Happiest place on...

Every two years in dentistry, a very special event takes place on the other side of the world. Over the course of five days, Cologne, Germany transforms into the global epicentre of dental ideas, technology and innovation as typically 150,000+ people descend on the city for the biennial staging of the International Dental Show, better known as IDS. For me, it’s the happiest place on earth!

Compared to dental exhibitions in Australia, IDS is on an unimaginable scale with visitors struggling to take it all in even over five days - with no real possibility of engaging with every stand individually! Every manufacturer is represented with massive displays brimming with hospitality, entertainment and products galore - to visit them all in depth would take a month! Plus, there are experts on hand that can answer all of your questions on the spot regarding current products and what’s in the pipeline for the future.

Whilst many people in the dental community pencil in a visit to IDS on their “bucket list”, I’m fortunate to be going for the 12th time this year, visiting every IDS since 1999, only missing the scaled down COVID version in 2021.

If you had to describe IDS in a single word, it would be ENERGY. There is an immediate sense that there is an immense energy attached to the event that you simply never see to the same extent elsewhere. Another word would be PASSION, as there is no shortage of that on hand either.

Hundreds of millions of Euros are invested to ensure visitors have the best possible experience by exhibitors from the corners of the globe... including Australian manufacturers like SDI Limited, Myofunctional Research Co, ASIGA, BioMiniatures, Ozdent, Blender4dental, Partmaker and others.

In times past, before the internet existed and electronic communication became second nature, exhibitors at IDS would launch products that represented years of tireless toil coming up with the next big thing. Product development was literally on a two-year cycle to ensure you had something new and special to talk about in Cologne. And whilst the internet has changed much, there is still a healthy dose of NEW to be found everywhere at IDS.

IDS 2023 is particularly special because it is the 40th staging of the event and its 100 year anniversary. And it’s also the first proper staging of the event after the COVID-19 disruptions of the past few years. Like many, I will be looking forward to seeing this grand event return to normal as it truly is one of the best experiences on offer in the dental world.

And... by the time our own Digital Dentistry & Dental Technology 2023 event is staged in Sydney on June 2-3, we’re hoping to see some of the new tech from IDS filtering down under. So remember to register now for the best time you will have at an Australian dental conference in 2023.

The first step is to ensure that your clinic is not already listed. To do that, visit www.dentist.com.au and click on “Find a Denture Clinic”, which appears prominently as an option on the home page. Then enter your suburb and search. www.dentist.com.au and its sister site in New Zealand - www.dentist.co.nz - are published by the publishers of this magazine, so if you receive eLABORATE and you’re a dental prosthetist, then there is a good chance that you’re already on the site. If so, click on your listing and check that all the information is correct. 3Shape rebrands FullContour to 3Shape Design Service

3Shape has announced the rebranding of its FullContour design service company as the 3Shape Design Service. The move enables 3Shape to bring the former FullContour’s design services into its innovation and research and development tracks as well as make it easier for labs around the world to take advantage of the service as an integrated 3Shape digital dentistry solution.

3Shape Design Service will offer both designing by dental professionals and AI driven design through 3Shape Automate, creating a unique design service with two powerful options that cater to professionals’ different needs.

With the new, highly accurate Detection Eye intraoral scanner, the patient’s jaw can be easily digitised in less than 60 seconds, making the impressiontaking more comfortable for the patient and ensuring real-time scan with realistic colours and preparation borders. The scanner has been designed to be lightweight, compact and ergonomic and the scanning areas do not need to be pre-treated with powder, which simplifies the acquisition process. The created model can be then milled in one of the five milling units of the new M2 comfort line or 3D printed using the P4000 Printer.

Zirkonzahn owns 23 branches and education centres in seven different countries worldwide. Customers can take part in the training offered within the framework of “Die Zirkonzahn Schule” - The Zirkonzahn School. The M2 milling unit comfort line includes 5 machines, provided with one or two milling chambers, the Teleskoper Orbit Ø 125 mm and a tool magazine with up to 84 tool slots (depending on the model).

Fully automatic, flexible and vibration free, the M2 milling unit comfort line exemplifies precision and flexibility. With the M2 Wet Heavy Metal, the M2 Teleskoper, the M2 Dual Wet Heavy Metal, the M2 Dual Teleskoper and the M2 Dual Double Teleskoper, the comfort line comprises five milling machines with fully automatic 5+1 axis simultaneous milling technology.

The highlight is the extra-large Teleskoper Orbit (ø 125 mm): in combination with special interchangeable holders, it allows the processing of all common soft and hard material blanks of ø 95, 98, 106 or even 125 mm. With the Teleskoper Orbit, blanks can be removed and reinserted back into the orbit at the same position with high precision in the micron range.

In addition, the pre-set Ultraspeed programs of the Zirkonofen Turbo, perfectly adapted to each kind of Zirkonzahn zirconia, allow to sinter small zirconia structures in about 1 hour and 30 minutes only, with no variation of flexural strength, colour and fit values. This cuts processing time by 81% and reduces electrical consumption by 73%.

No limitations! With the extra-large Teleskoper Orbit, it is now possible to mill material blanks with Ø 95, 98, 106 or even 125 mm, as well as glass ceramics and Raw-Abutments® blanks. Discs can be removed and reinserted back into the orbit at the same position with high precision in micron range - this is particularly useful for the two-stage fabrication of immediate restorations in case of implant-supported prostheses (Double Milling technique).

Optimal material and colour selection: monolithic sample teeth available in the shape of upper incisor, upper incisor with minimal cutback for further characterisation, lower incisor and premolar.

Prettau® zirconia – Manufactured with pride

Prettau® zirconia is the material of choice for functional and aesthetical solutions - from single crowns to monolithic full arches. The Prettau line includes different zirconia typologies available as discs (Ø 95 or 98 with step) as well as mini blanks. In the last years, the company’s range of zirconia typologies has recently embarked even more on the path of monolithic design with the new Dispersive® versions. In this case, ceramic layering and manual colouring are not necessary anymore, since materials are already provided with a very smooth, natural colour transition during the manufacturing process. However, for an even more patientspecific result, structures can be further characterised with colouring liquids or, alternatively, a minimal cutback can be applied for the subsequent application of a slight layer of Fresco Ceramics. Developed for both gingival and vestibular regions, these new ceramic pastes do not require staining anymore. Prettau 3 Dispersive is the latest material developed by the company and is characterised by a triple gradient of colour, translucency values and flexural strenght (Gradual-Triplex-Technology).

New sustainable company headquarters for Amann Girrbach

ith its new, sustainability-oriented building, which is to be constructed in just 15 months, Amann Girrbach will be consolidating its current sites in one location. Following its completion in late 2023, all 350 of its employees will be located in Mäder, Austria. The aim of the company’s €47 million investment is to create a state-of-the-art working environment with added value for all employees and, at the same time, to optimise all procedures and processes.

All Austrian facilities, from manufacturing and warehousing to research and development, administration and training, will in future be located in the premises and halls, covering a total of around 20,000 square metres.

W“With the new building in Mäder, we will be implementing our own high standards when it comes to creating an atmosphere for our employees in which everyone can feel comfortable and work productively,” said Dr Wolfgang Reim, CEO of Amann Girrbach, underlining the company’s intention. “We are pleased that in future we will be able to offer a canteen as well as various recreational lounges and green areas that meet the most up-todate and well thought-out requirements.”

The rapid growth of Amann Girrbach has also prompted the need to merge the individual sites while expanding them at the same time.

“There has been an increasing lack of space in recent years due to our very satisfying corporate development and we have had to resort to external premises on several occasions,” he said. “At the new,

central location, it will now also be possible to optimise cooperation between the individual departments, resulting in improved processes and workflows and the entire logistic chains between the present locations will be eliminated.”

QUALITY GUARANTEED

Another key brief in the planning of the new headquarters was the commitment to sustainably reduce the company’s carbon footprint. The building will be fitted with one of Vorarlberg’s largest photovoltaic systems, with an output of up to 1.15 MWp - Megawatt peak. Amann Girrbach utilises heat from the sintering furnaces to heat the building and the heat pumps operate with ground water. The planned green roof is also designed to promote biodiversity. The cooperation partner, Garbe, is a technical and structural codeveloper as well as being the owner of the new building.

Treating this condition is difficult and frustrating as treatment options can be uncomfortable or expensive if dentures require replacement more frequently. An opportunity arose to challenge me to consider an alternative treatment option and craft a digital copy denture to address possible premature wearing of a denture due to suspected bruxing. The combination of traditional experience with emerging digital technology provided inspiration to “get out of one’s own way” and, ergo, challenge professional complacency. This article explores the treatment option of how to manage the consequences of bruxing/clenching, not the causes associated with it and how to prevent them. By Terry Whitty 3D printers are all the rage and the projected new norm will be for every dental lab and every dental surgery to have at least one. The uptake in dentistry in general has been reasonable so far, however, there is still a lot of confusion with many about what technology to invest in as many don’t want to get burnt and left with an expensive boat anchor. This is understandable, though most of the well-known brands marketed for dentistry are now reliable and spawn very good results. Even better, they are great value compared to machines of, say, 10 years ago where you would mortgage a much loved item to purchase one.as I certainly did! Resin based vat polymerisation printers are really the “go to” printers for dentistry as they are relatively inexpensive and quite reliable. Many dentally-focussed companies are in their 2nd or 3rd generation of machines so lots of the bugs have been ironed out and all tend to produce good quality printers and materials. Invest in a good quality brand known in dentistry and you generally can’t go wrong. Which brings us to the new Accufab L4D 3D Printer from Shining 3D (already well-known for it’s range of intraoral and benchtop laboratory scanners). An opportunity arose to challenge me to consider an alternative treatment option and craft a digital copy denture to address possible premature wearing of a denture due to suspected bruxing. one’s own way” and, ergo, challenge professional complacency. This article explores the treatment option of how to manage the consequences of bruxing/clenching, not the causes associated with it and how to prevent them.

Patient case

An existing patient to the practice presented for a recall appointment with an approximate two-and-a-half-year-old complete upper denture to natural lower dentition. Having been a patient for many years, she has exhibited bruxing/clenching tendencies, which have been present for decades. These treatments have included hard and soft maxillary and mandibular splints, meditation, medications and hypnosis. The patient has learned trigger points which influence her bruxing and has also accepted the premature wearing of the denture teeth and with it, more frequent denture replacement. She must always wear the denture at night for temporomandibular joint support. Considering her condition, the existing denture was crafted with VITA Excell Anteriors and Lingoform Posteriors. These teeth were chosen for aesthetics, material durability (acrylic-glass) and Freedom in Centric design. A more detailed explanation is to follow.

Oval/Circular Ovoid Tear drop/Vertical

Although the denture was performing well during her recall appointment, slight premature wear was noted and discussions began on pre-emptive options.

Prior to her becoming a patient, her previous dentures had been crafted from acrylic teeth, which were well worn within a few years and fractured often. The current VITA product used in her denture was performing well and much better than acrylic teeth had. Since the option was now available, the decision was made to craft a digital copy of the existing denture, which she would wear at night or in trigger point situations. The thought was to have the copy denture bear the brunt of the consequences of bruxing/clenching and not the “good” denture. The existing denture was scanned using 3Shape and a copy made from Dentca tooth shade printable resin. This material was chosen with the expectation that it would be more durable than acrylic.

Treatment plan

In drafting the treatment plan, the copy denture would not be enhanced as the current denture was performing well. It functioned, fit, was comfortable, had not fractured and was not causing any issues to the supporting structures as had occured in previous dentures. The copy denture was to be worn at night or in trigger point situations.

Background information and theory

Existing denture teeth

Standard teeth available and used in most dentures are functional, with 10, 20 or 30-degree variety with the working and balancing cusp concept. However, the teeth are locked into a centric position and have no Freedom in Centric. Yes, they work and balance, but there is no “wiggle-room” in centric. In cases of single dentures, these denture teeth are ground into the natural teeth to achieve maximum contact and overlap and have no Freedom in Centric.

Freedom in Centric

Freedom in Centric is defined as “A flat area in the central fossae upon which opposing cusps contact, which permits a degree of freedom (0.5-1 mm) in eccentric movements uninfluenced by tooth inclines.” (Schuyler CH. Freedom in Centric. Dent Clin North Am. 1969;13:681-686.). All natural dentition have this concept, yet only a very few specific denture teeth available have it incorporated.

Why Freedom in Centric is important in dentures

This concept is important as denture teeth have no nerves. As such, a patient cannot feel when the teeth occlude or if they occlude properly. The important fact to remember is that there is food between the teeth during chewing and it is not possible to achieve perfect centric contact each and every time like we can achieve on an articulator during fabrication. What compounds the issue is that dentures are subject to moving as they fit against soft tissue.

Combine these points and it is not conceivable to think a denture patient can achieve perfect centric each time. This provides a reason as to why natural dentition has Freedom in Centric, even though we can feel our teeth, as it is not possible

to achieve perfect centric each time we chew. With this in mind, the concern is the working cusps hitting the corresponding guiding planes, sliding into position. The hit and slide aspect is a source of denture issues and ill fit. When present, because

the patient cannot directly feel it, they will try to grind away the offending contact, exasperating the bruxing/clenching condition. This is also often misdiagnosed. Freedom in Centric thus provides stability to dentures.

Why Freedom in Centric was important in this treatment

In considering that the upper denture rests against soft tissue and opposes a more rigid natural dentition, it is subject to dislodging forces. Combine this with the patient’s neurological impulse to brux/ clench; functional occlusion becomes paramount–functional in that there are no hit and slide contacts occurring within function to act as a trigger for bruxing/ clenching. Freedom in Centric was incorporated into the current denture to eliminate the occlusion as a trigger cause. Part of the reasoning was that the denture should “do no harm”.

The digital design

The existing denture was scanned and then designed using 3Shape. The copy denture was inserted and checked. As it was a replica of the existing, no refinements were necessary. Upon follow-up two, four and eight weeks post-insertion, the patient was wearing the copy denture as instructed and no discomfort was reported.

Key take away points

• Freedom in Centric designed occlusion, regardless of fabrication or platform used, provides denture stability and flexibility.

• The hit and slide on the guiding planes of cusps is the primary cause of common denture issues.

• Managing the hit and slide contacts provides a stable fit and, in this situation, control of bruxing/clenching. The denture is not acting as a trigger or causing bruxing/clenching as the patient is not trying to grind away an interfering contact.

• A “Freedom in Centric” copy denture is proving useful as a positive and affordable treatment option in bruxing/ clenching patients.

Conclusion

traditional or digital fabrication as we strive to “do no harm”, so please seek out and consider Freedom in Centric posterior teeth. “Freedom is Good!”.

The theory concept of this occlusion is that the teeth do not appear “knuckle tight” as one would usually expect. It was of open concept, free in centric and free within function.

The patient chew cycle

Apatient chews in a teardrop pattern and is unique to each individual. Some patterns are more horizontal, others more vertical (Figure 1).

The cusps, especially the non-working cusps of the buccal and lingual lowers, must not interfere while functioning within the chewing cycle of the patient. If there is interference, the cusps hit and then slide into position. As discussed previously, this hit and slide cannot be felt by the patient (Figure 2).

This article explored a “new” and trending treatment option for managing the consequences of bruxing/clenching.

Freedom in Centric is the ability to move within centric contact, thus not locking in a “bite”. This phenomenon is only incorporated in a few denture teeth and allows for flexibility in setup arrangements to suit patient requirements. Understanding this theory provided confidence to explore this digital copy denture treatment plan. We, as denture professionals, do not slap teeth together as we strive to be a superhero to our patients. The type of teeth you use matters in

The printer uses Masked Stereo Lithography technology or MSLA and uses an LED array as a light source and an LCD panel that “masks” the light source that creates the patterns for each layer to be formed. Its advantages include fast printing times and that it’s relatively inexpensive to build compared to other resin printing technologies. For MSLA, there are extra consumable items such as the LCD screen which needs to be replaced periodically but considering the entry price, this is not a deal breaker and these sorts of printers - if constructed well - have a good long life span.

The Shining 3D Accufab L4D uses a 4K high resolution LCD screen and this is more than enough to cope with anything you want to print dental-wise. People seem to get caught up in the frenzy of 4K, 8K, 16K but don’t actually understand how this impacts the dimensional accuracy of 3D printers - suffice to say, a 4K screen is very good and for more information please see my article on 3D printer accuracy in eLABORATE Vol. 18 No 4.

The light source is self-calibrating

Featuring a large build volume of 192 x 120 x 160 mm and a fast print speed of up to 50mm/h, the L4D is no slouch, however, always remember that the build time of your part, be it a model, splint or surgical guide, will depend on a few factors. Namely, height of the object, layer thickness, material selection and the parameters needed for successful building of the part.

The printer has a clip-in build plate that is spring loaded and auto levelling so this makes any Z axis calibration of the Accufab L4D a breeze. Levelling and zeroing the Z axis is a common calibration needed for just about any resin printer as it gives the printer a “starting point” to start building layers. Having the plate level is important because each layer formed needs to be a certain even thickness. If not level, you will get all sorts of errors.

Having a self levelling mechanism is a great addition because some printers have very cumbersome way of zeroing and levelling the Z xis of their printers.

The resin tank fits easily in the machine and locates with 4 lugs and a clamping mechanism that is super easy to remove, so changing materials is very quick. Also, once you remove the resin tank, you can easily set it down on a flat surface without fear of damaging the film as the lugs will hold it away from the surface.

The tank also boasts a long life time of 100,000 layers. Let’s put that into perspective - Let’s say and an average 100 micron dental model is made up of 250 layers and you print one model per print then you would get at least 400 models. If you print 4 models per print, then you would assume 1600 models as they share layers as they are on the same build. Either way, you are getting a large yield resin tank with this printer.

What about materials I hear you ask? This is where it gets interesting. The Accufab L4D is open to third party resins. Sure, Shining 3D have a whole range of materials available under their own brand but they do allow 3rd party resins to be printed as well. Now some are verified and the parameters are already worked out for you but if you want to go and try your luck with your favourite resin you can try it out because you can tweak the settings yourself to help you succeed. Will it print any resin? This will require a bit of trial and error but suffice to say, if it will work on an MSLA printer, it will probably work on the Accufab L4D, just be prepared to experiment.

The software supplied with the Accufab L4D is comprehensive and has some really neat additions.

I’ve used many 3D printing softwares and after you’ve used a few different ones, generally you will start to see similarities; the work flow is similar. Add a part to print, position it, add supports if needed, slice, send to the printer. This is the basic workflow and Shining’s software does a good job of this.

It has a wizard-like interface making the software user friendly. It has automatic part placement and it will create drain holes for those who want to print hollow models. Support generation is also good so all the basic functions stack up well.

An addition of note is the material calibration tool, where you can really work on getting the best dimensional accuracy from your materials. It will allow you to print the in-built calibration shapes for each type of material, then physically measure the results and adjust the parameters in the software accordingly by entering the measurements to get optimum dimensional accuracy. This is a really handy feature as a lot of companies do not add this sort of inbuilt material calibration feature.

Also just released is the Shining 3D Fabcure light curing unit and expect a washing station in the near future as well.

Summary

There you have it - a smartly priced quality 4K.MSLA printer. It’s easy to calibrate the machine and there are plenty of materials, z huge build platform, great software, it’s open to 3rd party resins and backed by a well-known customer focussed company.

What will it print? All types of dental models, trays, surgical guides, splints, IDB trays, denture teeth and bases, try ins, long term temporaries and probably anything else you throw at it! It’s hard to fault this printer and it would be great chairside or in a laboratory. Check one out today!

Playsafe 4u mouthguard graphics

Playsafe 4u graphics are designed to allow for the fast and easy application of custom graphics and identification on Playsafe Sports Mouthguards. The system is comprised of three

customisation options - preformed anterior teeth sections, custom printed graphics and clear label covers and all are applied externally on the mouthguard using the same technique (Figures 1-4).

Custom printed graphics

Ahigh-quality UV printing technique allows Erkodent to print the graphic design of your choice in vivid colour directly, using Erkoflex (thickness 0.7 mm) as a base material. The high resolution printing uses an opaque white layer to give the printed graphic extremely fine detail and vibrant colours regardless of the base mouthguard colour. This method makes it economical and

easy to add details such as club logos, surgery or lab branding without the time consuming and unreliable practice of adding graphics between structural layers of the mouthguard. The nature of this process makes one off and short run graphics viable. Up to two graphics can be printed on the smaller size label [40 x 60 mm] and 16 identical logos on the larger size sheet of 125 x 125 mm. The design data can be either in .jpg or .pdf format.

Preformed anterior teeth sections

Always popular on mouthguards, preformed teeth sections add a striking 3-dimensional graphic to the anterior of the mouthguard and can be shaped and moulded to achieve the look of vampire fangs or to standard teeth; extra customisation can be added, only limited by the imagination.

Playsafe label covers

Clear Playsafe Labels Covers are used to seal names, telephone numbers and graphics to the external surface of the mouthguard. Being exteral to the structure of the mouthguard means that the inserts do not affect the peformance of the guard. Label Covers are applied in the same manner as the Playsafe 4u graphics except the image or identification is positioned on the mouthguard and the Label Cover is bonded over.

Next generation desktop milling

These 1.5mm sized sintering beads are made from zirconia. The use of zirconia sintering beads will reduce the atmosphere inside the sintering tray to ensure perfect results. They prevent crowns and bridges from sticking or jamming during the sintering process. It must be ensured that single crowns are embedded in sintering beads up to their upper
