any whole percentage between 1% and 20% of your total compensation to purchase Cerner stock at a 15% discount on the date of purchase. The deferrals are accumulated and, at the end of the Option Period, are used to purchase shares of common stock. The purchase price will be 15% less than the Fair Market Value on the date of the purchase (the last business day of the quarter). The Plan operates on consecutive three month Option Periods, beginning on the 1st day of January, April, July and October of each year. Dependent Care FSA
Long-Term Care insurance provides you with the financial protection against the costs associated with custodial care. Typically this is the care needed when a person requires assistance with the activities of daily living (such as eating, bathing, dressing, transferring, etc) or care due to cognitive impairment (such as Alzheimer’s disease or dementia). Care can be received at home, in an assisted living facility or in a nursing home whether due to accident, chronic illness or aging.
The Dependent Care FSA (DCFSA) allows you to pay for eligible child care expenses with pre-tax dollars. A DCFSA may be used for reimbursement of eligible dependent care expenses (i.e., child care). Generally these expenses will allow you and your spouse (if married) to work or further your education.

The younger the individual is at the age of application the cheaper the rates. Your rates do not go up as you get older. This coverage is portable at the same affordable group rates and is paid for with post-tax premiums.
Your minimum annual deposit to the DCFSA is $250
Your maximum annual deposit is $2,500 for associates who are married and file separate tax returns, or $5,000 for associates who file as head of household or are married filing jointly

Children’s Learning Centers About Cerner Kids At Cerner, we believe our associates are our most valuable asset, and we know your most valuable asset is your family. Cerner offers two childcare facilities, one at our WHQ campus (Cerner Kids) which opened in 1995 and the other at IC (Innovation Kids) which opened in 2007. Both facilities use the Montessori curriculum to cultivate children’s natural desire and ability to learn, while creating a nurturing and loving atmosphere. We’ll also provide you and your family with the right resources and tools to make your healthcare experience the best it can be. U.S. based associates and their spouses or partners who elect Cerner’s 2014 Health Benefits can earn points to reduce their premiums and receive funds for their Health Reimbursement Account (HRA). Healthe Living with Rewards This wellness incentive program is for primary members and their spouse/partner who have elected Cerner’s Health Benefits. As a primary member, you will have opportunities to participate in wellness and preventive care activities to earn points/dollars to apply toward premium reductions and your Health Reimbursement Account (HRA). As a spouse/ partner, you will have the opportunity to earn HRA incentive points. By participating, you will earn points toward financial incentives, and more importantly, establish healthy behaviors that will enrich your life.
Premium Reductions By participating in the 2014 Healthe Living with Rewards (HLwR) program, you can earn points to reduce your health premiums in 2015. Primary members can earn a maximum of 500 premium reduction points, 300 of which must come from completing the Wellness Evaluation, which includes an online personal health assessment and lab, biometrics and advising session.
If you have a spouse/partner, you have the ability to earn up to 700 points, 200 of which the spouse/ partner can earn by completing the Wellness Evaluation. If the spouse/ partner chooses not to participate the primary member can earn a maximum of 500 points. HRA dollars will be available for immediate use in 2014 and will roll over into 2015 if not used entirely. Each point is equal to $1. Associates and spouses/partners earn points from Dec. 1, 2013, through Nov. 30, 2014.

Cost Share
Medically Necessary
Once the deductible is satisfied, the plan pays the majority of costs. Generally, the participant will be responsible for 15% of claims during cost share up to their annual maximum.
Health care services or supplies needed to prevent, diagnose or treat an illness, injury, disease or its symptoms and that meet accepted standards of medicine.
Deductible
Network
A fixed dollar amount during the plan year that a participant pays before the plan starts to make payments for covered services. This portion is referred to as the deductible .
The facilities, providers and suppliers your health insurer or plan has contracted with to provide health care services.
Allowed Amount Maximum amount on which payment is based for covered health care services. This may be called “eligible expense,” “payment allowance” or “negotiated rate.” If your provider charges more than the allowed amount, you may have to pay the difference.

Emergency Medical Condition
A request for your health insurer or plan to review a decision or grievance again.
An illness, injury, symptom or condition so serious that a reasonable person would seek care right away to avoid severe harm.
The most you pay during a policy period (usually a year) before your health insurance or plan begins to pay 100% of the allowed amount. This limit never includes your premium, balance-billed charges or health care your health insurance or plan doesn’t cover. Some health insurance or plans don’t count all of your co-payments, deductibles, coinsurance payments, out-of-network payments or other expenses toward the limit.
Balance Billing When a provider bills you for the difference between the provider’s charge and the allowed amount. For example, if the provider’s charge is $100 and the allowed amount is $70, the provider may bill you for the remaining $30. A preferred provider may not balance bill you. Cost Share Once the deductible has been satisfied, the plan pays the majority of costs. Generally, the participant will be responsible for 15% of claims during cost share up to their annual maximum. Coverage level The scope of protection provided by your benefits plan based on the amount of dependents you cover.

Prescription Drug Coverage Excluded Services Health care services that your health insurance or plan doesn’t pay for or cover.
Health insurance or plan that helps pay for prescription drugs and medications. Preventive Care
Health Account A personal account established in the name of each associate who participates in the plan. Money in the account may be used to pay expenses for the deductible, the cost share portion of the plan, and IRS qualified medical expenses.
Routine physical exams and health screenings (like routine blood tests and age-appropriate health screenings). Preventive care is not subject to deductible or cost share, the plan covers 100 percent. Prescription Drugs

Drugs and medications that by law require a prescription.
Health care services a person receives at home.
UCR (Usual, Customary and Reasonable)
Hospice Services Services to provide comfort and support for persons in the last stages of a terminal illness and their families.
The amount paid for a medical service in a geographic area based on what providers in the area usually charge for the same or similar medical service. The UCR amount sometimes is used to determine the allowed amount.
