With this issue of Monitor we mark the 40th anniversary of Evergreen, celebrating all this community has done to help Evergreen grow and expand as our community has grown. Over the past 40 years, Evergreen has come a very long way under the leadership of our Board of Commissioners and with the talent and dedication of our physicians, staff, volunteers and members of this community. We began as a small, general hospital in 1972, with only two floors. Today, we are still the deeply caring hospital this community created then; and we are a sophisticated medical center, using technological advances such 3D digital mammography and minimally invasive, robotic-assisted surgery to ensure that our people on the Eastside receive the best care possible. Our distinguished care and service have been recently recognized by HealthGrades, ranking us in the Top 5% of hospitals in the country for clinical excellence, and the Top 10% for our patient experience. To better reflect who we have become for you, we’ve decided this anniversary year to introduce a refreshed brand, name, and look for Evergreen that honors our 40-year tradition of exceptional clinical care and community service while more effectively reflecting what we have become and aspire to be—the hospital and medical center that aligns the technology, quality, expertise and service required to deliver breakthrough care for you. Evergreen’s logo was put into place 12 years ago, and with all of the changes at Evergreen and in the Eastside community over the past dozen years, that logo and brand no longer accurately describe the capabilities and capacity that we bring to the community. As the community has grown, so have we. Our employees and the physicians who practice at Evergreen also told us that the current logo did not resonate with them or reflect for them the sense of mission and calling that they bring to Evergreen. The new brand and logo enable us to better communicate, first, our full range of capabilities and capacity, especially in the areas where we excel, areas where our communities may not always be aware of the extent of Evergreen’s distinguished care. For example, HealthGrades, the nation’s leading provider of independent hospital ratings, has again named Evergreen Hospital a Distinguished Hospital

for Clinical Excellence in 2012—the only hospital in Washington State to receive this award for the past four consecutive years. Les Tiffany had been dealing with the occasional pain for three to four years, but it wasn’t until an acute attack in September 2011 that he went to his primary care provider for answers. An ultrasound revealed gallstones. Les would need surgery to remove the gallbladder, and he got a strong recommendation from his longtime primary care provider, Kenneth Wiscomb, PA-C. “He recommended Dr. Michael Towbin, who had operated on him,” Les recalls. “He said Dr. Towbin was the guy I needed to see.” DA VINCI FOR GALLBLADDER SURGERY There was another reason Wiscomb referred his patient to Dr. Towbin—he knew the surgeon had recently begun using the state-of-the-art da Vinci robotically-assisted surgical system for gallbladder removal. “The da Vinci is a new tool,” Dr. Towbin says, “and while it is renowned for prostate and gynecologic surgeries, it’s relatively new in the general surgery arena.” So Dr. Towbin, who’ll soon mark 25 years on staff at EvergreenHealth, began adapting the da Vinci for use in surgeries such as gallbladder removal. He says the da Vinci offers several advantages, starting with the ergonomics. “The wrist controls can do more than the human wrist, which means you can get different angles and approaches to anatomy,” he explains. “And the high-definition camera gives us 3-D images for incredible accuracy.” The da Vinci also gives the surgeon four “arms” to control the operation instead of the two arms

For patients, this means less blood loss, less pain and a faster recovery. FIRST IN THE NATION Les Tiffany’s surgery at EvergreenHealth last October marked the first time in the nation that the da Vinci had been used for gallbladder removal, but Les wasn’t as impressed by that milestone as he was by the fact that he went home the same day and needed no pain medication. In March, Dr. Towbin once again took the da Vinci to another level, this time performing a minimally invasive gallbladder removal using just a single incision in the patient’s bellybutton. The procedure had just been approved by the FDA. In response to growing demand from both patients and surgeons, EvergreenHealth has added a second da Vinci system. Da Vinci options are available for colorectal, gynecological, urogynecological, prostate, oncological and bariatric surgeries. Eleven of EvergreenHealth’s surgeons are trained on them, supported by a specialized robotics team in the operating room. For EvergreenHealth, it’s just another exciting technology that, combined with the skill of our highly trained medical staff, allows us to provide our patients with a safer surgical procedure with significantly reduced recovery times.

When the Bothell man turned 50, a prostate exam was added to his annual tests. The exam revealed a clinically significant prostate cancer. His primary care provider outlined the options: radiation seed therapy or surgery with the da Vinci robotic-assisted system. Some of his friends pressured Darrell to check out downtown hospitals, because they weren’t aware of the robotic surgery program available at EvergreenHealth. He met with urologist Dr. Scott Van Appledorn, the medical director of EvergreenHealth’s da Vinci surgery program, who told him there was no need to go anywhere else for robotic-assisted surgery. Robotics in the form of the MAKOplasty robotic arm offer a partial knee resurfacing procedure to relieve the pain caused by joint degeneration. EvergreenHealth was the first Eastside hospital to use the MAKOplasty procedure. (See page 10.) MAKOplasty joins the suite of other technologies used by EvergreenHealth’s hip, knee and spine surgeons. Nearly 10 years ago, EvergreenHealth was the first hospital in the state to perform “computer navigation” for total knee replacement. Today, it is a nationally and internationally acknowledged leader in this procedure. Last month, EvergreenHealth’s orthopedists achieved another first when Evergreen spine surgeons performed the first computer-navigated spine surgery on the Eastside. THE REVOLUTION IN SPINE SURGERY Seventy-one-year-old Karyl Lee Olels is no stranger to surgery, having had both hips replaced and work done on her rotator cuff. But there didn’t seem to be any answer for the back problems that had plagued her for years. “The pain was getting progressively worse,” she recalls. “I saw orthopedic specialists, and they told me that having back surgery was the worst thing I could do. So I tried physical therapy and pain injections, but neither was helping.” Karyl Lee mentioned the back pain during a follow-up appointment with the orthopedic surgeon who had treated her rotator cuff problems. He suggested that she see his Orthopedics International colleague, Dr. Addison Stone, a specialist in spine surgery.

“Computer navigation allows us to see exactly what we’re doing,” he notes, “so we leave the operating room knowing our screws are perfectly placed.”

MINIMALLY INVASIVE + COMPUTER NAVIGATION Dr. Stone diagnosed degenerative spondylolisthesis— the slippage of one vertebra over another in Karyl Lee’s lower back that was causing significant nerve compression and leg pain. He recommended a spinal-fusion surgery. “During surgery, we reduce the spondylolisthesis, so it’s no longer slipped,” he explains. “Then we reestablish the space between the two vertebrae and fuse them together. That stabilizes that segment of the back, and decompresses the nerves, to help eliminate the pain.” Dr. Stone goes on to explain, “Traditionally, and still the gold standard in many places, this surgery involves a long incision with stripping of the muscles off the spine in order to be able to place screws and the rods for the fusion. Due to the significant soft-tissue trauma, there is a higher risk of infection. In addition, recovery can be prolonged, with considerable pain.”

Karyl Lee was understandably apprehensive about having an extensive back surgery, but Dr. Stone had a secret weapon that the other surgeons didn’t: a minimally invasive approach to spine surgery that would speed recovery and lessen the pain. When he performed Karyl Lee’s TLIF (transforaminal lumbar interbody fusion) in March 2011, not only did Dr. Stone use minimally invasive techniques, with smaller instruments that allowed for smaller incisions—he also, for the very first time, used computer navigation. “Computer navigation allows us to see exactly what we’re doing,” he notes, “so we leave the operating room knowing our screws are perfectly placed.” Dr. Stone continues, “Evergreen is one of the first in the region to be doing computer navigation for the spine. It’s another example of Evergreen being at the forefront of medical technology to benefit its patients.” LESS PAIN, FASTER RECOVERY The combination of minimally invasive surgery and computer navigation had Karyl Lee Olels out of the hospital in 12 hours. “Those small incisions didn’t hurt nearly as much as the larger ones I’d had with my previous surgeries,” she marvels. “Three months later, I was having fun at Disney World!” She’s returned to the gardening she loves and has added bicycling to her outdoor activities. Karyl Lee is thrilled with her outcome, and with her experience at Evergreen. “This is the first time I’ve had surgery at Evergreen, and I will definitely make sure I keep my care at Evergreen in the future,” she says. “I’m really impressed with both the hospital and Dr. Stone.”

THE REVOLUTION IN SPINE SURGERY Seventy-one-year-old Karyl Lee Olels is no stranger to surgery, having had both hips replaced and work done on her rotator cuff. But there didn’t seem to be any answer for the back problems that had plagued her for years. “The pain was getting progressively worse,” she recalls. “I saw orthopedic specialists, and they told me that having back surgery was the worst thing I could do. So I tried physical therapy and pain injections, but neither was helping.” Karyl Lee mentioned the back pain during a follow-up appointment with the orthopedic surgeon who had treated her rotator cuff problems. He suggested that she see his Orthopedics International colleague, Dr. Addison Stone, a specialist in spine surgery.

“Computer navigation allows us to see exactly what we’re doing,” he notes, “so we leave the operating room knowing our screws are perfectly placed.”

MINIMALLY INVASIVE + COMPUTER NAVIGATION Dr. Stone diagnosed degenerative spondylolisthesis— the slippage of one vertebra over another in Karyl Lee’s lower back that was causing significant nerve compression and leg pain. He recommended a spinal-fusion surgery. “During surgery, we reduce the spondylolisthesis, so it’s no longer slipped,” he explains. “Then we reestablish the space between the two vertebrae and fuse them together. That stabilizes that segment of the back, and decompresses the nerves, to help eliminate the pain.” Dr. Stone goes on to explain, “Traditionally, and still the gold standard in many places, this surgery involves a long incision with stripping of the muscles off the spine in order to be able to place screws and the rods for the fusion. Due to the significant soft-tissue trauma, there is a higher risk of infection. In addition, recovery can be prolonged, with considerable pain.”

Karyl Lee was understandably apprehensive about having an extensive back surgery, but Dr. Stone had a secret weapon that the other surgeons didn’t: a minimally invasive approach to spine surgery that would speed recovery and lessen the pain. When he performed Karyl Lee’s TLIF (transforaminal lumbar interbody fusion) in March 2011, not only did Dr. Stone use minimally invasive techniques, with smaller instruments that allowed for smaller incisions—he also, for the very first time, used computer navigation. “Computer navigation allows us to see exactly what we’re doing,” he notes, “so we leave the operating room knowing our screws are perfectly placed.” Dr. Stone continues, “Evergreen is one of the first in the region to be doing computer navigation for the spine. It’s another example of Evergreen being at the forefront of medical technology to benefit its patients.” LESS PAIN, FASTER RECOVERY The combination of minimally invasive surgery and computer navigation had Karyl Lee Olels out of the hospital in 12 hours. “Those small incisions didn’t hurt nearly as much as the larger ones I’d had with my previous surgeries,” she marvels. “Three months later, I was having fun at Disney World!” She’s returned to the gardening she loves and has added bicycling to her outdoor activities. When David Bartos was troubled by painful knees, he turned to his primary care provider for help. Dr. Robert Kelley of Totem Lake Family Medicine knew who to refer Bartos to, Orthopedic Surgeon Dr. Richard Angelo of the Evergreen Orthopedic Clinic, recently named one of 125 Outstanding Knee Surgeons in America by the January / February 2012 issue of Becker’s Orthopedic, Spine & Pain Management Review.

Another benefit is that the computer and the robotic arm are essentially linked to the patient’s leg via transmitters—allowing the surgeon to make further refinements before any bone is cut. “MAKOplasty takes advantage of what are essentially tiny GPS units attached to the patient’s thigh and lower leg bone,” Dr. Angelo continues. “The computer knows to within one millimeter exactly where the knee is in space. This allows us a complete picture.”

Dr. Angelo treated the osteoarthritis and degeneration in David’s knees with a conservative, nonoperative treatment plan that managed his pain. But over time the pain became debilitating, and last fall both knew it was time for surgery. “I’d planned on having a minimally invasive knee replacement,” Bartos recalls, “but then Dr. Angelo said I’d be a candidate for a new procedure called MAKOplasty®.”

Finally, once the coordinates for size, position and other components of the surgery have been locked in, the robot will not allow the doctor to remove bone outside of those exact specifications. “The precision is remarkable,” Dr. Angelo says, “and can result in morenatural knee motion following surgery.”

ALTERNATIVE TO A COMPLETE KNEE REPLACEMENT MAKOplasty is a minimally invasive partial knee replacement designed to help alleviate knee pain and return people to their activities. “It uses a surgeon-actuated robotic arm to prepare the bones of the knee joint for new bearing surfaces while sparing the healthy bone and surrounding ligaments,” Dr. Angelo explains. “It’s a treatment option for adults living with osteoarthritis in the inside, outside or front compartment of the knee.” While partial knee replacement surgery has been around for years, Dr. Angelo says MAKOplasty offers some distinct advantages, particularly in the area of surgical precision. “Thanks to the sophisticated software, the surgeon is able to do a 3-D rehearsal of the surgery,” he explains. “The ability to fine-tune is incredible and especially helpful.”

“It’s such a great solution to a very difficult surgery.” When 84-year-old Seattle resident Barbara Allan saw a bit of blood in her stool last summer, she called her gastroenterologist and scheduled a colonoscopy. That was a smart decision, as it revealed a precancerous lesion on her rectum. “My gastroenterologist, Dr. John Hinds, told me it would have to come out because it could turn malignant,” Barbara recalls. Dr. Hinds thought she might be a candidate for a minimally invasive procedure called transanal endoscopic microsurgery, or TEMS. He referred Allan to the Evergreen Surgical Clinic’s Dr. Harry Kahn—a US News & World Report 2011 “Top Doc” and the only surgeon performing TEMS in the entire state of Washington.

AVOIDING MAJOR ABDOMINAL SURGERY “TEMS is a clean, precise technique that’s used for patients who have a precancerous growth in the rectum or a very-early-stage rectal cancer,” Dr. Kahn explains. “Before transanal techniques, the standard procedure would have been a major abdominal surgery and a much more involved bowel resection. The benefit of TEMS is there’s no major surgery, no large incision, no colostomy, less pain, faster recovery and a shorter hospital stay.” The TEMS procedure involves a microscope and a videoscope, and specialized microsurgical instruments, which allow Dr. Kahn to remove lesions in the rectum by operating through the anus. “In the case of early-stage cancer, it is critical to get clear margins, which this equipment facilitates,” he adds. Though the TEMS technique has been around a while, Dr. Kahn says it’s being used more now because of advances in the equipment that stages the tumors. “We can now tell whether the growth is precancerous, early-stage cancer or late-stage

cancer,” he says. “That’s critical, because TEMS is not appropriate for advanced cancers, especially if the lymph nodes might be involved.” In those cases, standard surgery is necessary to make sure all of the cancer is removed. Barbara Allan met with Dr. Kahn and had an ultrasound conducted by a gastroenterology specialist to determine the staging of her tumor. It turned out that she was an ideal candidate for TEMS. RECOVERY WITH LITTLE OR NO PAIN The lesion was located in the upper third of Barbara’s rectum, and Dr. Kahn was able to remove it without any problem. She spent just one night in the hospital and didn’t even need pain medication. Dr. Kahn says that is common. “Most TEMS patients just need Tylenol,” he says, “and they get back to work much more quickly.” Just a few weeks after her surgery, Barbara was back to her busy, active life—painting watercolors, taking long walks, and enjoying a full calendar of social activities with family and friends. MATERNITY CENTER TOUR A free 20-minute tour ideal for friends and family, and for those researching their birthing center options. Registration required; also offered in Spanish. LABOR & BIRTH BASICS This core series for first-time parents covers labor and birth, infant feeding, pain management options, cesarean birth and postpartum. Includes maternity center tour, $145/ couple. We also offer the series for teens and for multiples, $120/couple. HYPNOBIRTHING Use guided imagery, visualization and patterned breathing for a safe and satisfying birth. Five-week series, $135/couple, plus $25 materials fee due at class. SIBLINGS CLASS Prepare siblings for the arrival of a new baby. Includes a tour of the Family Maternity Center. For children ages 2 to 6. Parents attend with children. $15 for one child and $25 for multiple. DADS ONLY Learn the first steps to becoming a great dad, based on cutting-edge insights from psychology and developmental theory. $25.

HEALTHY EATING FOR PREGNANCY AND BEYOND Improve your diet with practical tips on shopping, dining out, healthy snacking and more. $15.

FIT4BABY PROGRAM A total-body interval workout adjusted to the various pregnancy phases of the attendees. Includes cardio, strength training and flexibility exercises. Six-week session, $90.
