It is exciting to see all of the opportunities that are available to children in the Our Towne Community. Summer is right around the corner! But first, we can enjoy spring, celebrate our mothers & those who have served our country on Memorial Day. On page 12, you will find a list of those in the community that have recently donated money to the Greenbush YMCA Annual Support Campaign. The $88,822 raised goes right back to members of our community through funding programs, memberships and various scholarships for camp and lessons. Since joining the Board of Directors recently, I have been educated on all of the ways that the Y has a positive impact in our community. I am beyond impressed at the variety of programs that are available to everyone in the community, regardless of income and ability to pay. The YMCA is a not-for-profit that has focused their goals to encompass Youth Development, Healthy Living and Social Responsibility. Partnering with community groups, businesses and schools is a top priority for the Y. If you haven’t already, stop in for a tour and find out how the Y can improve your quality of life. I hope you all enjoy this beautiful & exciting time of year! I am so grateful to be a mother- and to have a wonderful mother. I thank her for all of her love & support. She has been a role model for me and has been the most wonderful mom & grandmother for our girls. I am so very lucky and blessed. Word of Mouth: Dear Dr. Lapinski: My daughter broke her front tooth several years ago. It’s been fixed many times, with fillings, then with a root canal & most recently with a very large full tooth filling. She’s been complaining that it doesn’t look good and she hates her smile. What’s next? Marie L., East Greenbush Dear Marie: I sympathize with your daughter. Front teeth are right out there and so noticeable. Accidents that occur at a young age inevitably require more attention since permanent solutions, veneers & crowns, are often put off until growing has stopped, the tooth has matured & any orthodontic treatment is complete. It sounds like you’ve gone through the normal progression of restoring the tooth and the issue is more esthetic – how the tooth looks. So, to fix the look – you basically have 2 options. The first is to replace the full filling with a newer one. Sometimes replacing an older filling refreshes the look of the tooth. There are quality materials out there that I use that can successfully recreate a really natural looking tooth. The other option is to choose a lab generated restoration – like a crown or veneer. These are often made of porcelain. We can easily change the shape, size & color with beautiful results. They are so natural looking that most people won’t know it isn’t your original tooth. When I restore front teeth, we pay extra attention to choosing a color and shade you want by matching fillings & restorations to the overall color of your teeth. If you want a whiter smile, we would consider tooth whitening before replacing the tooth – so we can match the new tooth to your whiter and brighter smile. Remember, once a tooth has a filling or crown, it will never “bleach”. Under the right circumstances, I replace front teeth in pairs, and can go from 2 to as wide as 6 or even 8 teeth when remaking a smile. It creates a symmetry to the eye & we have the ability to change the shape, size & shade to achieve a look you desire and still repair the broken tooth. Now, if it was determined that the tooth wasn’t stable or was at risk for extraction, your options might include a bridge or an implant. But, that’s a different discussion. HEALTH INSURANCE CORNER… BY CANDY MARRERO OF KLEEBER INSURANCE SERVICES So many things are changing in the health insurance world right now so for now I say hold on tight….it’s going to be a bumpy ride!!! But I also say… don’t worry we will all get through it. Health insurance has always been an evolving industry. Back in the early 80’s everyone had BC/BS and we all had what we called back then “the 80/20” which was at that time just our normal plan. However, as healthcare costs began to rise, something new emerged and called “the HMO”. In our area the first HMO was offered by Community Health Plan (aka: CHP). Doctors bought a building together and formed a group practice in Latham and anyone who went to that facility and saw those doctors no longer paid the normal 20 coinsurance, but instead they paid $3 “copays”. At that time many were reluctant to the change and I remember people saying things like “they aren’t going to tell me what doctors I have to use”. Change is hard (good or bad change) because people just don’t like change at all. However, once we get through the initial shock of it all and cycled through it, the “new” HMO Plans were now the norm.

MIRACLE LEGUE BOWLING FUNDRAISER Friday, May 17, 2013 (6:30 p.m. - 8:30 p.m.) The Capital Region Miracle League established the Jaime M. Adams field as the first multi-sport facility developed for special needs athletes in the United States. Although physically in East Greenbush, the sporting programs held at the field are open to all special needs athletes within a 100 mile radius. They are be proud of this gem in the area, but it does need a few things. Right now there is no lighting, no rest rooms and no scoreboard. The East Greenbush Kiwanis is sponsoring a bowling fund raiser on Friday, May 17th from 6:30 - 8:30 PM at Spare Time Bowling Center. A donation of $10 minimum will get you 2 hours of bowling, with shoes. There will be a 50/50 raffle, chinese auction and bake sale. 100% of the proceeds will go towards building a club house with bathroom facilities for our atheletes and the spectators that come to the field. They are pleading with everyone to help make this a reality. Form a team. Even if you can't come, your donation of any amount would be greatly appreciated. We are again experiencing great times of change. We will all struggle to get through healthcare reform together and though I expect the next year or two to be quite challenging for all of us, I do believe, like everything else, we will accept the changes --kicking & screaming along the way until this new change too becomes “the norm”…..and it will. In the meantime I will keep you as informed as I can (so keep reading OUR TOWNE Magazine for your updates) and certainly you can feel free to call me with any questions, I will help you in any way that I can (and all of my services are absolutely free!!) I would like to leave you with one additional piece of information That I hope will help some small business. In 2012 there was a new tax credit for small businesses that employ less than 25 people. If you pay 50 or more of the health insurance costs and your employees don’t total an average of over $50,000, you may qualify for a 35 tax credit. EVEN IF YOU FILED YOUR TAXES ALREADY YOU CAN HAVE THEM AMENDED. This 35 tax credit will also be offered for this year (2013) and actually increase to 50 for next year (2014).

“TheWedding Planner’s Daughter” By Allison Felts

The “Wedding Planner’s Daughter” is a wonderful book by Coleen Murtagh Paratore. Coleen is a local author who lives in Troy, NY. Her books are very inspirational to writers and tell great stories kids can relate to. The first book in the “Willa” series, “The Wedding Planner’s Daughter”, is about a 12 year old girl named Willafred, whose new nickname is Willa. She lives with her mother Stella in Bramble, Cape Cod. Stella is the wedding planner. Willa doesn’t have a dad because after Stella’s wedding he got in an accident, and was sadly killed before Willa was born . So for Willa’s 13th birthday she wants a father. She is trying to find the perfect dad who might be Willa’s new neighbor Sam, her English teacher. With the help from her Nana and her almost best friend, Tina, Willa might get the perfect birthday present. The “Willa” series has many books and “The Wedding Planner’s Daughter” is just the beginning. Willa is a great character and is fun to learn about throughout the series. In this book, Coleen Murtagh Paratore does an awesome job introducing Willa for the next books, and inspires future writers with a spark of imagination. My daughter gave me a huge hug the other day along with a handmade card that read “WHEN I GROW UP I WANT TO BE JUST LIKE MY MOMMY !” It was amazingly heart-warming but confusing too – because it made me wonder what my daughter saw in me that she wanted to emulate? I had recently read an interview with the COO of Facebook. She was critical of women and postured that women don’t do enough to reach their potential and that if they tried (which they don’t) women would achieve more success, and have it all, in the boardroom, the bedroom and the playground. Analyzing my own career path that spans a shocking 30+ years, it has been very varied & not always stellar. My first “jobs” were babysitting…most notably when I was all of 12, and was (shocking) entrusted with the care and supervision of an 11 month old baby while the parent went out to an afternoon job. I was in all of 8th grade. Feeding, changing, entertaining, napping, cleaning up toys…you get the picture. My highschool job was for a CPA where I assembled, stapled and unstapled endless tax returns. When I wasn’t doing that, I replaced pages of tax code in reference binders with new pages of tax code in a room that was lined with floor to ceiling shelves of books on tax code. I learned that someone always had to do the tedious jobs, since the success of things is often in the details, but that you might not get the credit in the end. I went on to be a sleep-away camp counselor where I kept kids safe, followed routines, officiated sporting events, I cheered and hugged and disciplined, I encouraged reading and umpired arguments, I loved them and taught them to respect themselves and we laughed, a lot. I held their hands through homesickness, pre-teen angst and first kisses. We helped each other because being away from home and independent is a big part of growing

up and not always easy – but the lessons learned last a lifetime. An awesome decade followed working in the restaurant business – where cash was king and the social life was stellar. Those days taught me to listen, hustle, memorize a list of things to do, budget, balance plates & glasses, develop a love for good food and cheap wine, be on time and smile at the customer, because they are always right. After a lengthy stint as an event planner, weathering, well, the weather, last minute crises, politics, personalities, dropped balls and divas, I finally joined the family dental business– which led to learning to balance the line between work and home, because that line was a lot fuzzier now that we lived and worked together. I also had to learn time management to include in my days those things that were just mine, and carve that time to do them. So while I may not have broken through the glass ceiling and my resume may not impress the COO of Facebook, I am happily well defined by ambiguity. I can’t change the past. I’m here through a combination of choices, necessity, planning and fate. But this unsolicited greeting card from my most precious achievement is colorful & treasured. I think being a mom, an employee, a business owner, a friend, a volunteer, a cheerleader and a dreamer qualifies me to be emulated by Allison. There are Countless Women who make choices that are for not only for themselves but for other people in their lives too. Women like my friend who had the guts to go back to the business world after 16 years at home, and my own mother who had the guts to retire when she felt it was her time, and my coworker who has the guts to go back to school to follow a dream and my neighbor who hasn’t changed a thing in 30 years …These women all deserve a construction paper card. So, I say to them…”Happy Mothers Day, You Renaissance Woman You” Because you are the Best Mommy In the Whole World, And I want to be Just Like You!

Driven by a Passion for Horses and Riding Laura Fay’s passion for horses began 35 years ago with a riding lesson. She moved from lessons to training young horses while attending college. After graduation, she worked in the finance field but she never lost her desire to work with horses. In 2004, Fay recognized her dream to share her life long love and knowledge of horses with others. As the owner and trainer of Aering Green Equestrian Center (AGEC), she specializes in dressage and eventing. “I am very passionate about dressage,” says Fay, “But I also incorporate jumping into each horse's regimen to give them a balanced training program.”Dressage is defined as“a French term meaning‘training,’”according to the United States Dressage Federation website and eventing is“…termed an‘equestrian triathlon.’The three phases are: dressage, endurance (or cross-country), and show jumping,” according to the United States Eventing Association (USEA) website. Fay instructs riders of all ages–from those that are new to the sport to those with many years of riding experience. Her philosophy is that anyone with an interest can learn to ride¬. “Safety for horse and rider is my priority. All my riders wear both a helmet and protective vest.” For riders just starting out, she provides solid basics in a supportive and safe environment. For the more experienced equestrian, Fay provides advanced training and lessons, as well as showing opportunities. One of Fay’s students is a local young equestrian standout that placed 12th in the country last year at in the Novice Junior Championship held during the USEA American Eventing Championships. Also, the student, riding Obligato, a Dutch Warmblood gelding, owned by Fay, was Reserve Champion 1st Level and placed fourth at 2nd Level for Region 8 at the New England Dressage Association (NEDA) Championships held last year in Saugerties, NY. Fay also recognizes talented horses. In March, she traveled to Ireland and purchased new competition horses for her and a client. The horses, one Irish Sport Horse and one Dutch Warmblood,landed safely in New York and have begun their new American dressage & eventing careers. To encourage and teach young riders,each year Fay and AGEC host several riding camp experiences. In April, fourteen riders participated in a fun week of horsemanship and riding. The riders spanned in age and ability, including several with special needs, but all shared a love of horses and an interest in riding. “Riding camp is a great way to build confidence in young riders while providing an opportunity for them to make new friends,” says Fay. The farm is located on 10 acres in Schodack, NY and features a 100×200 outdoor dressage arena, indoor arena with attached barn and viewing room, several large paddocks, and miles of surrounding scenic trails. Where do I find my coupons for groceries?

• The Saturday or Sunday Paper: There are 1-5 different inserts each week in the paper. Saturdays paper can be bought at some dollar stores. Check some coupon web sites, they will tell you how many inserts are in the paper. For the extreme couponer, this will help you know how many papers to buy! •Store coupons: Many stores offer there own store coupons.(Target, CVS, Walgreens, Rite Aid) Check the stores coupon policy, because you can use a store coupon with a manufactures coupon. This is called "Coupon Stacking". This means you would be allowed to use 1 store coupon with 1 mfr. coupon for each item purchased. I love doing this, because to me, this is usually a free item!!! •Magazines: My favorite magazine, that has a ton of coupons in it, is "All You Magazine" You can buy this magazine at Walmart or get a monthly subscription. Even magazines like Redbook, Women's Day and Good Housekeeping have grocery coupons in them. I have been known to find coupons even in fashion magazines for makeup and coupons for my favorite clothing stores!!! •Company Web sites: Try this, Pick out a product you like and buy often, check the products web site. •Usually you can click on the "contact us" and leave a comment or compliment on how much you love the product and would love some coupons. Then 1 or 2 weeks later you will receive 1 or more coupons in the mail. Also, sign up for free samples, because they usually send a high value coupon with it! •Facebook: The only reason I am on facebook, is for the coupons. I have printed some of my best coupons (sometimes even free product coupons) from Facebook. So, do not request to be my friend, I will never respond!

Parents are stretched way too thin and have lost the importance of sending thank you's. My basic rule of thumb for everybody, if you give a gift in person and they thank you verbally then that is good enough. If you send a gift and they call and say thank you that is good enough. If you don't hear anything, on your next phone call to your brother and sister in law mention "How did the boys like my gift." That will usually get parents moving. I know its a hard thing to let go of but with technology. Kids are not sending letters and they are definitely not talking so try and let it go. I know its very hard because I was also raised in sending thank you's. In my own case, my niece and nephew will always send me a text thanking me. Would I prefer a phone call or note? Better off Dead? By David Stone, DVM BloomingroveVet Hospital Should we have gone forward with treatment? Would euthanasia have been a better? Would it have been better not to have tried so hard and let Nature take its course? I don’t decide courses of action. While my opinion may matter, ultimately the decisions are made by you. I pose options. I turn options into actions. Two recent scenarios highlight this dilemma. First is the dog attacked by coyotes…owners were fantastic—chased the attackers away, scooped up their ravaged pet and rushed him to the emergency vet. The CDEC vet was fantastic—assessed his condition, rushed to lifesaving chest surgery, cleaned and placed drains in all of the many wounds. Then to the regular vet…me…for post traumatic and postsurgical care. The pain he was dealing with was immense—while it was being cushioned by powerful pain meds—none the less, the pain was excruciating. The little guy wanted desperately to bite us as we cleaned his wounds several times each day but he never did. Instead, he would resort to licking his leg or my hand aggressively whenever I did something particularly painful. Each day he got better. Over the course of a week he was really shaping up—good eating, good energy, and while still very painful, it was obvious that in a few weeks he was going to be much better. His blood work looked good. His swelling was gone. We had a direction and a plan. One week after the attack his owners returned to the emergency hospital to report that he was losing energy and not acting right. It was determined that secondary effects of his trauma had damaged his heart and it was no longer functioning properly. That night his heart stopped beating and he died. He was supposed to make a great story of recovery and the power of Life. His owners spent several thousand dollars toward his recovery. Everyone did what they were supposed to do. Everyone tried. Client came early this morning with his long time pet dead in the back of his car. He had died the previous evening and brought him to be cremated. Our brief conversation led me to believe he had a tumor in his liver or spleen which had ruptured. My friend said that he was not in pain and it was over quickly. Despite the sound of it, bleeding to death is not a bad way to go—of the ways to go, that is. Unfortunately, a common scenario for this type of tumor is that the patient presents in the exam acting very tired, often unable to stand. Gums are white. Free blood is tapped on an abdominocentesis, usually xrays which are sometimes helpful. The owner has to make an immediate decision—thousand dollar abdominal exploratory and splenectomy with no guarantees. Do it or don’t. Take it or leave it. The problem is that many of these cases make it through the surgery just fine. Some make it past 3 months. And a few will be made completely whole by the surgery. Now what do you do? While the scenario is predictable, each case is different. I really loved this particular dog but I’m not sure invasive spleen surgery would have been the best thing for him…I think his owner would have followed my recommendations. I’m just not sure what they would have been. I’m glad—selfishly glad--that we didn’t have to make any decisions. The nature of things took care of everything. I am grateful to say that I have been part of many stories which involved heroics and miracles. As much as I may retrospectively question the value of lost attempts—as much as I hate to see people and pets suffer in the attempt, I will be on board again and try to be heroic when called upon. We can’t know unless we try—sometimes we need to give miracles an opportunity. Just know that when you look at me with tear filled eyes and say, “he’s had enough,” I’ll understand and hold your hand if we just need to let him go.


HOME DIALYSIS IS WHERE IT’S AT BY SHARI MEOLA CLINICAL SERVICES COORDINATOR RUBIN DIALYSIS CENTER

They say “Home is where the heart is.” At Rubin Dialysis, we believe that “home is where dialysis treatments should be done.” Having hemodialysis at home offers patients and their families many benefits over more traditional in-center dialysis. With dialysis done at home, patients can make their own schedule; those who have a care partner to assist them can schedule their treatments at any time. Even individuals who live alone or don’t have a care partner can enjoy the benefits of home therapy. Through Internet monitoring, Rubin staff members can serve as a virtual partner. With a portable home therapy unit the idea of sitting pool side in the back yard watching children or grandchildren play while having dialysis is more a reality than a dream. Patients can be at a summer home, or on vacation in a hotel room, and not have to stop their activities and fun to go to a dialysis unit for treatment. Patients can even spend a full day at the beach by doing treatment early in the morning or late in the evening. Home dialysis is about fitting the treatments into the patient’s life, not about fitting life around the treatments. With more frequent dialysis at home, dietary restrictions are much less stringent. Patients who have dialysis at night while they sleep (nocturnal dialysis) enjoy many fewer dietary restrictions simply because they are doing dialysis five to six times a week. Many home dialysis patients work during the day; those who are retired are free to spend their days golfing or enjoying other leisure time activities. Having dialysis during sleeping hours allows patients to keep their days free to do whatever they like and enjoy family time. With overnight dialysis, when patients are treated for seven or eight hours a day five to six days a week, the benefits are even greater. Most patients notice they have more energy and their pill burdens are often decreased. Some patients even mention that they often feel that their minds are “clearer” and they have increased energy. Patients who choose home dialysis receive training from our nursing staff for about three to four weeks, but once they are trained, they only need to come back to the dialysis unit once a month to check in with their nurse, dietitian, social worker and physician. With gas prices what they are, this can be a tremendous money saver. The benefits of home dialysis are many: no more driving in bad weather, no longer needing to be at the dialysis unit on a schedule that just doesn’t work, being able to vacation with family or spend several days away from home, and not having to eat meals in the dialysis unit away from family. An Appealing Home Starts at the Curb The old saying about making a good first impression could not be truer than when you are selling your home. That first impression has to create interest from the moment a buyer drives up to your home. Curb appeal is that subjective, intangible quality that entices buyers to think emotionally rather than logically. Perhaps it is the way the home sits on the property, the gracious columns, or the greenery and brilliantly colored flowers. How can you tell where your home registers on the curb appeal scale? One way to find out is to take pictures of your property from various angles. Show them to friends, family, colleagues, anyone known for providing painfully honest feedback. Find out what is appealing about the home and grounds, and what needs improvement. Even more important, consult your real estate professional. A local real estate professional has experience selling homes in your area and can be a great resource. Ask the agent to walk the property with you and view it from across the street. Develop a "to do" list to guide you in bringing your home up to showing condition, then brainstorm easy, cost-effective solutions. Surprisingly, even small enhancements can make a big difference. Building on your home's curb appeal might be as easy as replacing a broken screen and planting a few flowers near the front door. Here are some ideas to get you going. A fresh coat of paint breathes new life into a tired-looking home. If your home or just the front door looks dull, suffers from peeling, cracked or chipped surfaces, a paint job is a great investment. You will also want to clean any light fixtures by the entry. Mow and edge the grass, and trim the trees and bushes. Also, clear away dead leaves and flowers, and mulch and weed the beds. Check to see that tree branches are not touching the home's roof or outer walls. You can spruce up the property by hanging flowering baskets and placing planters of flowers in strategic spots. Do not forget the side and back yards; add some flowering plants to the back as well. Make any needed repairs. See if anything is unhinged, loose or just an eyesore. Fix everything including broken fencing, windows and screens. Try the doorbell, check stairs and railings. Test doors for squeaks and rusted hinges. Do not forget to take a critical look at the property at night. Make sure the lights work, and replace dim and burnedout bulbs. Tidy up the deck, patio and back yard. Rearrange the outdoor furniture to look inviting. Put away gardening tools. Clean up the barbecue area. Eliminate any "evidence" of Fido. Move extra vehicles from the view of passersby. You want buyers to think of the home as well maintained. To make a bright impression, clean the windows, inside and out, wash down the walks and driveway, and hose down the siding. Check for oil spots on cement surfaces, especially the garage floor. Remember, a sale can be made or lost as a direct result of your home's curb appeal. A little work now and you will captivate buyers at the curb. It is a sure way to a quicker sale.

A Retail Conversation DAN WELCH THE SWEATER VENTURE Is “commerce” one of the first words to come to your mind when youconsider your local independent business? For almost 10 years now our store mascot, Amber, has been eager to great you when you visit. Or, if she doesn’t actually greet you, she is after all a cat, she at least lets you tell her she is pretty as she sleeps by the fire. To continue the conversation we fill the store with merchandise that is intended to be different than the usual fare one commonly finds. And while this does include many styles of hand knit, natural fiber sweaters for which we have earned a reputation we offer much more then just sweaters. An abundance of colorful scarves in many fabrics, unique jewelry pieces, an ever changing mix of clothing along with ethnic treasures uncovered in our regular trips to Bolivia, Nepal, Ecuador, Thailand and other corners of the world are intended to pique your curiosity, excite your retail palette and encourage your feedback. Your feedback, offered to us in your questions, your return visits and in the kind words you share about us to friends and neighbors nourishes us, strengthens us and encourages us to seek ever more diverse offerings.
