Keeping Promises: Stories of Breakthrough Care from EvergreenHealth Dear Friends of EvergreenHealth, When EvergreenHealth was formed 40 years ago as the public hospital district on the Eastside, we made a promise to the people of this community to be wise stewards of the resources entrusted to us for their healthcare and well-being. The 2011 Community Annual Report for EvergreenHealth is our account of some of the many special programs and services that have kept that promise and led to breakthroughs in the care of our community in the past year. Every day we hear stories about “breakthroughs” that offer incredible new opportunities and experiences—breakthroughs in technology, in science, in research, in resources, and more—after which our lives are never quite the same. And EvergreenHealth is part of those breakthroughs: in our pioneering robotic surgeries, in the national recognition of our clinical excellence and safety, in our use of computer-assisted technology.

Perhaps less dramatic, but nevertheless just as life changing, are the breakthroughs for our patients that occur at EvergreenHealth every day. This special edition of Monitor, which is also our 2011 Community Annual Report, contains stories of some of those breakthroughs in the care we provide for the people of our community, in the technologies we offer, in our community’s access to our services and the experience of healthcare, in the clinical outcomes we achieve, and in our connections to our community. These are stories of everyday breakthroughs that, to our patients and their families, are life changing to them and hardly feel “everyday.” And these are the stories of the 4,400 physicians, providers, staff, volunteers, community advisors, and Board members who are dedicated to fulfilling the promise of EvergreenHealth: “Working together to enrich the health and well-being of every life we touch.”

In this issue of Monitor we present our 2011 Community Annual Report, Keeping Promises: Stories of Breakthrough Care from EvergreenHealth. This year marks EvergreenHealth’s 40th anniversary of providing care to our community, and our report is an account of our stewardship of the resources you have entrusted to our care and our fidelity to our mission: to advance the health of the communities we serve through our dedication to high-quality, safe, compassionate, and cost-effective healthcare. Today, we call that “breakthrough” care. Our duty as your stewards is to bring together the clinical quality, professional expertise, and highly personalized service that are required to deliver breakthrough care. As you will read, providing you with breakthrough care is the story of our people—our physicians, providers, employees, and volunteers—the technologies they employ, the access to our services that they offer through new facilities, the experiences they create for patients and families, their involvement in our communities, and the awards they have earned. You can see our breakthrough care, too, in the compassionate care we continue to provide through our levy-funded programs such as the EvergreenHealth Network and the Healthcare Access program.

As the stories in our report show, with the help and support of our communities, we have come a very long way in the 40 years since EvergreenHealth began as a small, two-story community hospital. We will go even farther through new initiatives such as our strategic partnership with Virginia Mason and our partnership with the Seattle Cancer Care Alliance. In short, we will continue to grow capabilities and capacity carefully to always be an inclusive community health system that is your most trusted source for healthcare solutions.

Celebrating 40 Years: Highlights of Our History Voters approve the formation of King County Public Hospital District No. 2. Thirty-five acres of land are purchased at $6,500/acre for the hospital site. 1972 On March 9th Evergreen General Hospital opens. 1982 Hospital’s 3rd floor completed. 1984 Hospital’s 4th floor is completed. New family maternity center and Level II nursery opens. 1986 New Emergency Department, Critical Care Unit, and auditorium opens. I’m Here Today Because EvergreenHealth Saved My Life The majority of patients are in their 40s and 50s, working people with families, most often without insurance. “We help them overcome a variety of obstacles on the path to health—whether it’s access to good nutrition or medicine, or a translator,” Green explains. In May 2011, after battling weeks of ill health, 54-year-old Dwight Schmidt blacked out while walking up the stairs. Unemployed, uninsured and scared, Schmidt recalled a previous EvergreenHealth Monitor story about King County Project Access … and wondered if it could help him. Project Access is an EvergreenHealth levy-funded program that helps eligible patients fi nd the care they need but can’t aff ord due to barriers such as not having insurance. Dwight Schmidt was matched with a primary care physician who encouraged him to head to EvergreenHealth’s Emergency Department. There, he was diagnosed with an abnormal heart rhythm and congestive heart failure. After 11 days in the cardiology unit to stabilize his blood pressure and heart rate, Schmidt underwent a procedure that shocked his heart back into a normal rhythm.

Green says that 68 percent of Project Access patients need one of four specialties: cardiology, GI, general surgery or orthopedics. “They’re people who perhaps have not had good care because they haven’t had a regular primary care physician or good insurance,” she explains. “So something like blood pressure doesn’t get monitored, and it turns into a huge problem. We’re a health coach and navigator of this complex healthcare system.” Dwight Schmidt saw fi ve diff erent EvergreenHealth specialists, and continues to be monitored by EvergreenHealth’s heart failure clinic. Since last June, the clinic staff has helped Schmidt manage his medication, nutrition and weight-loss eff orts. “And so far, so good. Today, Schmidt says, “the diff erence is like night and day. Now I can walk up the stairs. My heart isn’t pounding.” He’s losing weight, slowly but surely. And he doesn’t take any of it for granted. “I’m here today,” he says, “because EvergreenHealth saved my life.” For more information on Project Access at EvergreenHealth, call 425.899.3200.

EvergreenHealth Continues to Receive National and Regional Recognition At EvergreenHealth, our priority is providing patients with the best possible care and experience when they visit our facilities. National recognition of EvergreenHealth offers patients an objective evaluation of the quality of our care, a means to compare the quality of care at different hospitals, and one way of helping you choose where to receive care. In 2011, EvergreenHealth received numerous national and regional awards and recognitions, in particular, from HealthGrades, one of the nation’s leading providers of independent hospital ratings. The HealthGrades Hospital Quality in America study ranks 5,000 U.S. hospitals on a number of measures. To be considered for HealthGrades’s designations of excellence, hospitals must meet standard requirements for patient volumes, quality ratings, and the range of clinical services provided. In 2011, EvergreenHealth achieved the following honors from HealthGrades:

– General Surgery – Critical Care – Gastrointestinal Care • HealthGrades Recognitions rank hospitals against others in their state for specific services. Director of Volunteer Services. “Even though our volunteers don’t do direct patient medical contact, they are providing time for the staff to do their work while the volunteers help support them by doing other tasks,” says Long. Volunteers do help shape the patient experience in other ways. “A volunteer is often the first look at EvergreenHealth a person gets when they walk in the front door. They see the volunteers at the information desk, who greet them, help them find where they’re going, walk them to their destination, get them a wheelchair, make sure they don’t get lost. So the first time a person walks in the door, usually scared and nervous because they’re in pain or their loved one is in pain, a smiling volunteer face really, really helps calm them and makes them feel assured, and that’s our role,” says Long. Once EvergreenHealth gets the people with the right fit, it works hard to keep them. One method is to measure “employee engagement” and then take steps to increase engagement, according to Chrissy Yamada, Senior Vice President for Chrissy Yamada, Human Resources and Senior Vice President, Chief Financial Officer. Human Resources, EvergreenHealth defines Chief Financial Officer employee engagement as the extent to which employees are passionate about their work, emotionally committed to their organization and to their coworkers. EvergreenHealth, she says, started measuring employee engagement in November 2010 to create a benchmark for measuring changes. The goal is continuously improving engagement scores as management responds to concerns uncovered in an annual survey. “We embed employee engagement action plans as part of our organizational goals to continuously improve engagement,” says Yamada.

“When you see our HealthGrade scores go up, at the same time we also have seen our engagement scores go up,” Yamada points out. The Medical Staff Office works to ensure the right fit between the medical staff and the EvergreenHealth culture through a rigorous credentialing process and by a unique “on-boarding” process, says Diane Kovar, Manager of the Medical Staff Office. “Physician and provider on-boarding is a very personal one-on-one discussion between each newly credentialed physician or provider and the President of the Medical Staff and the Chief Medical Officer. The discussion focuses exclusively on the EvergreenHealth culture, its mission, vision, and values,” Kovar says. One of those standards is “excellence,” especially in clinical outcomes. To ensure clinical excellence, says Dr. Handley, “We recruit and retain the best and most qualified physicians and healthcare providers. We have an outstanding medical staff that ranks among the best in the region. We also have created clinical protocols—whether it’s for diabetes, hip fractures, or congestive heart failure—that use the latest medical literature to create what is the best way of practicing and delivering care. We then ensure those protocols are followed so that all patients are treated in the best manner and get well as quickly as possible.” Quality care finally comes down to what happens at the bedside. “The nurses and the healthcare team are all interacting with patients and their families to meet not just the care needs prescribed by the provider and the interventions that nurses are able to do. We are really caring for the whole person within nursing,” says Nancee Hofmeister, Vice President and Chief Nursing Officer. “We are not just meeting their physical needs. We’re meeting their emotional needs and spiritual needs in a holistic way. When you treat somebody in that holistic fashion that’s when you achieve excellence. We care for the whole patient and the whole family, whomever the patient considers their family to be.”

Innovation Stewardship Makes EvergreenHealth a Technology Leader

EvergreenHealth Director of Surgical Services Debbie Brown calls robotics and technology in the operating room “a new frontier.” “It’s amazing what we’re providing in operating rooms with innovative surgical technologies that give patients faster recovery times,” she says.

EvergreenHealth is a leader in these technological breakthroughs, starting with minimally invasive surgery, where smaller incisions create less bleeding and tissue damage, resulting in faster recovery times and better patient outcomes. Our goal in surgery relies on these innovations to help us keep our patients safer while allowing them to go home sooner. To assist EvergreenHealth in staying on the leading edge of surgical innovation, many new surgical technologies were adopted or expanded in 2011. These include: Computer-assisted spine navigation. In March, EvergreenHealth became the fi rst Puget Sound hospital to use new spine navigation technology to scan a patient’s body and use a camera to assist with precise positioning of smaller surgical instruments for greater accuracy and a better surgical outcome. Traditional spine surgery, with long incisions and signifi cant tissue damage, is replaced with computer-navigated surgery that allows surgeons to perfectly place spinal screws using a less invasive and more precise method.

System-wide Innovations The result is less pain and infection risk and faster recovery times for patients. da Vinci robotically assisted surgical system. Use of the da Vinci system was expanded in 2011 to general surgery, including the nation’s first da Vinci-assisted gallbladder removal in October. The robotic arms offer greater range of motion than the human wrist, plus four arms for greater flexibility. Its high-definition camera creates 3D images for incredible accuracy, allowing improved precision for the surgeon. Adopting the da Vinci system for general surgery allows EvergreenHealth to provide many more patients with its benefits. Our surgeons’ expertise is why EvergreenHealth was chosen as one of the first 50 hospitals in the country to use Single Site® da Vinci for gallbladder removal, leaving the patient with just one small incision. MAKOplasty®. In November, EvergreenHealth was the first Eastside hospital to use the MAKOplasty robotic arm for partial knee resurfacing, a minimally invasive alternative to total knee replacement. Surgeons use the MAKOplasty robotic arm to target the knee joint while avoiding damage to the surrounding area. Size and position coordinates are entered and the robotic system ensures that the surgeon follows the precise specifications, accurate to within one millimeter. EvergreenHealth is one of only two facilities in the Puget Sound area offering the procedure. TEMS. Transanal endoscopic microsurgery (TEMS) gives surgeons better visualization in areas that are hard to reach without invasive surgery. TEMS provides surgeons with greater precision when removing early-stage colon or rectal cancer by using a microscope, videoscope, and specialized microsurgical instruments. With no large incisions, patients experience less pain and have faster recoveries. Major abdominal surgery with a fiveday hospital stay is replaced by minimally invasive surgery and an overnight stay.

EvergreenHealth made system-wide technology innovations in 2011 as well. In May, the Computerized Physician Order Entry (CPOE) system went live. With it, physicians enter information directly into an electronic medical record, or EMR. Eliminating handwritten orders means eliminating transcription errors, according to Jamie Trigg, EvergreenHealth’s Director of Information Technology. “There are significant safety aspects to CPOE,” he says. “Medication transcription errors are almost non-existent now. Medication and test orders are entered electronically, so they are administered more quickly. Patients get exactly what their doctor orders and get care faster. Our average pharmacy medicine order time is down from one hour to less than 20 minutes.” Adoption of CPOE by EvergreenHealth physicians has exceeded expectations with 87% already using it. Physicians were also quick to adopt the new physician portal that allows physicians to access patient charts from anywhere, even via a smart phone. With the implementation of CPOE, EvergreenHealth met the last of 25 requirements to qualify for the Centers for Medicare and Medicaid Services (CMS) “meaningful use” incentive for adoption of EMR technology. As the needs in our community have grown in the last 40 years, so has EvergreenHealth. In 2011, we opened new facilities in Woodinville and Redmond to meet increasing needs for healthcare and fulfill our commitment to support patients wherever they are on the Eastside.

At 22,000 square feet, the new EvergreenHealth Medical Center—Woodinville has space to add even more providers. Plus, we’re now able to off er evening and weekend hours that are more convenient for many residents. Additionally, the Woodinville facility has an expanded urgent care center for those who need medically necessary treatment for an illness or injury that is not an emergency. With multiple exam rooms and expanded hours, we’ve been able to reduce wait times and see patients more quickly.

specimen collection—another convenience for patients because providing those services eliminates the need for them to visit more than one medical facility. “The mission of EvergreenHealth is to advance the health of our community through high quality, safe, compassionate, and costeffective care. A strong primary care physicianpatient relationship based on special trust Jeff Tomlin, MD, and confidence is at the Senior Vice President, core of the success of our Chief Medical Officer mission. This relationship must be supported by the hospital and its affiliated services when they are needed. As we look to the changes in the future of healthcare management, it is clear that even more care will be provided in an outpatient setting and directed by the primary care physician. That is why primary care is so important to our future and fundamental to everything we do today,” says Jeff Tomlin, MD, Senior Vice President and Chief Medical Officer. Redmond In Redmond, EvergreenHealth combined two primary care practices into one. “We thought there was power in numbers,” says Sherlock. “Together, we could offer the community much more than each practice could individually.” Additionally, EvergreenHealth Medical Center— Redmond added specialty services. To date, orthopedics, OB/GYN, and podiatry are offered at Redmond with more services coming this year. And like Woodinville, the Redmond facility houses an urgent care center and a PACLAB Patient Service Center. Redmond also has a 24/7 full analytic laboratory as well as a full-service medical imaging

center that provides advanced diagnostic imaging such as X-ray, ultrasound, CT, and MRI. “Everything is integrated and connected so our patients can get the majority of the services they need all in one place with less waiting time in between,” Sherlock says. The Redmond facility also offers emergency care. The 24/7 emergency department, which is staffed by board-certified emergency medicine physicians and trauma-trained nurses, is able to treat all medical conditions. Six Locations throughout the Community EvergreenHealth has “primary care homes” in six locations, providing easy access to care throughout the community. EvergreenHealth Home Care Along with establishing facilities throughout the community, we are also prepared to provide you with care in the location where you spend the most time: your home. EvergreenHealth’s Home Care program offers a wide range of healthcare services that can be provided in your home as you recover from surgery, illness, or an accident. In 2011, we provided nearly 6,000 patients with Home Care services, such as nursing, physical and occupational therapy, speech therapy, social work, and mental healthcare. Additionally, EvergreenHealth’s Hospice program provided end-oflife care for another 2,480 patients last year. Care is either provided in an individual’s home or in our home-like Gene & Irene Wockner Hospice Center at the EvergreenHealth Medical Center campus in Kirkland.

At retirement and senior centers In 2011, EvergreenHealth’s older adult Horizons program was off ered at 12 diff erent locations throughout the area, including retirement or senior centers, rehabilitation facilities, churches, and more. “We truly take Horizons out to the community,” says Lauren Bolen, Health Education Coordinator. “Our physicians, surgeons, nurses, and physical therapists off er free education on almost any healthcare topic you can imagine.” The Horizons courses provide older adults with the opportunity to ask questions and alleviate any concerns as well as receive encouragement to make changes to improve their health.

In 2011, approximately 200 Horizons classes or screenings were off ered to 2,700+ individuals. In our schools Good habits start young. That’s why EvergreenHealth also goes into area schools to teach important health topics.

In 2011, more than 1,600 people were trained in CPR in one of EvergreenHealth’s school programs, community, infant/child, adult, or special Neonatal Intensive Care Unit family CPR classes. Where you go for shopping and entertainment It’s not uncommon to see the people of EvergreenHealth at local community events, festivals, farmers markets, first aid booths, cooking demonstrations, and more. In fact, in 2011, staff members of EvergreenHealth appeared at more than 55 events.

In 2011, EvergreenHealth took Project Alert, a nationally recognized drug prevention program, to fi ve elementary schools. Stewards of the Levy Funds: Meeting Community Needs As a public hospital district, EvergreenHealth has a special role in the community. Every year we are charged, as the stewards of your resources, with using levy funds to meet community health needs. In 2011, levy funds for community programs totaled more than $4.2 million. “It is both a privilege and a great responsibility to manage these funds for the ongoing support of programs and services that are community and health based— and it’s a charge we take very seriously,” says Laurene Burton, Administrative Director, Governance and Community Aff airs. “We work hard to ensure the levyfunded programs together respond to the wide range of needs in the communities we serve.” Community partnerhips

Your Levy Funds at Work Here’s a look at the ways some of EvergreenHealth’s levy-funded programs served our communities in 2011: • Healthline: In 2011, Healthline served more than 128,000 phone callers through its levy-funded program, directed 26,800 of them to community resources, facilitated more than 4,000 physician referrals, and assisted 33,500 with nurse triage advice. “In nurse triage calls, we talk with individuals about symptoms they are experiencing and help them determine the appropriate level of care—calling 911, going to the ER, visiting a doctor, or doing self-care,” says Vickie Ravenscroft, Director of Healthline. In 2011, Healthline added after-offi ce-hours triage to support several EvergreenHealth specialty services. Ravenscroft says that while there is a wealth of health information online, Healthline, which is staff ed by EvergreenHealth registered nurses, off ers something more: an opportunity to speak oneon-one with a nurse navigator, any time of the day, who will systematically assess an individual’s current health concern to provide the appropriate recommendations for care. • Senior Health Specialists Clinic: As a specialty clinic, Senior Health Specialists provides care management for medically complex patients who meet specifi ed criteria. In 2011, the clinic created a new RN care management program to help the elderly in our community transition from hospital to home / nursing home with the goal of reducing the number of hospital readmissions. The new program has proven very successful. Only 11.7% of patients were readmitted within 30 days—well below the national average of 33 percent—thanks to the team approach that includes weekly care meetings, social worker visits, coordination of home care services, follow-up patient calls to review medication and discharge instructions, and assistance in scheduling follow-up medical appointments.

Levy Program Highlights • Healthcare Access / Project Access: Access to healthcare services was provided for more than 6,400 individuals last year. Individuals—many with limited or no income and insurance—were connected to primary or specialty healthcare resources and other essential community services and programs. “We screen, assess, and connect individuals to services and help them navigate the healthcare system,” says Shahzina Karim, Director of Healthcare Access. “We don’t just put them in touch with our services; we also connect them to broader community services, such as transportation, food banks, financial assistance, interpreters, and more.” • EnhanceWellness: During 2011, approximately 220 individuals took part in EvergreenHealth’s EnhanceWellness program at Northshore Senior Center. The program, which is specifically designed for older adults, includes topics such as Living Well with Chronic Conditions, Powerful Tools for Caregivers, Matter of Balance, and PEARLS (depression program). Additionally, nurse counselors work with participants to conduct a health assessment and personalized action plan, where health risks are identified and behavior changes are outlined. As a result, the program is helping seniors decrease the length of hospital stays, lower their psychoactive drugs, alleviate symptoms of mood disorders, and develop a greater sense of self-worth. Last year, the program resulted in 96 percent of the participants improving or maintaining a key part of their action plan, such as exercise, social activity, and health self-management.

More than 194,000 individuals benefited from community programs funded by the levy. • Health Education classes and screenings reached 21,000+ individuals. • Emergency Management developed and trained an Emergency Response Team and developed a training module for staff. • The Duvall Primary Care practice saw 10,457 patients including 700 new patients. • Grief and Bereavement continued community outreach through support groups, individual counseling, and increased referrals. • Diabetes Education increased its client base by 5% and developed an outcomes report card to track Hemoglobin A1C. • Community Electronic Medical Records implemented two systems and secured four contracts with private physicians’ offices. • Community Advisors actively served on the hospital’s Board Quality, Ethics, and Home Health Professional Advisory Committees.

EvergreenHealth Contributes $19.6 Million to Meet Community Needs Despite the continuing, challenging overall economic environment in 2011, EvergreenHealth was able to generate more than $19.6 million to meet community needs outside of direct patient care. Dear Friends of EvergreenHealth, In this anniversary year of celebrating 40 years of breakthrough care from EvergreenHealth, we at the EvergeenHealth Foundation have breakthroughs of our own to celebrate. Because of the trust the community has placed in EvergreenHealth, 2011 marked the coming of age of the Foundation. We successfully launched our fi rst capital campaign with a target of raising $10 million for a comprehensive cancer center. And we raised more than $1 million at our annual Gala for the second time in our history. With the support of our community, the capital campaign was spectacularly successful from its onset. Between the launch of the campaign in May and the end of the year, we raised more than half of our goal. The campaign started with a gift from Elling and Barbara Halvorson, the largest gift the Foundation has ever received from one family.

Because of the signifi cance of their gift, the center will be named the Elling and Barbara Halvorson Cancer Center at EvergreenHealth. Our community members also demonstrated their belief in EvergreenHealth’s mission through support of our annual Gala in May. The $1.1 million we raised provided for a 3-D tomosynthesis mammography unit at EvergreenHealth, the fi rst in the Seattle area. We are deeply grateful for the trust our community has placed in EvergreenHealth. Time and again we have seen the willingness of its members of our community to partner with us as we keep our promises, and implement our purpose: “Working together to enrich the health and well-being of every life we touch.” Earning and having that trust is a high honor. Doing Great Things: Barbara and Elling Halvorson When the EvergreenHealth Foundation leadership sought someone to make a major gift to launch fundraising for the new comprehensive cancer center, they thought immediately of Elling and Barbara Halvorson. The Halvorsons are long-time contributors to EvergreenHealth, having funded the Halvorson Chapel on our community campus, and are highly regarded for supporting causes that make life better for people in need. And when Barbara heard what EvergreenHealth leadership proposed—to name the cancer center after them—she said, “I am so humbled by the idea. We had so hoped to be able to do something truly valuable for this community. And this is it.”

Doctors predicted they would not live to be adults; they have just celebrated their 54th birthdays. And, like one in three Americans, the Halvorson family has been touched by cancer. Getting new ventures like the cancer center started is nothing new for the Halvorsons. Over more than 50 years, Elling has started or led 15 diff erent companies in industries ranging from construction to aviation to entertainment. Now that spirit of entrepreneurship will make it possible to bring all of EvergreenHealth’s cancer services together in one place and provide a home for the new partnership between EvergreenHealth and the Seattle Cancer Care Alliance.
