The posterior tibial tendon serves as one of the major supporting structures of the foot, helping it to function while walking. Posterior tibial tendon dysfunction (PTTD) is a condition caused by inflammation or weakening of the tendon, which decreases the strength and ability to lift the arch. This results in flattening of the foot. This can occur in one or both feet, and will usually get worse if not treated early. Causes are typically overuse such as climbing stairs, running or hiking. Symptoms include pain and swelling to the inside of the ankle and arch, flattening of the arch, rolling in on the ankle and difficulty walking. When treated early, surgery can often be avoided. If left untreated, severe arthritis of the foot or ankle may occur. If you are having these symptoms, come in to see one of our specialists today. A foot and ankle surgeon will evaluate your feet to determine the cause and severity of your condition. After a complete evaluation they will go over treatment options that can help get you back to walking again. Often times the treatment is painless and simple. Call for a consultation today!

Kinderhook Dog Park Open to the Public The northeast corner of the Kinderhook Town Park has gone to the dogs, literally and in a good way. This is the site of the Kinderhook Dog Park, established 10 years ago (that’s 70 in dog years). The park is open all year. With shade, fresh water & a small shower, bench & chairs. There are 2 sections; one for large dogs and a smaller section for smaller dogs. Membership is not limited to the Town of Kinderhook residents. Any dog can join, provided they are licensed in the town where they live, have the necessary rabies vaccination, are not aggressive and have a membership tag. There is no town funding for the park; it was established by the Friends of the Kinderhook Dog Park, a 501 (c)(3) not for profit. Operating costs, insurance, mowing and supplies, are funded primarily by the membership tags, $25 a dog plus $10 for each additional dog with the same owner. Applications available from the Kinderhook Town Clerk & in a box inside the entry gate. The Friends of the Kinderhook now consists of the owners of all dogs who have their current membership tags. DON’T DO UPGRADES WITHOUT A PROPER PERMIT Don’t do upgrades without a proper permit, if needed. When you sell your house, appraisers, inspectors, lenders, and buyers can all ask for proper permits. It may not always kill the deal, but it can, and it has. There are a lot of very capable do-it-yourselfers out there, but a buyer has no way of knowing your personal capabilities. Can they trust the work you’ve done? If they can’t be sure… some buyers will walk away from what appears to be a perfectly lovely home. Generally, in New York you must get a building permit for any project that involves construction, demolition (adding or removing walls), electrical work, plumbing work, carpentry, paving, or the installation of a gas or oil heating unit. This includes building fences, decks, interior remodeling, driveways, sidewalks, swimming pools, and fireplaces. Depending on the city you live in, it may also include re-roofing and siding work. Most finishing work such as painting and tiling will not require a building permit. However, it is always best to check with the Building Department in the town where your property is located before starting any major project. Generally, you will need to first find out what permits and approvals your project needs. Once you have received all the required approvals, you will file your permit application with the Building Department, along with any necessary documentation, and pay the permit fee. The department will review your plans and application and either issue your permit or return your application for amendment. After you receive your building permit, you may begin construction. If you hired a contractor for your project, the contractor usually will “pull” the permit. If you pull the permit, you will be considered the contractor. Typically, the person who pulls the permit is responsible for following code. There are subsequent permit inspections by the town. They help protect your family by ensuring the project is done safely and correctly. It can save you money by catching errors to make sure the project is done right the first time. They can also ease potential frustration by eliminating mistakes and preventing extensive rework.

Hi Everyone, This is a house favorite and I love making it! Check our all of our yummy recipes here- www.facebook.com/groups/crockpotandhomecookingrecipes Homemade Broccoli & Cheddar Soup Ingredients: 2 bunches broccoli; cut into small florets 1 onion, diced 3 cloves of garlic, minced 2 carrots; julienned (about 1 cup) 1/4 cup butter; melted 1/4 cup flour 2 cups half & half 2 cups chicken stock 8 oz sharp cheddar cheese; shredded 1/4 tsp nutmeg Salt & pepper, to taste Directions: Saute onion and garlic in 1 tbsp of melted butter until transparent. In a separate pot, make a roux using the remainder of melted butter and 1/4 cup flour. Cook about 5 minutes over medium heat, constantly stirring with a whisk. Slowly add in half & half, continue stirring. Add chicken stock, stir and let simmer for 20 minutes. Add in broccoli florets, carrots, and sauteed onions. Let cook for another 25 minutes over low heat. Stir in cheese until melted, add nutmeg and salt and pepper to taste. ENJOY!

School – Mom’s Perspective
The end of summer brings the first day of school. I am amazed that, if my math is correct, between my two kids I am entering my 15th year of this particular activity. Kindergarten is just the beginning. The very first day of school you place your most prized possession in the hands of a smiling-faced bus driver tasked with delivering them to a collection of amazing individuals who will not only educate our little ones but also grow to care about them. We, as parents, slowly adjust our lives in their absence and shift our worries to if they ate their lunch, remembered their gym sneakers or are they making friends. Slowly, steadily, our kids develop who they are without our constant attention. It is actually quite an amazing journey. Each year, with the help of not only school but their friends, sports and various activities, they gain friendships, learn life lessons, expand their horizons and, unfortunately, suffer heartbreaks along the way. It can sometimes be difficult for parents to stand aside and watch. Eventually, it is impossible to shield them from the world nor should we. Our job expands from kissing boo boos to helping them understand the harsh realities around them. The pain of letting go is almost as strong as the pride in watching them break away and succeed. They are successfully creating their unique character each year they step on that bus. Just as our children go through stages, I feel that parents do as well. We slowly transition from “so and so’s mom/dad” into a solo figure/couple again. However, we are not the same person we were before kids. Our lives are enriched by the same experiences that our children have traveled through. The tears I cried sending a cute, little five-year-old to school for the first time was nothing compared to the pride tempered with heartbreak on the day I drove away from a young man being left behind to begin his first year of college. I am grateful for every moment of their lives intertwined with mine. I will never be the same. Happy first day of school. Enjoy the journey.

We are also looking for gently used children's seasonal clothes for our back to school shoppers. Again, please drop them off during our operating hours or at a special bin at the Town Landfill ( which is picked up weekly). We are accepting adult seasonal clothes as well as housewares at all times. Please make sure that they are gently used and small appliances that do work. Our stock changes rapidly so stop in and look around. We have a great deal - fill a 13 gallon plastic bag with clothes for a $5.00 donation! The summer months have shown an increase in monthly food calls. Many community members think that the food is donated or FREE; this is not the case. The Anchor has to buy food monthly even at a reduced cost but it certainly is not free. That is why we are always asking our community for help either monetarily or donations to help us meet our clients needs. We ask our area Church's to post suggested items we are in need of as well as the local news; Our Towne, the Castletonian plus the Library. We can not meet these needs without the generous support of this community. Non perishable food can also be left at the Library and the Clerk's office at the Town Hall. Thanks to the Crisp Cannoli, management & staff for their generous donations. They collect tips and “pool” them to support a worthy community cause. We appreciate all you have done- thank you for picking us to receive donations! What a great bunch of people!! Their supplies of shampoo, soap, deodorant & toothpaste helped us tremendously, thanks again! They also had a Fair theme event on August 5th giving out cotton candy to everyone who brought in a school snack for the Anchor. We are looking for granola bars, juice boxes, snack bags, etc.; things the children can use for school snacks in their backpacks. Another local organizations that truly supports The Anchor is the Castleton Kiwanis & their annual Clove Run that was held in August. Their fund raiser was so generous & appreciated. We hope you had a wonderful summer and good luck getting the children back to school!

uestion: My primary care (general practitioner) doctor told me that one of my ears was not a candidate for hearing aids, but both of my ears have hearing loss. Doesn’t having a hearing loss make you a candidate for using a hearing aid?

There are instances where a hearing aid would not be effective as a communication aide. When an audiologist does a hearing test, one section of the test looks at something called word discrimination scores, where we are assessing how clearly you can hear speech at a comfortable listening level. This part of the test helps determine how clearly (not loudly) the hearing nerve is sending the speech signal to the part of the brain that processes speech. In cases where word discrimination scores have low percentages, speech would be perceived as distorted even if the hearing aids were programmed well. Audiologists and otolaryngologists (Ear Nose and Throat doctors) usually have different opinions on the "cutoff " point where the word discrimination percentage is too low to benefit from hearing aids for communication. This is discussed and determined on a case-by-case basis. Hearing aids also come with a 45-day trial and evaluation period , so this might be something to consider trying to see if any benefit is perceived

Siblings of children with chronic illnesses are more likely to suffer feelings of anger, jealousy, depression, and anxiety. They are also more likely to do poorly in school, abuse drugs, and suffer from mental illness as adults (Derouin & Jessee, 1996). The Bus Stop Club, Inc. is a local non-profit that offers encouragement and support of the siblings of children with chronic illness or developmental, physical, or intellectual disabilities. The overall goal of the Bus Stop Club is to provide emotional and recreational support in a fun and safe environment. Compared with the parent, the sibling relationship is most often the longest relationship in the family—one which evolves, matures, and changes over time, one in which the roles vary the most. Sibling relationships shape our behaviors, our thinking and our opinions, and our emotions for life. Though there is no denying that parents feel pain both for the ill child and for their own grief, the brother or sister of an ill child feels the pain on three fronts—for their sibling, for themselves, and for the grieving parent. As such, the sibling’s need for information and understanding is often greater than that of a parent’s .In January of 2005, Dr. Brian Sheridan, a third-year pediatric resident at the Children’s Hospital at Albany Med, launched the Bus Stop Club—a program which would allow for primary consideration of the well child in the comprehensive care of chronically-ill children. Bus Stop Club is held at 9 different locations in the Albany, New York, area. Each monthly session lasts for about two hours. The Bus Stop Club social worker leads the children in activities that incorporates different themes each month and provides the Bus Stop Club child the opportunity to explore and expose his or her feelings about the given topic. The themes employed range from the concept of illness and the disease process that is affecting a brother or sister, changes in the family, feelings about relationships with the sibling, and how to cope with feelings they are not able to put into words. On occasion, a community visitor such as an art therapist or music therapist may join the session and facilitates the group as a whole with a therapeutic activity. Also, during the monthly sessions, the children have an opportunity to take part in “typical” childhood activities such as a basketball game or a swim. In addition to the monthly sessions, Bus Stop Club also sends 50 children to a week long day camp during the summer and throughout the year, offers Family Events. Family Events include the entire Bus Stop Club Family to participate in outings such as a three-bus move to Six Flags, New England, a family BBQ, tickets to the Siena Men’s Basketball Game and Tri City Valley Cats Game, a ride on Yankee Trail’s Santa’s Magical Express to name a few. Dr. L “What can you share with folks as we close in on the last quarter of the year.” We’re lucky because we get to meet every patient. We especially love seeing all the kids grow up and hearing news about grandkids from our longtime patients. But it’s sometimes hard when a patient gets annoyed with us, for missing their own appointment, even when we confirm every appointment in advance, or for not knowing who their insurance carrier is, or if their information changed since the last visit. But, we really believe that everything can be solved by communicating. Whether you need a special appointment time or have an insurance issue, our goal is to advocate so that you have a smooth and positive experience. What about you, Dr. B? I’m glad we’re on the same page with patient care. It’s exciting to work in an office that prioritizes technology, and continuing education. We’re always learning about advances to really help patients, whether making appliances to help them sleep or not grind their teeth, deal with medication and medical side effects or make functional and cosmetic improvements. Ultimately, I like patients to know that when they ask me, “would you recommend this to your mom?” I have to be able to say “absolutely”. Dr. L: Thanks Dr. B, I couldn’t have said it better myself. We’ve taken a family vacation in August forever, despite it being the week where Joe’s garden always produces the best tomatoes and when SPAC inevitably has the concerts we want to see. As expected, both kids were immersed in their summer jobs and we had our own calendars to keep, all while looking ahead to our incredible upcoming vacation with our friends, touring parts of Ireland. Years ago, before school sports and teenagers, college tours and summer jobs, we used to spend our coveted August week in the Adirondacks, at a special retreat tucked away between Schroon and Ticonderoga. A “one-of-a-kind” place you simply feel lucky to know about, where families unplug and connect and can get a serious dose of the glorious upstate NY summer. You may have heard about Pyramid Life Center. Once a summer camp for the local parishes, these days many local churches and colleges use the grounds and private lake for special events. Pyramid has hosted a “Family Week” for as long as we can remember and has been a Lapinski tradition, in one form or another for more than 30 years. Our kids have learned to swim, kayak and play newcomb. Many of the cousins have had summer jobs in the kitchens and on the grounds. It’s a family place, a connected place. And, even though we had plans to travel across the pond, we were missing our Adirondack gem. So, with a rare empty square on calendar, Allison and I were able to sneak away for a quick day trip a few days before our big vacay, say hi to some old friends that we knew would be there and infuse ourselves with a little dose of the familiar. As expected, as soon as we turned onto the long gravel road that approaches the main camp, the air changed, the grace and gratitude couldn’t help but seep in. We have so many memories from our family & friend weekends at Pyramid Lake. Bon fires, hikes, communal meals, endless card games, theme days, nature in the cabins (yes mice) loons on the lake and especially friendships made. A place this special has to have someone in charge. But, Director is really too simplistic a title to encompass the role Sister Monica Murphy plays for Pyramid and the people there. The only way I can describe her is the beating heart of something bigger than ourselves, she’s the personification of place. The omniscient, all powerful Oz, the compassionate friend and the Magic 8 ball rolled into one. To sit with her is to make a connection, because she has that way of seeing thru the haze and cutting right to the center, to the point of the matter and see you, really see you. She’s the person you want your kids to work for in the summer, heck, I’d work for her if she asked, and when she did ask for some favor, there is only one answer. Like the time she asked the non-Catholic to assist with the Sunday Mass (yes me and that’s another story...haha) Allison and I had a great few hours, at the beach, on a hike to the waterfall, catching up with folks about lives that keep on moving at the pace of the real world, well, when we’re not at Pyramid. When the sun started to set, it was time to leave, saying good-byes as we walked towards the car, making sure to have a final word of thanks with Sister Monica. She shared some of her favorite Ireland memories, having been there many times. I said, “I can’t wait to tell you all about it”, and she gave me one of those real solid “until nextime” kind of hugs. We drove back down that gravel road, feeling the lists and stresses and excitement of our impending travel week creeping back in before we even hit the Northway. We chatted about the weather, like would it rain, would it be sunny? From what we heard, you couldn’t predict the weather in Ireland, and hoped we’d be prepared. In hindsight, the trip was truly epic, we were prepared. It was ambitious itinerary in a place none of us had ever been and we loved every single minute of it. Almost. One morning, while taking a moment to check some emails, I learned, with shock, that Sister Monica had died in a tragic car accident. Just like that. In an almost indescribable moment of reflection, I was looking out the window at the green fields, dotted with sheep and cows, at the ancient stone walls and thought about how lucky I felt that we had taken that moment to visit, just a few days ago. And right about then, I could feel that hug, and with a sigh I simply thought, “until next time”.
