Keeping Promises: Partnering With the Community to Live Our Healthiest Best 2013 Community Annual Report EvergreenHealth & EvergreenHealth Foundation
Message From the CEO and Board Chair Greetings, Eight years ago, we stated and set a new vision for EvergreenHealth, a vision to “create an inclusive community health system that is the most trusted source for health care solutions.” We have come a long way toward fulfilling that vision since that time, and in keeping a promise we made to this community 42 years ago to “work together to enrich the health and well-being of every life we touch.” Each year we share with our community a report on how we are doing in keeping that promise and the vision we set. The stories enclosed in this, our 2013 Community Annual Report, reflect the highlights of our work to earn your trust every day.

The membership-based practice, featuring a low patientto-provider ratio, is modeled to connect patients directly with their providers.
Specialty care expansion In 2013, EvergreenHealth increased access to a broader range of clinical services including vascular care and surgery; comprehensive neurological services in partnership with Virginia Mason; enhanced care and services for memory disorders; colon and rectal care; diabetes and endocrinology care; kidney care; orthopedic and sports care; women’s gynecological care and obstetrics; and sleep care with the addition of new durable medical equipment.

Community Health Education Services Community Health Education Services fulfills a key promise from EvergreenHealth – to serve the community with access to health care and resources outside the traditional health care setting. While some levy-supported programs specifically address the needs of the underserved, the chronically ill, people with disabilities and highrisk populations, the majority of the programs are for use by the entire community, and these out-of-the-box solutions can be lifesaving. Sixty-six-year-old Anita Berosek says that EvergreenHealth’s Community Health Education Services saved her life. In June 2013, the Snoqualmie resident attended the Healthiest Best Fair at EvergreenHealth’s main campus in Kirkland. Community Health Education Services had identified a need for increased access to cancer screenings and mammograms and, through a partnership with the Breast Health Center, ensured the availability of EvergreenHealth’s mobile mammography coach for fair-goers. EvergreenHealth staff saw Berosek for a walk-in mammogram at the fair – her first in several years. A week later, Berosek got a call – the 3-D mammogram had turned up something suspicious. It turned out to be an aggressive cancer that required immediate treatment, but Berosek was fortunate that the mammogram had caught it at an early stage. Within a month, she had a mastectomy and was cancer-free. “If I hadn’t gone to the fair that day, I probably wouldn’t have gone in for a mammogram for another year,” she says. “Who knows what would have happened then?” Community Health Education Services supports the annual Healthiest Best Fair, a family-friendly event that welcomes people at any level of health. Through healthy-cooking demonstrations, free screenings and health, safety and prevention tips, the fair gives families the tools they need to live safer, healthier and more active lives. 8 | EvergreenHealth Community Annual Report 2013
In addition to the Healthiest Best Fair, Community Health Education Services supports: • Safe Kids Eastside, a local coalition dedicated to preventing unintentional injury among children. • Year-round classes and programs that explore topics in nutrition, physical activity, wellness, CPR and first aid. • Community health tools including regular car seat checkups, helmet giveaways and medication safety resources.
Community Grief and Bereavement Services While each person deals with loss differently, Community Grief and Bereavement Services at EvergreenHealth is there to provide support and education for anyone in the community who has experienced a loss through death. In 2013, the Grief and Bereavement staff supported over 3,000 community members, including Sarah Renz. For the 32-year-old, the services meant she could get her life back.
“I would not be where I am today without this program.”
After dealing with the death of her father several months earlier, a loss that hit particularly hard on Father’s Day with her wedding just a few weeks away, Renz realized she needed help. “I didn’t want to feel that way anymore,” she explains. Renz started seeing Mary Hejnal, a Bereavement and Family Support Specialist with EvergreenHealth’s Community Grief and Bereavement Services. “Mary helped me navigate my grief and come out on the other side. She’s helped me make progress in integrating my loss into my life – I’m not getting away from it or getting over it, but I’m learning to manage it and live with it.” “The death of a loved one can be life changing,” says Hejnal. “Each person’s grief is unique. Our program offers emotional support while helping individuals and families begin to adjust to their loss.” Grief and Bereavement Services is levy funded and offers consultations and support groups for community members at no fee. Renz’s journey has included one-on-one consultations and participation in the 6-week Adult Bereavement Series. “It’s a safe place where you can be open and honest with yourself about how you are feeling. No one will judge you or put you on a timeline. They validate what you are going through and make it OK so you can start to move through it,” explains Renz. Levy-funded programs connect the community to essential resources In addition to supporting many of the programs and services featured in this year’s report, levy funds support a comprehensive range of community resources, often filling a gap for patients, residents and family members in need of care and services beyond clinical care. EvergreenHealth’s levy-funded programs include: • Community Health Education Services Educational activities and events promote topics related to health, wellness and injury prevention that are most needed and most appealing to the community. | “I love learning all the great things the classes have to offer.”
• Safe Kids Eastside Safe Kids Eastside is a local coalition dedicated to protecting children from unintentional injury – the number one cause of emergency room visits in East King County. | “The sessions were extremely helpful and were fun as well as informative.”
• Horizons EvergreenHealth offers senior health and wellness resources that encourage and support a healthy, happy and injury-free lifestyle at locations conveniently located throughout the community. | “The best part of the program was learning preventive concepts, knowledge and information critical for seniors.”
• Community Grief and Bereavement Members of our community who have experienced the loss of a loved one have access to complimentary education and support services including individual consultations, support groups, workshops, seasonal events and bereavement resources. | “It was a safe place to share my story and express my feelings after my husband died. • EvergreenHealth 24/7 Nurse Navigator & Healthline This free consulting nurse and referral service is available to all community members and connects callers to registered triage nurses who can help determine the appropriate level of care for a range of health care needs. | “It’s wonderful to be able to call and speak with a nurse when one needs to, especially on weekends and at night.”
• Kirkland Senior Council This dedicated group advocates for community programs and services that meet the needs of adults age 50 and over in the community, often making recommendations to the Kirkland City Council on matters affecting Kirkland seniors including fitness, pedestrian safety and elder abuse. | “Support from EvergreenHealth helped contribute to highly successful events and resources which has helped us reach out to our older adult community.”
• Kirkland Steppers Organized walks promoting health and wellness for walkers age 50 and beyond take place in downtown Kirkland twice a week throughout the summer months. | “I love Steppers, it is one of the best parts of coming to PKCC. I meet new people and I highly value the walks.”

In 2013, all CHAT staff became Certified Navigators to help community members sign up for health insurance under the Affordable Care Act. | “Thank you so much. I could not have done this without your help!”
• Partnerships with schools EvergreenHealth partners with local school districts to provide mobile health care resources for underserved students, in addition to providing STEM educational curriculum and career development opportunities for all students. | “There is not a doubt in my mind that I would not be the student and person I am today without this program and without these teachers.”

Board reduces taxpayer obligation by $9 million In 2013, the EvergreenHealth Board of Commissioners reduced taxpayer obligation in King County Public Hospital District No. 2 – EvergreenHealth – by more than $9 million over the next 10 years by refinancing tax-supported bonds issued in 2004. Thanks to historically low rates, the interest rate more than halved from nearly 5 percent to less than 2 percent, which reduced the total debt from $89.7 million to $80.4 million. The bonds were primarily used to build the Silver Tower on the EvergreenHealth Medical Center campus in Kirkland which houses EvergreenHealth’s emergency department, trauma services, the core part of the Halvorson Cancer Center and patient rooms.

Hours of volunteer service EvergreenHealth Community Annual Report 2013 | 13
Dear friends, Every year, we are inspired by the contributions of our generous community. In 2013, some of the most significant work in the history of the EvergreenHealth Foundation came to fruition. We surely would not have accomplished so much without the trust and dedication of our volunteers, donors and community leaders – your tireless efforts push us to set ambitious goals to meet the community’s health care needs in groundbreaking new ways. Most notably, as a result of the EvergreenHealth Foundation’s capital campaign, the Elling & Barbara Halvorson Cancer Center opened in March 2013, providing unparalleled cancer care for our patients and their families.
FROM LEFT TO RIGHT: MAJDI DAHER, BOARD CHAIR; KAE PETERSON, VP & EXECUTIVE DIRECTOR
Our 2013 gala raised a record-breaking $1.2 million to benefit the EvergreenHealth Family Maternity Center and Neonatal Intensive Care Unit. Specifically, funds raised supported the work of the highly skilled team of professionals who serve critically ill and extremely premature infants during their vital first hour of life, known as the “golden hour.” Gala proceeds funded expanded space and new technology that is required to care for these special babies as they enter the world. In the fall, the community stepped up once again for women’s health services at the Bellevue Neiman Marcus for an exclusive fashion event to fund screening mammograms for uninsured women. In November, the fifth annual Seasons of Hope luncheon raised $103,000 to support EvergreenHealth’s Hospice and Palliative Care Programs, including uncompensated care, palliative care consultations and the Gene & Irene Wockner Hospice Center, as well as many complementary therapies that include options for both patients and their families. The successes of the past year would not have been possible without the generosity and support of our amazing donors, volunteers and selfless members of our growing community. With your continued trust and partnership, we look forward to doing great work together in the years to come. In thanks,
Majdi Daher Board Chair EvergreenHealth Foundation

Suzanne Lewin Creates Legacy Suzanne Lewin grew up in the 1930s in the Bronx. Her mother was a third grade teacher who imparted her love of education to her only daughter. Suzanne had tremendous respect for her mother, who died from Parkinson’s disease in the 1970s. Suzanne saw the difficulties her mother faced and, at the time, how difficult it was to access supportive resources. It then became her personal mission to study the progress and treatment of patients affected by this disease. Through years of investigation she decided she could make an impact on Parkinson’s patients and decided to make a generous gift in her will. She designated 20 percent of her assets to create an endowment fund in memory of her mother, Henrietta L. Lewin, to help the clinical program and patient services at the Booth Gardner Parkinson’s Care Center at EvergreenHealth.
The EvergreenHealth Foundation gratefully acknowledges the following commitments to the Legacy Society: BENEFACTOR ($100,000-$499,999) Suzanne L. Lewin Estate
DIRECTOR ($25,000-$49,999) Wilber Curtis Mann Estate

Suzanne passed away in 2013 under the care of the EvergreenHealth Hospice Care Center, and her personal legacy has created an endowment of nearly half-a-million dollars that will help many generations to come in the Puget Sound area.
