We work with recipients experiencing a range of hardships including those living with disabilities, survivors of domestic violence, families experiencing medical emergencies, and veterans adjusting back to civilian life.

Bringing life-changing therapy to her home Four-year-old Aviannah is currently the only known person in the world with a specific type of a rare skeletal dysplasia. She has extensive orthopedic needs, issues with her kidneys and has already undergone 15 surgeries. In order to improve her quality of life and allow her to have therapy at home, the family needs to purchase a hydrotherapy tub. With the support of donors, the GR Foundation was able to assist with the purchase and installation of the tub, as well as supply additional funds for the older children in the family.

Independence for a busy business owner Born with cerebral palsy, 27-year-old Shawna is in a wheelchair and dependent on assistance from caretakers and her parents. She owns a small business designing hats that are sold at a local craft fair. Her family relies on an accessible van, but it’s over 12 years old and in dire need of repairs. The GR Foundation is happy to help Shawna’s family purchase an accessible van, assist with rent and a provide a little extra for Shawna’s business.

Reclaiming her mobility for her children Amanda was in a car accident that left her a T5 paraplegic with a traumatic brain injury. She receives physical, occupational and cognitive therapy and is working to complete her GED—determined to support her children. Amanda is frustrated with her lack of mobility, restricting her from day to day activity with her children. The GR Foundation is thrilled to provide an accessible vehicle for Amanda and chip in with living expenses while she completes her degree.

Marisol

A mom’s fight to get her son the food he needs

Marisol’s 14-year-old son, Jaden, has down syndrome and was recently diagnosed with celiac disease. Jaden also suffers from anxiety and depression and is non-verbal. Over the past year, Marisol has been working tirelessly to decrease his gluten levels and keep him healthy, but she’s struggling to afford the high cost of gluten-free foods on top of their other expenses. The GR Foundation was able to supply her with a gluten-free food-delivery, as well as help with the next couple years of rent to off-set the high cost of Jaden’s needs.

How do I apply? Applying is easy – simply go to GR-Foundation.org/assistance. Complete Phase 1 of the application to the best of your ability. You will be contacted by an employee of the Guaranteed Rate Foundation within 30 – 45 days after completing Phase 1. If your Phase 1 application is approved for further review, access to the Phase 2 application will be provided.

Who is eligible? The Guaranteed Rate Foundation welcomes applications from anyone that has experienced or is experiencing an extreme hardship or crisis. The Guaranteed Rate Foundation gives directly to individuals in hardship and does not make donations to other charitable organizations.

How long does it take for my application to be reviewed? Applying is a two-phase process and timelines can vary based on volume and timing of the request. The applicant will receive an application status update in 30 - 45 days from their Case Manager once they complete their application. If your application requires you to complete Phase 2 of the process, you will be provided more information from the Foundation Case Managers. Once that step is completed, you will be contacted by a Foundation representative with a status update or a decision on your application. The sooner Phase 2 is completed, the sooner your application can be reviewed.

What do I need to apply? The Guaranteed Rate Foundation requires financial documentation to evaluate your request for assistance. Based on your circumstances, the requested documentation may vary. The Guaranteed Rate Foundation will work with each applicant to determine what documentation is needed. These items may include, but are not limited to: • 1 full month of income and expense details • Mortgage statements or rental agreement • 1 month of utility bills, household bills and/or invoices supporting financial hardship • Medical invoices and/or past due bills • Link(s) to fundraising account created to fund the individual’s or family’s hardship (if applicable) • Last filed year of Tax return and W2s • Household’s latest bank and investment account statements • Verification of Employment for all individuals involved

Who decides what is granted? Grant decisions are made by an external voting committee. They review cases on a semi-monthly basis to discuss possible grants. The Foundation also has two Case Managers that act as a liaison to the voting committee and serve as a resource for the applicant start to finish.

How much money has been granted? Since its inception in 2012, the Guaranteed Rate Foundation has awarded over $6 million to over 450 families who have found themselves in difficult situations arising from unforeseen hardship.

Does the Guaranteed Rate Foundation donate directly to non-profits? The Guaranteed Rate Foundation does not donate to non-profits as it is our mission to give financial support directly to individuals. However, the Foundation does work together with non-profits to identify recipients within their programs to receive individual grants.

How to Apply • Initial application is submitted through gr-foundation.org • GR Foundation Case Manager will provide an application status update to the applicant • Decision is made if application will move on to Phase 2 • In Phase 2, the applicant will be asked to provide: - Monthly Income - Monthly Expenses - Debt/Additional Expenses - Additional personal information that tell their story

• GR Foundation Case Managers will: - Personally connect with each applicant - Provide resources/referrals along the way - Put forth most honest/accurate application for committee review

• Application is voted on for approval by an external committee. Voting occurs twice a month.

1. Would a grant from the GR Foundation offer a long-term solution for this potential applicant? 2. Does the applicant have a long-term plan? 3. Is there a documented crisis? 4. Would this type of assistance put the applicant in a better position at the end of the grant term than where they are right now? 5. Will the potential applicant be self-sufficient after the grant ends?
