In 2012, we reflected on 40 years of service to the EvergreenHealth community. We marked our 40th anniversary with a celebration and recognition to honor the providers, staff and volunteers who have helped shape our hospital into the leading-edge medical center and nationally recognized healthcare system it is today. Now, as we embark on our next decades of service, we renew the commitments we made to past generations and look ahead to our promise to the next generation—a pledge to help prepare the members of our growing community for a lifetime of living as their healthiest, best selves. We embrace our commitment to serving our community, both as a privilege and as a great responsibility. 2012 Highlights Strategic partnership with Virginia Mason Medical Center EvergreenHealth and Virginia Mason Medical Center created a strategic partnership designed to allow both organizations to collaborate to create new, patient-centric programs and services that build on the strengths of each organization. The collaboration creates a broader network of care in the Puget Sound region and provides greater value to patients, employers and the communities served by both organizations. By partnering, Virginia Mason and EvergreenHealth will work together to create the highest quality of health care services in the most cost-effective manner, without costly duplication of services, and without merging the two health care systems. Halvorson Cancer Center partnership with Seattle Cancer Care Alliance EvergreenHealth and Seattle Cancer Care Alliance (SCCA) opened the new Halvorson Cancer Center on the EvergreenHealth main hospital campus in Kirkland to provide patients with nationally recognized cancer care. The Halvorson Cancer Center brings SCCA’s leading-edge medical oncology and cancer research together with EvergreenHealth’s nationally recognized outcomes in cancer surgery, radiation oncology, diagnosis and screening, in one state-of-the-art, convenient Eastside location.

Valley General Hospital becomes affiliate of EvergreenHealth EvergreenHealth and Valley General Hospital in Monroe approved an agreement in which Valley General became an affiliate of EvergreenHealth. The affiliation deepens the health care services available to residents of the greater Snohomish County area. It also gives Valley General the ability to serve its community through enhanced clinical services adapted from EvergreenHealth’s approaches to clinical outcomes, patient safety and patient experience. EvergreenHealth received the 2012 Consumer Choice Award from the National Research Corporation (NRC), a trusted health care research firm. Residents of King and Snohomish counties recognized EvergreenHealth as one of two hospitals in the region with the best doctors and nurses, highest-quality care and best overall image. Top grade for patient safety from The Leapfrog Group

The Leapfrog Group, an independent nonprofit organization and advocate for hospital transparency, awarded EvergreenHealth an “A” grade, its highest rating for hospital safety. The Leapfrog Group’s annual Hospital Safety Scores are based on preventable medical errors, injuries, accidents and infections.

Consumer Reports recognition for patient safety EvergreenHealth’s commitment to patient safety was demonstrated again when it was recognized by Consumer Reports in 2012. The independent consumerprotection organization ranked EvergreenHealth among the top 10 percent of hospitals nationwide for safety, and scored it among the top two for patient safety in the state of Washington. Among the first in the nation to provide mobile 3-D mammography The EvergreenHealth Breast Health Center introduced its third three-dimensional digital-mammography imaging unit to its mobile coach, increasing access to the most advanced screening technology for women in the community. EvergreenHealth was the first to offer mobile 3-D mammography in the Puget Sound region, and its mobile coach is the only one of its kind in the western United States. Distinguished Hospital for Clinical Excellence In 2012, EvergreenHealth became the only hospital in Washington state to earn the “Distinguished Hospital for Clinical Excellence” recognition four years in a row from HealthGrades, the nation’s leading provider of independent hospital ratings. EvergreenHealth is rated among the top 5 percent of hospitals nationwide for clinical excellence.

Recognized for Outstanding Patient Experience HealthGrades also recognized EvergreenHealth for providing an outstanding patient experience and ranked the organization among the top 10 percent of hospitals nationwide for exemplary patient satisfaction, based on data measured throughout patients’ visits to EvergreenHealth medical centers. In 2012, EvergreenHealth was the only hospital in King County to receive the outstanding patient experience distinction two years in a row from HealthGrades. Maternity care is 5-Star rated, nine years in a row HealthGrades also recognized the Family Maternity Center for its superior care for the ninth straight year. In 2012, Maternity Center leaders also successfully reduced 39-week early-elective deliveries to zero, eliminating the risk of early-term deliveries and helping all babies receive the health benefits of the last weeks of pregnancy. Ranked No. 1 for stroke care by HealthGrades

Stewards of the Levy Funds: Meeting Each Generation’s Needs As a public hospital district, EvergreenHealth has an important role in the community. Every year we are entrusted, as the stewards of your resources, to use levy funds to meet community health needs. It is both a privilege and a great responsibility. In 2012, more than $4.6 million in levy funds supported a range of programs designed to help the community stay connected to essential community services and health care programs. The following stories share just some of the ways your resources make a difference in ensuring the health of the greater community. Community Healthcare Access Team The Community Healthcare Access Team (CHAT) at EvergreenHealth provides essential resources to community members who face barriers to receiving basic health care services. In 2012, the team’s work helped thousands of community members overcome challenges with navigating the health care system due to limited or no income and lack of insurance. “We strive to reach people in the community whose needs aren’t being met,” says Shahzina Karim, director of Healthcare Access. “We do that by aligning resources both in and outside the organization and helping the community gain access to those services.” Beyond introducing individuals and their family members to EvergreenHealth’s network of primary care and specialty clinics, the Community Healthcare Access team also connects eligible residents to

broader community resources: transportation, food banks, financial planning services, interpreters and many others. In 2012, the team of community-based social workers sought to deepen its relationships with community resources and increased its overall outreach efforts to churches, schools, health clinics and a number of other resources that provide vital connections for those in need. “Our work fulfills a promise made by EvergreenHealth to the community to assist the underserved, uninsured, underinsured, seniors and their families, and those who need assistance accessing health care and other community resources,” Karim adds.

EvergreenHealth Nurse Navigator & Healthline P8

Each year, thousands of families rely on EvergreenHealth’s 24-hour consulting nurse and referral service known as the Nurse Navigator & Healthline. The free service, available to all community members, connects callers to registered triage nurses who can help determine the appropriate level of care for a range of health care needs. “Our goal is to help patients get the right level of care at the right time,” Vickie Ravenscroft, RN, director of the Nurse Navigator & Healthline, explains. “Our nurses speak one-on-one with individuals to learn their symptoms and recommend next steps—whether it’s continued self-care, visiting a doctor, going to the ER, or even calling 911.” In addition to recommending appropriate health care services, Healthline also serves as a referral service

for community members looking to connect with a primary care physician. In 2012, the EvergreenHealth Nurse Navigator & Healthline served 92,500 callers and assisted nearly one-third of those individuals with nurse triage advice. Healthline also connected 4,800 residents to quality health care through physician referrals and registered 17,600 callers for community education classes offered at EvergreenHealth. “After each call, we always ask patients for their feedback and what they would have done if this service wasn’t available, in order to help us improve and use our levy resources in the most responsible way. Every year, we continue to be encouraged by an overwhelmingly positive response from the community,” Ravenscroft says.

Senior Health Specialists patient’s glucose management and is a good indicator of the effectiveness of diabetes treatment plans. Thanks to vigilant reviews of patients’ quarterly charts, the team far exceeded their goal and helped significantly more patients—87 percent, including Weber—reach the desired goal of sustaining an A1C of 7 percent. Beyond routine care coordination for seniors, the clinic also connects with patients who have been discharged from the hospital and follows up with them throughout their recovery. From help with scheduling follow-up doctor’s appointments to establishing home-health visits and managing prescription regimens, the care management team of nurses, social workers and pharmacists is a resource for every aspect of a patient’s recovery after hospitalization.

When Vivian Weber and her husband were planning their wedding, they made special arrangements to be sure that her future father-in-law, Ed Weber, would be healthy enough to attend.

“A big part of our team’s success is our care management program, which helps more than 4,000 seniors and their families navigate complex medical care needs,” Kelly Dibble, EvergreenHealth’s Senior Health Specialists clinic manager, explains.

That was 25 years ago. Last July, Ed celebrated his 90th birthday. His longevity—despite health setbacks over the years—has been a welcome surprise to his family and is thanks in part to his care from EvergreenHealth’s Senior Health Specialists.

In 2012, the clinic prioritized its work to help more diabetic patients like Weber manage blood-glucose levels by ensuring at least 65 percent of patients achieve an “A1C” of 7 percent or less. The blood test captures the overall success of the

Mobile Immunization Clinics for Schools “We know there are many barriers for some families in gaining access to health care, but one significant issue is simply scheduling. Many working parents find it challenging to visit a clinic during normal business hours,” Tracy adds.

With that in mind, EvergreenHealth worked with its partners to offer free immunization clinics in the evenings at locations that were familiar to many families— the schools their children attend.

In the fall, EvergreenHealth partnered with the Northshore and Lake Washington school districts, Northwest University and Puget Sound Christian Clinic to meet the needs of students and families with limited access to health care services. “Every year we have a population of students who have a challenge getting the immunizations

required for school attendance— and for the best protection against vaccine-preventable illness,” says Sandie Tracy, health and nursing supervisor for the Northshore School District. Along with other standard immunizations, it was particularly important for students to receive the pertussis vaccine in response to Washington state’s whooping cough epidemic.

With help from Puget Sound Christian Clinic and its 40foot mobile medical coach, EvergreenHealth nurses and staff provided immunizations to dozens of underserved students. Nursing students from Northwest University in Kirkland also joined the EvergreenHealth team to gain hands-on clinical experience as part of their own education. In all, the mobile clinics provided 45 immunizations to students in the Northshore School District and served another 100 students in the Lake Washington School District, effectively helping many students and their families gain better access not only to convenient, quality health care services but also to a quality education.

EvergreenHealth Contributes $17.7 Million to Meet Community Needs As a community-based hospital, EvergreenHealth plays an important role in fostering the health of the greater community. In 2012, the organization generated more than $17.7 million in funds beyond those earned for direct patient care to provide additional services where the community need was greatest. Of that total, $6.2 million supported charity care, $5.5 million covered subsidized Medicaid costs, $4.6 million made EvergreenHealth’s levy-funded programs possible, and $1.4 million was directed to subsidized health care services, which include the free 24-hour Nurse Navigator & Healthline.

Dear Friends, Every year we are inspired by the contributions of our generous community. In 2012, some of the most significant work in the history of the EvergreenHealth Foundation came to fruition. We surely would not have accomplished so much without the trust and dedication of our volunteers, donors and community leaders—it’s your tireless efforts that push us to set ambitious goals to meet the community’s health care needs in groundbreaking new ways. Most notably, we completed the major gifts phase of the foundation’s first capital campaign, raising $8.1 million for the Halvorson Cancer Center, which provides unparalleled comprehensive cancer care for our patients and their families. Our 2012 gala raised a record-breaking $1.1 million to benefit the EvergreenHealth Breast Health Center and its ability to provide women with access to breakthrough technology, education, diagnosis and quality care. In the fall, the community stepped up once again for women’s health services at an exclusive fashion event at the Bellevue Neiman Marcus to fund screening mammograms for uninsured women.

Finally, we welcomed to the EvergreenHealth Foundation Board of Trustees six new members who join with other local leaders on our 24-member board. We look forward to working with them to support the operations, facilities and equipment needs of EvergreenHealth. The Lee Johnson Family: Three Generations of Contribution to the Community in between, we have chosen the medical care and expertise EvergreenHealth has provided to our entire family throughout the generations,” says Tod Johnson, eldest son of Lee Johnson. “When my father faced serious illness, there was no question that we would count on EvergreenHealth to care for him.”

In 2002, Lee Johnson passed away, leaving a legacy of giving for his children Tod, Brett and JeNell to embrace as they continue to lead the family business and carry out the Johnson tradition of community service.

The Johnson family of Kirkland has embraced a philosophy of service to its community for more than 80 years. Beginning with patriarch LeRoy E. Johnson decades before EvergreenHealth opened its doors, the Johnson family hallmark of servant leadership has been passed down to each new generation. In recent years, that spirit of selfless service was evident in LeRoy’s son, Lee Jr., and his wife, Ginny, lifelong supporters of EvergreenHealth both personally and professionally through their

family-owned, Kirkland-based Lee Johnson Chevrolet.

This year the EvergreenHealth Foundation was honored to present the Lee and Ginny Johnson family with the annual Spirit of Giving Award at the 2013 EvergreenHealth Foundation Gala in April. The award recognizes individuals in the community who embody the charitable philosophy the Johnson family has personified for so many years.

Multiple Locations for All Levels of Care We make health care easy and convenient with seven primary care practices, two urgent care centers and two 24/7 emergency departments. Our primary care practices offer night and weekend hours, with Sunday walk-in care available at Canyon Park. We also provide a dozen specialty practices.
