While he was cheering on the sidelines at his stepdaughter’s soccer game, things suddenly went dark for Duvall resident Mike Snyder.

A debilitating headache, followed by tunnel vision and then complete darkness coupled with a flashing strobe-light effect in his left eye, left the young, otherwise healthy man grasping for what could possibly be wrong. “Am I having a stroke?” Snyder thought in those first few moments of darkness. Finally, after 20 excruciating minutes, his vision returned and the pain subsided, but he was left wondering what he had just experienced. “I knew something was off,” explains the now 35-year-old, and he immediately scheduled an appointment with Dr. Kristi Moffat, his primary care provider at EvergreenHealth Primary Care in Duvall. After a series of inconclusive tests, his care team referred him to a neurologist for what could only be described as severe migraine headaches.

But thorough examinations, followed by a battery of MRIs and other tests, and treatments including injections and other medications, couldn’t touch Snyder’s headaches. The nearly paralyzing darkness, stabbing pain and flashing strobe persisted. The episodes would come unannounced, often multiple times a day, and the young father feared he might be faced with living with this inexplicable pain indefinitely, or worse. “I couldn’t sleep and I was having a lot of anxiety about what was happening to me,” Snyder explains. “I was living in fear that any of these headaches could be the end.” After a full year of uncertainty and continued pain, Snyder sought help from Dr. Raymond Park, a board-certified specialist in both neurology and sleep medicine at EvergreenHealth. As the two consulted about Snyder’s health history, Dr. Park probed for any triggers that could explain Snyder’s insomnia and the sudden onset of his headaches. Their discussion eventually led to the story of a head injury Snyder had suffered at work several years earlier. A commercial plumber, Snyder was on a job as a first-year apprentice assigned to testing the seal of industrial castiron pipes his team was installing. As he was working, the dense rubber balloon that plumbers use to test a system for leaks exploded and shot like a missile out of the pipe, ricocheting off Snyder’s face. Sirens blared as Snyder was rushed by ambulance to the

emergency department, where providers determined the projectile had crushed his sinus cavities. After surgery and a few months of rehabilitation, Snyder was on the mend and back to work, left with minor lingering affects and a facial tenderness that eventually subsided. Despite Snyder’s full recovery from the accident, as he recounted the details, Dr. Park concluded that it was likely he had suffered a traumatic brain injury (TBI). “Insomnia is very common for people who experience a traumatic brain injury, but the type of headache Mike was having is not what you typically see in people with a TBI,” Dr. Park explains. “Mike also explained symptoms that are unusual for migraines – that frequent stabbing and shooting pain. Those things really stood out and helped us identify a better path to treatment.” Dr. Park’s assessments revealed that Snyder’s headaches seemed to localize to an area that is covered by the trigeminal nerve, a system of nerves in the head that conduct sensations from the face to the brain. Medications for treating a trigeminal nervoussystem injury are different from those traditionally used to treat migraines, according to Dr. Park. With these things in mind, Dr. Park diagnosed Snyder with TAC, or trigeminal autonomic cephalgia, and he prescribed a newly recognized drug called lamotrigine to treat it. Almost instantly, the headaches were gone.

Understanding Sleep’s Role in Neuroscience Whether we’re getting enough of it or not, sleep takes up a large portion of our 24-hour day and lifetime.
“I feel 100 percent better,” Snyder says. “After living with this for so long, it was such a relief to finally get some answers.”
Snyder continues to take a daily dose of lamotrigine – a prescription that costs him just a few dollars a month. “It’s really incredible. I do still get dull headaches from time to time, but it’s nothing close to the pain I had before,” Snyder says. “We’re just starting to understand that TAC can present very differently in patients and often mimics migraines, which makes it tricky to diagnose,” Dr. Park explains. Luckily for Snyder, the treatment is working and he’s back to doing the things he loves – being active outdoors and spending time with family – which he attributes to Dr. Park’s patient-centered approach. “He really took the time to listen, addressed my concerns and gave me the information I needed to get better. You can just tell – he has a special individual relationship with his patients.”
But those hours often get overlooked, according to EvergreenHealth sleep medicine specialist and general neurologist Dr. Raymond Park. “When we investigate patients for neurologic disorders, we often don’t pay enough attention to that large chunk of time we spend sleeping,” he explains. Dr. Park is uniquely qualified to opine on the matter, as one of the few physicians in this area doubleboarded in both neurology and sleep medicine.

Partnering for Parkinson’s Research
A
As both a biologist and an attorney, Riley Atkins has a keen ability to understand complex issues and relate the relevant details to people who need to be in the know. When the 59-year-old Mercer Island resident was diagnosed with Parkinson’s disease nine years ago, it was a natural extension to apply that same analytical approach to learning about the condition and to find ways to continue living his active lifestyle. “I’ve always been interested in the study of biology, and it was important for me to understand the science of the disease,” Atkins says. “Knowing and understanding the genetic components of Parkinson’s is an important way for me to stay engaged in life.”

PAIR’s goal is to encourage greater participation among people living with Parkinson’s in clinical trials and research to help bring about better treatments for the disease at a faster pace. It does this by equipping people affected by Parkinson’s with the knowledge and skills to connect directly with the scientists and health care providers on the front lines of research.
“As patients, it’s in our best interest to participate in research and help inform how it’s done. We stand to benefit the greatest by advocating for more participation in clinical trials and engaging in the research,” Atkins says.
Atkins’ interest in being an active participant in his own treatment was evident to Dr. Daniel Burdick, a Parkinson’s and movement disorders specialist at the Booth Gardner Parkinson’s Care Center at EvergreenHealth.
One of the biggest barriers to developing new treatments for Parkinson’s is low participation in clinical trials, Dr. Burdick explains. According to the Parkinson’s Disease Foundation (PDF), it’s estimated that less than 1 percent of people with the disease participate in clinical trials.
“From our very first meeting, it was clear Riley was eager to engage in understanding and treating this condition,” Dr. Burdick recalls.
According to PDF, it can take up to 15 years to develop a new treatment, due to low participation.
It’s what prompted Dr. Burdick to share a unique opportunity for Atkins to tap his passion for research while helping others living with Parkinson’s to become advocates for pursuing advances in treatment options. Dr. Burdick recommended Atkins for the Parkinson’s Disease Foundation’s Parkinson’s Advocates in Research (PAIR) program, which seeks to bring together the people who live with Parkinson’s with the researchers who are developing new treatments.

“We want to work toward shortening that process in order to help get effective drugs out sooner,” Atkins says. One of the benefits of the PAIR program is the peer-topeer rapport patient advocates can develop with other people living with Parkinson’s in a way that health care providers simply can’t, Dr. Burdick says. As part of the program, Atkins talks with other patients to encourage them to participate in research and clinical trials. “It’s a way for us to take control,” he says.

While he actively participates in the Booth Gardner Parkinson’s Care Center’s clinical trials today, Atkins was successful in managing his symptoms drug-free for five years after he was first diagnosed, and he continues to maintain a balanced diet, keep up with daily exercise and reduce stress – all of which have been shown to ease the effects of Parkinson’s.

Finding Balance: A Journey to Wellness
S
Seven years ago, Seattle resident Kari Fillipi received a life-altering diagnosis – multiple sclerosis. Since that unforgettable spring day, Fillipi has been on a journey to balance personalized care, spiritual healing and full-body wellness – a journey that includes health care partners at EvergreenHealth. “From the very beginning, I set my course,” explains Fillipi. “For the first few months, the diagnosis and what was portended for my future was all-consuming, but once I made peace with it, I found my new normal.” The autoimmune disease damages the nerves of the brain and spinal cord and can lead to impairment of muscular coordination, blurred vision and severe fatigue. At the time of her diagnosis, Fillipi initially pictured a future bound to a wheelchair, but the now 42-year-old soon realized that didn’t have to be her reality. Emotions and spirit play into how our bodies heal, she explains. Understanding her diagnosis and getting over her initial fear set her on a positive path – she refuses to let the diagnosis define her life and continues to live wheelchair-free. Twice a week,, she practices at a Seattle y yoga p g studio that focuses on integrated movement therapy, where yoga’s philosophical, physical and spiritual framework comes together
with a conventional neurophysiological perspective. During her journey with MS, she’s authored about 60 haikus, which she credits for much of her healing and strength.
“Kari is a great example of someone who is finding balance. She has a good wellness program, she practices yoga, eats a nutritious diet and manages stress,” says Dr. Ted Brown, Fillipi’s MS specialist at the MS Center at EvergreenHealth. Fillipi’s holistic approach, he explains, is part of her healing process. “An MS diagnosis is for life, so it was very important for me to find a provider who would not only be my doctor but also work collaboratively to achieve an ongoing care program that balances medical and scientific expertise with my desire to incorporate a holistic approach.” Fillipi speaks highly of Dr. Brown. He takes time to listen, puts himself in her shoes and highly regards her opinions and wishes, she says. EvergreenHealth Foundation News Evergreen Gala: A record-setting night raises $1.25 million to support Acute Rehabilitation Unit The EvergreenHealth Foundation extends its sincerest thanks to the 700 community leaders, patrons of EvergreenHealth, leading physicians and executives from top local companies who joined in raising $1.25 million to support patients served by EvergreenHealth’s Acute Rehabilitation Unit. This incredible fundraising success is a new record in the 13-year history of the Evergreen Gala. Gala proceeds will assist with the development of a specialized “home transition” apartment that will simulate the environment patients must adjust to once they return home, as well as provide support for additional program upgrades to enhance the therapeutic and facility environment. The intent of an acute rehabilitation stay is to prepare a patient for their transition home following hospitalization or a serious illness or injury that has led to impaired function. The ARU’s 24/7 medical monitoring and intensive, multidisciplinary rehabilitation provide equal parts safety and coordinated care, ensuring that patients return home with the training and confidence necessary to flourish in their home environments.
Freedman Family Foundation Creates Home Transition Unit Guests at the 2014 Evergreen Gala celebrated the presentation of the EvergreenHealth Foundation’s annual Spirit of Giving Award to the Gene and Ruth Freedman family. This award is part of EvergreenHealth’s ongoing commitment to recognize those whose selfless contributions are crucial to the health and well-being of our community. Members of the Freedman family have served the community over multiple generations, giving freely of their time and talents serving many local nonprofit agencies. Rick Freedman, Gene and Ruth’s son, then made a very special announcement from the stage – the Eugene Freedman Family Foundation’s incredibly generous gift to name the new home transition apartment in honor of Gene and Ruth. Gene and Ruth each benefited from EvergreenHealth’s Acute Rehabilitation Unit. Gene, one of its first patients, leveraged the ARU’s intense therapy to accelerate his return to work following hip replacement surgery in 2003. Ruth received care in 2010 following major hip surgery. Thanks to the Freedman family’s gift, and borrowing Gene Freedman’s signature phrase, “the best is yet to come” for ARU patients at EvergreenHealth.
