Thanks to a great person Twenty years ago Daphne Latter come to Jรกvea she was a gym teacher, and English teacher and she had left her native England. During the last twenty years she has always worked and helped in the British collective in Jรกvea, however two years ago when we decided to organize some English courses that could help the unemployed to find a job, Daphne joined without thinking.

She had taught English to immigrants in UK and she knew how to gain the trust of people who signed up for these courses, many of them middle-aged people who needed to gain a basic conversation level in English to find a job as a waiter in one of the restaurants of Javea.

Daphne presented in our offices the first day we published the announcement asking for teachers and the first thing she said is, HOW CAN I HELP. During these two and a half years she has earned the respect and admiration of all the students and ourselves. She has helped many people to find a job and especially to gain their trust, she has never charged for her services, she has worked for two years teaching others attending the voluntary language courses just for the gratification of helping others.

She has decided to return to England with here family and I wanted to write this little letter to thank all from the volunteers language courses and do a little tribute to Daphne.

EDITORS DESK If you are reading this then you have obviously found your new look, on line version of the Grapevine. I hope you like it. There will be lot’s more content in the future and Oscar has promised to keep us up to date with everything that is happening within the Town Hall. Another bonus of the Grapevine being on line is that we can keep you up to date on a daily basis. If news comes into us we can publish it straight away , instead of having to wait until the beginning of each month. Because our costs are reduced we can pass these savings on to our advertisers.

The cobblers shop ( Shoe repairers) is moving location yet again. The reason for this is simple, The rent is much lower.

Luckily they are not moving very far from their present location, just a few yards away opposite the Shish Majal Indian Restaurant. The shop will be extending it’s services to include baby hire. Travel cots, car seats, booster seats, high chairs etc.

The Caledonian Society Burns Supper

MAYOR OF JAVEA

MEETING MONDAY 25thFEBRUARY at 10.30 am at the PARADOR

Dear Friends, some months ago I set up a Facebook page (search for ‘Javea Mayor José Chulvi’) specifically to improve communications with our English speaking Xàbia residents who have access to social media sites. This seems to be have been quite successful and well received by the more than three hundred residents who have become my friends on Facebook. It is a useful means of keeping people up to date with the local news and events, and enabling our citizens to inform me of specific problems which they have encountered.

I have personally met a number of my FB friends but I thought that it would be a good idea to meet as many of them as possible during a face to face get-together. I am therefore planning to hold a meeting in the Parador on MONDAY 25TH FEBRUARY 10.30 am. All my Facebook friends are invited and are welcome to bring along friends who may not have access to the internet.

I will introduce you to my English speaking Facebook support team and make a short presentation on my ideas and vision for the future of Xàbia, a town where everyone can and should participate in building our future together.

I would like to extend a warm welcome to all the readers of the Grapevine to come along to the meeting, and look forward very much to this opportunity to meet as many of you as possible.

My wife and I were invited by Scottish friends to attend the Jávea Caledonian Society’s 2013 Burns Supper, and were really looking forward to it and to learning about Scottish culture, unfortunately official duties caught up with me and I was unable to attend and so missed sampling the Haggis and enjoying the evening. I did make a brief appearance and was able to say a few words which I will repeat now.

‘It is a pleasure and an honour for me to share a few minutes with you this evening.

First of all I wish to apologize, as I had planned to join you and some Scottish friends for dinner tonight, but my duties as mayor and deputy in the provincial government prevent me.

Today is one of those days when I wish I could divide myself and participate in all the events to which I have been invited. I have just come from speaking at the close of an important conference of local and international police which has been held throughout this week in Xàbia. Then I have to attend a function in Pedreguer in my capacity as provincial deputy.

I am fortunate to have Scottish friends, one of whom is more than just a friend, as every day I learn many things from him. I am convinced that for our town it is a real opportunity that so many different nationalities coexist together. This diversity enriches us all, and makes us more open as individuals and as a society. For this reason I really enjoy participating in events such as yours, people from other places, in this case Scotland, who one day decided they wished to live in our beautiful town.

And, as Mayor, I can only thank you for being with us, sharing your knowledge and experience, so that together we can make Xàbia a better place, more dynamic, more international, more open, and an example of coexistence and solidarity.

Doris Courcelles February already..... I suppose that you would prefer me to dedicate my column to this Saint Valentine who is used to bless all the lovers on the romantic date of the 14th but he, unfortunately, doesn't work in my department!

The first exciting news I have to give you is that your rubbish tax will cost you 125 € this year (less 40% if you are retired, invalid or older than 65 years).

See the voluntary payment period in Yvonne's “help desk” info.

Asking you this amount, the Town Hall is not making any profit. It is exactly what we pay to get rid of the garbage.

From the moment you throw the content of your dustbin in the containers, we need to collect it, transport it to the transference plant in Denia and load it again on lorries which have to reach Campello.

That is as far as the organic refuse is concerned. The recycling of paper, cardboard and plastic packings is very important.

Last year, “Ecoembes” which deals with these items, paid €170.000 back to the Town Hall. The glass (and we seem to be good drinkers in Jávea! Can you imagine 1.542 tons of bottles in 2012?) is collected free by “Ecovidrio” which avoid us to pay its management. If I repeat all this again, it is because we still have to make some efforts and recycle more. In this case and only in this case........ the next bills could go down.

With the purpose of teaching the kids about the advantage of the recycling and the improvement of the environment, we have distributed blue and yellow containers in every school of Jávea. Aren't the children the best citizens able to provoke big changes in our society?

At this time of the year when the gardens need all our attention, we should also be able to encourage private owners and gardeners to respect the right dumping spots.

That means using “Ramblars”, the different green containers at the “Punts Verts” or the “Ecopark” instead of throwing branches next to the normal containers!!!!! By the way, I still notice that even the citizens born in Jávea are still unaware of the situation of our “Ecopark”which is just between the sewage plant and the “Apasa“dogs pound in Cami de les Sorts. There, Nelson, our man in charge is very helpful and will advise you where to break down the most strange articles.

AND DON'T FORGET THAT IF YOU HAVE OLD FURNITURES TO GET RID OF, YOU ONLY HAVE TO CALL 900 102 149 TO GET THEM PICKED UP FREE!!!!

As you can see, my department has nothing to do with fiestas, tourism or culture but here we are, working for you and WITH you.

Till next month Doris Courcelles, councillor for “services, environment and beaches” was born in Belgium and studied at a French university. She speaks Castellano, English, French, Dutch, and German and is still trying to improve her Valenciano. As a consequence, she knows many residents and societies of Spanish and other nationalities here in Jávea. She moved to Spain in 1978. She has a son aged 29 and can empathise with parents in a foreign country.

Their experience is obvious at this well run establishment, which is very well run even though they are juggling a busy restaurant and raising a young family. The couple have three young children.

El Rancho is set in beautiful tropical gardens, and is perfect for Al Fresco dining in the Summer months. In the cooler months the interior is warm and welcoming. The staff are friendly and attentive without being obtrusive, Altogether a very pleasant dining experience. The menu of the day is varied and inexpensive at just 13.95 euros per person including wine. The food is of exceptional quality with lots of options to choose from. There is also a second menu which costs a little more, approx 20.00 euros per person with an even greater choice. Sunday’s is a traditional Roast of Lamb, beef, Pork or chicken for 13.95 euros. So if you are looking for a good to place to dine out look no further. El Rancho delivers on all counts. We hope you enjoy the experience as much as we did.

Agenda 21 is a non-political process whereby inhabitants of a town can have a say in the long-term sustainable development of their community. Jávea has been part of this UN initiative since 2004, but as proceedings are mostly conducted in either Castellano and Valenciano, the authors decided that as more than half the population of Jávea consists of expatriates, it would be useful to set up a Wiki website to disseminate information in English. Compiled by Chris Betterton-Jones and Tim Ladd Port to be re-vamped using part of the Participatory budget The Avenida Lepanto and Avenia Jaume I in the port are to receive a face-lift thanks to the Citizen's participatory budget for 2013. Jaume I will have the pavements widened to 4-5 metres, with angled parking next to the central plaza as well as the section furthest away from the sea, and the elimination of current parking spaces next to the pavements. The trees will be replaced by smaller ones (which cause less root damage) and the lights and pavement will harmonise with those of Avda Lepanto. In the latter street, the pots and central reservation will be removed and parking re-organised into angled parking with spaces for bus and taxi stops. The whole project will cost €120,000, start in April and take some 8 to 10 weeks. From XAD: El Ayuntamiento....

Promoting mountain walking to young people The Centro Excursionista de Xàbia is organising three events to encourage young people to get involved in mountain walking and learn about local geography, landscape and history. On Saturday February 23rd, they'll start at Mas y Mas car park at 9.00am and head off to Cap Prim (about 4 hours), on Saturday they'll visit the windmills on La Plana, meeting at the Sports Centre, and on 13th and 14th April there will be a camping weekend. The activities are aimed at kids aged 10 and over, but younger ones can come if accompanied by a guardian. Xàbia asks State for advance of 5 million Euros The Town Council unanimously agreed to ask the Secretaría General de Coordinación Económica y Local del Estado for an advance of 5 million Euros on its subvention from the State. A cash flow problem has arisen due to the reduction in IBI collected because of the "Catastrazo" and reversion of rateable property values to 1994. This left the town with a 3 million cash deficit at the end of 2012. Most of the Town's taxes will not be collected until October, so an early payment of some of the State subvention is needed to enable the payment of suppliers in the coming few months. Huge savings have already been made through cuts and efficiency savings, and the Town has not had to ask for emergency aid. Mayor Chulvi emphasised that the cash flow problem is not the result of poor municipal administration. From Press Release.

Excursion to Monastery of St. Jerome of Cotalba The Amigos del Museu de Xàbia association (AMUX) is organising an excursion to the Monastery of St.

Jerome of Cotalba (Alfauir) on Saturday 2 March. It will include a guided tour, lunch in a restored 18th century olive oil mill and transport . The Monastery itself is one of the most remarkable and best preserved monastic buildings in the Valencian Community and was declared of Cultural Interest (BIC) in 1994. It is hoped this course will help to promote tourism in the town as a cultural destination.

Through this course Marc will teach impressionistic techniques and how to produce a full range of vibrant colours using only three basic colours, so that the effects created by famous Valencia impressionist Sorolla can be reproduced. There will be courses for three langauge groups: Dutch/English in April, Spanish in May, German in October and French in November.

VALERY: Bueno, cada vez que enciendes la tele o la radio nada más hay malas noticias. (Well, every time you switch on the TV or the radio there’s nothing but bad news ) ¿ Es que no hay buenas noticias en ninguna parte del mundo? (Isn’t there any good news anywhere in the world?)

CATHY: Sí que hay, pero muchas veces no llegan a nuestras pantallas. (Yes there is, but so often it doesn’t reach our TV screens.)

Por ejemplo, el otro día me informé sobre un concurso muy original. (For example, the other day I found out about a very unusual competition.) No es un Miss Mundo, pero igualmente se realiza todos los años desde 2004. (It’s not a Miss World, but it has taken place every year since 2004.) Tiene un jurado y un ganador pero este evento es un poco diferente. (It has a jury and a winner but this event is a little different.) Una organización, con sede en Londres, empezó un concurso en Internet que busca, nada más y nada menos, que encontrar el mejor alcalde del mundo.

Fund raiser for victims of domestic violence

Letter From Chris Duffin president of WICB Oscar Anton, the Deputy Mayor of Javea, was so impressed with the work we were doing with victims of domestic violence that he offered to do a fund raiser for us. He has arranged a charity lunch at the Botanica Restaurant (above Achill on the Arenal in Javea) for Saturday the 16th February at 12.30pm.

They are hoping to get 100 people to attend at the cost of 20 euros per head and 5 euros of that will be for us. They are also planning a raffle with the first prize being a sculpture by the man who does the metal sculptures for the roundabouts and other amazing prizes. Members of the Javea women's group MEJ will also be there and it is open to all.

The Council now offers the possibility of arranging a personalised payment plan to allow payment of municipal taxes on a monthly basis. An estimated total for 2013 taxes is calculated and a payment structure agreed. Once the voluntary payment period for IBI opens and the definite amounts are known, an email will be sent or alternatively a message to a mobile indicating the shortfall payable resulting from the difference between the estimated figures and the real ones.

It is also possible to pay your 2013 IBI bill (estimated) in advance, at the Attention to the Citizen office (where the Help Desk is situated) and receive a discount: the sooner you pay, the higher the discount percentage .. those who paid before the end of January for example received a 4% discount. It is possible to use a credit/debit card to pay bearing in mind that debit cards have a limit of 900€ and taking into consideration that it may be necessary to introduce a pin nº and/or receive a payment authorisation

Hi Everyone!

Continuing with the previous posts theme, let’s talk now about one of the evident differences with previous versions of Windows: how to turn it off!!

Before it was easy, just clicking the Start button we could see the option to turn off our system, but this option has disappeared now. So, all very good looking, but how do we shut ‘the thing’ down?

Well To shut Windows 8 down, just move the mouse cursor to the bottom right corner of the screen, click the Settings icon - or just hold down the Windows key and press I - and you'll see a power button. Click this and choose 'Shut Down' or 'Restart'. Also, some of the tricks available in previous versions of Windows still apply.

Press Ctrl+Alt+Del, for instance, click the power button in the bottom right-hand corner and you'll be presented with the same 'Shut Down' and 'Restart' options. And last, if you're on the desktop, press Alt+F4 and you'll be able to choose 'Shut Down', 'Restart', 'Sign Out' or 'Switch User' options.

Now that we are getting familiar with our new Operating System, we might be interested in knowing what programs we have running at a certain point. For example, if you launch a Windows 8 app, play with it for a while, then press the Windows key you'll switch back to the Start screen. This app will still remaining running ‘in

the background’, but as there's no taskbar any more, then you might be wondering how you'd ever find that out. Again, you can press Alt+Tab, which shows you what's running, just as it always has or by holding down the Windows key and pressing Tab displays a panel on the left-hand side of the screen with your running apps. With the mouse you can also access this panel; simply move your cursor to the top left corner of the screen, wait until the thumbnail of one app appears, and then drag down.

Now we know how we can find out if we have apps or programs running on the back ground, we might realize that Windows 8 apps don't have close buttons, but this isn't the issue you might think. Apps are suspended when you switch to something else so they're only a very minimal drain on your system, and if you need the system resources then they'll automatically be shut down. Don’t worry about this, even though it sounds a little scary; their context will be saved so when you decide to launch them again, they'll carry on exactly where you left it.

Still, if you want to close down an app anyway, move the mouse cursor up to the top of the screen. When it turns from the regular mouse pointer to the icon of a hand, hold down the left mouse button and drag it down the screen. Your app should shrink to a thumbnail which you can drag off

the screen to close it. If you find that is complicated (it takes a while until you get the hang of this procedure), then simply press Alt+F4; it still works!!

And when all else fails then press Ctrl+Shift+Esc to launch Task Manager, rightclick something in the Apps list and select End Task.

Beware, though, close something you shouldn't and it's easy to crash or lock up your PC. Now here is a clever tip: Windows 8 apps are what Microsoft calls "immersive" applications, which basically means they run full-screen - but there is a way to view two at once.

For this, swipe from the left and the last app you were using will turn into a thumbnail; now drop this and one app displays in a sidebar panel while your current app will show in the rest of the screen. If you need to have better view or access to the app that is displayed in the panel, you can then swap these by swiping again.

Still, if Windows 8's search and navigation tools still leave you pining for the regular Start menu, installing external software like ‘ViStart’ or ‘Start 8’ will replace it with something very similar.

Wih this, the old Start button will return in its regular

place, and clicking it (or pressing the Windows key) will bring back the usual Start menu complete with search box and all the usual menus.

This lamb rosemary stew recipe offers the classic combination of lamb, rosemary, and garlic, which work so well together. Serves 4.

Ingredients: 2 tablespoons of olive oil 1/2 lb of lamb, cut into 1 inch cubes 1 large potato, peeled and cubed 1/2 onion, thinly sliced 3 cups of chicken broth 2 celery sticks, chopped 4-6 mushrooms, sliced 2 teaspoons of fresh rosemary 1/2 teaspoon of fresh thyme 1/4 teaspoon of oregano 1/4 teaspoon of sage 1/4 cup of red wine Cooking Instructions Heat olive oil in cooking pot. When hot add the onion, celery, and mushrooms. Cook the vegetables until soft. Remove the veggies from the cooking pot and set aside.

Cut the lamb into 1 inch cubes.

Heat the remaining oil in cooking pot and add the lamb. Saute the lamb cubes over a medium heat until browned on all sides. Add the cup of water and continue to cook the lamb stew for approximately 1 hour, stirring occasionally. Add the hot chicken broth and continue to stir.

Transfer the cooked vegetables back into the stew mixture. Add the rosemary and other herbs, potato and red wine, and continue to cook over a low heat and simmer for an additional 1 hour, until the meat is soft and developed flavor.

Serve the lamb stew immediately while warm with bread.

Six of us enjoyed a wonderful meal there on 31st December and we remain of the opinion that Benjamin's move away from front-line tourism (at Arenal) will enable his reputation better to spread through the resident community and especially to offer their excellent creative French-based cuisine to the Montgó area and Jesús Pobre. SIDNEY, male, shelter since 31.12.2011 Crossbreed, DOB 15.11.2010 – 52 cm What a bad luck! After being in the pound for a while, people adopted him, although we told them that Sidney was a very active dog that would need some training. After not even a week, he was back again as he indeed was too active, did not listen to commands and climbed over the fence. Without even trying to train, they brought the poor dog back. He is now totally confused and unhappy. If you are willing to give him a home, train him (which is surely possible, as he likes to work with people and is so young), and want to keep him for longer than a week...please come to the pound. We are more than happy to tell you more about Sidney and give you training advice.

Sidney walks very well on the lead and is a good and social boy.

EVENTS. Lots of APASA dogs and personal pets had the opporunity of being blessed at the Shelter on San Antonio Animal Day. Priest Clive Reed came along on a lovely sunny day and gave blessing to all the animals.

Conservatives Abroad, Jávea The visit of Nigel Evans MP last weekend coincided with the Javea Conservatives Abroad AGM Lunch at which he was the Guest Speaker.

Although this was a private visit to Spain, Nigel still found time to speak to members of the Branch and shared many anecdotes on his life as Deputy Speaker of the House of Commons.

SATELLITE KNOW HOW

Richard Pollock Consultant Electronic Engineer Satellite television reception has certainly improved of late and the demand for the larger 2.4 metre Satellite Dishes has fallen as for almost twelve months we have enjoyed much stronger reception of the U.K. National Stations including BBC, ITV Chanell Four and Five. This means that many viewers are considering their position, quite a few have cancelled local based suppliers of Cable or Beam as its now possible to purchase a Satellite System at a very competitive price for around 250 to 350 Euros and this includes a one Metre Satellite Dish, LNB, H.D. Satellite Receiver together with a wall mounting bracket, cable and full Installation. With such a system you are able to receive the U.K. National T.V. including H.D. and Radio programmes, Sky News and many of the Free to View programmes including several Film Channels. Quite a bargain, no more monthly payments which over a few years represents quite a saving.

When considering the purchase of one of these Packages its best to check out exactly what type of Satellite Dish and LNB is on offer, after examining some types I have noted that the difference in quality is quite surprising, some are of a very flimsy construction and will easily buckle in very high winds and also the wall mounting brackets again leave much to be desired. There are many LNB’s on the market and again require careful selection, I myself in ensuring that only top quality types are available, same goes for the H.D. Satellite Receivers, I make sure that they are of High Quality and also include an automatic Card Reader, thus anyone who wishes to upgrade to Sky reception can do so. For anyone who has a holiday home, then they can bring their Sky Card and use the Sky facility while here in Spain.

Peter, Geoff and I have examined several systems that were giving poor results and on each occasion it has been noted Satellite Dishes were of flimsy design, that the cheapest coaxial cable had been used, wall mounting brackets had twisted and the actual fixing bolts were of poor design and on occasion the wrong type had been selected, bearing in mind often we are dealing with hollow ‘Blockies’, it is possible to supply special types for this kind of problem, they are not expensive and so it begs the question,why not use the correct type ?

As usual you are in the hands of the Installer !!!

Installations The most important part of an Installation is to examine the proposed site in detail, we always try our best to keep away from an Installation requiring us to fit on the Chimney Stack (no we are not at all scared of heights, people who know our background will know why!) its simply that its always wide open too the elements and we do get some massive storms from time to time, another problem, is walking on a Spanish roof nearly always means several Tiles are cracked, unless you weigh around six stone! also this usually means a longer cable run, this increasing the signal losses en route to the Satellite Receiver. The signal we are interested in is Astra Transponder Family at 28.2 degrees South, South West (roughly in the direction of the Sun at 11 A.M. in a morning)

if we can we try our best to fit to the South facing wall, another benefit being protection against those nasty Northern winds. There is no advantage in fitting the Bracket to high, the signal is coming down at an angle of some 36 degrees and unless you have huge Trees or an Apartment Block in close proximity all should go well. If we are ever in doubt about reception, then we call on an ‘App’ in our iPad which allows us to site the exact position of the Satellite Transponder and at the same time gives a visual picture on any unforeseen obstructions on screen, a very useful piece of kit. We prefer to fit some 3 metres (beyond head height!) or so above ground level, this means we can concentrate on getting the adjustments to reflect the absolute best possible readings across the Spectrum.You will appreciate that as the signal is some 26,000 miles away, a slight incorrect adjustment, one millimetre means that you are missing the best possible settings by several hundred miles! Our reasoning on this is very simple, when we are suffering from a dip in the weather we require the best possible signal and even then, in a very heavy rainfall its still possible to loose many of your signals if not all. This is when those having invested in larger sizes of Satellite Dishes gain the benefit, however if conditions get really violent then we all have to get the Card Table out !!!

Assuming we have located an agreed placement for the Satellite Dish then we must mark off the drilling details and then start with an exploration hole? In Spain we are often up against Blockies (hollow concrete Blocks), very brittle Bricks that should you use Expandable Bolts will often crack resulting in a loose fit. For some time we have used a special type Expandable type, with a very strong plastic covering, when tightening up the Bolt tip pulls back, forcing the plastic cover to expand and get behind the Blockie ensuring its impossible to pull out the Bolt, they are not expensive and well worth using. Having secured the Bracket its time to clip the Satellite Dish into place and then we make very important adjustments to both the Dish and the LNB, using a quality LNB guarantees maximum performance, we are very fortunate in having some superb Test Equipment and guarantee our settings are spot on and have the added facility of a Spectrum Analyser to confirm this.

We always use a high specification type Coaxial Cable CT100 or equivalent this ensures minimum losses from LNB to Satellite Receiver. Care must be taken when laying cable and we leave a service loop and of course where the cable enters the property we allow for a drip loop external to the entry hole to ensure no water can run through the cable and of course we in addition seal round the entry hole. Inside the property,if possible we drill behind the selected position for the Satellite Receiver,however on occasion this is not possible and if that is the case we find that securing cables by using a Glue Gun is often a much better method of ensuring that no damage occurs to tiles,plastered walls etc.

Alicante Expat Statistics Expanding Business in Jávea at the height of a recession is a risk but maybe now is the time to consider it a risk worth taking.

Business premises are plentiful, properties are starting to move though prices are still low - most of us surviving here from the beginning of the recession are hoping to stay, rather than trying to leave… so we are finally a definable market and will soon be a quantifiable population.

It will be interesting to see the statistics prepared by the Town Hall based on the recent renewals to the Padron.

The most recent figures in the INE (the Spanish Office of Statistics) are 2011 - the province of Alicante had 92,089 ex-pats born in the UK registered as resident in the region.

There were

3,207 aged 14 or less 2,370 aged 15-19 So 5,577 who were 19 or younger 4,694 aged 20 - 34 12,321 aged 35 - 49 29,891 aged 50 - 64 So 49,206 adults up to the age of 64 34,953 aged 65 - 79 4,626 aged 80 - 94 97 aged 95 plus ( inc 7 aged over 100) So 39,676 aged 65 or older Surprisingly that means that the majority of expatriates settled in Alicante province in 2011 and registered on the Padron were working age or younger - with those 65 (or over) forming the minority of settlers. Of course, those statistics do not take into account those adults taking an early retirement to enjoy the Mediterranean lifestyle, but nor do they exclude the possibility that some of the over 65 population continues to work and earn.

This means that creating a business to cater for British consumers living in this area needs to consider families and working couples as well as catering for those in retirement.

It means that lifestyle requirements will not just be for quiet bars and pleasant restaurants, but needs to cater more for family experiences and family “down-time”. This is not a suggestion that we all rush out and open new tourist attractions - but more that, maybe, there could be more here in the way of consumer goods and services that reflect the needs of a younger population than most UK pundits predict come to the Costa Blanca to live.

Dr. Christoph Semler qualified as a doctor at the University of Frankfurt and then specialised in General Practice in the UK. He is a Member of the Royal College of General Practitioners, holds a Diploma from the Royal College of Obstetrics and Gynaecology, a Diploma in Child Health Surveillance. He opened his private practice in JĂĄvea in 2001.

Dr Semler is fluent in English, Spanish and French and has a working knowledge of Dutch.

Dear doctor, I am very uncertain about my situation regards screening for prostate cancer. My wife and I live here but often see our GP in the UK who did not want to do a prostate test on me when I asked for a screening test.

He said the NHS would not consider it useful any longer. I had had this test now quite a few times and so far the result was always normal. I am 73 years of age and am just suprised that my GP sent me away without doing the test. Is he right in what he says? Is it still sensible to be tested?

Thank you for your advice.

Dear Malcom from Benitachell, this is a tricky situation. On the one hand you have the NHS saying we will not screen you for prostate cancer any more. On the other hand there is you as the individual person who wants to do something to prevent ill health to himself.

It is true what your GP says, the NHS as the goverment funded organiza-

tion does not do prostate screening any longer as it used to be done by measuring the PSA concentration in your blood. The cause given for this is that it does not really reduce the number of men who die from prostate cancer.

And after all that is the whole point of a screening programm. To detect a state of ill health before the concerned person becomes aware of it and when it is still easily treatable. All this without being harmful to the person tested and within certain economical constrains.

Over the last 20 years and more the PSA test has been offered to many men, it was well accepted and became to be part of a normal medical check-up.

Lately however the medical opinion has been changing on the value of prostate cancer screening.

It has been shown that though it does save lifes, it does do so at an enormous cost. Not only in terms of money but also in terms of human suffereing. In most men with a raised PSA

reading there is no cancer but they still need all the further work-up to really come to this conclusion.

That means scans and finally a biopsy from the prostate. In the time it takes to organize the scan and biopsy men waiting for theses tests go through a period of unertainty and often worries or even grave anxiety. Most often than not only to be told that there was nothing.

And even should cancer have been found, it is still not quite clear how and when to start treament.

Because of all these uncertainties the NHS has decided not to use the PSA blood test any longer to screen apparently healthy men for prostate cancer.

This might well be a reasonable decision from the side of those who pay for the screening. It is also important to consider the amount of unnecessary suffering caused by false positive screening results.

And even more important it is to take into account the quite undesirable effects an operation for prostate cancer thus detected can have on men who do indeed have the cancer but would never have died from it.

That does not mean that on an individual basis it might well be a very good decision to have oneself checked out for prostate cancer. After all it is the second most common cancer for men and the sixth leading cause of cancer death. It is particularly important to screen if there are first degree family members affected, that means brothers or fathers.

But instead of just throw-

ing the screening test at anybody walking in the doctor doing it should explain the pros and cons.

In particular it is important to point out that a normal result does not automatically mean there is no cancer. The test can be blind and give a sense of false security. At the same time a positive result is not a one way road to prostate cancer. More often than not a positive result is what is called false positive, showing cancer where in reality there is none. It is the doctor´s duty to explain these limitations of the prostate test to his patient. It is also important to give an idea to the patient about what awaits him if the result is indeed positive.

No doctor can refuse to provide the PSA blood test. If your GP did not want to do it for you then at least he ought to refer you to somebody who does, that would be a urologist.

Alternatively, have it done here.
